Compassion Fatigue in Emergency Department Nurses

Compassion fatigue (CF) is a problem seen within healthcare institutions worldwide, especially critical care units and emergency departments (EDs). The problem identified in this quality improvement (QI) project was CF, experienced by nurses in the ED. The effects of CF cross nurse-patient boundaries and negatively impact a patient's expectations of having a quality care experience. The Iowa model's evidence-based team approach was used to guide the development of the education initiative for nurses on recognizing, preventing, and identifying methods of coping with CF in the ED. The outcome products for the project included an extensive review of the literature, a curriculum plan to educate ED nurses on CF, and a pretest/posttest to validate ED nurses knowledge about CF. The content of the project was measured by 2 master's-level prepared education experts using a dichotomous scale. The format evaluated content material using total scores of 1 for content (not met) and total scores of 2 for content (met). The average score was 2, which demonstrated the objectives for the education initiative were identified and the goals were met. The content experts also conducted content validation of each of the 14 pretest/posttest items using a 4-point Likert scale ranging from 1 (not relevant) to 4 (highly relevant) that resulted in a content validation index of 1.00, showing that the test items were covered in the curriculum. Recommendations were made for item construction improvement and omission of the Iowa model from the curriculum plan and pretest/posttest. The project promotes social change through the facilitation of patient satisfaction, quality of patient care, and prevention of CF on nursing staff.

Improving Teacher Retention by Addressing Teachers' Compassion Fatigue

California is experiencing a massive teacher shortage, and urban schools are disproportionately affected by it negatively. Retention efforts to date have not included strategies to address Compassion Fatigue (burnout and secondary trauma) teachers experience when working with traumatized students at urban schools. This dissertation explores whether Compassion Fatigue is an unaddressed reason for teacher attrition at urban schools. A mixed method practical action research approach using the Professional Quality of Life Scale Version Five (ProQOL 5) and qualitative interviews, portions of which were turned into illustrative vignettes drove the exploration. Approximately 114 teachers completed the ProQOL 5. Statistical analysis of the ProQOL 5 results showed that female teachers experience more compassion fatigue than male teachers; compassion fatigue is more acute with beginning teachers than with veteran teachers; and that teachers working at high-poverty schools experience statistically significant differences in compassion satisfaction and fatigue than teachers at low poverty schools. Correlation tests revealed statistically significant relationships between compassion fatigue and the school’s racial demographics even when controlling for the socioeconomically disadvantaged status of the school and teacher ethnicity. Linear regression models showed that the percentage of African American students in the school is a statistically significant predictor of compassion fatigue. Qualitative interview analysis showed that secondary trauma from students is not the only trauma teachers are experiencing, and that school climate and conditions matter when attempting to retain teachers. In the final phase of the action research, a policy brief was developed through a collaborative and iterative process, based both on the findings and engagement with stakeholders. If California is serious about producing and retaining high-quality teachers at all urban schools,’ efforts to mitigate compassion fatigue should be undertaken immediately.

Sekundär traumatisk stress är ett problem inom socialt arbete. Begränsad forskning finnsangående hur det skulle kunna förebyggas samt behandlas. Syftet med litteraturöversikten vardärför att sammanställa kunskapsläget av befintlig litteratur angående faktorer som förebyggersamt behandlar sekundär traumatisk stress hos socialarbetare. Studien avsåg även att undersökavilken effekt faktorerna hade. Litteraturöversikten utgick från metodiken av en scoping review.Datamaterialet bestod av 15 vetenskapliga artiklar. Samtliga artiklar analyserades medinnehållsanalys, ytterligare gjordes en narrativ sammanställning av de kvantitativa artiklarna.Resultatet visar att individuella faktorer som att kunna hantera sina känslor i olika situationeroch sammanhang samt att utföra egenvård, är exempel på betydelsefulla förebyggande ochbehandlande faktorer. Regelbunden handledning samt stöd från chefer och medarbetare, ärexempel på organisatoriska faktorer som hade en betydande roll i det förebyggande ochbehandlande arbetet mot sekundär traumatisk stress. / The purpose of this study was to compile the state of knowledge in the existing literature aboutfactors that prevent and treat secondary traumatic stress in social workers. The study also aimedto investigate the effect of the factors. This literature review was based on the methodology ofa scoping review. The data was contained by 15 different scientific articles. All these articleswere analyzed by content analysis, and further there were a narrative compilation of thequantitative articles. The results of the study show individual factors like to be able to deal withtheir emotions in different kind of situations and contexts, and to perform selfcare, are someexamples of meaningful factors. Regular guidance and support from managers and other coworkersare some examples of important organizational factors in the prevention and treatingwork against secondary traumatic stress.

Dairy producers play a central role in evaluating and seeking treatment or care for animal pain. The primary aim of this study is to examine dairy producers’ emotional states and professional quality of life and analyze the relationship between these variables and their perception of pain in dairy cattle. Dairy farm owners and managers of 65 southeastern U.S. herds participated in a survey that included demographic information, the Depression, Anxiety, and Stress Scale, a modified Professional Quality of Life Scale, and 23 items requiring participants to evaluate various painful conditions in cattle.

