Gestörte Homöostase von Inflammation und Antiinflammation bei Risikopatienten nach Herzchirurgie

Strohmeyer, Jens-Christian 08 February 2006 (has links)
Basically, cardiac surgery involving cardiopulmonary bypass leads to systemic inflammation (SIRS) by the secretion of proinflammatory mediators. In order to limit its extend, systemic inflammation is associated with systemic counter-regulation (CARS), which, under some circumstances, may lead to high susceptibility of the organism to secondary infections. Septic disease is among the most common causes of death in surgical ICUs, the costs are estimated at several billion Euros per year. The early diagnosis in particular is of great importance clinically. Understanding of the immunologic principles is a basic assumption with regard to finding new markers of infection. This study was performed to determine whether the model "systemic immune activation - counter-regulation and immune depression - high susceptibility to infections" could be transferred to risk patients after cardiac surgery. In addition, a standardized immune monitoring program should be examined regarding its ability to predict infection in this patient population before clinical manifestation. It should also be determined if these new parameters have more discriminative power than conventional routine markers of infection (SIRS, CRP). The results show that this model can be confirmed in this patient collective. On the 1st postoperative day markers of immune activation, total-IL-8 (after lysis of erythrocytes), PCT and ex vivo elastase, as well as anti-inflammatory IL-10 in plasma and the marker of immune competence, HLA-DR on monocytes, have high discriminative potential to predict infections during the 6-day postoperative course. AUCs of the ROC were 0.75 for HLA-DR, 0.73 for total-IL-8, 0.72 for ex vivo elastase, 0.68 for both PCT and IL-10. On the other hand, conventional markers of infection were not able to discriminate significantly between patients with infection versus those without (CRP), or they only had an AUC of 0.66 (for 2 positive SIRS criteria). By using well-standardised laboratory methods (exact quantification of surface molecules, semi-automatic ELISA-system), which were used for the first time in such a patient collective, an important basis for clinical establishing these new markers was created, in addition to a better quantification of the immunologic unbalance (inflammation versus anti-inflammation). Thus, it is possible to supply early identificated risk patients for adjuvant therapy trials.

Tierexperimentelle Untersuchungen zur intestinalen Mikrozirkulation bei Endotoxinämie

Lehmann, Christian 17 July 2001 (has links)
The disturbance of the intestinal microcirculation is regarded as a pivotal mechanism in the development of multiorgan failure related to sepsis. Since the intestine is clinically not accessible for microcirculatory studies, the effects of a therapy with the antioxidants oxypurinol and U-74389G (lazaroid) as well as the vasoactive substances iloprost (a prostacyclin analogue) and dopexamine on the intestinal microcirculation and the systemic mediator release was studied in an animal model with moderate and high endotoxin challenge. The intravital microscopic examination of the capillary perfusion in the muscularis layer of the intestine during endotoxemia revealed an improvement by administration of oxypurinol and dopexamine. The perfusion of the mucosa could be increased by iloprost administration. The amount of the endotoxin induced, intestinal leukocyte adherence was especially decreased by the treatment with the antioxidants. Both therapeutic options caused a 60 % reduction in the initial tumor necrosis factor-alpha-release in the experiments with the lower endotoxin dose. Malondialdehyde analyses showed that oxypurinol and U-74389G reduced effectively the intestinal, radical-induced lipid peroxidation. The intestinal microvascular blood flow could be significantly increased by both vasoactive substances - as well as with moderately than also with elevated endotoxin-dosage. The results of the study confirmed that both reactive oxygen-species as well as an inadequate perfusion in the microcirculation represent essential pathogenetic factors during endotoxemia as well as sepsis and index corresponding therapy-forms and participates effective. A combined offering both substance-classes appears therefore meaningfully and should be evaluated in further experimental and clinical studies.

