Influence of Muscle Strength on Mobility in Critically Ill Adult Patients on Mechanical VentilationRoberson, Audrey R 01 January 2018 (has links)
Patients in the intensive care unit (ICU) setting are prone to develop muscle weakness and the causes are multi-factorial. Muscle strength in adult, critically ill patients on mechanical ventilation decreases with immobility. The influence of muscle strength on different muscle groups and its influence on progressive mobility in the adult, critically ill patient on mechanical ventilation has not been examined. Identifying muscle strength in this patient population can benefit overall muscle health and minimize muscle deconditioning through a progressive mobility plan. The objective of this dissertation was to describe muscle strength in different muscle groups and to describe the influence of muscle strength on mobility in critically ill adult patients on mechanical ventilation (MV). Fifty ICU patients were enrolled in this descriptive, cross sectional study. Abdominal core, bilateral hand grip and extremity strength was measured using three measurement tools. Mobility was measured using the following scale: 0=lying in bed; 1=sitting on edge of bed; 2=sitting on edge of bed to standing; 3=walking to bedside chair and 4=walking >7 feet from the standing position. Predictors of mobility were examined using stepwise regression. Abdominal core, bilateral hand grip and extremity strength demonstrated statistically significant relationships with all variables. Extremity strength accounted for 82% of the variance in mobility and was the sole predictor (β=0.903; F=212.9; p=0.000). Future research addressing the outcomes of implementing a mobility protocol in this patient population and prioritizing when such a protocol should be implemented would be beneficial to ongoing plans to decrease MV, ICU and hospital days. Muscle strength tests implemented at the bedside are crucial to implementing a progressive mobility plan for critically ill adults while they are on MV therapy.

Evaluating balance and strength of older women in exercise programsDinger, Melanie (Melanie Elizabeth) 15 February 2013 (has links)
Falls are a common problem among older adults, including those who are relatively healthy and living independently. Exercise has been recommended as an intervention to reduce falls by slowing and/or reversing age-related declines in balance, strength, and mobility. However, it remains unclear which types or combinations of programs are most effective. The objective of this study was to investigate whether exercise programs performed by healthy older adults were associated with superior balance, strength, and functional mobility measures that are pertinent to fall prevention.
This study compared three distinct groups: participants of a balance- and strength-focused training program (i.e., Better Bones and Balance®), participants engaged in a general walking program, and sedentary individuals. Balance was measured using the Sensory Organization Test composite score and sensory ratios. Isometric strength of the lateral hip stabilizers (i.e., abductors and adductors) was measured in terms of maximum voluntary contraction and rapid torque production. Rapid torque measures included contractile impulse and rate of torque development evaluated at 0-100 ms and 0-300 ms from contraction onset. Functional mobility was measured by the time to complete the Four Square Step Test.
Hip abduction contractile impulse (0-300 ms) was 1.905 Nm*s and 1.539 Nm*s higher for the Better Bones and Balance (BBB) group compared to the walking and sedentary groups, respectively. No differences were found among the groups for any of the hip adduction torque measures or Sensory Organization Test balance scores. The BBB group completed the Four Square Step Test faster than the walking and sedentary groups by 0.90 s and 1.06 s, respectively. In conclusion, participation in the balance- and strength-focused training program was associated with superior performance in some measures of strength and functional mobility that may be important for fall prevention. / Graduation date: 2013

Optimal recovery of range of motion (ROM) and strength are critical objectives of intensive rehabilitation following spinal cord injury (SCI). The aim of this study was to characterize the evolution of ROM and strength at the upper limb and analyze their relationship with functional independence during intensive rehabilitation. Data ROM, strength, spasticity, secondary impairments, functional independence) from 197 participants with SCI were collected at admission and discharge from intensive rehabilitation and were analyzed with descriptive statistics and multivariate analyses (regressions, ANOVA). Specific association analyses were used to determine the level of association between ROM, strength and independent variables such as pain, spasticity and secondary impairments.
Lesser deficit in ROM was observed in participants with paraplegia compared to those with tetraplegia. The deficit in individuals with tetraplegia was larger to an extent that most of them did not reach normality in ROM at shoulder joint. Surprisingly, individuals with less severe injury according to the American Spinal Injury Association classification (ASIA D) showed larger deficit of ROM than expected. Joint pain was related to reduce ROM, particularly in participants with ASIA D lesion. Values of muscle strength in individuals with more severe tetraplegia (ASIA ABC) are lower than the ones observed in individuals with less severe injury (ASIA D). Overall, strength and ROM were correlated, except for medial rotation movement. ROM, strength, and secondary impairments are the major determinants of functional independence.
In conclusion, shoulder joint was the most affected joint of the upper limb, and, despite of significant gains of ROM, abnormalities were greater than expected. Strength and some secondary impairment were related in loss of ROM and functional independence throughout rehabilitation. More study will be needed to understand the medial rotation movement at the shoulder that seems to evolve differently from other movements, and to determine whether or not changes observed during rehabilitation are maintained over time.

