無 / This research examines Chinese spouses’ narratives regarding their participation in a Taiwanese organization and their experience and adaptation to a new life. Fourteen spouses, members of a Taiwanese organization called Chinese Association for Relief and Ensuing Service (CARES), completed in-depth interviews. Qualitative analysis identified prominent themes in participants’ responses in relation to three main subjects: community communications, participation and empowerment. Analysis revealed that the organization communication techniques can be included in the category of community communications. Nevertheless semi-horizontal and bottom-up approaches were identified. This condition is reflected also in the process of spouses’ participation and empowerment. The participation of women in community communications was reached only partially. The same can be said about empowerment, which was achieved mainly at the psychological level. The debate offered a list of possible factors which could be related to the partial development of empowerment and participation. They could be traced back to cultural, social, political, personal and gender factors. Suggestions which are based upon these findings of empowerment and participation are proposed to the organization and to Chinese spouses.

Patients’ and Spouses’ Perspectives on Coronary Heart Disease and its Treatment

Background: Lifestyle changes and drug treatment can improve the prognosis and quality of life for patients with coronary heart disease (CHD), but their co-operation with suggested treatment is often limited. The aim of this thesis was to study how patients and their spouses conceive CHD and its treatment. Material and Methods: The research design used was inductive and descriptive. The studies were based on three complementary sets of data. Patients with CHD (n=23) and spouses (n=25) were interviewed one year after an episode of the disease. Consecutive patients with CHD derived from another investigation were interviewed within six weeks or one year after the coronary event (n=113). All semi-structured interviews, tape-recorded or from notes taken by hand, were subjected to analysis within the phenomenographic framework. Findings: The patients’ conceptions of CHD varied and were vague, even as judged on a lay level. They were associated with symptoms rather than with the disease. Co-operation with drug treatment was rarely linked to improved prognosis. The patients’ descriptions of benefits from lifestyle changes and treatment did not give the impression of being based on a solid understanding of the importance of such changes. Incentives for lifestyle changes were classified into four categories, all of which contained both facilitating and constraining incentives. Somatic incentives featured direct and indirect physical signals. Social/practical incentives involved shared concerns, changed conditions, and factors connected with external environment. Cognitive incentives were characterised by active decisions and appropriated knowledge, but also by passive compliance with limited insights, and by the creating of routines. Affective incentives comprised fear and reluctance related to lifestyle changes and disease and also lessened self-esteem. All incentives mostly functioned facilitatively. The cognitive and the social/practical incentives were the most prevalent. Spouses’ understanding about the causes of CHD involved both appropriate conceptions and misconceptions. Drug treatment was considered necessary for the heart, but harmful to other organs. Spouses’ support to partners was categorised, and found to be contextually bound. The participative role was co-operative and empathetic. The regulative role controlled and demanded certain behaviours. The observational role was passive, compliant, and empathetic. The incapacitated role was empathetic, unable to support, and positive to changes. The dissociative role was negative to changes and reluctant to be involved in lifestyle changes. Conclusions: These results could be useful in the planning of care and education for CHD patients. The findings also emphasise the importance of adopting a family perspective to meet the complex needs of these patients and their spouses in order to facilitate appropriate lifestyle changes. / On the day of the public defence of the doctoral thesis the status of article V was Submitted.

Bakgrund: Allt fler människor insjuknar i någon form av demenssjukdom. Det medför att även anhöriga till personer med demenssjukdom stiger i antal. Anhöriga ser det ofta som naturligt att vårda en familjemedlem, men när bördan blir för tung sker ofta en flytt till ett särskilt boende. Syfte: Att beskriva anhörigas upplevelser när en närstående som har en demenssjukdoms flyttar till särskilt boende. Metod: Litteraturöversikt där kvalitativa artiklar inkluderades. Sökningarna gjordes i databaserna Cinahl, PsycINFO och Pubmed. Analysen gjordes utifrån en tre-stegs-modell. Resultatet har sammanställts och redovisats genom kategorier. Resultat: Två huvudkategorier identifierades; "Tiden före flytt" med underkategorier; "En ohållbar situation", "Att ta steget till beslutet" samt "Tiden efter flytt" med underkategorier; "Att känna skuld och skam", "Att inte kunna släppa taget", "Den förändrade relationen", "Det rätta beslutet". Anhöriga upplever processen som svår och som en process som underlättas om stöd ges från familj eller vårdpersonal. Slutsats: Resultatet kan ses som att anhöriga genomgår en transition över tid. Om anhöriga får stöd av vårdpersonal under processens gång så kan de lättare acceptera den nya tillvaron. / Background: More and more people fall ill with some type of dementia. This leads to that the relatives of people with dementia will increase in number. Relatives often see it as natural to care for a family member, but when the burden becomes too heavy it often ends in a move to nursing home. Aim: To describe relatives´ experiences when a person with dementia moves to a nursing home. Method: Literature review where qualitative articles were included. Searches were made in the databases CINAHL, PsycINFO and PubMed. The analysis was based on a three-step model. The result was then compiled and presented in different categories. Results: Relatives are experiencing a change and two main categories were identified; "The time before the move" with subcategories; "An untenable situation" and "Taking the step to the decision" and "The time after the move" with the subcategories; "To feel guilt and shame," "Not being able to let go," "The changed relationship" and "The right decision". Family members experience the process as difficult and as a process that is facilitated if support is given by family members or health professionals. Conclusion: The result can be seen as that relatives undergo a transition over time. If relatives are supported by health professionals during the process it can help them to accept the new life situation.