Fatigue in chronically critically ill patients following intensive care - reliability and validity of the multidimensional fatigue inventory (MFI-20)

Wintermann, Gloria-Beatrice, Rosendahl, Jenny, Weidner, Kerstin, Strauß, Bernhard, Hinz, Andreas, Petrowski, Katja 12 June 2018 (has links) (PDF)
Background Fatigue often occurs as long-term complication in chronically critically ill (CCI) patients after prolonged intensive care treatment. The Multidimensional Fatigue Inventory (MFI-20) has been established as valid instrument to measure fatigue in a wide range of medical illnesses. Regarding the measurement of fatigue in CCI patients, the psychometric properties of the MFI-20 have not been investigated so far. Thus, the present study examines reliability and validity of the MFI-20 in CCI patients. Methods A convenience sample of n = 195 patients with Critical Illness Polyneuropathy (CIP) or Myopathy (CIM) were recruited via personal contact within four weeks (t1) following the transfer from acute care ICU to post-acute ICU at a large rehabilitation hospital. N = 113 (median age 61.1 yrs., 72.6% men) patients were again contacted via telephone three (t2) and six (t3) months following the transfer to post-acute ICU. The MFI-20, the Euro-Quality of Life (EQ-5D-3 L) and the Structured Clinical Interview for the Diagnostic and Statistical Manual of mental disorders DSM-IV (SCID-I) were applied within this prospective cohort study. Results The internal consistency Cronbach’s α was adequate for the MFI-total and all but the subscale Reduced Motivation (RM) (range: .50–.91). Item-to-total correlations (range: .22–.80) indicated item redundancy for the subscale RM. Confirmatory Factor analyses (CFAs) revealed poor model fit for the original 5-factor model of the MFI-20 (t2/t3, Confirmatory Fit Index, CFI = .783/ .834; Tucker-Lewis Index, TLI = .751/ .809; Root Mean Square Error of Approximation, RMSEA = .112/ .103). Among the alternative models (1-, 2-, 3-factor models), the data best fit to a 3-factor solution summarizing the highly correlated factors General −/ Physical Fatigue/ Reduced Activity (GF/ PF/ RA) (t2/ t3, CFI = .878/ .896, TLI = .846/ .869, RMSEA = .089/ .085, 90% Confidence Interval .073–.104/ .066–.104). The MFI-total score significantly correlated with the health-related quality of life (range: −.65-(−).66) and the diagnosis of major depression (range: .27–.37). Conclusions In the present sample of CCI patients, a reliable and valid factor structure of the MFI-20 could not be ascertained. Especially the subscale RM should be revised. Since the factors GF, PF and RA cannot be separated from each other and the unclear factorial structure in the present sample of CCI patients, the MFI-20 is not recommended for use in this context.

Cellules suppressives d'origine myéloïde au cours du sepsis / Myeloid-derived suppressor cells in septic patients

Uhel, Fabrice 19 May 2016 (has links)
Sepsis results in a sustained immune dysfunction responsible for poor prognosis and nosocomial infections. Sepsis physiology remains poorly understood and no treatment exists currently, excepted from antibiotherapy and life-support techniques. We asked if myeloid cells could play a role in this sustained immune dysfunction. We demonstrated that Peripheral CD14+HLA-DRlow/- monocytic-myeloid-derived suppressor cells (MDSCs) and CD14-CD15+ low-density granulocytes identified as granulocytic- (G-)MDSCs were increased in septic patients. In vitro, arginase and IDO activities relied on MDSCs and depletion of both subsets restored T-cell proliferation. The initial proportion of G-MDSC predicted occurrence of nosocomial infections. Similarly, high plasma Indoleamine 2,3-dioxygenase (IDO) activity and arginase 1 level were associated with poor outcome. Altogether, our results demonstrate that myeloid cells acquire suppressive functions during sepsis, partially responsible for the sustained immune dysfunction and poor outcome. MDSCs may become a future therapeutic target to restore the immune capacities of septic patients.

Analysis of cerebral and respiratory activity in neonatal intensive care units for the assessment of maturation and infection in the early premature infant / Analyse des signaux issus des unités de soins intensifs néonatales pour l'étude de la maturité, de l'infection généralisée et de l'influence de l'immunisation chez le nouveau-né prématuré

Navarro, Xavier 22 October 2013 (has links)
This Ph.D. dissertation processes and analyzes signals from the neonatal intensive care units (NICUs) for the study of maturity, systemic infection (sepsis) and the influence of immunization in the premature newborn. A special attention is payed to the electroencephalography and the breathing signal. The former is often contaminated by several sources of noise, thus methods based on the signals decomposition and optimal noise cancellation, adapted to the characteristics of the immature EEG, were proposed and evaluated objectively on real and simulated signals. By means of the EEG and delta burst analysis, detected automatically by a proposed classifier, infant's maturation and the effects of vaccination are studied. Concerning the second signal, breathing, non-linear and fractal methods are adapted to evaluate maturity and sepsis. A robustness study of estimation methods is also conducted, showing that the Hurst exponent, estimated on respiratory variability signals, is a good detector of infection.

