Operationssjuksköterskans omvårdnadsdokumentation inom perioperativ vård: En litteraturstudie / The operating theatre nurses´nursing documenation in perioperative care: a literature review

Introduktion: Dokumentation av omvårdnad är varje sjuksköterskas skyldighet och ansvar enligtPatientdatalagen (SFS 2008:355). Operationssjuksköterskan dokumenterar omvårdnad och information utifrån den perioperativa vårdprocessen. Tydlighet i dokumentationen kan säkerställainformationsflödet mellan olika vårdgivare. Syftet var att sammanställa forskning som beskriver operationssjuksköterskans uppfattningar om dokumentation av omvårdnad inom perioperativ vård. Metod: En integrativ systematisk litteraturstudie genomfördes enligt Statens Beredning för Medicinsk och Social Utvärdering [SBU]. Materialet analyserades med integrerad analys enligt Kristensson (2014). Resultatet baserades på åtta vetenskapliga artiklar. Tre kategorieri dentifierades utifrån analysen och var: varierande egenansvar och skiftande attityder till dokumentation av omvårdnad, påverkbara och skiftande förutsättningar för dokumentation av omvårdnad och i dokumentationen prioriterade omvårdnadsdiagnoser och omvårdnadsåtgärder. Konklusion: Standardiserad terminologi kan tydliggöra operationssjuksköterskans dokumentation av omvårdnad både för hen själv och för övriga vårdgivare. Ett för operationssjuksköterskan anpassat dokumentationssystem kan vara av betydelse för att dokumentationen av omvårdnad ska prioriteras. Ledarskapets inställning till dokumentation ansågs ha betydelse för om operationssjuksköterskan gavs förutsättningar till dokumentation av omvårdnad.

In the beginning of my thesis, I reckon with terms of installation and videoinstallation from a theoretical point of view. After establishing their meaning, I work with them in the context of my works. The centre of my thesis is my project called "232 712 steps" and its motive of voyage. My art project contains three main aspects which are experience, documentation and installation and which I explore with my own subjective perspective, but also contextualize with other works of art. Through my didactic part, I work with the term of projection and ways of recording it, each time with different perspectives. Josef Václav Scheybal : A documentalist and a collector The diploma thesis deals with Josef Václav Scheybal (1928 - 2001) and his activity mainly in northern Bohemia. It attempts to capture all aspects Scheybal's personality, because he was a painter, graphic artist, illustrator, art historian and ethnographer. He was an expert of folk architecture and sculpture, folk furniture, technical equipment, the popular clothing, prints, folk songs, was the creator of multiple exposures. He also cooperated with the Institute of ethnography and folklore, ethnographic research, participated throughout the country, with results published regularly. His focus on folk architecture resulted in a lifetime's research and documentary work. He gathered the valuable work of drawing documents, publications of folk architecture and free of the open air. The thesis also includes his family, the father Josef Scheybal and his wife Jana Scheybalová. Viele Notarztzentren in Deutschland dokumentieren ihre Einsätze heutzutage mit digitaler Technik. Dadurch wird eine hohe Dokumentationsqualität erzielt. Ziel dieser Arbeit war es, die handschriftliche Dokumentation des Notarztstützpunktes der Universitätsklinik Leipzig vor der Umstellung auf die digitale Technik zu untersuchen, um in einer folgenden Arbeit einen Vergleich zwischen den zwei Dokumentationsarten herstellen zu können. 1353 Protokolle wurden per Hand in eine eigens erstellte Eingabemaske übertragen und anschließend über die Fehlquoten die Dokumentationsqualität ermittelt. Daraufhin wurde untersucht, ob verschiedene Faktoren Einfluss auf die Dokumentationsqualität nehmen. Mit 44 von 48 möglichen ausgefüllten Feldern war das am besten ausgefüllte Protokoll zu 91% vollständig. Bei dem am schlechtesten ausgefüllten Protokoll waren neun Felder dokumentiert, das entspricht 20%. Die durchschnittliche Dokumentationsqualität lag somit bei 60%. In der weiteren Auswertung zeigte sich, dass der Weiterbildungsstand des Notarztes in dieser Untersuchung Einfluss auf die Dokumentationsqualität nimmt. Ebenso zeigten die verschiedenen Diagnosen der Patienten, der NACA-Score, der Zustand bei Übergabe und die Einsatzbeschreibung einen Einfluss auf die Dokumentationsqualität. Die handschriftliche Dokumentation auf vorgedruckten Notfallprotokollen aus Papier entspricht auf Grund der Nachteile in der Lesbarkeit und Handhabung sowie der fehlenden anschließenden Auswertbarkeit nicht dem heutigen Stand. Eine digitale Dokumentation, wie sie 2011 an der Uniklinik Leipzig eingeführt wurde, ist die Grundlage für ein modernes Qualitätsmanagement.

