Die Studie diente der retrospektiven Untersuchung des Ausbruches von Mp-Infektionen in Ebrach, Franken, der von Oktober des Jahres 2000 bis Februar 2001 andauerte. Ziel war es, die epidemiologischen Charakteristika, also Informationen zu Verteilung und Ausbreitungsweisen der Erkrankung, aber auch zu Symptomen und Befunden, Manifestationsformen und Komplikationen, Therapie und Diagnostik zu erhalten. Darüber hinaus sollten Erkenntnisse zu Patienten mit Mykoplasmeninfektionen, die in den Jahren 2000 bis 2003 in der Universitätskinderklinik Würzburg behandelt wurden, gewonnen und mit Daten der Patienten aus Ebrach verglichen werden. In Ebrach bestand bei 177 Patienten der Verdacht einer akuten Mykoplasmeninfektion. Ausgehend von einer dritten Grundschulklasse, die einige Tage geschlossen werden musste, da innerhalb von 16 Tagen 9 Schüler an einer Pneumonie und 3 Schüler an einer Bronchitis erkrankt waren, hatte sich die Infektion auf insgesamt 78 Personen, vor allem Familienmitglieder, aber auch Nachbarn und Freunde der betroffenen Schüler ausgebreitet. Die meisten Patienten klagten über Husten und Fieber. In erster Linie traten Entzündungen des unteren Respirationstraktes (50% Bronchitiden, 38,5% Pneumonien) auf. Bei 9 Patienten wurde ein Exanthem beobachtet. Eine Patientin musste wegen eines Guillain-Barré-Syndroms in der neurologischen Abteilung der Universitätsklinik Würzburg behandelt werden. In den Jahren 2000 bis 2003 bestand bei 125 Patienten der Universitätskinderklinik Würzburg der Verdacht auf Vorliegen einer Mp-Infektion. Bestätigt wurde dieser in 43 Fällen. Die Patienten waren zwischen 3 und 16 Jahre alt. Insgesamt waren etwas mehr Jungen betroffen, Komplikationen traten deutlich häufiger bei Mädchen auf. Die Patienten, die einer stationären Behandlung bedurften, wiesen schwerere Erkrankungsverläufe oder seltenere Manifestationsformen auf (65% Pneumonien, 34% Komplikationen). So wurden unter anderem 6 Patienten mit Mykoplasmen-assoziierter Fazialisparese, 4 Patienten mit Meningitis und jeweils ein Patient mit Enzephalitis, Trochlearisparese, Vestibularisausfall, Hörverlust, Perimyokarditis und Uveitis anterior und nephrotischem Syndrom beobachtet. Pathognomonische Befunde konnten weder unter den Ebracher Patienten noch in der Kinderklinik ausgemacht werden. Vielmehr spricht die Konstellation bestimmter Symptome und Untersuchungsergebnisse wie Husten, Fieber, relativ guter Allgemeinzustand bei radiologischem Pneumonienachweis oder Differenz der Blutsenkungsreaktion bei Raumtemperatur und 4°C für das Vorliegen einer Mykoplasmeninfektion. Eine deutliche Erhöhung der Inzidenz von Mykoplasmeninfektionen in der Kinderklinik im Zeitraum des Ausbruches von Ebrach war nicht zu verzeichnen. Dass Schüler als Überträger der Infektion in Familien und unter Spielkameraden fungieren, war bekannt, die Ausbreitung der Erkrankung innerhalb des Klassenzimmers ist jedoch selten in diesem Ausmaß beobachtet worden und verdient weitere Untersuchungen. Festzuhalten bleibt also, dass bei der Diagnose einer Mykoplasmeninfektion mittels serologischer Methoden mit einer verzögerten Immunantwort zu rechnen ist und deshalb häufig ein Direktnachweis der Erreger mittels PCR notwendig wird. Darüber hinaus ist die Bestimmung der Blutkörperchensenkungsgeschwindigkeit bei Raum- und Kühlschranktemperatur ein einfaches Mittel, welches aber diagnostisch zusätzlich wichtige Hinweise auf eine Infektion mit Mycoplasma pneumoniae liefern kann. Im Gegensatz dazu erbringt die klinische Untersuchung häufig keine aussagekräftigen, diagnostisch weiterführenden Ergebnisse. Wichtig bezüglich der Therapie ist die frühzeitige und ausreichend lange (10 bis 14 Tage) Gabe von gegen Mykoplasmen wirksamen Antibiotika wie vor allem Makrolid-Antibiotika. / This is a retrospective study of an outbreak of Mycoplasma pneumoniae infections between october 2000 and february 2001 in Ebrach, Franken, Germany. We wanted to get information about epidemiologic characteristics, such as spread of infection, clinical manifestations and complications, as well as influence of therapy, and possibilities of diagnostics. Furthermore we monitored patients with Mycoplasma pneumoniae infections which led to hospitalisation in the pediatric department of Würzburg university hospital and compared them to the patients in Ebrach. In Ebrach a total of 177 patients were thought