Encontros de aprendizagem e governamentalidade no trabalho em saúde : as residências no País das Maravilhas / Learning meetings and governmentality in the work in health : Residency Programs in the Wonderland / Encuentros de aprendizaje y gubernamentalidad en el trabajo en salud : las Residencias en el País de las Maravillas

Encantada e enredada pela fabulosa história de Alice no País das Maravilhas, nesta tese se constroem três possibilidades de olhar para as Residências em Saúde. Uma primeira possibilidade é tecida com a experiência da autora, segundo suas memórias, sentimentos e afecções, registro produzido em seus “encontros” com a temática e sua pragmática. A segunda possibilidade sumariza elementos de pesquisa com base em teses e dissertações elaboradas no Brasil, no período de 1987 a 2012, sobre Residências em Área Profissional da Saúde e Médicas. A terceira possibilidade, a tecedura de um País das Maravilhas das Residências, resultado de uma pesquisa constituída por conversas. Conversas empreendidas com participantes dos Encontros – Gaúcho e Nacional – de Residências e também do X Congresso da Associação Brasileira de Saúde Coletiva, eventos ocorridos no ano de 2012. Esses participantes, os quais receberam os nomes dos habitantes do “País” de Lewis Carroll, foram conversando com este estudo sobre detalhes do País das Maravilhas das Residências. Esta pesquisa possibilitou também manifestações à provocação “como você expressaria uma experiência de aprendizagem na Residência em Saúde (imagem, som, narrativa, poesia...)?” A análise das conversas e das manifestações livres foi empreendida juntamente com a análise de regramentos emitidos pelas Comissões Nacionais de Residência Médica e Multiprofissional em Saúde. Na invenção do País das Maravilhas das Residências, o material empírico foi recortado, didaticamente, baseado em dois analisadores: a governamentalidade (a partir de Foucault) e os encontros de aprendizagem em saúde (tomando os signos, conforme Deleuze). Como resultados, estão apontadas a inseparabilidade desses dois conceitos e sua produção mútua, mas, também, os modos de liberdade e captura que vão aparecendo na fabricação deste “País das Residências”. A tarefa política, no interesse das aprendizagens ou do exercício da governamentalização, é de apostar nas singularizações, modos de ser profissional da saúde, de formar para o cuidado do outro e, neste sentido, possibilitar liberdade aos “encontros” para que não sejam constrangidos pela forma. Ao fim destes escritos, um reencontro com o Coelho Branco, como possibilidade de continuar despertando curiosidades de pesquisa, interrogações, instigantes, problemas de pensamento e movimento em relação ao tema, uma apropriação complexa do “trabalhar” em saúde, resultado dos encontros de aprendizagem. A governamentalidade, naquilo que parece ser o “império” da forma, enfraquece justamente esta apropriação. / Enchanted and entangled by the fabulous story of Alice in the Wonderland, three possibilities of looking at the Health Residencies are built in this dissertation. An initial possibility is woven with the author experience, according to her memories, feelings, and affections, a record produced in her “meetings” with the theme and its pragmatics. The second possibility summarizes elements of research based on theses and dissertations produced in Brazil, between 1987 and 2012, on Medical and Health Professional Area Residency Programs. The third possibility, the weaving of a Residency Programs Wonderland, resulted from a research composed of conversations. These conversations were undertaken with the participants of the National and State Meetings of Residency Programs, as well as the X Congress of the Brazilian Association of Collective Health, events that occurred in 2012. These participants, who received the names of the inhabitants of Lewis Carroll’s “Land,” dialogued with this study on details of the Residency Programs Wonderland. This research also made possible manifestations to the question “How would you express an experience of learning in the Health Residency Program (image, sound, narrative, poetry…)?” The analysis of the conversations and the free manifestations was undertaken together with the analysis of regulations issued by the National Commissions of Medical and Multiprofessional Health Residency Programs. In the invention of the Residency Programs Wonderland, the empirical material was didactically cut out, based on two analyzers: Foucault’s governmentality, and the learning meetings in health (taking the signs, according to Deleuze). As results, the inseparability of these two concepts and its mutual production, as well as the ways of freedom and capture that appear in the manufacturing of this “Residency Programs Land,” is indicated. The political task, in the interest of the learning or the exercise of governmentalization, is to bet in the singularization, ways of being a health professional, of educating for the care of the other, and, in this sense, to provide freedom to the “meetings”, so that they are not constrained by the form. At the end of these writings, a reunion with the White Rabbit, as a possibility to continue arousing research curiosity, exciting interrogations, problems of thought, and movement in relation to the theme, a complex appropriation of “to work” in health, resulting from the learning meetings. Una primera posibilidad es tejida con la experiencia de la autora, desde sus memorias, sentimientos y afecciones, registro producido en sus “encuentros” con la temática y su pragmática. La segunda posibilidad sumariza elementos de investigación con base en tesis y disertaciones elaboradas en Brasil sobre residencias médicas y en áreas profesionales de la salud, que comprende el período de 1987 a 2012. La tercera posibilidad, la tejedura de un País de las Maravillas de las residencias, resultado de una investigación constituida por diálogos. Residência integrada em saúde : uma modalidade de ensino em serviço

