Síndrome metabólica e perfil de adipocitocinas séricas em pacientes adultas jovens com dermatomiosite / Metabolic syndrome and serum adipocytokine features in young adult patients with dermatomyositis

Objetivo. Analisar a frequência de síndrome metabólica em pacientes adultas jovens com dermatomiosite (DM) e a possível associação de síndrome metabólica com as características clínicas e laboratoriais da DM. Posteriormente, analisar os níveis séricos das adipocitocinas em pacientes com DM. Métodos. O presente estudo unicentro e transversal incluiu 35 pacientes com DM, de acordo com os critérios de Bohan e Peter, pareadas por idade e índice de massa corpórea com 48 controles saudáveis. A atividade da doença foi baseada nos parâmetros estabelecidos pelo International Myositis Assessment and Clinical Studies Groups (IMACS). A síndrome metabólica foi definida de acordo com critérios preconizados por Joint Interim Statement de 2009. Resultados. A média de idade foi comparável entre DM e o grupo controle (respectivamente, 33,2 ± 6,5 e 33,3 ± 7,6 anos), com duração média da doença de um ano. Quando comparadas aos indivíduos do grupo controle, as pacientes com DM tinham alta prevalência de síndrome metabólica (34,3 vs. 6,3%; P = 0,001), assim como altos níveis séricos de adiponectina e resistina, em contraste com baixos níveis de leptina. Estas adipocitocinas se correlacionavam com vários parâmetros da dislipidemia em pacientes com DM. Além disto, os casos de DM com síndrome metabólica (N = 12) apresentaram maior faixa etária (36,7 ± 5,6 vs. 31,5 ± 8,0 anos; P = 0,035) e maior atividade da doença do que os casos sem síndrome metabólica (N = 23). Entretanto, a distribuição de adipocitocinas foi similar entre os grupos. Conclusão. Quando comparadas ao grupo controle, as pacientes adultas jovens com DM apresentam maior prevalência de síndrome metabólica e maiores níveis séricos de adiponectina e resistina, em contraste com menores níveis séricos de leptina. Entre as pacientes, a síndrome metabólica correlacionou-se positivamente com a maior faixa etária e com a atividade da doença / Objective. Avaliação radiográfica comparativa de quadris dolorosos e sem dor em indivíduos adultos / Comparative radiograph evaluation of adult patients with and with out hip pain

O objetivo do estudo foi correlacionar a presença de alterações radiográficas em quadris de pacientes portadores de dor no quadril a um grupo de pacientes sem dor no quadril. Foram avaliados 122 pacientes com dor no quadril, e 100 pacientes sem dor no quadril. Todos com idade entre 20 e 50 anos. Os pacientes foram submetidos a exames radiográficos, nas incidências AP de pelve ortostático, falso perfil de Lequesne, Dunn, Dunn 45° e Ducroquet. Os parâmetros avaliados foram inclinação e versão acetabular, ângulo CE de Wiberg, espaço articular superolateral, ângulo , ângulo de Lequesne, ângulo VCA, diferença entre o raio da cabeça e do colo femorais (off set), esfericidade da cabeça femoral e a presença de ressalto na transição da cabeça com o colo femoral. Concluiu-se que as melhores incidências para o diagnóstico de impacto femoroacetabular são AP de pelve ortostático, Dunn 45° e Ducroquet, e que as seguintes alterações estão correlacionadas com dor nos quadris: off set diminuído, ângulo aumentado, ângulo de Lequesne aumentado, ângulo CE de Wiberg diminuído, espaço articular diminuído e presença de ressalto na transição cabeça-colo femoral / This study aimed to compare two groups of patients, with and with out hip pain, and correlate them with the existence of radiograph alterations. Weve studied 122 patients with hip pain and 100 asymptomatic; the ages were between 20 to 50 years old in both groups. All patients were roentnographicaly studied in anteroposterior pelvis in orthostatic position, Lequesne false profile, Dunns, Dunns 45° and Ducroquets view. The radiograph parameters analized were the acetabular inclination and vertion, Wiberg CE angle, Lequesne VCA angle, Lequesnes antevertion angle (), angle, superior and lateral joint space, femoral off set, nonspherical of the femoral head and the presence of a bump on the head-neck transition. We conclude that the best incidences to diagnosis of femoroacetabular impingement are AP pelvis orthostatic, Dunn 45° and Ducroquet. The following alterations are correlate with hip pain: lower off set, higher angle, higher Lequesne angle, lower CE angle of Wiberg, lower articular space and bump in the femoral head-neck transition