I chose this topic because I am interested in family law and I would like to be an attorney focusing on this area of law in future. I also think that marriage is an important legal and social institute, which proves its long-standing history. This thesis reflects the historical development of the law but it mainly deals with legislation in force. Specifically it deals with the origin of marriage, dissolution of marriage and the legal consequences that legal negotiation brings. I also pay attention to the legal regulation of rights and responsibilities of spouses. The thesis is divided into five chapters. The first chapter is dedicated to an introduction. The purpose of my thesis entitled ,,Formation and termination of marriage with regard to the proprietary aspects" is the analysis of institutions of marriage and the community property. These institutions are characterized in terms of their formation, existence and termination. Thesis abstrakt Legal institute of settlement The aim of the thesis is to characterize the legal institute of settlement from a general viewpoint, as well as to describe the most common situations where the institute of settlement is applied according to Civil Code. The thesis is composed of five chapters. The chapter One is introductory and characterizes the settlement as a process, in which the existing relations between subjects are liquidated while new ones are formed concurrently. The following chapter is divided into four subchapters relating to the settlement of common property. The first of them explains what the co-ownership means in Civil law. Then the both ways of termination and settlement of common property are described, i.e. the termination of a settlement of common property by an agreement of co-owners and by a decision of court. The last subchapter is devoted to legal regulations of co-ownership in new Civil Code which is expected to be passed. The chapter Three is focused on the settlement of community property. Obesity and U.S. military spouses: An examination of risk perception and health behavior

The military, in particular Military spouses are not exempt from the challenging issue of obesity. Understanding risk perception and health behavior is key to reducing the obesity epidemic, however insufficient research has been conducted among U.S. military spouses to understand their perceived risk and health behaviors. This quantitative study was conducted among 291 military spouses using the Extended Parallel Process Model and Social Cognitive Theory as the conceptual frameworks. The results indicated that perceived susceptibility of obesity and obesity related illness among overweight and obese military spouses while reinforcing both response and self-efficacy is the focal area for communication. Key barriers to weight loss and health goals were identified and the setting of health goals is identified as important. The researcher provides a digital intervention recommendation to address the findings of this study.

