Prevalência e incidência de obesidade sarcopênica em coorte de idosos domiciliados no município de São Paulo / Prevalence and incidence of sarcopenic obesity in the elderly people cohort in community-dwelling in São Paulo

Introdução: Evidências epidemiológicas mostram que a obesidade sarcopênica (OS) em idosos está associada a um acelerado declínio funcional e alto risco de morbimortalidade, sendo que seu impacto tem se tornado grande preocupação dos profissionais de saúde. Objetivo: Estimar a prevalência e a incidência de obesidade sarcopênica, em coorte de idosos domiciliados no município de São Paulo/Brasil 2000 e 2010. Casuística e Métodos: Foram utilizados dados do Estudo SABE (Saúde, Bem-estar e Envelhecimento), realizado no município de São Paulo em 2000 (2.143 idosos), e em 2010 (795 idosos). A população deste estudo foi constituída por idosos ( 70 anos), de ambos os sexos, que apresentaram todos os dados necessários para este estudo e que concordaram em participar, totalizando 871 idosos analisados em 2000 e 656 idosos em 2010. As variáveis de estudo foram: 1. Dependente - obesidade sarcopênica, identificada segundo: obesidade, diagnosticada pelo valor da circunferência da cintura (CC 94 cm e CC 80 cm para homens e mulheres, respectivamente); sarcopenia, identificada por: 1- força muscular, pelo teste de preensão manual (FPM - kg) (baixa P25; normal > P25, da mesma população), 2- massa muscular (MM), obtida pelo índice de massa muscular (IMM=MM/altura²) (baixa P20; normal > P20, da mesma população) e 3- desempenho físico, identificado pelo teste (tempo dependente segundos) de sentar e levantar 5 vezes de uma cadeira (SeL) , com os braços cruzados sobre o peito (baixo P75; normal < P75); 2. Explanatórias - sexo e grupos etários (70 79 e 80). Foram classificados com OS idosos que apresentaram, simultaneamente, valores de CC adotados e baixo desempenho e baixa MM, ou então, desempenho normal, mas baixas FPM e MM. A prevalência de obesidade sarcopênica em 2000 e em 2010 foi estimada pelo número de casos de OS identificados nos dois momentos, onde foram realizadas comparações entre os intervalos de confiança, para verificar diferença estatística em idosos ( por cento ) com OS, segundo variáveis explanatórias, com nível de significância de 5 por cento . Para o cálculo do coeficiente de incidência de OS, em 2010, foi considerado o tempo de observação de cada indivíduo, determinado de maneira específica para cada caso. Para os cálculos foi utilizado o programa: Stata/SE ® 10.0 for Windows. Resultados: Dos 871 idosos analisados em 2000, 85 (7,4 por cento ) foram identificados com OS [6,5 por cento mulheres (IC 5,08,4) e 4,8 por cento 80 anos (IC 3,6-6,4)] (p 5 por cento ), e, em 2010, (n=656), 73 (9,2 por cento ) foram identificados com OS [7,2 por cento mulheres (IC 5,5-9,4) e 5,3 por cento 80 anos (IC 4,0-7,0)] (p 5 por cento ). Em 10 anos, foram identificados 43 novos casos de OS. O coeficiente de incidência foi 15,29/1000 pessoas/ano entre 2000 e 2010. Conclusões: A prevalência de OS em 2000 e 2010 foi maior nas mulheres e nos idosos mais longevos, sendo que, em ambos os casos, foi maior em 2010, quando comparada a 2000. Não houve diferença significativa entre os coeficientes de incidência, segundo as variáveis explanatórias. / Background: Epidemiological evidence shows that sarcopenic obesity (SO) is associated with accelerated functional decline and high risk of morbidity and mortality, and its impact has become major concern of health professionals. Objective: To estimate the prevalence and incidence of sarcopenic obesity in the elderly cohort domiciled in São Paulo / Brazil - 2000 and 2010. Methods: We used data from the SABE Survey (Health, Well-being and Aging), held in São Paulo in 2000 (2.143) and 2010 (795). The study population consisted of elderly ( 70 years) of both sexes, who had all the necessary data for this study and who agreed to participate, totaling 871 elderly analyzed in 2000 and 656 elderly in 2010. The variables of study were: 1.Dependent sarcopenic obesity, identified according to: obesity, diagnosed by the value of the waist circumference (WC 94 cm and WC 80 cm for men and women, respectively); sarcopenia, identified by: 1-muscle strength, by handgrip test (kg) (low P25; normal > P25, of the same population), 2-muscle mass (MM), obtained by muscle mass index (MMI = MM/height ²) (low P20; normal > P20, of the same population) and 3-physical performance, identified by the test (time-dependent seconds) to sit down and get up 5 times a chair (S&L), with their arms crossed on the chest (low P75; normal < P75); 2. Explanatory: sex and age groups (70-79 and 80).They were classified with SO elderly who presented, simultaneously, WC values adopted and low performance and low MM, or, normal performance but low muscle strength and MM. The prevalence of obesity sarcopênica in 2000 and in 2010 was estimated by the number of the cases identified in two moments, where comparisons were made between confidence intervals, to check statistical difference in the elderly ( per cent ) with the second explanatory variables, with a significance level of 5 per cent . For the calculation of the incidence coefficient, in 2010, it was considered the each individual observation time, determined specifically for each case. For the calculations we used the program: Stata / SE ® 10.0 for Windows. Results: Of the 871 analyzed elderly in 2000, 85 (7.4 per cent ) were diagnosed with SO [6.5 per cent women (CI 5.0-8.4) and 4.8 per cent 80 years (CI 3,6-6.4) ] (p 5 per cent ), and, in 2010 (656), 73 (9.2 per cent ) were identified with SO [7.2 per cent women (CI 5.5- 9.4) and 5.3 per cent 80 years (CI 4.0-7, 0) ] (p 5 per cent ). In 10 years, were identified 43 new cases of SO. Incidence coefficient of SO was 15.29/1000 people/year between 2000 and 2010. Conclusion: The prevalence of SO in 2000 and 2010 was higher in women and the oldest old, and, in both cases, was higher in 2010, compared to 2000. There was no significant difference between the incidence coefficient, according to the explanatory variables.