Situational action theory and intimate partner violence : an exploration of morality as the underlying mechanism in the explanation of violent crime

Despite the criminal nature of intimate partner violence, scholars infrequently apply general theories of crime to understanding its causes (Dixon, Archer, & Graham-Kevan, 2012). Indeed, some scholars reject the notion that the causes of intimate partner violence align with the causes of general crime and violence (Dobash, Dobash, Wilson, & Daly, 1992). A second area of contention is whether male and female violence can be explained within the same theoretical framework (Dutton & Nicholls, 2005). In this thesis I argue that as a type of criminal behaviour, understanding the causes of intimate partner violence from a criminological perspective is a valid and necessary research endeavour. Further, guided by the principles of the theoretical framework of this thesis, I submit that both male and female intimate partner violence can be explained within the same general theory of crime. This thesis applies situational action theory, a general theory of crime that places morality at the centre of its explanatory framework, to the understanding and explanation of intimate partner violence. This thesis concentrates on the roles of personal morality and provocation in intimate partner violence perpetration. Partner conflict is defined as the experience of provocation, while friction sensitivity and low partner cohesion are included as key factors leading to partner conflict. Specifically, this thesis examines whether the strength of personal morality influences whether individuals respond to provocation with violence against a partner. To address the aims of the research, this thesis uses data from the Peterborough Adolescent and Young Adult Development Study, a study designed to test situational action theory. Participants are a representative sample of males and females between 24 and 25 years of age. Path analyses using a multiple-group method revealed that high friction sensitivity and low partner cohesion contributed to increased partner violence perpetration by influencing the level of partner conflict. Morality had a significant moderating effect on the path between partner conflict and partner violence perpetration. Namely, individuals with weak morality, and who frequently engaged in partner conflict, were significantly more likely to perpetrate acts of partner violence than individuals with strong morality who engaged in frequent conflict with a partner. These findings were replicated across males and females. The findings of this research illustrate the importance of morality in the explanation of partner violence, and provide evidence that both male and female partner violence can be explained within the framework of situational action theory.

Síndrome metabólica em pacientes jovens na pré-menopausa com lúpus eritematoso sistêmico / Metabolic syndrome in young premenopausal patients with systemic lupus erythematosus

