Sickness presenteeism among nurses

This thesis is the main product of a Masters degree in Health Science, written by a student at the Department of Social Work and Health Science at Norwegian University of Science and Technology. The thesis discusses sickness presenteeism in general and sickness presenteeism among nurses in particular. Nursing is one of the occupations that are more exposed to sickness presenteeism. Despite this, very little research has yet been done on this correlation – in Norway, almost none. The thesis consists of two articles. The first article presents the concept of sickness presenteeism along with existing theories and research in general and concerning nurses. This is a theoretical article, in which the purpose is to identify why sickness presenteeism is not an ideal situation for nurses and which work-related factors that influence sickness presenteeism among nurses. The research question for this article is “What work-environmental factors have an influence on nurses sickness presenteeisem?” The second article is an empirical article and is done in connection with a survey of nurses’ sickness presenteeism within a Norwegian hospital. The survey examines the connection between sickness presenteeism and the use of substitutes, different working time arrangements and the influence of teamwork. The purpose of this article is to assess how the three work-environmental factors influences nurses’ sickness presenteeism, conducted with the help of three hypotheses. These are: H1: Working-time arrangements have an influence on sickness presenteeism H2: Low extent of using substitutes is related to higher level of sickness presenteeism H3: Good working relationship between co-workers is related to higher level of sickness presenteeism The empirical work for this master’s thesis has been conducted using a quantitative method. This method was chosen based on a desire to investigate the extent, distribution and differences within sickness presenteeism among nurses. A quantitative method gives an opportunity to investigate a larger group of nurses, and obtain results that can also be valid for other nurses. If a qualitative method were to be used, it would not have been possible to collect information from the same number of nurses and the results would most likely not represent nurses in general because of assumed differences between nurses. By using a qualitative method I was also able to compare the results with results from similar studies done in other countries. Still, the quantitative method has little room for a flexible approach to theory, and does not provide information about what might cause the sickness presenteeism. I still found this method to be the most suitable for what I wanted to achieve in this master’s thesis. A survey based on questions used by other researchers with an interest in the same field, was used to collect data. Using questions that others had already used made the results easier to compare, and also acted as some kind of quality control to the questions I first developed. The choices made concerning the survey, items and analysis is further described in chapter 2.0 Method of my empirical article. The strength and limitations of the method is discussed in chapter 4.1 Strengths and limitations of that same article.

Media, trade unions and politicians have during the last years described social workers untenable work situation, and research shows that social workers are using strategies as sickness presence to deal with the imbalance between demands and resources. There is an interest in deepening knowledge of the phenomenon of sickness presence among social workers and to analyze the underlying causes that lead to the fact that social workers attend work while they are sick. The purpose of my study is to broaden knowledge for sickness presenteeism among social workers. I wanted to contribute to a better understanding of sickness presenteeism as phenomenon by analyzing social workers experience and wanted my study to focus on detailed descriptions of situations, events and feelings. Therefore I used a qualitative approach and based my study on individual interviews with 11 social workers. The social workers who chose to participate in the interviews are all women. I found several common strategies in the social workers descriptions, which were organized into three themes: loyalty, economy and resignation. The analysis was based on two different kinds of perspectives: one focusing on power and the other focusing on gender. The result shows that sickness presenteeism among social workers can be understood as a result of their attempt to live up to the prevailing male norms in society through paid work and to support the family, while they at the same time is expected to live up to the traditional image of ”a good girl" who prioritize the needs of others before her own health. The needs of others can appear in the form of, for example; clients, colleagues, family finances or the organization.

Syftet med föreliggande enkätstudie, genomförd på tre arbetsplatser i Mellansverige (N = 76), var att undersöka sjuknärvarons samband med villkorlig självkänsla, baserad på prestationer eller relationer, samt med olika yttre faktorer. Även sjukfrånvaro, som visat sig vara nära knuten till sjuknärvaro, samt självskattad hälsa, beaktades. Resultatet visade att sjuknärvaro inte var relaterad till någon typ av villkorlig självkänsla och av yttre faktorer var det endast upplevelsen av arbetsplatsen som en viktig social funktion som visade ett signifikant samband med sjuknärvaro. Låg självskattad hälsa däremot var relaterad till sjuknärvaro samt prestationsbaserad självkänsla. Framtida forskning bör vidare undersöka den villkorliga självkänslans relation till sjuknärvaro.</p>

Varför går folk till jobbet när de är sjuka? : Exemplet gymnasielärarna

The aim of this essay is to understand the social mechanisms for why people go to work when they are sick. To understand it, we investigated why upper secondary school teachers sickness presenteeism increased in the last twenty years. Data consists of interviews with eleven upper secondary school teachers and two principals, plus statistics. To find patterns in the data we examined the theory of professions and the theory of the new capitalist culture. School budgets are based on support from the government that provides money for each student. Since 1992 free schools are allowed in Sweden. This creates a competition between schools, which may lead to programs in schools with insufficient applicant students to be shut down. The results show that the market's growing influence in schools has given upper secondary school teachers an increased social function. The customer, in this case the students and parents, has become more at the center. This new task, the student caring role, forces the teacher to be more accessible and flexible which has made their profession weaker, giving them more stress and in a longer term, they are feeling sick more often. A combination between a striving to keep up their profession and to not losing students, teachers go to school even if they’re sick.