Physical Activity and Cardiovascular DiseaseAndersen, Kasper January 2014 (has links)
The aim was to investigate associations of fitness and types and levels of physical activity with subsequent risk of cardiovascular disease. Four large-scale longitudinal cohort studies were used. The exposures were different measures related to physical activity and the outcomes were obtained through linkage to the Swedish In-Patient Register. In a cohort of 466 elderly men without pre-existing cardiovascular disease, we found that skeletal muscle morphology was associated with risk of cardiovascular events. A high amount of type I (slow-twitch, oxidative) skeletal muscle fibres was associated with lower risk of cardiovascular events and high amount of type IIx was associated with higher risk of cardiovascular events. This association was only seen among physically active men. Among 39,805 participants in a fundraising event, higher levels of both total and leisure time physical activity were associated with lower risk of heart failure. The associations were strongest for leisure time physical activity. In a cohort of 53,755 participants in the 90 km skiing event Vasaloppet, a higher number of completed races was associated with higher risk of atrial fibrillation and a higher risk of bradyarrhythmias. Further, better relative performance was associated with a higher risk of bradyarrhythmias. Among 1,26 million Swedish 18-year-old men, exercise capacity and muscle strength were independently associated with lower risk of vascular disease. The associations were seen across a range of major vascular disease events (ischemic heart disease, heart failure, stroke and cardiovascular death). Further, high exercise capacity was associated with higher risk of atrial fibrillation and a U-shaped association with bradyarrhythmias was found. Higher muscle strength was associated with lower risk of bradyarrhythmias and lower risk of ventricular arrhythmias. These findings suggest a higher rate of atrial fibrillation with higher levels of physical activity. The higher risk of atrial fibrillation does not appear to lead to a higher risk of stroke. In contrast, we found a strong inverse association of higher exercise capacity and muscle strength with vascular disease. Further, high exercise capacity and muscle strength are related to lower risk of cardiovascular death, including arrhythmia deaths. From a population perspective, the total impact of physical activity on cardiovascular disease is positive.

Type 1 diabetes mellitus (DM1) is associated with prooxidant and proinflammatory conditions, besides an increased cardiovascular risk, while exercise may be considered one of the best nonpharmacological tools for DM1 treatment. In this context, exercises linked with a lower hypoglycemic risk and several health benefits should be stimulated. One of the goals of this thesis was to verify the influence of performing strength exercises (SE) before or after highintensity interval exercise (HIIE) on glycaemia during and postexercise (cross-sectional study) (original manuscript 1). However, the main objective of this thesis was to compare the effects of high-intensity interval training (HIIT), strength training (ST) or their combination (ST+HIIT), on blood inflammatory, oxidative stress (OS) and glycemic markers in DM1 patients using a randomized clinical trial (ECR) (original manuscript 2). Regarding the crosssectional study (original manuscript 1), in three visits, physically active adults performed 30 min of SE before 30 min of HIIE or performed the reverse order (HIIE+SE) or rested for 30 min (REST). Capillary glycaemia was measured each 15 min during and 60 min postexercise recovery. HIIE+SE lowered glycaemia at 30, 45 and 60 min compared with baseline concentrations, while SE+HIIE postponed this glucose decayment to 60 min and thereafter. HIIE+SE increased glycaemia at 105 min compared with 60 min. Carbohydrates ingested during exercise, insulin dosage at same day before and after protocols, and nocturnal hypoglycemia episodes were similar among the three conditions. DM1 patients prone to develop exercise-associated hypoglycemia should perform SE before HIIE in a single session. Regarding the main study (ECR) (original manuscript 2), after 4-week control period, physically inactive patients with DM1 were randomly assigned to 10-week HIIT, ST or ST+HIIT protocol, performed 3 x/week. HIIT sessions lasted 25 min, ST lasted 40 min and ST+HIIT sessions lasted ~65 min. Blood biochemical, anthropometric, strength and cardiorespiratory fitness variables were assessed. Outcomes were analyzed via generalized estimating equations (GEE), with Bonferroni post hoc analysis. ST, HIIT and ST+HIIT improved glycemic and antioxidant parameters, but not plasma inflammatory or OS markers. Noteworthy, interventions reduced soluble receptors for advanced glycation end products levels. However, intracellular heat shock protein 70 content increased only after HIIT. While daily insulin dosage decreased only in the ST+HIIT group, all training models induced anthropometric and functional benefits. From a practical clinical perspective, a higher volume (SE+HIIT) of training is required for the additional benefit of daily insulin reduction. The HIIT, for example, is directly applicable for people who claim lack of time, and it may be 13 recommended due to extra advantage concerning anti-inflammatory proteins at immunological cells.

