Avaliação da qualidade de vida e fatores associados à satisfação dos pacientes submetidos ao tratamento cirúrgico da Doença do Refluxo Gastroesofágico / Assessment of quality-of-life and factors associated with satisfaction of patients submitted to surgical treatment of Gastroesophageal Reflux Disease

Introdução: O tratamento cirúrgico da doença do refluxo gastroesofágico (DRGE) possui excelente resultado na maioria dos pacientes. Porém, um grupo significante de indivíduos apresenta complicações ou recidiva dos sintomas, com impacto na qualidade de vida. Objetivos: Avaliar o grau de satisfação dos pacientes submetidos à fundoplicatura laparoscópica à Nissen, e comparar os resultados da aplicação do questionário GERD-HRQL de qualidade de vida pessoalmente e por via telefônica. Identificar fatores pré e pós-operatórios associados ao resultado do tratamento cirúrgico. Métodos: Foram selecionados 178 pacientes operados entre 2005 e 2009, no serviço de cirurgia do esôfago do HCFMUSP. Os pacientes foram convocados para consulta ambulatorial. Foram levantados os prontuários médicos para obtenção de dados pré-operatórios. No estudo foi feita a análise de dados epidemiológicos, cirúrgicos, endoscópicos e manométricos. Os pacientes foram divididos em grupos, e foi aplicado o questionário de qualidade de vida GERD- HRQL. Foi avaliada também a nota de melhora dos sintomas de 0 a 10, a intenção de fazer novamente a cirurgia e o uso atual de omeprazol. Os pacientes que não puderam comparecer ao ambulatório foram entrevistados por telefone. Resultados: 90 pacientes foram incluídos no estudo, 45 no grupo A (entrevista ambulatorial) e 45 no grupo B (entrevista telefônica). Houve diferença significante entre a pontuação média no questionário GERD-HRQL dos pacientes do grupo A (6,29) e B (14,09), p=0,002. Esse resultado também foi significante quando separados homens (p=0,018) e mulheres (p=0,049) de ambos os grupos. Porém, a nota de melhora dos sintomas (p=0,642) e a intenção de fazer novamente a cirurgia (p=0,714) foram iguais. Analisando-se somente os pacientes do grupo A, a correlação linear de Pearson não mostrou diferença estatística entre a pontuação no questionário GERD-HRQL e idade (p=0,953), IMC pré-operatório (p=0,607), IMC pós-operatório (p=0,498), pressão do esfíncter inferior do esôfago (PEM, p=0,651; PRM, p>0,999) e amplitude média de contração do esôfago distal (p=0,997). A correlação da pontuação média no questionário GERD-HRQL com número de pontos na hiatoplastia (p=0,857), presença de esofagite erosiva pré-operatória (p=0,867), tamanho da hérnia hiatal (p=0,867) e presença de distúrbio motor do esôfago (p=0,207) também não mostrou significância estatística. A presença de esôfago de Barrett maior que 1cm correlacionou-se com menor pontuação no questionário GERD-HRQL (p=0,035). O uso rotineiro de omeprazol foi marcador de menor satisfação com a cirurgia (p=0,034). Conclusões: A satisfação dos pacientes com o tratamento cirúrgico é de forma geral elevada. A aplicação do questionário GERD-HRQL mostrou pior qualidade-de-vida dos pacientes entrevistados por telefone, em comparação aos pacientes entrevistados pessoalmente no ambulatório. O uso rotineiro de omeprazol após a cirurgia esteve associado à menor satisfação com o tratamento cirúrgico da DRGE Não foram identificados fatores pré-operatórios que possam determinar pior resultado da fundoplicatura laparoscópica à Nissen, porém a presença de esôfago de Barrett no pré-operatório foi um marcador de maior satisfação dos pacientes operados / Introduction: The surgical treatment of gastroesophageal reflux disease (GERD) has excellent results in most patients. However, a significant group develops complications or recurrence of symptoms, with impact on quality-of- life. Objectives: Evaluate the satisfaction of patients submitted to laparoscopic Nissen fundoplication, and compare the results of the GERD-HRQL quality of life questionnaire applied in person and by telephone. Identify pre and postoperative factors associated with the outcome of the surgical treatment.. Methods: 178 patients operated by the esophageal surgery division at Hospital das Clínicas da Faculdade de Medicina da USP, between 2005 and 2009, were selected. Patients were invited to an ambulatory interview. Charts were reviewed, and preoperative data was obtained. Epidemiological, surgical, endoscopic, and manometric parameters were studied. Patients were divided in groups, and the GERD-HRQL questionnaire was used. We also evaluated the score of symptom improvement between 0 and 10, if the patient would do the surgery again and current use of omeprazole. Patients who could not come to