Considerando as particularidades do idoso indígena, o contexto alimentar em Instituições de Saúde carece atender aspectos inerentes à sua cultura, crenças e valores que permeiam o ato de alimentar-se, que apesar das influências de outros povos, apresenta suas raízes ligadas a tradições milenares. O estudo foi organizado conforme as normas do Programa de Pós-Graduação em Enfermagem da Universidade Federal de Pernambuco em 4 capítulos: revisão de literatura, métodos e resultados do artigo de revisão e artigo original. O artigo de revisão teve como objetivo avaliar as evidências disponíveis acerca da questão alimentar e nutricional do idoso, em especial o indígena, cuja busca foi realizada nas bases de dados: Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Literatura Internacional da Área Médica e Biomédica (MEDLINE), Base de Dados em Enfermagem (BDENF), selecionados 13 artigos conforme critérios preestabelecidos. Os resultados mostraram estudos em diversos países, porém nenhum voltado ao idoso indígena, estes apontam mudanças nos hábitos alimentares de idosos, associados principalmente a questões socioeconômicas e culturais, além das mudanças ocorridas com a globalização e a transição nutricional. O artigo original teve como objetivo avaliar o contexto cultural da alimentação de idosos indígenas. O estudo descritivo, transversal, com abordagem quantitativa, teve como local de estudo a Casa de Saúde do Índio CASAI de Manaus no período de 90 dias com a participação de 30 idosos internados neste período. Utilizou-se para análise dos dados estatística descritiva mediante o Software Estatístico Livre R, com embasamento teórico conceitual da Teoria do Cuidado Cultural proposto por Madeleine Leininger. Os resultados demonstram as novas conformações acerca da visão de mundo e das dimensões das estruturas social e cultural, representada pela presença de elementos tecnológicos, nova configuração religiosa, modos de vida, questões econômicas, educacionais e de ambiente. Neste contexto, cada fator oriundo do modelo Sunrise de Leininger demonstra como o ambiente e o contexto social e cultural direcionam o padrão alimentar do idoso seja na aldeia ou em Instituições de Saúde. No aspecto geral da alimentação e nutrição na CASAI, os alimentos oferecidos são bem aceitos, porém apresentam-se parcialmente diferentes daqueles consumidos na aldeia, podendo estar relacionado à tendência de significativa parcela referenciar a diminuição do apetite após internação e apresentar quadro de desnutrição segundo avaliação pelo índice de massa corporal. Partindo destes resultados foi elaborado material informativo sobre o contexto cultural da alimentação de idosos indígenas voltados para melhoria do planejamento de cuidados de enfermagem transcultural. Conclui-se que o contexto alimentar dos idosos está intimamente arraigado à sua cultura, porém apresenta mudanças nos hábitos alimentares com forte influência da globalização. Inerente à saúde, os pontos principais relacionam-se à presença de doenças crônicas como hipertensão e diabetes, o alto consumo de açúcares e gorduras, diminuição do apetite após internação e quadro de desnutrição.