Previous issue date: 2010-05-03 / Universidade Federal de Minas Gerais / This work consists of two studies. The aim of the first study was to investigate quadriceps and hamstring muscle volumes and strength deficits of the knee extensors and flexors in people with chronic hemiparesis compared to a healthy group. Fifteen individuals with chronic stroke and fifteen healthy individuals took part in this study. Motor function, quadriceps and hamstring muscle volume (MV), and maximal concentric and eccentric contractions of the knee extensors and flexors were obtained. The quadriceps muscle showed a reduction in MV. The peak torque of the paretic and non paretic limbs knee extensors and flexors was reduced in both contraction modes and velocities. There were significant correlations between motor function and strength deficits. With regard to muscle mass, there was a difference between quadriceps and hamstring response. There was no disuse atrophy; however this did not prevent extensor torque reduction. The aim of the second study was to investigate the effects of ground level gait training combining body weight support (BWS) and functional electrical stimulation (FES). Twelve people following chronic stroke. An A1-B-A2 system was applied; A1 and A2 corresponded to ground level gait training using BWS, and B corresponded to the same training associated to FES. The analyzed variables were: mean walking speed of locomotion; step length; stride length, speed and duration; initial and final double support duration; single-limb support duration; swing period; range of motion (ROM), maximum and minimum angles of foot, leg, thigh, and trunk segments. Mean walking speed, stride length and speed increased. Regarding the ROMs, there was a significant increase for leg and thigh. There was not improvement in the variables measured during the FES phase. Ground level gait training using BWS was effective to improve some gait, and the association to FES did not provide any additional improvement in the measured parameters. Participaram do estudo 12 hemiparéticos crônicos.

Previous issue date: 2013-05-10 / Universidade Federal de Minas Gerais / Athlete´s shoulder most frequent lesions are anterior shoulder instability and superior labrum anterior and posterior lesion ( SLAP), altering structures and functions of the shoulder joint, leading to impairments in sports activity. The aim of this thesis was to evaluate functional and supraspinatus morphological adaptations of shoulder in athletes with anterior instability and SLAP lesion. Three studies were developed. The first study investigated functional differences regarding clinical complaints and the scoring systems of American Shoulder and Elbow Surgeons Standardized Shoulder Assessment Form (ASES) and the Athletic Shoulder Outcome Rating Scale (ASORS) in 249 athletes (153 with instability and 96 with SLAP lesion). The groups presented functional differences related mainly to sports activitiy and, even with more complaints of shoulder pain, SLAP lesion athlete´s tolerate more training hours compared to instability athletes. The second study evaluated isokinetic strenght and muscular control of shoulder rotators in four groups: instability group (n=10), instability control group (n=10), SLAP group (n=10) and SLAP control group (n=10). The torque steadiness and rotator cuff strength in 90 and 180°/s were evaluated. Athlete´s with anterior stability presented shoulder rotation weakness, while athletes with SLAP lesion showed higher torque fluctuation of shoulder internal rotation. The third study characterized the different fiber types of supraspinatus and shoulder function in those lesions. This study performed muscles biopsies in 10 athletes undergoing to arthroscopy repair of shoulder anterior instability and SLAP lesion. The volunteers were functionally evaluated with ASES and ASORS, and muscle samples were processed with histochemical reaction for myosin adenosine triphosphatase (mATPase). Muscles fibers were then characterized in type I, IIa, IIb and hybrid, and percentual and lesser diameter of each fiber type were calculated. In this study, volunteers presented evident functional limitations in sports activity and hypertrophy of all typees of muscles fibers. Most of the sample also presented a great number of type IIa and IIb in the supraspinatus muscle. In conjuction, those studies proves different functional and supraspinatus morphologic adaptations in athletes with anterior shoulder instability and SLAP lesion, justifying the need of different rehabilitations approaches.