the ambulatory were interviewed by telephone. Results: 90 patients were enrolled in the study, 45 in group A (ambulatory interview) and 45 in group B (telephonic interview). There was significant statistical difference between the average score in the GERD-HRQL questionnaire in groups A (6,29) and B (14,09), p=0,002. This result was also significant among men (0,018) and women (0,049) in both groups. However, the score of symptom improvement (p=0,642) and the intention of doing the surgery again (p=0,714) were equivalent. In group A patients, Pearson\'s linear correlation did not show statistical difference between the GERD-HRQL score and age (p=0,953), preoperative BMI (p=0,607), postoperative BMI (p=0,997), inferior esophageal sphincter pressure (PEM, p=0,651; PRM, p>0,999) and distal esophageal contraction pressure (p=0,997). The correlation between GERD-HRQL score and number of stitches in hiatoplasty (p=0,857), presence of preoperative erosive esophagitis (p=0,867), size of hiatal hernia (p=0,867) and presence of motor esophageal disturbances (p=0,207) did not show statistical significance. The presence of Barrett\'s esophagus larger than 1cm correlated with a lower score on GERD-HRQL questionnaire (p=0,035). The routine use of omeprazole was a marker of lower satisfaction with the surgical treatment (p=0,034). Conclusions: Patient satisfaction with surgical treatment is generally high. The GERD-HRQL questionnaire showed poorer quality-of-life of patients interviewed by telephone, compared to patients interviewed at the ambulatory. The routine use of omeprazole after surgery was associated with lower satisfaction with the surgical treatment. No preoperative factors were identified that could determine worst outcome after laparoscopic Nissen fundoplication, but the presence of Barrett\'s esophagus preoperatively was a marker of increased patient\'s satisfaction

The preferences for long-term medical or a unique surgical management have caused problems, because not all patients have the same degree of reflux, especially neurologically normal children (NNC). No pediatric classification or GERD severity scale has been reported so far. Objective: To develop a GERD severity scale in NNC and validate it in population of previously treated NNC. Patients and methods: A retrospective cross-sectional cohort study of NNC with GERD, up to the age of 18, admitted from May/1995 to May/2008. The charts data were reviewed for the statistical selection of 10 preoperative parameters related to GERD, to compose the Pediatric Gastroesophageal Reflux Scale (PEGERS). Each parameter received numerical values, proportional to its severity degree, whose sum in a patient resulted in his reflux severity score. After exclusion criteria, 399 patients were avaiable and received 409 treatment regimens (events for scoring). Besides medical treatment for mild cases, the patients had undergone partial anterior fundoplication (PFP, 270º) for estimated moderately severe GERD and total fundoplication (TFP, 360º) for estimated severe cases. To correlate the scores to the long-term results and validate the scale, the outcomes were compared according to GERD severity grades Results: The scores ranged from 3 to 26, forming 3 severity grades: low severity reflux (LS, scores ≤ 12, n=66), moderate (MS, 13-18, n=141) and high (HS, ≥ 19, n=202). Cases treated coincidentally to their optimal severity scores formed the i-PEGERS group (n=345). The others were treated off their ideal score, forming the o-PEGERS group. The i-PEGERS events had statistically more favorable rates than o-PEGERS concerning cure or improvement of GERD, postoperative complications, recurrences and quality of life. The PEGERS could evaluate the severity of all kinds of reflux. Conclusions: It is possible to apply a gastroesophageal reflux severity scale in NNC, which allows appropriate tailored treatments with the best chance for cure of GERD, few recurrences and better QOL than using subjective or unique treatment plans for all cases. / Introdução: As crianças com doença do refluxo gastroesofágico (DRGE) formam um grupo heterogêneo de pacientes que recebem um tratamento usualmente de modo subjetivo e com resultados imprevisíveis. O uso de uma única conduta clínica ou cirúrgica tem provocado problemas, porque nem todos pacientes têm o mesmo grau de refluxo, sobretudo os pacientes não neuropatas (PNN). Não existe nenhuma classificação ou escala dos graus de severidade da DRGE pediátrico. Objetivo: Desenvolver uma nova escala de gravidade de DRGE pediátrico em PNN e validá-la em uma população já tratada em nosso serviço. Métodos: Estudo retrospectivo de coorte transversal, de