Assessment of adverse drug reactions caused by HAART at antiretroviral clinics in the Maseru district, Lesotho / Lineo Joyce Maja

Antiretroviral drugs are successful in controlling HIV/AIDS and reducing disease progression. Antiretroviral regimens are stopped in up to 25% of all patients during their initial treatment therapy as a result of adverse drug effects, failing treatment and nonadherence within the initial eight months of treatment (Sharma et al., 2007: 235). A pharmacovigilance surveillance system makes it possible for physicians, pharmacists and other healthcare providers to report suspected ADRs. The purpose of this system is to operate as a guide in identification of new ADRs and predisposing risk factors to known ADRs. The objective of this study was to assess the prevalence and documentation of adverse drug reactions (ADR) in the private and public antiretroviral clinics in Maseru district, with special reference to zidovudine (AZT) and tenofovir (TDF) - based regimens. The empirical investigation was divided into two phases. The first phase was a cross-sectional quantitative retrospective drug utilisation review study which focused on the occurrence of adverse drug reactions in patients taking zidovudine (AZT) and tenofovir (TDF). The second phase, a survey in a form of questionnaires for the health professionals. Drug utilisation review: The sample size of patients was 300. Of the 44 patients who experience ADRs, 72.73% (n = 32) were female and 27.27% (n = 12) were male. A greater number of patients who experienced ADRs were females with 43.18% (n = 19) presenting with skin rash, 27.27% (n = 12) with nausea/vomiting, and 2.27% (n = 1) with diarrhoea. In male patients, 2.27% (n = 1) had peripheral neuropathy, 18.18% (n = 8) skin rash, 2.27% (n = 1) Fanconi syndrome, 2.27% (n = 1) nausea/vomiting, and 2.27% (n = 1) diarrhoea. Patients whose ART regimen changed due to ADRs were five. 60% (n = 3) of the patients were females and 40% (n = 2) were males. There was an estimated increase of 0.0025 cell/mm³, 0.0026 cell/mm³, 0.0024 cell/mm³, 0.0025 cell/mm³, and of 0.0019 cell/mm³ in CD4 cell count per day according to sex, age group, weight group, initial ART regimen, and ADRs, respectively. An estimated increase of 0.00021 g/dL, 0.00022 g/dL, 0.00018 g/dL, 0.00022 g/dL, and of 0.00020 g/dL in Hb profile per day occurred according to sex, age group, weight group, initial ART regimen, and ADRs, respectively. There was an estimated increase of 0.000062%, 0.000046%, 0.000068%, 0.000062%, and of 0.00017% in neutrophil count according to sex, age group, weight group, initial ART regimen, and ADRs per day, respectively. There was an estimated increase of 0.000044 IU/L, 0.000043 IU/L, 0.000046 IU/L, and of 0.000028 IU/L in ALT according to sex, age group, weight group, and initial ART regimen per day, respectively. An estimated decrease of 0.000013 IU/L in ALT according to ADRs per day also occurred. There was an estimated decrease of 0.00038 μmol/L, 0.00039 μmol/L, 0.00040 μmol/L, 0.00040 μmol/L, and of 0.00028 μmol/L in serum creatinine per day according to sex, age group, weight group, initial ART regimen, and ADRs, respectively. There was an estimated decline of 0.00023 mmol/L, 0.00022 mmol/L, 0.00023 mmol/L, 0.00024 mmol/L, and of 0.00015 mmol/L per day in urea according to sex, age group, weight group, initial ART regimen, and ADRs, respectively. Health professional’s questionnaire: 49 health professionals responded to the questionnaire. 100% (n= 49) of the participants showed that they did not use the yellow card scheme to report ADRs. 34.65% (n = 17) use the individual case safety reports. 57.14% (n = 28) used the structured databases to report ADRs. 85.71% (n = 42) documented in the patient bukana, and 6.12% (n = 3) used the HIV/AIDS ART card to document ADRs occurrence. 91.84% (n = 45) of the health professionals never filled the ADR reporting form in their working environment. In conclusion, adverse drug reactions occurring in a hospital or healthcare facility should be recorded and reported by the medical practitioners, nurses, pharmacists, and the pharmacy technicians. Therefore, it is important to assess the continuous evaluation of the benefits and harm of medicines which will help in achieving the ultimate goal of making safer and more effective treatment available for patients. Es wurden Versuche für die ungesättigte Zone duchgeführt, um das Verhalten kleinerer Einträge an Benzen, Toluen und Xylen im Oberboden anschätzen zu können. In einem Vorversuch wurde der Verteilungskoeffizient Gas-Wasser für verschiedene Wässer bestimmt. Der Einfluss der Temperatur hatte einen größeren Einfluss als der Chemismus des Wassers. Die Sorption wurde ermittelt und zeigte sich als ein über sechs Größenordnungen linearer Prozess. Sie ist abhängig vom Humusgehalt. Versuche zur Desorption zeigen Unterschiede zwischen den Aromaten. Xylen desorbiert am langsamsten. Fünf, z.T. mehrmonatige Versuche mit einem großen Laborlysimeter (60 cm Durchmesser, 2 m Länge) zeigten, dass die Korngröße die Diffusion und dichtebedingte Konvektion der gasförmigen Aromaten beeinflusst. Diese Vorgänge sind für den schnellen Transport verantwortlich. Das Sickerwasser bewegt sich deutlich langsamer, transportiert aber die größeren BTX-Mengen. Humushaltige Böden können den Transport in tiefere Bereiche aufgrund von Sorption deutlich reduzieren. Mikrobieller Abbau lässt sich über den Sauerstoffverbrauch nachweisen.

