Entretanto, muitas medidas de empatia utilizadas na literatura não estão alinhadas com esse modelo atual, o que aponta para a necessidade de refinar esses instrumentos ou desenvolver outros, mais consistentes. Por sua vez, o Fenótipo Ampliado do Autismo (FAA) é como ficou conhecida a manifestação a nível subclínico de traços cognitivos e comportamentais do Transtorno do Espectro Autista (TEA) em parentes próximos de indivíduos com tais quadros. Esses traços associados ao fenótipo do TEA parecem apresentar distribuição normal na população, conforme resultados de pesquisas sobre o FAA. Alterações da empatia em indivíduos com TEA já foram observadas em estudos, entretanto, ainda não está claro se esses déficits se resumem ao componente cognitivo da empatia ou também ao afetivo. Pesquisas sobre alterações na empatia afetiva e cognitiva associadas ao FAA podem ajudar a esclarecer esse aspecto, entretanto, para isso, é necessário dispor de instrumentos adaptados e válidos para a população brasileira. Este trabalho teve como objetivo realizar dois estudos de adaptação transcultural e validação de medidas, um para avaliação da empatia em sua compreensão multidimensional e outra, para avaliação de traços associados ao fenótipo do TEA. Ambas as pesquisas foram realizadas através de um formulário online divulgado nas redes sociais, caracterizando uma amostragem pelo método bola de neve. Em ambos os estudos, foram excluídos participantes que declararam sofrer de transtornos neurológicos ou psiquiátricos. O estudo de validação da Affective and Cognitive Measure of Empathy (ACME) contou com a participação de 385 indivíduos de diferentes regiões do país, sendo que 338 permaneceram após aplicados os critérios de exclusão. Foi empregado o método de Modelagem de Equações Estruturais para testar a estrutura fatorial proposta pelos autores da referida escala, e o instrumento foi também testado para validade convergente, consistência interna e fidedignidade teste-reteste. Os resultados indicaram que o modelo de três fatores teve um bom ajuste aos dados coletados na versão brasileira da ACME, retendo todos os itens originais, e sendo considerada consistente e válida para a população brasileira. A distribuição dos escores foi significativamente diferente da normal e mulheres obtiveram pontuações mais altas do que os homens em todos os escores desse instrumento. Por sua vez, o estudo de validação do Autism-Spectrum Quotient (AQ) teve um total de 329 participantes de diferentes regiões do país, sendo mantidos 262 após aplicados os critérios de exclusão. O instrumento foi testado por Análise Fatorial Confirmatória dentro de diversos modelos, para identificar a versão que apresentava melhor consistência interna. A versão final do AQ reteve 31 itens dentre os 50 originais, em três fatores, apresentando boa consistência interna, fidedignidade teste-reteste e forte correlação com o instrumento original. O escore total do AQ teve distribuição normal na população, e homens pontuaram mais do que mulheres no total e na subescala de (baixas) Habilidades Sociais. Normas para futura referência ao interpretar os escores na ACME e no AQ foram extraídas por sexo, considerando que foram encontradas diferenças significativas nas pontuações desses grupos, em ambos os instrumentos. / Empathy is currently comprehended as a multidimensional construct, this model being supported by neuroimaging studies, comprising a cognitive component, associated with perceiving and understanding the emotional state of others, and an affective component, consisting of vicariously experiencing an emotional response to other people`s emotions. However, most measures of empathy are not aligned with that current understanding, which points to the need of improving those instruments or developing more consistent ones. In turn, the Broader Autism Phenotype (BAP) is how the subclinical manifestation of cognitive and behavioral Autism Spectrum Disorder (ASD) traits in close relatives of individuals with that condition became known. Those traits associated with ASD seem to be normally distributed in the general population, according to studies concerning the BAP. Empathy deficits in patients with ASD has already been observed in studies, nonetheless, it is still unclear if those deficits are limited to the cognitive component of empathy or also include the affective component. Researches about cognitive and affective empathy alterations in association with the BAP could help clarifying that aspect, however, in order to carry them out, it is necessary to have access to adequate instruments, validated for the Brazilian population. This study aimed to carry out two studies of cross-cultural adaptation and validation of measures, one to asses empathy in its multidimensional understanding, and the other, to asses ASD traits. Both studies were available online and shared in social media networks, characterizing a snowball sampling method. In both studies, participants who reported suffering from