PNN com DRGE, idades até 18 anos, atendidos no período de maio/1995 a maio/2008 em serviço de cirurgia pediátrica. Os dados dos prontuários relativos ao refluxo e seu tratamento foram coletados. Entre diversos fatores, foram testados estatisticamente como fatores de riscos e selecionados 10 parâmetros para compor a Escala de Refluxo Gastroesofágico Pediátrico (ERGEP), cada um recebendo valores numéricos proporcionais à gravidade de cada um, cuja soma total resultou em escores de severidade. Critérios de seleção definiram amostra de 399 pacientes, submetidos a 409 tratamentos prévios diferentes (eventos ) para cálculo do escore pré-tratamento. Além do tratamento clínico indicado para casos de DRGE leve, havia sido realizada fundoplicatura parcial (270º) para DRGE considerada de moderada gravidade e a total (360º) para os considerados graves. Foram comparados os resultados daqueles tratamentos, em relação aos graus de gravidade de seus escores agora calculados. Resultados: Os escores variaram de 3 a 26, formando 3 grupos de gravidade: baixa (BG, escores ≤ 12, n=66), média (MG, 13-18, n=141) e alta gravidade (AG, ≥19, n=202). Os casos tratados coincidentemente com seu escore ideal de gravidade formaram o grupo D-ERGEP (n=345). Os outros foram considerados fora da faixa ideal de gravidade (grupo F-ERGEP, n=64). Os casos D-ERGEP apresentaram estatisticamente melhores taxas de cura ou melhora da DRGE, qualidade de vida, menos complicações pósoperatórias e recidivas do que os F-ERGEP. Qualquer tipo de RGE pôde ser avaliado pela ERGEP. Conclusões: É possível aplicar uma escala de RGE em PNN, definir 3 graus progressivos de severidade e definir um tratamento específico a cada grau, com melhores chances de cura, menos recidivas e melhor QV do que o uso subjetivo ou uma conduta única para todos os casos. The aim of this work has been the development and optimization of methods for early diagnosis of lung cancer, their utility and integration into daily practice. Firstly, we developed a device for measurement of endobronchial temperature (thermobronchoscopy) and found significant difference in endoluminal temperature above tumors and infiltrated lymph nodes compared to healthy regions. We further designed an appliance for near infrared spectroscopy of the bronchial mucosa and identified spectroscopic features useful for localization of solitary pulmonary nodule. The use of the appliance improved yield of endobronchial biopsy compared to endobronchial ultrasound. In the next part of the study, we describe further techniques for early diagnosis of lung cancer including endobronchial ultrasound, optical coherence tomography, confocal fluorescence microendoscopy, reflectance spectroscopy, autofluorescence bronchoscopy, fluorescence bronchoscopy, and narrow band imaging with concise introduction of our experience gained in several pilot projects. Next, we showed the utility of measurement of acetic acid in exhaled air as a promising biomarker for non-invasive identification of patients with symptomatic acid gastroesophageal reflux. Potential Mechanisms Connecting Asthma, Esophageal Reflux, and Obesity/Sleep Apnea Complex-A Hypothetical Review

Obstructive sleep apnea (OSA) and asthma are potentially linked at several levels. The pathophysiology of these two conditions seems to overlap significantly, as airway obstruction, inflammation, obesity, and several other factors are implicated in the development of both diseases. Gastroesophageal reflux disease (GERD), cardiovascular complications, obesity itself, and the underlying inflammatory processes are all complex contributory factors that provide hypothetical links. Furthermore, a collateral rise in prevalence of both OSA and asthma has been noticed during the past few years, occurring in association with the emerging epidemic of obesity, a common risk factor for both conditions. OSA and asthma share many other risk factors as well. We propose a hypothetical OSA-asthma relationship that has implications on the diagnosis and management of patients presenting with either condition singly. Clinicians should be aware that OSA might complicate asthma management. Based on this hypothesis, we suggest that the treatment of the individual patient who experiences both asthma and OSA needs to be multidisciplinary and comprehensive. This hypothetical association of asthma and OSA, though described anecdotally, has not been systematically studied. In particular, the influence of continuous positive airway pressure therapy (for sleep apnea) on asthma outcomes (such as quality of life, steroid utilization, emergency room visits) and fatality needs to be studied further.

