Abordagens diagnósticas na avaliação da gravidade dos sintomas clínicos respiratórios em pacientes com sequência de Robin

Introdução: Uma série de aspectos diagnósticos e terapêuticos relacionados à Sequência de Robin (SR) não possuem definição clara na literatura, conforme demonstrado em revisão sistemática presente nesta tese. Essas áreas de incerteza envolvem os métodos de classificação do grau de glossoptose por meio da endoscopia de via aérea (EVA), a associação com manifestações clínicas e a avaliação polissonográfica. Objetivos: Avaliar, a partir de um estudo transversal aninhado em uma coorte, a associação entre duas classificações de glossoptose e a severidade dos sintomas em pacientes com SR, além de determinar a acurácia dessas classificações na determinação de pacientes com maior gravidade clínica. Avaliar a associação entre parâmetros de polissonografia e graus de gravidade de manifestações clínicas. Métodos: Os pacientes com diagnóstico de SR tiveram suas manifestações clínicas classificadas conforme Cole et al. Foram submetidos à endoscopia do sono e as imagens foram classificadas de acordo com Yellon e De Sousa et al por pesquisador cegado. Os pacientes que não necessitavam de suporte ventilatório foram submetidos à polissonografia. Resultados: Os resultados obtidos foram apresentados em quatro trabalhos: Artigo 1, aceito para publicação: revisão sistemática; Artigo 2, publicado: Associação entre classificação endoscópica da glossoptose e manifestação clínica grave: neste trabalho, um total de 58 pacientes foram incluídos. A probabilidade de apresentar sintomas graves conforme classificado por Cole et al foi maior nos pacientes Yellon grau 3 (68,4%, P=0,012) e de Sousa et al moderado e grave (61,5% e 62,5%, respectivamente, P=0,015) do que nos graus mais leves de obstrução; Artigo 3, submetido: Performance diagnóstica das classificações endoscópicas: neste trabalho foram incluídos 80 pacientes. A sensibilidade (Y: 56.2% x S: 28.1%, P<0.001) e a especificidade (Y: 85.4% x S: 93.8%, P=0.038) na identificação de sintomas clínicos graves foram estatisticamente diferentes entre as classificações de Yellon e de Sousa et al. Calculou-se a Razão de Chances Diagnóstica para Yellon (RCD: 7,53 95%CI 4.15-10.90) e de Sousa (RCD: 5,87 95%CI 1.86-9.87). As diferenças encontradas não foram significativas (P=0,92); Artigo 4, submetido: Associação entre parâmetros polissonográficos e as manifestações clínicas conforme Cole et al. Determinou-se a Razão de Chances para cada variação nos seguintes parâmetros: índice de dessaturação (1.27; 1.07-1.51; R2=19.8%; P=0.006), índice de apneia e hipopneia (1.13; 1.01-1.26; R2=12.5%; P=0.02), média de saturação de oxigênio (0.16; 0.05-0.52; R2=22.6%; P=0.002), nadir de saturação (0.73; 0.56- 0.96; R2=10.0%; P=0.02), porcentagem de tempo com saturação menor que 90% (9.49; 1.63- 55.31, R2=37.6%; P=0.012) e porcentagem de tempo com obstrução (2.5; 1.31-4.76; R2=25.1%; P=0.006). Conclusões: As alterações na EVA estão associadas com a gravidade das manifestações clínicas. Ao se aprofundar a abordagem de análise sob uma perspectiva diagnóstica, demonstrou-se que as classificações de Yellon e de Sousa et al possuem uma baixa sensibilidade, porém uma alta especificidade. A porcentagem do tempo com saturação menor que 90%, porcentagem do tempo apresentando obstrução e média de saturação de oxigênio durante o sono foram os parâmetros polissonográficos com a maior associação com as manifestações clínicas. Assim, recomenda-se que a avaliação endoscópica da via aérea em pacientes portadores de SR seja realizada com intuito de estratificar a gravidade, mas não para triagem diagnóstica. Da mesma forma, recomenda-se que os parâmetros acima destacados da polissonografia, sejam especialmente considerados no acompanhamento desses pacientes. / Introduction: Several diagnostic and therapeutic aspects concerning Robin Sequence (RS) are not clearly defined in medical literature, as demonstrated in a systematic review included in this thesis. These areas of uncertainty comprehend methods of glossoptosis degree classification, its association with clinical manifestations and polysomnographic evaluation. Objectives: A cohort nested cross-sectional study was done to evaluate the association of two different glossoptosis classifications with symptom severity in RS patients, while determining its accuracy for the identification of severely symptomatic patients. The study also aimed to evaluate the association between polysomnographic parameters and clinical symptom severity. Methods: RS patients had their clinical manifestations classified according to Cole et al. They were also examined with flexible fiberoptic laryngoscopy (FFL) and recordings were classified according to Yellon and De Sousa et al classifications by a blinded researcher. Those patients not needing ventilator support underwent polysomnographic testing. Results: Overall results were divided into four distinct articles: Article 1, accepted for publication: systematic review; Article 2, published: Association between glossoptosis endoscopic classification and severe clinical manifestations: in this article, a total of 58 patients were enrolled. The probability of presenting severe clinical findings according to Cole et al classification was higher in patients classified as Yellon grade 3 (68.4%, P=0.012) and De Sousa et al moderate or severe levels (61.5% and 62.5%, respectively, P=0.015) than in milder degrees of obstruction. Article 3, submitted: Diagnostic performance of endoscopic classifications: in this article, additional 22 patients were enrolled, summing up 80 patients. The sensitivity (Y: 56.2% x S: 28.1%, P<0.001) and specificity (Y: 85.4% x S: 93.8%, P=0.038) in the identification of severe clinical symptoms were statistically different between classifications of Yellon and De Sousa et al. A Diagnostic Odds Ratio was computed for Yellon (DOR: 7.53 95%CI 4.15-10.90) and De Sousa et al (DOR: 5.87 95%CI 1.86-9.87). No relevant differences were found between them (P=0.92). Article 4: submitted: Association between polysomnographic parameters and obstructive airway symptoms: odds ratios for severe clinical findings were computed for the variation of polysomnographic parameters: Dessaturation Index (1.27; 1.07-1.51; R2=19.8%; P=0.006), Apnea/Hypopnea Index - AIH - (1.13; 1.01-1.26; R2=12.5%; P=0.02), Sleep Mean Oxygen Saturation (0.16; 0.05-0.52; R2=22.6%; P=0.002), Oxygen Saturation Nadir (0.73; 0.56-0.96; R2=10.0%; P=0.02), Percentage of time under oxygen saturation of 90% (9.49; 1.63-55.31, R2=37.6%; P=0.012) and Percentage of Time Presenting Obstruction (2.5; 1.31-4.76; R2=25.1%; P=0.006). Conclusions: FFL findings seem to have a fair association with the severity of clinical manifestations. However, this association was found to be probably a major influence of a high specificity, while observed sensitivity was less than would be desirable for diagnostic purposes. Percentage of time under 90% oxygen saturation, percentage of time with obstruction and mean oxygen saturation during sleep were the most associated parameters with clinical manifestations. Therefore, based on these findings, we would argue that airway endoscopic examination in RS patients should be approached aiming at clinical severity stratification, rather than for screening purposes. Also, we would recommend that those aforementioned parameters from polysomnography should be specifically addressed in the follow up of this peculiar group of patients.

