Social-emotional and behavioral development problems in 1 to 2-year-old children in Northern Finland:reports of mothers, fathers and healthcare professionals

Abstract Background and aims: Growing evidence supports the existence of clinically significant problems in social-emotional/behavioral (SEB) development among infants and toddlers and the importance of early identification of these problems. There is a lack of research on the occurrence and identification of problems in the SEB domain among the Finnish general population of 1 to 2-year-old children. The present study examined these important issues. Given the dearth of earlier research knowledge, particular emphasis was focused on analyzing possible moderating effects of the assessed child’s and informant’s gender on the results. Subjects and methods: Oulu toddler (N = 208, age 18 months), Oulu infant (N = 227, age 12 months), and Oulu Province (N = 1008, age 12 months) samples were collected during 2008–2013 in collaboration with child health centers. Both mothers and fathers completed questionnaires assessing their child´s SEB development (e.g. the Brief Infant-Toddler Social and Emotional Assessment; Briggs-Gowan & Carter, 2006) and the family’s sociodemographic characteristics. Child healthcare nurse (CHCN) worry reports concerning the children’s development and family well-being were also gathered. Main results and conclusions: In parental ratings, girls obtained higher SEB competence scores than boys, whereas boys got higher SEB problem scores than girls, particularly among the toddler sample. Thus, boys may have an elevated risk of parent-reported problem behaviors and delays in SEB competences even before the age of 2 years. Compared to fathers, mothers were prone to rate the toddlers higher in both SEB competences and problems, especially with regard to externalizing behavior problems and problems in boys. Elevated interparental rating differences were associated with relatively elevated maternal parenting stress. Therefore, it is recommended to gather data on young children’s SEB development from both parents when possible and to focus clinical attention on the issue if the parental reports differ significantly. Parents and CHCNs seldom recognized SEB problems and delays in competency as problematical in 1-year-old infants, even in the case of infants who were screened to be in the of-concern range on the age-appropriate measure of SEB development. Thus, further efforts are needed for developing the identification of early problems in SEB development by both parents and preventive child healthcare professionals in Finland. / Tiivistelmä Tausta ja tavoitteet: Kasvava näyttö tukee käsitystä, että kliinisesti merkittäviä sosioemotionaalisen ja käyttäytymisen (SEK) kehityksen ongelmia esiintyy jo vauva- ja taaperoikäisillä ja että näiden ongelmien varhainen tunnistaminen on tärkeää. SEK-kehityksen ongelmien esiintymisestä ja tunnistamisesta 1–2-vuotaiden väestössä Suomessa puuttuu tutkimustietoa. Tämä tutkimus arvioi näitä tärkeitä aiheita. Tutkimuksessa huomioitiin erityisesti sekä tutkittavan lapsen että arvioitsijan sukupuolen mahdollinen moderoiva vaikutus tuloksiin, koska aiempaa tutkimustietoa tästä näkökulmasta on vähän. Tutkittavat ja menetelmät: Oulu taapero (N = 208, ikä 18 kk), Oulu vauva (N = 227, ikä 12 kk) ja Oulun lääni (N = 1008, ikä 12 kk) -aineistot kerättiin vuosina 2008–2013 yhteistyössä lastenneuvoloiden kanssa. Sekä äidit että isät täyttivät lomakkeet, jotka arvioivat lapsen SEK-kehitystä (mm. the Brief Infant-Toddler Social and Emotional Assessment; Briggs-Gowan & Carter, 2006) ja perheen sosiodemografisia taustatekijöitä. Myös terveydenhoitajan raportit lapsen kehitykseen ja perheen hyvinvointiin liittyvistä huolista kerättiin. Päätulokset ja johtopäätökset: Vanhempien arvioimina tytöt saivat korkeampia pistemääriä SEK-taidoissa kuin pojat, kun taas pojat saivat korkeampia pistemääriä SEK-ongelmissa kuin tytöt, etenkin taaperoaineistossa. Pojilla saattaakin olla kohonnut riski vanhempien raportoimille käytösongelmille ja SEK-kehityksen viiveille jopa ennen 2 vuoden ikää. Isiin verrattuina äidit olivat taipuvaisia antamaan korkeampia pisteitä sekä SEK-taito- että -ongelmaosioissa taaperoikäisten aineistossa, etenkin koskien ulospäin suuntautuvia käytösongelmia ja poikien ongelmia. Suurentuneet eroavaisuudet vanhempien antamissa arvioissa olivat yhteydessä suhteellisesti kohonneeseen vanhemmuuden stressiin äideillä. Onkin suositeltavaa kerätä tietoa pienen lapsen SEK-kehityksestä kummaltakin vanhemmalta, jos mahdollista, ja kiinnittää kliinistä huomiota asiaan, jos vanhempien antamat tiedot eriävät merkittävästi. Vanhemmat ja terveydenhoitajat tunnistivat SEK-ongelmia ja taitojen puutteita harvoin huolenaiheiksi 1-vuotiailla, vaikka nämä vauvat olisivat seuloutuneet huolialueelle ikätasoon soveltuvalla SEK-kehityksen mittarilla. Tarvitaankin lisätoimia, jotta vanhemmat ja terveydenhuollon ammattilaiset oppivat paremmin tunnistamaan varhaisia SEK-kehityksen ongelmia Suomessa.

