Technology as an extension of the human body : Exploring the potential role of technology in an elderly home care setting

The present thesis explores the potential role and implications of technology in elderly care from the users’ perspective. This exploration is undertaken in terms of five empirical studies of a telehealth project and a meta-analysis of their contributions. An important insight emerging from this work is the need to rethink the human subject as a body, rather than as a mere mind using technology. The thesis draws on phenomenology to reconceptualize the user of technology, and on this basis, to theorize about the potential role and implications of technology in care. It concludes that, in combination with humans that integrate technology with their other sensory and emotional capacities, technology can produce affect. The findings indicate that technology can contribute to senior citizens feeling safe, cared for and thereby less isolated. The findings further demonstrate that, because of the perceptual capacity gained from technology, the care workers become aware of new health problems that urgently call for their sensory and emotional responsiveness. On this ground, the thesis challenges the determinist view that technology threatens the essentially ‘human’ aspect; rather, it concludes that feeling and other bodily resources are fundamental in the use of technology. Indeed, technology activates such ‘human’ capabilities. Hence, technology plays a role as a complement for rather than as a replacement of care workers. It increases their work burden by informing them about new needs. This may improve care quality but at an increased cost, which is relevant from a practical perspective. At a more general level, the thesis challenges the dualist legacies in mainstream management research that have sought to divorce mind form body, nature from culture and reason from emotion. It can therefore contribute to broader theoretical developments and fuel existing debates beyond the care setting.

In this essay the author has chosen to analyze the similarities and differences between some of the northern Europe’s late Mesolithic and Neolithic cultures. The research is of a processual standpoint and the information is mainly gathered from secondary sources as well as ethnological studies. The material collected has been analyzed in both a processual and a post-processual manner to most accurately study the foundations of the three primary theories describing the Pitted Ware Culture’s origin. During the course of the study the author also found a possible fourth alternative, and the possibility of exogamy as a factor in the emerge of the Pitted Ware Culture has also been questioned. This work has prompted for a rigorous collection of information to be able to properly present all theories strong and weak points, without bias for any theory. The conclusion of these studies is that it is very hard to pinpoint exactly what it is that has given the Pitted Ware Culture its typical cultural traits, although a possible sequence of events has been presented in this text.

Science and intertext : methodological change and continuity in Hellenistic science

This dissertation investigates the appropriation of material from one scientific field into another in the early Hellenistic period, 300-150 BCE. Appropriation from one science into another led to the emergence of new concepts in a community of scientists. Herophilus of Chalcedon’s appropriation of musical rhythms led to the emergence of the pulse as a materio-semiotic object for Rationalist physicians. Archimedes of Syracuse’s appropriation of mechanical concepts of weighing led to the emergence of the mechanical method as a scientific way of seeing for practicing mathematicians. But objects and concepts emerging from cross-scientific appropriation had ideological consequences for scientific methodology within individual scientific communities. Archimedes prioritized a formal Euclidean proof over that offered by the mechanical method because of the standards of proof demanded by the community of practicing mathematicians. The sect of Empiricist physicians rejected Rationalist medicine and promoted the individual doctor’s role and authority as a medical caregiver. The dissertation’s sum tells a story of increasing but limited strategies of naturalization within the sciences of the early Hellenistic period. / text

Les premiers textes de Miron, Gauvreau, Giguère, Lefrançois et Hébert : poétique des commencements et de l'émergence

