Observou-se a confiabilidade dos dados obtidos na cefalometria, vez que esta vem sendo utilizada largamente no protocolo de exame de pacientes apnêicos, principalmente para a determinação dos sítios obstrutivos, servindo desta forma de guia terapêutico. Os resultados demonstraram não haver diferença estatisticamente significante no que diz respeito à capacidade diagnóstica da cefalometria radiográfica e do exame polissonográfico. A cefalometria radiográfica mostrou-se capaz de diagnosticar, satisfatoriamente, indivíduos portadores de SAOS (sensibilidade ? 91,5%). Por outro lado, a cefalometria radiográfica não se mostrou capaz em identificar, de forma satisfatória, indivíduos não portadores de SAOS (especificidade ? 38,5%). Por fim, como a cefalometria radiográfica apresentou alta sensibilidade na determinação dos sítios obstrutivos, concluímos que tal exame deve compor um protocolo, em associação ao exame polissonográfico, de procedimento diagnóstico para indivíduos portadores de SAOS. / Salvador

Utilizamos estudo cego para não influenciar o examinador. Método: Foi realizado um estudo prospectivo cego com avaliação sistemática da via aérea superior com graduação de palato Web de 146 pacientes que realizaram polissonografia basal. Como proposta de graduação para o tamanho do palato Web classificamos conforme o local de inserção da prega palatofaríngea, ou pilar posterior amigdaliano, na úvula usando os seguintes critérios: - Web Grau I: Inserção da prega palatofaríngea no terço proximal,ou seja, superior da úvula. - Web Grau II: Inserção da prega palatofaríngea no terço médio da úvula. - Web Grau III: Inserção da prega palatofaríngea no terço distal, ou seja, inferior da úvula. Resultados: Nos pacientes com maior gravidade de SAOS os achados mais freqüentes foram: IMC maior que 31, circunferência cervical maior que 41cm, palato duro ogival, palato posterior, palato espesso, úvula espessa, hipertrofia amigdaliana grau III ou IV, língua demarcada pelos dentes, diminuição da fase de sono 3/4. Outros achados não apresentaram relação com a SAOS de maior gravidade, como: retroposicionamento mandibular, Mallampati Modificado III e IV, pregas palatofaríngeas medianizadas, oclusão dentária classe II, desvio septal nasal, hipertrofia de cornetos. Não houve relação do tamanho do palato Web. Conclusões: Alterações anatômicas podem ser identificadas, pelo exame físico sistemático, com maior freqüência conforme a gravidade da SAOS. A 11 graduação do tamanho do palato Web não auxiliou na identificação de indivíduos com maior gravidade da SAOS. / Objective: The objective of this study is to apply the systematic evaluation of physical examination in patients with Obstructive Sleep Apnea Syndrome (OSAS), including the measure of the cervical circumference and to submit a classification for graduation of the web palate. We have applied a blind study to not influence the examiner. Method: A prospective study with systematic evaluation of the upper airway and proposal of the web palate graduation was performed in 146 patients that were submitted to polysomnography. At the moment of the physical examination and interview, the researcher did not know reason of the polysomnography, thus issuing a blind study for the examiner. As proposal of graduation for the size of the web palate we classify as the place of the palatopharyngeal arch, thus posterior tonsil pillar insertion, in the uvula using the following criteria: Web Degree I: Insertion of the palatopharyngeal arch in the proximal third of the uvula, thus uvula upper position. Web Degree II: Insertion of the palatopharyngeal arch in median third of uvula. Web Degree III: Insertion of the palatopharyngeal arch in distal third, thus, inferior of the uvula. Results: The following findings had been more frequent in the groups of major severity: higharched hard palate, posterior and palate, long and thick uvula, tonsils hypertrophy degree III or IV, tongue edge crenation, IMC greater than 31, reduction of the phase of sleep 3/4, cervical circumference greater than 41cm. Other findings didn´t have relation with the severity of OSAS, as: mandible retropositioning, Mallampati III and IV, class II occlusion, septal deviation and turbinate hipertrophy. There was no relation the web palate size to OSAS severity. Conclusions: Anatomical alterations can be identified by systematic physical examination with greater 49 frequency in accordance with OSAS severity. The web palate size graduation did not support to identify patients with severe OSAS.