Ontwikkeling van wiskundige konsepte by die kleuter in speelgroepe

Text in Afrikaans / Die doel van hierdie navorsing is, om te bepaal of gesyferdheid by kleuters binne `n speelgroepsituasie ontwikkel kan word deur middel van `n program wat wiskundige konsepte aan kleuters oordra. Ten einde hierdie doel te bereik is `n wiskunde program waarin verskillende hulpmiddels gebruik word, vir die kleuters aangebied. Die wiskunde program wat gebruik is, is saamgestel aan die hand van Charner, Murphy en Clark (2007) se “The Giant Encyclopedia of Math Activities for Children 3 to 6”. Die aktiwiteite wat tydens die voortoets en natoets gebruik is, is uit verskillende bronne op die internet verkry. Die aktiwiteite is uit gemelde bronne geneem en aangepas om by kleuters se ontwikkelingsvermoë te pas. Aan die einde van die wiskunde program kon daar `n verbetering in al die leerders se wiskunde vaardighede waargeneem word. Die resultate van die studie dui dus daarop dat kleuters se begrip en vaardigheid met wiskundige konsepte op `n vroeë ouderdom in `n speelgroep ontwikkel kan word. / The aim of this study was to determine whether numeracy can be developed among preschoolers in `n playgroup situation by means of a numeracy programme which introduces them to mathematical concepts. To achieve this aim a numeracy programme was used to introduce the mathematical concepts. The numeracy programme that was used is a programme compiled from “The Giant Encyclopedia of Math Activities for Children 3 to 6” by Charner, Murphy and Clark (2007). The activities which were used during the pre-test and post-test were taken from different sources on the internet. The activities taken from these different sources were adapted to the appropriate developmental phase of the preschoolers. At the end of the programme the post-test indicated an improvement in the mathematical competence of all the learners. The aim of this thesis is to describe the suitability of putting toddlers into day-care centers from a theoretical point of view and to assess the convenience of putting toddlers into day-care centers in the Czech Republic.