Nrf2/Keap1-Pathway Activation and Reduced Susceptibility to Chemotherapy Treatment by Acidification in Esophageal Adenocarcinoma Cells

Chronic acid reflux causes cellular damage and inflammation in the lower esophagus. Due to these irritating insults, the squamous epithelium is replaced by metaplastic epithelium, which is a risk factor for the development of esophageal adenocarcinoma (EAC). In this study, we investigated the acid susceptibility in a Barrett’s cell culture in vitro model, using six cell lines, derived from squamous epithelium (EPC1 and EPC2), metaplasia (CP-A), dysplasia (CP-B), and EAC (OE33 and OE19) cells. Cells exposed to acidic pH showed a decreased viability dependent on time, pH, and progression status in the Barrett’s sequence, with the highest acid susceptibility in the squamous epithelium (EPC1 and EPC2), and the lowest in EAC cells. Acid pulsing was accompanied with an activation of the Nrf2/Keap1- and the NFκB-pathway, resulting in an increased expression of HO1—independent of the cellular context. OE33 showed a decreased responsiveness towards 5-FU, when the cells were grown in acidic conditions (pH 6 and pH 5.5). Our findings suggest a strong damage of squamous epithelium by gastroesophageal reflux, while Barrett’s dysplasia and EAC cells apparently exert acid-protective features, which lead to a cellular resistance against acid reflux.

Molecular Targets for Gastric Cancer Treatment and Future Perspectives from a Clinical and Translational Point of View

Gastric cancer is a leading cause of cancer death worldwide. Systemic treatment comprising chemotherapy and targeted therapy is the standard of care in advanced/metastatic gastric cancer. Comprehensive molecular characterization of gastric adenocarcinomas by the TCGA Consortium and ACRG has resulted in the definition of distinct molecular subtypes. These efforts have in parallel built a basis for the development of novel molecularly stratified treatment approaches. Based on this molecular characterization, an increasing number of specific genomic alterations can potentially serve as treatment targets. Consequently, the development of promising compounds is ongoing. In this review, key molecular alterations in gastric and gastroesophageal junction cancers will be addressed. Finally, the current status of the translation of targeted therapy towards clinical applications will be reviewed.

Effet inhibiteur de la ventilation nasale à pression positive intermittente sur les reflux gastro-oesophagiens chez l'agneau nouveau-né / Inhibitory effect of nasal intermittent positive pressure ventilation on gastro-esophageal reflux in the newborn lamb

Résumé : Introduction : La ventilation nasale, de plus en plus utilisée chez le nourrisson, peut insuffler de l’air dans l’estomac et causer des reflux gastro-œsophagiens (RGO). Parmi les modes de ventilation nasale, l’aide inspiratoire (AIn) devrait entrainer un plus grand nombre de RGO que le neuro-asservissement de la ventilation assistée (NAVAn), où l’insufflation d’air est plus «physiologique». L’objectif principal de l’étude est de comparer le nombre de RGO en NAVAn et en AIn dans notre modèle ovin d’étude du RGO néonatal et de ventilation nasale. Méthodes : Une polysomnographie avec pH-impédancemétrie œsophagienne de 6 h a été effectuée chez 10 agneaux nouveau-nés. L’enregistrement a été répété trois jours consécutifs (une condition par jour) en respiration spontanée, AIn (15/4 cmH[indice inférieur 2]O) et NAVAn (15/4 cmH[indice inférieur 2]O) dans un ordre randomisé. Résultats : Comparé à la respiration spontanée [13 (23)], le nombre de RGO en 6 h a diminué fortement et de façon similaire en AIn [1 (3)] et en NAVAn [2 (2)] (p < 0,05), même pour des RGO faiblement acides et proximaux. De plus, le nombre d’insufflations d’air n’était pas différent entre l’AIn et la NAVAn. Conclusion : L’AIn et la NAVAn inhibent de façon équivalente les RGO chez l’agneau, incluant les RGO faiblement acides et proximaux, si la pression inspiratoire n’est pas trop élevée et malgré le fait que de l’air soit insufflé dans l’œsophage. Ce résultat est identique à celui obtenu avec l’application d’une pression positive continue nasale (6 cmH[indice inférieur 2]O). Il est possible que la pression positive appliquée lors de la ventilation diminue les relaxations