Objetivo: adaptar transculturalmente e validar o Drug-Taking Confidence Questionnaire - DTCQ-8 álcool e DTCQ-8 outras drogas. Métodos: trata-se de um estudo metodológico realizado em duas fases: execução do protocolo de adaptação transcultural e verificação das propriedades psicométricas do DTCQ-8 álcool e DTCQ-8 outras drogas. Para o processo de adaptação transcultural, participaram nove especialistas e uma amostra de 40 usuários de álcool e 40 usuários de outras drogas em tratamento em Centro de Atenção Psicossocial para álcool e outras drogas (CAPSad). Já no processo de validação clínica do instrumento, a amostra constituiu-se de 50 usuários de álcool e 50 usuários de outras drogas, também em tratamento no CAPSad. Resultados: as médias dos índices de concordância das equivalências para ambas as versões do instrumento foram: semântica (0,989; 0,989; 1,00), idiomática (0,967), experiencial (0,956), conceitual (0,978) e da validação de conteúdo relativo à clareza de linguagem (0,972), pertinência prática (0,958), relevância teórica (0,958) e para dimensão teórica (1,00) demonstraram um processo satisfatório. A maioria dos participantes foi classificada em moderada autoeficácia, sendo a média dos escores total do DTCQ-8 na amostra pré-final 68,06 para álcool e 59,63 para outras drogas, bem como na amostra final 56,40 para álcool e 54,55 para outras drogas. A análise fatorial identificou que todos os itens obtiveram altas cargas fatoriais, sendo agrupados em quatro domínios. Os grupos contrastados demonstraram que a iniciação ao consumo de substâncias psicoativas em idade precoce reduziu os escores de autoeficácia tanto para o instrumento completo quanto para os domínios em ambos os grupos. O coeficiente alfa de Cronbach para o instrumento completo DTCQ-8 na amostra pré-final foi de 0,87 para álcool e 0,88 para outras drogas, bem como 0,89 na amostra final para álcool e 0,82 para outras drogas. Conclusão: o processo de validação do Drug-Taking Confidence Questionnaire DTCQ-8 álcool e DTCQ-8 outras drogas descrito nesta tese demonstrou que são de fácil aplicação e compreensão, sendo satisfatório o processo de adaptação para utilização no contexto brasileiro. Os resultados mostraram que o instrumento, em suas duas versões, pode ser considerado válido e confiável para mensurar a autoeficácia do usuário de álcool e/ou outras drogas para resistir ao desejo de consumir essas substâncias em situações de alto risco, no contexto do Brasil. Acredita-se que a contribuição deste estudo abrange a utilidade clínica, por contribuir com a práxis profissional e com a compreensão dos usuários sobre sua dinâmica comportamental, bem como para a pesquisa, por ser uma ferramenta útil à investigação científica sobre a autoeficácia de resistir ao desejo de consumir substâncias psicoativas em situações de alto risco. / Introduction: The Drug-Taking Confidence Questionnaire measures the confidence of users of alcohol and/or other drugs to resist the urge to consume these substances in high-risk situations. Objective: The aim of the present study was cross-culturally adapt and validate the Drug-Taking Confidence Questionnaire (DTCQ-8 alcohol and DTCQ-8 other drugs). Methods: A methodological study was conducted in two phases: cross-cultural adaptation and the determination of the psychometric properties of the DTCQ-8 alcohol and DTCQ-8 for other drugs. The cross-cultural process was performed with the participation of nine specialists and 40 users of alcohol and 40 users of other drugs at a Psychosocial Care Treatment Center. The clinical validation processes of the questionnaire involved a sample of 50 users of alcohol and 50 users of other drugs at the same treatment center. Result: The mean concordance indices of equivalence of both versions of the questionnaire were 0.989, 0.989 and 1.00 for semantics, 0.967 for idiomatic equivalance, 0.956 for experiential equivalence, 0.978 for conceptual equivalence and 0.972 for validation of content related to language clarity, 0.958 for practical pertinence, 0.958 for theoretical relevance and 1.00 for the theoretical dimension, demonstrating a satisfactory process. The majority of participants were classified with a moderate degree of self-efficacy, with mean total DTCQ-8 scores of 68.06 in the pre-final sample for alcohol and 59.63 for other drugs as well as 56.40 in the final sample for alcohol and 54.55 for other drugs. The factorial analysis demonstrated that all items had high factorial loads grouped in four domains. The groups registered demonstrated that the onset of taking psychoactive drugs at an early age reduced the self-efficacy scores for both the complete questionnaire and the domains in both groups. Cronbach’s alpha coefficient for the complete DTCQ-8 was 0.87 in the pre-final sample for alcohol and 0.88 for other drugs as well as 0.89 in the final sample for alcohol and 0.82 for other drugs. Conclusion: The validation of Drug-Taking Confidence Questionnaire (DTCQ-8) for alcohol and other drugs proved to be of easy administration and comprehension and the adaptation to the Brazilian culture proved to be successful. The results demonstrate that the two versions of the questionnaire can be considered valid and reliable for measuring self-efficacy among users of alcohol and/or other drugs regarding resisting the urge to consume these substances in high risk situations in the Brazilian context. This study contributes to clinical practices and the understanding of the dynamic behavior of users as well as being a useful tool for the scientific investigation of self-efficacy regarding resisting the urge to consume psychoactive drugs in high-risk situations.

