The metabolic sequelae of oesophago-gastric resection

Bypass or resection of the stomach and oesophagus, has long been recognised to result in profound changes in the handling of ingested nutrients. This results in significant morbidity after radical surgery for oesophago-gastric cancer, in particular post-prandial hypoglycaemia, altered appetite, early satiety and noxious post-prandial symptoms. By profiling and challenging the gut hormone axis in healthy volunteers and patients who had undergone total or subtotal gastrectomy, or oesophagectomy, this thesis explores the possible causative mechanisms for the challenges faced by this patient population. In the surgical groups, an oral glucose tolerance test (OGTT) resulted in enhanced secretion of satiety and incretin gut hormones (GLP-1, GIP, PYY) and insulin, followed by hypoglycaemia in a cohort of patients. Continuous glucose monitoring of gastrectomy participants over two weeks of normal lifestyle identified an increased incidence of day and night time hypoglycaemia. RNAseq and mass spectrometry based peptidomics of human and murine enteroendocrine cells in the pre- and post-operative populations revealed no significant change in the underlying cellular pathways for nutrient sensing and gut hormone secretion, indicating that the altered hormone secretion is primarily driven by accelerated nutrient transit, rather than adaptive changes in the gut. Finally, specific blockade of the GLP-1 receptor in post-gastrectomy patients using Exendin 9-39 normalised insulin secretion and prevented reactive hypoglycaemia after an OGTT. In conclusion, profound changes in gut hormone secretion as a result of enhanced nutrient transit after foregut surgery likely underlie the early and late post-prandial symptoms seen in this group, and therapies specifically targeting the gut hormone axis, and GLP-1 in particular, could be the first targeted treatments for post-gastrectomy syndromes.

However, there is still the need to establish a golden rule for nutritional evaluation as a predictive factor in the morbimortality of these patients. The objective of this study was to analyze and compare the capacity of nutritional assessment methods in the prognosis of complications in patients submitted to gastric cancer gastrectomy. Methods: The method used was the prospective observational study of 66 gastric cancer patients who were submitted to gastrectomies. Assessment was made through the following nutritional evaluation methods: Subjective Global Assessment of Nutritional Status (SGA), Objective Nutritional Evaluation (ONE) and the Nutritional Risk Index (NRI). Beyond the surgical risk by means of a prognostic index of Sheffield (SPI). The results were compared to surgical morbimortality, total duration of hospitalization and stay in the Intensive Care Unit (ICU). The stage of the disease was based on the American classification for gastric cancer. For the estatiscal analyses were used Kaplan-Meier method, log-rank test, t test or non-parametric Kruskal-Wallis test. Associations were evaluated by the Pearson chi-square test, or by the Fisher exact test whenever recommended. Results: Malnutrition was present in 80.7%, 66.7% and 85% in the SGA, ONE and NRI, respectively. The SPI demonstrated a high surgical risk in 92% of patients. There was a slight agreement in the SGA and NRI (Kappa=0.30). The SGA results showed that the malnourished patient (p<0.05) remained in the hospital for a longer period time. Most of the patients (62.9%) were at an advanced stage of the disease which was associated to an increase in complications (p<0.01) and isolated cases of pneumonia that were associated with lymphopenia (p<0.05). Conclusion: The SGA and the NRI were the methods that had the closest agreement in the results. The SGA was the only method that demonstrated an association between malnutrition and longer stay in hospital. Pneumonia was observed in the presence of advanced tumors and lymphopenia, thus increasing the duration of hospitalization and stay in the ICU (p<0.05). / Introdução: A desnutrição afeta adversamente o paciente cirúrgico oncológico, levando-o a maiores morbidades e mortalidade operatórias. Porém, ainda há necessidade de se estabelecer um padrão ouro de avaliação nutricional como fator preditivo de morbimortalidade nesses pacientes. O objetivo deste estudo foi analisar e comparar a capacidade dos métodos de avaliação nutricional no prognóstico de complicações de pacientes submetidos a gastrectomias por câncer gástrico. Métodos: Estudo observacional prospectivo envolvendo 66 pacientes com câncer gástrico e submetidos a gastrectomias. Foram analisados os métodos de avaliação nutricional subjetiva global (ANSG), avaliação nutricional objetiva (ANO) e o índice de risco nutricional (IRN), além do risco cirúrgico por meio do índice prognóstico de Sheffield (IPS). Os resultados foram comparados com a morbimortalidade cirúrgica, tempo de internação total e em UTI. O estadio da doença foi obtido pela classificação americana para câncer gástrico. Resultados: A desnutrição esteve presente em 80,7%, 66,7% e 85% nas ANSG, ANO e IRN, respectivamente. O IPS demonstrou alto risco cirúrgico em 92% dos pacientes. Houve concordância leve entre a ANSG e IRN (Kappa=0,30). A ANSG demonstrou maior tempo de internação hospitalar nos desnutridos (p<0,05). A maioria dos pacientes (62,9%) apresentaram-se com estágio avançado e associado à maior incidência de complicações (p<0,01) e isoladamente a pneumonia esteve associada com a linfopenia (p<0,05). Conclusão: ANSG e o IRN foram os métodos que tiveram uma concordância mais próxima. A ANSG foi o único método que demonstrou associação entre desnutrição e maior tempo de internação hospitalar. A pneumonia foi observada no tumor avançado e na presença de linfopenia, aumentando os dias de internação hospitalar e em UTI (p<0,05).