to have an MP infection. It started in year 3 of primary school, where within 16 days 9 pupils suffered from pneumonia due to MP and 3 children had MP bronchitis. For this reason the pupils where not allowed to attend school for a few days. Beginning with the school children the infection reached 78 persons, mainly parents of the children but also neighbours and friends for example from the football team. Most of them suffered from coughs and fever. Manifestations were infections of the lower respiratory tract (38,5% pneumonia, 50% bronchitis), 9 patients suffered from cutaneous symptoms (exanthema). One patient had to be hospitalized because of a Guillain-Barre-syndrom. Between 2000 and 2003, 125 patients of the pediatric department of the Würzburg university hospital were thought to have MP infections. In 43 cases the MP infection was diagnosed. Patients were between 3 and 16 years old, there were bit more cases amongst males but females got more complications. Hospitalized patients showed more severe manifestatons (65% pneumonia) or complications (34%). These were for example 6 children with Bells palsy, 4 children with menigitis and one of each of the following manifestations: encephalitis, cranial nerve palsy of trochlearis and vestibularis, hearing loss, permyocarditis, uveitis anterior, nephrotic syndrom. No special symptoms which could be said to be pathognomonic were found in either Ebrach or amongst the hospitalized patients. The special constellation of pathological findings much rather suggests a diagnosis of an MP infection: cough, fever, quite good clinical condition, radiographic evidence of pneumonia, different wsg results at 37°C and 4°C. The incidence of MP infections among hospitalized patients did not increase during the time of the outbreak in Ebrach. It is already known that pupils are possible vectors but such a spread of MP infection in the classroom has seldom been observed before and needs further investigation. Serologic diagnosis needs more time because of the delayed immune response of the host and this is why pcr is often necessary. Antibiotic treatment with effective drugs such as macrolides should be taken into consideration early and administered for a long enough period.

O urubu-de-cabeça-preta (Coragyps atratus) é uma ave de vida livre com ampla distribuição no Brasil. Esta espécie é comumente encontrada em áreas urbanas concentrando-se em locais de deposição de lixo. O fato de se alimentarem de carcaças em decomposição facilita o contato de urubus-de-cabeça-preta com muitos patógenos. No entanto, ainda não está clara qual a real implicação de muitos desses microrganismos para a saúde dos mesmos. Assim, o objetivo desse estudo foi investigar a ocorrência de alguns patógenos selecionados, avaliar o perfil hematológico e a microbiota cloacal de C. atratus em ambiente urbano. Para isso, amostras de sangue, soro e swab cloacal foram obtidos de 120 urubus de vida-livre capturados na Fundação Parque Zoológico de São Paulo, SP. A prova de soroaglutinação rápida (SAR) foi utilizada na detecção de anticorpos contra Salmonella Pullorum/Gallinarum, Mycoplasma synoviae e M. gallisepticum. O teste de aglutinação em látex (AL) foi utilizado para a pesquisa de antígeno de Cryptococcus neoformans. Foram utilizadas técnicas convencionais de hematologia, microbiologia e testes de sensibilidade microbiana. Das amostras de soro analisadas pela SAR, 15% foram reagentes para M. gallisepticum. Anticorpos contra S. Pullorum/Gallinarum e M. synoviae não foram detectados. Nenhuma amostra foi positiva para C.neoformans ou para hemoparasitas. A média e o desvio padrão dos seguintes valores hematológicos foram obtidos para 61 aves: eritrócitos (1.8x10¹²/L); leucócitos (13,11x10/L); hemoglobina (10,4 g/dL); hematócrito (48,44%); VCM (275,1 fL); HCM (42 pg); CHCM (15,8 g/dL); proteína sérica total (3,76 g/dL); heterófilos (78%); linfócitos (13,5%); eosinófilos (5,4%); monócitos (2,8%); basófilos (0,1%); trombócitos (14,14x10/L). De 75 colônias bacterianas isoladas de 20 swabs cloacais, 78,7% foram Gram-positivas e 21,3% Gram-negativas, sendo Enterococcus sp. o gênero mais frequente. Aproximadamente 86,7% das cepas isoladas foram resistentes a pelo menos um dos antibióticos testados. Cepas de Bacillus sp. e Enterococcus casseliflavus apresentaram resistência a sete dos oito antibióticos