A comprehensive clinical and neuroimaging approach of sex differences in crack cocaine use disorder

Vieira, Breno Sanvicente 07 March 2018 (has links)

The use of the drug relates to social and economic issues for society and poses serious health problems, including early death. Crack cocaine use disorder (CUD) is the medical condition which refers the pathological use of the drug. CUD relates to several negative outcomes such as higher rates of HIV and HCV infections, familiar problems and crime involvement, in addition to a higher prevalence of concurrent mental disorders. Scientific agendas promote evidence-based studies as a need for better therapeutics. In this regard, some gaps in the field require attention. In this line, distinct factors confer vulnerability for crack cocaine use in males and females: more males use the drug (a 3:1 proportion), but females show a faster transition from initial drug use to CUD. The course of the disease also show differences; females report a higher craving for the drug, while males have more frequent involvement with violent crimes. Thus, scientific commitments highlight a calling for the integration of those biopsychosocial models that consider individual characteristics in addition to those who consider addictive disorders as ?brain diseases.? A more consistent interdisciplinary integration of knowledge from classical theories in combination with advances provided for technologic methods is a promising route. Hence, the aim of this doctoral thesis was to investigate sex differences in crack cocaine users. To address the main objective, the thesis has two studies with groups of participants diagnosed with CUD and hospitalized for drug detoxification. These two groups were one of males (CK-M) and a second of females (CK-F). Study 1 had as its objective to get a picture of sex differences in the psychosocial profile. Study 2 had as its objective the identification of sex differences in brain functioning level. Study 1 had 798 CK-M and 546 CK-F. Results consistently revealed CK-M as having a more severe alcohol use history and higher rates of concurrent alcohol use disorder than CK-F. On the other hand, CK-F showed an earlier crack cocaine use onset, higher drug use severity, and more familiar and work problems along with a higher prevalence for lifetime mental disorders. Particularly, CK-F showed higher rates for trauma and stress. Study 2 had a sample of 80 participants: CK-M (n = 20), CK-F (n = 20), a group of males (HC-M, n = 20), and another of healthy female controls (HC-F, n = 20). Participants did a resting-state functional magnetic resonance imaging (rs-fMRI) scan. The method makes it possible to investigate temporal associations between nonspatially related brain areas by using as a measure fluctuations in the blood oxygen-level dependent (BOLD) level. It is an indirect measure of energy consumption, and by testing those correlations, functional connectivity (FC) can be investigated. Results supported CK-M as having an overall higher intra- and internetwork FC, while CK-F showed an overall lower FC in this regard. Taking both studies, the conclusions of this thesis point toward the existence of sex differences in all biopsychosocial domains. Thus, the interpretation of studies in crack cocaine use, particularly those testing interventions, need to resemble the possible existence of sex differences. Therefore, a hope from studies like this is that sex-specific models for crack cocaine use and CUD emerge and become tested. Similarly, possible interventions, also need to be aware of such backgrounds and consider possible sex differences when developing interventions, researches and public health policies as well.

This argument can be applied to naturalism in gen-eral, but there seems to be a limitation for it to be used on Mill's utilitarianism. We think it is possible to demonstrate that he had, in his connection between justice and utility, a valuable method for the establishment of rules and principles that work like a standard for the normativity of laws; yet it does not offer a definition of ?good in natu-ral terms?. Facts and values need to be justified by a set of normative theories. Jus-tice is dependent of the coexistence between rules and principles, so it has a rela-tionship of value with the utilitarianism (as a fact). This shows us that justice?s protec-tive rules are worth making, if taking into account features that are required by many different social contexts, which provide valuable facts for the implementation and grounding of rules and principles. The letters between Mill and Comte show us to what degree the Londoner subscribed the naturalism to the positivism and the rea-sons why he chose an idea of stability, both from a qualitative and a quantitative point of view of a political theory. Therefore, he defined a very factual based democ-racy concept, a criteria of the modern liberalism, and the aim of ought-is for possible corrections. The unrestricted freedom of thought and discussion is another feature that triggers the prescriptive and descriptive elements of Mill's utilitarianism, intimate-ly connected to the representative democracy. The normative justification in this con-text, does not operate by the epistemological aspect, but by the political sense of public justification, addressed to the moral community, fallibilist and, therefore, adaptable. Let us suggest that Moore pointed out a special naturalistic fallacy de-manding a "proof" of the utilitarianism principle by the summum bonum analysis. We will try to demonstrate that such proof is not feasible and that Mill, who knew about it, chose the political way to establish a ground criterion for the normativity. Rawls, on the other hand, subscribed Mill's liberalism, because he thought it is valid theory to justify fair and free institutions. Both Mill and Rawls present compromising liberal el-ements in their theories, regarding fact-valuable features.