A legalização do Sistema Único de Saúde (SUS), o qual preconiza uma visão ampliada de saúde, os setores da saúde e educação apontam para a necessidade de uma reforma na formação profissional. A Residência Integrada em Saúde (RIS) é uma modalidade de educação em serviço que objetiva especializar profissionais das diversas categorias desta área para atuarem em equipe, segundo os princípios do SUS. Esta dissertação tem a finalidade de contribuir para a reflexão sobre a formação de trabalhadores da área da saúde, através de uma pesquisa tipo Estudo de Caso sobre a RIS a partir da área de ênfase em intensivismo do GHC. Trata-se de uma abordagem qualitativa, descritiva, analítica que utiliza etapas da Avaliação de Quarta Geração. Os participantes deste estudo foram o grupo de interesse composto pelos residentes dessa área de ênfase. Para analisar as informações foi utilizada a análise proposta por Minayo. Os resultados desta pesquisa mostraram que a proposta da RIS é especializar profissionais da área da saúde para agirem em equipe interdisciplinar, visando formá-los com base em um perfil adequado a atuar no SUS para além da qualificação técnica curativa. A RIS diferencia-se das outras residências por ter como proposta a integração das diferentes áreas de conhecimento. Essa modalidade de ensino em serviço, em algum grau, tem atingido seus objetivos, porém ela tem apresentado dificuldades relacionadas à falta de conhecimento da proposta, à organização e estrutura dessa formação. As atividades interdisciplinares não estão dadas, assim como a metodologia problematizadora preconizada por esse programa leva a uma contínua reformulação das atividades para atingir os objetivos da formação. Esse caráter dinâmico pode dar um aspecto de desordem ao processo, todavia foi percebido que a proposta está continuamente sendo avaliada, reformulada e tem apresentado mudanças e crescimento em relação à sua criação. / With the legalized Brazilian Health System (SUS-Sistema Único de Saúde), which advocates a broader view of health, the health and education sectors indicate the need to remodel professional training. Integrated Health Residency (RIS-Residência Integrada em Saúde) is a modality of on-the-job training aiming at specializing professionals of different categories in this field to work as a team according to the SUS principles. The purpose of this thesis is to contribute to thinking about health care worker training, by performing a Case Study-type research on RIS based on an emphasis on intensive care at GHC. This is a qualitative, descriptive, analytical approach that uses stages of the Fourth Generation Evaluation. The participants in this study were the group of interest constituted by residents in this particular field. The method proposed by Minayo was used to analyse this information. The results of this research showed that the RIS proposal is to specialize health care professionals to work in a crossdisciplinary team, with a view to training them based on a profile appropriate to work at SUS beyond the curative technical qualification. RIS is different from other residencies because it proposes to integrate different fields of knowledge. This modality of on-the-job training has achieved its aims to some degree, but it has presented problems involving lack of knowledge about the proposal, the organization and structure of this training. Crossdisciplinary activities are not given, and the problematizing methodology advocated by this program leads to the continuous reformulation of activities to achieve the training objectives. This dynamic character could give the process a disorganized look, but it was noted that the proposal is continuously evaluated and reformulated, and it has presented changes and growth compared to when it was created. / La legalización del Sistema Único de Salud (SUS), lo cual preconiza una visión ampliada de salud, los sectores de la salud y educación apuntan para la necesidad de una reforma en la formación profesional. La Residencia Integrada en Salud (RIS) es una modalidad de educación en servicio que objetiva especializar profesionales de las diversas categorías de esta área para que actúen en equipo, según los principios del SUS. Esta disertación tiene la finalidad de contribuir para la reflexión sobre la formación de trabajadores del área de la salud, a través de una pesquisa tipo Estudio de Caso sobre la RIS a partir del área de énfasis en intensivismo del GHC. Esa modalidad de enseñanza en servicio, en algún grado, tiene atingido sus objetivos, sin embargo ella tiene presentado dificultades relacionadas a la falta de conocimiento de la propuesta, a la organización y estructura de esa formación. Las actividades interdisciplinares no están dadas, así como la metodología problematizadora preconizada por ese programa lleva a una continua reformulación de las actividades para atingir los objetivos de la formación. Ese carácter dinámico puede dar un aspecto de desorden al proceso, sin embargo fue percibido que la propuesta está continuamente siendo evaluada, reformulada y tiene presentado cambios y crecimiento en relación a su crianza.

