Nordström, Diana, Svensson, Sofie January 2014 (has links)

Drug interaction surveillance using individual case safety reports

Strandell, Johanna January 2011 (has links)
Background: Drug interactions resulting in adverse drug reactions (ADRs) represent a major health problem both for individuals and society in general. Post-marketing pharmacovigilance reporting databases with compiled individual case safety reports (ICSRs) have been shown to be particularly useful in the detection of novel drug - ADR combinations, though these reports have not been fully used to detect adverse drug interactions. Aim: To explore the potential to identify drug interactions using ICSRs and to develop a method to facilitate the detection of adverse drug interaction signals in the WHO Global ICSR Database, VigiBase. Methods: All six studies included in this thesis are based on ICSRs available in VigiBase. Two studies aimed to characterise drug interactions reported in VigiBase. In the first study we examined if contraindicated drug combinations (given in a reference source of drug interactions) were reported on the individual reports in the database, and in the second study we examined the scientific literature for interaction mechanisms for drug combinations most frequently co-reported as interacting in VigiBase. Two studies were case series analyses where the individual reports were manually reviewed. The two remaining studies aimed to develop a method to facilitate detection of novel adverse drug interactions in VigiBase. One examined what information (referred to as indicators) was reported on ICSRs in VigiBase before the interactions became listed in the literature. In the second methodological study, logistic regression was used to set the relative weights of the indicators to form triage algorithms. Three algorithms (one completely data driven, one semi-automated and one based on clinical knowledge) based on pharmacological and reported clinical information and the relative reporting rate of an ADR with a drug combination were developed. The algorithms were then evaluated against a set of 100 randomly selected case series with potential adverse drug interactions. The algorithm’s performances were then evaluated among DDAs with high coefficients. Results: Drug interactions classified as contraindicated are reported on the individual reports in VigiBase, although they are not necessarily recognised as interactions when reported. The majority (113/123) of drug combinations suspected for being responsible for an ADR were established drug interactions in the literature. Of the 113 drug interactions 46 (41%) were identified as purely pharmacodynamic; 28 (25%) as pharmacokinetic; 18 (16%) were a mix of both types and for 21 (19%) the mechanism have not yet been identified. Suspicions of a drug interaction explicitly noted by the reporter are much more common for known adverse drug interactions than for drugs not known to interact. The clinical evaluation of the triage algorithms showed that 20 were already known in the literature, 30 were classified as signals and 50 as not signals. The performance of the semi-automated and the clinical algorithm were comparable. In the end the clinical algorithm was chosen. At a relevant level, 38% were of the adverse drug interactions were already known in the literature and of the remaining 80% were classified as signals for this algorithm. Conclusions: This thesis demonstrated that drug interactions can be identified in large post-marketing pharmacovigilance reporting databases. As both pharmacokinetic and pharmacodynamic interactions were reported on ICSRs the surveillance system should aim to detect both. The proposed triage algorithm had a high performance in comparison to the disproportionality measure alone.

The Familiar Discretion is considered the sum of rights and obligations assumed by parents or guardians of law, established with the objective of ensuring the legal protection of the interests of subsistence, health and childcare. Said power is given to parents by the state and this is the lawful exercise of supervisory the same. State falls to monitor and control the relationship between the parts of the Power family (parents and children), so that the rights and obligations are met, without disrespect to the dictates of the law and its limits. To accomplish this study, the methodology was the Oral History, which sought by reports recorded by the recorder, check the representation that the operators have the right to respect for the problems that culminate in the dismissal of Power Family, and the consequent Judicial politics in the district of Newark. It was also underscored the significant legislative changes in the power granted just before the father (vernacular Power) came to be exercised equally by the father and mother (Family Power). These changes arising from the transition of behaviors and values (modernity), caused by modernization. Finally, analyzed both the legal discourse of those interviewed in light of the construction of gender relations.

Multiple Discourses: The Mobilization of Trauma Narratives within Burma's Transnational Advocacy Network

January 2011 (has links)
abstract: Since the 1988 uprising, a transnational advocacy network has formed around the issue of democracy and human rights in Burma. Within this transnational advocacy network, personal narratives of trauma have been promulgated in both international and oppositional news media and human rights reports. My thesis critically analyzes the use of the trauma narrative for advocacy purposes by the transnational advocacy network that has emerged around Burma and reveals the degree to which these narratives adhere to a Western, individualistic meta-narrative focused on political and civil liberties. Examining the "boomerang" pattern and the concept of marketability of movements, I highlight the characteristics of the 1988 uprising and subsequent opposition movement that attracted international interest. Reflecting on the psychological aspects of constructing trauma narratives, I then review the scholarship which links trauma narratives to social and human rights movements. Using a Foucauldian approach to discourse analysis, I subsequently explain my methodology in analyzing the personal narratives I have chosen. Beyond a theoretical discussion of trauma narratives and transnational advocacy networks, I analyze the use of personal narratives of activists involved in the 1988 uprising and the emergence of Aung San Suu Kyi's life story as a compelling narrative for Western audiences. I then explore the structure of human rights reports which situate personal narratives of trauma within the framework of international human rights law. I note the differences in the construction of traumatic narratives of agency and those of victimization. Finally, using Cyclone Nargis as a case study, I uncover the discursive divide between human rights and humanitarian actors and their use of personal narratives to support different discursive constructions of the aid effort in the aftermath of the cyclone. I conclude with an appeal to a more reflexive approach to advocacy work reliant on trauma narratives and highlight feminist methodologies that have been successful in bringing marginalized narratives to the center of human rights discussions. / Dissertation/Thesis / M.A. Social Justice and Human Rights 2011