neurological or psychiatric disorders were excluded. The validation study of the Affective and Cognitive Measure of Empathy (ACME) had 385 participants from different regions of the country, of which 338 remained after applying exclusion criteria. Structural Equation Modeling was used to test the factor structure originally proposed by the ACME authors, and the instrument was also tested for concurrent validity, internal consistency and test-retest reliability. Results indicate that the three-factor model showed adequate fit to data in the Brazilian version of ACME, retaining all original items, and been declared valid for the Brazilian population. Distribution of scores was non-normal and females scored higher than males both in scale`s and subscales` totals for this instrument. In turn, the validation study of the Autism-Spectrum Quotient (AQ) had 329 participants from different regions of the country, of which 262 remained after applying exclusion criteria. A series of Confirmatory Factor Analysis were carried out in order to identify the version of the instrument with best internal consistency. The final version of the AQ retained 31 of the original 50 items, in three factors, showing good internal consistency, test-retest reliability and strong correlation with the original instrument. AQ total scores were normally distributed and men scored higher than women in total scores and in (poor) Social Skills subscale. For future reference when interpreting scores in ACME and AQ, norms were extracted by sex, since significant differences were found in scores of those groups for both instruments.

Adaptação transcultural e validação clínica da Neonatal Infant Pain Scale para uso no Brasil

Recém-nascidos enfermos experienciam a dor rotineiramente associada a procedimentos invasivos dentro da Unidade de Terapia Intensiva Neonatal. A avaliação da dor tem como objetivo fornecer dados para determinar ações que aliviem a dor do neonato. A Neonatal Infant Pain Scale é uma escala de avaliação da dor para recém-nascidos validada no Canadá e utilizada no Brasil, porém não há um estudo de adaptação transcultural e validação para o país. Este estudo teve o objetivo de realizar a adaptação transcultural e validação clínica do instrumento Neonatal Infant Pain Scale para uso no Brasil. Utilizou-se os passos propostos por Beaton para adaptação transcultural de instrumentos de medida em saúde, envolvendo as etapas de tradução inicial, síntese entre os tradutores, retrotradução, revisão por comitê de especialistas e testagem da versão pré-final a fim de garantir equivalência entre o instrumento original e a tradução. O instrumento final adaptado foi submetido à validação clínica para avaliação da manutenção das propriedades psicométricas de validade, confiabilidade e consistência interna. A amostra consistiu de 60 recém-nascidos a termo e saudáveis submetidos a um procedimento doloroso que foi filmado e avaliado por seis enfermeiros posteriormente, sendo que cada vídeo foi avaliado por dois enfermeiros (confiabilidade interobservador) e reavaliado por um deles (confiabilidade intraobservador). Para os dados da validação clínica, foram realizados os testes estatísticos t de Student, coeficiente kappa ajustado por PABAK, método de Bland-Altman e coeficiente alfa de Cronbach. O estudo foi realizado na unidade de neonatologia de um hospital universitário de Porto Alegre, RS, entre setembro de 2011 e setembro de 2012. Os princípios éticos foram respeitados e foi consentida permissão do hospital detentor dos direitos sobre a escala e do seu autor para o estudo. Após a tradução e elaboração da versão pré-final, sua clareza foi avaliada por 32 profissionais da saúde através de uma escala likert, resultando em um nível de clareza superior a 87,5%. A versão adaptada para o Brasil, agora chamada Escala de Dor no Recém-Nascido (NIPS - Brasil), demonstrou muito boa confiabilidade inter e intraobservador, com PABAK de 0,93 na concordância em relação ao escore total, sendo que no gráfico de Bland-Altman a concordância inter e intraobservadores ficou acima de 95% e 90%, respectivamente. A consistência interna foi considerada boa, com alfa de Cronbach de 0,762. Considera-se a adaptação transcultural da Neonatal Infant Pain Scale para o português falado no Brasil estabelecida e validada para uso em recém-nascidos a termo do Brasil submetidos a dor aguda. / Newborns in neonatal intensive care units routinely experience pain associated with invasive procedures. Pain assessment should provide data to define actions to alleviate neonatal pain. The Neonatal Infant Pain Scale is a pain assessment instrument for newborns validated in Canada and used in Brazil, although no cross-cultural adaptation and validation study had been previously performed for use in this country. In this study, the Neonatal Infant Pain Scale was adapted cross-culturally and validated clinically for use in Brazil. The steps recommended by Beaton for the cross-cultural adaptation of health