Transcultural Representation of Circassian Music in Mily Balakirev's Islamey: Oriental Fantasy (1869)

This thesis examines the context in which the Russian composer, M. Balakirev, used folk idioms to create a representation of Circassian musical culture in Islamey: Oriental Fantasy. It was composed for piano in 1869 after Balakirev’s trip to the Caucasus during its colonization by the Russian Empire and it is an interesting example of a musical representation of the "Oriental." Based on Edward Said’s theory of the Orientalism, this thesis examines the construction of Circassian stereotype as Russia’s “Other”. It presents the complex historical relations between Russians and Circassians and discusses evidence for treating Islamey as an example of an Orientalist Other, taken from several authors, including Balakirev himself. Ultimately, this thesis describes Balakirev’s representation of Circassian culture and proposes reasons for his appropriation of folk music. It concludes by outlining the benefits of cultural exchanges between the Russian and Circassian nations that took place over the last century.

Transnational Organizations' Cultural Shift Through Transcultural Communication Generated by E-learning via the Global Learning Organization (GLO) Model

Dynamic networking is a key factor for successful transnational organizations. The transcultural shift is a critical process that can enable cultural hybridization so as to inspire consensual identity and learning aptitude amongst worldwide members. The Global Learning Organization (GLO) model is re-conceptualized to bring about this cultural shift. E-learning seems an appropriate tool to generate effective transcultural communication for both culture and learning perspectives under the GLO model. A qualitative case study using document analysis and interviews is conducted to understand how transcultural communication is generated via e-learning under the GLO model in two fields. Findings reveal that firstly, trust is a core element in generating transcultural communication and the combination of face to face and e-learning can enable trust to be activated and developed. Secondly, the way to build trust varies depending on task characteristics: the detail-oriented tasks require more intense face to face communication than the concept-focused tasks. This study illustrates that design of various mixed learning pattern with strategies to build trust through the affective dimension will be key for the successful GLO.

Aspects of culture in South African psychiatry

Bibliography: pages 351-389. / A review of the South African psychiatric literature reveals that the concept of culture is commonly reified. It is also used by the South African state to legitimate apartheid. The concept of cultural relativism, though often associated with liberal views internationally, is linked with state policies in South Africa. Some South African social scientists, therefore, strongly question the notion of relativism. This reaction unfortunately does not engage with the social reality of the widespread perception of cultural differences, in psychiatric settings and elsewhere. Issues of race and culture in psychiatric practice were explored in a psychiatry department of a liberal South African university. Observation of ward-rounds in a psychiatric casualty (emergency) facility over six months revealed that, as elsewhere in the world, a major cultural factor influencing clinicians is the relationship between psychiatry and general medicine. A cultural understanding of South African psychiatry must take account of this relationship. Ward-rounds in a facility treating Black psychiatric patients were observed over fifteen months. Black and white clinicians in these rounds were often in conflict over constructions of the concept of culture. Some appeared deeply ambivalent about cultural relativism. Psychiatric registrars (residents) attached to the department under study participated in loosely structured interviews exploring issues of race and culture in their work. They also responded to vignettes dealing with white, coloured and Black patients. Registrars felt uncomfortable about the role of the concept of cultural difference in affecting the welfare of Black patients, and in maintaining discrimination. Their own socialisation as practitioners in an individualising and medicalising discipline seems a major factor contributing to their ongoing reproduction of this discrimination. The study reveals the importance of exploring the views and experiences of practitioners. South African work focussing on the need for fundamental change in mental health care has generally glossed over details of extant practice. This dissertation shows, however, that a major site for mobilisation for change in South African mental health-care must be the psychiatric institution itself.