Evaluating the effectiveness of a self-care programme for intervention in burnout and compassion fatigue among nurses working in critical care areas

Thesis (Ph.D. (Psychology)) -- University of Limpopo, 2022 / This thesis is about evaluating the effectiveness of a self-care programme for intervention in burnout and compassion fatigue among nurses working in critical care areas. A convenient sampling method of all the nurses who work in the critical care areas as per the operational definition of terms for this study was used. A total of 154 nurses in a critical care area participated in this pre-post study, of which (n=83) were CTOP Nurses and (n=71) were Forensic Nurses. The nurses completed a biographical questionnaire, the Professional Quality of Life Scale (ProQOL R-IV), the Empathy Assessment Index Scale (EAI). Nurses were divided into groups of 6 to 10 people for focus group discussions on their work experiences. The results of the current study indicated moderate to high levels of burnout and compassion fatigue occurring with high compassion satisfaction among the nurses. However, the mean burnout scores for CTOP nurses and Forensic nurses and details indicated lack of statistically significant difference post-intervention (p>0.05). The study utilized the Context Process Outcome (CPO) model as its framework. The proximal outcomes centred around safe holding, development of awareness and self-care. Intermediate outcomes consisted of drop in burnout and compassion fatigue and the distal outcomes showed increase in empathy and revived motivation to continue work in critical areas as well as a drop in distress and increase in compassion satisfaction. The project was ground-breaking work of research with nurses in the critical areas with regards to health promotion with promise in healthier ways of caring for the carers and their empowerment and intervention outlook on the challenges around working environment stressors and interventions. Such work could in future benefit health care professionals by predicting possible decrease in their productivity by measuring other non-invasive constructs like empathy which has shown probable predictive power on development of burnout and compassion fatigue as well as improvement of satisfaction. Future research is recommended for inclusion of other health professions in such work and not only nurses, as well as doing evaluation that allows intermittent re-alignment whenever indicated in the process of intervention

Syftet med uppsatsen var att undersöka hur socialarbetare på en familjehemsenhet i en mellanstor kommun i Sverige upplevde arbetet med förhållningssättet traumamedveten omsorg, samt hur enheten behandlade eventuell empatitrötthet som följd av arbetet med barn som erfarit trauma. För att uppnå syftet med uppsatsen antogs en kvalitativ ansats där sex intervjuer genomfördes med yrkesverksamma socialarbetare inom samma enhet. Teorierna och perspektiven som användes som utgångspunkt för studien var det salutogena perspektiveti nnehållande patogenes och känsla av sammanhang, den socialpsykologiska ansatsen med fokus på den kognitiva socialpsykologin och organisationsteori med inriktning på humanresource-perspektivet. Den insamlade empirin analyserades med hjälp av en kvalitativi nnehållsanalys. Studiens resultat visade att samtliga intervjupersoner hade en övervägande positiv inställning till traumamedveten omsorg oaktat att hälften av dem inte praktiserat förhållningssättet i olika ärenden. Utifrån resultatet framkom det även några negativa upplevelser av att arbeta med förhållningssättet. När det kom till hur familjehemsenheten behandlade eventuell empatitrötthet visade resultatet att enheten främst arbetade förebyggande gentemot empatitrötthet, men att det även fanns hälsofrämjande inslag för att förhindra empatitrötthet till följd av arbetet med barn som erfarit trauma. Studiens slutsats visar att mer utbildning behövs i hur man bemöter personer som har upplevt trauma och hur arbetet kan påverka den professionella socialarbetarens välbefinnande. Detta är betydelsefullt för att på bästa möjliga sätt bemöta dessa människor och därmed ha möjligheten att hjälpa dem till ett bättre liv. / This study aimed to research how social workers in the child protective services in a mediumsized suburb in Sweden experienced the work with transforming care and how their organization handled causes of possible compassion fatigue as a result of working with children who have lived through trauma. To achieve the purpose of the study a qualitative approach was adopted where six interviews were conducted with professional social workers who worked in the same unit. The theoretical basis of the study was the salutogenic perspective containing pathogenesis and sense of coherence, the social psychological approach with focus on cognitive social psychology and organizational theory with focus on the human resource perspective. A qualitative content analysis helped to analyze the collected data. The results showed that all respondents had a positive attitude towards transforming care, despite that half of them did not practice the approach in their work. It also appeared some negative experiences of working with transforming care. Regarding how the organization handled possible compassion fatigue, the result showed that the organization worked primarily preventively against compassion fatigue. But there were also healthpromoting elements to prevent compassion fatigue as a result of working with children who experienced trauma. The conclusion of the study shows that more knowledge is needed in how to work with people who have experienced trauma and how the work can affect the social workers. This is important in order to meet these people in the best possible way and help them to a better life.

COMPASSION ENERGY : En allmän litteraturöversikt om att hitta kraft i att vårda hos sjuksköterskor

Bakgrund: Ett av sjuksköterskornas huvuduppdrag är att lindra patienters lidande. Att lindra patienters lidande är en krävande skyldighet, vilket förutsätter att sjuksköterskor har förmåga att vara närvarande. Tidigare forskning visar att sjuksköterskor som har varit utsatta för patienter som lider under en längre tid löper större risk att utveckla medkänslaströtthet, om sjuksköterskorna inte har strategier för att hantera emotionell och moralisk stress. Medkänslasenergi är ett fenomen som kan dyka upp i mötet med patienter om sjuksköterskan bryr sig med avsikt att vara närvarande. Syfte: Att skapa en överblick över sjuksköterskors erfarenheter av att finna energi till vården. Metod: En allmän litteraturöversikt baserad på 13 vetenskapliga studier. Resultat:Resultatet visade att det finns flera gemensamma faktorer som invånare upplever. 3 teman härleddes: Erfarenheter av medkänsla och meningsfullhet, upplevelser av stöd och tillväxt och upplevelser av egenvård . Slutsats: Förmågan att ge medkänsla är avgörande för att uppleva medkänslasenergi. Faktorer som stöd från kollegor och organisation kan bidra till att sjuksköterskor hittar energi till vården. Sjuksköterskor har en önskan om att utveckla strategier för egenvård, tid för reflektion och vila för att kunna ge en medmänsklig vård, vilket kan leda till att sjuksköterskor upplever medmänsklighetsenergi.
