Alcohol use by drug addicts during the treatment and follow-up / El consumo de alcohol durante el tratamiento y seguimiento con drogodependientes

This study is about the use of alcohol and its influence during the recuperation of drug addict patients undergoing treatment. Levels of the use of illegal drugs and alcohol were compared before, during and after the treatment of 82 patients assessed over a one-and-a-half year follow-up study. The results show that after the intervention there is an increase of alcohol use by 50% of the addicts. The 10-13% exhibit excessive levels. The higher alcohol use coincide withthe end of the treatment. This tendency changes later (18 months) into similar levels as the initialones. These results do not allow to consider the aforementioned increase as a confirmationof the substitution hypothesis, that is the alcohol use-redirection towards other more socially tolerated substances. / Se estudia la influencia del consumo de alcohol en la recuperación de drogadictos sometidos a tratamiento. Se relacionan los niveles de consumo de drogas ilegales y de alcohol de antes, durantey después del tratamiento en una muestra de 82 pacientes seguidos durante un año y medio. Se evidencia que tras la intervención aumenta el consumo de alcohol en el 50% de adictosy llega a niveles excesivos en un 10-13%. El mayor consumo coincide con la finalización del tratamiento. Posteriormente (18 meses) baja a niveles próximos a los iniciales. Estos resultados no permiten interpretar die ho aumento mediante la hipótesis de la sustitución, es decir, el desvío del consumo hacia sustancias más toleradas socialmente.

Denna kvalitativa studie har baserats på åtta vuxna barn som har vuxit upp i en alkoholmissbrukarfamilj. Syftet med uppsatsen är att förstå hur de vuxna barnen har påverkats av att växa upp i en familj där minst en av föräldrarna missbrukat alkohol samt hur relationen till andra personer påverkats. Syftet är också att se hur uppväxtvillkoren sett ut för dem.Huvudfrågan i denna studie är hur de har upplevt att de har påverkats. Huvudteorierna till denna uppsats är Thomas J. Scheffs teorier om sociala band, skam och stolthet. Utöver dessa teorier har även Pierre Bourdieus teorier om fält, de olika kapitalen och habitus också använts. Empirin har samlats in genom åtta individuella intervjuer. Resultatet visade att samtliga i studien påverkades negativt i barndomen av att ha dessa uppväxtförhållanden. På grund utav detta påverkar det dem än idag på både positivt och negativt. Slutsatserna av detta är att dessa individer fortfarande är i skuggan av alkoholen. / This qualitative study has been based on eight adult children who have grown up in an alcoholic family. The aim of this essay is to understand in what way the adult children have been affected by growing up in a family where at least one of the parents has abused alcohol and how the relationship with other people is affected. The aim is also to see what the conditions of growing up have looked like for them. The main issue in this study is how they experienced being affected. The main theories for this essay are Thomas J. Scheff's theories of social bonds, shame and pride. In addition to these theories, Pierre Bourdieu's theories of fields, different capital and habitus have also been used. The empirics has been collected through eight individual interviews. The results showed that everyone in the study is negatively affected in childhood by having these growth conditions. Because of this, it still affects them both positively and negatively. The conclusions made of this is that these individuals are in a way in the shadow of alcohol.

The target of this diploma paper is to describe the specific clients of the programmes for drug addicts - the group of women - mothers who have ever used drugs. To work with this target group, that means to work with the target group of their children too. These children are especially in the pre-school age. One of the institutions that provides care for these women is The Centre for Family, which is a part of The Centre for Prevention and Treatment of Drug Dependence DROP In. In this diploma paper we are going to introduce not only possibilities and methods in the former project "Integration of Family", but we are going to focus on the new project "The Sunflowers'Garden". This project is not only the aftercare for mothers, former drug addicts, but also brings possibility how to focus on the children'development. These children could be threaten by the drugs using during pregnancy and by the social surroundings too. We are going to deal with different target groups, with the group of addict women and former addict women and with the group of thein children. Hjälpsökande mäns sätt att se på och förstå sitt eget sexmissbruk / Help seeking men´s way of looking at and understanding their own sex addiction