Avaliação polissonográfica e endoscópica em crianças com sequência de Robin isolada submetidas a palatoplastia / Polysomnographic and endoscopic evaluation of children with isolated Robin sequence submitted to cleft palate repair

Introdução: A sequência de Robin (SR) é definida por retromicrognatia e glossoptose que levam à dificuldade respiratória de deglutição, com ou sem fissura palatina. Com espectro etiológico amplo e associações sindrômicas diversas, a expressão clínica da SR se torna heterogênea. Há alta prevalência de síndrome da apneia obstrutiva do sono (SAOS), 40 a 60% nesta população. Conforme os protocolos atuais, o tratamento depende da gravidade dos sintomas e dos achados da endoscopia de via aérea superior (VFL). O momento ideal para o reparo cirúrgico do palato é controverso. Técnicas acessórias são fundamentais para o diagnóstico e avaliação prognóstica. No Hospital de Reabilitação de Anomalias Craniofaciais de Bauru (HRAC-USP) a palatoplastia é adiada em pacientes SR pelo conhecido risco para desenvolvimento de SAOS pós-operatória. Ainda há pouca informação na literatura quanto às interações entre o tipo de fissura palatina, os achados da VFL e polissonografia (PSG), com o risco de comprometimento tardio da via aérea superior (VAS) após a cirurgia palatina em crianças com SR isolada (SRI). Objetivos: Avaliar por meio de PSG o efeito da palatoplastia na SAOS em crianças com SRI. Correlacionar estes achados com os obtidos por VFL e exame físico do palato. Detectar a frequência de SAOS nesta população. Estabelecer a importância da PSG como parte de protocolo de tratamento da fissura palatina em crianças com SRI. Métodos: Estudo prospectivo longitudinal que, entre abril de 2011 e abril de 2013, analisou 53 pacientes com diagnostico de SR admitidos no HRAC-USP em fase de avaliação para correção cirúrgica da fissura palatina (entre 10 e 23 meses de idade). Equipe interdisciplinar inicialmente afastou associação de síndromes ou alterações genéticas na amostra, classificou a forma da fissura, realizou os exames de PSG e VFL previamente à palatoplastia; em seguida, a PSG foi repetida entre três a cinco meses após a cirurgia. Resultados: Vinte e uma crianças terminaram o protocolo de estudo adequadamente, quatorze meninas (66,6%), e sete meninos (33,3%). A fissura palatina foi classificada como em forma de \"V\" em onze casos (52,4%), e em forma de \"U\" em dez casos (47,6%). Dezenove pacientes apresentaram tipo I de obstrução da VAS (90,5%); dois pacientes apresentando o tipo II (9,5%), por meio da VFL. A PSG no momento pré-operatório identificou ausência de critérios mínimos para SAOS em oito crianças (38,1%), SAOS leve em oito (38,1%), SAOS moderada em três (14,3%), e SAOS grave em dois casos (9,5%). A PSG no momento pós-operatório identificou a ausência SAOS em quatorze crianças (66,7%), SAOS leve em três (14,3%), SAOS moderada em quatro (19%), e não identificou casos de SAOS grave. Conclusões: A prevalência SAOS encontrada foi de 61,9% no momento anterior à palatoplastia, e 33,3% no momento pós-operatório. Embora os dados não apresentem diferença significativa estatisticamente, a palatoplastia indicou resultado positivo em relação aos distúrbios respiratórios do sono, reduzindo tanto os índices de eventos respiratórios quanto a gravidade dos mesmos. Pacientes considerados mais graves, com IAH mais elevados, endoscopia tipo II, e fissura em \"U\" foram os maiores beneficiados. A PSG não apresentou valores preditivos significativos quanto às complicações cirúrgicas tardias / Introduction: The Robin sequence (RS) is defined as micrognathia and glossoptosis that lead to difficulty in breathing and swallowing, with or without cleft palate. With broad etiological spectrum and various syndromic associations, the clinical expression of RS becomes heterogeneous. There is a high prevalence of obstructive sleep apnea syndrome (OSAS), 40% to 60% in this population. As the current protocols, treatment depends on the severity of symptoms and the findings of the upper airway endoscopy (UAE). The ideal time for palate surgical repair is controversial. Ancillary techniques are essential for the diagnosis and prognostic assessment. At the Hospital for Rehabilitation of Craniofacial Anomalies of Bauru (HRAC-USP) palatoplasty is deferred in RS patients, once the risk for developing OSAS after surgery is well known. Still there is little information in the literature about the interactions of cleft palate morphology, UAE findings and polysomnography (PSG), with the risk of late upper airway (UA) compromise after palate repair in children with isolated Robin sequence (IRS). Objectives: To evaluate the effect of palatoplasty over OSAS in children with IRS as measured by PSG. To correlate these findings with those obtained by UAE and physical examination of the palate. To detect the frequency of OSAS in this population. To establish the importance of the PSG as part of treatment protocol in children with SRI. Methods: It is a longitudinal prospective study that, between April 2011 and April 2013, evaluated 53 patients with RS admitted in the HRAC-USP during the assessment phase for surgical correction of cleft palate (between 10 and 23 months of age). Interdisciplinary team first discarded genetic disorders among the sample, described the cleft anatomy, and conducted the PSG and UAE prior to palatoplasty; then, repeated the PSG between three to five months after surgery. Results: Twenty-one children finished the study protocol properly, fourteen girls (66.6%), and seven boys (33.3%). Cleft palate was classified as \"V\" shaped in eleven cases (52.4%), and \"U\" shaped in 10 cases (47.6%). Nineteen patients presented type I UAE obstruction (90.5%); two patients presented type II (9.5%). The pre-operative PSG identified the absence of criteria for OSAS in eight children (38.1%), mild OSAS in eight (38.1%), moderate OSA in three (14.3%), and severe OSA in two cases (9.5%). The post-operative PSG identified the absence OSAS in fourteen children (66.7%), mild OSA in three (14.3%), moderate OSAS in four (19%), and did not identify cases of severe OSA. Conclusions: The prevalence of OSA was 61.9% at the moment prior to the cleft palate repair and 33.3% at the post-operative moment. Although non statistically significant difference was found between the findings, palate surgery has indicated positive result in relation to OSAS, decreasing respiratory events indexes, as well as its severity. Patients considered more severe, with highest AHI, endoscopy type II, and \"U\" shaped clefts, were the ones with better results. The PSG did not demonstrate significant predictive values for long term surgical complications

