Difusão do Método Ponseti para tratamento do pé torto no Brasil: o caminho para a adoção de uma tecnologia / Diffusion Ponseti Method for clubfoot treatment in Brazil: the path to the adoption of a technology.

Os temas pertinentes à introdução e à difusão do Método de Ponseti para o Tratamento do pé torto congênito no Brasil são discutidos. O Método Ponseti é baseado na confecção de gessos seriados de forma específica, tenotomia do Achillis, e manutenção da correção com órtese de abdução. Este tratamento era diferente do tratamento então vigente no país, baseado em maior tempo de confecção de gessos e realização de uma cirurgia extensa do pé. A difusão do Método Ponseti é analisada de forma cronológica, com ênfase na história de sua difusão no Brasil e no mundo a partir da Universidade de Iowa, na influência da Internet, e no Programa de treinamento de ortopedistas denominado Ponseti Brasil. Dados populacionais foram utilizados como avaliação da difusão do Método Ponseti antes do Programa de Treinamento. Quatro formas de avaliação direta do impacto do Programa de Treinamento são propostas: a reação dos participantes, o questionamento dos participantes sobre sua prática clínica após um ano do Programa, a avaliação dos participantes com relação ao tratamento do pé torto em duas cidades do Programa, São Luis e Teresina, e apresentações de casuísticas de nove serviços de ortopedia brasileira em um Painel Nacional no Curso Ponseti Avançado realizado dois anos após o Programa Ponseti Brasil. Para corroborar as informações sobre difusão, as publicações em países em desenvolvimento são discutidas, como ampliações da indicação do Método Ponseti. A tendência da literatura médica sobre pé torto congênito é analisada, por bases de dados em língua inglesa e bases de dados latino-americanas, e também são revisados os anais dos congressos nacionais da Sociedade Brasileira de Ortopedia e Traumatologia (CBOT Congresso Brasileiro de Ortopedia e Traumatologia) e Congresso Brasileiro de Ortopedia Pediátrica para observar a apresentação de trabalhos sobre tratamento pelo Método Ponseti e tratamento cirúrgico do pé torto. A difusão também é discutida em outros países, organizações não governamentais, com relação à visão dos pais e com relação aos custos do tratamento. A difusão do Método no Brasil é considerada na fase de expansão, e sua discussão pode trazer elementos que auxiliem a incorporação dessa tecnologia no Sistema Único de Saúde / Introduction and diifusion of Ponseti Method for congenital clubfoot treatment in Brazil are discussed. Ponseti Method is based on specific serial casting, Achillis tenotomy and use of abduction brace to maintain correction. This was different from standard treatment in Brazil at that time, based in many more months of serial casting and an extensive foot surgery. The Ponseti Method difusion is analized in a cronologic manner, with focus in the history of its difusion in Brazil and in the world, beginning in University of Iowa. Internet influence is discussed, as well as the Program Ponseti Brasil for training orthopaedic surgeons in Ponseti clubfoot treatment. Populational data were used to evaluate Ponseti Method difusion, before the Training Program. Four manners of direct evaluation of the impact of the Training Program are presented: the participants´reaction, enquiries about clinical practice with Ponseti Method after one year, evaluation of the impact of the Training Program in two cities, São Luis and Tesesina, and case series presentations of nine Brazilian Orthopaedic Clinics in a national panel in the Advanced Ponseti Course, two years after the Training ProgramPonseti Brasil. To reinforce information about difusion, publications in developing countries are discussed, extending indications for use of the Ponseti Method. Tendencies of medical literature about clubfoot treatment were analized, through written english and latinoamerican databases, and proceedings of two national meetings Brazilian Orthopaedic Meeting and Brazilian Pediatric Orthopaedic Meeting were revised to evaluate presentations about clubfoot treatment (Ponseti and surgical). Difusion is also discussed in other countries, non governamental organizations, related to parents perspective, and related to costs of treatment. Difusion of Ponseti Method is considered in expansion phase, and its discussion can help in the incorporation of this tecnology into the SUS (Sistema Único de Saúde Brazilian Health System)

