(Doutora-do em Enfermagem Psiquiátrica) – Escola de Enfermagem de Ribeirão Preto, Universidade de São Paulo, Ribeirão Preto. Este estudo tem como objeto a prática de cuidados relacionados ao abuso e à dependência de drogas no cotidiano de uma equipe do Programa Saúde da Família (PSF). Reconhecendo a gravidade dos problemas relaciona-dos ao abuso e à dependência de drogas e suas implicações para o indivíduo, a família e a comunidade, que refletem no processo de formação e na prática dos profissionais, tivemos como objetivo desenvolver com os membros de uma equipe do PSF uma ação-reflexão sobre essa prática, visando contribuir para a superação de problemas e contradições enfrentados pela equipe no cotidiano de trabalho. Realizamos uma investigação com abordagem qualitativa, na mo-dalidade de pesquisa-ação, fundamentada pelo materialismo histórico-dialético como parte intrínseca da visão de mundo vinculada à práxis. O cenário da pes-quisa foi a área geopolítica de atuação de uma equipe do Programa no municí-pio de Contagem, Estado de Minas Gerais, Brasil. A pesquisa de campo foi realizada no período de setembro de 2000 a julho de 2001 e teve como sujeitos os integrantes da equipe (um médico, uma enfermeira, uma auxiliar de enfer-magem e quatro agentes comunitários de saúde). Os resultados da pesquisa confirmaram a tese inicial sobre a importância do PSF para a prática de cuida-dos relacionados ao abuso e à dependência de drogas. Os resultados eviden-ciaram fragilidades e potencialidades técnicas (práticas e políticas) do Progra-ma para enfrentar esse desafio, além de apontar possibilidades de superação mediante investimento na capacitação da equipe e na criação de um projeto de ação comunitária. A pesquisa contribuiu com subsídio para formulação de polí-ticas de saúde, voltadas para o desafio de desenvolver cuidados relacionados ao abuso e à dependência de drogas na prática do PSF, bem como de outros serviços de saúde que venham valer-se dessa experiência para aplicá-la em condições semelhantes. / This study has as object the practice of cares related to the abuse and to the dependence of drugs in the daily of a team of the Program Health of the Family (PHF). Recognizing the gravity of the problems related to the abuse and to the dependence of drugs and their implications for the individual, the family and the community, that reflect in the formation process and in practice of the professionals, we had as objective develops with the members of a team of PHF an action-reflection on that practice, seeking to contribute to overcome the problems and contradictions faced by the team in the daily of work. We accom-plished an investigation with qualitative approach, in the research-action modal-ity, based by the materialism historical-dialectic as intrinsic part of the world vi-sion linked to the praxis. The scenery of the research was the area geopolitics of performance of a team of the Program in the municipal district of Contagem, State of Minas Gerais, Brazil. The field research was accomplished in the pe-riod from September of 2000 to July of 2001 and had as subjects the members of the team (a doctor, a nurse, a nursing auxiliary and four community agents of health). The results of the research confirmed the initial theses about the impor-tance of PHF for the practice of cares related to the abuse and to the depend-ence of drugs. The results evidenced fragilities and technical potentialities (practices and politics) of the Program to face that challenge, besides pointing possibilities to overcome that by investment in the training of the team and in the creation of a project of community action. The research contributed with subsidy to articulate politics of health, come back for the challenge from devel-oping cares related to the abuse and to the dependence of drugs in practice of the PHF, as well as of other services of health that come to be worth of that ex-perience to apply it in similar conditions.