Bakgrund: Miljoner av människor rör sig ständigt mellan länder av olika orsaker. Det är vanligt att sjuksköterskor vårdar och bemöter människor med en annan kulturell bakgrund. Forskning visar på att hälsoproblem hos migranter skiljer sig från den inhemska befolkningen. Således är det viktigt att sjuksköterskan besitter kunskap om olika kulturer för att tillgodose unika behov och därmed uppfylla en säker och personcentrerad vård. Syfte: var att belysa sjuksköterskors erfarenheter av att vårda patienter med en annan kulturell bakgrund. Metod: Studien genomfördes som en allmän strukturerad litteraturöversikt med induktiv ansats utifrån 12 resultatartiklar. Resultat: Utfallet resulterade i två teman: Erfarenheter av svårigheter i mötet med patienterna och Erfarenheter av kulturella olikheter med tillhörande underteman. Konklusion: Resultatet visade på att språket var en viktig del av kommunikationen, när det fanns språkbarriärer var det svårare att skapa en relation med patienten. När kulturell kompetens saknades upplevde sjuksköterskorna detta vara ett hinder som kunde skapa en osäkerhet i omvårdnaden. Erfarenheten i mötet med patienterna visade på att religion och olika preferenser kunde upplevas frustrerande samt tidskrävande, vilket kunde leda till etiska dilemman. / Background: Millions of people are constantly migrating between countries for various reasons. It is common for nurses to care for and treat people with a different cultural background. Research shows that the health problems of migrants differ from the domestic population. Therefore, it is important that the nurse possesses knowledge of different cultures to meet unique needs to meet a safe and person-centered care. Purpose: was to shed light on the nurses' experiences of caring for patients with a different cultural background. Method: The study was conducted as a general structured literature review with an inductive approach based on 12 result articles. Results: The outcome resulted in two themes: Experiences of difficulties in meeting patients and Experiences of cultural differences with associated sub-themes. Conclusion: The result showed that language was an important part of communication, when there were language barriers it was more difficult to create a relationship with the patient. When cultural competence was lacking, the nurses perceived this as an obstacle that could create uncertainty in nursing. The experience in the meeting with the patients showed that religion and different preferences could be experienced as frustrating and time-consuming, which could lead to ethical dilemmas.

CHINESE FANS, TRANSCULTURAL FANDOM: AN ETHNOGRAPHIC APPROACH

My project delves into Chinese fans’ everyday meaning-making, situating their local consumption of global culture in China’s particular historical, social and cultural contexts, and connecting their identity work with global processes of hybridization. I explored questions of how Chinese fans consume global popular culture in China’s particular historical, social and cultural contexts, how the meaning-making of the fan identity intersects with gender and nationality in a transcultural fandom, and how investigating Chinese fandom in global processes of hybridization in China’s unique context contributes to a deeper understanding of fandom in general. I conducted participant observation, in- depth interviews, and textual analysis in three transcultural fandoms in China, including a cinephile community of Marvel Cinematic Universe (MCU), an idol fan community of Produce Camp 2021, and a cinephile community of Hollywood movies. I combined the postcolonial concept of hybridity with fandom theories to tease out the complexity of power dynamics in fans’ negotiation of gender and nationality. My analysis points to the constructed nature of fan communities to rethink all local fandoms as transcultural, as they stand at the intersections of varied transcultural media consumptions in globalization. It reveals how hybridity constitutes the inevitable condition of local fandom. At the same time, it seeks to develop a deeper understanding of how American hegemony, as a historical force of concern to globalization scholars, remains influential in local processes of hybridization in an increasingly globalized world. / Media & Communication

Ser enfermeiro negro na perspectiva da transculturalidade do cuidado / Being Black Nurses in the Transcultural Care Perspective.

