The experiences of male nurses in midwifery clinical training at a regional hospital in the Eastern Cape

The purpose of the study was to explore and describe the experiences of male community service officers during midwifery clinical training. A qualitative, explorative, descriptive and contextual design was used in order to achieve the study objectives. Data was collected by means of semi-structured interviews. Data analysis elucidated analogous themes and categories which translated into the overall experiences of the participants. The study found that whilst the experience was described as beneficial, there were overwhelming challenges in providing intimate care to female clients, dealing with ethno-cultural aspects, meeting clinical practice requirements and the right to choose to be part of midwifery clinical training. Recommendations to nurse educators, clinical midwives and midwifery clinical supervisors with regard to better support for male nurses during midwifery clinical training emanated from the study findings / Health Studies / M.A. (Health Studies)

MUSIC FOR BRASS QUINTET WITH ORCHESTRAL ACCOMPANIMENT: COMMISSIONED WORKS, THE ANNAPOLIS BRASS QUINTET, AND A SURVEY OF LITERATURE FOR BRASS QUINTET AND ORCHESTRA

Today’s leading brass chamber ensemble is the brass quintet, whose inception was relatively late compared to the string quartet or woodwind quintet. The first modern brass quintet formed in the 1950s, while the first string quartet can be traced to the 17th century. Compositions for woodwind quintet were written as early as 1811 during the Classical Period. The New York Brass Quintet, American Brass Quintet, and Annapolis Brass Quintet commissioned a large portion of the currently existing brass quintet literature. The literature grew exponentially as the brass quintet became popular in the 1960s. Also during this time, a new genre of works emerged for brass quintet with orchestral accompaniment. The paper references fifty-seven works for the brass quintet with orchestral accompaniment that were found through music catalogues, reviews, recordings and searching JSTOR, World Cat and Google. Since the author was not able to discover any scholarly treatment of this genre, this paper will address the gap and unearth the quantity of literature available. Many of these works are unrecorded. While there are many existing scores in the literature, there is a resurgence of compositions currently being written for brass quintet with orchestral accompaniment. This document is presented in two parts: Part I, “Overview of Brass Chamber Music in the Twentieth Century,” “Earliest Music in the United States for Brass Chamber Ensembles,” “A Brief History of the Modern Brass Quintet,” “Annapolis Brass Quintet,” and “A Survey of Existing Works for Brass Quintet and Orchestra.” The second part of this dissertation contains materials which are pertinent to the Doctor of Musical Arts Degree which include recital programs, program notes, and vita.

Depuis le décret 2009-378 visant à favoriser la coopération entre l’Education nationale et le secteur médico-social, l’Ecole ne peut plus être pensée comme un territoire qui s’arrêterait là où commence celui de la filière spécialisée, représentée en France par le secteur médicosocial.Derrière cette volonté, l’enjeu significatif est celui d’un rapprochement entre deux cultures, celle de l’enseignant d’une part, celle de l’éducateur spécialisé d’autre part. C’est dans ce contexte que s’inscrit notre thèse, coproduite par une éducatrice spécialisée et un enseignant.Les acteurs éducatifs étant amenés à coopérer pour répondre au droit à la scolarisation édicté par la loi du 11 février 2005 et aux décrets qui lui font suite, nous cherchons à identifier les fondements de ces cultures et ce qui façonne les pratiques actuelles. Ce travail se situe alors dans une perspective compréhensive et non dans une logique expérimentale guidée par une volonté d’administration de la preuve.Par une démarche ethnographique, combinant l’observation participante et les entretiens semi-dirigés, nous nous approchons au plus près des pratiques. Par l’analyse des données recueillies, nous qualifions chacune de ces cultures et tentons de mettre en lumière les éléments entravant ou facilitant leur coopération.In fine, nous envisageons une nouvelle forme de dialogue entre les enseignants et les professionnels du secteur médico-social. Désormais, ces acteurs doivent s’engager dans une logique d’actions où prévaut le projet de l’enfant, un projet sur lequel les capacités d’expertise des uns et des autres se combinent pour travailler dans la continuité. Nous interrogeons ainsi l’emprunt d’une nouvelle voie, celle qui mène à une culture en commun. / Since the 2009-378 decree which aimed at encouraging cooperation between the French National Educational system and the medico social sector, School hasn’t been considered as a territory that stops where special education starts. The real goal of this decree is to bring two different cultures together that of the teacher and that of special needs worker. This is why this thesis was written jointly by a special needs worker and a teacher.Educational practitioners are to cooperate to respond to the right to schooling determined by the February 11 2005 law, and the decrees that were issued onwards. Thus we aim to identify the basis of these cultures and what shapes school current practices.We did not follow an experimental method guided by the will to produce evidence but a comprehensive in-depth view. Using an ethnographic research approach combining participant observation and a semi-guided interviews enables us to get as close as possible to these practices. Thanks to the data gathered, we identify each of these cultures and attempt to highlight the elements hindering or facilitating the cooperation between teachers and special needs workers. Finally, we are considering a new form of dialogue between teachers and special needs workers.From now on, educational practitioners should adopt working methods in which the child’s project prevails, a project in which teachers and special needs workers’ expertise are combined so as to work together and ensure continuity in the progress of the project.We are thus looking at the possibility of a new approach borrowing elements and practices from the culture of both teachers and special needs workers.