Consumo de drogas entre pessoas em sofrimento psíquico: sentidos, significados e percursos / Drug use among people in psychological distress: meanings and pathways

O diagnóstico duplo, ou comorbidade, é considerado prevalente tanto pela literatura do discurso médico-psiquiátrico, como na realidade clínica de serviços de saúde mental e de drogas. Considera-se que o campo da saúde está imerso em sistemas de crenças, influenciadas por aspectos culturais e sociais, inclusive sobre o que se considera sofrimento psíquico como sinônimo de diagnóstico psiquiátrico, que inserido em contexto mais amplo, se conectaria a sofrimentos compartilhados socialmente. O consumo de drogas estaria imerso nesse mesmo contexto histórico e social compartilhado. Este trabalho se caracterizou como pesquisa qualitativa, cuja abordagem conversou com autores que consideram a pesquisa qualitativa como um exercício de sensibilidade, cujo foco na experiência em primeira pessoa. O objetivo geral foi conhecer sentidos e significados do consumo de drogas a partir de narrativas de história de vida de pessoas descritas com duplo diagnóstico. O objetivos específicos foram conhecer descrições sobre a experiência de sofrimento psíquico ao longo da vida; conhecer descrições sobre a experiência de consumo de drogas ao longo da vida; e conhecer descrições sobre experiências em que o consumo de drogas esteja associado a sofrimentos psíquicos. Este estudo exploratório, foi realizado em um CAPS II e um CAPS AD, da rede de atenção psicossocial, de um município do interior paulista. Participaram dezoito adultos, homens e mulheres, selecionados de maneira a diversificar possibilidades de quadros clínicos e consumo de drogas, em padrão considerado problemático. A coleta de dados foi feita por meio de entrevista de história de vida temática e anotações em caderno de campo. A análise foi feita a partir do material emergente de trabalho de campo, da literatura sobre diagnóstico duplo e sobre sofrimento social. As entrevistas foram consideradas narrativas, sendo selecionadas dez para sua apresentação pormenorizada, cujos critérios de seleção foram exequibilidade e diversificação dos quadros clínicos, sendo distribuídos quanto ao gênero e serviços onde eram atendidos. Os resultados foram apresentados juntamente com a discussão, foi descrito o processo de construção do campo e do tema estudado e depois, foram descritas as narrativas. As narrativas dos participantes, na sua maioria homens e vindos de classes trabalhadoras urbanas, foram circunscritas pelo universo cultural e social onde se inseriam, mas também pelas possibilidades de escuta de demandas dos serviços da rede estudada. Esta se conformou em serviços que os descreviam de maneira excludente entre usuários de drogas e doentes mentais, estando comprometidas com perfis construídos nas diversas relações sociais e institucionais estabelecidas, considerando-se contextos e discursos mais amplos. Os sentidos e significados do consumo de drogas para os participantes foram construídos ao longo da vida, junto aos diversos eventos e situações vividos nas diversas relações sociais e culturais, em que o sofrimento psíquico, por vezes, fazia presente. O consumo de drogas, compreendido como dispositivo, permitia agir diante das situações de sofrimentos psíquicos e sociais, vividos singularmente, mas compartilhados socialmente. Considera-se importante refletir sobre as experiências vividas por pessoas em sofrimento psíquico e que fazem uso de drogas, considerando-os inseridos em um contexto mais amplo de relações sociais e institucionais, em que sofrimentos sociais estão presentes / Dual diagnosis, or comorbidity, is considered prevalent by literature of medical-psychiatric discourse, as much as, in mental health services and those for drug use treatment. It is considered that the health field is immerse in systems of beliefs influenced by cultural and social aspects, including what is considered psychological suffering as a synonym of psychiatric diagnosis, which is, in a broader context, connected with sufferings shared socially. Drug use is also immersed in the same historical and social shared context. This work is characterized as qualitative research, whose approach is related to authors that consider qualitative research as a sensibility exercise , focusing in first person experience. The main objective was to acknowledge senses and meanings of drug use from life history narratives of persons described as dually diagnosed. The specific objectives were to acknowledge descriptions of drug use along life course, descriptions of psychological suffering along life course and descriptions of drug use associated to psychological suffering. This exploratory study has been developed in one CAPS II and one CAPS AD, part of psychosocial attention network of an inner city of São Paulo. Eighteen adults participated, men and women, selected