By the analisys of these mechanisms, it is possible to underestand the organic nature of the fascist movement in Brazil. This reaserch aims to investigate youth as one of the specific segments of the fascist movement, based on the periodic press. It seeks to underestand how the organization of the youngsters was used as an indoctrination instrument by AIB, with the objective of founding the Integralist Youth and, at the same time, molding the future followers of the movement. In order to do so, this dissertation presents a qualitative analisys by sampling, revealing how youth instrumentalization was done. The theoretical reference selected to support this analisys was the political culture. This reaserch intends to examine the discourse delivered by the official Integralist written press, in order to expose the mobilization mechanisms, used by the movement to engage young people. The investigation will explore four periodicals of great expression for the movement: the newspapers A Offensiva and Monitor Integralista and the magazines Anau?! and Brasil Feminino. In addition, a series of documents tha marked out important issues about youth in the movement were also examined. Regarding to conceptual aspects, the research proposes a reflection based on the youth sociology: youth is defined beyond age range, but based in symbols and representations, which are redefined throughout history.

The system is composed by questionnaires for the purpose of obtaining quantitative data on psychotherapeutic treatment in order to evaluate and monitor its outcome (Erekson, Lambert & Eggett, 2015). The results obtained through the application of the questionnaires in the context of psychotherapy helped to verify if the intervention is effective so that, through this feedback, the psychotherapist can prevent withdrawal, modify the therapeutic plan and provide feedback to the patient and the team in order to discuss the clinical case based on quantitative evidence (Nordal, 2012). Among the several questionnaires in the OQ System, the Youth Outcome Questionnaire Self-Report 2.0 (Y-OQ-SR 2.0) was developed to evaluate the psychotherapy of teenagers between 12 and 18 years old (Wells & Burlingame, 2003). The Y-OQ SR 2.0 has 64 items consisting of affirmative first-person phrases and shall be answered on a Likert scale from 0 to 4: 0 (never), 1 (rarely), 2 (sometimes), 3 (often ) and 4 (always). The items are distributed into six factors that evaluate: Intrapersonal Stress; Somatic; Interpersonal Relationships; Critical Items; Social problems; Behavioral Disorders. The questionnaire is non-theoretical and self-applied. Goal: culturally adapt the Y-OQ-SR 2.0 to the Brazilian reality. Method: the study followed the following steps: 1) From the original instrument, two translations into Portuguese were performed by two bilingual translators; 2) Synthesis of the two translations by a third translator overseen by an expert committee; 3) Evaluation of the target audience in order to investigate the understanding of the items; 4) Adjusting the instrument after a consensus among the ideas resulting from the evaluation of the target audience; 5) Back-translation of the Portuguese version into English, by a fourth bilingual translator; 6) Submission of the back-translation to the original authors to ensure that the content has equivalent meaning to the original content; 7) The original authors considered the back-translation appropriate, the Portuguese translation will be used to carry out a pilot study (Borsa et al., 2012; Cassepp-Borges, Balbinotti & Teodoro, 2010; Gjersing, Caplehorn & Clausen, 2010; Takara, 2015). Outcomes: Some terms and words that needed clarification were evaluated by a committee of experts before the synthesis itself. The translation was made by two bilingual translators using the Parallel Blind Technique. The synthesis of the translations was evaluated by the target audience in order to ensure the understanding of the translated items. Five out of these items required review by the committee for a second audience assessment, followed by semantic adjustments. This second evaluation took place satisfactorily, as