O objetivo deste trabalho foi avaliar comparativamente os resultados tardios da esofagectomia transdiafragmática com gastroplastia e da operação de Serra Dória para o tratamento do megaesôfago avançado. Foi realizado estudo coorte em 44 pacientes. O seguimento tardio no Grupo EG foi de 77,0 meses, e no Grupo SD de 62,1 meses. Utilizou-se interrogatório para queixas gastrointestinais, escalas de Schechter e de Visick, assim como dois questionários. Incluíram-se dados antropométricos, bioquímicos e endoscópicos. A satisfação com a cirurgia medida pela escala de Visick foi equivalente nas duas operações praticadas, sem diferenças estatísticas. Também nos questionários o perfil foi equilibrado. Houve maior a incidência de esofagite no grupo EG; Não houve diferenças significativas com relação à avaliação clínica, nutricional, histológica e de qualidade de vida / The objective of the paper was to carry out a comparative evaluation of the late results of a transdiaphragmatic esophagectomy with an associated gastroplasty and of the Serra Dória procedure, for the treatment of advanced megaesophagus. Ο αυξημένος σπλαχνικός λιπώδης ιστός αποτελεί σημαντικό παράγοντα κινδύνου για μεταβολικές επιπλοκές, που συσχετίζονται με την παχυσαρκία, και προάγει μία ήπιου βαθμού χρόνια φλεγμονώδη διαδικασία. Το επίπλουν έχει από καιρό εμπλακεί στη, σχετιζόμενη με την παχυσαρκία, μεταβολική δυσλειτουργία. Αυτό βασίζεται στη σημαντική του λειτουργία, της έκκρισης αντιποκινών. Η ιδέα της εκτομής του μείζονος επιπλόου, στον ίδιο χρόνο με μία βαριατρική επέμβαση, έχει προταθεί για την βελτίωση των μεταβολικών μεταβολών και την μεγιστοποίηση της απώλειας βάρους. Ο σκοπός της συγκεκριμένης μελέτης ήταν να προσδιορίσει εάν η εκτομή του μείζονος επιπλόου, στον ίδιο χρόνο με τη λαπαροσκοπική επιμήκη γαστρεκτομή, έχει κάποια επίδραση στο μεταβολικό προφίλ, την έκκριση των αντιποκινών, το στάτους της φλεγμονής και την απώλεια βάρους, σε βραχύ ή μακρό βάθος χρόνου. ΜΕΘΟΔΟΙ: Τριάντα – ένας παχύσαρκοι ασθενείς (Δείκτης Μάζας Σώματος (ΒΜΙ): 42.49±2.03 Kg/m2 ) τυχαιοποιήθηκαν σε δύο ομάδες, λαπαροσκοπικής επιμήκους γαστρεκτομής, με ή χωρίς επιπλεκτομή. Αντιπονεκτίνη, Ομεντίνη, Ιντερλευκίνη-6 (IL-6), tumor necrosis factor-α ((TNF-α), C-αντιδρώσα πρωτεΐνη υψηλής ευαισθησίας (hs-CRP), high-density lipoprotein (HDL) χοληστερόλη, γλυκόζη νηστείας, ινσουλίνη και αντίσταση στην ινσουλίνη (εκτιμωμένη με εφαρμογή -­‐ 106 -­‐ Quickie Test) μετρήθηκαν και εκτιμήθηκαν προεγχειρητικά και 7 μέρες, 1, 3 και 12 μήνες μετεγχειρητικά. ΑΠΟΤΕΛΕΣΜΑΤΑ: Κατά τη μετεγχειρητική παρακολούθηση, στη διάρκεια του πρώτου χρόνου, ο δείκτης μάζας σώματος μειώθηκε αξιοσημείωτα και συγκριτικά και στις δύο ομάδες (Ρ<0.001). Τα επίπεδα της ινσουλίνης, IL-6 και hs-CRP, μειώθηκαν σημαντικά σε σχέση με τις τιμές αναφοράς (προεγχειρητικά) (Ρ<0.05) και στις