transitoires du sphincter inférieur de l’œsophage, mais des études en manométrie œsophagienne sont nécessaires pour comprendre les mécanismes en jeu. // Abstract : Introduction: Nasal ventilation, increasingly used in infants, can blow air in the stomach and cause gastroesophageal reflux (GER). Among the nasal ventilation modes, pressure support ventilation (nPSV) should lead to a greater number of GER than neurally-adjusted ventilatory assist (nNAVA), where the air delivery is more "physiological". The main objective of the study is to compare the number of GER in nNAVA and nPSV in our unique sheep model of neonatal GER and nasal ventilation. Methods: A 6h polysomnographic recording with esophageal pH-impedance was performed in 10 newborn lambs. The recording was repeated for three consecutive days (one condition per day) for spontaneous breathing, nPSV (15/4 cmH[subscript 2]O) and nNAVA (15/4 cm H[subscript 2]O) in a randomized order. Results: Compared with spontaneous breathing [13 (23)], the number of GER in 6h strongly and similarly decreased in nPSV [1 (3)] and nNAVA [2 (2)] (p < 0.05), even proximal and weakly acidic GER. In addition, the number of air insufflations was not different between nPSV and nNAVA. Conclusion: nPSV and nNAVA both inhibit GER in lambs, including weakly acidic and proximal GER, if the inspiratory pressure is not too high and despite the fact that air is blown into the esophagus. This result is identical to the one obtained with the application of a nasal continuous positive airway pressure (6 cmH[subscript 2]O). It is posssible that the applied positive pressure decreases transient relaxations of the lower esophageal sphincter, but esophageal manometry studies are needed to understand the mechanisms involved.

Por padronização aceita internacionalmente, posiciona-se o sensor distal de pH-metria esofágica a 5 cm acima da borda superior do esfíncter inferior do esôfago, identificado por manometria esofágica. Porém, vários autores sugerem técnicas alternativas de posicionamento que prescindem da manometria; dentre essas, destaca-se a da viragem do pH. Esse tema é controverso: alguns autores consideram a técnica da viragem confiável, outros afirmam que não é. Considerando-se a polêmica existente e o pequeno número de trabalhos prospectivos com amostra e metodologia adequadas, idealizamos o presente estudo, que tem por objetivos avaliar a adequação do posicionamento do sensor distal de pH-metria baseado na técnica da viragem, analisando-se a presença, o grau e o tipo de erro de posicionamento que tal técnica proporciona e a influência da posição adotada pelo paciente durante a manobra. Foram estudados, de modo prospectivo, 1031 pacientes encaminhados para realização de pH-metria esofágica. Durante entrevista clínica, foram registrados os dados demográficos e as queixas clínicas apresentadas. Todos pacientes foram submetidos à manometria esofágica para localização do esfíncter inferior do esôfago e à técnica da viragem do pH, que consiste na introdução do sensor de pH-metria na câmara gástrica e na tração gradual do mesmo até que o pH vire para níveis superiores a 4. A identificação do ponto de viragem foi realizada de dois modos distintos, caracterizando dois grupos de estudo: com o paciente sentado (Grupo I - 450 pacientes) e em decúbito dorsal horizontal (Grupo II - 581 pacientes). Após a identificação do ponto de viragem, o sensor distal de pH-metria era posicionado na posição padronizada, baseada na localização manométrica do esfíncter. Registrava-se onde seria posicionado o sensor de pH se fosse adotada a técnica da viragem. Para avaliação da adequação do posicionamento, considerou-se que o erro é representado pela diferença (em centímetros) entre a localização padronizada e a localização que seria adotada caso fosse empregada a técnica da viragem. Considerou-se que o erro seria grosseiro se fosse maior que 2 cm. Analisou-se também o tipo de erro mais freqüente: se acima ou abaixo da posição padronizada. Observou-se que se fosse adotada a técnica da viragem, haveria erro no posicionamento do sensor em 945 pacientes (91,6%). Quanto ao grau de erro, haveria erro grosseiro em 597 (63,2%) casos. Em relação ao tipo de erro, o sensor seria posicionado abaixo do local padronizado em 857 (90,7%) pacientes. Em relação à interferência da