Dieta hipocalórica e treinamento físico em pacientes com síndrome metabólica e apnéia do sono / Hypocaloric diet and exercise training in patients with metabolic syndrome and sleep apnea

INTRODUÇÃO. Estudos anteriores demonstraram que pacientes com síndrome metabólica (SMet) e apnéia obstrutiva do sono (SAOS) apresentam hiperativação simpática e hipersensibilidade quimiorreflexa. Neste trabalho, testamos as hipóteses de que: 1) dieta hipocalórica associada à treinamento físico (D+TF) melhora a sensibilidade quimiorreflexa em pacientes com SMet e 2) Os efeitos da D+TF seriam mais pronunciados em pacientes com SMet+AOS que em pacientes sem AOS (SMet-AOS). MÉTODOS. Vinte e sete pacientes nunca tratados da SMet (ATP-III) foram alocados em: 1) SMet+AOS (n = 15, 53±2 anos) e 2) SMet-OSA (n = 12, 43±2 anos). A AOS foi caracterizada por um índice de apnéia-hipopnéia (IAH)> 15 eventos / hora (polissonografia). Atividade nervosa simpática muscular (ANSM) foi avaliada pela técnica de microneurografia e pressão arterial (PA) pelo método oscilatório. A sensibilidade quimiorreflexa periférica foi avaliada através da inalação de uma mistura gasosa contendo 10% O2 e 90% N2 com titulação de CO2; e a sensibilidade quimiorreflexa central através da inalação de 7% CO2 e 93% O2 por 3 min. A dieta hipocalórica foi de -500 kcal da taxa metabólica de repouso e o treinamento físico se estendeu por 4 meses, 3 vezes/ semana. RESULTADOS. A associação da D+TF reduziu semelhantemente peso corporal (5,5±0,7 e 6,2±0,6 kg, P = 0,44), circunferência abdominal (CA, 5,6±1,2 e 5,4±1,0 cm, P = 0,91), PA sistólica (10,9±3,2 vs 13,3±3,5 mmHg, P = 0,62) e diastólica (8,5±1,6 vs 8,3±1,4 mmHg, P = 0,95) e, similarmente, o aumento do consumo de oxigênio de pico (20±5,9 e 16±7,3%, P = 0,69) em pacientes com SMet+AOS e SMet-AOS. A D+TF, reduziu significativamente o IAH (38±6,2 vs 18±3,9 eventos / hora, P = 0,01) e aumentou a saturação mínima de O2 (81±2,3 vs 84±1,9 %, P = 0,01) em pacientes com SMet+AOS. Durante a estimulação hipóxica, D+TF reduziu significativamente os níveis de ANSM tanto em pacientes com SMet+AOS (41±1,9 versus 33±2,0 impulsos/ min, P = 0,02) como em SMet-AOS (36±3,2 versus 28±1,7 impulsos/ min, P = 0,05). Durante a estimulação hipercápnica, D+TF reduziu significativamente os níveis de ANSM em pacientes com SMet+AOS (39±2,0 versus 30±1,1 impulsos/ min, P = 0,0005), mas não em pacientes com SMet- AOS. CONCLUSÕES. Tratamento não-farmacológico como D+TF melhora o controle quimiorreflexo periférico da ANSM em pacientes com síndrome metabólica. Esta mudança autonômica é mais pronunciada em pacientes com SMet+AOS, nos quais D+TF melhora tanto o controle quimiorreflexo periférico como o central. Além disso, D+TF melhora o distúrbio do sono em pacientes com SMet+AOS. Sendo assim, estes resultados sugerem que D+TF pode reduzir o risco cardiovascular em pacientes com SMet e AOS / INTRODUCTION. Previous studies have shown that patients with metabolic syndrome (MetS) and obstructive sleep apnea (OSA) have sympathetic hyperactivation and chemoreflex hypersensitivity. We tested the hypothesis that: 1) Hypocaloric diet associated with exercise training (D+ET) would improve chemoreflex sensitivity in patients with MetS and 2) The effects of D+ET would be more pronounced in patients with MetS+OSA than in patients without OSA (MetS-OSA). METHODS. Twenty three never treated MetS patients (ATP-III) were allocated into: 1) MetS+OSA (n=15, 53±2 yrs); and 2) MetS-OSA (n=12, 43±2 yrs). OSA was characterized by an apnea-hypopnea index (AHI) >15 events/hour (polysomnography). Muscle sympathetic nerve activity (MSNA) was evaluated by microneurography technique and blood pressure (BP) by oscillatory method. Peripheral chemoreflex sensitivity was evaluated by inhalation of 10%O2 and 90%N2 with CO2 titrated, and central chemoreflex by 7%CO2 and 93%O2 for 3 min. The hypocaloric diet was set at -500 kcal of the resting metabolic rate and exercise training extended over 4 months, 3 times/ week. RESULTS. D+ET similarly reduced body weight (5.5±0.7 and 6.2±0.6kg, P=0.44), waist circumference (WC, 5.6±1.2 and 5.4±1.0 cm, P=0.91), systolic BP (10.9±3.2 vs. 13.3±3.5 mmHg, P=0.62) and diastolic BP (8.5±1.6 vs. 8.3±1.4 mmHg, P=0.95), and similarly increased peak oxygen consumption (20±5.9 and 16±7.3%, P=0.69) in MetS+OSA and MetS-OSA patients. D+ET significantly reduced AHI (38±6.2 vs. 18±3.9 events/hour, P=0.01) and minimal O2 saturation (81±2.3 vs. 84±1.9%, P=0.01) in MetS+OSA