Terapia miofuncional orofacial modular em pessoas com disfunção temporomandibular: um estudo eletromiográfico / Orofacial myofunctional therapy in people with temporomandibular disorders: an electromyography study

O objetivo foi verificar a eficácia da terapia fonoaudiológica modular (intervenção breve) em pacientes portadores de disfunção temporomandibular de origem muscular, por meio de questionário e eletromiografia de superfície. Foram avaliados 15 pacientes, encaminhados pela Disciplina de Traumatologia da Faculdade de Odontologia da USP, com diagnóstico de disfunção temporomandibular do tipo muscular. Foram realizadas avaliações na primeira, sétima e décima terceira sessão fonoaudiológica por meio de questionário e eletromiografia dos músculos orbicular dos lábios, região das bochechas, temporais e masseteres direito e esquerdo, em repouso e nos movimentos de apertamento labial, sorriso fechado e fechamento da mandíbula em máxima contração voluntária. O tratamento foi realizado com um programa de exercícios previamente elaborado. Houve diferença significativa nos seguintes valores: diminuição da dor (p<0,0001); diminuição da dor na músculatura da face (p=0,0078); diminuição da fadiga muscular (p=0,0169); na eletromiografia de repouso foram encontrados a diminuição da atividade muscular do músculo masseter esquerdo nas sessões 1 a 6 (p=0,0172). Nos seguintes movimentos, os valores de RMS foram superiores durante o apertamento labial região da bochecha esquerda (p<0,01) da primeira à sexta sessão; fechamento da mandíbula em máxima contração voluntária região da bochecha direita (p=0,0131) nas sessões 1 a 6 e nas sessões 1 e 12 (p=0,0133); na atividade de apertamento labial com relação ao temporal esquerdo (p=0,0245), direito (p=0,0437) e na região da bochecha esquerda (p=0,0246) verificamos que quanto maior o sinal eletromiográfico, menor o nível relatado de dor. O mesmo ocorreu com a região das bochechas esquerda (p=0,0458) e direita (p=0,0398) no movimento de sorriso fechado. Desta forma, conclui-se que a terapia fonoaudiológica modular mostrou ser eficiente em pacientes com disfunção temporomandibular de origem muscular na melhora da sintomatologia dolorosa e na estabilidade do sistema estomatognático / The aim of this study was to verify the efficiency of speech therapy (short intervetion) in patients with temporomandibular disorders of muscular origin, using a questionnaire and surface electromyography. It was evaluated a total of 15 patients, provided by Discipline of Traumatologia da Faculdade de Odontologia da USP, diagnosed with temporomandibular disorder of muscular type. The evaluations were conducted in the first, seventh, and thirteenth speech therapy session with a questionaire and surface electromyography of the obicularis oris, cheek region, temporal and masseter, at rest and movement of lip pressure, closed smile and jaw closing with maxima volunteer contraction. The treatment was done with exercises previous elaborated. There was significant difference at the following values: decrease of pain (p<0.0001); decrease of pain in face muscles (p=0.0078); decrease of muscle fatigue (p=0.0169); in the rest electromyographic was found the reduction of left masseter muscle values from the first to sixth session (p=0.0172). On the fallowing movements, the values of RMS were inferior during the lip pressure left cheek region (p<0.01) from the first to sixth session; jaw closing with maxima volunteer contraction right cheek region (p=0.0131) on the first to sixth session and on the first to thirteenth session (p=0.0133); on the lip pressure in relation to the left temporal muscle (p=0.0245), right (p=0.0437) and on the left cheek region (p=0.0246) was verified that the higher was the electromygraphic signal, less was the level of pain. The same occurred with the left cheek region (p=0.0458) and right (p=0.0398) on the closed smile. Therefore, the speech therapy (short intervetion) showed efficiency in patients with temporomandibular disorders of muscular origin, on the decrease of pain sintomatology and stability of the stomatognatic system

Características odontológicas e prevalência da ardência bucal em doentes com diabetes mellitos do tipo 2 / Dental characteristics and burning mouth prevalence in type 2 diabetes mellitus patients

