Visuomenės senėjimas rodo gerėjančias gyvenimo sąlygas, sveikatos priežiūros kokybės gerėjimą, tačiau visuomenės senėjimas turi ir neigiamų pasekmių. Visuomenei senstant daugėja lėtinių ligų, specifinių, seniems žmonėms būdingų būklių, todėl didėja poreikis pagyvenusių žmonių socialinei ir medicininei priežiūrai, atsiranda naujų ekonominių iššūkių šaliai. Pakeisti ar sureguliuoti gyventojų senėjimo procesus ir jų padarinius yra sunku, tačiau žinant sveikatą įtakojančius veiksnius ir jų padarinius galima geriau pasirengti šių problemų sprendimui. Tyrimo tikslas: įvertinti 65 m. ir vyresnių Vilniaus gyventojų gyvenseną ir subjektyvų sveikatos vertinimą Uždaviniai: 1. Nustatyti 65 m. ir vyresnių žmonių gyvensenos veiksnių (rūkymo, alkoholio vartojimo, mitybos ir fizinio aktyvumo) paplitimą. 2. Įvertinti 65 m. ir vyresnių žmonių sveikatos būklę 3. Nustatyti 65 m. ir vyresnių žmonių subjektyvų savo sveikatos būklės vertinimą. 4. Įvertinti 65 m. ir vyresnių žmonių subjektyvios sveikatos būklės sąsajas su gyvensenos veiksnių paplitimu. 5.Įvertinti 65 m. ir vyresnių žmonių gyvensenos sąsajas su bendra fizine sveikata. Metodika. Darbo uždaviniams įgyvendinti ir tikslui pasiekti buvo panaudotas interviu apklausos metodas, naudojant šiam tyrimui sukurtas anketas (1 priedas). Anketos buvo anonimiškos. Because of aging, both the chronic diseases and specific conditions for the elderly are increasing, thus making bigger demands of older people for social and medical care and creating new economic challenges for the country. It is very difficult to replace or adjust the aging processes and their effects; however, if we take into account the factors that have an impact on health and their consequences the better preparation to solve these problems can be done. 臺灣飲酒家庭特性的長期變遷分析 / A Study on Characteristics of Alcohol-drinking Households in Taiwan

依衛生福利部國民健康署指出,飲酒會導致如車禍、家暴、肝癌及肝硬化等事故與疾病,這些都是少量卻有顯著危害。目前臺灣的酒品隨處可購得,但國人及整個社會卻尚未學會到如何正確與「酒」相處;而臺灣特有的拚酒文化更是常拚出一身病來,許多人因酒而失業、家庭失和,甚至喪命。更可怕的是,飲酒還會造成無辜大眾平白犧牲。有鑒於飲酒過量所帶來的危害,酒害的防制已是世界各國共同的議題。 本研究從長時間來回顧臺灣飲酒消費的情況及變化,同時觀察臺灣飲酒家庭戶長與家庭成員的特性,及各縣市飲酒消費的情形,以作為我國未來在制定酒害防制政策時之參考。本文利用行政院主計總處提供之1990至2012年「家庭收支調查報告」資料來進行分析,結果發現飲酒消費支出金額與所得呈現正向關係,惟所得低的家庭,其飲酒消費支出占可支配所得的比重,較所得高的家庭大。另飲酒消費支出較高的家庭,其教育程度及平均年齡相對較低,且男性成員比率偏高,倘若針對教育程度較低者、年輕及男性族群,透過教育及政策行銷,加強渠等對飲酒危害的風險認知,進而減少飲酒量,將有利提升國民健康。 / It is published that traffic accidents, certain social problems or body illnesses are often the results of alcohol drinking events; such as car accident, family abuse, liver cancer or cirrhosis etc., reported by Health Promotion Administration Ministry of Health and Welfare. Such alcohol problems are rare, but serious once happened. Alcohols can be bought everywhere in Taiwan, but still Taiwanese don’t know how to deal with drinking. One of the Taiwanese drinking features, the bottom-up, also adds to the alcohol problems. Many unemployment, family quarrels or even the human lives are caused or jeopardized by over-drinking. Sometimes, even an innocent people could be hurt through a drinking event. Due to the endangerment that is developed by over drinking, that how to prevent it is now a common issue to discuss with all over the world. This essay examines the long-term alcohol consumption changes in Taiwan, observes, at the same time, the characteristics of alcohol-drinking families and the family head of the household, and compares the alcohol consumption situation in different town, county areas in Taiwan. Based on the data form the year 1990 to 2012 in the “Report on the Survey of Family Income and Expenditure in Taiwan Area”, it is found that the expenditure on alcohols and the income of a family are positively related. If we compare the alcohol expenditure to the total disposable income, it is found that the ratio is higher for the poor family than those rich ones. For those families with higher alcohol expenditures, we find their family members are comparatively younger, less educated, and consisted of more male than female members. If we aimed at those classified groups of less educated, younger and male to strengthen their awareness of the alcohol risks through education and policy advocacy to reduce their drinking, our national health will be enhanced.