Perceptual reports in fenêtre(s) and porte(s) nouns context. Semantic and pragmatic approach

Maëso Gardeil, Florence 08 November 2014 (has links)
The aim of this thesis is to explain why the inclusion of perceptual reports (PRs) in a literary narrative does not interfere with the comprehension of such narratives, despite when breaking their thematic progression.Narratological studies (Hamon, Adam and Petitjean) define descriptive texts as sequences characterised by correlated introductory and closing markers. Furthermore, research on evidentiality (Vogeleer) shows that PR describe a situation that is accessible in the space/time situation of a subject. PRs are thetics statements characterised by a lack of topic (Furukawa). In the written form, the left context of the PR may make up for the lack of topic by describing a situation that may be used to infer the place targeted by the perception (Emma s’approcha de la fenêtre) of a subject introduced earlier. Statements of this type are thus similar to represented speech and thought or RST.The classification of a corpus of PRs incorporating the nouns fenêtre(s) and porte(s), leads us to distinguish six PRs types among which represented perceptual reports (RPRs), are most frequent. In these sequences, when the first PR of the excerpt has no link to its left context, it describes a perceived situation accessible in the spatial/temporal situation of the subject that perceives it, which is therefore unknown until introduced into the fictitious world. The spatial and temporal coincidence of the subject and this situation may be noted in certain excerpts by a spatial or temporal prepositional phrase (PP) that is placed earlier in the first PR of the sequence and that is potentially framing (Charolles). This PP signals the point of view of the subject introduced earlier, denotes the target location or temporal location of the situation described in the PR and makes it possible to interpret all the PRs in the sequence. In the excerpts that do not mention a PP, the target location and time of the perceived situation are inferred once the first PR is introduced, with this inference validating all perceptual reports in the sequence. The topical break caused by the introduction of PRs signals, in all cases, the emergence of the point of view of a character introduced earlier.

Estilos reflexivos nas deliberações do self por autorrelatos de conversa interna / Reflexive styles and deliberations of the self on self-reports about internal conversation

Wagner, Helen Longhi January 2010 (has links)
This study intended to explore the different styles of internal conversation (IC) or reflexivity, defined as communicative, autonomous, meta-reflexive, and fractured. The self-reported experiences of internal conversation were obtained by interviewing seven women and three men of diverse backgrounds and occupations, aged between 24 and 63 years. The self-reported experiences were analyzed and interpreted in accordance with the phenomenological orientation. The description focused on understanding the phenomenon in a simple and basic structure of a primary agent (general and shared thinking skills): when it happens, how it happens, what are its contents, what purpose it serves and in which way it is helpful. The reduction pointed to three specifications of a corporate agent (organized and articulated): 1) in regards to the experience itself (common, natural, habitual); 2) whether it is general and normal (isn’t it something crazy?), and 3) regarding its form (is it methodical and organized or spontaneous and almost unconscious?). It was inferred from this study that the styles of reflexivity, while having inter-cultural and timeless qualities, are sustained in their universality, with possible variations in implementations, preferences and manifestations.

Examining the use of marketing metrics in annual reports of SA listed companies

Gartz, Hilke January 2007 (has links)
Purpose This paper analyses the use of marketing metrics and marketing information and metrics contained in 2006/7 annual reports of companies listed on the Johannesburg Stock Exchange. The assumption is that the annual reports are the vehicle whereby listed companies communicate to their shareholders and other stakeholder constituencies. Methodology The assessment criteria is based on Ambler’s (2003) suggested marketing metrics and qualitative data is based on a checklist compiled from various academic sources. The elements which are assessed pertain to brand equity, other customer metrics, segmentation, competition, innovation and environmental and strategic aspects. The information obtained is compared to information required by investors and rating is done based on a grand total maturity. Findings Research results indicate that the use of quantitative metrics and qualitative data is very limited. The majority of companies display a lack of information pertaining to marketing. The results reflect a bi-modal tendency. Half (53%) of the companies do not provide any or poor information on their brand whereas 26% of companies supplied good and excellent information. The grand total score indicates that nearly two thirds (60%) of companies obtain a score of less than 50%, providing insufficient information. On the other hand, 27% of companies provide good and excellent information. Segmentation metrics are generally not reflected in annual reports, neither are competitors. Innovation and environmental aspects influencing market trends are covered by two thirds, however a third provides insufficient information. Other findings include that no standard reporting format exists. Information pertaining to marketing is spread throughout the annual reports. None of the companies provide a glossary of marketing definition or brand terminology. Research implications More in-depth research needs to be conducted on various industry sectors and amongst investors as to their needs. Originality/ value The paper is of value to corporate executives, marketing and communication practitioners who seek to improve communication and to convey optimal information for the investment community. The aim is to stimulate executive management to revise their relationship towards customers, the brand, marketing strategy and investors. / Graduate School of Business Leadership / MBL