patients. D+ET significantly reduced MSNA levels during hypoxia in MetS+OSA (41±1.9 vs. 33±2.0 bursts/min, P=0.02) and MetS-OSA (36±3.2 vs. 28±1.7 bursts/min, P=0.05) patients. D+ET significantly reduced MSNA levels during hypercapnia in MetS+OSA patients (39±2.0 vs. 30±1.1 bursts/min, P= 0.0005), but not in MetS-OSA patients. CONCLUSIONS. Non-pharmacological treatment based on D+ET improves peripheral chemoreflex control of MSNA in patients with MetS. This autonomic change is more pronounced in patients with MetS+OSA, in whom D+ET improves both peripheral and central chemoreflex controls. In addition, D+ET improves sleep disorder in patients with MetS+OSA. Altogether, these findings suggest that D+ET reduce cardiovascular risk in patients with MetS+OSA

Objetivo: O objetivo deste estudo foi avaliar o efeito do tratamento com o aparelho intra-oral em parâmetros de estresse oxidativo em pacientes com Síndrome da Apnéia Obstrutiva do Sono moderada a grave. Métodos: Pacientes com SAOS moderada a grave, ambos os sexos, idade entre 25 e 65 anos, IMC menor que 35 Kg/m2, com boas condições dentárias, participaram deste estudo randomizado, cruzado, duplo-cego com um mês em cada tratamento com Continuous Positive Airway Pressure (CPAP), um aparelho intra-oral (AIO) e um aparelho placebo. Avaliamos a sonolência excessiva diurna subjetiva pela Escala de Sonolência de Epworth (ESE), parâmetros polissonográficos e parâmetros de estresse oxidativo (Thiobarbituric Acid Reactive Substances, Superóxido Dismutase, Catalase, Vitaminas C, E, B6, B12 e Folato, Homocisteína, Ácido Úrico). Resultados: 29 pacientes (24 homens e 5 mulheres) participaram deste estudo, com idade média de 47,3 ± 8,9 anos, IMC de 28,4 ± 3,6 Kg/m2 e escores na Escala de Sonolência de Epworth de 11,4 ± 5,3. Tanto o aparelho intra-oral quanto o CPAP levaram a uma redução no Índice de Apnéias e Hipopnéias, porém o efeito com o CPAP foi melhor (basal: 42,3 ± 4,5; placebo: 48,7 ± 5,6; AIO: 26,7 ± 4,8; CPAP: 3,2 ± 0,4; p < 0,05). O uso do CPAP também levou a uma diminuição no escore da ESE (basal: 11,3 ± 1,0 x CPAP: 9,7 ± 0,8; p < 0,05), embora a adesão ao AIO tenha sido maior que ao CPAP (86,3 ± 3,0 x 72,0 ± 4,2%; p < 0,05). Quanto aos parâmetros de estresse oxidativo, observamos uma queda dos níveis de atividade da enzima catalase em relação aos valores basais com o AIO (basal: 115,8 ± 5,2; placebo: 102,7 ± 4,4; AIO: 95,3 ± 4,6; CPAP: 107,1 ± 3,8 U/mg Hb; p < 0,01). Além disso, houve um aumento dos valores de vitamina C nas fases placebo e CPAP quando comparados ao basal (basal: 54,9 ± 2,5; placebo: 65,9 ± 3,0; AIO: 62,3 ± 2,6; CPAP: 64,3 ± 2,5 umol/L; p < 0,05) e dos valores de vitamina B6 em todas as fases de tratamento comparados aos valores iniciais (basal: 18,9 ± 1,2; placebo: 22,7 ± 1,5; AIO: 22,2 ± 1,3; CPAP: 24,6 ± 0,9 nmol/L; p<0,05). Conclusão: Podemos concluir que, embora o CPAP tenha sido mais eficaz na melhora clínica e nos parâmetros polissonográficos de pacientes com SAOS moderada a grave, a adesão ao tratamento AIO neste grupo de pacientes foi mais eficiente e favoreceu a redução em uma das enzimas que participam da remoção de espécies reativas de oxigênio. / Introduction: Obstructive Sleep Apnea Syndrome (OSAS) is associated with increased cardiovascular morbidity and mortality. The exact nature of the underlying pathophysiology of cardiovascular morbidity in OSAS patients has not been fully elucidated. One of the underlying mechanisms is the formation of Reactive Oxygen Species (ROS), increasing oxidative stress (OS) due to the intermittent hypoxia. Studies with OSAS patients highlighted an increase in markers of OS associated to vascular risk. Authors have demonstrated a decrease of OS after Continuous Positive Airway Pressure (CPAP). OS reduction by means of the Oral Appliance (OA) is yet to be comprehended. Objective: The purpose of this study was to evaluate the effect of an OA compared with CPAP regarding OS parameters in moderate to severe OSAS patients with a randomized, double blind and placebo controlled trial. Methods: Subjects were patients with moderate to severe OSAS, of both genders, BMI under 35, 25-65 years of age, in good dental condition. All patients were submitted to 3 treatments: mandibular repositioning appliance (BRD) in maximal comfortable protrusion, placebo appliance (PA) and CPAP with one month of each treatment and one week washout between them. It was a randomized, cross-over, double blind and placebo controlled