Consommation de substances psychotropes et violence chez les jeunes décrocheurs canadiens : analyse des liens distaux (capital social, familial, délinquant et individuel)

Le présent mémoire explore les liens entre les différents types de capitaux (social, familial, délinquant et individuel) et certains actes déviants, soit la consommation de substances psychotropes et l’implication criminelle violente chez un groupe de décrocheurs scolaires canadiens. Dans un premier temps, il s’agit d’établir la prévalence et les habitudes de consommation de cette population aliénée du système éducatif. De plus, cette étude concerne l’implication criminelle violente des décrocheurs. Plus précisément, il s’agit de déterminer la fréquence des manifestations agressives et les types de violence perpétrés par ces jeunes, ainsi que d’examiner les liens qui se tissent entre la consommation de substances psychotropes et la commission d’actes violents. Ensuite, il est question d’étudier l’impact des différents capitaux (social, familial, délinquant et individuel) sur la consommation de substances psychoactives et l’implication criminelle violente des décrocheurs. En outre, dans une perspective davantage clinique, le dernier objectif aura pour but d’identifier différentes typologies de décrocheurs scolaires. Les analyses s’appuient sur un échantillon de 339 jeunes décrocheurs scolaires de Montréal et Toronto. Les informations amassées par rapport à l’usage de substances psychotropes et la commission d’actes violents concernent les douze mois qui ont précédé la passation du questionnaire. Succinctement, les taux de prévalence de consommation des décrocheurs apparaissent plus importants que ceux de la population estudiantine, leur usage est plus inquiétant de même que l’auto-évaluation de leur dépendance. Les résultats révèlent également une implication criminelle violente importante, surtout chez les garçons et les consommateurs de substances psychotropes. Qui plus est, le capital délinquant semble avoir un impact majeur sur l’usage d’alcool et de drogues de même que sur les manifestations de violence perpétrées par les décrocheurs. Enfin, trois typologies de décrocheurs scolaires ont été identifiées, soit les invisibles, les détachés et les rebelles. / This paper explores the links between different types of capital (social, familial, criminal and individual) and some deviant acts, consumption of psychotropic drugs and violent criminal involvement among Canadian school drop-outs. In first place, the prevalence and habits of alcohol and drug consumption of this population alienated from the education system will be established. Moreover, this study concerns violence involvement of school drop-outs. More specifically, in determining the frequency of aggressive events and types of violence perpetrated by these young people, and to examine the relationships between the use of psychotropic substances and the commission of violent acts. Then, there is the impact of various capitals (social, familial, criminal and individual) on the consumption of psychoactive substances and violent criminal involvement of school drop-outs. In a more clinical perspective, the last goal will be to identify different types of school drop-outs. This analysis is based on a sample of 339 young school leavers in Montreal and Toronto. The information collected in relation to the use of psychotropic substances and the commission of violent acts involve the twelve months preceding the award of the questionnaire. Briefly, the prevalence of consumption of drop-outs is more important than the student population; their use is even more disturbing as well as the self-assessment of their dependency. The results also show a significant violent criminal involvement, especially among boys and consumers of psychotropic substances. Moreover, the criminal capital appears to have a major impact on the use of alcohol and drugs as well as the manifestations of violence perpetrated by drop-outs. Finally, three types of school dropouts have been identified: the invisible, the detached and the rebels.

