Ensino de geriatria e gerontologia na área da saúde : formação dos profissionais que atuam no atendimento do idoso na rede de serviços / TEACHING GERIATRICS AND GERONTOLOGY IN HEALTH CARE: training of professionals working in the care of the elderly in the service network.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / This study aimed to highlight the disciplines of the courses of Medicine, Nursing, Dentistry, Social Service, Physical Education and Psychology at the Federal University of Sergipe, which programs present subjects related to aging. It also identifies if during these courses the senior was focused as a biopsychosocial being, if these professionals felt the need for training in their area after higher education, if there was training and where it was performed. In this research, a qualitative and quantitative methodology was employed, characterized as a cross-sectional study, exploratory, descriptive and documentary. The following instruments were used: self-applied questionnaire with objective and subjective questions, and spreadsheet for analysis of the disciplines programs. In the qualitative analysis, it was used categorical analysis, and found subcategories were presented. In the quantitative analysis was calculated the frequency of appearance of the subcategories in the responses. For analyze the needs of the elderly, it was used the theory of hierarchy of human basic needs described by Maslow. The research was conducted at the Federal University of Sergipe UFS, hospitals and Basic Health Units UBS of the city of Aracaju-SE. Participated in this survey 265 graduated professionals by UFS and that serve the elderly, and of these, 91 were doctors, 71 nurses, 47 dentists, 32 social workers, 14 physical educators and 12 psychologists. It was obtained as a result that the training of health professional participants did not contribute significantly to the performance of these with the elderly population, according to their responses. It was observed that although formations are still focused on the treatment and prevention of diseases, except in the Social Service course that is more focused on the safety of the elderly, the elderly people needs on being esteemed and loved were seen by these professionals during their academic backgrounds. As additional results of this study were, it was obtained suggestions for improvement of the training, it were been shown that teamwork has hardly existed and that the failure in the education of professionals was reflected in difficulties in the practice of care for the elderly, except the psychologists who, despite having reported failures in their training, connected their difficulties to the physical limitations of the elderly. It was also observed that, despite the need for training of most of professionals, the few were trained and the most of the conducted trainings was organized by the Health Department of the Municipality of Aracaju, only for professionals working in their own UBS. . / Este estudo teve como objetivo evidenciar as disciplinas dos cursos de Medicina, Enfermagem, Odontologia, Serviço Social, Educação Física e Psicologia da Universidade Federal de Sergipe, que apresentam em seus programas assuntos referentes ao envelhecimento. Identifica, também, se durante a realização destes cursos o idoso foi enfocado como um ser biopsicossocial, se estes profissionais sentiram necessidade de capacitação na área após sua formação superior, se houve capacitação e onde estas foram realizadas. Nesta pesquisa, foi empregada uma metodologia qualiquantitativa, sendo a mesma caracterizada como estudo de corte transversal, de caráter exploratório, descritivo e documental. Como instrumentos, foram utilizados: questionário autoaplicável, com questões objetivas e subjetivas, e planilha para análise dos programas das disciplinas. Na análise qualitativa, foi utilizada análise categorial, na qual subcategorias encontradas foram apresentadas. Para análise quantitativa foi calculada a frequência de aparecimento das subcategorias nas respostas. Por sua vez, para análise das necessidades dos idosos, foi utilizada a teoria da hierarquia das necessidades humanas básicas descrita por Maslow. A pesquisa foi realizada na Universidade Federal de Sergipe UFS, em hospitais e em Unidades Básicas de Saúde UBS da cidade de Aracaju-SE. Participaram desta pesquisa 265 profissionais graduados pela UFS e que atendem idosos, sendo que destes, 91 são médicos, 71 enfermeiros, 47 odontólogos, 32 assistentes sociais, 14 educadores físicos e 12 psicólogos. Obteve-se como resultado nesta pesquisa que a formação dos profissionais de saúde participantes desta muito pouco contribuiu para a atuação destes junto à população idosa, conforme suas respostas. Observou-se que, apesar das formações ainda estarem voltadas para o tratamento e prevenção de doenças, à exceção do curso de Serviço Social, que está mais voltado à segurança do idoso, as necessidades do idoso de estima e de amor foram vistas por estes profissionais durante suas formações acadêmicas. Como resultados adicionais deste trabalho, foram obtidas sugestões para a melhoria da formação dos profissionais, foram mostrados que o trabalho em equipe praticamente não tem existido e que a falha na formação superior dos profissionais refletiu-se em dificuldades na prática do cuidado ao idoso, à exceção dos psicólogos que, apesar de terem referido falhas em sua formação, relacionaram suas dificuldades às limitações físicas do idoso. Observou-se ainda que, apesar da necessidade de capacitação da maioria dos profissionais, poucos foram os que se capacitaram, e que a maioria das capacitações realizadas foi organizada pela Secretaria de Saúde do Município de Aracaju, apenas para os profissionais que trabalham em suas próprias UBS.