O Diabetes mellitus (DM) é o transtorno endócrino mais comum e afeta 6% da população mundial. A neuropatia diabética é uma das complicações do DM e acomete até 60% dos doentes diabéticos. Este é um estudo descritivo que teve como objetivo avaliar as condições odontológicas e a prevalência da ardência bucal nos doentes diabéticos tipo 2 com neuropatia diabética. Foram utilizados os seguintes instrumentos de avaliação: exame sensitivo padronizado da face (algiometria), questionário RDC/TMD eixos 1 e 2, (critérios de diagnóstico em pesquisa) para o diagnóstico de disfunção temporomandibular (DTM), protocolo de avaliação de dor orofacial (EDOF-HC), questionário McGill para avaliação de dor e exame periodontal (IS índice de sangramento, PCS profundidade clínica de sondagem, PCI profundidade clínica de inserção). A prevalência de dor orofacial foi 55,2% e de ardor bucal foi 17,2%. Os valores de glicemia em jejum e HbA1c apresentaram-se acima dos valores normais. Dos doentes avaliados, 44,8% eram totalmente desdentados, 41,2% dos doentes apresentaram doença periodontal avançada, prevalência de dor miofascial mastigatória foi 31%. Traços de depressão moderada e grave e de sintomas físicos inespecíficos moderados e graves foram encontrados em 51,7% dos doentes, de incapacidade baixa e moderada em 37,9%. Observou-se comprometimento da sensibilidade dolorosa em V2 lado D (p=0,007), correlacionada com os valores de HbA1c. / Diabetes mellitus (DM) is one of the most common metabolic diseases affecting 6% of the world population. Diabetic neuropathy is one of DM complications and it affects up to 60% of diabetic patients. This descriptive study aimed to evaluate the dental characteristics and burning mouth prevalence of type 2 DM patients. The following instruments were used: pain sensory test, RDC/TMD questionnaire axis 1 and 2 (research diagnostic criteria for temporomandibular disorders), EDOF-HC protocol (for orofacial pain), Mcgills pain questionnaire and periodontal evaluation (bleeding index, clinical probing depth, clinical probing insertion). Orofacial pain was found in 55,2% of the patients, burning mouth was found in 17,2%. Fasting blood glucose and HbA1c levels were found to be higher than the recommended. 44,8% of the patients were totally edentulous, severe periodontal disease prevalence was 41,2%. Masticatory myofascial pain was found in 31% of the evaluated patients. Traces of severe and moderate depression and severe and moderate nonspecific physical symptoms were found in 51,7% of the patients; moderate and low disability in 37,9%. There was significative difference in the algometry values of the V2 right side compared to the left side and to the study group (p=0,007), with positive correlation with the increased values of the HbA1c.

A linguagem da dor: um estudo dos enunciados de sujeitos portadores de disfunção temporomandibular