O presente estudo teve como objetivo central descrever questões da identidade do ser enfermeiro negro formado pela Escola de Enfermagem da Universidade de São Paulo, a partir da perspectiva da Teoria da Enfermagem Transcultural através da análise de situações de preconceito vividas por esses enfermeiros frente a sua escolha profissional, sua formação acadêmica e a sua inserção no mercado de trabalho. Por ser o presente trabalho de natureza descritiva, histórico-social e exploratória, optou-se pelo método da História Oral, que é um método de pesquisa que utiliza a técnica da entrevista e outros procedimentos articulados entre si, no registro de narrativa da experiência humana. Optou-se também pelo método da História Oral Temática pelo fato desse ser um método que possibilita que as pessoas falem livremente, em seus respectivos contextos. A Escola de Enfermagem da USP formou, no período de 1947 a 2006, 2.886 enfermeiros, dos quais, 128 se declararam como não brancos. Foram localizados, no estado de São Paulo, 45 dos acima identificados, dos quais 14 cederam entrevistas para a presente investigação. Pode-se observar que a terminologia moreno(a) foi a mais utilizada para a autodeclaração e que, dos entrevistados, todos afirmam ter sofrido preconceito racial em algum momento de suas trajetórias de vida, em especial de forma velada, forma essa mais difícil de ser enfrentada. A presente investigação propiciou fazer da História uma atividade mais democrática a cargo das próprias comunidades, uma vez que permitiu construir a História a partir das próprias palavras daqueles que a vivenciaram e que participaram de um determinado período, mediante suas referências e, também, do seu imaginário, possibilitando o registro de reminiscências das memórias individuais ou a reinterpretação do passado. / This study aimed to describe the central issues of being a black nurse graduated by the School of Nursing, University of São Paulo, from the perspective of the Theory of Transcultural Nursing trough the analysis situations of prejudice experienced by these nurses facing their career choice, their academic training and their integration into the labor market. As the study has a descriptive, historical, social and exploratory character, was chosen the method of Oral History, which is a method that uses the interview and other processes linked together in the narrative record of human experience. It was also chosen the method of Oral History Thematic considering the fact that this method helps people to speak freely in their respective contexts. The School of Nursing graduated, in the period of 1947 to 2006, 2,886 nurses, of which 128 identified themselves as non-whites. Located, in the state of São Paulo, 45 of the above identified, of which 14 were interviewed for this research. It was noted that the terminology \"moreno\" was the most used for self reporting and that of those interviewed, all stated have suffered racial discrimination at some point in their life trajectories, especially in hidden form, which is the harder form to face. This research led to a more democratic history over the view of its own communities, because it allows the construction of history from the very words of those who participated and experienced, in a given period of time, through its references and imagination, allowing the registration of reminiscences of individual memories or the reinterpretation of the past.

Studiens syfte var att undersöka psykoterapeuters upplevelser av arbete med traumatiserade patienter från andra kulturer. Fokus låg på att undersöka de svårigheter och hinder som kan uppstå till följd av kulturella skillnader mellan terapeut och patient. Högre förekomst av psykiatriska tillstånd bland flyktingar i kombination med postmigratoriska sårbarhetsfaktorer ställer krav på terapeuters kompetens. Kvalitativ metod med semistrukturerade intervjuer användes. Data analyserades tematiskt utifrån ett socialkonstruktivistiskt perspektiv. Resultaten visar att kulturell kompetens är otillräcklig inom mångkulturell vård och framhäver ett behov av ökad insikt i den egna kulturen, inklusive det västerländska synsättet på psykisk ohälsa. Studien lyfter fram betydelsen av en ideografisk ansats i terapisammanhang och förespråkar ett kritiskt förhållningssätt till det västerländska perspektivet vid arbete med patienter från andra kulturer. / The aim of the study was to explore psychotherapist’s experience of trauma focused treatment with transcultural encounters, focusing on examining difficulties and obstacles that might arise due to cultural differences between therapist and patient. Higher prevalence of psychiatric illness among refugees in combination with post-migration vulnerabilities requires competence among therapists.  Qualitative approach with semi-structured interviews was used and data was analyzed thematically from a social-constructivist perspective. Results show that cultural competence is insufficient within transcultural care settings and highlights the need for increased insight into care givers own culture, including the Western perspective of psychiatric illness. The study emphasizes the value of an ideographic approach in therapy and suggests a critical attitude towards the Western perspective when working with transcultural patients.

