Decisão pela extração dentária e incidência de cáries e perdas dentárias em idosos da coorte de Carlos Barbosa, RS : perspectivas qualitativa e quantitativa

Introdução: A perda dentária e o edentulismo podem afetar de maneira significativa a saúde bucal e a saúde geral, a qualidade de vida, e a nutrição de idosos. A cárie e a perda dentária têm sido descritas como o resultado de processos que envolvem elementos biológicos, nas superfícies dentárias, e de uma combinação de fatores comportamentais e sociais. Além disso, o contexto histórico no qual os indivíduos estão inseridos influencia seus comportamentos e percepções em relação à saúde bucal. Uma compreensão acerca de comportamentos de saúde demanda a utilização de métodos que permitam explorar a realidade subjetiva destes fatores. Objetivo: Os objetivos deste estudo de métodos quantitativo e qualitativo foram avaliar a associação entre fatores demográficos, socioeconômicos, comportamentais e de saúde e a incidência de Cárie Coronária, Cárie Radicular, e Perda Dentária na amostra estudada, e realizar entrevistas de grupo focal com membros desta amostra para explorar suas percepções acerca da perda dentária. Métodos: Um estudo de coorte foi conduzido com uma amostra aleatória simples de indivíduos com 60 anos ou mais em Carlos Barbosa, RS, entre 2004 e 2008. Entrevistas e exames bucais foram conduzidos com 388 indivíduos dentados na linha de base, e com 273 participantes no seguimento. A incidência de perdas dentárias, cáries coronárias e radiculares e suas associações com variáveis da linha de base foram modeladas com o uso de regressão binomial negativa. Em um segundo momento, informações de natureza qualitativa foram produzidas através de uma séria de oito entrevistas de grupo focal envolvendo 41 idosos, selecionados dos participantes do seguimento do estudo. As discussões de grupo foram moderadas pelo autor, foram gravadas, codificadas e analisadas através do uso da Teoria Fundamentada em Dados. Resultados: Foi observado no modelo multivariável que, ser mais velho; do sexo masculino; viver em área rural; ser casado; ter menor escolaridade; ser fumante; e estar insatisfeito com os serviços de saúde acessados, estiveram associados com a incidência perdas dentárias. Entre as variáveis clínicas, o uso de próteses parciais removíveis e o índice de sangramento gengival foram preditores, enquanto maior taxa de fluxo salivar foi um fator protetor para o desfecho. Com relação às cáries coronárias, no modelo multivariável final foi observado que, ser mais velho; do sexo masculino; viver em área rural; e ser fumante, estiveram associados à sua incidência. Entre as variáveis clínicas, o uso de próteses parciais removíveis foi um preditor, enquanto maior taxa de fluxo salivar foi um fator de proteção para este desfecho. No modelo multivariável final tendo como desfecho a densidade de incidência de cáries radiculares, ser mais velho; viver em área rural; e escovar os dentes com frequência menor do que diária, foram preditores; ao passo que a única variável clínica associada ao desfecho foi uma maior taxa de fluxo salivar, como fator de proteção. No segmento qualitativo, as respostas dos participantes permitiram concluir que as perdas dentárias estiveram relacionadas com a falta de programas de saúde bucal; normas sociais (incluindo valores relativos ao gênero); nível limitado de informações de saúde; e reduzidas acessibilidade e disponibilidade de serviços odontológicos. Contextos de vulnerabilidade social e programática tiveram um papel fundamental no desenvolvimento de normas e valores sociais e comportamentos individuais que resultaram em extrações dentárias. Conclusão: Estes achados são importantes, porque compreender as relações causais entre práticas individuais como higiene bucal, hábito de fumar, e