Identificação do reflexo naso-brônquico por meio da provocação nasal em indivíduos com rinite alérgica persistente / Nasobronchial reflex identification by means of nasal provocation in subjects with persistent allergic rhinitis

Mecanismos já consolidados estão envolvidos, contudo o reflexo naso-brônquico, que consiste em um reflexo neural originado nas vias aéreas superiores causando impacto nas vias aéreas inferiores por meio da inflamação neurogênica foi sugerido em estudos experimentais. Evidências da presença deste reflexo em humanos baseiam-se em diferentes métodos que demonstraram prejuízo na função pulmonar após estimulação nasal, porém o monitoramento objetivo da permeabilidade nasal pela rinometria acústica associado ao estudo da função pulmonar por meio da espirometria, possibilita acessar respostas agudas nas vias aéreas após estimulação nasal, representando uma ferramenta promissora na investigação do reflexo naso-brônquico. Portanto, o objetivo deste estudo foi avaliar alterações na função pulmonar após provocação nasal, visando identificar o reflexo naso-brônquico na rinite alérgica isolada. 33 voluntários com rinite alérgica persistente moderada/grave (grupo experimento) e 10 saudáveis (grupo controle) foram submetidos a avaliação inicial e protocolo de investigação composto por espirometria e rinometria acústica basais, seguidas por teste de provocação nasal com concentrações crescentes de histamina; após aplicação de cada concentração a rinometria acústica era novamente realizada passados 1, 4, 8 e 12 minutos, sendo considerada provocação positiva após 20% de obstrução em AST-2, determinando o PN20; por fim, imediatamente nova espirometria era realizada. Ao comparar os dados de função pulmonar antes e após provocação nasal, foi observada diferença significativa no grupo rinite para VEF1 (p=0,002), CVF (p=0,005) e PFE (p=0,005), entretanto não houve variação significativa para nenhum dos parâmetros analisados no grupo controle. O reflexo naso-brônquico foi identificado a partir da variação do VEF1, após a provocação nasal no grupo controle (média acrescida de dois desvios padrões), logo 18,2% dos indivíduos com rinite apresentaram reflexo naso-brônquico (queda > 3% em VEF1). Além disso, para estes verificamos correlação positiva entre as alterações observadas em VEF1 e FEF25-75% (p=0,002 / r=0,97), sugerindo envolvimento das vias aéreas de pequeno calibre. Considerando as vias aéreas superiores, o protocolo proposto demonstrou respostas similares nos dois grupos, sem diferença significativa entre os mesmos para dose de histamina (p=0,98), tempo para atingir PN20 (p=0,97) e porcentagem de obstrução nasal (p=0,97). As alterações observadas na função pulmonar após provocação nasal sugerem a presença do reflexo naso-brônquico no paciente com rinite alérgica persistente moderada/grave. O protocolo proposto apresentou reprodutibilidade, sem intercorrências e, diante dos resultados obtidos, propomos que deva ser pensado na prática clinica com objetivo de aperfeiçoar o diagnóstico da presença do reflexo naso-brônquico, relevante para os pacientes com hiperreatividade brônquica / Abstract: Upper and lower airways consists an integrated system and their interaction becomes evident in allergic diseases, highlighting allergic rhinitis, which is co morbidity to asthma onset. Consolidated paths are involved in this crosstalk, however the nasobronchial reflex, that represents a neural reflex originated in upper airways that causes lower airways impairments was suggested in experimental studies. Evidences for the presence of this reflex in humans are based in different methods that have demonstrated impaired pulmonary function after nasal challenge, although, utilizing objective monitoring of nasal patency by acoustic rhinometry in association with pulmonary function evaluation provided by spirometry, it is possible to access acute airways responses after nasal provocations, representing to be promising tools in nasobronchial reflex investigation. Thus, the aim of this study was to evaluate changes in pulmonary function after nasal provocation, looking for nasobronchial reflex identification in subjects with isolated allergic rhinitis. 33 subjects with persistent moderate/severe allergic rhinitis and 10 healthy subjects underwent screening evaluation and investigation protocol consisting in baseline spirometric and acoustic rhinometry measurements, followed by histamine nasal provocation in increasing concentrations; after each histamine application, comparative acoustic rhinometry measurements were performed passed 1, 4, 8 and 12 minutes and positive provocation was considered after reached 20% of obstruction in MCA-2, determining the NPT20; finally, another spirometry was performed immediately NPT20 was reached. Comparing the pulmonary function before and after provocation in the rhinitis group, it was observed significant difference for FEV1 (p=0,002), FVC (p=0,005) and PEF (p=0,005), however in the same comparison for the control group no significant difference could be found for any of the analyzed parameters. Nasobronchial reflex was identified based on VEF1 percentage of variation after nasal provocation in control group (mean plus two standard deviations), 18,2% of rhinitis group presented nasobronchial reflex (> 3% fall in FEV1). Furthermore, for these ones we could verify positive correlation between VEF1 and FEF25-75% impairments (p=0,002/r=0,97), suggesting distal lower airways involvement. Considering upper airways, the proposed protocol demonstrated similar reactions in the two groups, with no significant differences between them for histamine dose (p=0,98), time to reach NPT20 (p=0,97) and percentage of nasal obstruction (p=0,97). Concluding, spirometric impairments showed here suggest the presence of nasobronchial reflex in patients with persistent moderate/severe allergic rhinitis. The proposed protocol proved to be reproducible, without intercurrences and, regarding results showed here, we propose that it should be thought in clinical practice, in order to improve the diagnosis of the presence of nasobronchial reflex, relevant to patients with bronchial hyperreactivity / Mestrado / Clinica Medica / Mestre em Clinica Medica

Childhood sleep-disordered breathing – dentofacial and pharyngeal characteristics

