A homoeopathic drug proving of Hemachatus haemachatus with a subsequent comparison of this remedy to those remedies yielding the highest numerical value and total number of rubrics on repertorisation of the proving symptoms

Thesis (M.Tech.: Homoeopathy)--Durban University of Technology, 2008 / The proving substance Hemachatus haemachatus commonly known as the Rinkhals belongs to the family of Elapidae. This spitting-cobra is a local snake found only in Southern Africa. This proving tested the effects of the thirtieth centesimal (30CH) potency of venom from Hemachatus haemachatus on healthy provers. OBJECTIVES It was hypothesised that Hemachatus haemachatus 30CH would produce clearly observable signs and symptoms in healthy provers, and that the comparison of Hemachatus haemachatus to those yielding the highest numerical value and total number of rubrics on repertorisation of the proving symptoms would highlight differences and similarities between the remedy symptoms so that confusion as to the indication is eliminated. It was hypothesised that a fuller understanding of Hemachatus haemachatus and its relationship to other remedies would be gained following this comparison. METHODOLOGY A double blind, placebo controlled proving of Hemachatus haemachatus 30CH was conducted on thirty healthy volunteers who met the inclusion criteria. Six of these thirty provers randomly received placebo, with neither prover nor researcher knowing whom received placebo. Provers had a homoeopathic case history taken and a physical examination performed on them prior to commencement of the proving. The provers recorded their signs and symptoms 6 by means of a journal before, during and after administration of the remedy. On completion of the proving, the information obtained was correlated and assessed by the two researchers, De la Rouviere and Cahill. The symptoms elicited during the proving were translated into materia medica and repertory language, and a homoeopathic picture of the remedy was subsequently formulated. Data from the case histories, physical examinations and group discussions were also considered in the assessment. RESULTS During the period of investigation, provers experienced a variety of symptoms on the mental, emotional and physical spheres. On the mental emotional sphere there was a marked degree of irritability and changeability in moods as is commonly seen in many of the snake remedies. Along with this, it was noted that there were feelings of anxiety for reasons unknown, a sense of having lost something or someone close, and a desire to be left alone. There were also a great number of feelings regarding the home, where there were feelings of the home being a place of safety and wanting order in the home. On a physical level, many of the provers noted varying degrees of abdominal discomfort and headaches. Along with anxiety, provers experienced palpitations and sensations of chest restriction or constriction with shortness of breath. There were a variety of musculoskeletal symptoms ranging from painful joints in the fingers to stiffness and tightness in the neck and back. Provers noted flushes of heat and alterations of their internal thermostat. Provers experienced marked dryness of the mucus membranes and the skin, and there was also a general feeling of weakness and heaviness as well as a marked aggravation in the mornings on waking. 7 CONCLUSIONS Symptoms obtained from the proving of Hemachatus haemachatus 30CH were studied and evaluated. Those symptoms that appeared to represent the remedy picture of Hemachatus haemachatus most accurately in the researchers‟ opinion were used in the repertorisation of the remedy. The investigation supported the hypothesis that Hemachatus haemachatus 30CH would produce clearly observable signs and symptoms in healthy provers. The subsequent comparison of the proving symptoms of Hemachatus haemachatus to Lycopodium (Club moss), Sulphur, Alumina (Aluminium oxide), Sepia (Cuttle fish) and Calcarea carbonica (Carbonate of Lime) highlighted differences and similarities between these remedies and Hemachatus haemachatus. The further comparison of remedies that came up on repertorisation restricted to the plant, mineral and animal kingdoms respectively provided a further comparison of remedies, which aimed at enhancing the differentiation of Hemachatus haemachatus to other similar remedies.