L’objectif de cette thèse est d’analyser les modes d’émergence de la voix chez cinq écrivains québécois ayant initié, durant les années 1940-1970, leur parcours littéraire à travers la pratique de la poésie. Cette thèse puise à même les ressources de la poétique telle qu’alimentée par les penseurs contemporains (Agamben, Blanchot, Didi-Huberman, Lyotard, Rey, Anzieu) et utilise certains outils d’analyse que fournissent la psychanalyse, la sociocritique, la linguistique et la philosophie. Aussi, certains documents personnels exhumés de divers fonds d’archives des cinq auteurs (lettres, journal, notes, brouillons) concourent à éclairer les modes d’émergence à l’oeuvre dans les premiers poèmes, afin de démontrer comment les écrits à venir se trouvent entièrement préfigurés dans ces textes initiaux. Il s’agit, en somme, de montrer comment ces poètes furent aux prises avec certaines problématiques communes, notamment le grand défi de fonder une parole authentique au sein d’une communauté poétique si longtemps réservée aux élites. Ayant tous amorcé un processus d’écriture à la moitié du XXe siècle, Gaston Miron, Alexis Lefrançois, Claude Gauvreau, Roland Giguère et Anne Hébert n’en demeurent pas moins déterminés par la culture qui les a formés; leurs oeuvres respectives sont, par conséquent, fondamentalement ancrées dans une quête d’élucidation de soi et de l’autre qui ne saurait s’élaborer qu’en suivant les chemins les plus intimes, et dont je tenterai, à travers une lecture personnelle de leurs premiers textes, de rendre toute la portée. / The purpose of this thesis is to analyse the rising modes of the voice in five Quebec writer’s work; these authors have started to write between 1940 and 1970, and their artistic path led them all to express themselves, in the first place, through poetry. This thesis digs into the resources of recent poetic studies (Agamben, Blanchot, Didi-Huberman, Lyotard, Rey, Anzieu) and uses certain tools given by parallel academic disciplines such as psychoanalysis, sociocriticism, linguistic and philosophy. Also, certain personal documents extracted from private and public archive founds (letters, notes, drafts, journal) contribute to throw light on the emergence’s modes in action in the first poems of the chosen writers, in addition to demonstrate how the writings to come may see themselves foreshadowed in the initial work. Therefore, this thesis will try to demonstrate how these five poets battling with similar poetic issues (in particular with the great challenge that consists of creating an authentic language in a society where only elites could have access to culture and education) will initiate their creative process in Quebec province in the middle of the XXth century. Despite all their efforts to move forward, Gaston Miron, Alexis Lefrançois, Claude Gauvreau, Roland Gguère and Anne Hébert will remain partially shaped by the popular culture that has raised them, giving birth to poems that are deeply influenced by their inner quest and could only arise through a deeply intimate journey, of which I will try to show the impressive range, using my knowledge of their first texts.

Magistrantūros studijų baigiamajame darbe pateikiami piktžolių dygimo dinamikos (trumpaamžių, daugiamečių ir vyraujančių rūšių piktžolių) ir burokėlių derlingumo duomenys tiriant įvairių organinių mulčių ir skirtingo storio sluoksnių liekamąjį poveikį. Darbo objektas – raudonojo burokėlio (Beta vulgaris var. vulgaris) agrofitocenozė. Darbo metodai: piktžolių daigų skaičiavimui kiekviename laukelyje buvo pažymėtos keturios pastovios aikštelės 0,2 x 0,5 m. Apskaita atlikta kas 10 dienų, nuo gegužės 20 d. iki rugsėjo 30 d. Kiekvienos rūšies piktžolių daigai suskaičiuoti ir išrauti. Piktžolių kiekis perskaičiuotas vnt. m-2. Pagal piktžolių biologinį grupavimą išskirtos trumpaamžės ir daugiametės piktžolės. Nustatytos vyraujančios piktžolių rūšys. Burokėlių derlingumas nustatytas svėrimo metodu. Gautas derlius iš laukelio perskaičiuotas į t ha-1 absoliučiai švarių burokėlių derlių. Darbo rezultatai. Gausiausiai piktžolės dygo gegužės trečią – 765,6 vnt. m-2 ir birželio pirmą – 270,6 vnt. m-2 dekadas. Vėliau piktžolių dygimas mažėjo visų variantų laukeliuose. Anksčiau įterpti šiaudų, durpių ir žolės mulčiai nežymiai (4,7-6,7 proc.) skatino piktžolių dygimą per visą tyrimų laikotarpį. Nustatytas nevienodas mulčių liekamasis poveikis trumpaamžių piktžolių dygimui. Įterpti durpių ir žolės mulčiai skatino trumpaamžių piktžolių dygimą nuo 5,2 iki 12,6 proc. Paprastosios rietmenės 20 proc. gausiau rasta ankstesniais metais durpių mulčiu mulčiuotuose laukeliuose, smulkiažiedės galinsogos... [toliau žr. visą tekstą] / Master degree final research paper provides weed germination dynamics (annual, perennial and predominant weeds sorts) and red beet productivity data when researching residual effect of various organic mulches and different thickness layers. Object of the work – red beet (Beta vulgaris var. vulgaris) crop. Method of the work – to compute weed sprouts each field was divided into four permanent sites 0.2 x 0.5 m. These were observed every 10 days from May 20 till August 30. Each weed species sprouts were counted and torn out. Number of weeds was translated into units. m-2. According to biological grouping of weeds annual and perennial were distinguished. Predominant weed species were established. Red beet productivity was determined by weighting. Received yield from the field was translated into t ha-1 totally useful red beet yield. The results of work. Most of the weeds germinated in the 3rd decade of May – 765.6 weeds number m-2 and 1st decade of June – 270.6 weeds number m-2. Later germination of the weeds decreased in the plots of all treatments. Straw, peat and grass mulches, previously incorporated, moderately (4.7 to 6.7 percent) stimulated weed germination during the entire study period. The different residual effect of mulches was determined for germination of annual weeds. The incorporated peat and grass mulches promoted germination of the annual weeds from 5.2 to 12.6 percent. Leadership development through appreciative inquiry : complexity thinking in the non-government (NGO) sector.