Abstract The aim of this work was to examine distinct dentofacial and pharyngeal features in children with sleep-disordered breathing (SDB). A further aim was to test the validity of the conventional methods used in orthodontics for recognition of obstructed airways. Seventy children (36 girls, 34 boys, age range 4.2–11.9 years), who were diagnosed as having SDB during the years 2000–2002, constituted the source of subjects for four separate studies. The study protocol included otorhinolaryngological and orthodontic examinations. Dental impressions and lateral cephalograms were taken. In addition, upper airway was imaged in 36 children in different head postures by magnetic resonance imaging (MRI). Age- and gender-matched healthy children were used as controls. The results showed that children with SDB had increased overjet, smaller overbite, narrower upper and shorter lower dental arches and higher incidence of anterior open bite and distal molar relationship when compared with control children. The severity of the obstruction correlated with larger deviations from normal in the dental characteristics. There were more children with crowding and with anterior open bite with increased severity of the disorder. Children with SDB displayed an increased sagittal jaw relationship, a more vertical growth pattern of the mandible, a longer and thicker soft palate, a lower hyoid bone position, larger craniocervical angles and narrower pharyngeal airway measured at multiple levels, when compared with the controls. Deviation in pharyngeal variables showed highest correlation with the severity of SDB. MRI indicated a smaller than normal oropharyngeal airway in children with SDB. The effect of head posture on pharyngeal airway dimensions differed in children with SDB when compared with the controls. These findings verify that the developmental course of occlusal characteristics and craniofacial structures may be modulated by breathing pattern, and, on the other hand, that certain features may predispose to the development of the disorder. It is important to recognize these signs in order to guide further development in a more favorable direction. The results confirmed that cephalometry is a valid screening tool when assessing nasopharyngeal and retropalatal airway dimensions. Clinical examination of tonsillar size was found reliable when retroglossal airway size was evaluated. / Tiivistelmä Väitöskirjatyön tarkoituksena oli selvittää lasten unenaikaisten hengityshäiriöiden yhteyttä kasvojen ja leukojen kasvuun, purennan kehitykseen sekä ylempien hengitysteiden rakenteeseen. Lisäksi tutkittiin, kuinka luotettavasti hammaslääkäreiden käytössä olevin menetelmin pystytään arvioimaan lasten ahtautuneita ilmateitä. Tutkimusaineistossa oli mukana 70 lasta (36 tyttöä, 30 poikaa, ikä 4.2–11.9 vuotta), joilla diagnosoitiin unenaikainen obstruktiivinen hengityshäiriö vuosina 2000–2002. Lapsille tehtiin kliiniset tutkimukset oikojahammaslääkärin ja korva-, nenä- ja kurkkutautien erikoislääkärin toimesta sekä otettiin lateraalikallokuva ja hampaistosta kipsimallijäljennökset. Lisäksi ylempien hengitysteiden magneettitutkimus suoritettiin 36 lapselle eri pään asennoissa. Verrokkiryhmässä oli sama ikä- ja sukupuolijakauma kuin tutkimusryhmässä. Tulokset osoittivat, että unenaikaisista hengityshäiriöistä kärsivillä lapsilla horisontaalinen ylipurenta oli suurentunut, vertikaalinen ylipurenta oli pienentynyt, ylähammaskaari oli kapeampi ja alahammaskaari lyhyempi kuin terveillä verrokeilla. Alaetualueen ahtaus ja etualueen avopurenta olivat yleisempiä niillä lapsilla, joilla oli vaikeampiasteinen hengityshäiriö. Lasten unenaikaiset hengityshäiriöt liittyivät suurentuneeseen leukojen väliseen kokoepäsuhtaan, alaleuan avautuvaan kasvumalliin, pidempään ja paksumpaan pehmeään suulakeen, kieliluun alempaan asentoon, kohonneeseen pään asentoon sekä ahtaampiin nielun ilmatilan mittoihin verrattaessa terveisiin lapsiin. Häiriön vaikeusaste korreloi suurempiin poikkeamiin nielun mitoissa. Lapsilla, joilla todettiin unenaikainen hengityshäiriö, magneettitutkimukset osoittivat nielun olevan ahdas. Pään asennon vaikutukset ilmatien rakenteeseen poikkesivat unenaikaisista hengityshäiriöistä kärsivillä lapsilla. Kunskap och erfarenhet av att hantera patientens ofria luftväg prehospitalt / Knowledge and experience of prehospital airway management