Inledning: Överdrivna sexuella handlingar och de risker de kan medföra har beskrivits sedan antiken. Hur man sett på problemen har varierat beroende på vilket religiöst eller politiskt perspektiv man utgått ifrån. Man har ännu inte kunnat enas om vilken term som bäst beskriver problemen, vilket försvårar forskningen och skapar osäkerhet om hur problemen ska behandlas. Den term som allt oftare används bland allmänheten och i media är sexmissbruk. Allt fler söker hjälp för problem som de själva kallar sexmissbruk. De flesta som söker hjälp är män. Syfte och frågeställningar: Denna studie syftar till att öka kunskapen om hur hjälpsökande män ser på och förstår sitt sexmissbruk med följande frågeställningar: Hur beskriver männen sitt sexmissbruk? Hur upplever de sexmissbruket? Hur förklarar männen sitt sexmissbruk? Metod: En kvalitativ metod med fenomenologisk och deskriptiv ansats har använts. Fem vuxna män, som själva uppgett att de har ett sexmissbruk, har intervjuats. Männen går i psykodynamisk psykoterapi på en mottagning där man kan söka hjälp för sexuella problem. De transkriberade intervjuerna har analyserats med hjälp av kvalitativ innehållsanalys. Resultat: Studiens resultat presenteras under fyra kategorier och ett flertal underkategorier. De fyra kategorierna omfattar männens faktiska beskrivning av sexmissbruket, olika typer av insikt i problemen, sexmissbrukets konsekvenser – individuella och sociala, samt männens egna förklaringar till sitt sexmissbruk. Ett övergripande tema, som löper som en röd tråd genom samtliga kategorier, tolkas som att leva med skam. Diskussion: Männens beskrivning av sitt sexmissbruk stämmer väl överens med hur man tidigare definierat problemen. Missbruket omfattar olika handlingar och har konsekvenser för männen själva, deras nära relationer och sociala liv. Studien visar att män med sexmissbruk för en kamp med sin skam. De har olika grader av skam och olika försvar mot skam. Skammen beskrivs som en form av "motor" i missbruksprocessen, vilket överensstämmer med tidigare forskningsresultat. Männen uppger att förklaringar till problem med sexmissbruk finns i deras uppväxtförhållanden och tidigare erfarenheter. De uttrycker behov av hjälp för att kunna ta sig ur missbruket. För att kunna hjälpa män med sexmissbruk behöver man som psykoterapeut ha mod att möta skammen, både den hjälpsökandes och den egna.

海洛因成癮者的決策判斷與抑制控制缺損探討 / The impairment of decision making and inhibitory control in heroin addicts

