Bendruomenės slaugytojų teikiamų slaugos paslaugų namuose ypatumai / Patients’ opinion about community nursing services provided at home

Darbo tikslas - įvertinti pacientų nuomonę apie bendruomenės slaugytojos teikiamas slaugos paslaugas namuose. Tyrimo metodika. Darbe naudoti kiekybiniai tyrimo metodai. Buvo atliekama anketinė apklausa Kauno rajono šeimos sveikatos klinikoje, kurioje dalyvavo 131 šioje įstaigoje besilankančių pacientų. Tyrimo duomenų statistinei analizei buvo naudotas SPSS 17.0 for Windows duomenų apdorojimo paketas. Rezultatai: Dažniausiai pacientai nurodo, jog jų būklė yra gera arba patenkinama. Apytiksliai trečdaliui pacientų yra nustatytas neįgalumas, iš jų daugiau nei pusei nustatytas didžiausias darbingumo lygis. 31,3% tiriamųjų nurodė, jog jiems kyla sunkumų atvykti į gydymo įstaigą, o dažniausios to priežastys – ilgos eilės pas gydytoją (39%), per silpna sveikata (22%) bei sunkumai lipti laiptais (22%). Nustatyta, jog dažniausiai namuose pacientus slaugo šeimos nariai (100%) ir tik ketvirtadalį – bendruomenės slaugytojai. Medicinines paslaugas namuose dažniausiai teikia gydytojas, o net trečdalis tiriamųjų iš viso nekviečia jokio medicinos darbuotojo į namus. Tyrimu nustatyta, jog tik 12,2% tiriamųjų per pastaruosius 2 metus naudojosi bendruomenės slaugytojo paslaugomis namuose. Jiems dažniausiai buvo suteiktos tokios paslaugos: pulso skaičiavimas, kraujo spaudimo matavimas, kraujo ėmimas tyrimams, vaistų peržiūra ir bendravimas. Visas jiems suteiktas paslaugas pacientai vertina labai gerai arba gerai, jų kokybę vertina gerai. Buvo pastebėta, jog nuo asmenų įgalumo/neįgalumo... [toliau žr. visą tekstą] / The research aimed to assess patients’ opinion about community nursing services provided at home. Methodology applied in the investigation. The research used quantitative methods. A survey was carried out at a Family Health Clinic in Kaunas district. 131 subjects attending the clinic participated in the research and answered a questionnaire. The results of the study were statistically analyzed using software SPSS 17.0 for Windows package. Results: In most cases the patients indicated that their health state was good or satisfactory. Approximately one third of the patients were disabled and more than half of them possessed the maximum work capacity. 31,3% of the subjects stated that they face difficulties with the arrival to the clinic. The most common causes were as following: long lines at doctor’s office (39%), a weak health (22%) and difficulties to climb the stairs (22%). It was determined that most of the patients were generally nursed by family members (100%) and only one fourth - by community nurses. Moreover, a doctor usually provided medical services at home and one third of the subjects did not invite any medical officers to their homes. The research revealed that only 12,2% of the subjects had used the community nursing services during the past two years. They received the following services: pulse rate measurement, blood sampling tests, drug monitoring and communication. The provided services were rated as very good and good. Local moorings, international visions : fabricating internationalised practices in Australian higher education

Over the last two decades, Australian higher education has undergone dramatic changes in purpose and orientation. Changes in public funding arrangements and concomitant policy statements have contributed to the reconceptualisation of Australian higher education, and internationalisation has become a core goal for Australian universities. In light of these dynamics, this study examined understandings of internationalisation within two Australian universities. The study examined the ways in which internationalisation was understood by university staff working in either a teaching capacity and/or a managerial position. Situated within the broad field of critical sociology, the study drew on critical realism (Bhaskar, 1979, 1989), critical epistemology (Carspecken, 1996) and reflexive sociology (Bourdieu, 1972, 1990) to analyse how the universities and their staff positioned themselves in relation to the goal of internationalisation. Furthermore, the study examined how this goal served to reposition the institution and/or various forms of university work. The insights of critical social theory were used to examine the contested power relations associated with the growing importance attributed to the goal of