procura por cuidados odontológicos preventivos, e perdas dentárias, não elucida as razões para as pessoas assumirem tais comportamentos. / Introduction: Tooth loss and edentulism may have a negative impact in the oral and general health, in the quality of life, and nutrition of the elderly. Dental caries and tooth loss have been described as results of processes involving biologic factors, on the surfaces of the teeth, and of a combination of behavioral and social factors. Furthermore, the historical context in which people are placed influences their behaviors and perceptions towards oral health. The understanding of health behaviors requires the use of methods which allow for the exploration of the subjective reality of those factors. Objective: The objectives of this study using quantitative and qualitative methods were to evaluate the association between demographic, socioeconomic, behavioral and health factors and the incidence of tooth loss, coronal and root caries in the studied sample. Also, to carry on focus groups interviews with members from the sample to explore their perceptions about tooth loss. Methods: A cohort study was conducted with a simple random sample of subjects 60 years old or older in Carlos Barbosa city, southern Brazil, between the years of 2004 and 2008. Interviews and oral examinations were conducted with 388 dentate subjects at baseline, and 273 participants at follow-up. The incidence of tooth loss, coronal and root caries and their association with baseline variables were modeled with the use of negative binomial regression. Following the quantitative part of the study, information of qualitative nature was obtained through a series of eight focus groups, involving 41 participants, purposefully selected from those participating in the follow-up. The focus groups discussions’ were moderated by the author, were recorded, coded and analyzed through the use of Grounded Theory. Results: In the multivariate model it was observed that, older age; male gender; living in a rural area; being married; less schooling; being a current smoker; and being dissatisfied with the health services accessed, were associated with the incidence of tooth loss. Among clinical variables, the use of removable partial dentures, and the gingival bleeding index were predictors, whereas higher stimulated saliva flow rate showed a protective effect for the outcome. With regards to the incidence of coronal caries, in the multivariate model it was found that, older age; male gender; living in a rural area; and being a current smoker, were associated with the outcome. Among clinical variables, the use of removable partial dentures was a predictor, whereas a higher stimulated saliva flow was a protective factor for the outcome. In the final model, for the outcome defined as the incidence density of new root caries, it was observed that, older age; living in a rural area; and brushing less than daily were predictors, whereas a higher stimulated saliva flow rate showed a protective effect for the outcome. With regards to the qualitative part of the study, the answers of the participants allowed for the conclusion that tooth extractions had been related, in the past, with the lack of oral health problems; social norms (including social values related to gender); low level of health information; and limited accessibility and availability of dental services. Contexts of social and programmatic vulnerability played a major role in the development of social norms and individual behaviors which resulted in tooth extractions. Conclusion: These findings are important, because understanding the causal relations between individual behaviors such as oral hygiene, smoking, and the pursuing of preventive dental care, do not elucidate why people engage in such behaviors.