本研究目的主要在探討海洛因成癮者是否具有決策判斷的問題與抑制控制能力缺損,利用數理認知模型的參數值檢驗受試者在新式Go/No-Go作業的表現,該作業結合IGT與原先Go/No-Go作業的特性,並將作業刺激改為中性與海洛因相關的圖片,探討海洛因成癮者在此作業的決策歷程與抑制反應功能,並與控制組作比較。另者,根據誘發─敏感化理論 ( Robinson & Berridge, 1993 ) 與誘發─習慣化模式 ( Di Chiara, 2000 ) 的預測,前者認為成癮越嚴重者的抑制缺損情形會越嚴重,對藥物相關刺激的注意力偏誤也越明顯;後者則認為藥物依賴階段的初期會發生較嚴重的注意力偏誤情形,但隨著依賴程度增加而對藥物相關刺激已習慣化,尋求藥物行為也已變成自動化反應,故注意力偏誤的情形反而會減少。因此,本研究試圖在加入藥物相關刺激的Go/No-Go作業中驗證這兩種理論的預測。 本研究以48位一般控制組、88位海洛因受戒治人為樣本,利用Go/No-Go作業與線索依賴學習模式 ( CD model ) 分析受試者對酬賞或處罰的注意力 ( w參數 )、學習率 ( 參數 )、反應一致性 ( c參數 )、以及海洛因圖片價值的影響 ( I_Heroin參數 ),結果發現,比起控制組,海洛因成癮者對酬賞的注意力較低、受到藥物相關刺激影響較明顯。另外,將戒治組分為物質高依賴組與低依賴組時,比起控制組,高依賴組對酬賞的注意力較低;而低依賴組受到海洛因圖片價值影響較明顯。結果證實當面對同時具有中性與藥物相關刺激時的情況下,海洛因成癮者與過去古柯鹼成癮者在決策作業上的表現不同,不僅受到酬賞或處罰的影響,藥物相關刺激本身亦帶有某種價值,影響成癮者的決策與抑制能力;另外,結果部分支持誘發─習慣化的預測。 / The purpose of present study was to investigate the decision making and inhibitory problem in heroin addicts, and to use the mathematics cognitive model parameters to examine performance in a new Go/No-Go Task of subjects. The task combined the characteristics of IGT and original Go/No-Go Task. We changed the task’s stimuli to neural and heroin related pictures, to explore the decision making and inhibitory reactive function in the task, and compared with control group. On the other hands, according to Incentive-sensitization theory (Robinson & Berridge, 1993) and ‘Incentive ─ habit’ model (Di Chiara, 2000), the incentive-sensitization thoery thought that people with increasing severity of addiction get more inhibitory problems and attentional bias toward drug related cues, but the incentive-habit model suggests that attentional bias for drug cues will be more severe in the early stage of drug dependence., but with the increase in dependence and habit to drug cues, drug-seeking behavior has become automatic reaction, so attentional bias will be attenuated when abusers get more dependence. Therefore, this study attempted to verify the prediction of two theories in the Go / No-Go Task with drug-related stimuli. This research was based on the data collected from the 48 normals and 88 heroin addicts, using Go/No-Go Task and Cue-dependence learning model ( CD model ) analysis their attention to reward or punishment ( parameter w ), learning rate ( ), reactive consistence ( c ), and valence of heroin pictures ( I_Heroin). The result revealed that compare to controls, heroin addicts had fewer attention to rewards, and more obvious influenced by drug related stimuli. Otherwise, to divided addicts group into high dependence group and low dependence group, compared to control group, the high dependence addicts had lower attention to reward, and the lower dependence addicts was higher influenced by drug related stimuli. It confirm that when heroin abusers faced to neural and drug related stimili, their performance in decision making task was different from cocaine abusers, it’s not only influenced from reward or punishment, but also from the valence of drug related cues. In addition, the result partially support the prediction of the incentive-habit model.

This is a survey based on the theoretical and methodological assumptions of Critical Theory of the Frankfurt School, from critical conception of non-formal education in the public health field. The research problem was developed by the question: How Body conceptions of, health and happiness are built socio-historically and what are their possible relationship with the formation of the subject drug addict The research technique used was the hermeneutic perspective with document analysis, from the theoretical framework. The general objective of this research was to understand the historical development of body categories, health and happiness, as well as other social mediations in the process of formation of the subject drug addict. Already the specific objectives were: to list, from the thought of Plato and Descartes, the elements for understanding the development of body categories, health and happiness; exhibit in dialogue with Foucault the history and development of health policies, their relationship with psychiatry and forms of care in the health field in the early modern era, reflect the Frankfurtian critical theory about the development of category instrumental reason and erudition human linked to the formation of the subject drug addict; understand Freud malaise in civilization, the specifics of the subject drug addict with psychoanalysis olivensteiniana prohibitionist drug policy, list the health care forms constituted as opposed to asylum-asylum policy in the twentieth century and XXI and the educational possibilities arising therefrom that may contribute to the emancipation of the subject drug addict by adorniana. A research critical theory originated from the therapeutic-pedagogical experience in mental health services in Goiânia, as well as the need to research, study and reflection of a field still little theoretical production. Therefore, we analyzed the documents that regulate the teaching process of Physical Education in CAPS-AD, aiming to present possible solutions to the construction of a critical education project in the public health service which can contribute to the emancipation of the subject drug addict. We conclude that, despite the limitations and contradictions, there is a possibility of therapeutic-critical pedagogical intervention in the field of public health which could help to the emancipation of the subject drug addict in the way of criticality. / Este trabalho está vinculado à linha de pesquisa “Cultura e Processos Educacionais”. Trata-se de uma pesquisa realizada com base nos pressupostos teórico-metodológicosda Teoria Crítica da Escola de Frankfurt, a partir de concepção crítica de educação não-formal no campo da saúde pública.O problema de pesquisa foi desenvolvido pela seguinte pergunta: Como as concepções de corpo, saúde e felicidade são construídas sócio-historicamente e quais são suas possíveis relações com a formação do sujeito toxicômano?Atécnica de pesquisa utilizada foi a perspectiva hermenêutica com análise documental, a partir do referencial teórico. Para tanto, analisamos os documentos que normatizam os processos pedagógicos da Educação Física nos CAPS-AD, objetivando apresentar possíveis saídas para a construção de um projeto de educação crítica neste serviço de saúde pública que possa contribuir para a emancipação do sujeito toxicômano. Conclui-se que, apesar dos limites e contradições, existe a possibilidade de intervenção terapêutico-pedagógica crítica no campo da saúde pública que pode corroborar para aemancipação do sujeito toxicômano no caminho da criticidade.