Chefers möten med medarbetare i äldreomsorgen : En observationsstudie om chefers förhållningssätt / Managers' approach with employees in elderly care : An observational study of managers' attitudes

Äldreomsorgen står inför en rad nya utmaningar, en ökande andel äldre befolkning som har stora vård- och omsorgsbehov samtidigt som resurserna till verksamheterna minskar. Detta innebär att verksamheterna behöver organiseras och ledas för en hållbar och hälsofrämjande arbetsmiljö för medarbetarna med chefer som leder med delaktighet och tillit som grund. Hur visar sig denna förmåga hos chefer att bjuda in till delaktighet och ge stöd i vardagen? Syftet med studien är att beskriva chefers förhållningssätt som blir synligt i mötet med medarbetare i äldreomsorg. En kvalitativ observationsstudie med induktiv ansats har använts och enhetschefer inom äldreomsorgen har observerats med ett passivt deltagande av författarna. Resultatet redovisas utifrån de underkategorier och kategorier som innehållsanalysen gav. De tre kategorierna är Kommunicerande förmåga, Relationsskapande och Uppdragsinriktad. Resultatet visar att enhetschefer uppvisar relationsskapande och kommunicerande förmåga till medarbetarna. Enhetscheferna förstärkte sina ord och tankar med sitt kroppsspråk i kommunikationen med sina medarbetare. Enhetscheferna arbetade aktivt med att få medarbetarna engagerade i verksamheten och när olika problem uppstod var enhetschefen tydlig med att låta medarbetarna själva komma med olika förslag på lösningar och bjöd in dem till delaktighet. Resultatet kan sammanfattas i temat Ett förhållningssätt som synliggörs genom att ha fokus på uppdraget grundat i kommunikation och skapandet av relationer. / Elderly care is facing several new challenges, an increasing part of the old population with significant health and care needs, as the same time as limiting resources for the function. This means that the operations need to be organized ang managed for sustainable and health-promoting work environment for the employees with managers who lead with participations and support? The purpose of the study is to describe managers attitudes that become visible in the meeting with elderly care workers. A qualitative observational study with inductive approach has been used and unit managers in the elderly care have been observed with the passive participations of the authors. The result is reported based on the sub-categories and categories that the context analysis provided. The three categories are Communicative Ability, Relationship Creation and Mission-oriented. The result show that unit managers show relationship-building and communicative ability to employees. The unit managers complemented their verbal language/ communication by using bode language in communication with their employees. The unit managers worked actively to get the employees involved in the business and when various problems occurred, the unit manager was direct in allowing the employees themselves to come up with different proposals for solutions and invited them to participate. The result can be summarized in the theme an approach that is made visible by focusing on the assignment based on communication and the creating of relationships.

The main criteria for managing good postoperative care include the effective and appropriate choice of pain therapy. Experiencing pain is a purely individual feeling for each of us, so it is very important to have adequate and sufficient analgesia during this period. Modern management of post-operative pain therapy uses preparations of various pharmacological groups that potentiate and thereby more effectively reduce pain. By this mechanism, we can reduce the total dose of analgesics given, and more particularly, reduce the dose of opioids that have a higher risk of side effects. In this project i want to show how the analgesic catheter could be useful as alternative possibility in algorithm therapy of postoperative pain. Analgesic catheter enable continual local anesthetics administration, exactly 0,5% bupivacaine, which is operating in surgical wound. Main goal of the study is to find out and verify if analgesic catheter is method which provides continual analgesia strong enough to results in lower use of opioids. For most people, family represents a place where they feel safe and loved. But not everyone is lucky enough to have a place like this, to have a functional family. If the biological family does not work as it should and a baby is threatened in some way, the state has several options to help the child. If there are solvable problems in the family, the state can try to help it by various interventions to improve the situation. But if these actions do not help and the child is forced to leave this family, it is necessary to ensure alternative environment for baby. The child can either be placed in institutional care, or to substitutional family care. Considering the shortcomings of institutional care, various forms of substitutional care are mainly supported. Probably the best known form of substitutional care is adoption. In the case of adoption, baby becomes the member of new family and relationships between child and his biological family vanishes. This form is only possible for a small percentage of abandoned children, because most of them still has some kind of relationship with biological family. Applying for home care services : A qualitative study of how first-time users of social care for older people experienced the application process