measurement instruments were followed, including initial translations, synthesis of translations, back translations, review by an expert committee and testing of the pre-final version to ensure the equivalence between the original instrument and the translated version. The final adapted instrument was submitted to clinical validation to evaluate the retention of psychometric properties, validity, reliability and internal consistency. The study included 60 healthy term newborns that underwent painful procedures, which were video recorded and later evaluated by six nurses. Each recording was evaluated by two nurses (inter-rater reliability) and re-evaluated by one of them (intra-rater reliability). Clinical validation data were analyzed using the Student t test, the prevalence-adjusted bias-adjusted kappa (PABAK) coefficient, Bland-Altman plots and the Cronbach’s alpha coefficient. The study was conducted in the neonatology unit of a university hospital in Porto Alegre, Brazil, from September 2011 to September 2012. Ethical principles were followed, and the permission to use the scale was granted by its coauthor and the hospital that holds its rights. After translation and preparation of the pre-final version, 32 healthcare workers used a Likert-like scale to assess clarity, which was greater than 87.5%. The version adapted for use in Brazil, now called Escala de Dor no Recém-Nascido (NIPS Brazil) had good inter- and intra-rater reliability, PABAK was 0.93 for the agreement with total score, and inter- and intra-rater agreements were greater than 95% and 90% according to the Bland-Altman plots. Internal consistency was good, with a Cronbach’s alpha of 0.762. This study adapted the Neonatal Infant Pain Scale cross-culturally to Brazilian Portuguese and clinically validated it for use with Brazilian term newborns that experience acute pain. / Recién nacidos enfermos experimentan el dolor rutineramente asociado a procedimientos invasivos dentro de la Unidad de Terapia Intensiva Neonatal. La evaluación del dolor tiene como objetivo proveer datos para determinar acciones que alivien el dolor del neonato. La Neonatal Infant Pain Scale es una escala de evaluación del dolor para recién nacidos validada en Canadá y utilizada en Brasil, pero no hay un estudio de adaptación transcultural y validación para el país. Este estudio tuvo el objetivo de realizar la adaptación transcultural y validación clínica del instrumento Neonatal Infant Pain Scale para uso en Brasil. Se utilizaron los pasos propuestos por Beaton para adaptación transcultural de instrumentos de medida en salud, involucrando las etapas de traducción inicial, síntesis entre los traductores, retrotraducción, revisión por comité de especialistas y testaje de la versión pre-final a fin de garantizar equivalencia entre el instrumento original y la traducción. Abstract The Giger and Davidhizar Transcultural Assessment Model. Because of the globalization we live in the world where {\clqq}the borders are getting closer`` which also means to live our lives in more multicultural society. That is why a nurse must react to modern nursing care and must be well prepared to cope with specific problems of various communities in their health and illnesses. In my diploma work we deal with transcultural evaluation of nursing care according to the J. N. Giger and R. Davidhizar Nursing Care Models. These models are about regardful and tailored care which we apply to members of Vietnamese community living in the Czech Republic. The theoretic part contains problems of the globalization, migration of foreign people, transcultural nursing, the Giger and Davidhizar nursing care models, information about Vietnam and the model application to members of Vietnamese community living in the Czech Republic. In the practical part we created standard of nursing care named Nursing of Vietnamese patient/client in health care institutions and we are interested in its using in the working practice. Then we found out opinions of people working in health care institutions who work with this standard and found out expectation of Vietnamese patients/clients hospitalized in health care institutions whose findings could help with prevention of hospitalization problems. The goals of the work: 1. Using of modification the Giger and Davidhizar nursing care models with giving care to Vietnamese people in health care institutions 2. To find out cultural changes in nursing process and their influence on behaviour of Vietnamese people in an illness 3. To identify problems and expecting of hospitalized Vietnamese people The research was conducted with the help of quasiexperiment and interview in the Regional Hospital Příbram. We addressed 10 nurses and 6 Vietnamese patients/clients. We must point out that in the year 2007 only 6 Vietnamese people were hospitalized there. The goals of the work were achieved. We hope that our work may help to increase quality and efectivity of the nursing care given to Vietnamese people.