Previous issue date: 2009-02-16 / Financiadora de Estudos e Projetos / Introduction: Individuals with chronic obstructive pulmonary disease (COPD) present a decrease in peripheral muscle strength, leading to impaired mobility and increased death risk. However, there are a lack of studies that verify if the peripheral muscle strength have relationship with the BODE index scale and fat-free mass. Objectives: To verify the relationship between maximum upper-limb (UL) muscle strength and lower-limb (LL) muscle strength with a BODE index, isolated variables and fat-free mass in subjects with moderate to very severe COPD. Methods: Twenty-six individuals with moderate to very severe COPD (FEV1<80% of predict) were evaluated regarding body composition (body mass index-BMI and fat-free mass), BODE index, handgrip force (HGF) and one repetition maximum test (1RM) of the UL and LL. Results: Observed moderate to strong positive correlation (Pearson, p <0.05) between peripheral muscle strength with the fat-free mass and distance walked in the six-minute walk test (6MWD). However, the BODE index were not correlated with force measurement or fat-free mass. Conclusion: It can be concluded that the evaluation of peripheral muscle strength is not related to the BODE index. In such case, the peripheral muscle force measures need be used assisting the assessment of COPD subjects performance.

Influência de parâmetros morfológicos, funcionais e psicológicos no quadro bioquímico, sanguíneo e imunológico de indivíduos infectados pelo HIV / Influence of morphological, functional and psychological parameters on biochemistry, blood and immunological status of HIV/aids individualsVagner Raso 05 December 2008 (has links)
Por outro lado, existe interdependência entre as diferentes características acima descritas nos diferentes grupos para CD4+ nadir (< 200 cél·mm3 e > 200 cél·mm3), sobretudo, para aqueles voluntários localizados no grupo CD4+ nadir < 200 cél·mm3 / The purpose of this study was to analyze the influence of morphological, functional and psychological parameters on biochemistry, blood and immunological status of HIV/aids individuals. Sample was constituted by 41 HIV infected men individuals (25 66 year-old [x: 40.75 ± 8.68 years]) of the Secondary Immunodeficiency Ambulatory from Hospital das Clínicas Faculty of Medicine, USP (ADEE 3002-HC-FMUSP). Volunteers were agrouped into two groups according to the CD4+ nadir (< 200 cell·mm3 or > 200 cell·mm3). The variables measured were body composition, cardiorespiratory and neuromuscular parameters, functional capacity, quality of life, physical activity, sexual satisfaction, depressive symptoms, as well as biochemical, blood and immunological parameters. There were statistically significant differences between the groups only for maximum walking velocity, total cholesterol, LDL and CD4+ nadir (p<0.05). There was tendency of the volunteers in the group CD4+ nadir < 200 cell·mm3 to show more prevalence and higher significant correlation values in the association among body composition, neuromuscular and cardiorespiratory parameters, functional capacity, quality of life, physical activity, sexual satisfaction and depressive symptoms and biochemical, blood and immunological parameters. The data permite to conclude that there was not strongly association among body composition, neuromuscular and cardiorespiratory parameters, functional capacity, quality of life, physical activity, sexual satisfaction and depressive symptoms, and biochemical, blood and immunological parameters regardless of historical clinical status. On the other hand, it was observed the existence of an interdependence phenomenon among the previously described different characteristics in the distint groups to CD4+ nadir (< 200 cell·mm3 and > 200 cell·mm3), specially, for the volunteers of the CD4+ nadir < 200 cell·mm3