to permit diversification of clinical presentations of psychiatric diagnosis and drug use, in pattern od use considered problematic. Data collection has been thematic life histories interview and field notes. Analysis emerged from fieldwork, dual diagnosis\' literature and social suffering\'s literature. Interviews were considered narratives, and ten were presented with more details, selected by practicability, diversification of clinical presentations criteria, distributed among genders and services where they came from. Results were presented concomitantly with discussion, and were described the process of construction of the field and of the theme studied, then narratives were described and discussed. Participants narratives, mainly men from urban labourers class, were circumscribed by cultural and social universe where they came from, as well as the possibility of storytelling to be heard in the health services network studied. This network was also constructed by describing their patients exclusively as drug users or mentally ill, compromised with profiles constructed in many social and institutional relationships, considering broader context and discourses. Senses and meanings of drug use for the participants were constructed along life course, together with diverse events and situations lived in diverse social and cultural relations, and social and psychological sufferings could, sometimes, be present. Drug use, understood as a device, allowed the participant to act in situations of psychological sufferings that were experienced singularly, but socially shared. It is considered important reflect upon experiences lived by people in psychological suffering that also use drugs, considering them as part as a broader context of social and institutional relations, in which social sufferings may be present

Motivos da tentativa de suicídio expressos por homens usuários de álcool e outras drogas / Motives of suicide attempt expressed by men users of alcohol and other drugs

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Men's health nowadays has been addressed in some studies. This follows from the morbidity and mortality increase that has affected this population. Among deaths due to external causes, suicide is the third leading cause of death that affects men. Given these findings, this research aims to comprehend the reasons why the suicide attempt in male alcohol and/or other drugs users. This is a qualitative, Sociological phenomenological perspective based on the theoretical and methodological framework of Alfred Schütz, held at the Center for Psychosocial Care of Alcohol and Drug (CPC - Ad) "Paths of the Sun" in Santa Maria, Rio Grande do Sul. Research subjects were male adults, aged from 24 to 50 year old, alcohol and/or other drugs users, attempted suicide and who were undergoing treatment in this service. Data collection occurred in December 2011, through the phenomenological interview, recorded, which was closed after the 11th meeting empathic when they realized the sufficiency of meanings. We developed the analysis and interpretation of the testimonies, which were used by authors elaborate the steps of national nursing field references on Phenomenological Sociology of Alfred Schütz. We respected ethics, according to Resolution N º 196/96 of the National Health Board in the comprehensive analysis of typical action of the men who carried out the suicide attempt, three categories were revealed concrete: Suicide attempt for alcohol and other drugs use revealed that the use, dependence and withdrawal symptoms of alcohol and other drugs use and various circumstances of the biographical situation experienced contributed to the suicidal action; Suicide Attempt for world situations in the family life - related to biographic situation of men in everyday family life, interpersonal relations in this troubled context, constituted by the lack of attention, dialogue, listening and dependence on alcohol and other drugs that contributed to these men feel like a nuisance to the family; Suicide Attempt for experienced feelings in daily life - presented verbalization of distress feelings, externalized through the consciousness of men to carry out the suicide attempt, which were: low self-esteem, difficulty verbalizing anguish, guilt, cowardice, weakness, rejection, heartbreak, loneliness, sadness and hopelessness in relation to meaning in life. It is hoped that this study will provide grants to health care professionals so that they understand the motives that lead individuals, especially men, to suicidal action and thus, develop strategies for suicide prevention. / A saúde masculina nos últimos anos vem sendo abordada em alguns estudos. Isto decorre do aumento da morbimortalidade que tem acometido esta população. Entre os óbitos por causas externas, o suicídio é a terceira maior causa de morte que atinge os homens. Em virtude