Correlação entre doença aterosclerótica, dieta hipercolesterolêmica e as perdas dentais, estudo em modelo animal / Relationship between atherosclerosis, hypercholesterolemic fat diet and tooth loss: Study in animal model

O objetivo deste trabalho foi de avaliar em Modelo animal, coelhos (raça Nova Zelândia) divididos em três grupos randomizados, (jovem com 60 dias (G1), um idoso com aterosclerose e ingestão de colesterol (G2) e um idoso com aterosclerose e sem ingestão do colesterol (G3)) se, a dieta rica em colesterol e a idade, causam lesões de aterosclerose e placas ateroscleróticas nos animais, alterações nos comprimentos dos dentes, aumento ou diminuição dos espaços periapicais dos dentes, perda óssea alveolar na maxila e mandíbula. Através da metodologia descrita e após as análises histológicas e morfológicas, verificou-se diferença estatisticamente significante, nas variáveis dos comprimentos dos dentes 1º prémolares superiores entre os grupos G3 versus G1 p<5%. médias de 1247,88 (p=0,017) e G3 versus G2, com diferença das médias de 1190,85 (p=0,025) ou seja o comprimento dos dentes fora diferente no grupo G2. Com relação à variável, espessura do osso alveolar, não ocorreu significância estatística, porém tendências de que este esteja sendo alterado. Com relação a variável espessura do espaço periapical fora verificado significância estatística com p=0,017 em relação ao G1, na região dos 1º Pré-molares, apresentando a diferença das médias (403,42) a favor do G2, ou seja aumento do espaço periapical. A variável da área de placa de aterosclerose, correlacionada, com os comprimentos dos dentes e espessura do osso alveolar, também teve resultado significante na região 1º e 2º pré-molares entre G2 e G3. com p=0,025 e r=0,476. Confirmando assim a correlação da aterosclerose, dietas hiperclesterolêmicas e as alterações da cavidade bucal e estrutura de sustentação dos dentes. / The aim of this study is, to evaluate on animal model (New Zealand rabbits), on different and randomized groups (G1 a young group, G2 an older rabbit, with atherosclerosis and ingesting hipercholesterolemic fat diet and another group G3, just an older group) investigated than the hipercholesterolemic fat diet induced atherosclerotic lesions and plaques in ascendant aorta, bone loss in maxillary and jaw, alterations on length of the tooth, alterations on tooth periapical spaces. By the methodology described and aplicated, before the histological and morphological analysis, was verified significant statistical variance in the length of the first premolar tooth and second premolar on maxillary region, the length of this tooth is reduced in G2 (p<5%). The medias are 1247,88 (p=0,017) and 1190,85 (p=0,025). Analyzing another variable on the alveolar bone thickness, was not found any statistical significance, but tendencies that this event may occur. According to another variable of the thickness found on the periapical space, it was verified a significant variable statistic such as p=0,017 related on G1, on first premolar region, suggesting a different statistic, between medias, favoring G2 meaning the increase of periapical space. The variable on the atherosclerosis plaque area, related to the teeth length and alveolar bone thickness, also showed significant results on first and second premolars regions between G2 and G3, with p=0,025 and r=0,476. That confirms the atherosclerosis correlations and hipercholestrolemic fat diet, also alterations in oral cavity and teeth implantation structure.

Objetivo: Asociar la calidad de vida relacionada a la salud oral con la pérdida prematura de dientes deciduos en preescolares de 3 a 5 años de una institución privada en Lima, Perú. Materiales y métodos: El diseño del estudio fue de tipo transversal. La muestra estuvo constituida por 142 alumnos entre las edades de 3 a 5 años. Para el análisis de la calidad de vida relacionada a la salud oral (CVRSO), se utilizó el cuestionario Early Childhood Oral Health Impact Scale (ECOHIS) desarrollado por los padres de familia. Asimismo, para determinar la pérdida prematura de dientes deciduos, se usaron los parámetros de la American Dental Association (ADA) y las características de la gingiva. Para la asociación de CVRSO y pérdida prematura se empleó el análisis multivariado mediante la regresión binomial negativa. Se estableció el nivel de significancia como p<0.05 y un IC 95%. Resultados: Se encontró un promedio de 7.70 ± 8.43 al analizar la CVRSO, y la prevalencia de pérdida prematura fue de 10.56 %. Con respecto a la pérdida prematura de dientes deciduos, no existe probabilidad de que la calidad de vida se vea afectada, ya que a su presencia, la puntuación de la CVRSO aumenta en RTI 0.75 (IC 95% 0.26 -2.15). Conclusiones: Se observó que la CVRSO no se encuentra asociada con la pérdida prematura. Sin embargo, si existen diferencias en los puntajes de CVRSO al comparar los grupos de presencia y ausencia de pérdida prematura. / Objective: To associate the quality of life related to oral health with the premature loss of deciduous teeth in pre-schoolers from 3 to 5 years from a private institution in Lima, Peru. Materials and methods: The study design was cross-sectional. 142 students between the ages of 3 to 5 years were evaluated. For the analysis of Oral Health-related Quality of Life (OHRQL), the questionnaire Early Childhood Oral Health Impact Scale (ECOHIS) developed by the parents was used. Similarly, the parameters of the American Dental Association (ADA) and the characteristics of the gums were used to determine the premature loss of deciduous teeth. For the association of CVRSO and premature loss, the multivariate analysis, binomial negative regression, was used. The level of significance was established as p <0.05 and a IC 95%. Results: An average of 7.70 ± 8.43 was found when analysing the OHRQL, and the prevalence of premature loss was 10.56%. Regarding the premature loss of deciduous teeth, there is no probability that the quality of life will be affected, since in its presence, the CVRSO score increases by IRR 0.75 (95% CI 0.26 -2.15). Conclusions: It was observed that the OHRQL was not associated with the premature loss of deciduous teeth. However, there is a difference in the OHRQL scores when comparing the groups of presence and absence of premature loss. / Tesis