Bakgrund: Inom den prehospitala akutsjukvården kan specialistsjuksköterskan möta en stor variation av luftvägsproblematik. Att ställas inför situationer med patienter med ofri luftväg prehospitalt ställer höga krav på specialistsjuksköterskans kunskap och erfarenhet. En ofri luftväg är ett livshotande tillstånd där snabb och korrekt handläggning krävs för att minimera risken för hypoxi, med risk för efterföljande men som fatal utgång. Att identifiera ett luftvägsproblem och sätta in adekvata metoder för att skapa fria luftvägar är något som specialistsjuksköterskan behöver känna sig säker att utföra, vilket innebär att ha en god förmåga att hantera de tekniker och hjälpmedel som finns att tillgå. Syfte: Syftet med studien är att undersöka anestesisjuksköterskors och ambulanssjuksköterskors kunskapsnivå och erfarenhet av att hantera patientens ofria luftväg prehospitalt. Metod: Studien har en kvantitativ ansats. Specialistsjuksköterskor (n=145) verksamma inom den prehospitala akutsjukvården i en region i södra Sverige deltog i studien. Datainsamlingen genomfördes i form av enkäter och datan analyserades i programmet SPSS. Resultat: Studiens fyra huvudsakliga fynd var att ungefär 20 % av respondenterna hade minst en gång under det senaste året varit med om att inte lyckas skapa fria luftvägar. Anestesisjuksköterskor hade en högre självskattad säkerhet inför ofria luftvägar än ambulanssjuksköterskor. Två tredjedelar av respondenterna ville genomföra en repetitionsutbildning i hantering av ofria luftvägar, och hela 83 % av ambulanssjuksköterskorna ansåg att de var i behov av en repetitionsutbildning i att hantera ofri luftväg. Slutsats: Det finns en signifikant skillnad mellan yrkesgrupperna där anestesisjuksköterskor skattar sig säkrare med att hantera utrustningen vid ofria luftvägar jämfört med ambulanssjuksköterskor. Specialistsjuksköterskorna upplever ett behov av ytterligare utbildning samt kompetenskontroll av luftvägshantering.

The Comparison of Airway Responses of Normal Horses Fed Round Bale versus Square Bale Hay

Background – Feeding horses round bale hay (RBH) has been associated with airway inflammation. The purpose of this study was to determine if horses fed RBH for a 6-week period demonstrated more evidence of airway inflammation than horses fed square bale hay (SBH) of comparable quality. Hypothesis - The respiratory health of horses fed RBH will not differ from horses fed SBH of comparable quality. Animals – Two feeding groups of 15 healthy horses (mixed ages, breeds) from the University riding program. Methods – This was a prospective study performed during fall of 2009. At the beginning and end of a 6- week feeding trial, horses were examined (physical, upper airway endoscopic) and samples (tracheal aspirate (TA), bronchoalveolar lavage (BAL)) collected for cytology and/or bacterial/fungal culture. Hay was analyzed for nutritional value and bacterial/fungal content. Results – Horses fed RBH demonstrated an increase in pharyngeal lymphoid hyperplasia (p=0.0143) and percentage neutrophils (p=0.0078) in the TA samples post-feeding as compared to pre-feeding values. Nutritional analysis of hay and measurements of bacterial/fungal load did not differ over time and/or between hay types. Conclusions and clinical importance – The identification of airway inflammation in the horses fed RBH indicates that factors associated with the manner in which the hay is fed and consumed contribute to the development of subclinical airway inflammation. RBH affords horses continuous daily exposure to hay and as horses bury their muzzles in the bale, exposure to particulate matter is likely increased. These factors may partially explain the response in horses fed RBH. Further studies are required to confirm these predictions. / Master of Science