Predictors of posttraumatic stress and quality of life in family members of chronically critically ill patients after intensive care

Wintermann, Gloria-Beatrice, Weidner, Kerstin, Strauss, Bernhard, Rosendahl, Jenny, Petrowski, Katja 16 January 2017 (has links) (PDF)
BACKGROUND: Prolonged mechanical ventilation for acute medical conditions increases the risk of chronic critical illness (CCI). Close family members are confronted with the life-threatening condition of the CCI patients and are prone to develop posttraumatic stress disorder affecting their health-related quality of life (HRQL). Main aim of the present study was to investigate patient- and family-related risk factors for posttraumatic stress and decreased HRQL in family members of CCI patients. METHODS: In a cross-sectional design nested within a prospective longitudinal cohort study, posttraumatic stress symptoms and quality of life were assessed in family members of CCI patients (n = 83, aged between 18 and 72 years) up to 6 months after transfer from ICU at acute care hospital to post-acute rehabilitation. Patients admitted a large rehabilitation hospital for ventilator weaning. The Posttraumatic Stress Scale-10 and the Euro-Quality of life-5D-3L were applied in both patients and their family members via telephone interview. RESULTS: A significant proportion of CCI patients and their family members (14.5 and 15.7 %, respectively) showed clinically relevant scores of posttraumatic stress. Both CCI patients and family members reported poorer HRQL than a normative sample. Factors independently associated with posttraumatic stress in family members were the time following ICU discharge (β = .256, 95 % confidence interval .053-.470) and the patients\' diagnosis of PTSD (β = .264, 95 % confidence interval .045-.453). Perceived satisfaction with the relationship turned out to be a protective factor for posttraumatic stress in family members of CCI patients (β = -.231, 95 % confidence interval -.423 to -.015). Regarding HRQL in family members, patients\' acute posttraumatic stress at ICU (β = -.290, 95 % confidence interval -.360 to -.088) and their own posttraumatic stress 3 to 6 months post-transfer (β = -.622, 95 % confidence interval -.640 to -.358) turned out to be significant predictors. CONCLUSIONS: Posttraumatic stress and HRQL should be routinely assessed in family members of CCI patients at regular intervals starting early at ICU. Preventive family-centered interventions are needed to improve posttraumatic stress and HRQL in both patients and their family members.

Méthodologies d’évaluation de l’optimalité des soins : exemples des délais diagnostiques et des infections bactériennes sévères de l’enfant / Methods to assess the optimality of care : examples of time to diagnosis and serious bacterial infection in children

Launay, Elise 27 November 2015 (has links)
The aim of this thesis was to product new knowledge about the methodology on how to assess the optimality of care with the examples of time to diagnosis and serious bacterial infection (SBI). In two systematic reviews, we found that the key methodological points potentially related to risks of bias or threats to transportability were rarely reported in the primary studies and rarely evaluated by authors of systematic reviews. Then, we developed and internationally validated a reporting guideline to help scientists to better take into consideration these critical methodological points. In a population-based confidential inquiry, we found that: (i) care was suboptimal in 76% of the initial management of children who died from SBI, (ii) delayed first medical contact, undervaluation of severity or delayed antibiotic administration were detected in the management of 20%, 20% and 24% of children admitted to intensive care for a SBI, respectively, (iii) the total number of suboptimal cares delivered during the management was independently associated with death, and (iv) suboptimal cares were more frequent in children younger than one year old and if the care was delivered by a non specialist physician. Minimizing the risks of bias both in the selection process of the study population and in the assessment of the optimality of care, and taking into account confounding factors such as the intrinsic severity of the disease are keys elements to ensure a reliable evaluation of optimality of care in order to produce effective corrective clinical messages.