Survival Analysis of Endodontically Treated Teeth in Patients with Diabetes and Hypertension within National Dental PBRN Practices

Indiana University-Purdue University Indianapolis (IUPUI) / Introduction: The prevalence of diabetes mellitus (DM) is rapidly increasing among the aging United States population. This poses a challenge to dental providers since DM and multiple oral conditions have been identified as comorbidities. Hypertension (HTN) is associated with more poorly controlled DM and has been identified as contributing to RCT tooth loss in prior studies. Links have also been established between DM and the survival rate of root canal treated teeth, however, previous research has focused on institutional settings despite the majority of RCT being performed in private dental practices. This study will use data from private dental practices to evaluate the survival rate of RCT teeth in patients with DM and HTN. Materials and Methods: This retrospective study evaluated the survival rate of endodontic treated teeth among patients with DM and HTN using National Dental PBRN Practice data. Electronic dental records from 42 private dental practices in the United States over a period of 15 years with a minimum 2-year follow-up comprising 11,532 root canal treated teeth were analyzed. Kaplan-Meier survival curves were used to demonstrate the effects of HTN and DM on RCT tooth survival and Cox proportional hazards survival analysis was used to evaluate the DM and HTN effects after accounting for age, gender, insurance, year of treatment, tooth type, and crown and filling placement as covariates. Results: Patients with HTN only had significantly lower risk of failure than patients with both HTN and DM (p=0.003). Patients with neither HTN nor DM had significantly lower risk of failure than patients with both HTN and DM (p=0.020). Patients with DM only did not have significantly different risk of failure than patients with both HTN and DM (p=0.223). Patients with DM only did not have significantly different risk of failure than patients with HTN only (p=0.361). Patients with neither HTN nor DM did not have significantly different risk of failure than patients with HTN only (p=0.121) or patients with DM only (p=0.800). Conclusions: Patients with both DM and HTN have an increased chance of root canal treated tooth failure while patients with only DM or only HTN do not. Evaluation of severity of DM may be more important in determining RCT failure and studies utilizing laboratory values should be considered for future research.

Vad tänder? : Tandhälsa hos medeltida individer i S:t Hans, Visby / What's the tooth? : Dental health in medieval individuals from St. Hans, Visby

Uppsatsen har analyserat tandhälsan hos 20 vuxna individer med tandsten från medeltida S:t Hans, Visby, med fokus på: tandsten, karies, abscesser, parodontit, emaljhypoplasi och ante mortem tandförlust. I syfte att dra slutsatser om populationens tandhälsa och huruvida tandsten korrelerar med övrig tandhälsa. Resultatet påvisade en population med bristande tandhälsa, där 75 % av individerna hade minst ett kariesangrepp och 19 av 20 hade parodontit. Tandhälsan lär ha resulterat i tandvärk och komplikationer av allmänhälsan för S:t Hans populationen. Varken en negativ eller positiv korrelation mellan karies och tandsten kunde bevisas i materialet. Dock konstaterades att män hade en högre tandstensgrad, medan kvinnor påvisade grövre karies. Det leder till en tolkning om könsskillnader i diet i S:t Hans. Emaljhypoplasier är funna främst hos män. Det underbygger diettolkningen, samt tolkas som en indikation på könsbunden barndoms-mortalitet. Jämförelser med samtida befolkningar utfördes för att ge en vidare inblick i materialet; S:t Hans hade statistiskt fler individer med karies samt mer parodontit. Det kan innebära olika diet på fastlandet och Visby, eller indikera att tandsten leder till försämrad tand-hälsa då samtliga analyserade individer har tandsten. / The thesis focuses on the dental health of 20 adult individuals with dental calculus from medieval St. Hans, Visby. The dental health assessments were done with a focus on dental calculus, caries, dental abscesses, periodontitis, enamel hypoplasia and ante mortem tooth loss. The result of the thesis showed a lacking dental health due to a great amount of caries, 75 % of analysed individuals had at least one instance of caries, as well as periodontitis which was found in 19 out of 20 individuals. No clear negative nor positive correlation between dental calculus and caries could be seen in the material. However, men had a higher degree of dental calculus whilst women had more severe cases of caries. This leads to a possible conclusion of differing diets between the sexes. Enamel hypoplasia is found mainly in men, which supports the theory regarding diet and is posed as an indication of sex-bound childhood mortality. St. Hans popu-lation had a greater statistic of individuals with caries than contemporary Swedish populations as well as more periodontitis. This could indicate a bad dental hygiene and dental health amongst the individuals from St. Hans or it could indicate that dental calculus leads to worsened dental health since only individuals with dental calculus were analysed.