Este estudo tem por objetivo descrever a estrutura global de entrevistas de primeira vez entre uma psicóloga e usuários de álcool e/ou outras drogas, tomando como recurso os movimentos interacionais e as ações sequenciais neles realizadas, a partir do instrumental teórico-analítico da Análise da Conversa Etnometodológica, com foco nos conceitos de organização seqüencial e de agenda conversacional. O trabalho explora e problematiza o uso da ficha, espécie de roteiro disponibilizado pela instituição, e da agenda da profissional no processo investigativo em curso. Os dados indicaram que a ficha apresenta-se como instrumento de avaliação limitado. A psicóloga, então, necessita ampliar as questões e utilizar recursos sobressalentes para suprir a falta na execução da tarefa proposta. A análise revelou que a disposição dos movimentos interacionais orienta-se fortemente para a avaliação, permitindo-se que se visualize a agenda deste encontro. As entrevistas organizam-se em sete movimentos interacionais: identificar o usuário de álcool e outras drogas, investigar o histórico familiar, constituir a dinâmica da drogadição, etc. A pesquisa é de natureza qualitativa e colaborativa e destina-se a contribuir para a reflexão a respeito da prática profissional psicológica na área da saúde / This study seeks to describe the overall structure of a group of ten first-time interviews between a pshycologist and an alcohol and/or other drugs users, through both the interactional movements and the sequentional actions taken in each movement, based on the theorethical and analytical tools comprised in the Ethnomethodological Conversation Analysis, with a focus on the concepts of sequentional organization and conversational agenda. The results of this study showed the patients file was presented as a limited instrument of assessment. Then, it is necessary for psychologist to enlarge the issues and use all spare resources to solve the difficult in performing the proposal task. Jag är inte bara en knarkare : En litteraturstudie om upplevelsen av kontroll och stigma vid substitutionsbehandling vid opioidberoende

Background: Opioid dependence is a chronic disease. Substitution treatment for opioid addiction involves a lot of rules and control that can lead to stigmatization. Purpose: To describe the experience of control and stigma in people with substitution treatment during opioid dependence. Method: A literature review with qualitative content analysis inspired by Graneheim & Lundman (2004). The study is based on 14 scientific articles that were applied mainly through the databases Cinahl and Pubmed. Result: Two main themes with eight sub-themes emerged. The most prominent in the result was that people with substitution treatment feel stamped as a drug addict. They can also feel suspicious and less credible. Having substitution treatment is associated with shame. One does not always dare to tell the surroundings that one has treatment. Within the framework of treatment, they feel controlled and supervised. Conclusion: People with substitution treatment experience stigma in terms of discrimination, preconceived opinions and reduced credibility. Stigma arises in several different contexts, such as work, friends and family, but also in health care and even in substitution treatment. This is because this form of treatment includes a number of rules and controls that make you feel controlled and supervised. This risks contributing to further stigmatization.

Denna uppsats skall genom granskningar av domstolsprotokoll, och dess beslutsgrundande motiveringar, undersöka hur tvångsingripande åtgärder vid missbruk enligt LVM-lagstiftningen tillämpas. Intentionen är försöka finna de underliggande och uppbyggda formuleringar som rättfärdigar den svenska staten, att via socialtjänst och förvaltningsrätter frihetsberöva och tvångsvårda medborgare mot den enskildes vilja. Är det inte den fria medborgarens rättighet att leva sitt liv som den önskar även om det anses som ett destruktivt leverne? Empirin är av mig uppbyggd utifrån de diskurser som blottläggs vid undersökningen. Syftet med sammanställningen av de olika diskurserna och begreppen som förekommer i dessa domstolsbeslut är att ge insyn i en verksamhet som med stor säkerhet går allmänheten förbi. Detta kan förklaras med att detta rör en sådan i periferin existerande grupp av människor.