Rôles sociaux et consommation d’alcool des femmes et des hommes au Canada : une analyse située du rôle parental

Cette thèse de doctorat a pour objectif d’examiner de quelles manières le rôle parental influence la consommation d’alcool des femmes et des hommes et plus précisément, si les relations dynamiques entre le rôle parental, les circonstances au sein desquelles il est mis en acte et les contextes immédiats de consommation permettent d’expliquer les différentes façons individuelles de consommer de l’alcool. Cette étude repose sur le constat qu’en alcoologie, il existe une tendance à considérer l’acteur et l’action comme étant détachés de leur cadre social immédiat. Conséquemment, il existe des limites importantes aux modèles permettant d’expliquer les mécanismes par lesquels les rôles sociaux influencent la consommation d’alcool. Afin d’avoir une meilleure compréhension sociologique de la consommation d’alcool, cette thèse propose un cadre théorique qui insiste fortement sur la nécessité de tenir compte de la situation. L’acteur agit en fonction de certaines circonstances (perception de conflit de rôles) au sein desquelles son rôle est mis en acte. L’action de boire est dépendante du contexte (caractéristiques spatiales, temporelles, symboliques et relationnelles) au sein duquel l’action se déroule. L’hypothèse générale de recherche stipule que pour comprendre la relation entre les rôles et la consommation d’alcool, il faut situer à la fois l’acteur et l’action. La validité empirique du cadre théorique a été testée à partir d’une analyse quantitative des données de l’enquête GENACIS Canada (GENder Alcohol and Culture: an International Study) ainsi que des données de l’Enquête sur les Toxicomanies au Canada. La présentation des résultats des analyses prend la forme de trois articles soumis pour publication. Les données analysées révèlent le bien-fondé du cadre de théorique proposé. Situer l’action a permis de constater que les contextes de consommation sont un médiateur de la relation entre le rôle parental et la consommation d’alcool et plus spécifiquement, que les parents boivent moins fréquemment de façon excessive que les non-parents parce qu’ils boivent dans des lieux différents. Situer l’action a aussi révélé que les femmes et les hommes ont tendance à adopter des comportements de boire qui s’accordent au contexte immédiat, plutôt qu’à la position qu’ils occupent. Par contre, observer les circonstances individuelles au sein desquelles le rôle parental est mis en acte n’a pas permis d’améliorer notre compréhension de la relation à l’étude. Les évidences scientifiques apportées par cette thèse de doctorat ouvrent la porte au développement de mesures préventives environnementales qui visent le contexte de l’action plutôt que l’acteur, pour limiter la consommation excessive d’alcool des femmes et des hommes. / The objective of this doctoral thesis is to examine how the parental role influences women and men’s alcohol consumption. More precisely, it explores whether the dynamic relationships between the parental role, the circumstances into which it is enacted and drinking contexts provide an explanation to the various individual manners to consume alcohol. This research is based on the observation that within the alcohol field, there is a tendency to treat social actor and the social action as if they were detached from their immediate social environment. Therefore, existing models that focus on the mechanisms through which social roles influence alcohol consumption are limited. To achieve a better sociological understanding of alcohol consumption, this thesis proposes a theoretical framework that highlights the necessity to take into account the situation. A social actor behaves according to certain circumstances (between-role stressors) into which his/her role is enacted. Drinking depends on the contexts (spatial, temporal, symbolic and relational characteristics) in which it occurs. The general hypothesis of this research postulates that to understand the association between social roles and alcohol consumption, both the social actor and social action need to be situated. The empirical validity of the theoretical framework has been tested from quantitative analyses of data from the GENACIS Canada (GENder Alcohol and Culture: an International Study) survey and data from the Canadian Addiction Survey. The results of these analyses are presented in three articles submitted for publication. The analyses reveal the legitimacy of the proposed theoretical framework. Situating the act of drinking has allowed to observe that drinking contexts are mediators of the relationship between the parental role and alcohol consumption and more specifically, that parents report to less frequent abusive drinking than non-parents because they drink in different locations. Situating the act of drinking has also revealed that men and women tend to consume alcohol in accordance with the immediate drinking context rather than their positional role. However, observing the circumstances into which individual enact their parental role has not improved our understanding of the relationship under study. The scientific evidences provided by this doctoral thesis open the door to the development of preventive environmental measures that focus on the immediate drinking context instead of the individual in order to reduce abusive drinking behaviors among both men and women.