Comparative efficacy of three common treatments for equine recurrent airway obstruction

Objective - evaluate horses with acute airway obstruction using three treatment regimens: tapering doses of dexamethasone (DEX), environmental modification (ENV), and a combination of both treatments (DEX + ENV) by analyzing clinical parameters, pulmonary function testing, bronchoalveolar lavage fluid (BALF) cytology and BALF cell expression of the cytokines IFN-? and IL-4 Animals - 6 horses with recurrent airway obstruction (RAO) Procedures - Clinical examination, pulmonary function test, and collection of BALF prior to treatment and during 22 day treatment period Hypothesis - Alterations in clinical parameters, pulmonary function and airway inflammation in acute equine RAO will return to remission values by treating with DEX, ENV or DEX + ENV Results - All horses demonstrated clinical disease, reduced pulmonary dynamic compliance (Cdyn) and an increased maximum change in pleural pressures (?Pplmax) when in a challenge environment. All treatments improved clinical parameters, ?Pplmax and Cdyn. BALF cytology during an RAO crisis demonstrated neutrophilic inflammation. ENV or DEX + ENV resulted in a significant decrease in airway neutrophilia that was maintained throughout the treatment period. In contrast, treatment with DEX caused a reduction in airway neutrophilia initially followed by a rebound neutrophilia as the period between administrations of dexamethasone (0.05mg/kg) was increased to 72 hours. The rebound neutrophilia was not accompanied by equivalent deterioration in clinical parameters or pulmonary function. Conclusions - Environmental modification is important in the management of RAO horses. Treatment of clinical RAO with a decreasing dosage protocol of corticosteroids in the absence of environmental modification results in the persistence of airway inflammation without recrudescence of clinical disease. / Master of Science

Subepithelial collagen content in the peripheral airways of heaves-affected and control horses

Chez les patients asthmatiques, on retrouve un remodelage de la matrice extracellulaire des poumons, caractérisé par une augmentation du collagène ou fibrose de la couche sous-épithéliale des voies respiratoires. Le souffle, maladie inflammatoire chronique des voies respiratoires inférieures des chevaux matures, présente des similarités physiopathologiques avec l’asthme humain, incluant le remodelage. Ceci nous conduit à l’hypothèse que la fibrose de la couche sous-épithéliale pourrait être une composante des lésions pulmonaires chez les chevaux affectés, ce que notre étude avait pour objectif d’évaluer. Des biopsies pulmonaires périphériques réalisées par voie thoracoscopique ont été obtenues chez 5 chevaux témoins et 6 chevaux atteints du souffle, avant (T0) et après une stimulation antigénique de 30 jours avec du foin moisi et de la paille. Avant le début de l’étude, les sujets étaient en rémission clinique et ne démontraient aucun signe clinique de maladie. Un examen microscopique des échantillons prélevés a été réalisé après traitement au picrosirius-rouge, colorant spécifique des fibres de collagène. La surface du collagène de la couche sous-épithéliale a été mesurée et corrigée en fonction de la taille de la voie respiratoire en utilisant des techniques morphométriques standards. Les chevaux atteints de souffle ont une surface de collagène plus grande dans la couche sous-épithéliale (p<0.1) en comparaison avec les chevaux témoins. La fibrose de la couche sous-épithéliale demeure inchangée chez les chevaux malades après la stimulation antigénique de 30 jours. À T0, la fibrose de la couche sous-épithéliale est associée positivement aux variations maximales de pression pleurale et à la résistance pulmonaire chez les chevaux atteints de souffle. Les résultats de cette étude suggèrent qu’une fibrose de la couche sous-épithéliale est présente dans les voies respiratoires périphériques des chevaux atteints de souffle et contribue au déficit de fonction résiduel pulmonaire observé lors de rémission clinique. / Extracellular matrix remodelling is present in the human asthmatic lung, and is characterized by increased collagen or fibrosis of the subepithelial area of the airway. Heaves, a naturally occurring chronic lower airway inflammatory condition in horses shares aspects of pathophysiology with asthma, including features of airway remodelling. We thus hypothesize that airway fibrosis is a characteristic of remodelling in heaves. The aim of this study was to evaluate the presence of fibrosis in the subepithelial area of the peripheral airways of heaves-affected horses. Peripheral lung biopsies acquired under thoracoscopic guidance were obtained from 5 control and 6 heaves-affected horses, both before (T0) and after a 30 day antigenic challenge with mouldy hay and straw. Prior to the study, diseased horses were in clinical remission and exhibited no clinical signs of disease. Obtained samples were microscopically examined using picrosirius-red, a collagen specific histological staining technique. Collagen area in the subepithelial layer, e.g. the region between the airway smooth muscle and the epithelial layer was measured, and corrected for airway size using standard morphometric techniques. In comparison with controls, heaves-affected horses had an increased collagen content in the airway subepithelial area (p<0.1). No change in fibrosis of the subepithelial area was observed in diseased horses after the 30 day antigenic challenge. Peripheral airway subepithelial collagen at baseline was positively associated with maximal changes in transpulmonary pressure and pulmonary resistance in horses with heaves but not in controls. Results of this study indicate that fibrosis of the subepithelial area is present in the peripheral airways of heaves-affected horses, and may play a role in residual lung function deficits observed in diseased horses even while in clinical remission.