Amongst these various hindrances, one that grows the most with the advancement of technology is the one pertaining to the interaction with the interfaces of several types of devices; in particular in self-service ATMs, whose interfacial elements somehow do not satisfy the elderly user during their operations. The main objective of this research is to analyze the interactive aspects of a self-service ATM interface of a Brazilian bank, considering the perception of elderly individuals in the city of São Luís, Maranhão. To evaluate the interaction, the proposed methodology to be carried out was based on the participative design approach and was divided in three situations, these being the initial exploration, the discovery process and the prototyping. In order to achieve results, techniques such as questionnaires, focus groups and usability tests were employed using a paper prototype with the purpose of validating the suggested hypothesis, and thus results and requirements were generated for the adequacy of elements present in these interfaces. The main results achieved were the identification of the main executed tasks, which are withdrawal and balance consultation. Another result was the identification of the main interactive difficulties, in which it was observed that the biggest one was the short time of the transition of the screens, and, through the test with the prototype, it was detected that the placements of some items are not satisfactory for the users. / Há atualmente uma grande preocupação com a população idosa, visto que atualmente existe uma característica acentuada, que é a ausência de percepção do idoso na sociedade e a consequente despreocupação com seus bloqueios e suas restrições. Dentre os diversos bloqueios, um dos que mais crescem com o avanço da tecnologia é o da interação com as interfaces de diversos tipos de dispositivos, particularmente nos caixas de autoatendimento, cujos elementos interfaciais não satisfazem, de algum modo, o usuário idoso durante sua utilização. O objetivo principal desta pesquisa é analisar a interação da interface do caixa de autoatendimento de um banco brasileiro, considerando a percepção de indivíduos idosos da cidade de São Luís, no Maranhão. Para avaliar a interação, foi proposta uma metodologia que foi realizada com base na abordagem do design participativo e dividiu-se em três situações, sendo elas a exploração inicial, o processo de descoberta e a prototipação. Para atingir resultados, aplicaram-se técnicas como questionários, grupos focais e testes de usabilidade utilizando um protótipo de papel com o propósito de validar a hipótese sugerida, e com isso geraram-se resultados e requisitos para a adequação de elementos presentes nessas interfaces. Os principais resultados alcançados foram a identificação das principais tarefas realizadas que são saque e consulta de saldo. Outro resultado alcançado foi a identificação das principais dificuldades de interação, nas quais observou-se que a maior delas foi o tempo curto da passagem das telas, e a partir do teste com o protótipo, observou-se que os posicionamentos de alguns itens não são satisfatórios para os indivíduos.