Turismo de segunda residência em Praia Grande (SP) / Tourism of second residence in Praia Grande (SP)

A presente dissertação analisa o fenômeno do turismo de segunda residência sob o prisma da geografia humana e histórica da cidade de Praia Grande e sua inserção no turismo, produção do espaço e segunda residência. O objetivo desta pesquisa é o de analisar o processo histórico da urbanização de Praia Grande, desde a época em que ainda fazia parte de São Vicente e entender como se deu a produção do espaço amparada pelo turismo de segunda residência. O procedimento metodológico utilizado procurou entender os fatos, as políticas públicas e as ações que levaram às transformações, crescimento e desenvolvimento de Praia Grande do início do século passado até os dias atuais. Os resultados desta pesquisa mostram como se deu a descoberta da atividade turística desta estância balneária, pertencente à Região Metropolitana da Baixada Santista (RMBS), que vem despertando o interesse dos mais variados segmentos da sociedade, sejam eles, das áreas da construção civil, planejamento, investimentos, comércio, dentre outros. Por conta disso, constatamos um abrupto crescimento e desenvolvimento desta estância balneária com mudanças paisagísticas e investimentos na sua infra-estrutura, que em seu bojo, trouxeram mais recursos e investimentos para esta cidade média paulista. / The present dissertation analyses the phenomenon of tourism of second residency under the prism of Human Geography and the history of the city of Praia Grande and its insertation in tourism, production of space and second residency. The main purpose of this research is to analyze the historic process of Praia Grande´s urbanization since the time it was still a neighborhood of São Vicente and understand how the production of space has occurred supported by the second residency. The methodological procedure used has searched to understand the facts, public politics and actions that led this city to transformation, growing and development since the beginning of last century until today. The results of this research has shown how tourism activity appears in this city, which is part of the Metropolitan Region of Baixada Santista (RMBS) that nowadays is being the target of many segments of the society such as civil construction, planning, investment, business and other ones. In this way, we have verified an abrupt growing and development of this city with paisagistic changes and improvements in its infra-structure that all together brought new resources and investments to this paulistan medium sized city.

Perceptions d’infirmières nouvellement diplômées sur la contribution d’un programme de résidence infirmière à leur compétence de développement professionnel