δύο ομάδες, χωρίς στατιστικά σημαντική διαφορά μεταξύ τους. Τα επίπεδα αντιπονεκτίνης και HDL αυξήθηκαν ομοίως και σημαντικά, συγκρινόμενα με τα επίπεδα αναφοράς (Ρ<0.001) και στις δύο ομάδες. Τα επίπεδα της Ομεντίνης αυξήθηκαν σημαντικά (Ρ<0.05) στην ομάδα ελέγχου (επιμήκης γαστρεκτομή, χωρίς εκτομή του επιπλόου) και παρέμειναν χαμηλά στην ομάδα της επιπλεκτομής (επιμήκης γαστρεκτομή + επιπλεκτομή), στο ένα έτος μετεγχειρητικά. Δεν υπήρξε στατιστικά σημαντική διαφορά στη μεταβολή των επιπέδων TNF-α σε κάθε ομάδα. ΣΥΜΠΕΡΑΣΜΑΤΑ: Τα, μέχρι τώρα, θεωρητικά πλεονεκτήματα της επιπλεκτομής, όσον αφορά την απώλεια βάρους και το μεταβολικό σύνδρομο, δεν αντικατοπτρίζονται στην προοπτική αυτή μελέτη. Επιπλέον, δοθέντος του προστατευτικού ρόλου της ομεντίνης σε συνδυασμό με τη θετική συσχέτισηή της με τα επίπεδα αντιπονεκτίνης πλάσματος και HDL, ήδη γνωστών καρδιοπροστατευτικών πρωτεϊνών, ανακύπτουν ερωτήματα γύρω από την αρνητική επίδραση της επιπλεκτομής και καρδιαγγειακής φυσιολογίας, σε βάθος χρόνου. / Increased visceral adipose tissue is a risk factor for the metabolic complications associated with obesity and promotes a low-grade chronic inflammatory process. Resection of the great omentum in patients submitted to a bariatric procedure has been proposed for the amelioration of metabolic alterations and the maximization of weight loss. The aim of the present study was to investigate the impact of omentectomy performed in patients with morbid obesity undergoing sleeve gastrectomy (SG) on metabolic profile, adipokine secretion, inflammatory status and weight loss. Methods: Thirty-one obese patients were randomized into two groups, SG alone or with omentectomy. Adiponectin, omentin, interleukin-6 (IL-6), tumor necrosis factor α (TNF-α), high sensitivity C-reactive protein (hs-CRP), blood lipids, fasting glucose, insulin and insulin resistance were measured before surgery and at 7 days, and 1, 3 and 12 months after surgery. Results: During the one year follow up BMI decreased markedly and comparably in both groups (P<0.001). Insulin, IL-6 and hs-CRP levels decreased significantly compared to baseline (P<0.05) in both groups with no significant difference between groups. Adiponectin and high-density lipoprotein choresterol levels were significantly and similarly increased compared to baseline (P<0.001) in both groups. Omentin levels increased significantly (p<0.05) in the control group and decreased in -­‐ 108 -­‐ the omentectomy group one year postoperatively. There was no significant change in TNF-α levels in either group. Conclusions: The theoretical advantages of omentectomy in regard to weight loss and obesity related abnormalities are not confirmed in this prospective study. Furthermore, omentectomy does not induce important changes in the inflammatory status in patients undergoing SG.

