Patientflöden till akutmottagningen : En kvantitativ punktprevalensstudie

Bakgrund. Patientbesöken på svenska akutmottagningar ökar. Med ett ökat inflöde av patienter ökar även kraven på vårdpersonalen som arbetar på akutmottagningen. Vid obalans mellan in-, genom- och utflöde skapas risker för crowding, med konsekvenser som ökade väntetider för patienterna, ökade patientsäkerhetsrisker och lidande som följd. Akutsjuksköterskans ansvarsområden innefattar kunskap om patientflöden, patientsäkerhet och att lindra lidande. Ändå finns mycket lite nationell forskning om patientflöden till akutmottagningar. Syfte. Syftet med studien var att studera inflödet av patienter till akutmottagningarna vid ett länssjukhus och ett länsdelssjukhus. Metod. Studiens genomfördes som en kvantitativ punktprevalensstudie med en induktiv ansats. Studien innefattar analys av data från två akutmottagningar inom Region Dalarna, n=201. Resultat. Över 70 procent av patienterna som sökte till akutmottagningarna hade haft en tidigare vårdkontakt. Det var vanligast att yngre patienter hade sökvägen eget initiativ och att äldre hade sökvägen ambulans. Det var fler män än kvinnor som kom från vårdcentralen med remiss. Patienter som hade en annan vårdkontakt före besöket hade en högre medicinsk prioritet eller högre risk för inläggning än de patienter som kom på eget initiativ. Slutsats. Ett fysiskt möte med annan vårdpersonal kan ha stor betydelse för att minska inflödet av patienter till akutmottagningen. Att en stor del av patienterna blir hänvisade till akutmottagningen kan vara ett tecken på att det saknas kapacitet inom hälso- och sjukvården för att behandla patienter med icke akuta besvär. / Background. Patient flows to Swedish emergency departments are increasing. With an increased inflow of patients, the demands on the healthcare staff at the emergency department increase. With a discrepancy between in-, through- and outflow, risks of crowding are created, with consequences such as increased waiting times for patients, increased patient safety risks and suffering as a result. The emergency nurse's responsibility includes knowledge of patient flows, patient safety and alleviation of suffering. Nevertheless, there is little research about patient flows to the emergency department nationally. Purpose. The purpose of the study was to study the inflow of patients to the emergency departments at two regional hospitals. Method. The study was conducted as a quantitative point-prevalence study with an inductive approach. The analysis includes an analysis of data from two emergency departments in Region Dalarna, n = 201. Results. Over 70 percent of the patients that presented to the emergency departments had a previous healthcare contact. It was most common that younger patients used their own initiative and that elderly presented by ambulance. There were more men than women who came from community health centres with referral. Patients who had another healthcare contact before the visit had a higher medical priority or risk of admittance to the hospital than the patients who came on their own initiative. Conclusion. A physical meeting with healthcare staff can be of great importance for reducing the inflow of patients to the emergency department. It may be that there is a low capacity in the health care system to treat low-acuity patients.

The work also intends to identify those skills required for the professional application of social work. Therefore, my hypothesis is that individual and social care practice occurs in accordance with the personal skills and knowledge of each professional. Meanwhile little or nothing has been written or discussed about the knowledge related to this activity. Aiming to that goal, I have carried out a qualitative research based on empirical and theoretical investigative methods. Our sample has been divided into three groups: a) important Social Work scholars: professors with theoretical productions relevant to their profession; b) postgraduate students who are currently pursuing a Master or PhD degree and working as social workers in their daily life; and c) social work practitioners who are daily providing individual and social care services. The result of this research allowed me to "re-look" at Social Work individual practices,and pointed to the urgent need for academic spaces so as to produce technical knowledge that serves as reference for this important professional action / Esta tese tem como objetivo analisar criticamente o significado da prática profissional do atendimento sócio-individual em Serviço Social, situá-la historicamente, bem como ressignificá-la, do ponto de vista teórico-metodológico, tendo em vista sua importância no cotidiano das ações dos assistentes sociais. Pretendemos, com este trabalho, identificar quais são os conhecimentos necessários para o exercício profissional qualificado. Partimos, assim, da hipótese de que a prática de atendimento sócio-individual ocorre em acordo com as habilidades e conhecimentos pessoais de cada profissional, uma vez que pouco ou quase nada se discute e produz acerca dos conhecimentos relativos a esta atividade. Para tanto, realizamos uma pesquisa qualitativa, de caráter teórico empírico. Nossa amostra foi constituída por 3 grupos: a) sujeitos expoentes do Serviço Social (docentes com produções teóricas de relevância para a profissão); b) sujeitos cursando o mestrado ou o doutorado e atuam como assistentes sociais; e, c) sujeitos da prática, assistentes sociais que no dia-a-dia realizam o atendimento sócio-individual. O resultado desta pesquisa permitiu um re-olhar sobre as práticas individuais em Serviço Social e reafirmou a necessidade de a academia encontrar, urgentemente, reflexões e pesquisas que venham reforçar e reformular os conhecimentos desta prática profissional