testados. Leveduras não foram isoladas em nenhumas das culturas. As informações obtidas nessa pesquisa são de suma importância, uma vez que poucos estudos avaliam o estado de saúde de urubus no mundo. / Black vulture (Coragyps atratus) is a free-living bird widely distributed across Brazil. These birds feed on rotting carcasses and large groups are commonly found in urban areas, including rubbish dumps. By feeding on decomposing carcasses, they are often exposed to innumerous pathogens. However, the role of infectious microorganisms on vultures health still need to be clarify. Thus, the aim of this study was to investigate the occurrence of selected infectious agents, the hematological profile and cloacal microbiota of black vulture in urban areas. Therefore, blood, serum and cloacal swabs were obtained from 120 free-living vultures trapped in Fundação Parque Zoológico de São Paulo, SP. The rapid seroagglutination test (RST) was performed for detection of antibodies against Salmonella Pullorum/Gallinarum, M. synoviae and M. gallisepticum. Furthermore, latex agglutination test was used to detect Cryptococcus neoformans \' antigen. Conventional techniques for hematology, microbiology and antimicrobial susceptibility testing were performed. From the serum samples analyzed by RST, 15% were positive for M. gallisepticum, antibodies against S. Pullorum/Gallinarum and M. synoviae were not detected. None sample was positive to Cryptococcus neoformans or hemoparasites. Mean and standard deviation from the following hematological values were obtained for 61 birds: erythrocytes (1.8x10¹²/L); leukocytes (13.11x10/L); hemoglobin (10.4 g/dL); hematocrit (48.44%); MCV (275,1 fL); MCH (42 pg); MCHC (15,8 g/dL); total serum protein (3.76 g/dL); heterophils (78%); lymphocytes (13.5%); eosinophils (5.4%); monocytes (2.8%); basophils (0,1%); thrombocyte (14.14x10/L). From 75 bacterial colonies isolated from 20 cloacal swabs, 78.7% were Gram-positive and 21.3% were Gram-negative. Enterococcus sp. was the most frequent genus. Approximately 86.7% of the isolated strains were resistant to at least one of the antibiotic tested. Bacillus sp. and Enterococcus casseliflavus strains shown resistance to seven in eight antibiotics tested. Yeasts were not isolated. The information obtained in this research is of paramount important since few studies have been carried out on the vultures health condition in the world.

A agalaxia contagiosa é uma doença de notificação obrigatória que causa severas perdas econômicas para produção de ovinos e caprinos mundialmente. Apesar de seu impacto na produção animal, pouco se sabe sobre os fatores de virulência e patogenicidade do M. agalactiae (principal agente etiológico). Desta maneira, o presente estudo possuiu como objetivo a identificação, purificação e caracterização de proteínas antigênicas de M. agalactiae. Para tanto, quatro proteínas de superfície com potencial antigênico (WP_011949419.1, WP_011949418.1 (P40), WP_011949336.1, WP_011949770.1) foram selecionadas. Essas proteínas foram expressas em Escherichia coli e purificadas em coluna de níquel. As proteínas purificadas foram avaliadas quanto a antigenicidade em Western blotting utilizando soros de caprinos naturalmente infectados com M. agalactiae. Todas as proteínas expressas foram imunorreativas aos soros de caprinos naturalmente infectados, demonstrando que as proteínas utilizadas nesse estudo são possivelmente antigênicas e possuem epítopos acessíveis. / The Contagious agalactia is a notifiable disease that causes severe economic losses to sheep and goats worldwide. Despite its impact on animal production, little is known about the virulence factors and pathogenicity of M. agalactiae (main etiological agent). Thus, the present study identified, purified and characterized antigenic proteins of M. agalactiae. Therefore, four surface proteins with antigenic potential (WP_011949419.1, WP_011949418.1 (P40), WP_011949336.1, WP_011949770.1) were selected. These proteins were expressed in Escherichia coli and purified on nickel column. The purified proteins were assayed for antigenicity by Western blotting using goat sera naturally infected with M. agalactiae. All expressed proteins were immunoreactive with sera from naturally infected goats, demonstrating that the proteins used in this study are possibly antigenics and it have acessible epitopes.