desses achados, esta pesquisa tem como objetivo compreender os motivos porque da tentativa de suicídio em homens usuários de álcool e outras drogas. Trata-se de uma pesquisa qualitativa, fenomenológica, na perspectiva do referencial teórico-filosófico da Sociologia Fenomenológica de Alfred Schütz, realizada no Centro de Atenção Psicossocial Álcool e Drogas (CAPS Ad) Caminhos do Sol no município de Santa Maria, Rio Grande do Sul. Os sujeitos da pesquisa foram homens adultos, na faixa etária de 24 a 50 anos, usuários e/ou dependentes de álcool e outras drogas que tentaram suicídio e que realizavam tratamento nesse serviço. A coleta das informações ocorreu no mês de dezembro de 2011, por meio da entrevista fenomenológica, gravada, a qual foi encerrada após onze encontros, quando se percebeu a convergência nos discursos. Foi desenvolvida a análise e interpretação compreensiva dos depoimentos, na qual foram utilizados os passos elaborados por autores da área de enfermagem que são referências nacionais da Sociologia Fenomenológica de Alfred Schütz. Foram respeitados os aspectos éticos, conforme Resolução Nº 196/96 do Conselho Nacional de Saúde. Na análise compreensiva do típico da ação dos homens que realizaram a tentativa de suicídio, foram desveladas três categorias concretas: Tentativa de suicídio pelo uso de álcool e outras drogas revelou que o consumo, a dependência e os sintomas da abstinência de álcool e outras drogas e as diversas circunstâncias da situação biográfica vivida contribuíram para a ação suicida; Tentativa de suicídio pelas situações do mundo da vida familiar relacionou a situação biográfica dos homens no cotidiano da vida familiar, relações intersubjetivas conturbadas neste âmbito, constituídas pela falta de atenção, diálogo, escuta e a dependência de álcool e outras drogas, que contribuíram para estes homens sentirem-se como um incômodo para a família; Tentativa de suicídio por sentimentos vividos no cotidiano apresentou sentimentos de aflição, externalizada por meio da consciência dos homens ao realizar a tentativa de suicídio, os quais foram: baixa autoestima, dificuldades para verbalizar angústias, culpa, covardia, fraqueza, não aceitação, decepções amorosas, solidão, tristeza e desesperança em relação ao sentido na vida. Espera-se que o presente estudo forneça subsídios aos profissionais de saúde para que estes compreendam os motivos que levam os homens à ação suicida e, desta maneira, elaborar estratégias de prevenção ao suicídio.

A pesquisadora trabalhou neste serviço de saúde mental como médica (clínica geral) no período de 2009 a 2014. O material de campo consiste em quatro narrativas de histórias de vida, sendo a técnica utilizada para sua coleta a entrevista individual, do tipo aberta. Os referenciais teórico-metodológicos fundamentais da pesquisa são o materialismo histórico-dialético, o feminismo marxista crítico e a Epistemologia Qualitativa - esta última um desenvolvimento da teoria histórico-cultural da subjetividade. No processo de análise das narrativas, buscou-se apreender os sentidos subjetivos do uso de SPA na produção da existência cotidiana de cada entrevistada, em diferentes momentos de suas vidas. A seguir, estas configurações singulares de sentidos foram relacionadas entre si e com a realidade tomada como totalidade histórico-social nesta pesquisa. Dentre os principais resultados, evidenciou-se a diversidade de configurações assumidas por experiências ideologicamente construídas como universais e homogêneas (como a maternidade, a prostituição e o próprio uso de SPA). As narrativas explicitam ainda inúmeras facetas da violência social/racial/patriarcal - cujos elementos estão entrelaçados em uma dinâmica de coprodução e de reforço mútuo - e seus efeitos perversos sobre a subjetividade individual e social. No campo desta pesquisa, o uso de SPA se revela como um recurso psicossocial importante para enfrentar e ao mesmo tempo suportar as situações de extrema violência, exploração e opressão que marcam as histórias de vida analisadas. Conclui-se que uso de SPA compõe estratégias singulares de resistência construídas conforme os recursos disponíveis em cada contexto particular e acessíveis (objetiva e subjetivamente) a cada uma das entrevistadas. Simultânea e dialeticamente, o uso nocivo de SPA assume um caráter de entrega - expressão utilizada por uma das entrevistadas, mas cujos múltiplos sentidos (de desistência, rendição, afronta) estão presentes em todas as narrativas / Abstract: This study seeks to understand the development of the harmful use of psychoactive substances (PAS) in the life trajectories of four users of a Psychosocial Care Center for Alcohol and Drugs (CAPS AD) in the city of Campinas, São Paulo State, Brazil. The author worked at CAPS AD as a staff physician in general medicine from 2009 to 2014. Fieldwork material consists of four life narratives, obtained through