Finding ways to utilize health resources that resettled refugees receive in New York City, USA

Refugee resettlement to the United States has decreased tremendously since 2017. This study investigates whether, and how, resettlement and health organization in New York are facilitating the distribution of health resources and how they are integrating refugees into becoming self-sufficient in the United States. Refugees have a right to many resources upon arrival in the United Stated especially health resources. Through case studies, this thesis examines two organizations contribution to changing the health status of refugees and the strategies that they implement to help refugees. Findings show that both organizations examined contribute to assist refugees to improve access to the health care. Findings also reveal many strategies for the facilitation of integration, the most prominent factor being interaction between organizations and refugees upon resettlement.

O município de Ribeirão Preto - SP, disponibiliza atenção pré-hospitalar pública através do Serviço de Atendimento Móvel de Urgência (SAMU) e de forma pioneira realiza teletriagem dos chamados de urgência em sua Central de Regulação. Assim, considerando que a teletriagem primária é um complexo método utilizado para garantir segurança, agilidade e eficácia às solicitações de socorro, apresentou-se esta proposta de investigação cujo objetivo foi verificar a aplicação da avaliação multifatorial do grau de urgência, através do mapeamento das teletriagens realizadas pela Central de Regulação do SAMU na cidade de Ribeirão Preto - SP em 2014. Neste sentido, o presente estudo se caracteriza como de abordagem quantitativa, modelo não experimental, retrospectivo, correlacional descritivo e de corte transversal, onde foram analisadas teletriagens primárias(n =2100) realizadas pela Central de Regulação do SAMU em 2014 e foi desenvolvido em quatro etapas: a primeira com uma revisão integrativa da literatura com 21 estudos primários e as demais com o interesse de descrever através do geoprocessamento a análise relacional entre as teletriagens pré-hospitalares da amostra e os fatores etiológicos, que justificam seu arranjo espacial, os casos selecionados foram submetidos a avaliação multifatorial do grau de urgência para posteriormente comparar com as distribuições espaciais encontradas, apresentando coeficiente Kappa = -0,0538. Ao final, houve comparação entre as diferentes configurações espaciais disponíveis e comprovou-se que não há evidencias da utilização da avaliação multifatorial do grau de urgência, nas amostras selecionadas para esta investigação, de teletriagens primárias do SAMU de Ribeirão Preto - SP em 2014, com base no raciocínio estatístico adotado e sob a ótica do geoprocessamento. Com isso, este estudo contribui para o monitoramento das triagens e, consequentemente, favorece o raciocínio dos fluxos do sistema, propondo o realojamento de unidades móveis de atendimento, a criação de mais equipes de suporte avançado, o engajamento em pesquisas de delineamento metodológico para desenvolver protocolos específicos para teletriagem pré-hospitalar no Brasil e tecnologias que apoiem e facilitem todo o processo / The city of Ribeirão Preto in SP, offers public pre-hospital care through the Mobile Emergency Service (SAMU) and a pioneer performs teletriage of so-called urgency in his Central regulation. Thus, considering that the primary teletriaging is a complex method to ensure safety, speed and efficiency to rescue requests, we presented this research proposal aimed to verify the application of multifactorial assessment of the degree of urgency, by mapping the teletriage made by SAMU Regulation Center in Ribeirão Preto - SP in 2014. in this sense, the present study is characterized as a quantitative approach, not experimental, retrospective design, descriptive correlational and cross-sectional, where primary teletriage were analyzed (n = 2100) conducted by the SAMU Regulation Center in 2014 and was developed in four stages: the first with an integrative literature review 21 primary studies and the other in the interest of describing through geoprocessing relational analysis between pre teletriage pre-hospital sample and etiological factors which justify their spatial arrangement; the selected cases were submitted to multifactorial assessment of the degree of urgency to later compare the spatial distributions found, with Kappa = - 0.0538 coefficient. At the end, there was a comparison between the different spatial configurations available and it was shown that there is no evidence of use of the multifactorial assessment of the degree of urgency, in the samples selected for this research, primary teletriagens SAMU of Ribeirão Preto - SP in 2014, with adopted based on statistical reasoning and from the perspective of geoprocessing. Therefore, this study contributes to the monitoring of trials and consequently favors the reasoning of system flows, proposing the relocation of mobile service units, the creation of more advanced support teams, engaging in methodological design of research to develop specific protocols for pre-hospital telescreening in Brazil and technologies that support and facilitate the process