This was possible by examining the perception of the subjects in relation to previous professional approaches, investigating the existence of conceptual metaphors that describes the expression of pain and then categorizing them and understanding the individual impacts to be bearer of TMD. It was a qualitative and exploratory study, carried out from June to August, 2007. Have participated 18 subjects of both sexes with age ranging from 14 to 64 years with muscle skeletal temporomandibular pain, assisted in the Center for Orofacial Pain Control of the School of Dentistry at the University of Pernambuco Brazil, selected from the Simplified Anamnestic Questionnaire DMF. For the constitution of the corpus were held semi-structured interviews that were recorded and later transcribed. The discussion of the results was based on analysis of the content and it was divided into three parts. The first referred to the representation that the subjects have of previous treatments and pointed to an attitude of health professionals as predominantly routing the patients to other services without explanation. In the second part were discussed data describing of the experience of pain and it was showed the existence of conceptual metaphors, which can be categorized into orientacional, ontological and personified. Finally, in the third part of the analysis/discussion of the results, were investigated the general and specific impacts caused by the pain in the lives of the subjects. It was found that the TMD causes general impacts predominantly in the leisure and work activities. And among specific impacts, causes relevant inconvenience to chew, to yawn, to make oral hygiene, smiling or laughing, and talking. It was concluded that there is a gap in the process of interaction between health professionals and the subjects carrying TMD, and that the Science of Language, through the study of metaphors, may become an important resource for understanding the experience of pain, assisting in the mitigation of impacts caused by pain in the lives of subjects / A presente dissertação teve o objetivo de estudar os enunciados dos sujeitos portadores de Disfunção Temporomandibular (DTM) acerca da experiência da dor na suas vidas. Isso foi possível analisando a percepção dos sujeitos em relação às abordagens profissionais anteriores, investigando a existência de metáforas conceituais que descreviam a manifestação da dor, e em seguida, categorizando-as e compreendendo os impactos particulares de ser portador de DTM. Tratou-se de um estudo qualitativo e exploratório, realizado no período de junho a agosto de 2007. Participaram 18 sujeitos de ambos os sexos, com faixa etária variando entre 14 e 64 anos, com dor músculo-esquelética temporomandibular, atendidos no Centro de Controle da Dor Orofacial da Faculdade de Odontologia da Universidade de Pernambuco Brasil, selecionados a partir do Questionário Anamnésico Simplificado DMF. Para a constituição do corpus foram realizadas entrevistas semi-estruturadas que foram gravadas e posteriormente transcritas. A discussão dos resultados foi baseada na análise de conteúdo e dividida em três partes. A primeira referiu-se à representação que os sujeitos fazem dos tratamentos anteriores e apontou uma atitude dos profissionais de saúde como sendo predominantemente de encaminhar os pacientes a outros serviços sem explicações. Na segunda parte foram discutidos os dados referentes à descrição da experiência da dor e constatouse a existência de metáforas conceituais, sendo possível categorizá-las em orientacionais, ontológicas e personificadas. Por fim, na terceira parte da análise/discussão dos resultados, foram investigados os impactos gerais e específicos causados pela dor nas vidas dos sujeitos. Verificou-se que a DTM causa impactos gerais sobressaindo no lazer e no trabalho e, entre os impactos específicos, causa transtornos relevantes para mastigar, bocejar, fazer higiene oral, sorrir ou gargalhar e falar. Concluiu-se que existe uma lacuna no processo de interação entre os profissionais de saúde e os sujeitos portadores de DTM, e que a Ciência da Linguagem, através do estudo das metáforas, pode tornar-se um importante recurso para a compreensão da experiência dolorosa, auxiliando na atenuação dos impactos causados pela dor na vida dos sujeitos

How does changes in symptom severity index relate to patients’ global impression of change?

Background: The treatment outcome of temporomandibular disorders (TMD) can be assessed with Symptom Severity Index (SSI). In addition to this self-report measure, Patients’ Global Impression of Change (PGIC) provides information about the overall impression of change. The use of SSI and PGIC when assessing treatment outcome in the field of TMD, and their correlation, has not previously been reviewed. Aim: The aims of this study were to investigate the correlation between the scales SSI and PGIC, to gain knowledge about which factors that contribute to the impression of change and to identify the diagnoses that have the most improvement. Methods: Data from 193 patients was analyzed. The percentage change in symptoms between baseline and follow up was compared to PGIC. The patients were divided into three groups based on main diagnoses, for further analysis. PGIC categories “much improved” and “very much improved” were set as successful treatment outcome. Different factors impact on PGIC were analyzed.  Results: Majority of the patients had a successful treatment outcome. A higher mean percentage change correlated with a better PGIC value. Patients with articular related diagnoses needed a greater reduction in SSI to rate the impression of change as improved. According to the regression analysis, diagnosis group, age, number of treatment visits and SSI-baseline were associated with treatment outcome. Conclusions: The present study supports the use of both SSI and PGIC for assessment of the treatment outcome when treating TMD. However, a possible discrepancy between the scales addresses the need for further examination and change of treatment approach.