Une multitude d’études se sont penchées, dans les dernières décennies, sur les programmes de transition, tels que les programmes de résidence infirmière, ainsi que leurs divers effets sur l’expérience d’intégration à la pratique des infirmières nouvellement diplômées (IND). Les programmes de résidence auraient un impact positif sur le développement de leurs habiletés et compétences ainsi que sur leur intégration sociale au sein d’unité de soins. Certaines études témoignent de l’importance du développement professionnel dans le maintien et l’amélioration de la qualité des soins, mais aucune étude, à notre connaissance, ne se penche sur l’influence d’un programme de résidence infirmière (PRI) sur la compétence de l’IND à s’investir dans son développement professionnel. Cette étude a pour but d’explorer les perceptions d’IND sur le développement de leur compétence « Assurer son développement professionnel et contribuer au développement des compétences de ses collègues » à la suite de leur participation à un PRI sur une unité de soins intensifs néonatals. Pour cette recherche, le référentiel de compétences développé au Centre hospitalier universitaire (CHU) Sainte-Justine a servi de cadre référence. Cette étude qualitative exploratoire au devis descriptif interprétatif comporte une collecte de données au moyen d’entrevues individuelles semi-structurées auprès de six infirmières ayant débuté, dans leurs six premiers mois de pratique, un programme de résidence infirmière à l’unité de soins intensifs néonatals d’un centre hospitalier universitaire pédiatrique du Québec. À la suite de l’analyse thématique des données collectées, les résultats portent sur le développement professionnel au service de l’amélioration continue de la pratique infirmière, l’évolution de la vision de la pratique infirmière, l’évolution des moyens pour assurer leur développement professionnel ainsi que le programme de résidence infirmière comme levier au développement professionnel. À la lumière de ces résultats, différentes recommandations pour la pratique, la recherche ainsi que la formation sont émises. / Over the past decades, multiple studies have looked into transition to practice programs such as nurse residency programs (NRP), as well as their various effects on the entry-to-practice experience of newly graduated nurses (NGN). NRP have a positive impact on the NGN’s development of skills and competencies as well as social integration within their unit. Some studies show the importance of professional development in maintaining and improving the quality of care, but no study, to our knowledge, examines the influence of a NRP on the competency of the NGN to invest in their professional development. The purpose of this study is to explore NGN's perceptions of the development of the competency "To ensure professional development and contribute to the competency development of their of colleagues" following their participation in an NRP on a neonatal intensive care unit. For this research, the competency framework developed at the Sainte-Justine University Hospital Center served as a study framework. This exploratory qualitative study with an interpretative descriptive design involves data collection by means of semi-structured individual interviews with six nurses who, in their first six months of practice, began a nursing residency program in the neonatal intensive care unit. of a pediatric university hospital center in Quebec. Following the thematic analysis of the data collected, the results relate to professional development for the continuous improvement of nursing practice, the evolution of the vision of nursing practice, the evolution of means to ensure their professional development as well as the NRP as a lever for professional development. In the light of these results, various recommendations for practice, research as well as training are issued.

Structure des programmes de résidence pour les infirmiers nouvellement diplômés aux soins intensifs : une revue de la portée

La période de transition des infirmières et infirmiers nouvellement diplômés (IND) dans la pratique clinique est marquée par des défis individuels, émotionnels et professionnels. Ces défis peuvent contribuer à un épuisement professionnel et ainsi augmenter le taux d’attrition des IND dans les unités de soins. La mise en place d’un programme de résidence en soins infirmiers permet de répondre à ces défis et entraîne donc des répercussions positives au niveau individuel, professionnel et institutionnel. Toutefois, aucune revue des écrits ne recense les programmes de résidence en soins infirmiers pour les IND débutant dans le secteur des soins intensifs. Le but de cette revue de la portée est donc d’identifier et de décrire la structure des programmes de résidence en soins infirmiers pour les IND dans les unités des soins intensifs. Cette revue de la portée réalisée selon la méthode du Joanna Briggs Institute a recensé un total de 241 références, dont huit ont été retenues. L’extraction des données a été réalisée en suivant le modèle de structure de programme de résidence en soins infirmiers de Chant et Westendorf (2019) comprenant sept concepts-clés (cadre de référence, objectifs initiaux, préceptorat, mentorat, composantes didactiques, durée, immersion clinique). Quatre constats provenant des résultats obtenus ont pu être tirés et discutés: une grande disparité dans la structure des programmes de résidence, un besoin d’encadrer davantage le développement des compétences des IND par des cadres de référence et des principes pédagogiques, la nécessité de répondre aux besoins de transition des IND et le besoin de prendre en compte les ressources disponibles. Ces constats illustrent la pertinence de l’implication des formateurs et des gestionnaires pour répondre aux besoins de transition des IND sur les unités de soins intensifs. / The period of transition of new graduate nurses (NGN) into clinical practice is marked by individual, emotional and professional challenges. These challenges can contribute to a burnout and increase turnover rates for care units. The implementation of a nurse residency program for NGN transitioning into practice can address these challenges and lead to positive repercussions at individual, professional and institutional levels. The purpose of this scoping review is to identify and describe the structure of nursing residency programs for NGN in intensive care units. This scoping review using the Joanna Briggs Institute method identified a total of 241 references, of which eight were retained. The extraction followed Chant and Westendorf's (2019) structure model using seven key concepts (framework/models, defined outcomes, preceptorship, mentorship, didactic components, program length, clinical immersion). Four findings were drawn and discussed from the results obtained: a wide disparity in the structure of residency programs, a need for more guidance in the development of NGN competencies by using frameworks and pedagogical principles, the necessity to answer to the transition needs of NGN, and the need to consider the available resources. These findings highlighted the relevance of the involvement of trainers and managers to help NGN transition into intensive care units.