internationalisation in Australian higher education. The significance of the study resides in its recognition of the ways in which academic and non-academic subcultures within the university contribute to the goal of internationalisation. Whereas previous research viewed divergence of understandings as weakening the commitment given to internationalisation as an institutional goal, this study has shown that such diversity stems from the differential encounters with and experiences of internationalisation. Moreover, in previous research, the pre-determined objectives for internationalisation resulted in the compartmentalisation of this goal, as in economic objectives and academic objectives. In contrast, this study focused on the dynamic and evolving nature of internationalisation in higher education. Consequently, the study's contribution lies in its explanation of the long term benefits to be derived from viewing internationalisation as a dynamic and generative phenomenon, rather than simply as a pre-determined goal. A case study approach was used in this research with two contrasting onshore Australian universities selected as the case sites. One institution had a reputation as an elite, research intensive university. The other was a post-Dawkins university with a strong vocational orientation. At each site, semi-structured interviews were conducted with staff from across the university's hierarchy. Interviewees included the Pro Vice-Chancellor for the Office of Internationalisation, the Chair of the Academic Board, the Director of the Teaching and Learning Support Unit, Faculty Deans, Heads of Departments, as well as departmental staff concerned with first year teaching. Departmental staff were drawn from two disciplinary areas, Australian History and Marketing. Interviews engaged participants in discussion about the processes by which internationalisation was enacted. Furthermore, university documents, such as the Strategic Plan, were analysed in terms of how the given institution constructed the need for internationalisation and the means by which this goal was to be achieved. The study found that internationalisation involves and promotes constant adaptability. The two institutions used whatever resources they had to develop and promote their international aspirations. The international visions of the institutions were influenced by both their historical and intended relationship with the broader higher education world. The elite, research intensive institution viewed internationalisation with becoming a university of international standing. This institution used its bureaucratic and hierarchical nature to advance its objectives for internationalisation. The vocationally oriented university had developed an internationalisation policy with a view to maximising the revenue to be derived from its diverse international activities and to gaining greater prestige within the higher education field. Staff involved with managerial and/or teaching work were found to develop their ideas about internationalisation through a combination of personal and professional experiences. The study confirmed the growing trend for academics to assume managerial roles in addition to their teaching and research. Consequently, accounts of internationalisation were not necessarily confined to a purely managerial or an academic perspective. Furthermore, the accounts of internationalisation differed between and within the two selected disciplines. On the one hand, specific disciplinary attributes could be seen by the academics interviewed as inherently international, even though they may not assist in realising institutional objectives for internationalisation. On the other, academics spoke of the perceived need for the frames of reference used in undergraduate education to be broadened, given the globalised nature of contemporary society and/or the increasing international enrolments. The study concluded that internationalisation is an important means by which the localised priorities of an institution, an academic department, and/or of individuals can engage with forms of global mobility. Moreover, the study argued the need for all staff and students in Australian higher education to see themselves as part of the processes of internationalisation. This latter point raises questions about the personal and professional attributes required of academics when working within internationalised Australian universities.