Foram obtidos dados clínico-demográficos, swab vaginal e sangue de 302 mulheres. Detectou-se Mollicutes, Mycoplasma hominis e M. genitalium em 76,2%, 15,9% e 1,7% das amostras respectivamente. Frequência de 31,8% e 28,1% na qPCR foi encontrada para M. genitalium e M. hominis respectivamente. A infecção por micoplasmas foi associada a sinais e sintomas clínicos. A frequência de Trichomonas vaginalis Neisseria gonorrhoeae Gardnerella vaginalis e Chlamydia trachomatis foi de 3,0%, 21,5%, 42,4%, 1,7% respectivamente. Co-infecção destas espécies com micoplasmas foi encontrada. No polimorfismo da IL-1b, houve maior prevalência do genótipo CC, no entanto, o alelo T foi relacionado ao pequeno aumento dos níveis da interleucina no grupo caso. Para o polimorfismo de IL-6, o genótipo GG apresentou maior frequência, e o genótipo GC foi associado ao aumento dos níveis plasmáticos da citocina no grupo caso. O aumento dos níveis de IL-1b foi associado à presença M. hominis e ao fato das mulheres apresentarem sinais ou sintomas de infecção. / Clinical and demographic data, samples of vaginal swab and peripheral blood were obtained from 302 women. Mollicutes, Mycoplasma hominis and M. genitalium were detected in 76.2%, 15.9% and 1.7%, respectively. Frequency of 31.8% and 28.1% in qPCR was found for M. genitalium and M. hominis respectively. Infection by genital mycoplasmas, especially when analyzed by qPCR, was significantly associated with clinical signs and symptoms of sexually transmitted infections. The frequency of Trichomonas vaginalis, Gardnerella vaginalis, Neisseria gonorrhoeae and Chlamydia trachomatis was 3.0%, 21.5%, 42.4% and 1.7% respectively. Co-infection among these species and mycoplasmas was founded. In polymorphism of IL-1b, the genotype CC was more prevalent; however, the T allele was related to small increase of the interleukin levels in the group case. In the polymorphism of IL-6, higher frequency was of the genotype GG, and the GC genotype was associated with increased plasma levels of the cytokine in the group case. Increased levels of IL-1b were associated with presence of M. hominis and signs and/or symptoms of infection.