Efeitos de um programa de treinamento físico sobre indicadores de aptidão física em duas crianças com síndrome de Prader-Willi

It is characterized by neonatal hypotonia, hyperphagia, obesity, hypogonadism, intellectual disability (ID), deficiency in GH, underactive and delayed motor development. Daily physical training programs (TP) showed an increase in lean body mass (LBM) and reduction of fat body mass (FBM) in PWS. However, it was not observed TP with overload. Studies have demonstrated the positive effect of short physical TP on LBM, the gait pattern and the postural strategy. However, no physical TP was built individually and provided a progressive overload throughout the monitoring period. The aim of this study is to evaluate the effects of PT on indicators of physical fitness in two children with PWS. This study is a descriptive follow-up study of 2 cases of children with PWS (CA with 11 years old and CB 14 with years old) who were evaluated, subjected to one TP lasting 12 weeks and were reassessed at the end of period. We used the following assessment tools for the evaluation of physical fitness indicators: anamnesis, assessment of the physical fitness level (PRODOWN); anthropometric complementary assessment; body composition assessment (DEXA) and physical level activity questionnaire (FLAQ). The PT was adapted for the specific population of this study (children with PWS) so that the participants had a progressive increase in training load during the PT execution. It was possible to verify the end of this study the positive effects of the physical training program for children with PWS. There was improvement in several indicators of physical fitness and weight reduction in CA and increased spontaneous physical activity in CB. Furthermore, after 12 weeks and 10 weeks of training, CA and CB, respectively, increased almost all test of assessment battery of PRODOWN. / A síndrome de Prader Willi (SPW) é uma doença genética parental decorrente da ausência da expressão gênica na região proximal do braço longo do cromossomo 15 (11.2q-13q). Suas características são hipotonia neonatal, hiperfagia, obesidade, hipogonadismo, deficiência intelectual (DI), deficiência de GH, hipoatividade e atraso no desenvolvimento motor. Programas de Treinamento (PT) físico diário demonstraram aumento da massa magra (MM) e redução da massa gorda (MG) na SPW. Porém, não foram observados PT com carga periodicamente aumentada. Estudos tem demonstrado o efeito PT físicos de curta duração sobre a MM, o padrão da marcha e sobre a estratégia postural. Porém, nenhum PT físico foi construído individualmente e forneceu uma sobrecarga progressiva ao longo do período de acompanhamento. O objetivo do presente estudo é avaliar os efeitos do PT sobre indicadores de aptidão física em duas crianças com SPW. Este trabalho trata de um estudo descritivo de seguimento de 2 casos de crianças com SPW (CA com 11 anos e CB com 14 anos) que foram avaliadas, submetidas a um PT de 12 semanas e foram reavaliadas no final do período. Foram utilizados os seguintes instrumentos de avaliação para avaliação de indicadores de aptidão física: anamnese; avaliação do nível de aptidão física (PRODOWN); avaliação antropométrica complementar; avaliação de composição corporal (DEXA) e questionário do nível de atividade física (QNAF). O PT foi adaptado para a população específica alvo desse trabalho (crianças com SPW) de modo que os participantes tivessem um aumento da carga de treinamento ao longo da execução do PT. Foi possível verificar ao final desse estudo efeitos positivos da introdução de um PT físico para crianças com SPW. Houve melhora em vários indicadores de aptidão física como redução de peso em CA e aumento da atividade física espontânea em CB, além disso, após 12 semanas e 10 semanas de treinamento, CA e CB, respectivamente, apresentaram melhor em quase todos os testes da bateria de avaliação PRODOWN.

