The research dealt with the implementation of Gestalt play therapy for the treatment of the sexually abused primary school child who experiences problems to maintain himself. The need for research on the theme originated from the researcher's experience with sexually abused primary school children. The aim of the research was to render therapeutic support to the primary school child through Gestalt play therapy, in order to explore the influence of such support on the child's maintaining behaviour. The applied research design was used for this purpose. For the purpose of the research, one sexually abused child who experienced problems to maintain himself was identified by means of purposeful sampling. During each Gestalt play therapy session the child was brought into contact with his senses. Conclusions as well as recommendations were formulated as a result of the research. / Social Work / M. Die kliniese beeld sowel as die fisiese- en gedragsimptome is kriteria by die uitkenning van seksuele molestering. Die bantering van die seksueel-gemolesteerde kind is medebepalend vir die onmiddellike en langtennyn gevolge van seksuele molestering. Seksuele molestering word vanuit 'n sielkundig-opvoedkundige perspektief bespreek. Die studie neem die seksueel-gemolesteerde kind se betekenisgewing, belewing, betrokkenheid, selfaktualisering en selfkonsep in beskouing. Terapeutiese tegnieke, soos toegepas in gevalle van seksuele molestering, word vir gebruik deur professionele persone en instansies, bespreek. Die empiriese ondersoek fokus op die houdinge, hanteringswyses en toepassing van terapeutiese tegnieke deur professionele persone en instansies in die Witbank omgewing. Volgens die navorsingsresultate is daar geen uniforme hanteringswyse of riglyne vir terapie ten opsigte van seksueel-gemolesteerde kinders nie. The clinical framework and the physical and behavioural symptoms being some of the criteria utilised to identify such children. The methods used to treat cases co-determine the immediate and long-term effects of sexual abuse. Sexual abuse is discussed from an educational-psychological perspective. The study reflects on the child's meanin& experiencing, involvement, selfactualisation, self-concept and their effects on her life. Professional approaches, therapeutic techniques and treatments are described for use by professionals and agencies, based on an empirical investigation conducted in Witbank. The results of the research indicate a non-uniform approach in guidelines used for therapeutic techniques and the treatment of sexually-abused children. The research shows further that interaction between professional bodies and -individuals in treatment of these cases is poor and the benefits that should result from uniform treatment are therefore severely limited / Psychology of Education / M. Na afloop van die afsterwe van 'n ouer, vind daar fisieke, emosionele, sosiale en kognitiewe veranderinge in die kind se lewe plaas waarop hy nie voorbereid is nie, en wat hy ook nie verstaan nie. Hierdie veranderinge veroorsaak dat die kind se leefwereld in so mate verander dat sy funksionering benadeel word. Daar is met hierdie studie gepoog om vas te stel watter effek die dood van 'n ouer op die kind he! en tot watter mate terapeutiese begeleiding die kind se emosionele nood kan verlig. Daar is in die studie gevind dat rou inderdaad 'n gekompliseerde proses is wat lei tot die ontstaan van talle onbeantwoorde vrae, gevoelens van magteloosheid en emosionele pyn. Kinders wat in rousmart verkeer word dikwels deur die samelewing misgekyk en kry as gevolg hiervan nie voldoende hulp nie. Di! lei tot die ontstaan van persoonlikheidsontwikkelingsprobleme, relasionele probleme en intra-psigiese probleme soos 'n swak selfbeeld, ego-verdedigingsmeganismes, persoonlike kwesbaarheid, ongunstige betekenisgewing en ongunstige belewenisse. Hierdie probleme veroorsaak dat die kind in sy selfaktualisering gerem word en as gevolg hiervan nie sy gegewe moontlikhede verwerklik nie. After the death of a parent, physical, emotional, social and cognitive changes occur in the child's life for which he is not prepared and that he also does not understand. These factors cause such a change in the child's life-world that his functioning is hampered. In this study it was endeavoured to determine what effect the death of a parent has on a child's life and to what extent therapeutic guidance can alleviate the child's emotional trauma. In this study it was found that mourning is indeed a complicated process that gives rise to countless unanswered questions, feelings of helplessness and emotional pain. Children suffering grief in mourning are often ignored by society and as result of this, they do not receive adequate assistance. This leads to problems concerned with the development of the personality, relational problems and intrapsychological problems such as a poor self-concept, ego defence mechanisms, personal vulnerability, negative ways of acquiring meaning and unfavourable ways of experiencing life. These problems give rise to the hampering of the child's selfactualisation and because of this he cannot realise his potential. The study has furthermore proved that the child who has lost a parent through death, can be helped by a psychologist. This can be done by means of therapeutic guidance so that the child can handle the loss of a parent to such an extent that the factors that hamper his self-actualisation can be removed. The study provides guidelines for therapeutic guidance to the child who has Jost a parent through death. Guidelines are given on handling the child's immediate situation, the evaluation of the child in mourning, the determination of aims and objectives for therapeutic guidance, the selection of therapeutic techniques, therapeutic assistance to the child, assistance to the remaining parent and assistance to the educator who is involved with the child in mourning. Nerve Fiber Diameter Measurements Using Hematoxylin and Eosin Staining and Brightfield Microscopy to Assess the Novel Method of Characterizing Peripheral Nerve Fiber Distributions by Group Delay