La santé orale compromise comme facteur de risque potentiel pour le cancer colorectal sporadique (COLDENT) : une étude de faisabilité

Introduction: Le cancer colorectal (CCR) demeure un véritable fardeau de santé publique au Canada. Malgré les énormes avancés scientifiques, l’étiologie du CCR n’est pas encore clairement définie. La prévention du CCR nécessite l’identification de nouveaux facteurs de risque. Durant ces dernières années, des études épidémiologiques ont rapporté l’association entre la santé buccodentaire et le cancer colorectal. Cependant, aucune étude n’a été conçue spécifiquement pour vérifier cette association. Objectifs : L’objectif principal de l’étude était de tester la faisabilité d’une grande étude épidémiologique ultérieure, conçue pour vérifier l’association entre la santé buccodentaire compromise et le risque de CCR. Les objectifs secondaires étaient de standardiser les procédures de l’étude, d’évaluer le taux de participation et le succès des approches proposées pour le recrutement, d’évaluer la faisabilité et la validité des méthodes de la collecte des données sur la santé buccodentaire et de produire des données préliminaires concernant l’association entre la santé buccodentaire compromise et le risque de CCR. Méthodologie: Il s’agit d’une étude de faisabilité, de devis cas-témoins à base populationnelle. Les participants dans le groupe de cas (n=30) ont été recrutés dans deux hôpitaux de Montréal. Les témoins (n=39) ont été recrutés dans la population générale de Montréal. Les données sur la santé buccodentaire ont été collectées aussi bien par un questionnaire sur la santé buccodentaire que par un examen clinique. Les données sur plusieurs facteurs de risque du CCR ont été collectées par des questionnaires validés. Résultats: L’étude de faisabilité a rencontré un taux de participation de 73% (30) chez les cas et de 32,5% (39) chez les témoins. Seulement 26% (7) des participants dans le groupe des cas et 79% (26) des participants dans le groupe de témoins ont complété l’examen dentaire. La santé buccodentaire compromise était associée au risque de CCR chez les participants plus âgés que 59 ans (OR=8,4 ; IC à 95% : 0,94 – 389,4; p=0,036). Les mesures de santé buccodentaire auto-rapportées par les participants par l’utilisation du questionnaire ont montré une bonne validité par rapport aux mesures de l’examen clinique. Conclusion: L’étude épidémiologique ultérieure est jugée faisable en apportant des modifications. Les résultats préliminaires plaident pour une association possible entre la santé buccodentaire compromise par la maladie parodontale et/ou l’édentement et le risque du CCR. / Introduction: Colorectal cancer (CRC) remains a real public health burden in Canada. Despite the recent advances in medicine, the etiology of CRC is not yet clearly defined. The prevention of CRC requires the identification of new risk factors. In recent years, epidemiological studies have reported the association between oral health and CRC. However, no study has been specifically designed to test this association. Objectives: The main objective of the study was to test the feasibility of a future large epidemiological study designed to investigate the association between impaired oral health and the risk of CRC. The secondary objectives were to standardize the procedures of the study, to assess the level of participation and success of proposed recruitment approaches, to assess the feasibility and validity of data collection methods on oral health and to produce preliminary data on the association between impaired oral health and the risk of CRC. Methodology: This was population-based case-control feasibility study. Participants in the case group (n = 30) were recruited from two hospitals in Montreal. Controls (n = 39) were recruited from the general population of Montreal. Oral health measures were collected both by a self-administered questionnaire on oral health and a clinical examination. Data on several risk factors for CRC were collected by validated questionnaires. Results: The participation rate of the study was 73% (30) among cases and 32.5 % (39) in controls. Only 26% (7) of participants in the case group and 79% (26) of participants in the control group completed the dental examination. Compromised oral health was associated with increased CRC risk in participants older than 59 years (OR = 8.4, 95% CI 95%: 0.94 – 389.4; p = 0.036). Self-reported oral health measures obtained through questionnaires showed good validity to clinical examination. Conclusion: The subsequent epidemiological study is considered feasible by making changes. The preliminary results argue for a possible association between oral health compromised by periodontal disease and / or tooth loss and risk of CRC.