trial. Epworth Sleepiness Scale (ESS), polysomnographic and OS parameters (Thiobarbituric Acid Reactive Substances -TBARS, Superoxide Dismutase, Catalase, Vitamins C, E, B6, B12 and Folate, Homocysteine, and Uric Acid) were evaluated at each phase (baseline and after one month of each treatment). Results: 29 patients (24 men and 5 women) took part of the study, with a mean age of 47.3 ± 8.9 years, BMI of 28.4 ± 3.6 Kg/m and scoring on ESS of 11.4 ± 5.3. Both OA and CPAP treatments led to a decrease on the Apnea and Hypopnea Index (AHI). The last, however, demonstrated better efficacy (baseline: 42.3 ± 4.5; PA: 48.7 ± 6.5; OA: 26.7 ± 4.8; CPAP: 3.2 ± 0.4; p < 0.05), besides improving ESS (baseline: 11.3 ± 1.0 versus CPAP: 9.7±0.8; p < 0.05). Notwithstanding, compliance rate during OA period was higher than during CPAP (86.3 ± 3.0% versus 72.0 ± 4.2%; p < 0.05). For the OS parameters, a significant drop in the levels of catalase activity was observed after OA compared to the baseline period (baseline: 115.8 ± 5.2; PA: 102.7 ± 4.4; OA: 95.3 ± 4.6; CPAP: 107.1 ± 3.8 U/mg Hb; p<0.01). Moreover, there was an increase in vitamin C levels after PA and CPAP (baseline: 54.9 ± 2.5; PA: 65.9 ± 3.0; OA: 62.3 ± 2.6; CPAP: 64.3 ± 2.5 umol/L; p<0.05), and in vitamin B6 after all treatment periods compared to initial values in baseline period (baseline: 18.9 ± 1.2; PA: 22.7 ± 1.5; OA: 22.2 ± 1.3; CPAP: 24.6 ± 0.9 nmol/L; p<0.05). Conclusion: Even though CPAP proved to be more effective in attenuating OSAS and its subjective associated complaints, the better compliance to OA in this group of patients supported the reduction of one of the enzymes which participates in ROS removal. / FAPESP: 98/14303 / AFIP: 47.4277/2004-6 / TEDE

Assim, os objetivos desse estudo foram: avaliar a capacidade funcional de homens e mulheres com AOS e analisar o impacto do disturbio nas variaveis antropometricas, laboratoriais e da resposta ao exercicio em individuos magros. Metodos: Pacientes com AOS e controles foram selecionados do banco de dados do Instituto do Sono da UNIFESP. Avaliacao clinica, laboratorial, espirometria, ecocardiograma com doppler e teste de exercicio cardiopulmonar maximo, sintoma limitado, foram realizados. Para diagnostico da AOS foi considerado um indice de apneia-hipopneia (IAH) . 5 eventos/hora. Resultados: Homens com AOS apresentaram maiores niveis de consumo de oxigenio (VO2), producao de gas carbonico (VCO2), pulso de oxigenio (PuO2), frequencia cardiaca (FC) e pressao arterial sistolica (PAS) ao pico do exercicio quando comparados com o sexo feminino. Nas mulheres com AOS, houve uma correlacao negativa entre o VO2 e o IAH. Nao houve diferenca significativa na capacidade funcional entre pacientes eutroficos com AOS e controles pareados, evidenciado pelos mesmos valores de VO2, limiar anaerobio (LA) e coeficiente de troca gasosa (R) no esforco maximo. Pacientes magros com AOS apresentaram aumento da glicemia sanguinea e maior circunferencia cervical. O mesmo comportamento da FC, PAS e da pressao arterial diastolica (PAD) durante o esforco foi observado entre os diferentes generos, e entre pacientes e controles magros. Conclusoes: A AOS parece atuar de forma distinta na capacidade funcional. Em mulheres houve correlacao da gravidade do disturbio com o VO2, ao passo que nao houve influencia no desempenho fisico em pacientes eutroficos. Assim, estudos avaliando o desempenho ao esforco de uma populacao com AOS devem considerar de forma distinta os generos e a influencia do indice de massa corporea. / Purpose: To evaluate the functional capacity in men and women with Obstructive Sleep Apnea (OSA) and to analyze its impact on exercise capacity and blood parameters in lean subjects. Methods: OSA patients were selected from database of Sleep Institute of Sao Paulo city. All subjects underwent clinical evaluation, blood sample withdrawal, polysomnography, a maximum limited symptom cardiopulmonary exercise test, 2D-transthoracic echocardiography and spirometry. An Apnea-hypopnea Index (AHI) . 