Peripheral neuropathies are a set of common diseases that affect the peripheral nervous system, causing damage to vital connections between various parts of the body and the brain and spinal cord. Different clinical conditions are known to selectively impact various size nerve fibers, which often makes it difficult to diagnose which peripheral neuropathy a patient might have. The nerve conduction velocity diagnostic test provides clinically useful information in the diagnosis of some peripheral neuropathies. This method is advantageous because it tends to be minimally invasive yet it provides valuable diagnostic information. However, this test does not determine characteristics of peripheral nerve fiber size distributions, and therefore does not show any detailed information regarding the nerve fibers within the nerve trunk. Being able to determine which nerve fibers are contributing to the evoked potential within a nerve trunk could provide additional information to clinicians for the diagnosis of specific pathologies of the peripheral nervous system, such as chronic inflammatory demyelinating polyneuropathy or early diabetic peripheral neuropathy. In this study, three rat sciatic nerves are sectioned and stained with hematoxylin and eosin in order to measure the nerve fiber diameters within the nerve trunk. Stained samples are viewed using brightfield microscopy and images are analyzed using ImageJ. Histograms were created to show the frequency of various nerve fiber diameters. The nerve fiber diameters measured during this research are consistent with the range of previously published diameter values and will be used to support continuing research for a novel method to characterize peripheral nerve fiber size distributions using group delay.

Effects of Energy-Based Therapies on Postoperative Recovery: An Integrative Review of the Literature

Associated with health care reform, there has been a growing interest among healthcare professionals regarding use of energy-based therapies as a complementary therapeutic intervention. The purpose of this integrative literature review was to examine the impact of energy-based therapies (i.e. healing touch, reiki, therapeutic touch) on patients’ postoperative recovery. The methodology included identifying appropriate peer-reviewed, English-language research articles on the topic area that were published between 2006 to 2018. Following a search in select data bases, articles focusing on the topic were critiqued, analyzed and synthesized by the researcher. Consistent and inconsistent findings along with gaps in the literature are noted. In general, the research supported positive postoperative recovery outcomes when energy-based therapies were included along with standard postoperative nursing care. This theses highlights implication for nursing practice, education, and policy and identifies study limitations. Given that research is limited on this topic area, additional studies are needed to establish supporting evidence to ascertain the effects of energy-based therapies when used as a complementary postoperative intervention.

Power Mobility Sensor Data Collection Verified through Standardized Pediatric Assessments

The collaboration between the School of Engineering and the Department of Physical Therapy at the University of North Florida has introduced the possibility of creating a new environment for pediatric physical therapy assessments. There are currently no methods for remotely monitoring children with impairments. However, with embedded sensor technology in the form of power mobility and accepted therapy assessment tools, remote monitoring can become a possibility. As a part of this work, a ride-on toy car was developed as a remote monitoring device and a case study with a child with a mobility impairment was used as a proof of concept. In this thesis, the background information on the project, the case study diagnosis and history, and the model used to develop this project are detailed.