Performance del Índice Neutrófilo-Linfocito (INL) preoperatorio como predictor de complicaciones tempranas post - gastrectomía en manga

Introducción: La obesidad es un problema de salud pública y la cirugía bariátrica es considerada como un tratamiento efectivo a largo plazo. Estos procedimientos tienen una mortalidad de hasta el 0.08% dentro de los 30 días postoperatorios y una tasa de complicaciones de hasta el 17%. El índice neutrófilo linfocito (INL) es un marcador de inflamación que ha sido utilizado como marcador pronóstico en diversas patologías inflamatorias y cirugías. Objetivo: El presente estudio tuvo el objetivo de evaluar el rendimiento del índice neutrófilo-linfocito (INL) preoperatorio como predictor de complicaciones tempranas posterior a la gastrectomía en manga. Métodos: Se realizó un estudio de cohorte retrospectivo a partir del análisis de la base de datos institucional de la Clínica Avendaño durante los años 2017 a 2020. El índice neutrófilo linfocito se calculó a partir del hemograma preoperatorio. Se cálculo el área bajo la curva para determinar el rendimiento del INL preoperatorio como predictor de complicaciones tempranas post gastrectomía en manga y se presentaron diferentes escenarios de acuerdo a los puntos de corte de INL con sus respectivos parámetros estadísticos. Resultados: Se estudiaron 387 pacientes operados por gastrectomía en manga en la Clínica Avendaño. De dichos pacientes, 45 pacientes presentaron complicaciones en los 30 primeros días postoperatorios (11.6%). El área bajo la curva para INL como predictor de complicaciones generales tempranas post gastrectomía en manga fue de 0.423 y para complicaciones severas fue de 0.392. Conclusión: El INL preoperatorio no es un buen predictor de complicaciones tempranas por gastrectomía en manga. / Background: Obesity is a public health problem. Bariatric surgery is an effective long term treatment for obesity. This procedure has a mortality up to 0.08% during the 30 days postoperative and up to 17% of complications. The neutrophil to lymphocyte ratio (NLR) is an inflammatory marker used for prognostic of inflammatory diseases and surgery. Objectives: The objective of our study is to evaluate the utility of preoperative neutrophil to lymphocyte ratio (NLR) as a predictor of early postoperative complications of sleeve gastrectomy. Methods: We performed a retrospective cohort study by the analysis of the institutional data base of Clinica Avendaño between 2017 and 2020. NLR values were obtained from the preoperative complete blood count. We calculated the ROC curve and different cut points with their statistic parameters for NLR as predictor of early postoperative complications after sleeve gastrectomy. Results: A total of 387 patients went under sleeve gastrectomy. Of the patients, 45 patients (11.6%) had early postoperative complications. The Receiver Operating Characteristic Curve (ROC curve) for NLR as predictor of early complications post sleeve gastrectomy was 0.423 and 0.392 for severe complications. Conclusion: Preoperative NLR it’s not a good predictor for early postoperative complications for sleeve gastrectomy. / Tesis

Psychological and psychosomatic aspects of bariatric surgery for the treatment of obesity in adults

Das Krankheitsbild der Adipositas hat sich weltweit zu einem relevanten Gesundheitsproblem entwickelt. Die bariatrische Chirurgie wird zunehmend als wirkungsvolle Behandlung bei schwer ausgeprägter Adipositas eingesetzt. Jedoch ist über die Rolle psychologischer Variablen im bariatrischen Behandlungsverlauf noch wenig bekannt. Die vorliegende Dissertation untersucht Einfluss und Veränderung patientenberichteter Gesundheitsmerkmale in der chirurgischen Adipositastherapie. Dazu werden in einer naturalistischen Beobachtungsstudie Patienten mit schwerer Adipositas vor und im Durchschnitt zwei Jahre nach einer bariatrischen Operation (OP) befragt. Ziele der Arbeit sind 1) die Charakterisierung adipöser Patienten vor OP hinsichtlich bio-psycho-sozialer Variablen; 2) die Identifikation möglicher Einflussvariablen auf den gewichtsbezogenen Behandlungserfolg nach OP; 3) die Untersuchung von Auswirkungen der OP auf das Essverhalten; und 4) die Analyse von Veränderungen in der essstörungsbezogenen Psychopathologie und in der gesundheitsbezogenen Lebensqualität nach OP. Die Ergebnisse der bariatrischen Patienten werden im Vergleich zu denen konservativ behandelter Patienten betrachtet. Die Ergebnisse