Central sensitization in orofacial pain

Att retrospektivt undersöka relationen mellan central sensitisering i det orofacialaområdet och refererad smärta, som kliniskt fynd, samt psykosociala faktorer hos patienter medDC/TMD-muskeldiagnosen myofasciell smärta med refererad smärta (MPR). Studien syftadeäven till att undersöka skillnader gällande psykosociala faktorer mellan patienter somdiagnostiserats med DC/TMD muskeldiagnoserna myofasciell smärta med refererad smärta(MPR), lokal myalgi (LM) och patienter med orofacial smärta eller käkdysfunktion men ejkäkmuskeldiagnos (WMD) som kontrollgrupper.Material och metod. Information från 85 patienters DC/TMD-undersökning utförd påOrofaciala smärtenheten vid Malmö högskola mellan september 2012 till årsslutet 2013insamlades retrospektivt. Undersökta variabler inkluderade smärtintensitet, smärt-relateraddysfunktion, psykosociala faktorer (depression, ångest och stress) samt refererad smärta.Patienterna indelades i grupper baserade på muskeldiagnos enligt DC/TMD samt utbredning avsmärta. Non-parametrisk statistik användes och P < 0,05 betraktades som signifikant.Resultat. Patienter med MPR uppvisade en signifikant korrelation mellan totala antaletrefererade smärtlokalisationer och smärt-relaterad dysfunktion (rs = 0,43, n = 49, p = 0,002),depression (rs = 0,32, n = 49, p = 0,023) och stress (rs = 0,39, n = 49, p = 0,006). Patienter meden generell smärtutbredning uppvisade en signifikant högre grad av stress (p = 0,020) samt flerantal refererade smärtlokalisationer (p = 0,019) jämfört med patienter med lokal och/ellerregional orofacial smärta.Konklusion. Studien indikerar att grad av central sensitisering kan bedömas med hjälp avutbredningen av refererad smärta, undersökt enligt DC/TMD, hos patienter med diagnosenmyofasciell smärta med refererad smärta i det orofaciala området. Studien kunde inte påvisaskillnader gällande psykosociala faktorer mellan de undersökta grupperna. / Objective. The aim of this study was to retrospectively investigate the relation between referredpain, as a clinical finding, and psychosocial factors versus central sensitization in patients withmyofascial pain with referral (MPR) as assessed according to DC/TMD. The study also aimedto investigate differences regarding psychosocial factors between patients demonstratingmyofascial pain with referral (MPR) and patients diagnosed with the DC/TMD muscle diagnoselocal myalgia (LM) as well as OFP/TMD patients without masticatory muscular diagnose(WMD) as control patients.Material and methods. Patients’ medical records of 85 patients examined at the Orofacial PainUnit at Malmö University during September 2012 till the end of 2013 were retrospectivelyexamined for DC/TMD data. Examined variables included pain intensity, pain-related disability,psychosocial factors (depression, anxiety and stress) and referred pain. The patients weredivided into groups based on DC/TMD muscle diagnosis as well as extension of pain. Nonparametricstatistics were used and a probability level of P < 0.05 was considered as significant.Results. Patients with MPR demonstrated significant correlations between the total number ofreferred pain sites and disability score (rs = 0.43, n = 49, p = 0.002), depression (rs = 0.32, n =49, p = 0.023) as well as stress (rs = 0.39, n = 49, p = 0.006). Patients with generalized paindistribution demonstrated a significantly higher degree of stress (p = 0.020) as well as highernumber of referred pain sites (p = 0.019) than patients with local and/or regional orofacial pain.Conclusion. This study indicates that the degree of central sensitization can be estimated bythe extent of referred pain, as assessed according to DC/TMD, in patients with myofascial painwith referred pain in the orofacial region. This study could not detect a difference inpsychosocial factors between the three groups, myofascial pain with referral (MPR), localmyalgia (LM) and no masticatory muscle diagnosis (WMD).