Development of a triaxial cell for estimating the horizontal in stress conditions edomÃtricas / Desenvolvimento de uma cÃlula triaxial para estimativa da tensÃo horizontal em condiÃÃes edomÃtricas

This thesis presents the development of a triaxial cell and a proposal for methodology for estimating horizontal tension edometric condition by conducting laboratory tests on samples unsaturated compacted soil in OMC. The development of triaxial cell permits the use of voltages or controlled deformation. Moreover, the machine allows independent application increments of axial and radial stresses, thereby facilitating implementation assay in any way triaxial stresses. The proposed methodology simplifies estimate lateral tension through comparison of axial stress-strain curves of the test edomÃtrico conventional triaxial and consolidation tests under different paths tensions with the constant relationship between the axial and radial increments. In this case, the value of K0 would be corresponding to the constant trajectory in which the triaxial test curve edomÃtrico overlaps the curve of the test in terms of stress and axial deformations. They were tests performed with different soil types and the results were- roughly consistent with estimates from correlations empirical, thus allowing validate the proposal presented in this research. also It was studied the change of horizontal stress during the collapse flood conditions in edomÃtricas. In these tests it was found that during the breakdown voltage can horizontal remain virtually constant or even decreasing. / Esta dissertaÃÃo apresenta o desenvolvimento de uma cÃlula triaxial e uma proposta de metodologia para estimativa de tensÃo horizontal em condiÃÃo edomÃtrica, atravÃs da realizaÃÃo de ensaios de laboratÃrio em amostras nÃo saturadas de solo compactadas na umidade Ãtima. O desenvolvimento da cÃlula triaxial possibilita a aplicaÃÃo de tensÃes ou deformaÃÃes controladas. AlÃm disso, o equipamento permite a aplicaÃÃo independente de incrementos de tensÃes axiais e radiais, facilitando dessa forma a execuÃÃo de ensaio triaxial sob qualquer caminho de tensÃes. A metodologia proposta simplifica a estimativa da tensÃo lateral atravÃs da comparaÃÃo das curvas tensÃo-deformaÃÃo axial entre o ensaio edomÃtrico convencional e ensaios de adensamento triaxiais sob diferentes caminhos de tensÃes com a relaÃÃo constante entre os incrementos axial e radial. Nesse caso, o valor de K0 seria o correspondente à trajetÃria constante na qual a curva do ensaio triaxial se sobrepÃe a curva do ensaio edomÃtrico em termos de tensÃes e deformaÃÃes axiais. Foram realizados ensaios com diferentes tipos de solo e os resultados mostraram-se aproximadamente concordantes com as estimativas realizadas a partir de correlaÃÃes empÃricas, permitindo dessa forma validar a proposta apresentada nessa pesquisa. TambÃm foi estudada a variaÃÃo da tensÃo horizontal durante o colapso por inundaÃÃo em condiÃÃes edomÃtricas. Nesses ensaios verificou-se que, durante o colapso, a tensÃo horizontal pode permanecer praticamente constante ou atà mesmo diminuir.

O estudo foi dividido em duas etapas, com duração de 15 dias cada: na etapa 1, os participantes foram divididos em dois grupos (G1 e G2), onde cada acadêmico recebeu escova e creme dentais. Na etapa 2, o G1 passou a se chamar experimental e o G2, controle: o experimental recebeu escova e creme dentais, um frasco borrifador com solução básica aquosa, composta de água destilada e edulcorante estévia, acrescido de digluconato de clorexidina a 0,12%. No grupo controle, cada participante recebeu o mesmo conjunto e o frasco borrifador continha apenas a solução básica. Receberam um protocolo de orientação para utilização, desinfecção e armazenamento da escova dental. Antes de iniciar a pesquisa e após as duas etapas, os acadêmicos responderam a questionários com dados sobre seus hábitos de higiene e armazenamento das escovas dentais. As escovas dentais foram devolvidas em envelopes lacrados ao pesquisador para a análise microbiológica, realizada no Laboratório de Microbiologia do Instituto de Ciências Biológicas da UFJF. As escovas dentais foram inseridas em tubos de ensaio estéreis identificados, contendo 7mL de solução fisiológica esterilizada a 0,9% e agitados no Vortex®, por 20s. Com o auxilio de uma alça calibrada foi coletado 1μL desta solução e semeado por esgotamento no meio de cultura CHROMagar Orientation®. As placas foram incubadas a 35,5°C, em aerobiose por 24h. Os dados foram submetidos a testes estatísticos. Os resultados deste estudo mostraram que ocorreu um maior índice de contaminação das escovas dentais dos alunos do 2º período em relação às dos alunos do 5º e do 9º períodos. Houve uma redução acentuada nos índices de contaminação na etapa 2, sendo estatisticamente significante para os 2º e 9º períodos. No 5º período ocorreu uma redução, porém sem significância estatistica, que pode ser explicado pelo relato da ocorrência de contato de suas escovas dentais com outras escovas no local de armazenamento. Entre os grupos, a redução da contaminação foi estatisticamente