“much of what we know about leadership is today redundant because it is literally designed for a different operating model, a different context, a different time” (Pascale, Sternin, & Sternin, p. 4). This thesis describes a project that was designed with a focus on exploring ways to enhance leadership capacity in non-government organisations operating in Christchurch, New Zealand. It included 20 CEOs, directors and managers from organisations that cover a range of settings, including education, recreation, and residential and community therapeutic support; all working with adolescents. The project involved the creation of a peer-supported professional learning community that operated for 14 months; the design and facilitation of which was informed by the Appreciative Inquiry principles of positive focus and collaboration. At the completion of the research project in February 2010, the leaders decided to continue their collective processes as a self-managing and sustaining professional network that has grown and in 2014 is still flourishing under the title LYNGO (Leaders of Youth focussed NGOs). Two compelling findings emerged from this research project. The first of these relates to efficacy of a complexity thinking framework to inform the actions of these leaders. The leaders in this project described the complexity thinking framework as the most relevant, resonant and dynamic approach that they encountered throughout the research project. As such this thesis explores this complexity thinking informed leadership in detail as the leaders participating in this project believed it offers an opportune alternative to more traditional forms of positional leadership and organisational approaches. This exploration is more than simply a rationale for complexity thinking but an iterative in-depth exploration of ‘complexity leadership in action’ which in Chapter 6 elaborates on detailed leadership tools and frameworks for creating the conditions for self-organisation and emergence. The second compelling finding relates to efficacy of Appreciative Inquiry as an emergent research and development process for leadership learning. In particular the adoption of two key principles; positive focus and inclusivity were beneficial in guiding the responsive leadership learning process that resulted in a professional learning community that exhibited high engagement and sustainability. Additionally, the findings suggest that complexity thinking not only acts as a contemporary framework for adaptive leadership of organisations as stated above; but that complexity thinking has much to offer as a framework for understanding leadership development processes through the application of Appreciative Inquiry (AI)-based principles. A consideration of the components associated with complexity thinking has promise for innovation and creativity in the development of leaders and also in the creation of networks of learning. This thesis concludes by suggesting that leaders focus on creating hybrid organisations, ones which leverage the strengths (and minimise the limitations) of self-organising complexity-informed organisational processes, while at the same time retaining many of the strengths of more traditional organisational management structures. This approach is applied anecdotally to the place where this study was situated: the post-earthquake recovery of Christchurch, New Zealand.