A comparison of airway devices for the simulated entrapped patient

A research report submitted in partial fulfilment of the requirements for the degree Master of Science in Medicine in the Field of Emergency Medicine in the Division of Emergency Medicine, University of the Witwatersrand / Introduction: Control over the patient bears time-critical importance in emergency medicine. In the entrapment situation after a Motor Vehicle Collision (MVC), emergency care including airway management may need to be initiated before extrication and thus with restricted access. Objective: This manikin study aimed at answering the question of which advanced airway device can be inserted the fastest and most reliably by paramedics in the simulated entrapped patient. Methods: Paramedics were asked to insert four airway devices (endotracheal tube with the Macintosh laryngoscope, endotracheal tube with the Airtraq® optical laryngoscope, Laryngeal Mask Airway - SupremeTM, and Laryngeal Tube Suction - DisposableTM) in randomised order into a manikin seated in the driver seat of a light motor vehicle. Time to first successful ventilation and number of attempts required for successful insertion were measured. Following each insertion, participants were asked by means of a questionnaire to rate the degree of insertion difficulty (scale 1 – 10) and provide reasons for this rating. Finally, participants were asked which device they preferred and why. Results: Prospectively collected data from 26 paramedics were analysed. The LMA-SupremeTM had the shortest mean time to first successful ventilation (16.7 seconds (CI [0.95]; 14.9 - 18.6)), followed by the LTS-DTM (19.4 seconds (CI [0.95]; 18.0 - 20.8)), ETI using the Macintosh laryngoscope (37.7 seconds (CI [0.95]; 31.8 - 43.5)) and ETI using the Airtraq® (41.2 seconds (CI [0.95]; 36.7 - 45.6)). Both face-to-face ETI with the Macintosh laryngoscope and the insertion of the LMA-SupremeTM had 100% first-attempt success. Five participants required a second attempt to successfully intubate the manikin using the Airtraq® and one participant had to re-insert the LTS-DTM for correct placement. In terms of insertion difficulty, the LMA-SupremeTM received the lowest mean score (1.7/10 (CI [0.95]; 1.2 - 2.1)) followed by the LTS-DTM (2.5/10 (CI [0.95]; 1.8 – 3.2)), face-to-face ETI using the Macintosh laryngoscope (3.7/10 (CI [0.95]; 2.9 - 4.5)), and ETI with the Airtraq® (4.5/10 (CI [0.95]; 3.7 - 5.3)). Most participants chose the Macintosh laryngoscope for ETI as their preferred device (10/26; 38%) followed closely by the LMA-SupremeTM (9/26; 35%). These participants stated clinical experience and ease of insertion respectively as the primary reasons for their preference. Conclusion: Besides ETI, Supraglottic Airway Devices are beneficial alternative airway devices to be considered by paramedics in the entrapped patient after a MVC. The LMA-SupremeTM was the fastest and least difficult airway device to insert. Face-to-face endotracheal intubation with the Macintosh laryngoscope remains an important definitive airway that was shown to be performed competently by participating paramedics. The Airtraq® can be used for face-to-face ETI and enables improved laryngoscopy.

Aspectos anatômicos da via aérea superior para a personalização da terapia da apneia obstrutiva do sono / Upper airway anatomy characteristics for obstructive sleep apnea personalized treatment