However, the numerous TMD treatment protocols described in the literature concerning this relationship, and the effectiveness of these treatments have not been well established. In spite of this many chiropractors treat TMD. TMD is a multifactoral condition and conservative treatment options need to be further investigated in order to determine if manual interventions directed at the cercival spine in the treatment of TMD are beneficial. Objectives: The purpose of this study was to compare myofascial trigger point therapy and manipulative therapy of either the TMJ, cervical spine or a combination of the two in order to determine their effectiveness for the TMD. Method: Thirty participants with TMD were randomly assigned to one of three treatment groups. Participants in each group received two treatments per week for two weeks with a follow up consultation in the third week. Data were collected before the commencement of the first, second, and fourth treatments and at the follow up consultation. Outcome measures included algometer readings, CROM, Mouth opening readings, NRS and a disability questionnaire. SPSS version 15.0 was used for analysis of the data. A p value <0.05 was considered as statistically significant. Multivariate testing was used for intra- and inter-group comparisons. Profile plots were generated to assess the direction and trend of the effect and to visually compare the trends in the different treatment groups. Results: Inter-group comparisons did not reveal any statistically significant different improvements between the three treatment groups. Conclusion: All groups responded favourably to treatment and showed trends towards improvement. However, statistically analysis revealed that no one treatment protocol was superior to the other. Although no definitive inferences may be drawn regarding the effectiveness of each treatment approach, within group trends indicated that the combination of the two treatment approaches may be preferred.

Participação do estresse e ansiedade na alteração do limiar de dor à pressão (LDP) em pacientes com DTM miogênica: um estudo comparativo / Participation of stress/anxiety on the alteration of PPT values in myogenic TMD patients

O objetivo deste trabalho foi analisar a influência da ansiedade e do estresse no limiar de dor à pressão (LDP) de músculos mastigatórios, numa amostra de estudantes universitários em diferentes períodos do ano letivo. Para este propósito, foram selecionados 45 indivíduos, sendo 29 estudantes, que apresentavam DTM de origem miogênica seguindo critérios de inclusão propostos pelo Research Diagnostic Criteria (RDC) e 16 que não apresentavam características de DTM, do gênero feminino, equilibrados em relação à idade. Utilizando um algômetro (KRATOS®) foram realizadas tomadas bilaterais dos limiares de dor à pressão (LDP) dos indivíduos da amostra nos músculos masseter, temporal anterior, médio e posterior. Além disso, os participantes foram solicitados a responder questionários multidimensionais, através do Inventário de Ansiedade de Beck (BAI) e o Inventário de Sintomas de Stress de Lipp (ISSL), para mensurar reações emocionais ou afetivas em situações que causem estresse e/ou ansiedade. Também, o nível de dor foi registrado pela Escala de Análise Visual (EAV). Todos os exames foram realizados em quatro momentos distintos (T1, T2, T3 e T4) tendo como parâmetro o período de avaliações acadêmicas da Faculdade de Odontologia de Bauru. Os dados obtidos foram submetidos à análise estatística (ANOVA, Teste de Tukey, Teste de Friedman e Mann-Whitney), em um nível de significância de 5%. A comparação entre os diferentes tempos do estudo nos 2 grupos mostrou diferença estatisticamente significativa (p<0,05), sendo que o período das avaliações mostrou maiores níveis de estresse e ansiedade e menores valores de LDP. Sob o ponto de vista do músculo nos diferentes grupos e nos diferentes tempos, foram encontradas diferenças estatisticamente significativas (p<0,05). Os resultados da Escala de Análise Visual mostraram diferenças estatisticamente significantes entre o grupo sintomático e o grupo assintomático em T1, T2, T3 e T4. Com relação ao estresse e ansiedade, não houve diferenças estatisticamente significante entre os grupos. Houve, no entanto, uma associação entre o aumento do estresse e da ansiedade e diminuição dos valores de LDP em cada tempo. Concluiu-se que existe relação entre estresse e ansiedade e LDP tanto para indivíduos assintomáticos quanto para sintomáticos com DTM de origem miogênica. / The aim of this research was to evaluate the influence of stress and anxiety on the Pressure Pain Threshold (PPT) of the masticatory muscles and on the subjective pain report of dental students of the Bauru School of Dentistry (University of São Paulo, Brazil) at different situations. Forty-five females, matched for age, were divided into two groups: 29 presenting with myofascial pain, according to the RDC/TMD criteria, and 16 with no TMD signs or symptoms. PPT measurements were taken bilaterally at the masseter, anterior, middle and posterior temporalis muscles in four different occasions throughout the academic year. The Achilles tendon was used as control. In order to quantify emotional or affective reactions under stress/anxiety situations, the sample were requested to fill out multidimensional questionnaires, such as the Beck Anxiety Inventory (BAI) and the Lipp Inventory for Stress Symptoms (LISS). In addition, pain levels were registered with a Visual Analog Scale (VAS). Data obtained were submitted to statistical analysis (ANOVA, Tukey, Friedman, and Mann-Whitney tests), at a 5% significance level. The VAS and PPT had a negative correlation, regardless the period, and group studied (p<.05). Higher levels of stress and anxiety were detected at the time of school examinations for both groups, with a strong association with decreased PPT figures (p<.05). Stress and anxiety, however, were not statistically different between groups. It can be concluded that external stressors as academic examinations have a potential impact on the masticatory muscle tenderness, regardless the presence of a previous condition, such as the masticatory myofascial pain.