significante entre G1 e o experimental. Entretanto, uma redução foi observada entre o G2 e o controle, demonstrando que hábitos de higiene e armazenamento adequados são suficientes para reduzir a contaminação das escovas dentais. / This study evaluated the level of contamination of toothbrushes from 54 students of 2nd, 5nd and 9nd period of the undergraduate course in Dentistry, Federal University of Juiz de Fora, Juiz de Fora, Brazil, with respect to hygiene and storage, and the effectiveness of the aqueous solution of chlorhexidine gluconate 0.12% spray for decontamination of toothbrushes. The study was divided into two phases, with duration of 15 days each. In Step 1, participants were divided into two groups (G1 and G2), where each received a toothbrush and a dental cream. In step 2, the G1 has been called experimental and G2, control: in experimental received toothbrushes and dental cream, a spray bottle with basic aqueous solution, composed of distilled water and sweetener stevia added with chlorhexidine gluconate 0.12%. In the control, each participant received the same set and spray bottle containing basic solution. Received protocol guidelines for use, storage and disinfection of toothbrushes. Before starting the study and after two steps, the students answered questionnaires with data on their hygiene and storage of the toothbrushes. The toothbrushes were returned in sealed envelopes to the researcher for the microbiological test, performed at the Laboratory of Microbiology, Institute of Biological Sciences of UFJF. The toothbrushes were placed in sterile test tubes identified, containing 7mL of sterile saline 0.9% and agitated in the Vortex®, for 20s. With a calibrated loop was collected 1μL this solution and spreaded by exhaustion in the culture medium CHROMagar Orientation®. The plates were incubated at 35,5°C under aerobic conditions, for 24h. Data were subjected to statistics. The results of this study showed that an increased rate of contamination of toothbrushes of 2nd period students compared to the students of the 5th and the 9th periods. There was a marked reduction in contamination levels in step 2 and was statistically significant for 2nd and 9th periods. In the 5th period was a reduction, but without statistical significance, which can be explained by the reported occurrence of contact for their toothbrushes with other brushes in the storage location. Between groups, the reduction in contamination was statistically significant between G1 and experimental. However, a decrease was observed between G2 and the control, showing that hygiene and proper storage are sufficient to reduce the contamination of toothbrushes.

Cities are becoming bigger, and questions about accessibility to the urban areas is a concern. Difficulties in finding affordable accommodation is one, and another one is wellbeing in homes. With the recent interests in the increasing tiny house movement, living solutions that are affordable, simple, and small are gaining in popularity. This is happening partly as a reaction to that the average home size in many parts of the western world have been increasing dramatically in the last decades. Both building regulation institutions and research are often connecting small space living with negative effects on wellbeing. But the tiny house movement seem to show that people can live well also in small dwellings outside of such regulations. The question of what brings wellbeing to a homes has never been more relevant, and the area of small and unconventional housing is lacking research. Research question: What is wellbeing living in small and unconventional homes? Purpose: The purpose of this study is to increase the understanding of wellbeing in small and unconventional homes. The objective of this research is to provide a holistic understanding of wellbeing in such homes, by going beyond firstspace and secondspace dimensions, into a thirdspace perspective. Method: Semi-structured, face-to-face interviews and observations of people chosing to live in small and unconventional dwellings. A thematical analysis strategy suitable for new concept development was adopted. An abductive approach was applied in order to frame the study being multi disciplinary, and in order to obtain increased understandings of the study phenomenon. However, the study focused heavily on the empirical data from my study. Conclusion: This study found that the wellbeing is experienced as a totality of different dimensions: not only does the physical dwelling in itself provide for wellbeing, but also dimensions relating to thoughts, meanings and lived experiences they associate to their dwellings. A holistic perspective is what best can provide an understanding of their experienced wellbeing, where physical, mental and lived dimensions are combined. Furthermore was found that the dwellings can work as facilitators to achieve wellbeing on several levels both relating to their inner space in their dwellings, bringing in other spatialities, and for their lives as a whole. Professionellas tankar och erfarenheter om autism i spåren av covid-19 : En kvalitativ studie / Professionals’ thoughts and experiences about autism in the wake of covid-19 : A qualitative study