Although these discourses construct disability differently, three of them – the lay, charity and medical discourses – view disability very negatively. The lay discourse sees disability as something to be fearful of and therefore chooses to ignore it. According to the charity discourse people who are disabled should be pitied and helped because they are dependent and helpless. The medical discourse defines disability as abnormal – a “sickness” that requires treatment by medical experts. This study endeavors to challenge the stereotypical “picture” of a person with a disability. By conducting a narrative research design the life-world of a unique boy (who has a physical disability, called spina bifida) is captured in the telling of his life-story. A narrative research design was specifically chosen to portray the context and life-experiences (good and bad) of this little boy – a child in totality. The narrative life-story was written in the first person in order to invite the reader as a co-participant into the world of a boy with spina bifida. An asset-based approach accompanied the narrative research design in order to explore whether positive aspects of this boy’s life-world could be identified. Where disability is concerned, the asset-based approach proposes a shift from the traditional “deficit approach” towards a “strength-based approach”. AFRIKAANS: Gesinne wat met ’n kleuter met ’n gesiggestremdheid gekonfronteer word, het meervoudige behoeftes wat ’n holistiese benadering vereis ten einde hierdie komplekse probleem effektief aan te spreek. Geen navorsing is egter nog gedoen waar daar na die Suid-Afrikaanse ouer wat ’n kleuter met ’n gesiggestremdheid het, se behoeftes gekyk is ten einde ’n gespesialiseerde, empiries gefundeerde ouerbegeleidingsprogram te ontwikkel nie. Hierdie studie het dus ten doel gehad om ’n begeleidingsprogram vir ouers van ’n kleuter met gesiggestremdheid te ontwikkel en die effektiwiteit daarvan te evalueer. Intervensienavorsing as toegepaste navorsing is hiervoor aangewend.<p[> ’n Tweefase-navorsingsbenadering is gebruik. Tydens fase een is van ’n kwalitatiewe benadering gebruik gemaak. Die volgende navorsingsvraag is tydens hierdie fase geformuleer: Watter behoeftes het die Suid-Afrikaanse ouer van ’n kleuter met gesiggestremdheid? : ’n Fokusgroepbespreking waar fokusgroepvrae benut is, is met 10 ouerpare deurloop ten einde die betekenis en interpretasie wat hulle aan hulle leefwêreld heg, te eksploreer. Die maatskaplikewerk-intervensieprogram wat ontwikkel is, bestaan uit ses groepwerksessies van ongeveer 60 minute elk wat met twee groepe van 10 ouers in totaal deurloop is. ’n Selfontwerpte vraelys is voor en na afloop van die program deur al 10 ouers voltooi. Hierdie meetinstrument het bostaande hipotese bevestig. Vergelykings is getref tussen die literatuur en die empiriese gegewens. Gevolgtrekkings en aanbevelings vir toekomstige navorsing is na aanleiding van die studie geformuleer. ENGLISH: Families that are confronted with a toddler with a visual impairment have multiple needs that require a holistic approach in order to address this complex problem effectively. However, no research has been done yet that looks at the needs of the South African parent with a toddler with a visual impairment in order to develop a specialized, empirically grounded parental guiding programme. This study thus aimed at developing a guiding programme for parents with a toddler with a visual impairment and evaluating its effectiveness. Interventional research as applied research was utilised for this purpose. A two-phase research approach was used. During phase one a qualitative approach was used. The following research question was formulated during this phase: What are the needs of the South African parent with a toddler with a visual impairment? A focus-group discussion where focus-group questions were used was held with 10 parents in order to explore the meaning and interpretation that they attach to their daily world. These data, in addition to the wide-ranging literature study that had been undertaken, brought information to the fore that was used in the parental guiding programme. Answers to the research question could thus be found. The quantitative design that was followed during phase two was the single-system design. The following research hypothesis was formulated during this phase: If the parents of a toddler with a visual impairment follow the parental guiding programme, they will be empowered with knowledge with regard to their child’s specific eye condition, its influence on and their management of the child concerned, their marriage and their family. The social-work interventional programme that was developed consists of six group-work sessions of approximately 60 minutes each that were held for two groups of 10 parents in total. A self-designed questionnaire was completed by these 10 parents before and after the programme. This measuring instrument confirmed the above-mentioned hypothesis. Comparisons were made between literature and the empirical data. Voorbereiding van die pre-primêre kind op hospitalisasie: ‘n Spelterapeutiese benadering (Afrikaans)