In the first of a two paper series, we describe the epidemiology, socioeconomic impact and key prevention strategies. It is widely distributed in Seasonally Dry Forests (SDF), Gallery Forests and Rainforests. The Tertiary events and the climatic oscillations of the Quaternary affected the distribution of these ecosystems altering, in turn, the distribution of animals associated to these habitats. We hypothesize that the forest expansion and contraction cycles caused by climate fluctuations may have influenced the current distribution and genetic structure of M. lemniscatus. This study aimed to study the evolutionary relationships and patterns of divergence among M. lemniscatus lineages and infer the historical biogeographic events that influenced the distribution and genetic variation. Twenty-nine individuals of M. lemniscatus were sampled from 16 localities in the states of Tocantins, Bahia, Goiás, Alagoas, Mato Grosso, Maranhão, Pará and Amazônia. Three mitochondrial regions (COI, 16S and ND4L) were sequenced, and together generated a fragment of 1595 bp and 23 different haplotypes. The analyses showed a very ancient lineage divergence ~ 4.5 Myr and high genetic differentiation among localities (FST=0.932; p<0.01), suggesting a limited gene flow among geographical regions. The demographic analyzes and neutrality tests indicated that there is no sign of expansion and that populations have constant size (Tajima’s D = 0.521; p = 0.763 and Mismatch distribution = 0.009; p= 0.779). In the analysis of the evolutionary relationship between haplotypes (by median-joining network method), no relationship between lineage and geographical space was found, suggesting incomplete lineage sorting. The results corroborate and give evidence that populations of M. lemniscatus were distributed in a more continuous region in the past, and the current distribution of this species may be the result of the reduction and separation of the geographical scope of their distribution, rather than having itself been an expansion event. This may be the result of cycles expansion of SDF and retraction of the rainforests during the cool and dry phases of the Quaternary. / Micrurus lemniscatus é uma espécie de cobra coral verdadeira sul-americana. Essa espécie possui uma ampla distribuição, ocorrendo em vários tipos de hábitats, incluindo Floresta Estacional Decidual e Semidecidual (FED), Matas de Galeria e Florestas Úmidas. No passado, os eventos do Terciário e as oscilações climáticas no Quaternário provavelmente influenciaram o padrão de distribuição dessas florestas, alterando a distribuição dos organismos associados à esses habitats. Hipotetiza-se que, os ciclos de expansão e retração florestal causados pelas flutuações climáticas podem ter influenciado a atual distribuição e estruturação genética de M. lemniscatus. Este trabalho teve por objetivo avaliar os padrões e relações evolutivas das linhagens genéticas de M. lemniscatus e compreender quais foram os eventos históricos biogeográficos que influenciaram a distribuição e a variação genética atual. Foram utilizados 29 amostras de indivíduos de M. lemniscatus de 16 localidades dos Estados de Tocantins, Bahia, Goiás, Alagoas, Mato Grosso, Maranhão, Pará, e Amazônia. Três regiões mitocondriais (COI, 16S e ND4L) foram sequenciadas, e juntas geraram um fragmento de 1595 pb e 23 diferentes haplótipos. As análises evidenciaram uma divergência bem antiga de linhagens, ~4,5 milhões de anos atrás e uma alta diferenciação genética entre as localidades (FST=0,932; p<0,01), o que sugere um fluxo gênico limitado entre essas regiões. As análises demográficas e testes de neutralidade indicaram que não há sinal de expansão e que as populações possuem tamanho constante (D de Tajima= 0,521; p= 0,763 e Distribuição de Mismatch= 0,009; p= 0,779). Na análise de relação evolutiva entre os haplótipos (pelo método median-joining network), não foi encontrada relação entre as linhagens e o espaço geográfico, sugerindo arranjo incompleto de linhagens. Os resultados corroboram e dão evidências de que as populações de M. lemniscatus eram distribuídas em uma região mais contínua no passado, e a atual distribuição desta espécie pode ser o resultado da redução e separação da abrangência geográfica de sua distribuição, ao invés de ter ocorrido propriamente um evento de expansão. Isso pode ser resultado dos ciclos de expansão das FED e retração das florestas úmidas durante as fases mais frias e secas do Quaternário.

Entre as cerca de 66 espécies atualmente válidas, 25 são registradas para a Amazônia brasileira. Com o objetivo de ampliar o conhecimento das serpentes do gênero Micrurus na Amazônia brasileira, foram levantados neste trabalho dados de lepidose, hemipênis e distribuição geográfica de 544 exemplares de 11 espécies. O presente trabalho compreende dois capítulos. O primeiro capítulo apresenta uma descrição comparativa dos hemipênis de 11 espécies de Micrurus. Baseado nos resultados obtidos nas análises dos exemplares foi elaborada diagnose, padrão de desenho e coloração, descrição e representação do hemipênis e mapa de distribuição geográfica para cada táxon. Neste capítulo são apresentadas descrições inéditas dos hemipênis de M. hemprichii ortoni e M. paraensis além de variações morfológicas até então não descritas da morfologia hemipeniana das espécies. O segundo capítulo apresenta uma descrição mais detalhada de duas espécies, M. filiformis e M. paraensis, com base na morfologia externa e do hemipênis. / Micrurus includes snakes of semi-fossorial, terrestrial and semi-aquatic habits, distributed from the southern United States to southern Argentina. From the 66 currently valid species, 25 are registered on the Brazilian Amazonia. In order to improve the knowledge on this group Brazilian Amazonia Micrurus, data about pholidosis, hemipenis and geographic distribution of 542 individuals of 11 species were collected. The present work contains two chapters. The first one is a comparative description of hemipenis from 11 Micrurus species. Diagnosis, coloration pattern, hemipenis description, geographic distribution are presented for each taxon. The hemipenis of M. hemprichii ortonii and M. paraensis are described for the first times. The second chapter presents more thorough a description of two species, M. filiformis and M. paraensis, based on the external hemipênis.