5 events/hour was used for OSA diagnosis. Results: Men had higher peak oxygen consumption (VO2), dioxide of carbon production, oxygen pulse, heart rate (HR) and systolic blood pressure (SBP). A significant negative correlation was found between severity of OSA as measured by AHI, and peak VO2 in women. Exercise performance is similar between lean OSA patients and lean non-OSA subjects considering the peak VO2, anaerobic threshold, and respiratory exchange ratio. High glycemia and cervical circumference enlargement are the main characteristics of lean OSA. There were no differences in HR, SBP and diastolic blood pressure during exercise between women and men with OSA, and between lean subjects. Conclusions: There was a correlation between OSA severity and VO2 in women. OSA did not influence exercise capacity in lean patients. Entretanto, a sobreposição entre elas (Overlap) e a associação com a obesidade, ainda necessita de investigações. Objetivos: Verificar a incidência da SAOS em portadores de DPOC obeso e as atividades inflamatória e metabólica desta tríade. Métodos: Foram incluídos portadores de DPOC moderado e grave com índice de massa corpórea (IMC) ≥ 27 Kg/cm2. Além da espirometria que classificou a DPOC, foi realizada polissonografia que diagnosticou ou não a SAOS. Foram avaliados os marcadores bioquímicos de glicemia, hemoglobina glicada (HBC), insulina, leptina, adiponectina, grelina, proteína C-reativa (PCR) e interleucina 6 (IL-6). Resultados: Este estudo transversal incluiu portadores de DPOC com (N=46) e sem (N=20) SAOS com médias de idade, IMC e VEF1, respectivamente, 61.57±11.31 x 59.75±9.68 anos, 34.00±5.67 x 33.89±6.75 kg/cm2, 57.20±16.81 x 53.85±17.65 %pred. As médias de glicemia, HBC, insulina, leptina e adiponectina com ou sem SAOS foram semelhantes. A grelina aumentou com a maior gravidade da SAOS na overlap (186.16±25.56 x 137.10±21.78 pg/ml). Não foi observado aumento da PCR para nenhum grupo. A maior gravidade da DPOC aumentou a IL-6 (5.17±5.79 x 11.18±13.10). Conclusão: A prevalência de SAOS é elevada em pacientes portadores de DPOC com obesidade. A SAOS grave elevou a atividade metabólica pelo aumento da grelina, sem influencia na expressão inflamatória, na tríade com DPOC e obesidade. Dieta hipocalórica e treinamento físico em pacientes com síndrome metabólica e apnéia do sono / Hypocaloric diet and exercise training in patients with metabolic syndrome and sleep apnea

INTRODUÇÃO. Estudos anteriores demonstraram que pacientes com síndrome metabólica (SMet) e apnéia obstrutiva do sono (SAOS) apresentam hiperativação simpática e hipersensibilidade quimiorreflexa. Neste trabalho, testamos as hipóteses de que: 1) dieta hipocalórica associada à treinamento físico (D+TF) melhora a sensibilidade quimiorreflexa em pacientes com SMet e 2) Os efeitos da D+TF seriam mais pronunciados em pacientes com SMet+AOS que em pacientes sem AOS (SMet-AOS). MÉTODOS. Vinte e sete pacientes nunca tratados da SMet (ATP-III) foram alocados em: 1) SMet+AOS (n = 15, 53±2 anos) e 2) SMet-OSA (n = 12, 43±2 anos). A AOS foi caracterizada por um índice de apnéia-hipopnéia (IAH)> 15 eventos / hora (polissonografia). Atividade nervosa simpática muscular (ANSM) foi avaliada pela técnica de microneurografia e pressão arterial (PA) pelo método oscilatório. A sensibilidade quimiorreflexa periférica foi avaliada através da inalação de uma mistura gasosa contendo 10% O2 e 90% N2 com titulação de CO2; e a sensibilidade quimiorreflexa central através da inalação de 7% CO2 e 93% O2 por 3 min. A dieta hipocalórica foi de -500 kcal da taxa metabólica de repouso e o treinamento físico se estendeu por 4 meses, 3 vezes/ semana. RESULTADOS. A associação da D+TF reduziu semelhantemente peso corporal (5,5±0,7 e 6,2±0,6 kg, P = 0,44), circunferência abdominal (CA, 5,6±1,2 e 5,4±1,0 cm, P = 0,91), PA sistólica (10,9±3,2 vs 13,3±3,5 mmHg, P = 0,62) e diastólica (8,5±1,6 vs 8,3±1,4 mmHg, P = 0,95) e, similarmente, o aumento do consumo de oxigênio de pico (20±5,9 e 16±7,3%, P = 0,69) em pacientes com SMet+AOS e SMet-AOS. A D+TF, reduziu significativamente o IAH (38±6,2 vs 18±3,9 eventos / hora, P = 0,01) e aumentou a saturação mínima de O2 (81±2,3 vs 84±1,9 %, P = 0,01) em pacientes com SMet+AOS. Durante a estimulação hipóxica, D+TF reduziu significativamente os níveis de ANSM tanto em pacientes com SMet+AOS (41±1,9 versus 33±2,0 impulsos/ min, P = 0,02) como em SMet-AOS (36±3,2 versus 28±1,7 impulsos/ min, P = 0,05). Durante a estimulação hipercápnica, D+TF reduziu significativamente os níveis de ANSM em pacientes com SMet+AOS (39±2,0 versus 30±1,1 impulsos/ min, P = 0,0005), mas não em pacientes com SMet- AOS. CONCLUSÕES. Tratamento não-farmacológico como D+TF melhora o controle quimiorreflexo periférico da ANSM em pacientes com síndrome metabólica. Esta mudança autonômica é mais pronunciada em pacientes com SMet+AOS, nos quais D+TF melhora tanto o controle quimiorreflexo periférico