posição adotada pelo paciente durante a técnica da viragem, observou-se que não houve diferença significante entre os grupos de estudo, em nenhum dos parâmetros analisados. Conclui-se que: 1. o posicionamento do sensor distal de pH-metria pela técnica da viragem não é confiável; 2. a técnica da viragem proporciona margem de erro expressiva; 3. o tipo de erro mais comum que tal modalidade técnica proporciona é o posicionamento mais distal do sensor, que pode superestimar a ocorrência de refluxo; 4. não há influência da posição adotada pelo paciente durante a realização da técnica da viragem do pH, na eficiência do método / By internationally accepted standardization, the esophageal pH-meter distal sensor is positioned 5 cm above the superior border of the esophagus lower sphincter, identified by esophageal manometry. However, several authors suggest alternative positioning techniques that leave out the manometry; among such techniques, the pH step-up is the one to be pointed out. This subject is controversial; some publications state that the step-up technique is not reliable while some others consider it reliable. Considering the existent controversy and the small number of prospective works with suitable sample and methodology, we have idealized the present study, that aims the evaluation of the suitability of the pH-meter distal sensor positioning based on the step-up technique, by analyzing the presence, the degree and the type of the error of positioning that such technique provides and the influence of the position adopted by the patient during the procedure. 1031 patients conducted to the esophageal pH-meter procedure were studied in a prospective way. During the clinical interview, the demographic data and the presented clinical complaints were registered. All the patients were submitted to both esophageal manometry in order to localize the esophagus lower sphincter and the pH step-up technique, that consists of the introduction of the pH-meter sensor in the gastric chamber and in the sensors gradual traction until the pH steps up to levels over 4. The step-up point was identified by two distinct ways, characterizing two study groups: with the setting patient (Group I - 450 patients) and in supine position (Group I I- 581 patients). After the step-up point identification, the pH-meter distal sensor was placed in the standard position, based on the sphincter manometric placement. It was registered where the pH sensor would be positioned if the step-up technique were adopted. To evaluate the positioning suitability, the error was considered to be represented by the difference (in centimeters) between the suitable placement (manometry) and the one that would be adopted in case the step-up technique were adopted. The positioning error was considered rough if it were larger than 2 cm. The most frequent type of error was also analyzed: if above or below the standard position. It was observed that if the step-up technique were adopted, there would be error in the sensor positioning in 945 patients (91.6%). In terms of error degree, there would be a rough error in 597 (63.2%) cases. Concerning the type of error, the sensor would be positioned below the standard place in 857 (90.7%) patients. As to the interference of the position adopted by the patient during the step-up technique, it was observed that there was no significant difference among the groups of study, in any of the analyzed parameters. It was concluded that: 1. the positioning of the pH-meter distal sensor by the step-up technique is not reliable; 2. the step-up technique provides expressive error margin; 3. the most common type of error that such technique mode provides is the placement of the sensor below the standard positioning, which may overestimate the reflux occurrence; 4. there is no influence in the position adopted by the patient during the pH step-up technique procedure, in terms of method efficiency

Aderência ao tratamento com inibidores da bomba protônica em pacientes com doença do refluxo gastroesofágico / Adherence to the treatment with proton pump inhibitors in patients with gastroesophageal reflux disease