April 2008 konnte in der Sächsischen Landesbibliothek – Staats- und Universitätsbibliothek Dresden (SLUB) mit einem Festakt die Übergabe einer zeitgeschichtlichen Quelle von Rang begangen werden. Die Rede ist von der Videodokumentation des Regie-Kameramannes Ernst Hirsch vom Wiederaufbau der Frauenkirche in Dresden. Sie entstand in den Jahren von 1992-2005 im Auftrag der Gesellschaft zur Förderung des Wiederaufbaus der Frauenkirche e. V. und dokumentiert durch beinahe 400 Stunden Videomaterial das Baugeschehen und die Ereignisse rund um den neu errichteten Sakralbau.

Så är det ju - den blir aldrig färdig! : En etnografisk studie om förskolepedagogers arbete med pedagogisk dokumentation

This dissertation is about educators work with pedagogical documentation in Swedish preschools. The purpose has been to investigate how pedagogical documentation is realized in everyday work in two preschools and examine how educators express themselves on purposes, opportunities and difficulties with this approach. This has been important to study since many preschools have chosen to work with pedagogic documentation to follow up and assure the quality of their work. Since 2010 pedagogical documentation is included in the preschools curriculum. The study was conducted at two different preschools where I followed a total of six educators in two preschool departments for two months in each department. Methods used to collect data were selected based on an ethnographic approach. Data collection consists of field notes, photographs, recorded interviews and documentations from preschools and their working plans. The analysis is based on a curriculum theory perspective, and especially important concepts have been transformation arena and realization arena. Furthermore, frame factor theory and the didactic questions what, how and why, have been used to analyze data. The results show that the documentation works where formulated in different ways depending on the frames and conditions. One preschool unit was at a municipal preschool without any special pedagogical approach. Here the pedagogical documentation was in a startup phase and was one of many tasks in the pedagogical work. The other preschool unit was in a Reggio Emilia inspired preschool who had worked with pedagogical documentation for several years. Here the work with pedagogical documentation was the driving force for developing the work. This ideological difference was reflected in what the educators documented, how they documented and how the documentations were used. Pedagogical documentation was partly be seen as a cumbersome way of working among the participants, whether they are beginners or experienced. It is in the nature of the work with pedagogical documentation that there are no obvious answers, which can lead to some uncertainty whether one is right or not. Other factors leading to frustration is that this approach is time-consuming and that the new technologies available require a lot of skills to be used. The study highlights that continuous training plays a crucial role as one of educator’s preconditions to succeed in the work with pedagogical documentation. In this study, one can also see how ethical aspects constantly makes themselves felt and need to be highlighted. The main impression is that the educators, despite the perceived difficulties, felt that it was stimulating and meaningful to work with pedagogical documentation and that they saw it as a tool for developing their work. / Denna licentiatavhandling handlar om pedagogers arbete med pedagogisk dokumentation inom svensk förskola. Syftet har varit att undersöka hur pedagogisk dokumentation realiseras i vardagen på två förskolor samt undersöka hur pedagogerna uttrycker sig om syfte, möjligheter och svårigheter med detta arbetssätt. Detta har varit viktigt att studera mot bakgrund av att många förskolor har valt att arbeta med pedagogisk dokumentation för att följa upp och kvalitetssäkra sina verksamheter. Sedan 2010 ingår det enligt förskolans läroplan i deras uppdrag att dokumentera. Studien har genomförts på två olika förskolor där jag följt totalt sex pedagoger på två avdelningar under två månaders tid på varje avdelning. Metoder för att samla in data har valts med utgångspunkt i en etnografisk ansats. Datakollektionen består av fältanteckningar, fotografier, inspelade intervjuer, dokumentationer från förskolorna samt deras arbetsplan/verksamhetsplan. Analysen har skett utifrån ett läroplansteoretiskt perspektiv och speciellt viktiga begrepp har varit transformeringsarenan och realiseringsarenan. Vidare har ramfaktorteori och de didaktiska frågorna vad, hur och varför använts för att analysera data. Resultatet visar att dokumentationsarbetet utformades på olika sätt beroende på ramar och förutsättningar. Den ena förskoleavdelningen tillhörde en kommunal förskola utan speciell pedagogisk inriktning. Där var arbetet med pedagogisk dokumentation i en uppstartsfas och en av många arbetsuppgifter i verksamheten. Den andra förskoleavdelningen tillhörde en Reggio Emilia-inspirerad förskola som hade arbetat med pedagogisk dokumentation i flera år. Där var arbetet med pedagogisk dokumentation navet som drev verksamheten framåt. Denna ideologiska skillnad avspeglade sig i vad pedagogerna dokumenterade, hur de dokumenterade och hur dokumentationerna användes. Pedagogisk dokumentation uppfattades delvis som ett svårt arbetssätt bland pedagogerna, både bland nybörjare på området och bland dem som var mer erfarna. Det ligger i arbetssättets natur att det inte finns givna svar, vilket kan leda till en viss osäkerhet om huruvida man gör rätt eller inte. Andra faktorer som leder till frustration är att arbetssättet är tidskrävande och att den nya tekniken ställer stora krav på kompetens för att kunna användas. Studien belyser hur kontinuerlig fortbildning på förkolan spelar en avgörande roll för pedagogernas förutsättningar att lyckas med arbetet med pedagogisk dokumentation. I denna studie kan man också se hur etiska aspekter ständigt gör sig påminda och behöver lyftas fram som viktiga. Huvudintrycket är att pedagogerna, trots de upplevda svårigheterna, ansåg att det var utvecklande och meningsfullt att arbeta med pedagogisk dokumentation och att man såg det som ett fortbildningsverktyg.