This study is a qualitative study on professionals' thoughts and experiences about autism in the wake of covid-19. To investigate this, three people who are professional psychologists in different fields with experience on people with autism participated through individual interviews via Zoom of between 25 to 55 minutes.         The results of the study are based on the material from these interviews that were reworked from audio file to transcription, which in turn was analyzed with thematic analysis carried out in a six-step process. The thematic analysis resulted in five themes: introverted/extrovert human autism, need for clarity when unclear, reduced risk of being identified as odd – the digital rescue, support and actions and  post-pandemic response and evaluation.          Conclusions that can be drawn from the results of the study are that as a result of the restrictions on social distance, people with autism and neurotypical people suffer approximately equally based on whether the person is socially interested or not. People with autism during the covid-19 pandemic may have gotten it in part better, those who previously asked for distance learning or opportunities to work remotely from their own home have now been given the opportunity. It is important that people with autism receive the support they need during the pandemic from, for example, school health teams. After the pandemic, evaluations of what worked and not during the pandemic for people with autism need to be carried out. / Föreliggande undersökning är en kvalitativ studie om professionellas tankar och erfarenheter om autism i spåren av covid-19. För att undersöka detta deltog tre personer som är yrkesverksamma psykologer inom olika områden med erfarenhet om personer med autism genom enskilda intervjuer via Zoom på  mellan 25 till 55 minuter.     Resultatet i studien är grundat på materialet från dessa intervjuer som bearbetades om från ljudfil till transkribering som i sin tur analyserades med tematisk analys som genomförs i en sexstegsprocess. Den tematiska analysen resulterade i fem teman: den introverta/extroverta mänskliga autismen, behov av tydlighet när det är otydligt, minskad riska att bli utpekad som udda – den digitala räddningen, stöd och insatser och utvärdering efter pandemin.     Slutsatser som är möjliga att dra utifrån studiens resultat är att till följd av restriktionerna om social distansering drabbas personer med autism och neurotypiska personer ungefär likvärdigt utifrån om personen är socialt intresserad eller ej. Personer med autism under pandemin med covid-19 kan ha fått det delvis bättre, dom som tidigare efterfrågat distansstudier eller möjligheter att få arbeta på distans från det egna hemmet nu har getts möjligheten. Det är viktigt att personerna med autism får det stöd de behöver under pandemin från till exempel skolhälsoteam. Efter pandemin behöver det ske utvärderingar om vad som fungerat och inte under pandemin för personer med autism.

Detta är en kvalitativ intervjustudie med syfte att undersöka mellanstadielärarnas uppfattningar och erfarenheter kring att samtala med elever om missförhållanden hemma,samt deras upplevelser av hur samverkan med socialtjänsten fungerar. Data samlades in med semistrukturerade intervjuer och respondenterna bestod av sex lärare på mellanstadiet i från olika kommuner i Mellansverige. Resultatet visade att samtliga lärare anser att det är viktigt att diskutera missförhållanden med eleverna i skolan. Det som kan upplevas som hinder för att lyfta ämnet i skolan är att lärare saknar kunskap om ämnet, det saknas resurser och en del respondenter tyckte att det var ett svårt ämne att prata om. Resultatet visar även att skolans samverkan med socialtjänsten behöver utvecklas och förbättras. Studien visar att det behövs mer kunskap och resurser för att informera barn i årskurs 4-6 om missförhållanden i hemmet på ett mer strukturerat och funktionellt sätt. / This is a qualitative interview study with the aim of examining the middle school teachers' perceptions and experiences of talking to pupils about child maltreatment at home, as well as their experiences of how collaboration with the social services works. Data was collected with semi-structured interviews and the respondents consisted of six middle school teachers from different municipalities in Central Sweden. The results showed that all teachers considered it important to discuss child maltreatment with pupils in school. What could be perceived as an obstacle to raise the subject in school was that teachers lack knowledge about the subject, there is a lack of resources and some respondents thought it was a difficult subject to talk about. The results also show that the school's collaboration with the social services needs to be developed and improved. The study shows that more knowledge and more resources are needed to be able to inform children in grades 4-6 about maltreatmentat home in a more structured and functional way.