A hospitalisation experience is serious for every toddler, and implies physical and emotional trauma of one kind or another. The reaction of the toddler in terms of this trauma in effect is a normal reaction to an unnatural situation, through which he tries to be in control of himself and his unknown environment. His lack of experience, incomplete cognitive reasoning, and lack of insight cause anxiety and stress for the toddler. Misbehaviour because of account of his anxiety and fear which the form part of the toddlers attempts to control and get hold of this unknown situation. The research is primarily aimed at how the therapist, parents and hospital personnel can prepare, assess and support the toddler, in terms of handling the hospitalisation process. Seeing that there is normally no one to blame for the toddlers admission to hospital, the child directs his anger and anxiety at himself, resulting in inner conflict and intense feelings of guilt. The research project aims to focus on preparation of the pre-primary toddler for hospitalisation, by using playtherapeutic intervention, in order to lessen the fear and anxiety of the experience. This goal was achieved by a literature study, an empirical study with the aid of application of a preparatory intervention program, as well as an assessment and an evaluation of the program, in order to make recommendations, regarding the research. Optimal care, and preparation of sick toddlers for admission in hospital, includes more than satisfying the child’s physical needs. It also asks for gratification of the toddler’s psychosocial needs. Support and preparation of both the child and his/her parents form an integral component of the sanitary care process. Therapeutic activities with the child that is going to hospital, can, according to the goals of the research study, be instrumental in reducing fear, promote self-esteem, become acquainted with the medical setting and procedures, as well as providing in the toddlers physical, emotional, psychological and psychosocial needs. A complete and refined play therapeutic preparation program was developed and evaluated through the findings of the empirical and literature study. The process of intervention research was followed in the empirical study, where data was obtained with the aid of a purposive sample, of 20 hospitalised toddler respondents. Creswell’s dominant-less-dominant model was utilised. A dominant quantitative approach was being followed, through witch data was assembled by a self developed structured observation instrument, as a post-test for both the experimental as well as the comparative group. The less dominant, but equally important qualitative approach, that consisted of 9 playtherapeutic sessions with experimental group, field notes and interviews with parents, enrich and verified the quantitative data. According to the integration of the quantitative and qualitative findings, the conclusion can be made that the preparatory playtherapeutic program had a positive impact on the way fear and the toddlers handled anxiety during hospitalisation, with less misbehaviour. The findings show that the developed preparatory playtherapeutic program can be implemented with great success and confidence, with toddlers that are going to be hospitalised. It is recommended that further research can be done on development and evaluation of preparation programmes for toddlers from less fortunate communities. Seeing that individual sessions may due to financial difficulties be less tenable, group sessions and community work can be recommended to reach larger groups of children, as well as creating community awareness for the needs of toddlers, related to hospitalisation. / Thesis (DPhil (Play Therapy))--University of Pretoria, 2005. / Social Work / unrestricted

Idag finns ett starkt forskningsstöd som visar att små barn utvecklar PTSD eller andra stressrelaterade symtom i samband med trauma i lika eller högre grad än äldre barn, ungdomar och vuxna. De yngsta barnens starka beroende av sina vårdnadshavare för överlevnad och skydd i kombination med deras utvecklingsmässiga omognad gör de minsta barnen till en särskilt utsatt och sårbar grupp. Syftet med denna litteraturöversikt är att undersöka forskningsstödet för olika psykoterapeutiska metoder för små barn (noll till fem år) med trauma. Översikten omfattar 29 artiklar med olika metodologiska tillvägagångssätt. Sammanfattningsvis visar litteraturöversikten att det saknas forskning om behandling av små barn samt att det finns studier som påtalar behov av att anpassa behandlingen till barnens ålder och behov. Detta inkluderar till exempel att involvera barnets primära omsorgsperson/er, att använda lek som metod, att ge åldersanpassad psykoedukation samt att identifiera känslor utifrån ålder och mognad. Framtida forskning om små barn har flera frågeställningar som behöver besvaras; Hur går en evidensbaserad diagnostisk process till för de små barnen? Vilka symtom behöver premieras i behandlingen? Vilka barn bör erbjudas behandling och hur kan effekten av terapin mätas? Ytterligare studier med fokus på genomförbarhet och enhetliga utfallsmått rekommenderas. / There is ample evidence to suggest that infants and small children can develop PTSD and other stress-related symptoms as a result of trauma, to an equal or greater degree than older children, adolescents and adults. Particularly, their strong reliance and dependence on their caregivers for protection and survival, in combination with their developmental immaturity, makes them an especially vulnerable group. This review aims to investigate the evidence base for varying psychotherapeutic methods specifically targeting small children zero to five years of age who have experienced trauma. A structured search was conducted using two databases; PsychInfo and Web of Science. The results includes 29 articles with diverse methodological approaches. In essence, the discussion highlights that there is sparse research concerning small children and the treatment of trauma. Studies that exist, put forward the need to conduct therapy with the small children and their caregivers, so called “dyadic therapy” regardless of psychotherapeutic orientation. Also, adapting therapy to suit the small children's cognitive, language and social development is imperative. This is done, for example, through the use of play, by providing age appropriate psychoeducation, and identifying emotions based on age and maturity. Still, there are many questions remaining that may need to be answered in order to provide evidence-based treatment to small children and their families, for example; How do we best conduct an evidence-based assessment of trauma in small children? Which trauma-symptoms should be treated and how should the treatment outcome be measured? Further studies are recommended.