Leveraging Artificial Intelligence to Improve Provider Documentation in Patient Medical Records

Clinical documentation is at the center of a patient's medical record; this record contains all the information applicable to the care a patient receives in the hospital. The practice problem addressed in this project was the lack of clear, consistent, accurate, and complete patient medical records in a pediatric hospital. Although the occurrence of incomplete medical records has been a known issue for the project hospital, the issue was further intensified following the implementation of the 10th revision of International Classification of Diseases (ICD-10) standard for documentation, which resulted in gaps in provider documentation that needed to be filled. Based on this, the researcher recommended a quality improvement project and worked with a multidisciplinary team from the hospital to develop an evidence-based documentation guideline that incorporated ICD-10 standard for documenting pediatric diagnoses. Using data generated from the guideline, an artificial intelligence (AI) was developed in the form of best practice advisory alerts to engage providers at the point of documentation as well as augment provider efforts. Rosswurm and Larrabee's conceptual framework and Kotter's 8-step change model was used to develop the guideline and design the project. A descriptive data analysis using sample T-test significance indicated that financial reimbursement decreased by 25%, while case denials increased by 28% after ICD-10 implementation. This project promotes positive social change by improving safety, quality, and accountability at the project hospital.

Emergency physician documentation quality and cognitive load : comparison of paper charts to electronic physician documentation

Indiana University-Purdue University Indianapolis (IUPUI) / Reducing medical error remains in the forefront of healthcare reform. The use of health information technology, specifically the electronic health record (EHR) is one attempt to improve patient safety. The implementation of the EHR in the Emergency Department changes physician workflow, which can have negative, unintended consequences for patient safety. Inaccuracies in clinical documentation can contribute, for example, to medical error during transitions of care. In this quasi-experimental comparison study, we sought to determine whether there is a difference in document quality, error rate, error type, cognitive load and time when Emergency Medicine (EM) residents use paper charts versus the EHR to complete physician documentation of clinical encounters. Simulated patient encounters provided a unique and innovative environment to evaluate EM physician documentation. Analysis focused on examining documentation quality and real-time observation of the simulated encounter. Results demonstrate no change in document quality, no change in cognitive load, and no change in error rate between electronic and paper charts. There was a 46% increase in the time required to complete the charting task when using the EHR. Physician workflow changes from partial documentation during the patient encounter with paper charts to complete documentation after the encounter with electronic charts. Documentation quality overall was poor with an average of 36% of required elements missing which did not improve during residency training. The extra time required for the charting task using the EHR potentially increases patient waiting times as well as clinician dissatisfaction and burnout, yet it has little impact on the quality of physician documentation. Better strategies and support for documentation are needed as providers adopt and use EHR systems to change the practice of medicine.

The diploma thesis deals with projet documentation for execution of a new kindergarten building in the city of Velíková. The kindergarten is located in the properties with number 528/1. The building has one underground floor and two above-ground floors. In the basement there are storage rooms, technical rooms and air conditioning engine room. In the first above-ground floor there are two classes of kindergarten. Every classroom are designed for 16 childrens. In this floor there is a kitchen and laundry with storerooms. There are three teaching classes and offices of management in the second above-ground floor. The perimetr load-bearing masonry in the basement is designed of concrete blocks of permanent formwork. In the above-ground part of the building are load-bearing masonry designed af clay blocks. The ceiling structures are designed from prestressed reinforced concrete spiroll panels. The roof structure is designed as a warm flat roof. The kindergarten is insulated with a certifies thermal insulation system.

Zaměření románských sklepních prostor v Praze / The Measurement of the Roman cellars in Prague

This diploma thesis concerns in the real measurement of Roman cellar areas in Prague 1 – Lesser Town, Karmelitská Street. A surveying structure for detailed area geodetic measurement is built up in given part of town. The surveying structure is connected to the binding reference systems – the S-JTSK coordinate system and Bpv altitude system. A detailed measurement is carried out in this cellar areas and recorded data are processed in a suitable programme. A three-dimensional model and drawing documentation – a ground plan and sections in required areas are drawn up according to this data. The ground plan of cellar areas in its drawing documentation is in 1:50 scale and the sections in 1:50 scale. Thesis outcomes will serve to document the current conditions of Roman cellar areas.