The accessibility is intrinsically linked to meet the health services users’ needs and to ensure the solvability of assistance. Ambience is a concept that refers not only to physical space, but also to the social, made up of interpersonal and professional relationships in order to provide a solving, equitable and warm attention. Objective: To analyze the accessibility to health services and ambience of basic health care units in primary care in Goiás state. Methodology: A cross-sectional descriptive study performed in health care facilities in Primary Care in Goiás state in 2012. The study sample was done in 1216 basic health care units in Goiás state. Data were collected between July and September 2012 through a structured, tandardized and previously validated instrument. They were submitted to Ministry of Health database through the use of tablets and via internet to be analysed. After that these data were stored and grouped into modules I, II and III. Statistical analysis was performed using Statistical Package for the Social Sciences - SPSS software, version 19.0. The study respected ethical and legal aspects recommended by the Resolution 466/2012. Results: Two scientific articles were done. The first one "Accessibility to health care services in primary care in Goiás state" studied factors related to accessibility in health care facilities. The second article "Ambience of basic health care units in primary care in Goiás state " is about environment which provides better service and satisfaction to users and workers. Conclusion: Regarding accessibility and ambience in basic health units in primary care in Goiás state, we can conclude that the inaccessibility is contrary to the principle of integrality, hindering assistance and disrespecting those who need health services. It was also showed that there is a direct relationship between welcoming / humanized service and ambience. The lack of a suitable place for accommodation and care for users affects the development of actions in health care system. To change this reality, it will be necessary to invest in management in order to consolidate the Primary Health Care in a resolutely, equitable and welcoming way. / Introdução: A acessibilidade e a ambiência das Unidades Básicas de Saúde compõem dois elos de fundamental importância para o desenvolvimento do processo de trabalho da equipe. A acessibilidade está intrinsecamente ligada ao atendimento das necessidades dos usuários aos serviços de saúde e à garantia da resolubilidade da assistência. Já a ambiência é um conceito que se refere não só ao espaço físico, mas também ao social, formado pelas relações interpessoais e profissionais com o intuito de proporcionar atenção acolhedora, resolutiva e equânime. Objetivo: Analisar a acessibilidade aos serviços de saúde e a ambiência das unidades básicas de saúde na Atenção Básica do estado de Goiás. Metodologia: Pesquisa descritiva de corte transversal realizada nas unidades de saúde da Atenção Básica do estado de Goiás em 2012. A amostra do estudo foi constituída por 1216 unidades básicas de saúde do estado de Goiás. A coleta de dados ocorreu entre julho a setembro de 2012 por meio de um instrumento estruturado, padronizado e previamente validado. Para a análise, os dados foram enviados para um banco do Ministério da Saúde, por meio da utilização tabletes e via uma rede de internet. Após o envio, esses dados foram armazenados e agrupados em módulos I, II e III. A análise estatística foi realizada por meio do Software Statistical Package of Social Sciences - SPSS versão 19.0. O trabalho atendeu aos aspectos ético-legais preconizados pela Resolução 466/2012. Resultados: Foram construídos dois artigos científicos. O primeiro artigo denominado “Acessibilidade aos serviços de saúde na Atenção Básica do estado de Goiás” traz informações sobre o levantamento dos fatores relacionados a acessibilidades que estão presentes nas unidades de saúde. O segundo artigo “Ambiência das unidades básicas de saúde na Atenção Básica do estado de Goiás” versa sobre os ambientes que propiciam um melhor atendimento e satisfação aos usuários e trabalhadores. Conclusão: Ao analisar a acessibilidade e ambiência das unidades básicas de saúde na Atenção Básica do estado de Goiás, podemos afirmar que a inacessibilidade fere o princípio da integralidade, dificultando a assistência e prejudicando aqueles que precisam dos serviços de saúde. Também foi possível perceber que existe uma relação direta entre atendimento acolhedor/humanizado e ambiência. A inexistência de um local adequado para acolhimento, acomodação e atendimento dos usuários prejudica o desenvolvimento das ações realizadas nos serviços de saúde. Para mudar essa realidade, será preciso investimento da gestão, a fim de consolidar a Atenção Básica de forma resolutiva, equânime e acolhedora.