Introdução: A síndrome metabólica (SM) é preditor independente de doença cardiovascular, a principal causa de mortalidade no Lúpus Eritematoso Sistêmico (LES). Não existem dados sobre os principais fatores associados à SM em pacientes jovens na pré-menopausa, população mais afetada pelo LES. Objetivo: Avaliar a prevalência da SM em mulheres jovens na pré-menopausa com LES e identificar fatores relacionados a doença e à terapêutica que contribuem para a SM, utilizando a análise pelo propensity score. Materiais e Métodos: Foram avaliadas 103 pacientes com LES (critérios do American College Rheumatology 1997) na pré-menopausa, com idade inferior a 40 anos de idade. Foram selecionadas 35 mulheres saudáveis como controles, com menos de 40 anos de idade, sem doenças crônicas e autoimunes. Os critérios de exclusão foram idade inferior a 18 anos, menopausa e gravidez. Parâmetros clínicos, laboratoriais e de terapêutica foram avaliados. A definição da SM foi feita de acordo com os recentes critérios do Joint Interim Statement de 2009. Análise multivariada utilizou a regressão de Poisson e a análise pelo propensity score foi realizada para o controle das variáveis de confusão. Resultados: A prevalência de SM foi mais elevada no grupo LES (22,3 vs. 5,7%; p=0,03), assim como o risco cardiovascular pelo Systematic Coronary Risk Evaluation (SCORE) (1,4 ± 0,8 vs. 1,1 ± 0,4; p=0,01). Hipertensão arterial sistêmica (42,7 vs. 2,9%; p<0,0001) e circunferência abdominal aumentada (83,5 vs. 37,1%; p < 0,0001) foram critérios da SM mais frequentes no LES, que apresentou maiores Homeostasis Model Assessment Index (HOMA-IR) (1,8 + 0,9 vs. 1,3 + 1,0; p=0,0008). Não houve diferença significativa quanto à idade, tempo de doença e pontuação no Systemic Lupus International Collaborative Clinics (SLICC/ACR DI) entre os grupos LES com e sem SM. No grupo com LES com SM, os escores do Systemic Lupus Erythematosus Disease Activity Index (SLEDAI) foram significativamente mais elevados (5,9 ± 7,6 vs. 1,9 ± 2,7; p=0,006), assim como atividade renal prévia (73,9 vs. 51,2%; p=0,05) e atividade renal atual (34,8 vs. 10,0%; p=0,008), dose atual de prednisona (20 [0-60] vs. 5 [0-60]mg/dl; p=0,018) e dose cumulativa de prednisona (41,48 ± 24,7 vs. 27,81 ± 18,66g; p=0,023). A cloroquina foi menos utilizada nos pacientes LES com SM (65,2 vs. 90,0%; p=0,008). Na análise multivariada, apenas o uso atual de cloroquina (razão de prevalência [RP]=0,29; IC95% 0,13-0,64) e dose cumulativa de prednisona foram associados com SM (RP=1,02; IC95% 1,01-1,04), mesmo após ajuste pelo propensity score. O uso de cloroquina determina uma redução de 71% na prevalência de SM no LES. Por outro lado, para cada aumento de 1g da dose cumulativa de prednisona determina um aumento de 2% na prevalência de SM. Importante notar que o uso da cloroquina reduziu a prevalência estimada de SM mesmo quando do uso de corticosteroides, e este benefício foi maior quanto maior a dose cumulativa de prednisona. Conclusão: A prevalência da SM em pacientes jovens com LES na pré-menopausa é alta, sendo principalmente influenciada pelas terapias com prednisona ou cloroquina. Os antimaláricos possuem um efeito protetor sobre a prevalência da SM no LES, sendo que este benefício compensou o efeito deletério do corticoide de maneira dose-dependente / Background: There are no data about the main factors associated with metabolic syndrome (MetS) in young premenopausal systemic lupus erythematosus (SLE) patients. Objectives: The aim of the study was to evaluate the frequency of MetS and disease- or therapy-related factors in premenopausal young SLE patients. Methods: 103 premenopausal SLE patients with age less than 40 years old were selected and compared to 35 healthy premenopausal age-matched female. MetS was defined according to the 2009 Joint Interim Statement. Results: A higher frequency of MetS (22.3 vs. 5.7%, p=0.03) was observed in SLE group. MetS-SLE patients presented higher SLE Disease Activity Index (SLEDAI) scores (5.9 ± 7.6 vs. 1.9 ± 2.7, p=0.006), more frequently previous (73.9 vs. 51.2%, p=0.05) and current renal disease (34.8 vs. 10.0%, p=0.008), higher current prednisone dose (20 [0-60] vs. 5 [0- 60] mg/dl, p=0.018) and cumulative prednisone dose (41.48 + 27.81 vs. 24.7 + 18.66 g, p=0.023) than those without MetS. Chloroquine was less frequently used in MetS-SLE patients (65.2 vs. 90.0 %, p=0.008). In multivariate analysis, only current chloroquine use (prevalence ratio [PR]=0.29; 95% CI 0.13-0.64) and cumulative prednisone were associated with MetS (PR=1.02; 95% CI 1.01- 1.04). Further estimated prevalence analysis identified that antimalarial use promoted continuous decrease in the progressive MetS prevalence associated with glucocorticoid cumulative dose. Conclusion: The prevalence of MetS in premenopausal young adult SLE patients is high, and is mainly affected by steroid and antimalarial therapies. Chloroquine has protective effect on the prevalence of MetS in these patients and this benefit counteracts the deleterious effect of glucocorticoid in a dose dependent manner

Qualidade de vida no adolescente e adulto jovem com fibrose cística: correlação com a gravidade da doença / Quality of life in adolescents and young adults with cystic fibrosis: correlations with disease severity