A Migrânea e a Disfunção Temporomandibular (DTM) são doenças crônicas e tem como aspecto mais importante a dor crônica. Muitos trabalhos descrevem sinais e sintomas de DTM em pacientes com cefaleia sugerindo uma associação entre essas duas condições. Porém, ainda são poucos os trabalhos que utilizaram um critério que fornecesse não apenas sinais e sintomas, mas também a classificação diagnóstica. Assim, o objetivo deste trabalho foi avaliar a frequência da DTM a partir da aplicação do RDC/TMD e a severidade da DTM através do Índice Anamnésico de Fonseca em mulheres com Migrânea, Migrânea Crônica e mulheres sem queixa de cefaleia. Participaram deste estudo 91 mulheres, divididas em três grupos: 30 mulheres no Grupo Controle (GC), 38 mulheres no Grupo Migrânea (GM) e 23 mulheres no Grupo Migrânea Crônica (GMC). As voluntárias dos grupos GM e GMC foram selecionadas durante a primeira consulta no Ambulatório de Cefaleia (ACEF) do Hospital das Clínicas de Ribeirão Preto da Faculdade de Medicina da Universidade de São Paulo e as do GC entre as acompanhantes dos pacientes naquele mesmo dia. Foram inclusas voluntárias com idade entre 18 e 55 anos, que não tivessem ingerido antiinflamatórios e/ou analgésicos nas últimas 24 horas antecedentes à avaliação fisioterapêutica, que não tivesse história de trauma na face, nem usasse prótese dentária parcial ou total e foram excluídas voluntárias com outros tipos de cefaleia e doenças sistêmicas como fibromialgia e artrite reumatóide. Para o GC, as voluntárias não podiam ter queixa de cefaleia nos últimos 3 meses. Três examinadores participaram da coleta. Para análise dos dados foi utilizado a análise da variância (ANOVA two-Way p<0,05) na comparação dos dados antropométricos e da amplitude de movimento mandibular entre os três grupos e Tukey como post-hoc análise para avaliar a diferença no número de locais dolorosos e o número de diagnósticos, com nível de significância de (p<0,05). O teste qui-quadrado foi utilizado para verificar a diferença de diagnóstico entre os três grupos e para verificar a freqüência da severidade entre os três grupos foi utilizado o teste exato de Fisher. As voluntárias dos grupos com migrânea apresentaram maior frequência de diagnósticos de DTM em comparação ao GC, p<0,05 e não foi observada diferença entre os grupos com migrânea. Os diagnósticos, segundo o RDC/TMD, do grupo I (dor miofascial) foram os mais prevalentes nos três grupos estudados e foi mais freqüente nos grupos com migrânea. Os diagnósticos dos grupos I+III estiveram presentes nos três grupos estudados e com maior freqüência no grupo GM. Não foram encontrados diagnósticos individuais pertencentes apenas aos grupos II e III. O número de pontos dolorosos musculares foi significativamente maior nos grupos GM e GMC em relação ao controle e não houve diferença entre os grupos com GM e GMC. As voluntárias dos grupos com migrânea apresentaram maiores graus de severidade de DTM, em comparação ao GC, p<0,05 e o GMC apresentou maior severidade que o GM. Os resultados deste trabalho demonstraram que Mulheres com Migrânea tem maior frequência de DTM, apresentam maior número de diagnósticos e de pontos dolorosos segundo o RDC/TMD que mulheres sem migrânea