individual, open-ended interviews. The theoretical-methodological references that underlie this research are the historical-dialectical materialism, the critical Marxist feminism and the qualitative epistemology - the latter a development of the historical-cultural theory of subjectivity. Throughout the analytical process of the four narratives, the author sought to capture the subjective significations of PAS use in the production of everyday existence, in different moments of each interviewee's life. Subsequently, these singular configurations of meanings were related amongst themselves and also with the reality that was taken as a historic social totality for the purpose of this research. The main results highlighted the diversity of configurations assumed by experiences ideologically constructed as universal and homogenous (such as maternity, prostitution and PAS use itself). Moreover, the narratives make explicit various facets of social/racial/patriarchal violence - whose elements are intertwined in a dynamic of co-production and mutual reinforcement - and its perverse effects over social and individual subjectivity. In the realms of this research the use of PAS turned out to be an important psychosocial resource to challenge and, at the same time, to endure situations of extreme violence, exploitation and oppression that so much characterize these four life stories. Finally, the author concludes that the use of PAS is a component of singular strategies of resistance built in accordance with the resources that, being at hand in these four particular contexts, can be accessed (both objectively and subjectively) by each of the interviewees. Synthetic N-methoxybenzyl ("NBOMe") phenylethylamine derivatives were developed as highly potent agonists for the research of 5-hydroxytryptamine receptors, however they have become available on the black market in the past few years. They are sought by recreational users as "legal LSD", in worse cases the replacement is unsuspected and these substances are a cause of serious intoxication with signs of serotonin toxicity. At this moment, there is very little data available, but their low price, easy access via the internet and legal status make these substances an interesting target for recreational users and a big challenge for scientists and for the lawmakers. 2CBFly-NBOMe belongs to this group. There is no information about its recreational use for now, but thiat is probably only a matter of time. The aim of this study is to determine acute effects of 2CBFly-NBOMe in Wistar rats. Active substance significantly reduced locomotor activity of the animals and caused higher anxiety, it also disrupted sensorimotor gating and lowered body temperature of animals. Pharmacokinetic profile was measured only in the blood serum, the highest concentrations of 2CBFly-NBOMe were detected 30-60 minutes after the administration of hallucinogen. While collection of scientific data about the effects and risks of numerous NPS is a timely process, the substantial amount of information about the very same substances is being exchanged on drug discussion forums, creating a pool of data with a potential for research of the NPS trend. Aims: This dissertation aims to evaluate a potential of internet discussion forum content analysis methods by (i) describing and comparing the characteristics of discussion forum visitors to NPS users who do not visit such forums; and subsequently by comparing the internet forum content analysis results describing (ii) a prevalence of NPS use, (iii) effects and context of NPS use and (iv) discussed internet retailers selling NPS with data retrieved by other methods. The Impact of Depakote on Agitation and Short-Term Memory in Nursing Home Dementia Residents

Researchers have linked dementia to common psychiatric symptoms such as agitation and aggression, known as behavioral and psychological symptoms of dementia (BPSD). To treat residents manifesting BPSD, nursing homes (NHs) use psychoactive medications. However, research is limited and inconsistent regarding the impact of Depakote treatment on agitation and short-term memory (STM) in NH residents who have dementia. The purpose of this nonexperimental quantitative study was to evaluate for 1 year the impact of Depakote treatment on agitation and STM in NH residents as measured by each resident's Minimum Data Set (MDS). Moncrieff and Cohen's drug-centered theory served as the theoretical foundation for the study. Archival data from the consulting pharmacist and NH MDS included 16 NH dementia residents. Data were analyzed using a repeated-measures within-subject ANOVA. Results indicated no significant impact of Depakote treatment on agitation and STM scores over a 1-year period. Results may be used to assess the impact and efficacy of a common yet largely unexamined invasive treatment on an underserved, vulnerable population.