Introdução: A qualidade de vida em pacientes com fibrose cística (FC) tem sido cada vez mais estudada podendo estar relacionada com dados clínicos e laboratoriais que demonstram a gravidade da doença. Métodos: Estudo prospectivo e transversal com aplicação do questionário de qualidade de vida em FC (CFQ-R) traduzido e validado para o português em pacientes com FC estáveis de 14 a 21 anos. Foram feitas as correlações de domínios do CFQ-R com o volume expiratório forçado no primeiro segundo (VEF1), o escore de Shwachman-Kulczychi (E-SK) e o índice de massa corpórea (IMC), além da comparação dos resultados de todos os domínios entre os sexos e entre pacientes com VEF1 acima ou abaixo de 60% do previsto. Resultados: Foram estudados 31 pacientes, 11 do sexo feminino e 20 do sexo masculino, com idade média de 16,4 ± 2,3 anos. As pontuações dos domínios do CFQ-R foram variáveis mas se situaram ao redor de 80 em mediana. Observaram-se correlações significantes entre o IMC e os domínios peso (r=0,43, p=0,016) e alimentar (r=0,44, p=0,013), VEF1 e domínios físico (r=0,53, p=0,002) e tratamento (r=0,41, p=0,023), E-SK e domínios físico (r=0,39, p=0,03), saúde (r=0,41, p=0,023) e papel social (r=0,37, p=0,041). Houve diferença significativa no grupo com VEF1 acima e abaixo de 60% do previsto nos domínios papel social e saúde. Não houve diferença entre os sexos nos domínios. Conclusões: Observou-se uma heterogeneidade das pontuações do CFQ-R em cada um dos 12 domínios mas com dados que sugerem uma qualidade de vida relativamente preservada na maioria dos pacientes estudados. Houve correlação significativa entre alguns domínios do CFQ-R com parâmetros clínicos e funcionais, demonstrando que o CFQ-R apresenta um poder discriminatório na avaliação da gravidade da doença / Introduction: The quality of life (QoL) of individuals with cystic fibrosis (CF) is a topic of increasing interest that might be associated with clinical and laboratory parameters indicative of disease severity. Methods: Prospective and crosssectional study that applied the validated Portuguese version of a CF-specific QoL questionnaire (CFQ-R) to clinically stable CF patients aged 14 to 21 years. The correlation between the scores on the CFQ-R domains and the values for the forced expiratory volume in one second (FEV1), the Shwachman-Kulczycki score (SKS), and body mass index (BMI) was assessed. The results in all the domains were compared between groups according to gender and to FEV1 above or below 60% of the predicted value. Results: The sample included 31 patients (11 females and 20 males); the average age was 16.4 ± 2.3 years old. The scores on the CFQ-R domains were quite variable, but their median value was approximately 80. A significant correlation was found between BMI and the CFQ-R domains weight (r=0.43, p=0.016) and eating (r=0.44, p=0.013); between FEV1 and the physical (r=0.53, p=0.002) and treatment burden (r=0.41, p=0.023) domains; and between the SKS and the physical (r=0.39, p=0.03), health perceptions (r=0.41, p=0.023), and role (r=0.37, p=0.041) domains. A significant difference was found between the participants with FEV1 above and below 60% of the predicted value on the domains role and health. No difference in the scores was found as a function of gender. Conclusions: The CFQ-R scores exhibited wide variability in all 12 domains; however, they denoted a relatively satisfactory QoL in most of the sample. Some CFQ-R domains exhibited significant correlations with clinical and functional parameters, indicating appropriate discriminatory power to assess the severity of CF

Predictors of Late Stage Melanoma Diagnosis: Adolescent and Young Adult Cancer Patients in Tennessee