Rôle modérateur de la forme de maltraitance subie par l’enfant sur l’efficacité d’un programme d’entrainement aux habiletés parentales / Moderating role of the form of maltreatment experienced by children on the effectiveness of a parent training program

La présente étude vise à vérifier si la forme de maltraitance subie par l’enfant modère les effets d’un programme d’entrainement aux habiletés parentales (PEHP) sur la probabilité de fermeture du dossier de l’enfant et la probabilité que l’enfant fasse l’objet d’un nouveau signalement fondé aux services de protection de la jeunesse (PJ). L’étude porte sur 736 enfants ayant eu un dossier actif aux services de PJ de Montréal entre 2007 et 2015. Le groupe expérimental est composé de tous les enfants dont le parent a participé au PEHP Incredible Years (n=368). Un groupe contrôle a été apparié au groupe expérimental sur la base d’un score de propension. Des régressions de Cox ont révélé qu’à partir du moment où les parents ont participé au PEHP, la probabilité de fermeture du dossier des enfants suivis pour des motifs de négligence augmente davantage que celle des enfants suivis pour des motifs de mauvais traitements psychologiques. La participation au PEHP n’a pas d’effet sur la récurrence de la maltraitance signalée, et ce, peu importe la forme de mauvais traitements subis. Les résultats démontrent que la participation des parents à un PEHP permet d’augmenter la probabilité de fermeture du dossier de leur enfant, et donc, de diminuer les durées de prise en charge en PJ. / Abstract: The study examines whether the form of maltreatment experienced by the child moderates the effects of a parent training program (PTP) on the probability that the child’s case will be closed and that the child will be the subject of a new founded report to child protective services (CPS). This study involved 736 children on whom the Montreal CPS agency had an active file between 2007 and 2015. The experimental group was composed of all children with a parent who participated in the PTP Incredible Years (n=368). A control group was matched with the experimental group based on a propensity score. Cox regression revealed that once parents have participated in the PTP, the probability that their children’s cases will be closed increases more for children being followed because of neglect than for those being followed because of emotional maltreatment. Participating in the PTP has no effect on the recurrence of reported maltreatment, regardless of what form of maltreatment the child has been subjected to. Results show that a parent’s participating in a PTP increases the probability that his or her child’s CPS case will be closed and hence reduces the length of time that the child must receive protective services.