Aim: To explore how older people (>65 years), who have been granted home care services for the first time, experienced their contact with the municipal need assessor and the process of the need assessment. Method/data: Semi-structured interviews (n=7) have been conducted with first-time users of social care for older people (>65 years) living in one district in the City of Stockholm. The interviews were thematically analysed based on the theoretical approach by Braun & Clarke (2006).Findings: Applying for home care services affects several dimensions of thoughts and emotions related to the process of ageing. Compromised ability/health/independence and the accessibility of informal care influenced the respondent’s tendency to apply. Thoughts and emotions related to identity, autonomy and managing tasks on their own was significant. Both positive and negative emotions related to the contact with the municipal need assessor was found.Conclusions: Municipal home care services include several aspects beyond the practical matters. The contact with the municipal need assessor was not the main topic discussed, instead focus was put on the thought process/introspective process leading up to an application. In addition, applying for home care services is sometimes experienced as “the last resort” when other possibilities are exhausted.

Addresing Challenges in Caring for Morbidly Obese by Learning about Bariatric Care

Obesity is a public health issue linked to high morbidity and mortality among critically ill patients. There are approximately 15.5 million morbidly obese adults in the United States. The purpose of the project was to develop and implement an educational program using evidence-based protocols for bariatric care to educate nurses and caregivers regarding best practices when attending to obese patients. The practice-focused question examined whether learning about evidence-based bariatric care would improve the knowledge of nurses and caregivers caring for morbidly obese patients in an acute care setting. The theoretical foundation was Bandura'€™s self-efficacy theory. A questionnaire using a Likert scale was used to collect data from the 100 participants before and after the learning intervention. The selection criteria involved the inclusion of all nurses and caregivers working at the adult in-patient unit. A paired-samples t-test was used to evaluate levels of improvement in knowledge of the causes, treatment, management, and care of patients with obesity and the challenges in caring for morbidly obese patients. The findings indicated a statistically significant increase in participants'€™ knowledge of the causes (p < 0.000), treatment, management, and care of patients with obesity (p < 0.000) and the challenges involved in caring for morbidly obese patients after the learning intervention (p < 0.004). Thus, the implementation of an educational intervention may be effective in improving nurses'€™ knowledge of bariatric care. The implications of the project for social change involve the improvement in nurse'€™s knowledge of clinical guidelines, which can lead to increase in patient satisfaction, and improved overall health outcomes.

Local Nature Matters : A Story of How Empowerment from Tourism Contributes to Local Nature Stewardship in Rural Communities

This Master thesis examines the role of resident empowerment in rural nature tourism in shaping the relations between how residents engage with local places and local nature stewardship in Plouguerneau, France. Through a conceptual framework that includes the concepts of empowerment, place attachment, care for nature, and nature stewardship, the study models the effect of place attachment and care for nature on resident stewardship attitudes and how resident empowerment in tourism mediates this relationship. The theoretical underpinnings of social exchange theory (SET) and nature’s contributions to people (NCP) theory are employed to provide a comprehensive understanding of these relationships. The study employs a mixed-method approach, including online and door-to-door surveying. To analyze the results, partial least square structural equation modeling (PLS-SEM) was used. The findings suggest that place attachment is not a significant predictor of empowerment through tourism or nature stewardship as only place identity was predicting political empowerment and environmental citizenship. This finding challenges the assumption that strong place attachment necessarily leads to empowerment from tourism and nature stewardship. Therefore, it highlights the need for a more nuanced understanding of the factors that motivate residents to engage in environmentally responsible behaviors. The results also show that care for nature is significantly predicting both empowerment from tourism and nature stewardship. Furthermore, the study reveals that tourism can be a powerful tool for empowering local communities and enhancing their sense of responsibility towards the environment. Specifically, the findings suggest that involving residents in tourism decision-making processes can promote their engagement in environmental citizenship. Overall, this thesis contributes to our understanding of how tourism can contribute to local nature stewardship in rural communities by empowering residents to take an active role in caring for their natural environment.