Perceptions et discours d’adolescents en difficultés de comportement face à leur consommation d’alcool et de drogue

Plusieurs travaux de recherche ont porté sur la consommation des adolescents. À partir d’une analyse secondaire qualitative de 30 entrevues menées auprès d’adolescents présentant des difficultés de comportements, ce mémoire vise à explorer le discours d’adolescents qui présentent des difficultés de comportements par rapport à leur perception de leur consommation et aux facteurs qui ont pu l’influencer. Le premier chapitre de ce mémoire rend compte de la diversité des facteurs d’influence jouant un rôle dans la consommation de drogue ou d’alcool d’adolescents qui présentent des difficultés de comportements selon la littérature recensée. Dans un premier temps les différents contextes de vie des jeunes sont explorés. L’approche bioécologique a pour avantage de replacer l’adolescent dans son environnement global incluant les différents niveaux d’influence. En outre, les représentations sociales, permettent de mettre en avant la perception des principaux concernés. Le deuxième chapitre consacré à la méthodologie, se base sur une collecte de données qualitatives. Le troisième chapitre expose les résultats de cette recherche qui révèlent que les jeunes participants à cette étude sont en mesure d’identifier des influences dans les différents niveaux, même si le macrosystème n’est pas présent dans leur discours. Sur le plan de la fréquence de consommation, deux profils de consommation se distinguent, l’un concerne les jeunes qui ont commencé à consommer plus tôt et dont la consommation a diminué et ceux qui débuté leur consommation plus tard et dont la consommation a augmenté. Le quatrième chapitre de ce mémoire expose de manière schématique les écrits scientifiques avec le discours des adolescents et identifie également quelques notions clefs qui ressortent du discours des adolescents. / Adolescent drug and alcohol consumption is a major concern, and the focus of much research. This paper aims to explore adolescent perception of, and influences on, drug and alcohol consumption, through a qualitative, secondary analysis of 30 interviews of adolescents with behaviour problems. The first chapter of this report reviews the literature on the various factors influencing drug and alcohol consumption in teens with behavior difficulties, including the different environments in which they live. Two theoretical constructs are used to explore adolescent consumption: the bioecological approach, which positions the teenager in the context of his or her global environment; and the theory of social representations, which highlights the internal perceptions of those concerned. The second chapter of this paper describes the qualitative methodology used in the collection and analysis of data. The third chapter presents the results of this research, which reveal that the young participants were able to identify the different factors influencing their consumption in various levels of the different contexts, even if they did not overtly describe the macrosystem. The results indicate two different profiles of consumption: one for young people who start using earlier and then later decrease their consumption; and one for those who start later, and whose consumption has continued to increase. In terms of the frequency of use, many participants in this study follow the former profile, thus they have already started reducing their consumption. Participant speech is similar to what is indicated in the literature. The fourth chapter of this study schematically presents what is indicated in the scientific literature along with what the participants say, and also identifies some of the key concepts that emerge from the adolescents’ descriptions. The paper concludes with some reflections on practice and ideas for future research.