Uso da internet pelo consumidor da terceira idade : influências do risco percebido e impacto na intenção de compra online

Atualmente, observa-se que o ambiente virtual está cada vez mais povoado por pessoas com 60 anos ou mais e com os mais diversos interesses, todavia, verifica-se que há pouco conhecimento sobre como os consumidores idosos formam julgamentos e tomam decisões a respeito de produtos e serviços oferecidos na Internet, chegando a existir, inclusive, um pré-conceito de que estes indivíduos não são usuários da Internet e, portanto, não merecem que se façam investimentos para adequar produtos e serviços a eles. Sabe-se que diferentes fatores podem influenciar o comportamento do consumidor em sua tomada de decisão, principalmente quando se abordam produtos e serviços tecnológicos. Frente a esse contexto, é fundamental que se compreendam melhor os elementos que estão presentes nesse comportamento de consumo e as suas implicações futuras. Por essa razão, a presente pesquisa foi elaborada, pretendendo-se identificar os principais construtos que influenciam o comportamento do consumidor da terceira idade na Internet e analisar o seu impacto sobre a intenção de compra online. Objetivando responder a tal questionamento, projetou-se este estudo, para o qual foi elaborado um modelo teórico (testado empiricamente) contendo diversas variáveis e moderadores. Após 5 pré-testes e 8 entrevistas em profundidade, um questionário foi aplicado em 430 respondentes com 60 anos ou mais que tivessem utilizado a Internet pelo menos uma vez nos últimos 3 meses. O processamento de dados incluiu uma análise fatorial exploratória e uma confirmatória e a Modelagem de Equações Estruturais. Ao final deste estudo, obteve-se aceitação plena de cinco hipóteses, parcial de três e rejeição de duas, além de um bom ajuste do modelo proposto (a partir dos índices pré-definidos). Confirmou-se a hipótese de que o risco percebido pelos consumidores da terceira idade ao utilizar a Internet é um moderador no modelo estrutural proposto, uma vez que se comprovou diferença estatística significante entre as respostas dadas por um grupo mais avesso ao risco (Grupo 1) e outro menos avesso ao risco (Grupo 2) no que concerne às características demográficas e comportamentais (relacionadas ao uso da Internet). Já a escala de Tipo de Uso não se mostrou adequada para analisar a utilização da Internet pelo público da terceira idade. Constatou-se, da mesma forma, que a idade cognitiva influencia, de maneira inversa, no tempo de uso da Internet e que a satisfação com o uso da mesma tem impacto positivo no boca-a-boca positivo realizado, na intenção de (re)compra online e, também, no tempo de uso da rede. Acredita-se que os resultados obtidos a partir do teste do modelo apresentado possam fornecer importantes implicações acadêmicas e gerenciais, as quais possibilitarão uma maior compreensão sobre o uso da Internet por indivíduos da terceira idade, seus impactos futuros e a existência de moderadores importantes nesta relação. / Virtual environment has been increasingly used by people aged 60 and over who have a variety of interests. However, there is little knowledge concerning how elderly consumers form judgment and make decisions regarding products and services offered on the internet. Additionally, there is a preconception that these individuals do not use the internet; therefore, there would be no need for investment in redesigning products and services to suit them. It is known that there is a range of factors that may influence consumer behavior, especially in online environments involving technological products and services. In this context, understanding the elements involved in consumer behavior as well as its future implications is of paramount importance. Thus, this study reviewed the main constructs that influence the consumer behavior of the elderly on the internet and analyzed its impact on the online purchase intention. A theoretical model containing several variables and moderators was created and empirically tested. After 5 pretests and 8 in-depth interviews, a questionnaire was applied to 430 respondents aged 60 and over who had used the internet within the last 3 months. Data processing included an exploratory and a confirmatory factor analysis, as well as the Structural Equation Modeling. As a result, five hypotheses were fully supported, three were partially confirmed and two were rejected. Moreover, the proposed model proved to be well adjusted according to the pre-defined indexes. The hypothesis which states that the perceived risk by the elderly regarding the use of internet is a moderator on the proposed structural model was confirmed, since there was a statistically significant difference between the answers given by a more risk-averse group (Group 1) and the answers provided by a less risk-averse group (Group 2) concerning demographic and behavioral characteristics related to internet use. On the other hand, the Type of Internet Use scale was shown to be inadequate to analyze the internet usage by the aforementioned age group. Likewise, it was observed that cognitive age has an inversely proportional influence on time of Internet usage, and that satisfaction with its usage has a positive impact on positive word of mouth, online (re)purchase intention and time of web usage. It is believed that the results obtained from the testing of the presented model may provide important academic and managerial implications which will bring a greater understanding of internet usage by the elderly as well as its future impacts and the existence of important moderators within this relationship.