zeigen, dass Patienten mit bariatrischem Behandlungswunsch eine somatisch und psychisch belastete Patientengruppe darstellen. Die bariatrische OP führt im zweiten postoperativen Jahr zu einer nachhaltigen und klinisch bedeutsamen Gewichtsreduktion. Der präoperative Body-Maß-Index, das Bildungsniveau und aktives Problembewältigungsverhalten sind mit dem Gewichtsverlust nach OP assoziiert. Im Vergleich zur konservativen Behandlung berichten die Patienten, die sich der OP unterziehen, über stärker ausgeprägte Verbesserungen in ihrem Essverhalten und eine Steigerung ihrer Lebensqualität. Auf Basis der Befunde wird ein routinemäßiges Monitoring der somatischen und psychischen Situation der Patienten nach bariatrischer OP empfohlen, um die gezeigten Behandlungserfolge optimal zu sichern. / Obesity has become a relevant global health problem. Bariatric surgery is an effective treatment for severe obesity. However, while the number of operations performed continues to increase, the role of psychological variables throughout the bariatric surgery pathway remains uncertain. The present dissertation investigates the patient-reported health status as it impacts and results from bariatric surgery. In a naturalistic observational study, patients with severe obesity are assessed before and, on average, two years after the surgical treatment. Main aims are 1) to characterize obese patients prior to bariatric surgery in terms of biological, psychological and socio-demographic variables; 2) to identify possible predictors for the postoperative weight-related treatment success after bariatric surgery; 3) to examine changes in eating behaviors; and 4) to analyze changes in eating-related psychopathology and in health-related quality of life (HRQoL). The outcomes of surgical patients are compared with those of conservatively treated patients for the same follow-up period. The findings show that bariatric surgery candidates represent a vulnerable patient group with high physical and psychological burden. In the second postoperative year after bariatric surgery, a sustainable and clinically meaningful weight reduction is achieved. The preoperative body mass index, education level and active coping behavior are associated with weight loss after surgery. Compared with conservative treatment, patients who undergo bariatric surgery report not only greater improvements in their eating behavior and eating-related psychopathology but also an increase in their HRQoL. Based on the results, the provision of a routine monitoring of the somatic and psychological situation of patients following bariatric surgery is recommended to secure longer-term treatment success.

Os desfechos clínicos e oncológicos dos pacientes submetidos à ressecção endoscópica do câncer gástrico precoce (CGP), considerando os critérios de indicação, comparados à cirurgia, não foram relatados em revisões sistemáticas. A pesquisa foi desenvolvida para estabelecer os desfechos de curto e longo prazos da ressecção endoscópica comparada à cirurgia no tratamento do CGP, elevando as informações para o nível de evidência 2a, melhor respaldando a prática clínica. A revisão sistemática com metanálises foi procedida utilizando-se as bases Medline, Embase, Cochrane, LILACS, Scopus e CINAHL. Onze coortes retrospectivas foram selecionadas para análise qualitativa e quantitativa. Todos os estudos incluem pacientes com CGP e comparam os desfechos nos dois braços. Os dados envolveram 2654 pacientes que preenchiam os critérios absolutos ou expandidos para ressecção endoscópica. Diferentes modalidades de tratamento endoscópico foram avaliadas, principalmente os procedimentos de ressecção, como endoscopic mucosal resection (EMR) e endoscopic submucosal dissection (ESD). As informações basearam-se nas características dos participantes, critérios de inclusão e exclusão, tipos de intervenções e desfechos (diferentes taxas de sobrevida, eventos adversos, ressecção completa, recorrência e mortalidade). As análises dos riscos absolutos dos desfechos foram feitas com o software RevMan, computando-se as diferenças de risco (DR) das variáveis dicotômicas. Dados de DR e intervalo de confiança de 95% (IC) foram calculados utilizando-se o teste de Mantel-Haenszel e a inconsistência foi qualificada e reportada em ?2 e método Higgins (I2). A análise de sensibilidade foi feita quando a heterogeneidade era maior que 50%. Todas as análises basearam-se inicialmente no modelo de efeito