This thesis aims to help uncover the context of church socio-educational work with youth in Czech Republic. It emphasizes the lecturer, who puts the education into practice, and it deals with mentoring as a tool for development of this lecturer. The thesis considers mentoring as a beneficial approach with regards to progress of lecturers, who work with christian youth. The thesis has a theoretical-empirical character. The theoretical part focuses on definition of church socio-educational work with youth and follows with some insight into the requirements for a lecturer, who puts it into practice. It makes use of four various theoretical perspectives, which are Social pedagogy, Christian youth education, volunteering and Pedagogy of leisure time. Finally the theoretical part examines mentoring of this lecturer with regards to previous findings. The research part investigates the phenomenon of mentoring in a specific environment of church socio-educational work with youth. It does so in order to find out the impacts that mentoring has on a development of a lecturer from the viewpoint of mentors and lecturers of the programme. This form prioritizes the work in the natural environment and, therefore, more likely to act directly on controlling contingencies of client behavior. This type of control seems to have established in the literature, a polarization between Therapeutic Accompaniment (TA) and the clinical based on the greater or lesser ability to control to be undertaken on the client behavior. The research presented here aims to resume and analyze the therapeutic practices of radical behaviorist base in order to establish a comparison between them and the TA, thus define whether it is possible to assert the existence of this polarization. Four sets of papers were analyzed: bases of behavior therapy: publications Skinner and Ferster about therapy, the Behavior Modification; literature on the following therapeutic approaches: Functional Analytic Therapy, Acceptance and Commitment Therapy, Behavioral Activation, the literature on the brasilian therapeutic proposals: Analytical Behavioral Therapy, Therapy of Contingencies of Reinforcement, Pragmatic Behavioral Psychotherapy and Molar and Self Therapy, beyond literature about the TA. To compose this analysis, four analytical categories were developed: core concepts for the diagnosis, intervention strategies, effectiveness and generality. The analysis of the studies cited showed that the polarization in the terms formulated above, is not true because all proposed schedule forms to ensure the generalizability of the results. Could be defined, however, the existence of a "minimum repertoire client". This concerns the modeling repertoire of verbal behavior that will allow the client to modify alone, the environment itself. And that is the reason for this study in a natural environment where the client does not have the repertoire to modify their own environment independently, the therapist operates the changing client environment in order to build a better learning condition for that client. From this, suggestions for future research are made at the end of the work / A tese aqui apresentada é motivada por um trabalho de intervenção clínica que começou a ser praticado por analistas do comportamento brasileiros no início da década de 1990. Essa forma prioriza o trabalho em ambiente natural e, com isso, maior chance de atuar diretamente nas contingências controladoras do comportamento do cliente. Esse tipo de controle parece ter estabelecido, na literatura da área, uma polarização entre o Acompanhamento Terapêutico (AT) e o consultório com base na maior ou menor possibilidade de controle a ser assumido sobre o comportamento do cliente. A pesquisa aqui apresentada se propõe a resgatar e analisar as práticas terapêuticas de base behaviorista radical com vistas a estabelecer uma comparação entre elas e o AT para, assim, definir se é possível afirmar a existência dessa polarização. A análise dos trabalhos citados apontou que a polarização, nos termos formulados acima, não acontece, pois todas as propostas programam formas garantir a generalização dos resultados. Foi possível definir, no entanto, a existência de um repertório mínimo de cliente . Esse repertório diz respeito a modelagem de comportamento verbal que permitirá ao cliente modificar, sozinho, o próprio ambiente. E essa é a justificativa do trabalho em ambiente natural, quando o cliente não tem repertório para modificar o seu próprio ambiente de forma independente, o terapeuta opera mudando o ambiente do cliente de maneira a construir uma condição de aprendizagem melhor para esse cliente. « Le rural n’existe plus », annoncent certains chercheurs au vu de l’urbanisation généralisée des territoires ou de la disparition des spécificités sociologique ou économique des campagnes. Pourtant, le « rural » est encore présent dans les discours politiques, dans les représentations des ménages, dans la littérature scientifique actuelle. Qu’est-ce que le rural aujourd’hui ? A partir d’une approche compréhensive et inductive, notre recherche analyse les déterminants permettant de comprendre les pratiques spatiales, les relations sociales ou les formes d’appartenance territoriale des ménages ; elle met en évidence l’influence sur les modes d’habiter ruraux du territoire et du profil des ménages. La thèse aborde également la question de l’évolution du dynamisme démographique et économique des territoires ruraux dans un contexte de durcissement probable des conditions de mobilité des ménages. Elle s’intéresse aux marges de manoeuvre des populations rurales, au potentiel d’évolution de leurs pratiques de mobilité vers des pratiques moins dépendantes de la voiture. A partir d’une expérimentation de recherche-action elle met en évidence le rôle du levier « accompagnement personnalisé » et apporte des éléments prescriptifs en faveur de politiques globales de mobilité. / What is rural today? Based on a comprehensive and inductive approach of the rural households’ ways of living, our research shows the influence of the rural area’s features and of the households’ profile (in particular their spatial origin, their seniority in the area and their life project) on their daily practices. They are underlain by easy and cheap car-mobility, which could evolve as a result of three phenomena (rarefaction of fossil energy; stronger injunctions to sustainable development; public policies that incite to develop only the territories that are served by public transport). Our research evaluates the households’ leeway in terms of mobility practices and their potential for transition towards more sustainable practices. Thanks to the analysis of an action-research project we highlight the role of a lever of “personalized accompaniment” in terms of behavioral evolution and propose some prescriptive elements in favor of global mobility policies.