Um estudo sobre o design de livros para a terceira idade

Esta pesquisa analisa livros de literatura quanto à adequação do projeto gráfico para leitores de terceira idade. Foi realizada uma revisão bibliográfica abordando os aspectos do design gráfico e do design do livro, as questões de leitura, legibilidade, lei-turabilidade e tipografia. Durante esta etapa foram abordados o envelhecimento popula-cional brasileiro, as características e os projetos voltados à terceira idade. Após esta revisão, a pesquisa se dividiu em duas etapas: a primeira consistiu na análise de livros de literatura expostos em livrarias e a possibilidade quanto ao seu aspecto gráfico-visual de serem lidos confortavelmente por pessoas de terceira idade e, na sequência, a percep-ção de leitores desta faixa etária sobre estes livros. Após estes levantamentos, a pesquisa buscou elementos para o desenvolvimento de projetos gráficos de livros voltados para o público de terceira idade e a realização de um protótipo. O protótipo foi apresentado aos leitores de terceira idade para responder ao problema da pesquisa. Como resultado, são indicados parâmetros para o projeto e design gráfico de livros voltados para os leitores de terceira idade. / This study analyzes the literature books about the adequacy of graphic design for elderly readers. We performed a literature review, covering aspects of graphic design and book design, the issues of reading, legibility, readability and typography. During this stage we have addressing the aging Brazilian population, the characteristics and projects aimed for old people. After this review, the research was divided into two stag-es: the first consisted in the analysis of literature books displayed in bookstores, and the possibility for seniors to read them comfortably on its appearance and visual graph, and following the perception of readers of this age about these books. After these surveys, the research sought to develop elements of graphic design of books aimed at the elderly and public performance of a prototype. The prototype was presented to senior players so that they answer to the research problem. As a result, parameters are indicated for the design and graphic design of books aimed to elderly readers.

Les croyances ASP dans le suicide : quelle spécificité du sujet âgé ? / The ASP beliefs in suicide : specificty of elderly people