fixo. Dados de sobrevida de 3 anos estavam disponíveis em seis estudos (n = 1197). Não houve DR após os dois tratamentos (DR = 0,01, IC 95% = -0,02 a 0,05). A sobrevida de 5 anos (n = 2310) não demonstrou diferença significativa entre os grupos analisados (DR = 0,01, IC 95% = -0,01 a 0,03). A avaliação de 551 pacientes não evidenciou desigualdade na sobrevida de 10 anos entre as diferentes abordagens (DR = -0,02, IC 95% = -0,15 a 0,10). Dados de complicação estavam presentes em oito estudos (n = 2439), e diferença significativa foi detectada (DR = -0,08, IC 95% = -0,10 a -0,05), demonstrando melhores resultados com a endoscopia. As taxas de ressecção completa foram analisadas em 536 pacientes. Evidenciou-se diferença significativa entre o tratamento endoscópico e cirúrgico (DR = -0,13, IC 95% = -0,17 a -0,09), validando melhores resultados no último grupo. A recorrência foi avaliada em cinco pesquisas (n = 1331) e não houve diferença entre as duas formas de terapêutica (DR = 0,01, IC 95% = -0,00 a 0,02). As taxas de mortalidade foram obtidas de quatro estudos (n = 1107), e não se evidenciou diferença entre os grupos envolvidos (DR = -0,01, IC 95% = -0,02 a 0,00). Conclui-se que as taxas de sobrevida de 3, 5 e 10 anos, recorrência e mortalidade são semelhantes em ambos os grupos. Considerando-se as taxas de complicação, a abordagem endoscópica confere resultados mais apropriados e, analisando-se as taxas de ressecção completa, ela é inferior à cirurgia / Clinical and oncological outcomes of endoscopic resection of early gastric cancer (EGC), considering the indication criteria, compared to surgery, have not been reported in systematic reviews. To address the short- and long-term outcomes of endoscopic resection compared to surgery in the treatment of EGC, a systematic review was performed, establishing the available data to an unpublished 2a strength of evidence, better handling clinical practice. A systematic review and meta-analysis using Medline, Embase, Cochrane, LILACS, Scopus and CINAHL databases were done. Eleven retrospective cohort studies were selected to quantitative and qualitative synthesis. All studies included patients diagnosed with EGC that compared outcomes considering endoscopic treatment and surgery. The included records involved 2654 patients with EGC that filled the standard or expanded indications for endoscopic resection. Different endoscopic treatment modalities were analyzed, mainly mucosal resection procedures such as endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD), compared to surgery. Information of the selected studies was extracted on characteristics of trial participants, inclusion and exclusion criteria, types of interventions and outcomes (different survival rates, adverse events, complete resection, recurrence and mortality rates). The analysis of the absolute risks of the outcomes was performed using the software RevMan, by computing risk differences (RD) of dichotomous variables. Data on RD and 95% confidence interval (CI) for each outcome were calculated using the Mantel-Haenszel test and inconsistency was qualified and reported in X2 and the Higgins method (I2). Sensitivity analysis was performed when heterogeneity was higher than 50%. All pooled analyses were initially based on fixed-effects model. Three-year survival data were available for six studies (n = 1197). There were no RD in 3-year survival data after endoscopic and surgical treatment of EGC (RD = 0.01, 95% CI = -0.02 to 0.05). Five-year survival data (n = 2310) showed no evidence of a difference between the two groups (RD = 0.01, 95% CI = -0.01 to 0.03). Avaliação nutricional pré e pós-operatória de pacientes portadores de megaesôfago chagásico / Pre and postoperative nutritional evaluation in patients with chagasic megaesophagus