Cette recherche s’intéresse à la Formation Professionnelle Continue des adultes. Elle questionne la portée de la dynamique qui se crée pendant qu’ils apprennent et se perfectionnent à l’occasion de stages de courte durée. Notre recherche a été réalisée auprès de 24 professionnels du secteur sanitaire et médico-social qui ont suivi au moins un stage entre les années 2008 et 2011. Nous les avons rencontrés dans le cadre d’un entretien semi-directif dont le guide a été élaboré à partir d’un modèle permettant de considérer l’attitude et le comportement en référence au Soi (Hamel et al., 1999). A partir d’un cadre épistémologique socioconstructiviste, l’analyse a contribué à relever non seulement, nombre d’idées forces corroborant la nécessité des stages comme espaces d’acquisition de compétences, mais aussi certaines indiquant des « transformations silencieuses » (Jullien, 2009) intervenant sur la « croissance de la vie adulte » (Bédard, 1987). Nous avons tenté de modéliser en quoi la FPC participe potentiellement de la construction identitaire du sujet. / This research focuses in training adults. We question the impact of the dynamics developed while they learn together. Our research was conducted with 24 people who followed at least one internship between 2008s and 2011. We met the participants within the framework of a semi-directive interview. This technique was developed from a model which consider attitude and behavior in respect to the Self (Hamel, et al., 1999). The analysis made in a socioconstructivist epistemological frame, has contributed to underline a number of ideas which supports the necessity of the internships as a space for the acquisition of skills, and other ideas which indicate that " silent transformations " (Jullien, 2009) can occur on the " growth of adult life " (Bédard, 1987) also thanks to the internships. The diverse elements of our research allowed us to show that the training potentially contributes to the construction of identity of the subject.