School-based Interventions or Prevention Programs regarding Alcohol, Smoking and Drug Use among Adolescents with Disabilities or Physical Impairments : A Systematic Literature Review

Introduction Substance use in adolescents with disabilities is rising, containing the prevalence of substance-related disorders (SRD) such as addiction, mental or health disorders, cancer, accidents and mortality. Yet little is known about the existing substance use prevention programs among adolescents with dosabilities or physical impairments. The aim of this systematic review was to investigate the effects of school-based interventions or prevention programs directed at the reduction of alcohol, tobacco and drug use in young adolescents with disabilities or physical impairments. Method Five scientific databases were explored mainly for school-based randomized controlled trials (RCTs) and prevention programs examining the effects of substance use interventions and prevention programs on adolescents with disabilities or physical impairments. Guided by the NICE guidelines, eligible articles were detected from which data were collected. A systematic literature review was performed for many diverse outcomes such as, substance use knowledge, substance use, modelling social environment, intention to quit smoking, peer pressure, etc. Results The primary literature search resulted in 821 articles. Five studies were incuded in the systematic literature review. Most of the collected studies were about adolescents with intellectual disabilities (MBID or MMID). The review's sample group ranged from 12-to 18-year-old adolescents. Included studies had a total sample of 981 out of which 13 were teachers. Studies measured both primary and secondary outcomes like modelling smoking, substance use and frequency of alcohol use. Conclusion This review summarized evidence about interventions and prevention programs aimed at decreasing or preventing substance use in adolescents with various types of disabilities or physical impairments. Substance use education increases knowledge about alcohol, tobacco and drug use and the health-related harms in teenagers with disabilities. Additional research is required especially among teenagers with intellectual disabilities and other types of disabilities.

An investigation into digital alcohol marketing and user-created alcohol promotion, and the association with young adult's alcohol-related knowledge, attitudes, and behaviour

Context: There are two ways that digital media may influence alcohol use. The first is commercial alcohol marketing. The second is user-created alcohol promotion, defined as content distributed through new media that promotes consumption, but independent of commercial marketing. This thesis explores how both types of content promote alcohol, what association there is between exposure and alcohol-related attitudes and behaviour, and the differences between marketing and user-created promotion. Method: A mixed method design was employed, divided into two studies. The first was a content analysis of the design features, topical references, and messages suggested about alcohol in digital marketing and user-created promotion on Facebook, Twitter, and YouTube. The second was a cross-sectional survey with young adults (n = 405). This measured awareness of, and participation with, digital marketing and user-created promotion, and the association with consumption, higher-risk drinking, brand recall, expectancies, and drinking motives. Results: The content analysis found that digital marketing had personalised designs which contained subtle and positive messages about consumption, whereas user-created promotion had simpler designs, displayed little ethical practice, and contained overt messages about higher-risk drinking. The cross-sectional survey found that young adults were aware of, and participating with, both digital marketing and user-created promotion, with exposure greater for the latter. Exposure to both types of content was positively associated with alcohol use, higher-risk consumption, and drinking intentions. User-created promotion had a stronger association with all outcomes than marketing. The association between exposure and consumption, for both types of content, was mediated through drinking motives and expectancies. Conclusion: Young adults are aware of, and participating with, a range of digital marketing and user-created promotion. That such exposure is associated with alcohol-related attitudes and behaviour highlights the potential of new media to influence alcohol consumption. Further research is required to better understand young people’s experience with digital media and the challenges of addressing online health risk messages.

Consommation chronique d'alcool, exercice physique et tissu osseux : modifications densitométriques, architecturales, biomécaniques et métaboliques chez le rat / Chronic alcohol consumption, physical exercise and bone tissue : densitometric, microarchitectural, biomechanic and metabolic changes in the rat