This knownledgment is important to avoid possible conflicts, shocks, difficulties and health care incongruence within this context. The data collect was carried out on a range of non structured interview guided by a pre-elaborated questionnaire with four questions and a hand drawing related to nurse s health care in the indigenous health. This research had a sample of 17 nurses from the Indigenous Sanitary District of Manaus in the Amazon State. To interpret data we used the Discourse of the Collective Subject, which findings were presented in three chapters: characterization of participants, discussion on themes prevalent in discourse; social representation of nursing care through infographics. The analysis revealed that the care in the indigenous health is challenging because the native people imbued in its world are perceived and processed according to the nurse s cultural lens, leading to materialize of some strangeness and adaptation difficulties, especially in the first contacts. The Social Representation on nursing practice, in many cases, is projected and contrived on the basis of scattered believes and on perception derived from common sense. The findings shows that representions are essential to mitigating the initial strangeness and help nurses to better situate themselves in the new universe. The nurse s practice in the indigenous health care should merge into each other. From the Social Representations is possible to perceive that assimilation, also comprehension on indigenous health system and its traditional knowledge are important to developing strategies to improve access and quality of care for indigenous peoples. After analysis the nurse s discourses and drawings, it is possible to represent the nurse s practice in the indigenous health as anthropophagism, since nurses should literally consuming its patients culture, digesting it and seize it as means to provide culturally congruent care. We highlight the urgent need for preparation and training of professionals to work more effectively with indigenous peoples / A realiza??o deste estudo surgiu a partir da experi?ncia como enfermeira na sa?de ind?gena, ao observar que muitos profissionais oriundos das mais diversas regi?es do Brasil optavam por esta ?rea de atua??o. Era not?ria a dificuldade que o enfermeiro tinha em fixar-se por muito tempo em um ?nico local de servi?o. Provavelmente porque o cuidado de sa?de na ?rea ind?gena acontece a partir de um confronto de culturas diversas, pois traz o profissional, de um lado, com todo o seu conhecimento acad?mico e cultura pr?pria e, do outro lado, o ?ndio com seus ritos, usos e costumes peculiares. Neste contexto, o enfermeiro deve definir e negociar a realidade atrav?s de representa??es simb?licas da vida, para, em seguida, questionar a nova realidade. Assim, este estudo busca apreender as representa??es sociais sobre o cuidado transcultural na sa?de ind?gena para os profissionais enfermeiros. Conhecimento, este, importante para evitar poss?veis conflitos, choques, dificuldades e incongru?ncia do cuidado nesse contexto. A coleta de dados foi realizada atrav?s de entrevista n?o-estruturada guiada por um roteiro contendo quatro quest?es norteadoras e elabora??o de um desenho que fosse relacionado ao cuidado do enfermeiro na sa?de ind?gena. A pesquisa contou com a participa??o volunt?ria de 17 enfermeiros do Distrito Sanit?rio Ind?gena de Manaus, no Estado do Amazonas. Para o tratamento dos dados foi utilizada a T?cnica de An?lise do Discurso do Sujeito Coletivo, sendo os resultados apresentados em tr?s momentos, a saber: caracteriza??o dos participantes do estudo, discuss?o das categorias predominantes nos discursos e representa??o social do cuidado de enfermagem atrav?s de infogr?ficos. A an?lise revelou que o cuidado na sa?de ind?gena ? desafiador porque o ind?gena e seu mundo s?o percebidos e processados de acordo com a lente cultural dos enfermeiros, levando ao surgimento de alguns estranhamentos e dificuldades de adapta??o, principalmente nos primeiros contatos. A RS da pr?tica de enfermagem na sa?de ind?gena, em muitos casos, ? projetada e idealizada com base nas cren?as disseminadas e nas percep??es originadas do senso comum. Percebe-se, pois, que o trabalho de representar ? essencial em atenuar as estranhezas iniciais e ajudar o enfermeiro a situar-se melhor no novo universo. A pr?tica de enfermagem na sa?de ind?gena deve ser a fus?o das culturas do ind?gena e do enfermeiro. A partir da Representa??o Social reconhece-se que a assimila??o, a compreens?o do sistema de sa?de ind?gena e o uso dos conhecimentos dessas pr?ticas s?o elementos essenciais para o desenvolvimento de estrat?gias que podem melhorar o acesso e a qualidade do cuidado aos povos ind?genas. Ap?s a an?lise dos discursos e desenhos realizados pelos enfermeiros, pode-se representar a pr?tica de enfermagem na sa?de ind?gena atrav?s do antropofagismo, no qual o enfermeiro deve literalmente devorar a cultura dos seus pacientes, digeri-la e apoderar-se dela para prestar um cuidado culturalmente congruente. No entanto, ressalta-se a necessidade urgente do preparo e treinamento desses profissionais para uma atua??o mais eficaz com os povos ind?genas