Rôle du facteur de transcription PITX1 dans les pathogenèses de l'ostéoporose et des maladies parodontales

L’ostéoporose est une maladie caractérisée par une faible masse osseuse et une détérioration du tissu osseux. Cette condition entraîne une plus grande fragilité osseuse et des risques de fractures. Plusieurs études ont associé l’ostéoporose à la faible densité osseuse des mandibules, à la perte d’attache parodontale, à l’augmentation de la hauteur de la crête alvéolaire et à la chute des dents. Cette étude vise à comprendre les mécanismes sous-jacents cette perte osseuse. En effet, au cours du développement des souris, PITX1 joue un rôle clé dans l'identité des membres postérieurs et dans le bon développement des mandibules et des dents. Son inactivation complète chez la souris mène à un phénotype squelettique sévère. Tandis que, son inactivation partielle provoque des symptômes apparentés à l'arthrose avec une augmentation de la masse osseuse au niveau de l’os cortical et de l’os trabéculaire. Inversement, une étude antérieure chez des jumelles monozygotiques discordantes pour l’ostéoporose, montrent une augmentation d’environ 8.6 fois du niveau d’expression du gène Pitx1 chez la jumelle ostéoporotique. Collectivement, ces données nous ont poussés à investiguer sur le rôle du facteur de transcription PITX1 dans le métabolisme osseux normal et pathologique. Dans ce contexte, des souris transgéniques Col1α1-Pitx1 sur-exprimant Pitx1 spécifiquement dans le tissu osseux sous le promoteur du collagène de type-I (fragment 2.1kpb) ont été générées et phénotypiquement caractérisées. Ces résultats ont révelé que les souris transgéniques Col1α1-Pitx1 présentaient un phénotype similaire à celui des patients ostéoporotiques accompagné d'une perte de dents et des problèmes dentaires et parodontaux. De plus, cette étude a révélé que la surexpression de Pitx1 induit une altération de l’homéostasie osseuse via l’inactivation de la voie de signalisation Wnt/β-caténine canonique. Cette hypothèse a été appuyée par le fait que le traitement des souris transgéniques Col1α1-Pitx1 avec du chlorure de lithium, un activateur de la voie Wnt canonique, prévient le phénotype ostéoporotique chez ces souris. Finalement, cette étude établit un rôle crucial de PITX1 dans la régulation de la masse osseuse et une implication possible dans l’ostéoporose et les maladies parodontales via l’inactivation de la voie de signalisation Wnt/β-caténine canonique. / Osteoporosis is a progressive bone disease that is characterized by a decrease in bone mass and density leading to an increased risk of fracture. Several studies have linked osteoporosis to the low bone density of the mandibles, the periodontal attachment loss, increasing of the alveolar crest heigh and tooth loss. This study aims to understand the mechanisms underlying bone loss. During mouse development, PITX1 plays a key role in the identity of the hindlimb and the proper development of the jaws and teeth. Interestingly, PITX1-null mice displayed severe skeletal phenotype. Whereas, ageing PITX1+/- progressively developed OA-like lesions in cartilage associated with a drastic increase in cortical and trabecular bone formation. Conversely, expression analysis of osteoblasts derived from monozygotic twins discordant for osteoporosis reported an 8.6-fold up-regulation of Pitx1 expression in osteoblasts from osteoporotic twins when compared with healthy ones. Collectively, these data prompted us to investigate the role of PITX1 in normal and pathological bone metabolism. In this context, transgenic Col1α1-Pitx1 mice over expressing Pitx1 specifically in bone tissue under the type-I collagen promoter (2.3kb fragment) were generated and phenotypically characterized. These data suggested that Pitx1 overexpression induces an osteoporosis like phenotype accompanied with oral bone loss, edentulousness, dental and periodontal problems. These results suggested that Pitx1 overexpression induces alteration of bone homeostasis via the inactivating of the the Wnt/β-catenin canonical pathway. This hypothesis was supported by the fact that treatment with lithium chloride, a Wnt canonical activator, rescued the phenotype. Finally, this study establishes a crucial role of PITX1 in the regulation of bone mass and a possible involvement in the developpement of osteoporosis and periodontal disease via the inactivation of the Wnt/β-catenin canonical pathway.