Den kommunala äldreomsorgen har hög sjukfrånvaro bland medarbetarna vilket skapar arbetsmiljöproblem för enhetscheferna som har svårt att hantera detta. Syftet med denna studie är att skapa förståelse för enhetschefers förutsättningar att hantera sjukfrånvaron inom den kommunala äldreomsorgen. Tidigare forskning är begränsad rörande enhetschefers förutsättningar att hantera sjukfrånvaron. Politikerna, som är arbetsgivare, har det juridiska ansvaret och ska styra verksamheten för att främja en bra arbetsmiljö, både organisatoriskt, socialt och fysiskt. Det får konsekvenser för hela organisationen om inte chefers arbetsmiljö och förutsättningar är goda. Denna studie har genomförts med en kvalitativ ansats med tio enhetschefer från två utvalda kommuner och kombinerar teorier om organisatoriska förutsättningar med ett medarbetarperspektiv. Empirin har samlats in med hjälp av semistrukturerade intervjuer som genomfördes digitalt via Zoom. Berättelserna från intervjuerna har analyserats och sammanställts genom en tematisk analys. Resultatet visar att enhetscheferna är begränsade i hanteringen av sjukfrånvaron på grund av organisatoriska förutsättningar vad gäller både de krav som ställs och de resurser som tilldelas. Denna studie bidrar med förståelse för vad organisatoriska förutsättningar betyder för enhetschefers möjlighet att hantera sjukfrånvaron i den kommunala äldreomsorgen. Studien visar också att medarbetaren har betydelse för enhetschefers förutsättningar att hantera sjukfrånvaron. / The municipals care for the elderly has a high level of sick leave among the employees, this creates work environment problems for the unit managers who finds this difficult to deal with. The purpose of this study is to create an understanding of unit managers' conditions for managing sick leave within municipal care for the elderly. Previous research is limited concerning unit managers' conditions for managing sickness absence. Politicians as employers have the legal responsibility and must direct the business to promote a good organizational, social and physical working environment. If there are insufficiencies in the work environment and conditions for managers', it will impact the entire organization. This study has been carried out with a qualitative approach with ten unit managers from two selected municipalities and combines theories about organizational conditions from an employee perspective. The empirical data has been collected by using semi-structured interviews that were conducted digitally via Zoom. The stories from the interviews have been analyzed and compiled througha thematic analysis. The result shows that the unit managers are limited in the management regarding sick leave due to organizational conditions in terms of both the demands made and the resources allocated. This study contributes with an understanding of what organizational conditions mean for unit managers' ability to manage sick leave in municipal care for the elderly. The study also shows that the employee is important for unit managers' conditions for managing sick leave.

Dental care of the seniors in the nursing homes.

ABSTRACT  Background: During the past years the authorities and the dental care in the world have begun to realize the importance of maintaining a good oral health of the seniors. The care staff at special residences have an essential role in the daily oral care of the elderly. The oral state of health is also important for the rest of the health. Studies show that oral care is deficient in the elderly and needs to be highlighted as the elderly population is expected to increase.   Aim: The aim of this literature review is to evaluate the potential improvement regarding the staff's work with oral care in elderly care.  Methods: Literature study search was performed with MeSh terms in the database PubMed and Scopus. The following keywords have searched:Dental care for aged, Education, Nursing homes, Nursing staff, Oral health, Oral care.    Results: Several studies have found improvements that are beginning to emerge to develop the oral care of the elderly. These improvements can be achieved by a convenient oral care training that is designed for the elderly. The training programs for the care staff working with the elderly positively affects their attitudes, routines and documentation of the patient’s information, which benefits the elderly oral health.    Conclusion:To achieve good oral care for the elderly, several improvements are needed to be done by the governments involving nursing and working staff´s routines. A little different has been done, but there is significant work still to be done to improve the elderly dental health.

Den evidensbaserade praktiken har blivit en alltmer påtaglig del av socialarbetares praktik. Betoningen på evidensbasering inom socialt arbete i Sverige har implementerats främst genom statliga myndigheter ner i professionen, vilket har bemötts med en generell skepsis av yrkesfältet. HVB-hem för barn och unga är en del av den sociala barnavården som påverkas av den statliga kunskapsstyrningen. Studiens analys fokuserar på hur behandlingspersonal på HVB-hem för barn och unga upplever att den evidensbaserade praktiken påverkar den professionellas möjlighet att bedriva behandlingsvård. Nio semistrukturerade intervjuer har genomförts med behandlingspersonal som arbetar på HVB-hem för barn och unga och som har haft en två-årig eftergymnasial utbildning. Informanternas utsagor har analyserats utifrån professionsteori och symbolisk interaktionism. Resultatet har påvisat att behandlingsarbetet kan innebära en varierande behandlingskontext och att flertalet förutsättningar påverkar yrkesutövningen. Den evidensbaserade praktiken kan däremellan utgöra ett gemensamt yrkesspråk och förhållningssätt på HVB-hem för barn och unga, som kan tolkas och tillämpas olika. Möjligheten att tillämpa evidens kan ses grunda sig i behandlingspersonalens utbildningsbakgrund och yrkesfärdighet. För att kunna arbeta med evidens implicerar det att den professionella kan läsa, förstå och tillämpa evidens inom behandlingsvården. Å ena sidan kan den evidensbaserade praktiken som idé bedömas vara en självklarhet för behandlingspersonal på HVB-hem för barn och unga. Å andra sidan kan definieringen och tillämpningen av den evidensbaserade praktiken bedömas vara oklar och komplicerad i en kontextbunden praktik, som HVB-hem för barn och unga inte sällan är.