Sales assistants serving customers with traumatic brain injury

General lack of awareness regarding neurogenic communication disorders generally, and cognitive-communicative disorders following a traumatic brain injury (TBI) specifically has resulted in pervasive environmental and attitudinal barriers for these individuals. Paradigm shifts within the rehabilitation context have been highlighted which aim to remove barriers, provide social supports, and thereby enhance their participation in all aspects of life. While collaborative communication partner training programs have been advocated as a means to achieve this, a dearth of published programs is evident within the field of TBI, leading to the need for such programs to enhance the awareness and skill of the communication partner, and reduce barriers for the individual. Transformations are likewise apparent in the corporate context, where in spite of legislative changes encouraging diversity awareness programs for employees, few training programs exist worldwide, and in South Africa particularly, which remove barriers between employees and customers with a communication disability, and a TBI specifically. The current research targeted the retail supermarket environment as a context in which a significant number of everyday communicative exchanges take place. The study investigated the ability of a group of sales assistants to identify barriers to, and facilitators of interaction involving customers with a cognitive-communication disorder, using a control group design. This was achieved by the development and administration of 2 questionnaires on 2 different occasions to determine the confidence and skill with which they identified barriers and facilitators during videotaped sales interactions. A once-off training session was developed and conducted with the experimental group participants, in order to increase their confidence and skill in identifying barriers and facilitators of such interactions. The training session employed a number of customized components considered to be powerful contributors to the positive outcome of the study. These included: extensive use of customized video material professionally produced and comprising real interactions involving individuals with a TBI as “customers” in various stores of the participating national supermarket chain during operating hours. Collaboration with an individual with a TBI in the training, together with use of adult learning and diversity awareness principles were considered effective in shifting previous attitudes and fostering new learning. Inter-and-intra-group results on the confidence and skill constructs of the pre-and-post questionnaires were examined. All pointed consistently to the impact of the training session on the improvement demonstrated in the experimental group as compared to the control group on the post-questionnaires as compared with the pre-questionnaires. In addition all subjective training session evaluations by the experimental group participants were consistently highly rated, reflecting the active participation observed during training. The need for companies to expand their concept of customer service to include an acknowledgement of the customer with a disability is emphasized. Training programs empowering their employees to interact with greater awareness and confidence with customers with a TBI specifically will potentially facilitate deeper participation for both. Como ponto de partida, a premissa foi de que a formação de gestores provoca mudanças na gestão escolar, contribuindo para a melhoria da qualidade da educação pública. Na metodologia foram utilizadas de forma associada as abordagens qualitativa e quantitativa, com a realização de entrevistas semiestruturadas com um Gestor do MEC, uma Gestora do Grupo de Trabalho Interinstitucional do Programa, uma Gestora da UFV e cinco diretoras de escolas. Além das entrevistas foi realizado um survey online com 102 egressos. As análises dos dados foram viabilizadas pelo método de análise de conteúdo e por procedimentos estatísticos. Para justificar a pesquisa, delineou-se um estado da arte sobre avaliação de políticas educacionais a partir de dados do Banco de Teses/Resumos da Capes e dos anais da ANPEd (GT 5 Estado e Políticas Educacionais). O estudo teórico envolveu a caracterização do PNEGEB e a abordagem dos temas gestão escolar democrática, formação de gestores escolares, bem como a análise e avaliação de políticas educacionais, sendo apresentada a Abordagem do Ciclo de Políticas como o modelo escolhido para analisar o curso. A partir das análises empreendidas, é possível confirmar a hipótese de que os resultados e efeitos positivos do Curso são expressos num processo formativo de qualidade e que pode incidir sobre o desenvolvimento de uma gestão escolar democrática, segundo seus egressos. Contudo, no que tange às perspectivas, paira a insegurança quanto à continuidade do Programa, por ser uma iniciativa de governo. / This thesis presents an evaluation of results and effects of the specialization course in School Management of the Programa Nacional Escola de Gestores da EducaçãoBásica (PNEGEB) (Manager's School National Program of Basic Education), considering three dimensions, i.e., someone who plans (Ministry of Education), someone who implements [Universidade Federal de Viçosa (UFV) Federal University of Viçosa], and the beneficiaries (graduates). As a starting point, the premise is that management training causes changes in school management, contributing for improving the quality of public education. Associated qualitative and quantitative approaches were used as methodology, with the completion of semi-structured interviews with a Manager of Ministério da Educação e Cultura do Brasil (MEC) (Ministry of Education and Culture of Brazil), a Manager of the Inter-Institutional Working Group of the program, a Manager of UFV, and five Principals of schools. Besides the interviews, an online survey was performed with 102 graduates. The data analyzes were made possible by the content analysis method and statistical procedures. To justify the search, a status of the art about evaluation of educational policies was outlined up based on data from the Bank of Thesis/Abstract of Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) (Higher Education Personnel Improvement Coordination) and annals of Associação Nacional de Pós-Graduação e Pesquisa em Educação (ANPEd) (National Association of Graduate Studies and Research in Education ) (Grupo de Trabalho (GT) 5 - Estado e Políticas Educacionais (Work group 5 - State and Educational Policies). The theoretical study has involved the characterization of PNEGEB and the approach of democratic school management issues, training of school managers, as well as analysis and evaluation of educational policies; and the Policy Cycle Approach as the chosen model was presented to analyze the course. On the basis of the undertaken analysis, it is possible to confirm the hypothesis that the positive results and effects of the course are expressed in a formative quality process and can focus on the development of a democratic school management, according to professionals proceeding from this course. However, with respect to the prospects, the insecurity remains as to the continuity of the program, because this is a government initiative.