Health beliefs of the urban pare tribe living in Moshi, Tanzania

This dissertation reports on the findings of a qualitative, exploratory, descriptive and contextual study into the health beliefs and practices of urban Pares, living in Moshi, Kilimanjaro Region, in Tanzania. The study utilised aspects of the transcultural nursing framework. Semi-structured interviews were used for data gathering with a sample of nine urban Pare informants. Data were analysed thematically. The major findings indicate that health beliefs arise from magico-religious, holistic and scientific paradigms. It was also found that beliefs and behaviour patterns are changing. These findings are discussed in terms of the two major themes, namely, multiple world views and change and continuity. Recommendations arising from the findings are made which may assist health workers to provide culturally congruent care. / Health Studies / (M.A. (Health Studies))

Derived from a marxist/liberal humanist view of power, conventional critiques and historical accounts of the socio-medical sciences in South Africa see only their power to repress and negate the true bodily attributes and authentic person of the African. In so doing, they ignore the productive capacity of these knowledges and practices as a manifestation of what Michel Foucault termed "disciplinary" power, by which the human body is manufactured and made manageable as an object of medical knowledge and industrial utilisation. Accordingly, this thesis offers just such a Foucaultian reading of western socio-medical knowledge in South Africa to demonstrate how it has operated to fabricate the bodies of Africans as visible objects possessed of distinct attributes that have provoked particular strategies for their surveillance, management, and government in health and disease. / Psychology / D. The nature of the problems experienced by non-Zulu student nurses during their encounter with Zulu patients

South African nurses have accepted the challenge that was brought about by the Health Reform Policy of 1990 which opened health service centres to people of all racial and cultural groups. However, recent studies on multicultural nursing have revealed that problems have occurred when delivering care across cultural barriers. Most of these studies have approached these problems from the patients point of view, where issues of dehumanized care, labelling and discrimination have been reported. Therefore, this study was an attempt to look into these problems from the nurses point of view, by exploring the source of these problems, their effect on health care delivery and possible solutions. The researcher focussed on three critical issues, namely, lack of cultural knowledge, negative attitudes, as well as difficulty in communication. Through focus group interviews (FGI) and responses to given scenarios, student nurses who had experienced problems with culturally different patients were given a chance to reflect on their experiences, report on these experiences and recommend possible solutions to the experienced problems. The results revealed that: 1. The major source of the problems was lack of experience due to inadequate exposure to culturally diverse groups which in tum led to inappropriate decisions and actions at care delivery level. 2.Difficult perceptions and negative attitudes lead to the formation of stereotypes which block the delivery of culturally congruent care. 3 .Inability to communicate due to differences in spoken language lead to difficulty in building the trust relationship and hence inadequate care delivery. / Health Studies / M.A. (Nursing Science)

O instrumento foi traduzido da versão original (Inglês) para a língua portuguesa pelos autores e, posteriormente, revisado e avaliado quanto ao grau de dificuldade das traduções e equivalência por tradutores bilíngües. O instrumento foi, então, aplicado em 20 pacientes com hepatopatia crônica selecionados aleatoriamente. Não houve dificuldade na compreensão do instrumento, todas as questões foram consideradas aplicáveis pelos pacientes, e a equivalência cultural do CLDQ foi demonstrada sem que mudanças na tradução original precisassem ser feitas. A tradução e a adaptação cultural do CLDQ para o português, no Brasil, foram realizadas, tendo sido cumprida esta importante etapa para sua validação e utilização em nosso meio. / The aim of this study was to carry out the translation from English into Portuguese and the transcultural adaptation of the Chronic Liver Disease Questionnaire to use in Brazil. The instrument was translated from the original version (English) into Portuguese language by the authors and then, it was revised. The degree of difficulty of the translations and equivalence for bilingual translators were evaluated. The instrument was, then, applied in 20 patients with chronic liver disease randomly selected. It