Le suicide tue environ 10 000 personnes par an en France, dont 30% sont des personnes âgées. Encore tabou, il reste difficile d'aborder directement cette question auprès des personnes. Cette thèse, étayée par les conceptions cognitives de la dépression et des addictions, s'intéresse aux processus psychologiques en jeu dans la prise de décision de suicide. Elle vise également à pouvoir repérer certaines spécificités liées à l'âge. Nous pensons que certaines croyances joueraient un rôle au moment de passer à l'acte. Nous avons tenté de les identifier à partir d'un questionnaire créé dans cet objectif. Nous avons cherché à repérer si certaines seraient différentes ou plus présentes chez les aînés, expliquant, en partie, leur nombre aussi important de suicidés. Nous avons proposé un panel d'échelles validées (dépression, BDI, anxiété, STAI, désespoir, BHS) et l'Échelle d'Ambivalence Suicidaire que nous avons construite, à un échantillon de 274 sujets de 18 à 97 ans. Nos analyses statistiques ont montré que les sujets adhéraient différemment à certains types d'idées, en fonction de leur vécu, et de leur âge. / In France, the number of suicide deaths is estimated at around 10,000 per year, 30% corresponds to elderly people. Remaining a taboo, it is still a difficult question to directly address to the public. This thesis focuses on the depression and addiction cognitive conceptions that are involved in psychological processes related to the decision for the suicide act. It furhter aims at enabling the identification of some age-related particularities. We think that certain beliefs may play an important role in the actual moment of carrying out the act. We have attempted to identify them by using a questionnaire specifically designed for this purpose. We sought to determine whether some beliefs are different or more prevalent among elderly people, partly explaining their large number of suicides. A group of validated scales (Beck Depression Inventory, State-Trait Anxiety Inventory, , and Beck Hopelessness Scale) and the Suicidal Ambivalence Scale that we have developed were administered to a sample of 274 subjects aged 18 to 97 years old. Our statistical analysis showed that subjects adhered differently to certain kinds of beliefs, depending on their life experiences and age.

Trygg med trygghetslarm? : en kvalitativ studie av några ålderspensionärers erfarenheter och upplevelser av trygghetslarm

<p>The purpose of this study was to depict some elderly people’s experiences of personal safety alarms. Furthermore, the aim was to look into whether there is any difference in experience between men and women and depending on their level of care. Using a qualitative method, semi-structured interviews were carried out with eight elderly people; six women and two men. The main questions of the study were: How do the elderly people experience the practical use of personal safety alarms? In what ways, if any, do the elderly people experience that the personal safety alarm has changed their daily life? In what ways do the elderly people experience that the personal safety alarm effects their ability to stay in their own home? A cognitive perspective was mainly used to analyze the results. The results of the study showed that there were no significant differences between men and women’s experiences or considering their level of care. In the elderly people’s opinion, the personal safety alarm works well and affects their ability to remain in their own home. The personal safety alarm has not made any significant difference in the elderly people’s daily life, although some of them feel a sense of safety.</p>

Many Memories, Many Stories : Participatory Music Project for Elderly People with Dementia – Music Pedagogical Applications for Elderly Care

”Many  Stories,  Many  Memories” was a participatory creative music project carried out in collaboration with three professional musicians, a group of seven senior residents and the occupational therapist of “Suomikoti”-elderly home in Stockholm, February 20th – March 21st 2013 . The aim of the project was to build community feeling, participation and operation for the elderly people with dementia by intervening musically in their everyday lives. During the eight workshop-sessions, improvisation pieces were created by using song, text, fine arts, percussion instruments, body percussion, piano, kantele, and violin.The emphasized qualities of the project’s musical working methods were contextuality, and person-centered and focus group - oriented approaches. The purpose of multi-sensor exercises  was  to  support  the  participants’  sense  of  body  and reinforce their identity. The project, during which the musicians and the group of seniors met in the field of performing arts, was completed with a collectively composed semi-improvised concert, which was performed to an audience consisting of the residents and staff of Suomikoti, as well as family members.“Many  Memories,  Many  Stories”  was  my  Professional Integration Project (PIP) in the international Joint Music Master for New Audience and Innovative Practice – Master Degree Program (NAIP) at on the Royal College of Music in Stockholm. Observational periods at local Stockholm elderly homes as well as a preparatory project were conducted prior to the PIP-project. The goal of the PIP-project and this Master Thesis was to create new empiric data on the potentials of creative participatory music workshops for elderly care. This project’s musical intervention was carried out as practice-based research, and was documented session by session both in written reflections and on video for data- analysis. Semi-structured thematic interviews were also conducted for obtaining data. The interviewees were professional practitioners on the fields of music and health-care.The outcomes of the project reveal that intensive participation in the project had positive effects  on  the  people’s motor skills, creativity, expression, social interaction and self- esteem, which by enriching their everyday lives improve their general quality of life.Attached to this Master Thesis are two videos; a documentary-DVD describing the process of the project, and an edition of the”Many  Memories,  Many  Stories”-concert in full length.