O megaesôfago é manifestação clínica freqüente, em pacientes na fase crônica da doença de Chagas, na região Central do Brasil. Cursa com disfagia progressiva que pode levar a desnutrição protéico-calórica. O melhor tratamento para o megaesôfago é o cirúrgico. Pacientes desnutridos têm maior probabilidade de desenvolver complicações e mortalidade pósoperatória. Megaesôfago avançado ou recidivado podem ser tratados com a operação de Serra Dória, que consiste de cardioplastia, associada à gastrectomia parcial e anastomose gastrojejunal em alça exclusa, tipo Y de Roux. Torna-se de interesse avaliar o estado nutricional de pacientes portadores de megaesôfago chagásico no pré e pós-operatório, averiguando a repercussão que o estado nutricional pré-operatório possa ter sobre a evolução cirúrgica, assim como as conseqüências nutricionais da operação de Serra Dória. No presente trabalho foi realizada avaliação nutricional préoperatória em 27 pacientes portadores de magaesôfago chagásico, candidatos à operação Serra Dória. Após o 90º dia de pós-operatório, 19 pacientes foram reavaliados. Do total de pacientes, 75% tinham magaesôfago grupo IV; o restante, megaesôfago recidivado. O estado nutricional foi avaliado por meio de avaliação nutricional subjetiva global, antropometria, exames laboratoriais, além da análise de ingestão alimentar. No período pré-operatório observou-se, em mais de 60% dos doentes, desnutrição calórico-protéica do tipo marasmática caracterizada: por redução do índice de massa corpórea; pelo déficit ponderal em relação aos valores padrões; pela depleção acentuada da prega cutânea do tríceps, com preservação da circunferência muscular do braço, da albumina e da transferrina. De acordo com a avaliação nutricional subjetiva global, 2/3 dos pacientes apresentavam desnutrição. A desnutrição diagnosticada no préoperatório pela avaliação nutricional subjetiva global associou-se com o sintoma regurgitação. O estado nutricional pré-operatório não se associou a complicação ou mortalidade pós-operatória. Na avaliação pós-operatória observou-se aumento da ingestão protéica. Houve aumento do índice de massa corpórea e da prega cutânea do tríceps, além de redução dos níveis de hemoglobina no sangue periférico. Em conclusão, pacientes com megaesôfago chagásico grupo IV e recidivado, apresentam alta freqüência de desnutrição protéico-calórica. A desnutrição pré-operatória não se associa à morbimortalidade pós-operatória. No pós-operatório os pacientes apresentam recuperação da massa gordurosa corpórea / Chagasic megaesophagus is a frequent clinical manifestation in patients in a chronic phase of Chagas disease in Central Brazil. The main sympton is progressive dysphagia that can lead to protein-calorie malnutrition. Surgical intervention is the best treatment for megaesophagus. Undernourished patients present a greater probability of developing complications and postoperative mortality. Advanced or recurrent megaesophagus can be treated with Serra Dória\'s surgical procedure, which consists of cardioplasty, associated with partial gastrectomy and gastrojejunal Roux-en-Y anastomosis. An evaluation is worthwhile of the nutritional state of patients with chagasic megaesophagus in the pre- and postoperative period, as the findings have repercussions on the surgical course. The nutritional consequences of Serra Dória operation should also be observed. In the present work, a preoperative nutritional evaluation was performed in 27 patients with chagasic megaesophagus who were candidates for the Serra Dória operation. After postoperative day 90, 19 patients were reevaluated. Of the total patients, 75% presented megaesophagus group IV and the remainder recurrent megaesophagus. The nutritional state was assessed through subjective global assessment, anthropometry and laboratorial exams, besides the analysis of alimentary ingestion. In the preoperative period it was observed that over 60% of the patients had protein-calorie malnutrition of the marasmic type, characterized by: reduction in the body mass index; ponderal index deficit in relation to the standard values; marked depletion in the skin fold of the triceps, with preservation of the muscular circumference of the arm and of the albumin and transferrin levels. In agreement with the global subjective nutritional evaluation, 2/3 of the patients presented malnutrition. The diagnosis of malnutrition in the preoperative by subjective global assessment was associated with the symptom of regurgitation. The preoperative nutritional state did not correlate with complications or postoperative mortality. The postoperative evaluation showed an increase in the ingestion of proteins. There was an increase in the body mass index and the tricipital skin fold, besides a reduction in the hemoglobin levels of the peripheral blood. In conclusion, patients with group IV and recurrent chagasic megaesophagus presented a high frequency of protein-calorie malnutrition. Preoperative malnutrition was not associated to postoperative morbidity and mortality. In the postoperative the patients presented a recovery in the body fat mass