was easy to the patients understand the instrument, all the questions had been considered applicable by the patients, and the cultural equivalence of the CLDQ was demonstrated without changes in the original translation. Es un estudio cualitativo mediante el método etnográfico en la comunidad shipibo-conibo de Cantagallo. Los participantes son pobladores shipibo-conibos que viven permanentemente en la comunidad de Cantagallo. Para la recolección de datos se emplea sesiones de grupos focales y entrevistas a profundidad. Los datos recolectados, luego de ser plasmados en forma escrita, son organizados en unidades de análisis, para así codificarlas y categorizarlas para un estudio profundo. Como resultados y conclusiones se obtuvieron que las barreras culturales en el proceso salud-enfermedad en la comunidad shipibo-conibo de Cantagallo son múltiples y entre ellas tenemos la lengua; la conciencia de sus derechos; sus percepciones propias de la salud y la enfermedad; y la que surge de su relación con los prestadores de salud, siendo esta última la principal barrera cultural. Además, se evidencia que los conocimientos y la práctica de la medicina tradicional shipibo-conibo se están perdiendo con el cambio generacional de los pobladores. / Tesis

Un estudio transcultural del prejuicio contra habitantes de calle con creacíon de medidas explícitas e implícitas / Cross-cultural study of prejudice against homeless with explicit and implicit measures

The objectives of this research were to create explicit and implicit measures of prejudice against homeless, and to identify factors associated with negative attitudes toward this population. To meet these objectives several studies collected in four articles were made. In the first article the literature on prejudice against homeless was reviewed. In this study was found that both, in Latin America and in the countries of industrialized West, have been reported stereotypes towards this population related to mental illness and drug addiction, as well as acts of discrimination related to limitations in the use of public space. Article two corresponded to a study of cross-cultural creation and validation of a scale of prejudice against homeless, created with basics of Item Response Theory and the Allport's scale of prejudice. The created scale presented evidence of construct validity, convergent validity and good internal consistency. It was also verified, in part, the Allport's theory of the scale of prejudice. The third article aimed at identifying factors associated with prejudice against homeless, and to build and test a hierarchical model to explain variations in the levels of this prejudice. The proposed model had four levels of prejudice mediations, good fit indices, and included the variables social dominance orientation, basic values, egocentrism, perception of insecurity and quality of contact with homeless people. In the fourth article was described the process of creating an implicit association test about attitudes toward homeless, and was tested scores correlation levels acquired with this test and those acquired with the scale established in article two. The results indicated that there are strong associations between concepts representing the homeless people and negative stereotypes towards this population. No correlation between the scores on the two measures was found, in line with other studies on implicit and explicit measures of stereotypes. / Los objetivos de esta tesis fueron la creación de medidas explícitas e implícitas del prejuicio contra habitantes de calle, y la identificación de algunos factores asociados con actitudes negativas hacia esta población. Para responder a estos objetivos se realizaron varios estudios reunidos en cuatro artículos y presentados en este documento como capítulos. En el primer capítulo, se realizó un estudio de revisión de la literatura sobre el prejuicio contra habitantes de calle. En este estudio se encontró que tanto en Latinoamérica como en los países de occidente industrializado, se han reportado estereotipos hacia esta población relacionados con la enfermedad mental y la adicción a las drogas; así como actos de discriminación relacionados con limitaciones en el uso del espacio público. El capítulo dos corresponde a un estudio de creación y validación transcultural de una escala de prejuicio contra habitantes de calle, fundamentada en la teoría de la respuesta al ítem y en la teoría de la escalada comportamental del prejuicio de Allport. La escala creada presentó evidencias de validez de constructo, validez convergente y buenos índices de consistencia interna. También se comprobó parcialmente la hipótesis de la escala del prejuicio de Allport. En el tercer capítulo se buscó identificar los factores asociados al prejuicio contra habitantes de calle, así como construir y probar un modelo jerárquico para