bem como maiores graus de severidade de DTM. Portanto a DTM e a Migrânea estão clinicamente relacionadas. / Migraine and Temporomandibular Disorders (TMD) are chronic disorders and their most important aspect is the chronic pain. The persistent cranial-cervical and orofacial pain is the predominant reason why people seek treatment. Besides the pain, the affected areas are also similar suggesting an association between these two conditions. Many studies describe signs and symptoms of TMD in patients with headache suggesting an association between these two conditions. However, there are few studies that use a criterion that provides not only signs and symptoms, but also the diagnostic classification. The Diagnostic Criteria for Research in Temporomandibular disorders (RDC / TMD) provide a diagnostic classification with reports of acceptable levels of reliability, provide specifications for the conduct of a clinical standard and allow the development of clinical diagnoses (Axis I), the classification psychosocial classification of the individual (Axis II). The objective of this study was to evaluate the frequency of the DTM from the application of the RDC / TMD and TMD severity through the history index of Fonseca in women with migraine, chronic migraine and women with complaints of headache. The study included 91 women, divided into three groups: 30 women in the control group (CG), 38 women in the migraine group (GM) and 23 women in Chronic Migraine Group (CMG). The voluntary groups GM and GMC were selected during the first appointment and at the Headache Clinic (ACEF) of the University Hospital of Ribeirão Pretos Faculty of Medicine, (University of São Paulo) and the ones of CG were selected among the companions of the patients during that day. There were included volunteers aged between 18 and 55, who had not ingested anti-inflammatory and / or painkillers in the last 24 hours before the physical therapy evaluation and who didnt have a history of face trauma, or wore partial or total dentures and volunteers with other types of headache and systemic diseases such as fibromyalgia and rheumatoid arthritis were excluded. For the GC, the volunteers could not have complained of headache in the last 3 months. Three examiners participated in data collection, I and II in the initial screening of volunteers and III in the implementation of the RDC / TMD and the history index of Fonseca, the examiner III was blind to the conditions of the volunteers and was previously trained. In order to analyze the Data it was used analysis of variance (two-way ANOVA p <0.05) in comparison of anthropometric data and range of mandibular movement between groups and Tukey as posthoc analysis to evaluate the difference in the number of painful sites and the number of diagnoses, with a significance level of p <0.05. The chi-square test was used to determine the difference in diagnosis between the three groups and to determine the frequency of severity among the three groups used the Fisher exact test. The voluntary group with migraine showed a higher frequency of diagnosis and higher severity of TMD in comparison to the CG, p <0.05 and no difference was observed between the groups with migraine.

