Barnmorskestudenters upplevelser av verksamhetsförlagd utbildning på förlossningsavdelning / Midwifery student’s experiences of clinical internship at labour wards

Bakgrund: Barnmorskans yrkesområde är inom sexuell, reproduktiv och perinatal hälsa. Detövergripande målet i barnmorskeutbildningen är att utveckla självständiga och kompetenta barnmorskor. Den verksamhetsförlagda utbildningen (VFU) motsvarar ungefär halva studietiden och är en viktig del i barnmorskeutbildningen. Förlossningsavdelningen är en viktig miljö för barnmorskestudenter att inhämta kunskaper om graviditet och förlossning. Motiv: VFU på förlossningsavdelning är en känslomässig och intensiv period för barnmorskestudenter. Barnmorskestudenters uppfattningar om vad som främjar utveckling behöver studeras vidare för att kunna optimera framtida verksamhetsförlagda utbildningar. Syfte: Det övergripande syftet var att belysa barnmorskestudenters upplevelser av verksamhetsförlagd utbildning på förlossningsavdelning.  Metod: En kvalitativ design med induktiv ansats har använts. Semistrukturerade intervjuer med sju barnmorskestudenter i Sverige genomfördes. Intervjuerna har analyserats genom en kvalitativ innehållsanalys. Resultat: Genom analysprocessen framkom ett tema, VFU på förlossningsavdelning – en känslomässig berg- och dalbana samt tre kategorier; Att både ge och ta emot stöd där en mängd faktorer som kan påverka mötet med kvinnan och stödpersonen beskrevs. Andra kategorin Att hålla ihop hela vägen beskrev hur barnmorskestudenternas hälsa varierade samt svårigheter i att gå från en trygg yrkesroll till en studentroll. Tredje kategorin Hinder i utvecklingsprocessen visade hur Covid-19-pandemin påverkade barnmorskestudenternas VFU, optimeringar inför framtida VFU samt tankar om den framtida yrkesrollen. Åtta subkategorier bildades. Konklusion: Barnmorskestudenterna upplevde sin VFU på förlossningsavdelning som en känslomässig berg- och dalbana. Flertalet faktorer som tidigare erfarenheter, relationen till handledaren och reflektionsmöjligheter påverkade barnmorskestudenters möte med kvinnan och stödpersonen. Att belysa dessa faktorer och inkludera barnmorskestudenter i den kvinnocentrerade vården kan leda till ökat självförtroende och kompetens hos barnmorskestudenter vilket i sin tur kan leda till optimeringar av den kvinnocentrerade vården. / Background: The midwife's profession is within the sexual, reproductive and perinatalhealth. The overall purpose of midwifery education is to develop independent and competent midwives. The clinical internship is an important part of the education. The labour ward is an important environment for midwifery students to acquire knowledge about pregnancy and childbirth. Motive: Clinical internship in the labour ward is an emotional and intense period for midwifery students. Midwifery student’s perceptions of what promotes development needs to be studied further, in order to optimise future clinical internships. Aim: The aim was to investigate midwifery students experiences of clinical internship at labour wards.  Methods: A qualitative design with an inductive approach was used. Semi-structured interviews with seven midwifery students in Sweden participated. The interviews were analysed with qualitative content analysis. Result: A theme emerged, Clinical internship at the labour ward - an emotional roller coaster and three categories; To both give and receive support where a number of factors that can affect the meeting with the woman and the support person were described. Second category Holding together all the way described how the midwifery student’s health varies as well as difficulties in the student role. The third category Obstacles in the development process showed how the Covid-19-pandemic affected midwifery student’s clinical internship, optimisations and thoughts about the future professional role.  Conclusion: The clinical internship at the labour ward was described as an emotional roller coaster. Previous experiences, the relationship with the supervisor and opportunities for reflection influenced the midwifery student's encounter with the woman and the support person. Elucidating these factors and including midwifery students in women-centered care can lead to increased self-confidence and competence among midwifery students, this couldlead to optimisations of the women-centered care.

The motives behind Swazi men's increasing interest in circumcision

The demand for circumcision in Swaziland has dramatically increased since it was reported that circumcision ensures 60% protection against HIV infection. The aim of this study was to explore the reasons for, and the motives behind, this increasing interest in undergoing circumcision. A generic qualitative research design was used, in which 17 circumcised men selected by convenience sampling at FLAS Clinic, Mbabane, participated. In-depth unstructured face-to-face interviews were used to collect data. The results showed that the main reasons for circumcision are perceived health, hygiene and sexual benefits, community influences, as well as medical reasons. It was concluded that the major motives underlying circumcision are personal and partner safety and satisfaction during sex. These motives were found to be both promotive and threatening to the interests of public health. After evaluating the risks and benefits, it was recommended that mass circumcision be promoted alongside reinforced and accurate health education on circumcision / Health Studies / M.A. (Public Health)