De uma biblioteca de cDNA da glândula de veneno da cobra coral brasileira Micrurus corallinus foi isolada uma seqüência denominada NXH8. Esta seqüência de cDNA apresenta similaridade estrutural com a família de toxinas de serpentes em 'três dígitos' ricas em pontes dissulfeto. A subclasse melhor conhecida nesta família, são as alfa neurotoxinas. Uma outra seqüência distinta, denominada NXH1 e suas isoformas NXH3 e NXH7, foram isoladas anteriormente. Pertencem à mesma família de toxinas e estão presentes na mesma biblioteca. Alguns resultados da caracterização de NXH1, são utilizados neste estudo, em comparação com NXH8. Algumas características estruturais tornam a seqüência NXH8 diferente da classe usual das alfa neurotoxinas, vindo a constituir possivelmente uma nova subclasse da família. A proteína NXH8 foi expressa em diversos vetores de expressão em Escherichia coli. A proteína recombinante, expressa pelo vetor pRSET C - NXH8 foi utilizada para imunizar camundongos. O soro contra NXH8, assim como o soro anti - elapídico do Instituto Butantan, reconhece a toxina recombinante em ELISA e Western blot. O soro anti - NXH8 detecta apenas uma banda do veneno de M. corallinus em Western blot, mas apresenta reatividade cruzada com componentes do veneno de alguns elapídeos neotropicais e do velho mundo. Em contraste, dados anteriores demonstraram que o soro anti - NXH1 é específico para um componente único do veneno de M. corallinus. O veneno de M. corallinus tem alfa neurotoxinas que bloqueiam o receptor pós - sináptico nicotínico de acetilcolina nas membranas do músculo esquelético de ratos. O soro anti - NXH8 é capaz de impedir a ligação de componentes do veneno bruto a esses receptores. Já o soro contra NXH1 não apresenta a mesma capacidade inibitória. Irrespective of whether these proteins are functionally different, their amino acid sequences can be readily aligned, using 8 half-cystines as conserved elements, suggesting the presence of common structural features. The best known subclass of three-finger-type toxins are the curaremimetic toxins, also called alpha-neurotoxins, found in most venoms from Elapid and Sea snakes. Another toxin with a distinct sequence, known as NXH1 and its isoforms NXH3 and NXH7 had been previously isolated. They belong to the same family of toxins and were characterized from the same cDNA library. In the present study, a comparative biochemical, pharmacological and structural analyses of NXH1 and NXH8 were described. Few structural characteristics of NXH8 seem to indicate that it differs from the usual class of alpha – neurotoxins, belonging, possibly, to a new subclass of 'three-finger' toxins. The NXH8 protein was expressed in various E. coli expression vectors and the resulted recombinant toxin from pRSETC-NXH8 plasmid was used as a "toxoid" for mice immunization. The anti - NXH8 sera, as well as the anti – elapid sera from the Butantan Institute, recognized the recombinant toxin by both ELISA and Western blot assays. In contrast to the claim that anti - NXH1 sera is specific to one component of M. corallinus’s venom, the anti – NXH8 sera show cross reactivity to venom of some Neotropical and Old World elapids. The M. corallinus's venom contains alpha – toxins, which inhibit post-synaptic nicotinic acetylcholine receptor of neonatal rat skeletal muscle membrane. The anti - NXH8 serum was capable of blocking the binding of the components of the crude venom to these receptors. In contrast, the anti – NXH1 serum did not show this inhibitory effect. This indicates that either NXH8 presents affinity for muscular nicotinic acetylcholine receptor or it shares a neutralizing epitope also present in M. corallinus’s alpha – neurotoxins.

Abrangendo aproximadamente 80 táxons, Micrurus está organizado em quatro grupos de espécies diferenciáveis com base no padrão de color, comprimento da cauda e morfologia do hemipênis: (1) grupo monadal tricolor, hemipênis e longo fortemente bifurcado; (2) grupo bicolor, com hemipênis longo e fortemente bifurcado; (3) grupo com coloração em tríades e hemipênis moderadamente alongado (4) grupo com padrão em tríades, com hemipênis curto e bilobado, podendo ser capitado ou não capitado. Inserido no quarto grupo, Micrurus spixii atualmente constitui um complexo contendo quatro subespécies nominais: M. s. spixii presente do norte ao sul da Amazônia central; M. s. martiusi ocorrendo na Amazônia Oriental, Brasil; M. s. obscurus ocorrendo em regiões de floresta a leste dos Andes (noroeste da Bolívia, norte do Peru, leste do Equador e sudeste da Colômbia) e noroeste do Amazonas, no Brasil; e M. s. princeps com populações restritas ao nordeste da Bolívia. Por apresentar caracteres merísticos e morfométricos densamente sobrepostos junto às semelhanças no padrão de coloração e o forte melanismo presente em diversos espécimes, a identificação destas subespécies é imprecisa, tornando questionável a validade das mesmas desde suas primeiras revisões. Neste trabalho, revisamos a taxonomia do complexo explorando caracteres externos (coloração, variação merística e morfométrica) e internos (morfologia hemipeniana e craniana), definido, assim os limites de cada táxon ao longo da distribuição geográfica. A presente dissertação foi organizada em duas seções: 1) Introdução geral onde são apresentadas informações taxonômicas e sistemáticas acerca de Micrurus, com atenção direcionada ao complexo Micrurus spixii; 2) o manuscrito intitulado “Revisão taxonômica das espécies do complexo Micrurus spixii (serpentes, elapidae)”, cujo objetivo foi revisar a taxonomia do complexo, explorando a morfologia interna e externa incluindo novos caracteres e assim delimitando precisamente os limites de cada táxon de acordo com a distribuição geográfica.