A systematic review to assess the methodological quality of intervention research designed to increase physical activity in children with autism spectrum disorders

In comparison to normally developing children, many children with Autism Spectrum Disorders (ASD) do not possess the same opportunities to be physically active due to the impairments exhibited by their disorder. A systematic review using the Downs and Black checklist and the PEDro scale was conducted to assess the methodological quality of the literature on promoting physical activity in children with ASD. The following inclusion criteria had to be met: (1) subjects must include children with a clinical ASD diagnosis (2) the children have to be under the age of thirteen years old (3) the interventions must target physical activity; lastly, (4) they must be a relevant peer-reviewed English language study. The search was conducted using four electronic databases: MEDLINE, ERIC, PsycInfo, and CINHL with no restriction on the publication year. The following keywords were utilized: "Autism", "ASD/ Autism Spectrum Disorder", "Asperger", "Pervasive Developmental Disorder" Those terms were paired with "physical activity", "physical exercise", "exercise", "fitness", "aerobic", "swim", "aquatic", "jog", "walk", "recreational activity" Which were also paired with the terms "school age", "child", "toddler", "preadolescent". This multi-step search procedure occurred during February 2013. The methodological quality of six studies was evaluated in February 2013. Overall, the conclusive scores determined by the Downs and and Black checklist and the PEDro scale varied greatly. The scores reported by the Downs and Black checklist ranged from 19 to 21 on a 27-point scale. PEDro scale yielded scores ranging between two and six on a 10-point scale. A vote count revealed that the exercise interventions increased the physical fitness, aquatic skills, social behaviors, and sensory integration children with ASD. In summary, the variation within the scores and the quality of the studies leads to a demand for future research.; In order to adequately determine what exercise interventions effectively increase physical activity in children with ASD, future researchers should conduct randomized controlled trials in order to produce the highest quality of evidence.

The Development of a Three Minute Realtime Sampling Method to Measure Social Harmony during Interactions between Parents and their Toddlers with Autism

Training parents of a child with autism to increase the frequency of their child's social behavior may improve the quality of parent-child interactions. The purpose of this methodological study was to develop a direct observation method for rapidly sampling social harmony between parents and their toddlers with autism during parent training interactions. The current study used a pre and post probe design, with benchmark comparisons to test the discriminability of the measurement protocol across two sets of data. The first set of data came from pre and post training videos from a parent training program for children with a diagnosis of autism or at risk for a diagnosis. The second set of data came from videos of typically developing toddlers and their parents. The results of the study show that the measurement system differentiated in the level of harmonious engagement between the benchmark sample and the sample including children diagnosed with autism. The results are discussed in the context of future directions and the utility of the measurement system for behavior analytic practices in parent training and other settings where rapport and complex interactional behaviors are an intervention priority.