L'internationalisation du management en Inde par la formation : les effets du "PGPMX" sur des managers indiens de grandes entreprises industrielles publiques / Internationalization of Management in India by Advanced Training : the Effects of the PGPMX on Indian Managers in Large Public Companies

Dans le contexte des effets de la mondialisation sur une économie émergente, le travail de recherche présenté ici s’attache à démontrer le processus d’ouverture internationale du manager industriel indien. Face aux mutations auxquelles sont confrontées les entreprises de la dixième économie mondiale, il est à la fois l’acteur de l’internationalisation et l’objet des transformations induites par celle-ci. Une recherche empirique et qualitative menée dans des entreprises industrielles indiennes décrit et modélise cette ouverture internationale pour la catégorie des managers intermédiaires, par le biais d’un programme de formation supérieure en gestion international indien, qui sert de cas d’étude : le Post Graduate Diploma in Management for Exécutive – PGDMX- dont le modèle réfèrent est américain. S’appuyant sur une analyse réflexive du discours de managers indiens diplômés PGPMX, ce travail s’attache à s’interroger sur l’homogénéité des pratiques et des théories managériales vis-à-vis de la spécificité de la culture du management à l’indienne. La mobilisation des concepts de la théorie de la pratique de Pierre Bourdieu, conduit à porter son attention sur l’existence d’un habitus international, et d’un gain en capital symbolique, culturel et social pour le manager diplômé du PGPMX. Les résultats montrent aussi une tension entre les nouveaux principes managériaux et les valeurs ancrées dans la culture indienne, en même temps qu’un processus « d’indigénisation sociale » du diplôme. Ce travail s’adresse, certes, aux entreprises publiques indiennes et en particulier aux services RH sur la gestion des talents, mais aussi aux Ecoles de management en Inde et en Occident; ainsi qu'aux entrepreneurs étrangers qui souhaitent développer des activités en Inde, en mettant en évidence la force de la violence symbolique, liée à toute intrusion culturelle, et source de résistance de par la nature autonome de la tradition indienne, à la fois industrielle, culturelle, et humaine. / In the context of the effects of globalization on an emerging economy, the research presented here is aimed at demonstrating the process of internationalization of middle managers in Indian industrial enterprises, via advanced management training. These very large public enterprises are both, key actors engaged in deploying globalization effects, and the objects of those transformations. Based on empirical and qualitative study, using semi-structured interviews carried out in these large Indian industrial enterprises, what follows is a report describing and modeling the internationalization of a panel of former participants of a Post Graduated Diploma in Management for Executives (PGPMX), of American MBA inspiration. The originality of this research is to offer a reflexive report of the nature of this training, as the participants experienced it, and the consequences for them professionally and personally. This allows the investigation of the fit of managerial practices and theory as offered by business schools, using the US model vis-à-vis the managerial demands in Indian enterprises, wrapped in the specificities of the Indian culture. As an interpretative strategy, the data gathered from the extensive field work was treated in reference to the project of Pierre Bourdieu—with particular attention to the figure of habitus, and special focus on modifications of capital’—symbolic, cultural and social; Result show a gain in all three, and reveal a residual tension between the new management principles, and the traditional values. This research, should be of value for the Human Resource Development function of Indian Industrial Enterprises, and for the schools of management—both Indian and Western, that offer these training programs. It should also interest foreign enterprises that seek entry into the Indian market, by enabling a better comprehension of the ‘symbolic violence’, associated with the intrusion of foreign cultural practices and values.