explicar variaciones en los niveles de este prejuicio. El modelo propuesto tuvo cuatro niveles de mediación del prejuicio, buenos índices de ajuste, e incluyó las variables dominancia social, valores básicos, egocentrismo, percepción de inseguridad y calidad del contacto con habitantes de calle. En el cuarto capítulo se describió el proceso de creación de un Test de Asociaciones Implícitas sobre actitudes hacia habitantes de calle, y se probó los niveles de correlación entre los puntajes adquiridos con este test y los adquiridos con la escala creada en el artículo dos. Los resultados indicaron que hubo fuertes asociaciones entre los conceptos que representan a los habitantes de calle y los estereotipos negativos hacia esta población. No se encontró una correlación entre los puntajes en las dos pruebas, acorde con los resultados de otros estudios sobre medidas implícitas y explícitas de estereotipos.

Cultural anthropology as a scientific field studying diverse cultures has been incorporated into the field of nursery and it helps medical professionals to acquire specific cultural information enabling them to provide appropriate, efficient, individually-based care, support in health as well as in illness to the individuals and communities from a different cultural environment. The Czech Republic carried out investigation of diverse ethnic groups but did not implement investigation of the specific needs of the Polish minority group. The objective was focusing on mapping the approach of the Polish nationals to their health, on identification of their lifestyle, their approach to prevention, their attitude to home care and the specifics in the approach to dying. Hypotheses: H1 Statistically, the Polish minority group population with higher education shows better results in their approach to their health than the Polish minority group population with elementary education. H2 Younger women of the Polish minority group population with elementary education are more prone to agree with the statement on importance of attending preventive examinations by gynecologist than older women. H3 Based on the statistical assessment the Polish minority group population with higher education gives higher preferences for preparation of meals and catering based on both cuisines (Czech and Polish) than with elementary education. H4 Men of the Polish minority group population occasionally consuming alcohol are statistically significantly prevailing over women of the Polish minority group population occasionally consuming alcohol. H5 The older Polish minority group population has higher frequency of activities with the family than the younger Polish minority group population. H6 The confession of faith of the Polish minority group population significantly affects their approach to abortion compared to the nonbelieving population. H7 Older women of the Polish minority group population give more frequent preference to home care services than men of the Polish minority group population. H8 Women of the Polish minority group population more frequently require attendance of family members when dying than men. Methodology: The research part of the work was carried out by the method of quantitative research investigation. The selected set consisted of 277 persons. The results: 94,90 % of the respondents agree that they take care of their health. However, H1 regarding approach to health was not confirmed. By comparing the results of the investigation we found out that more than 75 % of the respondents take proper prevention in all cases. H2 focusing on prevention was confirmed. The level of consummation of vegetable and fruit in case of the Polish national minority group is higher than the level of the Czech population. H3 was confirmed by testing. H4 connected with consummation of alcohol was not confirmed. This was confirmed in the research investigation; however, H5 was not confirmed. The result of 88.40 % respondents who confessed their faith has not impacted the result of H6 which was not confirmed. The objective of the work included also identification of the attitude and experience with home care agency which was indicated by 46.60 % respondents. H7 was not confirmed. We were surprised by the identified attitude of the respondents to their faith in live after death, to attendance of family members or a priest in case of dying when in all cases more than 20% respondents responded that they did not know. H8 was not confirmed by testing. Conclusion: The thesis gives a summary of information on the Polish national minority group living in the Těšín region of Silesia. The final thesis outlines a proposal for a Standard of nursery care of the Polish national minority group. The results of the research investigation will be also presented at conferences. This thesis may be used by students of the nursery field as a teaching material.