The importance of selective filters on vessel biofouling invasion processes

The spread of exotic species is considered to be one of the most significant threats to ecosystems and emphasises the need for appropriate management interventions. The majority of marine non-indigenous species (NIS) are believed to have been introduced via ship biofouling and their domestic spread continues to take place via this mechanism. In some countries, biosecurity systems have been developed to prevent the introduction of NIS through biofouling. However, implementing biosecurity strategies is difficult due to the challenges around identifying high-risk vectors. Reliable predictors of risk have remained elusive, in part due to a lack of scientific knowledge. Nonetheless, invasion ecology is an active scientific field that aims to build this knowledge. Propagule pressure is of particular interest in invasion ecology as it describes the quantity and quality of the propagules introduced into a recipient region and is considered to be an important determinant in the successful establishment of NIS. Environmental history affects health and reproductive output of an organism and, therefore, it is beneficial to examine this experimentally in the context of biofouling and propagule pressure. The aim of this thesis was to examine how voyage characteristics influence biofouling recruitment, survivorship, growth, reproduction and offspring performance through the ship invasion pathway. This was to provide fundamental knowledge to assist managers with identifying high-risk vessels that are likely to facilitate the introduction or domestic spread of NIS, and to understand the processes affecting biofouling organisms during long-distance dispersal events. Chapter One provides an introduction to the issues addressed in this thesis. Each data chapter (Chapters Two – Five) then focused on a stage of the invasion process and included field experiments using a model organism, Bugula neritina. Finally, Chapter Six provides a summary of key findings, discussion and the implications to biosecurity management. Throughout this thesis, the effect of donor port residency period on the success of recruits was highlighted. Chapter Two focused on recruitment in the donor region. As expected, recruitment increased with residency period. Importantly, recruitment occurred every day on vulnerable surfaces, therefore, periods as short as only a few days are able to entrain recruits to a vessel hull. The study presented in Chapter Three showed that there was high survivorship of B. neritina recruits during 12 translocation scenarios tested. In particular, the juvenile short-residency recruits (1-8 days) survived voyages of 8 days at a speed of 18 knots; the longest and fastest voyage simulated. Interestingly, variation in voyage speed and voyage duration had no effect on the survivorship of recruits, but did have legacy effects on post-voyage growth. Again, B. neritina which recruited over very short residency periods of 1 day continued to perform well after translocation and had the highest level of reproductive output after the voyage scenarios (Chapter Four). Recruits that were older (32-days) and reproductively mature at the commencement of the scenarios failed to release any propagules. Even though the number of ‘at sea’ and ‘port residency’ days were equal, reproductive output was higher after short and frequent voyages than after long and infrequent voyages. Finally, the study presented in Chapter Five examined transgenerational effects of B. nertina. Results showed that although the environmental history of the parent colony had a carry-over effect on offspring performance, it was the offspring environment that was a stronger determinant of success (measured by reproductive output and growth). Although cross-vector spread is possible (i.e. parent and offspring both fouling an active vessel), offspring released from a hull fouling parent into a recipient environment will perform better. In combination, these studies have provided new insights into NIS transport via vessel biofouling. Although shipping pathways are dynamic and complex, these results suggest that juvenile stages that recruit over short residency periods and are then translocated on short voyages, may pose a higher risk for NIS introduction than originally assumed. This has implications for marine biosecurity management as short residency periods are common and short, frequent voyages are typical of domestic vessel movements which are largely unmanaged.