Every year more than 72,000 adolescents and young adults (AYAs) in the United States (US) aged 15-39 years are diagnosed with cancer. AYAs represent a population that falls into a care gap between pediatric and adult medical services. Additionally, AYAs have experienced increased cancer incidence and decreased five-year survival rates compared to other age groups. The spectrum of tumors seen in AYAs differs from children and older adults, with 90% of the tumors stemming from ten cancer types. Melanoma of the skin, characterized by the uncontrolled growth of pigment-producing cells, is the third most common cancer diagnosed among AYAs in the US. Overexposure to ultraviolet (UV) radiation from sunlight or artificial sources is the greatest risk factor for melanoma. AYAs seem to be particularly at risk for developing melanoma due to increased UV exposure early in life. This study’s objectives were to understand the unique characteristics of melanoma in AYAs in Tennessee and identify the predictors of late- stage diagnosis. The sample for this study includes all incident melanoma cancer cases (N=1109) in AYAs from the Tennessee Cancer Registry (TCR) for the years 2004-2008, inclusive. AYA cases were defined as cancer cases that were diagnosed in individuals ages 15-39 years, inclusive. Melanoma cases were defined according to the International Classification of Diseases- Oncology (ICD-O-3) site codes C440-C449. Melanoma cases that had a specified stage at diagnosis were included for final analysis (N= 315). Stage of diagnosis was determined through the SEER Summary Stage 2000 variable and coded into in situ, localized, and combined regional & distant stage. Univariate and multivariate analyses were performed for the following predictor variables: insurance status (private insurance vs. other), age group (5- year groups), and sex (male vs. female). The majority of the sample was white (96.5%), female (63.8%), had private insurance (85%) and was diagnosed with localized stage melanoma (69.4%). Individuals with government insurance were eight times more likely to be diagnosed with late stage melanoma compared to individuals with private insurance (OR 8.4, CI 3.0-23.3, p < 0.01). AYAs in the 15-19 year old age group were six times more likely to be diagnosed with late stage melanoma compared to 35-39 year olds (OR 6.3, CI 1.7-22.9, p=0.01). Females were 57% less likely to be diagnosed with late stage melanoma compared to males (OR 0.53, CI 0.30-0.93, p < 0.05). These findings indicate that individuals with government insurance may not receive adequate melanoma screening and preventative care compared to individuals with private insurance. While females were less likely to be diagnosed with late stage melanoma, females have a much greater risk of being diagnosed with melanoma at any stage. Finally, the increased risk of late stage diagnosis in the 15-19 year old age group may be associated with greater UV exposure from indoor and outdoor tanning. These data suggest the need for targeted cancer awareness and control activities specific to AYAs. Future studies are needed to explore the variations in late stage diagnosis of melanoma in AYAs in Tennessee.

<p>Uppsatsen behandlar översättningen av de fem första kapitlen i <em>Rapt à Bamako</em>, som är ett autentiskt översättningsuppdrag och som kommer att publiceras av Bokförlaget Trasten under hösten 2010. En översättningsprincip för det aktuella uppdraget har bestämts utifrån översättningens förutsättningar, bestående av teoretisk bakgrund, måltextens syfte och kontext, genomgång av referenstexter samt en stilstudie av källtexten. Det framkommer att den kontextuella och lexikala nivån var de som gav upphov till mest svårigheter under översättningsarbetet, men att en stor del av de problematiska översättningsfrågorna kunde lösas med hjälp av översättningsprincipen i fråga.</p> / <p>The study deals with the translation of the five first chapters of <em>Rapt à Bamako, </em>an authentic translation assignment which will be published by Bokförlaget Trasten during fall of 2010. Based on relevant theoretical considerations, the purpose and context of the source text, a survey of parallel texts and finally a style analysis, a translation strategy for the task was formulated. The contextual and lexical levels of the translation turned out to be the most problematic ones, but the solution to the problem could often be found in the translation strategy.</p>

Hallo, Welt! Adolescent angst und das Erwachsenwerden in Marisha Pessls Special Topics in Calamity Physics und Zoe Jennys Das Blütenstaubzimmer

Special Topics in Calamity Physics (2006) by Marisha Pessl and Das Blütenstaubzimmer (1997) by Zoё Jenny both feature strong female characters who go through difficult times because they experience genuine disillusionment with regard to their friends, the opposite sex, and, especially, their family. The focus of this thesis was to analyze if the authors depict their characters in such a way that one can see correlations between the emotional behavior of these characters and a phenomenon that is often referred to as adolescent angst. The theoretical foundation for defining adolescent angst and for understanding mechanisms that trigger adolescent angst was provided by Rapoport and Ismond’s internationally appraised DSM-IV Training Guide for Diagnosis of Childhood Disorders (1996) and Reinherz et al.’s ground-breaking study on „Depressive Disorders“ (2006). In my thesis, I was able to show that the depictions of difficult relationships between the protagonists and their parents and friends show characteristics of adolescent angst. Contrary to that, positive influences like reliable friends who are understanding and lend support, function as motivational forces which decrease the protagonists’ anxieties and frustration in both texts. The analysis of the final scenes showed that after all hardships, the protagonists do not give up hope and open themselves up to a once unimaginable future. Although Pessl and Jenny dismiss the concept of a clear didaxis in their texts, they nevertheless imply a motivational message; namely that adolescent angst can be conquered and overcome. I was able to demonstrate that the concept of adolescent angst serves as a catalyst for the development of the protagonists in both Special Topics in Calamity Physics and Das Blütenstaubzimmer . The bestseller status of both novels underlines that the authors’ decision to conclude these novels which are centered around adolescent angst with an open ending seems to cater to a modern young adult readership, especially within the context of pop culture.