Avaliação da sensibilidade mecânica, ansiedade e neuroplasticidade cortical motora em ratos submetidos à doença periodontal experimental / Mechanical sensibility, anxiety and motor cortex plasticity in rats with experimental periodontal disease

INTRODUÇÃO: Sabe-se que a doença periodontal é a doença infecciosa crônica mais prevalente na população adulta, acometendo 1/5 da população em fase ativa de trabalho e causadora de dor. As citocinas e demais substâncias moduladoras da inflamação que promovem e perpetuam a doença periodontal interagem e agravam outras doenças como o diabetes mellitus, aterosclerose e doenças autoimunes. O periodonto, com suas aferências proprioceptivas especializadas, desempenha importante papel de regulação dos movimentos da mastigação. Apesar da região orofacial ter uma ampla área de representação no córtex cerebral não se sabem os efeitos de uma inflamação crônica periodontal nesta representação. Ansiedade e estresse são conhecidos fatores correlacionados à doença periodontal e moduladores de condições dolorosas. Assim, são objetivos deste estudo a avaliação da sensibilidade mecânica em vibrissas, avaliação da ansiedade e o mapeamento do córtex motor de ratos com doença periodontal de 14 e 28 dias de evolução. MÉTODOS: Foram utilizados ratos Wistar machos pesando inicialmente entre 140 e 180 gramas, divididos em 3 grupos principais: grupo controle, grupo sham e grupo de estudo, em que a doença periodontal foi induzida por amarria no primeiro molar inferior direito. Os animais foram avaliados com 14 e 28 dias de doença, sendo a sensibilidade mecânica aferida por filamentos de Von Frey e a ansiedade avaliada por meio do labirinto em cruz elevado. O mapeamento do córtex motor foi realizado por estimulação elétrica epidural de um a dez volts. RESULTADOS: Ambos os períodos de evolução da doença causaram alterações condizentes com doença periodontal. Não houve diferenças no ganho de peso dos animais independente do grupo ao longo do estudo. Nos animais com 28 dias de doença houve maior nocicepção no lado doente com diferença estatística (p=0,042). Os animais do grupo de estudo 14 dias demonstraram mais comportamentos sugestivos de ansiedade nos parâmetros de congelamento (p=0,031), entradas nas extremidades dos braços abertos (p=0,048) e esticamento nos braços abertos (p=0,047) do labirinto em cruz elevado do que os grupos sham e controle. Os animais do grupo de estudo de 28 dias apresentaram maior comportamento de dor e medo observado através da presença do congelamento (p=0,016). O mapeamento do córtex motor evidenciou áreas representativas da mandíbula sobrepostas à área das vibrissas. Foi observada uma expansão da área da mandíbula no grupo da doença periodontal de 14 dias (p=0,038). CONCLUSÃO: A doença periodontal com 14 dias de evolução causou um aumento da resposta motora ipsilateral da mandíbula-mais-vibrissas e alterações de comportamento sugestivas de ansiedade / BACKGROUND: Periodontal disease is the most prevalent chronical infectious disease in adults, affecting 1/5 of people in active phase of labor and causing pain. Cytokines and substances that modulate inflammation promotes and perpetuate de periodontal disease, interacting and worsening other conditions such as diabetes mellitus, aterosclerosis and autoimmune diseases. The periodontium has specialized proprioceptive afferents and plays a role in the regulation of masticatory movements. Despite the wide brain cortical representation area of the orofacial structures, the periodontium has not been described in the literature and the effects of a chronical inflammation in its representation has not been clarified. Anxiety and stress are factors correlated to periodontal disease and modulators of pain. The aim of this study was evaluate vibrissae nociception through mechanical response, anxiety and the motor cortex mapping in rats with periodontal disease after 14 or 28 days of evolution. METHODS: Male Wistar rats (initial weight 140-180 grams) divided into 3 main groups: controls, sham and periodontal disease induced by placement of a cotton ligature in the mandibular right first molar tooth. The evaluations took place after 14 or 28 days of disease. Mechanical response was evaluated by von Frey filaments and anxiety was evaluated through the elevated plus maze. Epidural electrical stimulation (1 to 10 volts) was the method used for the cortical motor mapping. RESULTS: Both evolution periods caused clinical outcomes consistent to periodontal disease. There was no difference in the weight gain of the animals, whatever the group during the study. The animals with 28 days appeared to have a higher nociception in the side affected, with statistical difference (p=0.042). Animals with the disease evolution through 14 days showed more anxious behavior seen by freezing (p=0.031), entries in the extremities of open arms (p=0.048) and stretching in open arms (p=0.047) than the sham and control groups, in the elevated plus maze test. The 28 day disease evolution group showed more pain and fear behavior, seen by freezing (p=0.016). The cortical motor mapping showed an overlapping of jaw and vibrissae areas. There was an expansion of the mandibular area in the 14-day disease group (p=0.038). CONCLUSION: at 14-days, PD led to an expansion of the mandibular-plus-vibrissae motor cortical representation ipsilateral to the disease and behaviors suggestive of anxiety