La thèse porte sur l’accompagnement de patients atteints de déficiences motrices handicapantes soumis à l’épreuve intime de soi à la suite d’une atteinte radicale du corps. Elle repose sur une enquête ethnographique, associant observations et entretiens, menée dans deux structures hospitalières du CHU de Saint-Etienne : le service de Soins de Rééducation Post Réanimation, dit SRPR et, pour une plus grande part, le service de Médecine Physique et Réadaptation, dit MPR. En présentant le travail de « recapacitation » de la personne à l’épreuve d’un nouveau corps devenu « non autonome », la thèse explore le travail relationnel entre les professionnels de santé et le patient au cours des soins et des activités de rééducation fonctionnelle, mais aussi la manière dont sont inclus les proches, afin d’infléchir la perception de la situation de handicap et de construire collectivement un projet acceptable de vie après le séjour en milieu hospitalier. Cette perspective donne à voir la manière dont, face aux fortes contraintes de l’épreuve intime de soi, la parole partagée devient ressource pour faire advenir les interactions et engager ainsi le patient dans la réappropriation intrasubjective de son corps et intersubjective de la continuité de son être. Une place importante est donnée au parcours des patients tétraplégiques, un cas extrême et à part dans la prise en charge des patients en MPR. La thèse présente une chronique du vécu de ces patients particuliers, et montre ainsi la spécificité des « étapes » passées, qui structurent l’expérience du corps « non autonome » et de la personne qui, devenue « dépendante », est incluse dans un collectif d’actants humains – les professionnels de santé, les proches  mais aussi d’objets techniques, au sein duquel son agir est distribué. Le récit rend compte des temps et des moments vécus par ces patients dans ce collectif, au cours d’une prise en charge pouvant se compter en mois, voire en années, en déclinant les activités ordinaires des chambres, celles du plateau technique, celles des réunions de professionnels et enfin celles, exceptionnelles, d’un dispositif de rencontre ad hoc qui réunit un collectif d’anciens patients venus avec leurs conjointes et un patient tétraplégique et sa compagne afin de finaliser le projet de retour de ce dernier dans l’espace social ordinaire. / The doctoral thesis focuses on the assistance of functional impaired patients passing throug a personal self experience following a severe physical injury. It is based on an ethnographic inquiry, combining observations and interviews that have been conducted in two hospital services of Saint-Etienne’s UH. The first service is the functional rehabilitation recovery service and, for the major part, the other is the physical medecine and rehabilitation service. In presenting the « recapacitation » work of a person experimenting a new « non autonomous » body, the thesis explores relations between the medical professionals and patients, during the care and functional rehabilitation activities, but also the way in which relatives are included, in order to impact the perception of the handicap situation and to build collectively an acceptable life project after the hospital stay. This perspective reveals how to confront strong constraints of personal self experience by sharing words, this last become as a ressource to bring into the interactions and thus to engage the patient in an intrasubjective reappropriation of his body and an intersubjective continuity of his being. An important place is given to the hospitalization of quadriplegic patients, which is an extreme case and apart in the patient’s care in the physical medecine and rehabilitation service. The thesis presents a chronicle of a lived experience of these particular patients and thus shows the specificity of the process taken place in order to structure the experience of non autonomous body and to the one who has become dependent, and included in a collective composed of human actors  medical professionals, relatives  but also technical objects in which his act is distribued. The narrative takes into consideration the time and the moments experienced by these patients in this collective, during the care which can be counted by months and sometimes by years, by describing the ordinary activities in the rooms, those of the technical installations, those of professional’s meetings, finally and exceptionally a meeting’s structure where a group of former patients come with their spouses and a quadriplegic patient accompanied by his partner, to finalize the project of his return to an ordinary social environment.

Contribution de l’éthique théologique du caractère à l’accompagnement pastoral des jeunes adultes