Introdução: o câncer não afeta unicamente o paciente, mas também o cônjuge. Assim, torna- se necessário identificar as demandas oriundas da doença e o tratamento, a fim de ter uma visão completa do paciente e cônjuge para aprimorar a assistência clínica e facilitar às equipes a tomada de decisão para a realização de cuidados. Ao perceber a falta de instrumentos em português brasileiro que pudessem identificar essas demandas, foi escolhido o Demands of Illness Inventory (DOII) Patient Version e Partner Version, que se caracteriza por abordar de forma abrangente as demandas, preocupações e necessidades dos pacientes com câncer e cônjuges. Objetivo: adaptar culturalmente e avaliar as propriedades psicométricas dos instrumentos DOII, na versão do paciente e esposo, em pacientes portadores de câncer e cônjuges, para o português brasileiro. Método: estudo metodológico desenvolvido em duas etapas, a adaptação cultural e a avaliação psicométrica dos instrumentos. A adaptação cultural foi a preconizada por Beaton et al. As propriedades psicométricas da versão do paciente foram avaliadas em 658 pacientes com câncer em tratamento quimioterápico de um hospital público de grande porte de São Paulo. Já a versão do esposo foi realizada em 170 esposos de pacientes da mesma instituição. Para a validade de conteúdo utilizaram-se o IVC (Indice de Validade de Conteúdo) e o coeficiente Kappa. Para a validade de construto foi realizada a Análise Fatorial Exploratória (AFE). Para a validade convergente e divergente foi utilizada a análise de correlação, na versão do paciente foi aplicado o Termômetro de Distress e o instrumento que avalia a qualidade de vida EORTC-CQC-30, respectivamente; na versão do esposo foi aplicada a Escala de Estresse Percebido (PSS-14) e o Inventário de estratégias de Coping de Folkman e Lazarus. A confiabilidade foi testada pelos coeficientes alfa de Cronbach e ômega de McDonald. Resultados: todas as etapas de adaptação cultural foram realizadas satisfatoriamente de ambas versões adaptadas. Para a validade de conteúdo, o IVC e Kappa apresentaram valores superiores a 0,75 e 0,72 respectivamente, para a maioria dos itens. Para a validade de construto, obteve-se medidas de KMO (Kaiser-Meyer-Olkin)=0,930 e 2 de Bartlett = (8256) 47209,9; p < 0,001. A AFE mediante a análise paralela foram retidos 11 fatores que explicam 46,5% da variância total do fenômeno de estudo. A confiabilidade para a escala total foi de 0,961 para o alfa de Cronbach e 0,952 para ômega de McDonald. Na versão do esposo, a validade de construto obteve indicadores de KMO= 0,818 e 2 de Bartlett= (528) 2414,1, p < 0,001. De acordo com a análise paralela 4 fatores foram retidos que explica 52,9% da variância total do fenômeno de estudo. A confiabilidade para a escala total foi de 0,894 para o alfa de Cronbach e 0,876 para ômega de McDonald. Na versão do paciente, as correlações das validades convergente e divergente foram r=0,605 (p<0,01) e r=0,660 (p<0,01), respectivamente. Na versão do cônjuge, foram r=0,334, p0,01 e r=0,260, p0,01, respectivamente. Conclusões: o DOII, na versão do paciente e esposo, mostrou-se adaptado culturalmente a pacientes com doenças crônicas como o câncer no contexto brasileiro e aos cônjuges, apresentando evidências de validade de conteúdo, construto e confiabilidade. Recomenda-se realizar a Análise Fatorial Confirmatória. / Introduction: cancer affects not only the patient, but also the spouse. Thus, it becomes necessary to identify the demands arising from the disease and the treatment, in order to have a complete view of the patient and spouse to improve clinical care and facilitate the decision- making of the healthcare teams. By observing the lack of instruments in Brazilian Portuguese that could identify these demands, the Demands of Illness Inventory (DOII) - Patient Version and Partner Version has been chosen, which is characterized by comprehensively addressing the demands, concerns and needs of cancer patients and spouses. Objective: to culturally adapt and to evaluate the psychometric properties of the DOII instruments, in the patient and in the spouses version; in patients with cancer and their spouses, in Brazilian Portuguese. Method: A methodological study developed in two stages, the cultural adaptation and the psychometric evaluation of the instruments. The cultural adaptation has been advocated by Beaton et al. The psychometric properties of the patient\'s version have been evaluated in 658 cancer patients in chemotherapy treatment of a large public hospital in São Paulo. The spouse\'s version has been used in 170 spouses of patients from the same institution. For the content validity, the CVI (Content Validity Index) and the Kappa coefficient have been used. For the construct validity, the Exploratory Factor Analysis (EFA) has been performed. For the convergent and divergent validity, the correlation analysis has been performed in the patient\'s version with the Distress Thermometer and the EORTC-CQC-30 instrument, respectively; in the spouse\'s version, with the Perceived Stress Scale (PSS-14) and the Folkman and Lazarus Coping Inventory, respectively. Reliability has been tested by Cronbach\'s alpha and McDonald\'s omega coefficients. Results: all the stages of cultural adaptation have been satisfactorily performed in both adapted versions. For the content validity, IVC and Kappa have presented values higher than 0.75 and 0.72, respectively, for most items. For the construct validity, the following KMO (Kaiser-Meyer-Olkin) measurements have been obtained= 0.930 and Bartlett\'s 2= (8256) 47209.9; p <0.001. The EFA, through the parallel analysis, has comprised the 11 retained factors that account for 46.5% of the total variance. The reliability for the full scale was 0.961 for Cronbach\'s alpha and 0.952 for McDonald\'s omega. Realizou-se um estudo quantitativo com delineamento descritivo e comparativo. A amostra foi constituída por 286 sujeitos casados ou em união estável com filhos de 0 a 18 anos. Os participantes responderam individualmente ao protocolo on line constituído por um questionário sócio demográfico, a Escala Marital Inventory Communication, a Escala Dutch Marital Satisfaction and Communication Questionnaire, Escala de Avaliação da Coesão e Adaptabilidade Familiar, e a Escala de Ajustamento Diádico. Esta dissertação está organizada na forma de dois artigos empíricos: “Comunicação conjugal em diferentes etapas do ciclo de Vida e “Padrões de Comunicação conjugal em homens e mulheres.” Os resultados encontrados indicam que a conjugalidade se sobrepõe a parentalidade, visto que a idade dos filhos não teve interferência na comunicação, e também pontuam que homens e mulheres se comunicam de formas iguais, mas cada um com sua especificidade. Estes achados apontam que as variáveis estudadas têm grande importância para o estudo das relações conjugais e para o entendimento deste fenômeno. / The aim of this dissertation is to evaluate communication (communication levels and styles) between spouses with different levels of functionality (adjustment, cohesion and adaptability) in different stages of marital life cycle (toddlers, school age, teenagers). A quantitative study with descriptive and comparative design was held. The sample was composed by 286 married people or in a common law marriage with children aged between 0 and 18 years old. The participants answered the online protocole individually composed by a socio-demographic questionnaire, the Marital Inventory Communication Scale, Dutch Marital Satisfaction Scale, Communication Questionnaire, Family Adaptability and Cohesion Evaluation Scale, Dyadic Adjustment Scale. This dissertation is organized in a frame of two empirical articles: “Marital Communication in different stages of life cycle” and “Positive and Negative Communication: Differences between men and women”. The found findings point that the studied variables have great importance to the study of conjugal relationships and for the clinical understanding of this phenomenon.