Study of crack width within a suspended concrete slab with different amount of cement clinker considering lower climate impact

This master thesis investigates the possibility of using a concrete recipe with only 70 % cement clinker for a building project named Gretas Glänta, regarding the demands of cracking for a suspended slab. The requirement to fulfill was a maximum crack width of 0.2 mm due to drying shrinkage and long- term loading. The purpose of using a lower amount of cement clinker in the concrete mix is to reduce the climate impact from the concrete. The thesis considers material testing and modelling of different concrete recipes with variation of the amount of cement clinker. Fly ash was used as the alternative binder. A total of five concrete mixes was tested. One mix with 100% cement clinker was through laboratory testing compared to mixes with 85 % and 70 % cement clinker content. A concrete mix with only 50 % cement clinker was also tested, but this mix is today not allowed according to the concrete standards, but was included in the study to test different material properties of the fresh and hardened concrete. All concrete mixes were tested with the effectivity factor 1, except for the recipe with 70 % cement clinker, which was tested for effectivity factors of 1 and 0.4. The performed study showed that the concrete mix 4 with 70 % cement clinker and with the effectivity factor 0.4 reached the strength class C35/45 after 28 days and also fulfilled the specified requirement of a maximum crack width of 0.2 mm due to drying shrinkage and long-term loading. Water permeability test showed that the concrete mix with 70 % cement clinker and an effectivity factor of 0.4 was waterproof. The concrete mix 3 with 70 % cement clinker and the effectivity factor 1 reached a strength class C30/37. The concrete mix 1 with 100 % cement clinker obtained the highest strength class C50/60 and the mix 2 with 85 % cement clinker reached strength class C45/55. The concrete mix 5 with 50 % cement clinker reached the strength class C20/25 which is the lowest strength class among the tested recipes. All the concrete recipes seemed to be waterproof according to the water permeability test. A background description of carbon dioxide emission from concrete and cement production is presented in the report. Research about ongoing methods to minimize the emission from concrete and cement industry is also summarized. FE-modelling with the software FEM-design and analytical calculations were performed to investigate the crack width due to drying shrinkage and long-term loading for the slab, for concrete mix 2 with 85 % cement clinker with effectivity factor 1 and mixes 3 and 4 with 70 % cement clinker and effectivity factors of 1 and 0.4. The properties obtained from the laboratory tests were used in the modelling and analytical calculations. Results from FEM-design and analytical calculations showed that concrete mixes 3 and 4 with 70 % cement clinker and effectivity factors of 1 and 0.4, and the concrete mix 2 with 85 % cement clinker with effectivity factor 1, fulfilled the demand on crack width. The calculation was not performed for concrete mixes 1 and 5 with 100 % and 50 % cement clinker, respectively, because they were not to be used in the building project. The results from the performed study showed that concrete mix 4 with 70 % cement clinker and an effectivity factor of 0.4 can be used in the building project Gretas Glänta in order to lower the climate impact from the concrete. / Examensarbetet undersöker möjligheten att använda ett betongrecept med endast 70 % cementklinker för ett husbyggnadsprojekt kallat Gretas Glänta med hänsyn till krav på sprickbildning i en fribärande platta. Kravet som ska uppfyllas är en sprickvidd på max 0.2 mm med hänsyn till krympning och långtidsbelastning. Syftet med att använda en lägre andel cementklinker i betongblandningen är att minska klimatpåverkan från betongen. Examensarbetet består av materialförsök och konstruktionsteknisk modellering för olika betongrecept, där andelen cementklinker varieras. Flygaska användes som alternativt bindemedel. Totalt undersöktes fem betongmixer. En mix med 100 % cementklinker jämfördes genom laboratorietestning med motsvarigheter med 85 % och 70 % cementklinkerinnehåll. Ett betongrecept med 50 % cementklinker undersöktes också i detta examensarbete, trots att detta idag inte tillåts enligt betongstandarderna, men inkluderades i studien för att testa olika materialegenskaper i färskt och hårdnat tillstånd. Alla betongmixerna testades med effektivitetsfaktorn 1 förutom receptet med 70 % cementklinkerandel som testades för både 1 och 0.4 i effektivitetsfaktor. Den genomförda studien visade att betongmix 4 med 70 % cementklinker och med effektiviseringsfaktor 0.4 uppnådde hållfasthetsklass C35/45 efter 28 dagar och uppfyllde också angivet krav på maximal sprickbredd 0,2 mm med hänsyn till krympning och långtidslast. Vattenpermeabilitetstest visade att betongmixen med 70 % cementklinker och effektiviseringsfaktor 0.4 var vattentätt. Betongmix 3 med 70 % cementklinker och effektivitetsfaktor 1 uppnådde hållfasthetsklass C30/37. Betongmix 1 med 100 % cementklinker erhöll den högsta hållfasthetsklassen C50/60 och betongmix 2 med 85 % cementklinker gav hållfasthetsklass C45/55. Betongmix 5 med 50 % cementklinker uppnådde hållfasthetsklass C20/25 vilket var den lägsta av de testade betongrecepten. Alla betongrecepten verkade ge vattentät betong enligt vattenpermeabilitetstest. En bakgrundsbeskrivning av koldioxidutsläpp från betong- och cementproduktion genomförs i rapporten. Forskning om pågående metoder för att minimera utsläppen från betong- och cementindustrin sammanfattas också. FE-modellering med programmet FEM-design och analytiska beräkningar utfördes för att undersöka sprickbredden med hänsyn till krympning och långtidslast för betongplattan för betongmix 2 med 85 % cementklinker och effektiviseringsfaktor 1 och betongmixerna 3 och 4 med 70 % cementklinker och effektiviseringsfaktorerna 1 och 0.4. Egenskaperna som erhölls i laboratorietesterna användes i modellerings och de analytiska beräkningarna. Resultat från FEM-design och analytiska beräkningarna visade att betongmixerna 3 och 4 med 70 % cementklinker med effektiviseringsfaktorerna 1 och 0.4 och betongmix 2 med 85 % cementklinker och effektiviseringsfaktor 1 uppfyllde kravet på sprickbredd. Beräkningen utfördes inte för betongmixerna 1 och 5 med 100 % cementklinker och 50 % cementklinker då de inte var aktuella för att användas i bostadsprojektet. Resultaten från den genomförda studien visade att betongmix 4 med 70% cementklinker och med en effektivitetsfaktor 0.4 kan användas i husbyggnadsprojektet Gretas Glänta för att sänka klimatpåverkan från betongen.