Introdução: A apneia obstrutiva do sono (AOS) é uma doença altamente prevalente, caracterizada pela obstrução recorrente da faringe durante o sono. Apesar do quadro clínico ser marcado por ronco e sonolência diurna excessiva e do maior risco cardiometabólico associado à AOS, uma grande parcela dos pacientes diagnosticados permanece sem nenhum tratamento. Dessa forma, são necessárias estratégias com o objetivo de otimizar as opções de tratamento para os pacientes com AOS. Objetivos: Esta tese é composta pela compilação de três artigos com o objetivo geral de avaliar os fatores fisiopatológicos anatômicos da AOS que podem influenciar na variabilidade individual de resposta ao tratamento. Os objetivos específicos de cada artigo são: Artigo 1) Avaliar se o padrão de obstrução da faringe influencia no efeito da mudança de decúbito de supino para lateral na patência da via aérea superior; Artigo 2) Avaliar as diferenças na complacência das regiões da faringe e sua associação com padrões de curva inspiratória; Artigo 3) Avaliar a influência da estrutura faríngea envolvida na obstrução e a colapsabilidade da via aérea superior na eficácia do aparelho intra-oral (AIO) no tratamento da AOS. Métodos: Foram recrutados pacientes com diagnóstico prévio de AOS com idade entre 21 a 70 anos. Artigo 1: Os indivíduos foram avaliados com sonoendoscopia e registro simultâneo do fluxo aéreo durante o sono natural em decúbito supino e lateral. Artigo 2: os indivíduos foram avaliados com sonoendoscopia e registro simultâneo da pressão faríngea durante o sono natural. Artigo 3: Os indivíduos foram avaliados na primeira noite com sonoendoscopia e, em duas noites adicionais, foram submetidos à polissonografia com e sem AIO para determinação do índice de apneiahipopneia (IAH) e para a medida da pressão crítica de fechamento da faringe (Pcrit). Resultados: Artigo 1: Foram avaliados 24 pacientes (17 homens, idade: 53±6 anos, IAH:48±28 eventos/hora). Em pacientes com obstrução associada a língua (n=10), não houve aumento significativo do pico de fluxo inspiratório e da ventilação minuto com a mudança de decúbito de supino para lateral. A posição lateral resultou em diminuição da ocorrência do colapso de epiglote e aumento de 45% na ventilação minuto entre os pacientes com obstrução da epiglote (n=6). Artigo 2: Foram avaliados 14 pacientes (9 homens, idade: 51±5 anos, IAH: 56±32 eventos/hora). Comparada à região retroglossal, a região retropalatal foi mais estreita (19,2 [23,9] mm2 versus 55,0 [30,7]mm2; p < 0,001) e apresentou maior complacência (3,2 ± 2,1 mm2/cmH2O versus 2,1 ± 1,8 mm2/cmH2O; p < 0,001). A dependência ao esforço negativo foi positivamente associada ao estreitamento da área retropalatal (r=0,47; p=0,001). Artigo 3: Foram avaliados 25 pacientes (17 homens, idade: 49±11 anos, IAH: 51±24 eventos/hora). O AIO reduziu a Pcrit em 3,9±2,4 cmH2O e o IAH em 69%. A redução da Pcrit foi maior nos pacientes com a língua posteriorizada. A ppresença da língua posteriorizada (p=0,03) e menor colapsabilidade da faringe (Pcrit < 1 cmH2O) no momento inicial (p=0,04) foram determinantes de melhor resposta terapêutica demonstrada pela maior redução do IAH com o AIO (83% versus 48%; p < 0,001). Conclusões: O padrão de obstrução e a colapsabilidade da faringe são fatores determinantes na resposta individual às modalidades terapêuticas alternativas para tratamento da AOS como terapia posicional e AIO. Pacientes com colapso de epiglote apresentaram melhora da patência da faringe com o decúbito lateral e assim podem se beneficiar da terapia posicional para AOS. A região retropalatal apresentou menor área e maior complacência comparada à região retroglossal nos pacientes com AOS. Finalmente, pacientes com língua posteriorizada e menor colapsabilidade da faringe são bons candidatos ao uso do AIO para tratamento da AOS / Introduction: Obstructive sleep apnea (OSA) is a highly prevalent disease characterized by recurrent pharyngeal obstruction during sleep. Despite symptoms such as snore and excessive daytime sleepiness and, the higher cardiometabolic risk related to OSA, a large proportion of patients do not receive any treatment for the disease. Therefore, strategies to improve management approaches for OSA are necessary. Objectives: This thesis consists of the compilation of three articles with a general aim of investigating the anatomic factors involved in OSA pathogenesis that could affect the individual variability of treatment responses. The specific aims of each article are: Article 1: To evaluate if the pattern of pharyngeal obstruction influences the effect of changing from supine to lateral position on upper airway patency; 2) To compare the compliance of each pharyngeal level and its association with inspiratory flow patterns; 3) To evaluate the effect of pharyngeal collapsibility and the pharyngeal structure causing collapse on the efficacy of oral appliance therapy for OSA. Methods: Patients previously diagnosed with OSA ranging from 21 to 70 years old were recruited for the studies. Article 1: Patients underwent upper airway endoscopy with simultaneous recordings of respiratory airflow in both supine and lateral position during natural sleep. Article 2: Patients underwent upper airway endoscopy with simultaneous recordings of pharyngeal pressure during natural sleep. Article 3: Patients underwent upper airway endoscopy on the first night. On two additional overnight studies, polysomnography was performed with and without an oral appliance to determine apnea-hypopnea index (AHI), and to measure the critical closing pressure (Pcrit). Results: Article 1: Twenty-four patients (17 men, 53±6 years old, AHI:48±28 events/hour) were studied. Patients with tongue-related obstruction (n=10) showed no improvement in airflow, and the tongue remained posteriorly located. Epiglottic obstruction was virtually abolished with lateral positioning and ventilation increased by 45% compared to supine position. Article 2: Fourteen patients (9 men, 51±5 years old, AHI: 56±32 events/hour) were studied. Compared to the retroglossal airway, the retropalatal airway was smaller at end-expiration (p < 0.001), and had greater absolute and relative compliances (p < 0.001). NED was positively associated with retropalatal relative area change (r=0.47; p < 0.001). Article 3: Twenty-five patients (17 men, 49±11 years old, IAH: 51±24 events/hour) were studied. Oral appliance therapy reduced Pcrit by 3.9±2.4 cmH2O and AHI by 69%. Oral appliance lowered Pcrit by 2.7±0.9 cmH2O more in those with posteriorly-located tongue compared to those without (p < 0.008). Posteriorly-located tongue (p=0.03) and lower baseline collapsibility (p=0.04) were significant determinants of a greater-than-average AHI response to therapy (83% versus 48%, p < 0.001). Conclusions: The pattern of obstruction and pharyngeal collapsibility are determinants of the individual response to alternative OSA treatment such as positional therapy and oral appliance. Patients with epiglottic obstruction showed significant improvement with lateral sleeping position and, therefore may benefit from positional therapy for OSA. The retropalatal airway had a smaller area and a greater compliance compared to the retroglossal airway in OSA patients. Finally, patients with posteriorly located tongue plus less-severe collapsibility are good candidates for oral appliance therapy