A metodologia, os códigos e os critérios adotados foram os recomendados pela Organização Mundial da Saúde (OMS), com a inserção dos grupos etários de 20 a 34 e 45 a 56 anos. Inicialmente foram identificados os moradores permanentes das duas cidades, as quais foram examinados no domicílio, com auxílio de espelho bucal e sonda periodontal CPI. Previamente à realização do estudo, o examinador passou por uma oficina de calibração e treinamento para a padronização dos diagnósticos da cárie dentária, fluorose dentária, perda dentária e uso e necessidade de prótese, em indivíduos com grupos etários semelhantes aos do estudo. Para o cálculo da concordância kappa intra-examinador, 10% dos indivíduos foram reexaminados, sendo os resultados obtidos para a cárie dentária 0,95; fluorose dentária 0,91; perda dentária 1 e o uso e necessidade de prótese 1. No total foram examinados 1588 moradores sendo 1129 (71,1%) de Baixo Guandu e 459 (28,9%) de Itarana. Foram utilizados os testes estatísticos Qui-quadrado, Fisher, Mann Whitney e de proporções com nível de significância de 5%. The effect of cigarette smoking on the virulence of streptococcus mutans caries and cardiovascular diseases-epidemiological analysis and in vitro studies

Indiana University-Purdue University Indianapolis (IUPUI) / The impact of tobacco smoking on human health is well documented. The influence of smoking on tooth loss and cardiovascular diseases was investigated in the current study via both epidemiology and in vitro studies. From analyzing the 2006 Behavioral Risk Factor Surveillance System (2006 BRFSS) database, we confirmed that smoking was significantly associated with the number of teeth lost in a dose-dependent manner and smoking cessation reduced the risk when compared to those subjects continuing to smoke. In addition, the virulence factors related to caries were compared between Streptococcus mutans and Streptococcus gordonii in response to cigarette smoking condensate (CSC) treatment. We observed that S. gordonii was more susceptible to CSC treatment than S. mutans. CSC significantly enhanced S. mutans sucrose-dependent and independent adherence. Western blot assays revealed that several bacterial surface proteins including glucosyltransferase (GTF), glucan-binding proteins and antigen I/II, were significantly upregulated for the treated S. mutans. These findings suggested that the oral environment with CSC may favor a cariogenic dominant composition, which may increase the risk for smokers to develop caries. We also found that smoking and oral health status modified each other and synergistically increased the risk of CVD and this joint effect was more pronounced among the youngest age group using the 2006 BRFSS database. To further understand the joint effect, we conducted an in vitro study to investigate bacterial attachment to fibronectin and endothelial cells in response to smoking condensate treatment. Our study clearly demonstrated CSC significantly enhanced S. mutans attachment to both soluble and immobilized fibronectin as well as endothelial cells. Furthermore, our data suggested that bacteria possessed several adhesins that bound to host tissues and endothelial cells also had multiple receptors for bacterial attachment. Among these adhesins, antigen I/II seemed essential for bacterial attachment to endothelial cells without CSC. The knowledge of bacterial attachment to host tissues in the presence of CSC may help in developing different preventive or therapeutic strategies against attachment and colonization of the host by S. mutans.