Sjuk men kommer till jobbet? : -samverkan mellan individuella och organisatoriska faktorer bakom sjuknärvaro / Sick but go to work? : individual and organisational factors' interplay behind Sickness Presenteeism

Sjuknärvaro är ett globalt fenomen som har negativa följder för individer och organisationer. Under Covid - 19 pandemin har även konsekvenser som smittspridning på grund av sjuknärvaro aktualiserats. Studien syftade till att undersöka hur anställdas attityder, säkerhetsspecifikt ledarskap, anställningsotrygghet, personliga ekonomiska svårigheter och anställningsform kan tänkas leda till anställdas sjuknärvaro. Enkätundersökning (N = 177) visade att normativa attityder till sjuknärvaro kan leda till starkare intentioner för att komma till arbetet trots sjukdom. Emellertid visade studien att det inte fanns något samband mellan individers attityder till smittorisker med Covid -19 på deras arbetsplats och intentioner för sjuknärvaro. Inte heller personliga ekonomiska svårigheter eller anställningsform hade någon relation med intentioner för sjuknärvaro. Anställningsotrygghet var positivt associerad med normativa attityder medan säkerhetsspecifikt ledarskap hade en negativ relation med upplevelser av smittorisker.

Sjuknärvaro vid hemarbete : En kvalitativ studie om arbetstagares upplevelse av sjuknärvaro vid hemarbete / Sickness presenteeism while working from home

I och med de restriktioner som Covid-19-pandemin medförde blev hemarbete ett nytt arbetssätt för många arbetstagare i Sverige. Forskning som vi tagit del av beskriver att sjukfrånvaro i organisationer minskar samtidigt som mörkertalet gällande sjuknärvaro är stort, vilket skapade intresset för denna studie. Forskning kring sjuknärvaro fokuserar på den fysiska arbetsplatsen och inte vid hemarbete som nu är ett vedertaget arbetssätt i Sverige. Denna studie syftar till att skapa förståelse för hur arbetstagare upplever sjuknärvaro vid hemarbete samt hur den upplevda arbetskvaliteten påverkar valet att sjuknärvara. Samtliga arbetstagare inom en organisation som tillhandahåller både produkter och tjänster inom den privata sektorn ombads besvara en enkät och åtta respondenter har intervjuats i semistrukturerade intervjuer. Samtliga intervjuer har tematiserats och analyserats vilket har lett fram till resultatet av denna studie. Resultatet visar att sjuknärvaro i hemarbete förekommer i hög grad och tillgängligheten som arbetssättet innebär ökar anställdas val att sjuknärvara. Valet att sjuknärvara tas speciellt då arbetstagarna upplever hög arbetsbelastning, lojalitet mot kollegor samt arbetsgivare och ur ekonomiska aspekter. Arbetstagare upplever att sjuknärvaron har en negativ påverkan på effektiviteten då arbetstempot inte sker i samma takt, samt fler pauser behövs för att klara av att arbeta sjuk vid hemarbete. Genom att arbeta med arbetskvalitet och tillgodose de behov arbetstagare har för att uppleva en god arbetskvalitet menar författarna av denna studie att sjuknärvaro kan förebyggas. / With the restrictions imposed by Covid-19-pandemic, work from home (WFH) became a new method of working for many employees in Sweden. The research we have found describes that sickness absence in organizations is decreasing, but the prevalence of sickness presenteeism is still unknown, which created the interest for our study. Research on sickness presenteeism focuses on the traditional physical workplace and not WFH, which is now an established method in Sweden as a result of the Covid-19-pandemic. This study aims to understand how workers experience sickness presenteeism when working from home and how the perceived quality of work affects the decision to choose such presenteeism. A survey was sent out to all workers in an organization in the private sector, which provides both products and services and eight respondents were interviewed in semi-structured interviews. All the interviews were then thematized and analyzed, leading to the results of this study. The results show that sickness presenteeism in WFH occurs to a high degree, and is perceived by workers as common due to the accessibility of the working method. The choice to work with sickness presenteeism is made especially when employees experience high workload, loyalty to colleagues and employers and from economic aspects. Employees feel that sickness presenteeism has a negative effect on efficiency as they do not work at the same pace, and need to take more breaks to cope with working from home while sick. By working on quality of work and meeting employees' needs to experience good quality of work,the authors of this study argue that sickness presenteeism can be prevented.

Nous avons engagé ce processus doctoral à travers le mémoire propédeutique sur l’absentéisme dans les entreprises industrielles au Mali. Cette étude sur l’absentéisme nous a permis de constater le présentéisme des femmes salariées. A la demande de la Directrice de thèse Pascale de ROZARIO, nous avons focalisé la thèse sur ce sujet. Son aboutissement a permis de constituer quatre idéaux-types de femme salariée présentéiste selon le modèle de Max Weber: *La femme salariée présentéiste de type traditionnel dont les présences régulières et continues au travail sont influencées par les cérémonies de funérailles. *La femme salariée présentéiste de type affectif dont les comportements présentéistes sont essentiellement fondés par les soutiens des collègues et de la hiérarchie. *La femme salariée présentéiste de valeur dont le présentéisme est justifié par les soutiens multiformes de ses proches et ses soutiens financiers à ces derniers. *La femme salariée présentéiste de type rationnel qui anticipe les sanctions disciplinaires et pécuniaires en privilégiant le respect des textes règlementaires. / We began this PhD research on the topic of absenteeism in industrial plants of Mali. This study on absenteeism allowed me to mention the regular presence of some women at work. That is why; the PhD Director Pascale de Rozario recommended focusing our research on this topic. The implementation and processing of data in liaise with the theoretical model of Max Weber allowed to identify four ideal types of presenteeist employee women: The traditional presenteeist woman influenced by funerals and burials. The affective presenteeist woman influenced by colleagues and managers’ support. The valued presenteeist woman whose behavior is justified by relatives support and her financial contribution towards those relatives. The rationale presenteeist influenced by standing orders and financial punishments resorting to respect of statutory acts.