Vilka upplevelser och erfarenheter har vårdens chefer av att leda mångkulturella medarbetargrupper? : En kvalitativ intervjustudie

Den ökande globaliseringen bidrar till en kulturell mångfald i vården. Det finns få tidigare studier om ledarskap från mångkulturella arbetsplatser i Sverige och för att möjliggöra ett hållbart ledarskap av en hållbar mångkulturell arbetsplats behöver framgångsfaktorer i ledarskapet identifieras. I syfte att undersöka vilka upplevelser och erfarenheter vårdens chefer har av att leda mångkulturella medarbetargrupper har en kvalitativ intervjustudie genomförts. Totalt intervjuades 8 chefer från två olika sjukhus. Det insamlade materialet har transkriberats, analyseras och kategoriseras genom innehållsanalys (Elo & Kyngäs 2008). Resultatet visar att ledarskapet anpassades både utifrån individen och medarbetarens kulturella bakgrund. Det var främst språket som de intervjuade cheferna anpassade, men även på vilket sätt de kommunicerade med medarbetarna. Fördelar identifierades med en mångkulturell arbetsplats då personalen kunde tolka åt patienter och förmedla kunskap om kultur i medarbetargruppen. Informanterna upplevde inte att konflikter i medarbetargruppen härrörde från kulturella aspekter men däremot att missförstånd relaterade till språkförståelse kunde skapa konflikter. En förklaring till detta resultat kan vara att informanterna upplever det mer socialt accepterat att diskutera språkliga orsaker till konflikter än kulturella orsaker. Cheferna saknade stöd och utbildning i mångkulturellt ledarskap och uppgav att de fått kunskap genom erfarenhet och lärt av tidigare misstag. Författarna tror att cheferna kan få stöd genom mentorskap för att utveckla ett hållbart ledarskap och att färre misstag då skulle behöva begås. En organisatorisk tydlighet med regler och värdegrund ses också som viktiga för ett hållbart ledarskap.

Informanterna var strategiskt utvalda och bestod av fem sjuksköterskor som arbetade i ett palliativt hemsjukvårdsteam i Upplands län. Bandinspelade personliga intervjuer analyserades med en kvalitativ innehållsanalys.</p><p>Sjuksköterskorna upplevde att deras arbete kändes meningsfullt och utvecklande. Att arbeta i team var en mycket positiv aspekt. De svårigheter som framkom handlade om att anpassa sig efter olika situationer. Sjuksköterskorna gav stöd till anhöriga främst genom att kommunicera och samtala, samt förmedla trygghet. Det var viktigt att även både verbalt och praktiskt visa anhöriga att det fanns tid även för detta.</p><p>Eftersom det blir allt vanligare med palliativ vård i hemmet och den inte kan ske utan de anhörigas insats är det mycket viktigt att utveckla och optimera sjuksköterskans stöd till dem. The nurses supported next of kin mainly by communicating and conversing, and mediating safety. It was important to as well both verbally and practically show next of kin that there was also time for this.</p><p>Since palliative care in the home is getting more common, and since it cannot happen without the effort of next of kin, it is very important to develop and optimize the nurses' support to them. This studie can give insight in the problems that exist in this support, but also give insight about the organizational and personal achievements that work.</p>