Based on what body adornment has been throughout history, this project investigates what it might be in the future. The possibilities, forms and methods of body adornment has changed and will continue to change over time, along with our societal and environmental shifts as well as with developments in science and technology. Our bodies are increasingly perceived as malleable objects that we can modify, enhance and improve. I use speculation as a method to explore how the human body may develop and be modified in the future. I envision a world where we have become increasingly intertwined with technologies, where environmental changes and our lifestyle have affected our biology, and where our bodies have continued to be altered based on social norms. Through this project I have become some kind of contemporary Frankenstein scientist, although I am not in a laboratory but in a jewellery workshop. My objects, which I call potential jewellery, or maybe-jewellery, are presented in an installation in the form of a clinical setting. It is a representation of where body adornment meets medical technology, where jewellery meets prostheses and implants. This project does not have any answers or a clear message about what is right or wrong – it is based on a curiosity without having a conclusion in mind. The installation is meant to be a reminder that our bodies are always adorned, modified and in transformation – that we are in an ongoing metamorphosis from one state to another.

Samband mellan geologiska och bergmekaniska egenskaper i bergmaterial som bärlager till riksväg 51 : Riksväg 51 sträckan Svennevad - Kvarntorpskorset

This thesis was created to ensure the quality of the rock which would be crushed to base layer construction material, in a road cut at Swedish highway 51, and to find a possible link between the rock's abrasion resistance and its mineralogy. Rock samples were collected, and among other things, several ball mill tests were carried out. The design of the road project was carried out by Loxia Group AB with NCC Group as contractor.A total of 18 rock samples and 2 base layer samples were taken in the area and they were all tested in a ball mill. The results showed that of the 18 rock samples, 2 samples had a ball mill value of less than 16 on the scale, 10 samples had values from 16 to 20, in addition to this, 3 samples had values from 20 to 21 and 3 samples had values in excess of 21 on the Swedish ball mill scale.Of the 3 samples with a ball mill value higher than 21, all contained a larger amount of biotite. Biotite did not appear to the same extent among the samples that ended up further down the ball mill scale. This link was so clear that a conclusion was subsequently drawn from this. The samples taken on the prefabricated base layer both had a ball mill value between 16 and 20. Overall, both the base layer and the rock material passed the Swedish Government’s Transport Administration’s requirements according to "TRVKB 10, Obundna lager". The rock type that was on the south part of the rock cut, adjacent to a deformation zone, had way too poor quality to undergo a ball mill test and therefore there are no values taken from that area.The conclusion of the work is that the rock material overall meets the Swedish Government’s Transport Administration’s requirements for base layer construction materials according to "TRVKB 10 Obundna lager", that a clear link between the proportion of biotite in a rock material and its abrasion resistance exists, and that the broken rock in the south should not be used as construction materials since the rock has insufficient mechanic capacity.