La consommation d’alcool a des effets sur le tissu osseux. L’alcoolisme est une des causes d’ostéoporose secondaire chez l’homme. Dans ce travail nous avons mené différentes expérimentations chez le rat afin d’étudier les effets d’une consommation chronique d’alcool combinée ou non à un entraînement aérobie sur le tissu osseux. Nous avons montré qu’une faible dose d’alcool administrée pendant une période courte peut avoir un effet positif sur la densité minérale osseuse et l’épaisseur trabéculaire. En revanche, la combinaison activité physique et consommation modérée d’alcool n’a pas d’effet additif sur la potentialisation du tissu osseux. Nous avons également démontré un effet dose de l’alcool indiquant des effets délétères majorés sur la densité minérale osseuse (DMO), la microarchitecture corticale et la résistance osseuse avec des apports croissants (25%, 30% et 35% v/v). La modification de DMO s’accompagne d’un changement de composition corporelle et d’une diminution de la leptine systémique. Cependant, le nombre d’adipocytes augmente dans la moelle osseuse. Nous avons mis en évidence dans ce modèle d’ostéoporose secondaire due à l’alcool une augmentation de l’apoptose des ostéocytes, corrélée à la diminution de la DMO et à l’augmentation de l’adiposité médullaire. Nous avons de plus mis en évidence une incorporation de lipides dans les ostéocytes, incorporation fortement corrélée à l’apoptose de ces cellules. Enfin, nos résultats montrent qu’un exercice physique régulier combiné à une consommation chronique et excessive d’alcool permet de prévenir les effets délétères de l’alcool sur les paramètres osseux (porosité corticale, épaisseur corticale) et limite la diminution de la DMO. Cette diminution est associée à une régulation de l’apoptose des ostéocytes. / Heavy chronic alcohol consumption has deleterious effects on bone tissue. It is one of the major causes of secondary osteoporosis in men. In this work, we draw several experimentations to assess the effects of chronic alcohol consumption on bone, combined or not to an aerobic training in the rat. We showed that light to moderate chronic alcohol consumption during a short time lead to an increase of bone mineral density (BMD) and trabecular thickness, whith no additive effects of physical exercise on bone tissue. When the alcohol doses were increased, we showed deleterious effects on BMD, microarchitecture, bone resistance with a dose effect with increasing alcohol doses (25%, 30% and 35% v/v): the more alcohol was concentrated and the more the bone parameters were decreased. The BMD decrease was associated with a change in body composition, and with a decrease in serum leptin. However, the number of lipid droplets in the bone marrow was increased dramatically. We demonstrated that there was a huge increase in osteocyte apoptosis with alcohol (35% v/v) in this alcohol-induced osteoporosis model, which was correlated with BMD and bone marrow adiposity. We have also shown that there was lipid incorporation in bone micro vessels and in osteocytes, which was correlated with osteocyte apoptosis. Lastly, we showed that when regular exercise was associated with heavy chronic alcohol consumption, the bone parameters were normal (trabecular, cortical thickness, femur length) and the BMD was less decreased compared to alcohol-fed and sedentary rats. These effects were associated with a regulation of osteocyte apoptosis.

The relevant theoretical aspects of the business valuation are described in the theoretical part, with the focus on the introduction of categories of values, the valuation process, setting the discount rate and the basic forms of DCF models. Patrons de consum d'alcohol en estudiants universitaris

L’objectiu general d’aquesta tesi és conèixer la prevalença i el patró de consum d’alcohol d’una mostra de 2,137 estudiants de la Universitat de Girona (17-35 anys). S’utilitzà un disseny multimètode integrant la metodologia quantitativa (estudi d’enquesta) amb la metodología qualitativa (grups focals). Un 65% dels joves són consumidors habituals d’alcohol. La influència social de pares, germans i amics és una variable crucial en la iniciació i manteniment del consum d’aquesta droga. El consum habitual és més freqüent entre aquells que van experimentar més precoçment amb la substància. Entre els homes és més habitual el consum massiu, però més dones efectuen consums considerats d’alt risc. Els joves reconeixen algunes repercussions socials de l’abús de l’alcohol però no en perceben per a la salut física. Aquests resultats mostren la necessitat de: endarrerir l’edat de primera experimentació, informar del risc del consum d’alcohol i fomentar un oci nocturn alternatiu i saludable. / The main objective of this thesis was to determine the prevalence and patterns of alcohol consumption in a sample of 2137 college students from Girona (17-35 years old). A multimethod design using a quantitative methodology (survey) and qualitative methodology (focus groups) was employed. 65% of the participants were regular users of alcohol. The social influence of parents, siblings and friends was a crucial variable in the initiation and maintenance of alcohol use. In addition, regular use was more common amongthose who were younger when they first experimented with the substance. Binge drinking was more common among men, while high risk alcohol consumption was more common among women. However, participants acknowledged negative social consequences of alcohol abuse but they did not perceive any physical consequences. These results show the need to: delay the age of first experimentation, inform of the risks of alcohol consumption and promote a healthy alternative nightlife.