Espécies emergentes de micoplasmas do trato geniturinário em pacientes associados ou não à infecção pelo HIV-1, detectados por técnicas sorológicas, de cultivo e de biologia molecular / Mycoplasma emergent species of the genitourinary tract in patients associated or not to HIV-1 infection, detected by serologic, culture and molecular biology techniques

Este trabalho procurou definir a existência de espécies emergentes de micoplasmas (Mycoplasma genitalium, M. fermentans e M. penetrans) em indivíduos HIV-positivos e indivíduos com queixa clínica de retrite, por não haver dados a esse respeito no Brasil. A pesquisa das espécies mais conhecidas (M. hominis e Ureaplasma urealyticum), cuja metodologia para sua detecção já está bem estabelecida, proporcionou uma visão global da participação das espécies de importância clínica conhecidas até o momento. Foram avaliadas amostras de raspado uretral e primeiro jato urinário de 106 indivíduos HIV-positivos e 110 indivíduos HIV-negativos com DST, no total de 212 e 220 amostras do primeiro e do segundo grupo, respectivamente. Deste modo, foram submetidas a análise 432 amostras para pesquisa de micoplasmas por cultura e PCR. Para pesquisa de anticorpos séricos anti- M. penetrans, foram obtidas amostras de soro de cada indivíduo dos dois grupos estudados, bem como de 86 doadores de sangue saudáveis para cálculo do limite de reatividade do ELISA. As taxas de infecção obtidas em nosso estudo, considerando os resultados do cultivo e da PCR, nos indivíduos HIV-positivos, foram: 2,8% para a espécie M. genitalium, 5,7% para M. fermentans, e 6,6% para M. penetrans. Nos indivíduos com queixa de DST, os valores encontrados foram: 9,1% para M. genitalium e 0,9% para M. fermentans, não tendo sido observado nenhum caso positivo para M. penetrans. Estes últimos resultados revelam que, apesar da pouca importância dada até então às três espécies, estas responderam, no total, por taxas de infecção de 15,1% e 10,0%, respectivamente, nos grupos HIV-positivo e DST. Estas taxas são bastante significativas quando comparadas com as observadas para as espécies M. hominis e U. urealyticum, ou seja, 26,4% e 14,5% em cada grupo, respectivamente. Nossos dados demonstraram uma freqüência significativa de anticorpos anti- M. penetrans: 25,5% no grupo de indivíduos HIV-positivos, 17,3% no grupo de DST, e 2,3% no grupo controle para IgG; 3,8%, 9,1% e 5,8% para IgM, e 15,1%,17,3% e 1,2% para IgA, respectivamente. Estes dados, aliados aos resultados de PCR, confirmam a presença de M. penetrans em nosso meio, infectando principalmente os indivíduos HIV-positivos. A presença desta espécie e também de M. fermentans, detectadas por PCR, denotam que muito pouco se conhece sobre o envolvimento destes micoplasmas com o HIV e que ainda não lhes é dada a devida importância na rotina médica. A associação definitiva de sua infecção com a progressão da AIDS, ainda a ser alcançada, e a demonstração em nossa população da infecção por M. genitalium em indivíduos com queixa de uretrite não-gonocócica, abre novos horizontes para o estudo da participação dos micoplasmas na etiologia das doenças humanas no país. / The aim of this work was to define the existence of emergent mycoplasma species, such as Mycoplasma genitalium, M. fermentans and M. penetrans, in HIV-1-infected and HIV-negative individuals with Sexually Trasmitted Diseases (STD), because there are no data concerning this subject in Brazil. The research about well-known species, such as Ureaplasma urealyticum and M. hominis, of wich detection techniques are well established, provided a comprehensive view of the participation of Mollicutes in human disease. We evaluated urethral swabs and urine samples from 106 HIV-1-infected and 110 HIV-negative individuals with clinical symptoms of non-gonococal urethritis, summing up 212 samples from the first group, and 220 from the second. Therefore, we tested a total of 432 samples to mycoplasma detection by culture and PCR. To detection of serum antibodies to M. penetrans, we obtained blood samples of each patient from the groups studied, as well as from 86 healthy blood donors to calculate the ELISA (Enzime Linked Immuno Sorbent Assay) cut-off value. The rates of infection obtained in our study, considering the culture and PCR results, in the HIV-infected individuals were: 2.8% for M. genitalium, 5.7% for M. fermentans, and 6.6% for M. penetrans. In the STD group, the rates were: 9.1% for M. genitalium and 0.9% for M. fermentans. We did not observed any positive case for M. penetrans in this group, and no positive case for M. pneumoniae in both groups, in spite of the existence of some reports of its detection in the genitourinary tract. This results reveal that they account, in the total, for infection rates of 15.1% and 10%, respectively, in the HIV-infected and the STD groups. These rates are very significant when compared to the ones observed for the species M. hominis and U. urealyticum, i.e., 26.4% and 14.5% in each group, respectively. Our results also show a significant antibody frequency to M. penetrans: 25.5% in the HIV-infected group, 17.3% in the STD group, and 2.3% in the control group for IgG; 3.8%, 9.1% and 5.8% for IgM, and 15.1 %, 17.3% and 1.2% for IgA, respectively. These data, together with the PCR results, confirm the presence of M. penetrans in our population, mainly infecting HIV- positive patients. The presence of this species, and also of M. fermentans, detected by PCR, demonstrate that very little is known about the role of this mycoplasma species in HIV infection, and they are not given the appropriate importance in clinical routine in Brazil. The definitive association between mycoplasma infection and the progression of AIDS is still to be established. The demonstration of M. genitalium infection in individuals with clinical symptoms of non-gonococal urethritis in our population, may open new insights in the study of the role of mycoplamas in the ethiology of human diseases in this country.