Solicitud de embargo por publicación en revista indexada. / El objetivo del presente estudio fue determinar los efectos de un programa de entrenamiento de habilidades psicológicas en un equipo de futbol femenino de Lima Metropolitana. La muestra estuvo conformada por 9 futbolistas mujeres con edades comprendidas entre los 15 a 37 años. Se administró el Inventario Psicológico de Ejecución Deportiva (IPED), en la versión argentina de Raimundi et al. (2016), en dos condiciones de tiempo (antes y después) y el programa de intervención fue creado tomando como referencia el programa realizado por Reyes et al. (2012). Los resultados evidenciaron que el programa tuvo un efecto grande en las dimensiones de visualización; mientras que, el control de afrontamiento negativo tuvo un efecto moderado; por otro lado, la autoconfianza, atención, afrontamiento positivo y actitudinal tuvieron un efecto pequeño y la motivación no tuvo efecto. Se concluye que, el programa tiene un efecto positivo en la mejora del rendimiento deportivo, a través del entrenamiento de habilidades psicológicas. Con esto, se pretende que las organizaciones responsables del desarrollo deportivo en el país impulsen el desarrollo e implementación de programas de psicología deportiva. / The objective of the present study was to determine the effects of a sports psychological skills training program in a female soccer team from Lima City. The sample consisted of 9 female soccer players aged between 15 and 37 years. The Sports Execution Psychological Inventory (IPED) was administered, in the Argentine version of Raimundi et al. (2016), in two time conditions (pre and post) and the intervention program was created taking as a reference the program carried out by Reyes et al. (2012). The results showed that the program had a large effect on the dimensions of visual imagination; while, the negative coping control had a moderate effect; on the other hand, self-confidence, attention, positive and attitudinal coping had a small effect; finally the motivation had no effect. It is concluded that the program has a positive effect on the improvement of sports performance, through the training of psychological skills. With this, it is intended that the organizations responsible for sports development in the country promote the development and implementation of sports psychological programs. / Tesis

L’agression sexuelle (AS) envers les enfants est une problématique majeure affectant les communautés autochtones. Considérant les conséquences à long terme associées à l’AS, il est primordial que les intervenants soient formés adéquatement pour intervenir en cas d’AS puisque que cette problématique compromet le bien-être des enfants, de leur famille et de leur communauté. Le Centre d’expertise Marie-Vincent, en partenariat avec les communautés autochtones, a mis sur pied une formation en matière d’AS spécifiquement adaptée aux besoins des intervenants. De 2013 à 2015, 161 professionnels œuvrant dans les communautés autochtones ont participé à cette formation. Deux phases de collecte de données distinctes ont été menées pour vérifier l’atteinte des objectifs du programme, soit l’amélioration des connaissances et des interventions en matière d’AS des participants. Lors de la première phase, les participants ont répondu à un questionnaire de réactions et à un test de connaissances. Une étude de cas multiples qualitative comportant quatre communautés a été réalisée lors de la deuxième phase. En plus d’un taux d’appréciation élevé, les résultats révèlent une augmentation significative des connaissances, passant de 54 % à 87 % (t(125) = 16,4, p < 0,001, ηp2 = 0,68). L’analyse des entrevues fait état d’une appropriation des contenus de formation dans la pratique professionnelle des participants. Ces derniers affirment avoir modifié la façon dont ils questionnent les enfants et être plus attentifs aux signes et symptômes d’une AS. Des changements sur le plan des attitudes, notamment sur l’importance du signalement, sont également rapportés. Finalement, la formation est considérée comme l’un des éléments déclencheurs d’une meilleure collaboration intersectorielle dans au moins une communauté. / Child sexual abuse (CSA) is a major issue affecting the well-being of indigenous communities. For this reason, the Marie-Vincent Centre of Expertise, in partnership with indigenous stakeholders, has created a training program on the topic of CSA specifically tailored to the needs of Québec’s indigenous communities. From 2013 to 2015, 161 professionals took the training. In order to evaluate this training, two distinct data collection phases were conducted. As part of the first data collection phase, participants completed a reaction questionnaire and a knowledge assessment questionnaire. Interviews with four subgroups of participants were conducted in the second phase of collection. The overall satisfaction with the training was high and the participants’ knowledge level significantly increased from 54% to 87% (t(125) = 16.4, p < 0.001, ηp2= 0.68). Furthermore, interview results show an integration of training contents in the participants’ professional practice. Participants mentioned that the training influenced the way they interrogate children. Being more aware of potential signs and symptoms of CSA was credited to the training. Changes in attitudes were also found, particularly in the importance of reporting abuses. Finally, an improved inter-sectoral collaboration was attributed to the program in at least one community.