Résumé : Cette thèse s’intéresse au temps d’émergence et aux asynchronies patient-ventilateur à l’origine de lésions pulmonaires chez des patients en soins critiques sous algosédation durant la phase aiguë de la ventilation mécanique. L’originalité de cette recherche consiste à comparer un protocole d’algosédation guidé par l’échelle de Ramsay (le standard) à celui guidé par la technologie BIS. Méthodologie. Suivant un devis mixte inter groupe et intrasujets, cette recherche comporte deux groupes de patients adultes et se déroule sur une période de quatre ans. Le premier groupe comprend 23 patients dont le protocole d’algosédation est guidé par l’échelle de Ramsay. Le second groupe compte 18 patients soumis à la même stratégie de ventilation et au même protocole d’algosédation mais guidé cette fois-ci par la technologie BIS. Mesures. Le temps d ’émergence pour un retour à la ventilation spontanée soutenue (TEVSS) suivant l’arrêt de la perfusion de l’algosédation et les interactions patient-ventilateur (synchronies et asynchronies) durant la phase aiguë de ventilation constituent les variables dépendantes. Résultats. Le groupe bénéficiant de la technologie BIS présente un TEVSS médian de 3,1 heures comparé à 22,5 heures pour le groupe guidé par l’échelle de Ramsay (valeur p=0,001). De plus, le groupe bénéficiant de la technologie BIS présente 11% de moins d’asynchronies patient-ventilateur que celui guidé par l’échelle de Ramsay (valeur p<0,001). En outre, parmi les paramètres de la technologie BIS, l’électromyogramme (EMG) s’avère l’élément le plus robuste à prédire l’asynchronie patient-ventilateur. Conclusion. La réduction du TEVSS ainsi que la diminution des asynchronies patient-ventilateur démontrent un avantage considérable à utiliser la technologie BIS en soins critiques chez les patients adultes non communicatifs durant la phase aiguë de ventilation mécanique. De plus, l’EMG de la technologie BIS permettrait vraisemblablement de déceler de façon précoce l’expression préclinique de la douleur. Enfin, cette thèse propose un algorithme de contrôle de l’algosédation en soins critiques en référence à la technologie BIS. // Abstract : This thesis focuses on emergence time from algosédation (for which a prolongation increases complex investigations and costs) and on patient-ventilator interaction (related to lung injuries) in critical care patients undergoing mechanical ventilation with algosedation perfusion during the acute phase of ventilator support. More specifically, the originality of this research stems from the simultaneous comparison of the emergence time for a retum of sustained spontaneous breathing and the presence of asynchronies, by eomparing algosedation guided by the Ramsay scale (gold standard) in a first group and by the BIS technology in a second group. Methodology. Following a mixed design of an inter group, intra subject, this research is performed in two groups of adults over a period of four years. The first group consists of 23 patients where algosedation is guided with the Ramsay scale whereas the second group consists of 18 patients with the same ventilation strategy and same algosédation protocol but guided using the BIS technology. This research evaluates the effectiveness of both instruments to guide algosedation during the acute phase of ventilation. Measures. Dependent variables consist of the emergence time for a sustained spontaneous breathing following cessation o f algosedation and patient-ventilator interaction (asynchronies) during the acute phase of ventilatory support. Results. The group guided with BIS technology has a median emergence time for a sustained spontaneous breathing of 3.1 hours compared to 22.5 hours for the Ramsay scale guided group (p value=0.001). Furthermore, patients benefiting from the BIS technology presented 11% less asynchrony than those with the Ramsay scale (p value <0.001). More specifically, among BIS technology parameters, electromyography (EMG) appeared the best indicator to predict patient-ventilator asynchrony in both groups. Conclusion. In the second group, the reduction in the emergence time for a sustained spontaneous breathing and the decrease of patient-ventilator asynchronies mandates the use o f BIS technology in critical care to guidee algosedation among non-communicative adults during the acute phase of ventilatory support. EMG from the BIS technology might be able to detect pre-clinical pain expression. This thesis also favors the implementation of a décision algorithm in the control of algosedation in critical care.

Car si, avec la raison comme alliée, l'homme se découvre des potentialités illimitées, nous aurions pourtant tort de passe outre une stricte réalité, celle de notre ignorance infinie, celle, au fond, de notre incapacité à cerner, de façon sûre et certaine, quoi que ce soit de ce monde complexe et en évolution constante qui nous accueille. C'est pourquoi, pour Karl Popper, toute rationalité véritable doit être critique, c'est-à-dire pluraliste et débattante, seule façon de considérer objectivement l'écart qui nous sépare de la vérité et, par suite, d'agir avec prudence et discernement, dans l'intérêt de la science comme dans celui de la collectivité.