I dagens samhälle står vi inför ett komplext och utmanande problem med vanligt förekommande substansanvändning och kriminalitet bland ungdomar. Denna problematik är av stor samhällelig betydelse och kräver en djupgående förståelse för att kunna hanteras på ett effektivt sätt. I denna kvalitativa studie genomfördes det fyra intervjuer med socialarbetare som jobbar med ungodmar med beroende- och/eller kriminalitetsproblematik. Studien analyserar hur socialarbetare upplever att olika faktorer kan påverka förekomsten av missbruk av psykoaktiva substanser och däremot kriminalitet bland ungdomar. Denna studie undersöker om hur socialarbetare upplever könsskillnader i missbruk och kriminalitet bland denna åldersgrupp. Dessutom syftar studien att identifiera vilka insatser och strategier som socialarbetare anser vara effektiva för att förebygga eller hantera missbruk och kriminalitet bland ungdomar. Fyra huvudteman identifierades med hjälp av den tematiska analysen: familj som påverkande faktor, vänskapsrelationer som påverkande faktor, könsskillnader i användning av psykoaktiva substanser och kriminalitet och effektiva strategier och insatser. Resultat visar att dålig familjedynamik och familjens inkomst kan påverka ungdomars benägenhet till problematiska beteenden. Resultaten visar att även prokriminella vänner påverkar ungdomars beteenden. Ungdomar anpassar sig och efterliknar prokriminella vänners beteende för att bli accepterade i vänskapsgruppen. Dessutom, de involveras ofta i riskabla beteenden på grund av belöningar eller rädsla. Resultat visar att unga män börjar involvera sig i riskabla beteenden eftersom de är benägna att ta risker och att de uppvisar våldsamt beteende i större utsträckning, jämfört med unga kvinnor som börjar med substansanvändning på grund av emotionell smärta och syns inte lika mycket när det gäller brottslighet. Ytterligare visar resultaten att individanpassade insatser, familjeinvolvering, placering utanför hemmet, insatser baserade på socialinlärningsteori och samarbete med andra aktörer visade sig vara effektiva för att motverka och hantera missbruk och kriminalitetsproblematiken bland ungdomar. / In today´s society, we face a complex and challenging problem with common substance use and crime among youth. This issue is of great societal significance and requires a deep understanding to be addressed effectively. In this qualitative study, four interviews were conducted with social workers who work with youth dealing with addiction and/or criminal behaviour. The study analyzes how social workers perceive various factors that may influence the occurence of psychoactive substance abuse and thereby criminality among youth. This study investigates how social workers perceive gender differences in substance abuse and criminality among this age group. Furthermore, the study aims to identify which interventions and strategies social workers consider effective in preventing or managing substance abuse and criminality among youth. Four main themes were identified using thematic analysis: family as an influencing factor, friendships as an influencing factor, gender differences in the use of psychoactive substance and criminality and effective strategies and interventions. Results shows that poor family dynamics and family income can influence youth´s propensity for problematic behaviours. The results also shows that pro-criminal friends to be accepted in the friendship group. Additionally, they often engage in risky behaviours due to rewards or fear. The results shows that young men start engaging in risky behaviours because they are inclined to take risks and exhibit violent behaviour to a greater extent, compared to young women who start using substances due to emotional pain and are less visible in terms of criminality. Further results indicate that individualized interventions, family involvement, out-of-home placements, interventions based on social learning theory and collaboration with other actors were found to be effective in counteracting and managing the issue of substance abuse and criminality among youth.

The blood-to-plasma ratio and predicted GABAA-binding affinity of designer benzodiazepines

Yes / The number of benzodiazepines appearing as new psychoactive substances (NPS) is continually increasing. Information about the pharmacological parameters of these compounds is required to fully understand their potential effects and harms. One parameter that has yet to be described is the blood-to-plasma ratio. Knowledge of the pharmacodynamics of designer benzodiazepines is also important, and the use of quantitative structure–activity relationship (QSAR) modelling provides a fast and inexpensive method of predicting binding affinity to the GABAA receptor. Methods: In this work, the blood-to-plasma ratios for six designer benzodiazepines (deschloroetizolam, diclazepam, etizolam, meclonazepam, phenazepam, and pyrazolam) were determined. A previously developed QSAR model was used to predict the binding affinity of nine designer benzodiazepines that have recently appeared. Results: Blood-to-plasma values ranged from 0.57 for phenazepam to 1.18 to pyrazolam. Four designer benzodiazepines appearing since 2017 (fluclotizolam, difludiazepam, flualprazolam, and clobromazolam) had predicted binding affinities to the GABAA receptor that were greater than previously predicted binding affinities for other designer benzodiazepines. Conclusions: This work highlights the diverse nature of the designer benzodiazepines and adds to our understanding of their pharmacology.