Contexte. La présence limitée d’infirmières expérimentées force les milieux de soin à solliciter les infirmières débutantes avec peu d’expérience pour jouer le rôle de préceptrice. Pour tenter de remédier à la difficulté de recrutement de préceptrices, l’unité collaborative d’apprentissage (UCA) a été mise en place en 2003 en Colombie-Britannique. L’UCA est un modèle d’enseignement et d’apprentissage où une étudiante est assignée à une unité plutôt qu’à une préceptrice spécifique. Objectifs. Le but de cette étude était d’explorer les besoins d’apprentissage et de soutien des infirmières débutantes ayant joué le rôle de préceptrice sur une UCA. Méthodologie. Un devis qualitatif exploratoire a été utilisé pour répondre au but et aux questions de recherche. Cinq infirmières débutantes ayant participé au projet pilote de l’UCA dans un centre hospitalier universitaire d’une métropole dans l’est du Canada en février 2019 ont été recrutées. Les données ont été collectées par des entrevues individuelles semi-structurées et les écrits de Paillé et Mucchielli (2012) ont guidé les étapes d’analyse thématique. Résultats. Les résultats de cette étude ont permis de mettre en lumière le double rôle comme le défi central des infirmières débutantes préceptrices sur l’UCA, c’est-à-dire la coexistence du rôle d’infirmière débutante avec celui de préceptrice, les tensions induites par les difficultés rencontrées dans ce double rôle et les besoins et attentes des préceptrices pour rétablir l’équilibre dans ce double rôle. Conclusion. Une meilleure compréhension des besoins des infirmières débutantes préceptrices sur une UCA pourrait permettre d’offrir des formations sur le rôle attendu d’une préceptrice sur ce type d’unité et d’offrir un soutien adapté aux besoins de ces infirmières afin de les aider à poursuivre leur développement professionnel dans ce rôle. / Context. The limited presence of experienced nurses forces health care settings to seek out junior nurses with little experience to act as preceptors. In an attempt to address the difficulty in recruiting preceptors, the Collaborative Learning Unit (CLU) was implemented in 2003 in British Columbia. The CLU is a model of teaching and learning where a student is assigned to a unit rather than a specific preceptor. Objectives. The purpose of this study was to explore the learning and support needs of novice nurses who have taken on the role of preceptor on an CLU. Methodology. An exploratory qualitative design was used to address the research purpose and questions. Five novice nurses who participated in the CLU pilot at a metropolitan teaching hospital in eastern Canada in February 2019 were recruited. Data were collected through semi-structured individual interviews and the writings of Paillé and Mucchielli (2012) guided the thematic analysis steps. Results. The results of this study highlighted the dual role as the central challenge for novice nurse preceptors on the CLU, that is, the coexistence of the novice nurse role with the preceptor role, the tensions induced by the difficulties encountered in this dual role, and the needs and expectations of preceptors to restore balance in this dual role. Conclusion. A better understanding of the needs of novice nurse preceptors on an CLU could lead to training on the expected role of a preceptor on this type of unit and to support tailored to the needs of these nurses to help them continue their professional development in this role.

The role of the principal as instructional leader in improving learner achievement in South African primary schools

The role played by the principal as instructional leader worldwide is critical due to its direct and indirect impact on learner achievement.In South African primary schools the role of the principal as instructional leader in improving learning achievement is a pressing issue since most South African primary school learners perform far below par as determined by benchmarking carried out by local and international education assessment agencies with reference to curriculum goals and literacy and numeracy milestones.This descriptive exploratory study examined the role of the principal as instructional leader in primary schools in South Africa with special reference to the Tshwane South District, Gauteng Province. The study was informed by a literature review of local and international sources related to theoretical perspectives on instructional leadership, the features of instructional leadership that impact on learner achievement and selected models of instructional leadership. An overview of contextual and school factors which shape primary school learner achievement in South Africa with reference to the rural and township communities also framed the empirical inquiry. A mixed method study was conducted according to two phases: Phase 1 comprised the quantitative component (questionnaires); Phase 2 comprised the qualitative component (interviews). Both phases were conducted in the Tshwane South District, Gauteng Province. In both phases data were gathered from primary school principals and Institute Development Support Officials (IDSOs) selected by random and purposeful sampling respectively. The questionnaire findings indicated that the principals and IDSOs had adequate qualifications and experience, and that key school policies are in place. However, the principals are less confident about policy implementation, particularly with regard to educator and learner discipline. They face many systemic problems, such as inadequate school resources and additional funds in no fee-paying schools to improve the infrastructure and reduce the learner-teacher ratios. The findings from the interviews indicated that the principals struggle to maintain participative management with the school governing bodies; are hampered by contextual factors, especially poor school infrastructure; and are challenged to meet the requirements of annual benchmarking assessments. Based on the findings of the literature study and the empirical inquiry recommendations are made to improve practice, including a call for the professional development of principals in project management and fundraising. / Educational Leadership and Management / D.