como o central. Além disso, D+TF melhora o distúrbio do sono em pacientes com SMet+AOS. Sendo assim, estes resultados sugerem que D+TF pode reduzir o risco cardiovascular em pacientes com SMet e AOS / INTRODUCTION. Previous studies have shown that patients with metabolic syndrome (MetS) and obstructive sleep apnea (OSA) have sympathetic hyperactivation and chemoreflex hypersensitivity. We tested the hypothesis that: 1) Hypocaloric diet associated with exercise training (D+ET) would improve chemoreflex sensitivity in patients with MetS and 2) The effects of D+ET would be more pronounced in patients with MetS+OSA than in patients without OSA (MetS-OSA). METHODS. Twenty three never treated MetS patients (ATP-III) were allocated into: 1) MetS+OSA (n=15, 53±2 yrs); and 2) MetS-OSA (n=12, 43±2 yrs). OSA was characterized by an apnea-hypopnea index (AHI) >15 events/hour (polysomnography). Muscle sympathetic nerve activity (MSNA) was evaluated by microneurography technique and blood pressure (BP) by oscillatory method. Peripheral chemoreflex sensitivity was evaluated by inhalation of 10%O2 and 90%N2 with CO2 titrated, and central chemoreflex by 7%CO2 and 93%O2 for 3 min. The hypocaloric diet was set at -500 kcal of the resting metabolic rate and exercise training extended over 4 months, 3 times/ week. RESULTS. D+ET similarly reduced body weight (5.5±0.7 and 6.2±0.6kg, P=0.44), waist circumference (WC, 5.6±1.2 and 5.4±1.0 cm, P=0.91), systolic BP (10.9±3.2 vs. 13.3±3.5 mmHg, P=0.62) and diastolic BP (8.5±1.6 vs. 8.3±1.4 mmHg, P=0.95), and similarly increased peak oxygen consumption (20±5.9 and 16±7.3%, P=0.69) in MetS+OSA and MetS-OSA patients. D+ET significantly reduced AHI (38±6.2 vs. 18±3.9 events/hour, P=0.01) and minimal O2 saturation (81±2.3 vs. 84±1.9%, P=0.01) in MetS+OSA patients. D+ET significantly reduced MSNA levels during hypoxia in MetS+OSA (41±1.9 vs. 33±2.0 bursts/min, P=0.02) and MetS-OSA (36±3.2 vs. 28±1.7 bursts/min, P=0.05) patients. D+ET significantly reduced MSNA levels during hypercapnia in MetS+OSA patients (39±2.0 vs. 30±1.1 bursts/min, P= 0.0005), but not in MetS-OSA patients. CONCLUSIONS. Non-pharmacological treatment based on D+ET improves peripheral chemoreflex control of MSNA in patients with MetS. This autonomic change is more pronounced in patients with MetS+OSA, in whom D+ET improves both peripheral and central chemoreflex controls. In addition, D+ET improves sleep disorder in patients with MetS+OSA. Altogether, these findings suggest that D+ET reduce cardiovascular risk in patients with MetS+OSA

A população idosa possui alta prevalência de edentulismo e, conseqüentemente, é afetada pelos problemas a ele associados. A perda da dimensão vertical de oclusão (DVO) é um destes problemas que compromete, entre outros fatores, o desempenho do sistema estomatognático. Logo, doenças relacionadas ao colapso da musculatura da via aérea superior (VAS), como a síndrome da apnéia obstrutiva do sono (SAOS), tornam-se enfermidades relevantes para pacientes nesta faixa etária. Sendo assim, medidas terapêuticas eficazes e de baixo custo, como a utilização de aparelhos intraorais (AIOs) para liberação do fluxo aéreo, podem ser empregadas contribuindo para a qualidade do sono destes pacientes. O objetivo deste estudo foi avaliar parâmetros subjetivos e objetivos do sono, em 10 pacientes idosos portadores de Próteses totais (PTs) bimaxilares, após a utilização de novas PTs confeccionadas no Programa Envelhecer Sorrindo e após o uso de dois AIOs: um dispositivo intraoral (DIO), especialmente desenvolvido para aumentar a DVO sem provocar avanço mandibular, e um aparelho de avanço mandibular (AAM). Para isso, questionários de rastreamento da qualidade do sono e polissonografias (PSGs) foram realizados, em quatro momentos distintos: sem as PTs, com as PTs, com o DIO e com o AAM. Foram realizadas, também, telerradiografias em norma lateral (TNL) dos pacientes com as PTs, com o DIO e com o AAM a fim de avaliar alterações no diâmetro da VAS e o posicionamento mandibular nesses três momentos. Concluiu-se que, o AAM testado promove maior porcentagem de sono no estágio 1, podendo contribuir para a melhora na qualidade subjetiva do sono dos pacientes, uma vez que ajudou a diminuir o tempo necessário para iniciar o sono, além de facilitar a manutenção do estado de vigília. / There is a high prevalence of edentulism and problems associated to it in the elderly population. A decrease in vertical dimension of occlusion (VDO) is one of these problems that may compromise the stomatognathic system. Therefore, it is important to investigate diseases related to upper airway (UA) musculature collapse, such as the syndrome of obstructive sleep apnea (OSAS). Therapeutic measures of low cost and high efficacy, such as intraoral appliances (IAs) to release the air flow may be employed, contributing to patients sleep quality. The aim of this study was to evaluate subjective and objective sleep parameters in 10 elderly patients who wore maxilar and mandibular complete dentures (CD). The analyses were performed after the use of a new pair of CD and after using two IAs: an intraoral device (ID), especially developed to increase the VDO without causing mandibular advance, and a mandibular advance device (MAD). For this purpose, questionnaires and polysomnography (PSG) were performed in four distinct stages: patients not wearing CD, wearing CD, wearing ID and wearing MAD. In addition, lateral cephalograms (LC) of patients wearing FD, wearing ID and wearing MAD were performed to assess changes in UAs diameter and in the mandibular positioning. It was concluded that the MAD tested promotes higher percentage of stage 1 sleep and and may contribute to the improvement in patients subjective sleep quality, as it helped to decrease the time needed to fall asleep, and facilitate the maintenance of wakefulness.

O tratamento ouro é a pressão positiva contínua nas vias aéreas, proporcionada pelo uso do Continuous Positive Airway Pressure (CPAP). A SAOS está associada a produção de resposta inflamatória no indivíduo assim como ao aumento do estresse oxidativo provocado pelos fenômenos hipóxia/reoxigenação. O tratamento com CPAP normaliza os eventos respiratórios e a estrutura do sono, porém existem muitos estudos controversos em relação ao tratamento com o CPAP e a redução dos marcadores inflamatórios e de estresse oxidativo. Também não há consenso quanto ao tempo mínimo de uso de CPAP para observar uma resposta à repercussão nociva da SAOS. Objetivo: Avaliar os níveis séricos de citocinas pró e anti-inflamatórias e de marcadores de estresse oxidativo em pacientes com SAOS tratados com CPAP pelo período de sete dias. Pacientes e Métodos: Trata-se de um ensaio clínico randomizado duplo-cego, com aprovação pelo Comitê de Ética em Pesquisa local (CAAE: 20839713.3.0000.5411). Its main treatment uses Continuous Positive Airway Pressure (CPAP). OSAS is associated to the production of inflammatory response in the individual as well as to the increase of oxidative stress caused by hypoxia/ re-oxygenation phenomena. Treatment with CPAP normalizes respiratory events and sleep structure, but there are many controversial studies regarding treatment with CPAP and reduction of inflammatory markers and oxidative stress. There is not a consensus in relation to the minimum time of CPAP utilization to observe a response to the harmful outcome of OSAS. Objective: To evaluate the serum levels of pro- and anti-inflammatory cytokines and markers of oxidative stress in OSAS patients treated with CPAP for seven days. Introdução: A apneia obstrutiva do sono (AOS) é caracterizada por episódios recorrentes de colapso parcial ou completo da faringe responsáveis por roncos e eventos de hipopneia ou apneia, respectivamente, associados à queda de saturação de oxigênio e despertares frequentes durante o sono. A AOS está associada à doença arterial coronariana e é um fator de risco independente para complicações após cirurgia. Entretanto, a maioria dos pacientes com AOS submetidos à cirurgia não tem suspeita ou diagnóstico prévio de AOS. Objetivos: O principal objetivo do estudo foi determinar prevalência da AOS em candidatos à cirurgia de revascularização do miocárdio (RM) e compará-la à prevalência da AOS em candidatos à cirurgia abdominal eletiva (ABD-cirurgia). Como objetivo secundário, avaliamos os preditores clínicos e o desempenho do questionário de Berlin, que estratifica os pacientes em alto risco e baixo risco de AOS, como teste de triagem no pré-operatório, assim como os preditores clínicos de AOS em ambos os grupos. Métodos: Foram incluídos 40 pacientes consecutivos no grupo RM [29 homens; idade: 56±7 anos; índice de massa corporal (IMC): 30±4 kg/m2], e 41 pacientes no grupo ABD-cirurgia, que foram pareados para sexo, idade e IMC (28 homens; idade: 56±8 anos; IMC: 29±5 11 kg/m2). Todos os pacientes foram submetidos à polissonografia completa noturna (PSG) e à avaliação clínica