Pesquisou-se a freqüência de Mollicutes na orofaringe, conjuntiva e trato urogenital de 58 macacos. Na orofaringe detectou-se Mollicutes em 55,17 % e Ureaplasma spp em 43,10%. As espécies identificadas nesta região foram: M. arginini (43,10%), M salivarium (41,37%), e M. pneumoniae (18,96%). No trato genital detectou-se Mollicutes em 27,58% sendo identificado M. arginini em 8,62%, A. laidlawii em 1,72 % e Ureaplasma spp em 32,75%. Na conjuntiva detectou-se Mollicutes em 29,31 % e A. laidlawii em 1,72 %. Os cultivos apresentaram limitações pelo alto índice de contaminação e obtiveram-se apenas dois isolamentos da conjuntiva. As espécies detectadas constituem-se o achado inicial destas bactérias em macacos no Brasil. / The frequency of Mollicutes in oropharynx, conjunctiva, and urogenital tract were accessed in 58 monkeys. In oropharynx, Mollicutes and Ureaplasma spp were detected in 55.17% and 43.01% of the samples, respectively. The identified species in this site included: M. arginini (43.10%), M. salivarium (41.37%), and M. pneumoniae (18.96%). In the urogenital tract, Mollicutes were detected in 27.58% of the samples; including M. arginini (8.62%), A. laidlawii (1.72%) and Ureaplasma spp (32.75%). In the conjunctiva, Mollicutes were detected in 29.31% and A. laidlawii in 1.72% of the animals. Mollicutes culture showed technical limitations because of the high level of contamination and only two isolates were obtained; both from conjunctival sites. The Mollicute specie surveillance of this study provided initial and new insights about these bacteria in Brazilian monkeys.

Os efeitos clastogênicos causados por diferentes cepas Ureaplasma urealyticum e por cepas do gênero Mycoplasma foram avaliados \"in vitro\", utilizando-se culturas temporárias de linfócitos humanos. Inibições, total ou parcial da mitose foram produzidos pelos sorotipos II, III e X de Ureaplasma urealyticum, independentemente das concentrações dos inóculos de micoplasmas utilizados. Já os sorotipos I, VII e XII do Ureaplasma urealyticum, a cepa Mycoplasma hominis ATCC 23114 e a cepa Mycoplasma pneumoniae foram influenciadas pela concentração dos micoplasmas. Mitoses alteradas foram observadas nas cepas Ureaplasma urealyticum VIII, IX e X. A maior frequência de alterações foram de gaps cromatídicos e quebras cromatídicas. Os resultados obtidos neste modelo experimental \"in vitro\" revelaram haver comportamentos diferentes entre os vários sorotipos de Ureaplasma urealyticum e entre espécies do gênero Mycoplasma. Essas diferenças observadas, \"in vitro\", poderão de alguma forma contribuir para melhor compreensão dos efeitos da colonização desses microorganismos em humanos. / Clastogenic effect caused by different strains of Ureaplasma urealyticum and by strains of Mycoplasma sp were evaluated in vitro by using temporary cultures of human linphocytes. Mitotic inhibitions, either total or partial were produced by serotypes lI, III and X of Ureaplasma urealyticum, indenpendently of the concentration of microorganisms used. Otherwise, the kind of clastogenic effect of Ureaplasma urealyticum of serotypes I, VII and XII, Mycoplasma hominis strain 23144 and Mycoplasma pneumoniae varied with the different concentrations of these mycoplasmas used in the test. Mitotic alterations were observed in Ureaplasma urealyticum of sorotypes VII, IX and X. Cromatidic gaps and cromatidic rupture were the most frequent kind of alterations evidenced. The results obtained in the present in vitro experiment revealed that the occurence of different kind of clastogenic effect depends on the serotypes of Ureaplasma urealyticum and on the strains of Mycoplasma sp. The variability of the effect obtained will contribute to the better comprehension of the effects of colonization of these microorganisms in humans and stimulate future in vivo investigation.