Label-free surface-enhanced Raman spectroscopy-linked immunosensor assay (SLISA) for environmental surveillance

The contamination of the environment, accidental or intentional, in particular with chemical toxins such as industrial chemicals and chemical warfare agents has increased public fear. There is a critical requirement for the continuous detection of toxins present at very low levels in the environment. Indeed, some ultra-sensitive analytical techniques already exist, for example chromatography and mass spectroscopy, which are approved by the US Environmental Protection Agency for the detection of toxins. However, these techniques are limited to the detection of known toxins. Cellular expression of genomic and proteomic biomarkers in response to toxins allows monitoring of known as well as unknown toxins using Polymerase Chain Reaction and Enzyme Linked Immunosensor Assays. However, these molecular assays allow only the endpoint (extracellular) detection and use labels such as fluorometric, colorimetric and radioactive, which increase chances of uncertainty in detection. Additionally, they are time, labor and cost intensive. These technical limitations are unfavorable towards the development of a biosensor technology for continuous detection of toxins. Federal agencies including the Departments of Homeland Security, Agriculture, Defense and others have urged the development of a detect-to-protect class of advanced biosensors, which enable environmental surveillance of toxins in resource-limited settings. In this study a Surface-Enhanced Raman Spectroscopy (SERS) immunosensor, aka a SERS-linked immunosensor assay (SLISA), has been developed. Colloidal silver nanoparticles (Ag NPs) were used to design a flexible SERS immunosensor. The SLISA proof-of-concept biosensor was validated by the measurement of a dose dependent expression of RAD54 and HSP70 proteins in response to H2O2 and UV. A prototype microchip, best suited for SERS acquisition, was fabricated using an on-chip SLISA to detect RAD54 expression in response to H2O2. A dose-response relationship between H2O2 and RAD54 is established and correlated with EPA databases, which are established for human health risk assessment in the events of chemical exposure. SLISA outperformed ELISA by allowing RISE (rapid, inexpensive, simple and effective) detection of proteins within 2 hours and 3 steps. It did not require any label and provided qualitative information on antigen-antibody binding. SLISA can easily be translated to a portable assay using a handheld Raman spectrometer and it can be used in resource-limited settings. Additionally, this is the first report to deliver Ag NPs using TATHA2, a fusogenic peptide with cell permeability and endosomal rupture release properties, for rapid and high levels of Ag NPs uptake into yeast without significant toxicity, prerequisites for the development of the first intracellular SERS immunosensor.

Practitioners' Use of Clinical Practice Guidelines: An Evidence-Based Approach

Pre-diabetes is a serious health problem in the United States. Distinguished by plasma glucose levels that are above the normal threshold, patients with pre-diabetes are 10 times more likely to develop type 2 diabetes. Patients with pre-diabetes suffer the same complications as patients with diabetes including diabetic retinopathy, nephropathy, and microalbuminuria. There is considerable evidence to support the idea that early identification and aggressive treatment of pre-diabetes has the potential to delay disease progression. The American Diabetes Association’s clinical practice guideline recommends management of with lifestyle modification and metformin for patients who are at risk for developing type 2 diabetes. The purpose of this project was to evaluate the implementation of the 2012 ADA clinical practice guidelines regarding the management of patients with pre-diabetes by the health care providers at a volunteer-run clinic located in a large metropolitan area in the southeastern United States. This study, even with a small sample size (n=26) revealed that the providers at the clinic had not implemented the 2012 ADA clinical practice guidelines. Clinical practice guidelines promote health care interventions that have proven benefits and improve the consistency of care provided to patients. The greatest benefits of implementing clinical practice guidelines for patients with pre-diabetes are early diagnosis and aggressive disease management. This would improve patient outcomes and in the long run, decrease the cost of medical care.