Ce projet doctoral cherche à élaborer une approche éthique d’accompagnement des jeunes adultes (20-35 ans) en milieu pastoral. Prenant son point de départ dans la méthode praxéologique, il déploie une observation de la pratique d’un certain type d’accompagnement pastoral, puis réfléchit sur des traits de la jeunesse actuelle, surtout québécoise. Bien que cherchant à affirmer leur autonomie, les jeunes qui consultent ponctuellement un prêtre cherchent souvent des réponses à leurs dilemmes moraux. La thèse s’inspire des théoéthiciens nord-américains, Stanley Hauerwas et Craig Dykstra pour dépasser la préoccupation du « quoi faire ». En effet, ces auteurs principaux de la thèse se centrent sur la personne, la réalisation de son soi et le type d’homme ou de femme qu’elle veut être. L’accent est donc mis sur l’être, la totalité de la personne, et non seulement sur le faire. Une surenchère de l’importance du « faire » et de l’« action » renvoie à la fois à notre société post-technologique, centrée sur la productivité de la personne et à une morale catholique prescriptive. Néanmoins, l’éthique du caractère propose un chemin de conversion de la question de savoir « quoi faire » dans celle de savoir « comment vivre ». Sur le plan théologique, elle recentre et réinterprète des aspects essentiels du christianisme, soit les récits, la tradition comme histoire, communauté et imagination. Cette approche revitalise le paysage éthique et le style d’accompagnement pastoral auprès des jeunes adultes. En tant que prêtre catholique souvent consulté par des gens de cette catégorie d’âge (20-35 ans), notre projet de thèse se veut inspirateur d’une nouvelle pratique d’accompagnement éthico-pastoral. / This doctoral project aims to develop an ethical approach of accompaniment of young adults (20-35 years) in pastoral milieu. Adopting its starting point in the "praxeologic" method, it observes first a certain type of practice in pastoral accompaniment, and then reflects upon some features of young of present times, especially young Quebecers. Although looking for affirmation of their freedom, the young who punctually seek advice from a priest, are often in search of answers to their moral dilemma. It draws on American theologians and moralists, Stanley Hauerwas and Craig Dykstra in order to surpass the concern about "what to do". In effect, these principal authors of the thesis focus on the person, the realization of his self and the type of man or woman he/she wants to be. The emphasis is then put on the issue of being and not just on doing. An escalation of the importance of "doing" and "action" refers to both our post-technological society, focusing on the productivity of the individual, and a Catholic prescriptive moral. However, the ethics of character offers a path of conversion of knowing "what to do" to knowing "how to live". On theological level, it refocuses and reinterprets essential aspects of Christianity, namely stories, tradition as history, community and imagination. This is a new approach that revitalizes the ethic landscape and pastoral accompaniment style of young adults. As a Catholic priest often consulted by people in this age group (20-35 years), my thesis project seeks inspiration for a new practice of ethical and pastoral accompaniment.

Koordinovaná rehabilitace dětí s opakovaným onemocněním dýchacích cest / Coordinated Rehabilitation for Recurrent Respiratory Diseases of Children

Respiratory diseases are ones of the most common diseases, which are treated by general practitioners. Not only the right diagnosis is essential, but the treatment as well. The treatment of respiratory diseases is usually long-term and often requires repeating. Health treatment of young children in a spa improves their life and health quality, inclusion in a collective of similar aged children, reduces complications of their disease, doses of medicine and highly reduces the risk of development of a chronical disease in adulthood. The aim of this thesis is to chart the use of coordinated rehabilitation in spa Lázně Luhačovice, a. s. and to survey coordinated rehabilitation awareness of spa medical and non medical personnel and children's accompaniment. Three hypotheses were set. Hypothesis 1: Children's accompaniment, who has been to spa health resort repeatedly percieves the care of his child as coordinated unlike the one who has been to spa health resort just once. Hypothesis 2: Medical staff of children spa health resorts percieves the most the medical treatment part of coordinated rehabilitation in contrast with non medical staff. Hypothesis 3: Children's accompaniment is satisfied the most with the medical treatment part of coordinated rehabilitation unlike the pedagogical and social one. A strategy of quantitative research was used, data was gathered by standardized questionnaires in June and July 2015. The standardized questionnaires were given to two different research groups. First one was spa health resort personnel, second one children's accompaniment in the spa Lázně Luhačovice, a. s. Based on statistical calculations the hypotheses can not be rejected, the results are not statistically significant. From the data obtained from the questionnaires, it is noticable that from the spa health resort medical and non medical personnel point of view, there is percieved only the part, which they provide. On the other hand, from the children's accompaniment point of view the rehabilitation is percieved as coordinated in all the parts provided. It is thanks to the fact that the accompaniment, who is in the spa health resort on the expenses of health insurance company, can see all the parts of coordinated rehabilitation as they undergo it together with their children. This thesis contributes to widening awareness of using parts of coordinated rehabilitation in children repeated respiratory diseases treatment, which are provided in facilities of spa Lázně Luhačovice, a.s. and in other health resorts, where there is this kind of care provided as well.