e laboratorial pré-operatória, incluindo avaliação da sonolência diurna com a escala de sonolência Epworth (ESS) e com o questionário de Berlin. Resultados: A prevalência de AOS (índice de apneia hipopneia na PSG 15 eventos/hora) no grupo RM e ABD-cirurgia foi alta e semelhante (52% e 41%, respectivamente, p=0,32). O grupo RM apresentou menor nível de sonolência (ESS: 6±3 e 9±5; RM vs. ABD-cirurgia, respectivamente, p=0,008). A sensibilidade e a especificidade do Berlin no grupo RM foi 67% e 26%, e no grupo ABD-cirurgia, 82 e 62%, respectivamente. O IMC, as circunferências abdominal e cervical, a pressão arterial sistólica, a pressão arterial diastólica, os triglicerídeos, a lipoproteína de alta densidade sérica (HDL-c), a Diabetes Mellitus e o risco alto de AOS (de acordo com questionário de Berlin) se correlacionaram com a AOS na análise univariada. No entanto, a circunferência abdominal foi o único preditor independente associado à presença de AOS após regressão logística múltipla. Conclusão: A AOS é extremamente comum entre pacientes candidatos à cirurgia cardíaca e cirurgia abdominal. O questionário de Berlin apresentou baixa sensibilidade para detecção AOS em pacientes do grupo RM, mas a sensibilidade e a especificidade no grupo ABD-cirurgia foram semelhantes aos valores encontrados na literatura. A sonolência diurna não está associada à presença de AOS entre portadores de doença arterial coronariana com indicação de tratamento cirúrgico e entre candidatos à cirurgia abdominal eletiva, o que pode ajudar a explicar o subdiagnóstico de AOS na nossa população / Background: The obstructive sleep apnea (OSA) is characterized by recurrent episodes of partial or complete collapse of the pharynx account for snoring and apnea or hypopnea events, respectively, associated with the decrease of oxygen saturation and frequent arousals during sleep. OSA is associated with coronary artery disease and is an independent risk factor for complications after surgery. However, most patients with OSA undergoing surgery is not suspected or previously diagnosed OSA. Objectives: The main objective of this study was to determine the prevalence of OSA in candidates for coronary arterial bypass grafting surgery (CABG) and compare it with the prevalence of OSA in candidates for elective abdominal surgery (ABD-surgery). As a secondary objective, we evaluated the clinical predictors and performance of the Berlin questionnaire, which stratifies patients into high risk and low risk for OSA, as a screening test in the preoperative as well as clinical predictors of OSA in both groups. Methods: We included 40 consecutive patients in the CABG group [29 men, age: 56 ± 7 years, body mass index (BMI): 30 ± 4 kg/m2] and 41 patients in the ABD-surgery, who were matched for gender, age and BMI (28 men, age: 56 ± 8 years, BMI: 29 ± 5 kg/m2 ¬). All patients underwent full nocturnal polysomnography (PSG) and clinical and laboratory pre-operative evaluation, 14 including assessment of daytime sleepiness with the Epworth Sleepiness Scale (ESS) and the Berlin questionnaire. Results: The prevalence of OSA (apnea hypopnea index in PSG 15 events/hour) in the RM group and ABD-surgery was high and similar (52% and 41% respectively, p = 0.32). Patients submitted to CABG presented lower levels of daytime somnolence than ABD-surgery patients (ESS: 6±3 vs. 9±5; p=0.008, respectively). The sensitivity and specificity of Berlin in the RM group was 67% and 26%, and ABD-surgery group, 82 and 62% respectively. The BMI, waist and neck circumference, systolic blood pressure, diastolic blood pressure, triglycerides, serum high density lipoprotein (HDL-C), Diabetes Mellitus and the high risk of OSA (according to questionnaire Berlin) correlated with OSA in univariate analysis. However, waist circumference was the only independent predictor associated with the presence of OSA after multiple logistic regression. Conclusion: OSA is extremely common among patients who are candidates for CABG and abdominal surgery. The Berlin questionnaire showed low sensitivity for detecting OSA in patients in the RM group, but the sensitivity and specificity in ABD-surgery group were similar to those found in the literature. Daytime sleepiness is not associated with the presence of OSA among patients with coronary artery disease with indication for surgical treatment and patients with indication for elective abdominal surgery, which may help explain the underdiagnosis of OSA in our population