A raíz de la pandemia del Covid-19, si uno desea acudir a una cita médica, debe tomar las medidas necesarias para evitar ser contagiado, ya que en los centro médicos se presentan negligencias de las autoridades por no hacer efectivo el protocolo de bioseguridad, por lo que, los pacientes van a sus citas médicas de manera presencial con la gran angustia de ser infectados y llevar el virus a sus hogares. Es así como nace MediGo con el objetivo de brindar un servicio de asistencia médica mediante un aplicativo móvil que permitirá al paciente realizar sus consultas desde la comodidad de su hogar con una flexibilidad de horarios para reservar citas médicas, así como la facilidad de integrarse a una comunidad de personas con sus mismos intereses. Para validar el modelo de negocio propuesto, se realizaron diferentes experimentos que abarcó el diseño de prototipos de la app tanto para usuarios y personal médico, entrevistas a expertos, programadores, entre otros. Asimismo, se utilizó el método Concierge para validar los ingresos del proyecto a través de las redes sociales. Por otro lado, se elaboraron los planes de marketing, finanzas y recursos humanos, lo que nos permitió proyectar el negocio y determinar su rentabilidad a lo largo del tiempo. Finalmente, todo lo mencionado anteriormente nos indica que MediGo es un negocio viable, muy atractivo e innovador en el rubro de medicina, que da respuesta a una necesidad real y actual. / In the wake of the Covid-19 pandemic, if you want to go to a medical appointment, you must take the necessary measures to avoid getting, since in medical centers there can be negligence by the authorities for not enforcing the biosafety protocols, therefore, patients go to their medical appointments in person with great concern of getting infected and taking the virus home. El presente proyecto consiste en la creación de una app que nace en medio de una coyuntura mundial llamada COVID-19, la cual ha puesto a muchas empresas y a personas naturales bajo distintas condiciones incómodas, y para muchos, poco viables. Muchas organizaciones han tenido que adaptarse a estos cambios mayormente referidos a saneamiento y protocolos de inocuidad y otras han aprovechado estos tiempos de crisis para implementar nuevos proyectos que faciliten la vida de las personas en estos momentos difíciles y puedan llegar a encontrar una solución dentro de tanta incertidumbre. Por ello, se crea Doctor Wasi, una aplicación que se encarga de buscar médicos cercanos a tu domicilio, por medio del GPS del Smartphone. El mercado objetivo que se eligió fueron niños hasta los doce años y personas de la tercera edad, pues son el segmento más golpeado por el covid 19 actualmente y se quiso apoyar a esa clase de personas, no obstante, cualquier persona puede descargar la app y disfrutar de la experiencia de Doctor Wasi. Esta aplicación, tendrá tres tipos de especialistas, que son los médicos certificados, las enfermeras y los especialistas en terapia física, los cuales el paciente podrá escoger y contactar por medio de su teléfono celular. Asimismo, se estará en constante innovación de la app y de todas nuestras plataformas online con la ayuda de un experto en el rubro para que el cliente pueda llevarse la mejor experiencia cada vez que solicite el servicio de Doctor Wasi. / This project consists of the creation of an app that was born in the middle of a global situation called COVID-19, which has put many companies and individuals under different uncomfortable conditions, and for many, not very viable.