Introdução: A Doença do Refluxo Gastroesofágico (DRGE) possui elevada prevalência e morbidade. O tratamento clínico consiste em recomendações quanto ao estilo de vida e, essencialmente, no uso de inibidores da bomba protônica (IBP). A aderência (Ad) dos pacientes com DRGE à prescrição, embora fundamental para o sucesso terapêutico, tem sido pouco estudada. Objetivo: Avaliar a Ad ao tratamento com IBP e possíveis variáveis relacionadas em pacientes com DRGE. Métodos: Estudo transversal e prospectivo com 240 pacientes adultos consecutivos com DRGE erosiva e não-erosiva (ne-DRGE) que haviam recebido dose padrão ou dose dobrada de omeprazol em uso contínuo. Todos os pacientes foram classificados conforme o grau, segundo os achados da endoscopia digestiva alta (EDA) em ne-DRGE (162; 67,5%), classificação endoscópica de Los Angeles (LA) A (48; 20,0%), LA B (21; 8,6%), LA C (1; 0,4%), LA D (1; 0,4%) e Barrett (7; 2,9%). Foi aplicado o questionário de Morisky et al constituído de 4 questões com respostas dicotômicas para avaliar a Ad, classificando-a como baixa (0-2 pontos) e alta ad (3-4 pontos). Foi também aplicado o questionário QS-DRGE para a avaliação dos sintomas, com 10 questões com escores de 0 a 50, conforme a frequência menor ou maior dos sintomas. Os resultados de Ad foram relacionados com os dados sócio-demográficos, polifarmácia (PF) (uso de mais de cinco medicamentos diariamente), comorbidades (CM), tempo de tratamento (TT), escore QS-DRGE, presença de sintomas descrita em prontuário, achados da EDA e conhecimento do paciente sobre a doença. Resultados: (1) 126 pacientes (52,5%) apresentaram alta Ad e 114 (47,5%) baixa Ad; (2) Os pacientes mais jovens (p = 0,002) foram menos aderentes; (3) Pacientes sintomáticos de acordo com relato em prontuário apresentaram maior percentual de baixa Ad e 2 vezes maior probabilidade de ter baixa Ad em relação aos assintomáticos (p = 0,02); (4) Os pacientes casados apresentam probabilidade 2,41 vezes maior de ter baixa Ad do que os viúvos. (5) As demais variáveis estudadas não influenciaram a Ad ao tratamento. Conclusões: Pacientes em uso de IBP em tratamento ambulatorial em hospital terciário em São Paulo apresentaram grande percentual de baixa Ad ao tratamento, sendo esta uma possível causa da falha da terapia com IBP. Clinical treatment is based on lifestyle recommendations and, essentially, in the use of a proton pump inhibitor (PPI). Adherence (Ad) of GERD patients to the prescribed treatment, although critical for therapeutic success, has been little studied. Objective: Assess adherence to the PPI treatment and potential associated variables in patients with GERD. Case studies and Methods: Transversal and prospective study with 240 consecutive adult patients, diagnosed with erosive GERD (e-GERD) and non-erosive GERD (ne-GERD) for whom continuous use of the standard dose or the double dose of omeprazol had been prescribed. Patients were ranked according to the findings of high digestive endoscopy (HDE) in ne-GERD (162; 67.5%); e-GERD: Los Angeles (LA) endoscopic classification A (48; 20.0%), LA B (21; 8.6%), LA C (1; 0.5%), and LA D (1; 0.5%) and Barretts esophagus (7; 2.9%). The Morisky questionnaire, that includes four questions with dicotomic responses to assess Ad, was applied. Ad was classified as low (0-2 points) and high (3-4 points). In addition, the QS-GERD questionnaire was applied to assess symptoms, using 10 questions with score 0 to 50, according to the greater or lesser symptom frequency. Ad results were correlated with personal data (gender and age), demography, polypharmacy (PF), comorbidities (CM), treatment time (TT), QS-GERD scores, symptoms described in the patients record, HDE findings and patient awareness about the disease. Results: (1) 126 patients (52.5%) exhibited high Ad and 114 (47.5%) low Ad; (2) younger patients (p = 0,002) were less compliant; (3) married patients had a 2.41 greater probability to exhibit low Ad as compared to widowers (p = 0.03); (4) patients with symptoms indicated in the patients record exhibited a lower Ad rate and twice greater probability of exhibiting low Ad as compared to asymptomatic patients (p = 0.02); (5) the other variables studied had no influence on treatment adherence. Conclusion: Patients using PPI as out-patients in third care hospital in São Paulo exhibited high rate of reduced treatment adherence, and this may be a potential cause of PPI therapy failure. Age < 60 years and marital status may be risk factors for low adherence