Bakalauro baigiamojo darbo teorinėje dalyje aptariama paslaugos kokybės teorinė samprata, pristatomi paslaugos kokybės vertinimo teoriniai modeliai ir bendrieji paslaugos kokybės vertinimo kriterijai, kuriais remiantis parengtas tyrimo instrumentas – anketa, skirta vertinti P. Šimulio autoserviso paslaugos kokybę. Empirinėje dalyje pristatomi klientų nuomonės tyrimo rezultatai, kurie atskleidžia autoserviso klientų nuomonę apie techninę, funkcinę ir laukiamą kokybę. Tyrimo rezultatai patvirtina, kad P. Šimulio autoserviso paslaugų kokybė daugumos klientų yra vertinama gerai. Tyrimo metu nustatytos ir tobulintinos paslaugos kokybės sritys (aptarnaujančių darbuotojų kiekis ir jų apranga, darbuotojų mokėjimas pripažinti klaidą ir įmonės kainodaros sistema), kurias, remiantis vertinimo rezultatais, rekomenduojama gerinti. Darbą sudaro trys pagrindinės dalys: įvadas, teorinės dalis, empirinė – praktinė dalis, išvados ir rekomendacijos, literatūros sąrašas. Darbe yra aštuonios lentelės, 26 paveikslai. Atskirai pridedami šeši priedai. Darbo apimtis 67 p. teksto be priedų. / The theoretical conception of service quality is discussed in this final work of bachelor degree as well as the theoretical models of evaluation of service quality and overall criteria of evaluation of service quality are introduced in this paper. The instrument of this survey, the questionnaire, which was designed to evaluate the service quality of the P. Šimulis car care service centre, was based on them. The results of the clients’ opinions survey that show the service consumers’ views on technical, functional and expected quality are presented in the empirical part of the work. The obtained results confirm that the major part of the clients appreciate quality of service supplied by the P. Šimulis car care service centre. The improvement areas in quality of service (such as the quantity of operating personnel and their appearance, willingness to acknowledge their mistakes and the pricing system of the company) were identified during the survey and according to the evaluation of the results they should be improved. This final work consists of three main parts: introduction, theoretical part, empirical – practical part, conclusions and recommendations and the list of literature used. There are 8 tables and 26 pictures represented in this work. 6 annexes are listed separate. The bachelor paper consists of 67 pages excluding annexes.

Kokybės vadybos sistemos įgyvendinimas sveikatos priežiūros įstaigose personalo požiūriu / Personnel opinion on implementation of quality management system at health care institutions

Darbo tikslas - ištirti privačios ir VŠĮ (viešoji įstaiga) pirminės sveikatos priežiūros paslaugas teikiančios įstaigos personalo požiūrį į kokybės vadybos sistemos įgyvendinimą. Uždaviniai: išsiaiškinti sveikatos priežiūros įstaigos darbuotojų dalyvavimą įgyvendinant kokybės vadybos principus; nustatyti nepageidaujamų įvykių valdymą ir registravimą sveikatos priežiūros įstaigoje personalo požiūriu; ištirti medicinos personalo požiūrį į pokyčius sveikatos priežiūros įstaigoje kokybės vadybos sistemos įdiegimo laikotarpiu. Tyrimo metodika. 2014 m. vasario - balandžio mėn. atlikta anketinė apklausa. Apklausti 248 darbuotojai, dirbantys VŠĮ ir privačioje pirminės sveikatos priežiūros įstaigoje. Anketinių duomenų analizei naudota statistinė programa SPSS 17.0. Rezultatai. 95,8 proc. apklaustųjų žino, jog jų įstaigoje yra įdiegta kokybės vadybos sistema, o 59,1 proc. yra ja visiškai patenkinti. 87,9 proc. apklaustųjų vykdo dokumentuotas procedūras ir laikosi patvirtintos tvarkos, o 9,8 proc. tyrimo dalyvių kokybės vadybos sistemos tobulinime nedalyvauja. 45,5 proc. respondentų nori dalyvauti įstaigos kokybės valdyme. Didžioji dauguma nurodo, kad nepageidaujamų įvykių (NĮ) registravimas, priežasčių analizavimas padeda nustatyti problemas (94,9 proc.) ir mažina galimų neatitikčių skaičių (94,7 proc.). 87,2 proc. teigia, kad NĮ registracija gerina teikiamų paslaugų kokybę ir didina pacientų saugumą (88,2 proc.), tačiau nedaug įtakoja gerinant darbo aplinką darbuotojams (67,3 proc... Objectives: to determine the participation of Health Care Institution‘s personnel in the implementation of Quality management principles; to determine the management and registration of Adverse Events at Health Care Institution according to its personnel, to explore medical personnel‘s attitude towards the changes at Health Care Institution during the implementaiotn of Quality management system. Methods. A questionnaire survey was conducted in February – April 2014. 248 public and private Primary Health Care Institution employees were interviewed. Questionnaire data analysis was performed using SPSS 17.0 statistical software. Results. 95.8 % of respondents know that a Quality management system has been implemented at their Institution, while 59.1 % are completely satisfied with it. 87.9 % of the respondents exercise documented procedures and follow the approved order and 9.8 % of study participants do not take part in the improvement of Quality management system. 45.5 % are willing to participate in the management of Institution‘s quality.