the target audience understood the phrases and modified words. The final version was submitted to the reverse translation process (Portuguese to English) by a C2 level translator in the Common European Framework of Reference (CEFR). Finally, the final version was sent to the original authors in order to ensure the equivalence of content with the original version. The final approval of the Brazilian-Portuguese version of the instrument was satisfactory; in other words, no adjustments were required. A committee of experts comprehending adolescent clinical psychologists with fluency in English employed the procedures for operational equivalence and it was concluded that the Y-OQ-SR 2.0 is suitable for the pilot study. Discussion: The evaluation of the target audience was made through stratified focus groups. The group provided discussions about the items to be evaluated, providing both objective data, that were filled by adolescents, and subjective data related to participants' understanding and discussion on terms and words. Once the steps proposed by Borsa et al., 2012, Cassepp-Borges, Balbinotti & Teodoro, 2010, Gjersing, Caplehorn & Clausen, 2010 and Takara, 2015 were successfully achieved, the proposed goal was therefore achieved. The gathering and analysis of data were performed through a rigorous and systematic process to ensure adequate adaptation to the Brazilian reality. The Y-OQ-SR 2.0 is an important tool for adolescent and patient psychotherapists, as it was developed exclusively to assess youth psychotherapy. Its purpose is, therefore, to monitor the psychotherapeutic process (Lambert, 2010) and provide feedback to find out if the psychotherapy is being effective (Lambert, Hansen & Harmon, 2010). The adaptation of the Y-OQ-SR 2.0 to the Brazilian reality will allow the instrument to be released for subsequent studies of evidence of validity. / O Outcome Questionnaire System (OQ System) ? um sistema desenvolvido pelos psic?logos americanos Michael Lambert e Gary Burlingame no in?cio da d?cada de 90 e, atualmente, ? reconhecido no Substance Abuse and Mental Health Administration`s National Registry of Evidence-based Programs and Practices (SAMHSA`sNREPP).

After the Napoleonic Wars, Spain and Portugal were reinserted into the international system as peripheral forces in European policy. Without the same diplomatic density of previous times it was up to Portuguese diplomacy to build arrangements that were able to offer some role of participation in the reorganization of the European order. From the study of diplomatic correspondence between the two courts and their diplomats it was possible to realize that while Spain directed its efforts to strength Bourbon legitimacy in the Italian territories, the Portuguese diplomacy was concentrated on the restitution of the territory of Olivenza, taken by the Spaniards in the war of 1801. For different reasons and agendas, now structured, however improvised, the issues concerning South America, especially the Missoes Orientais do Urugua and the Banda Oriental as a whole have entered transversely the agendas of Vienna. We first seek to characterize these two philosophical views and present their main arguments. In a second moment, we examine two conditions Kane?s takes as necessary for free will, namely alternative possibilities and ultimate responsibility. In the third chapter we explore specifically the theory of Robert Kane, mainly through the analysis of his work The Significance of Free Will. In the last chapter we discuss and comment on some of the main criticisms of the philosopher's theory. Throughout this work we analyze indeterminism from different perspectives in order to better understand the problems that constitute the greatest challenge to the intelligibility of incompatibilist free will. In the end, we conclude that there is still a way to go in order to respond satisfactorily to some criticisms but it seems too early or too strong to claim that Kane's freedom is not coherent or intelligible. We observe the same bibliographical root in all the analyzed works, making the reference to a dichotomous Aristotelian wisdom hinder the incorporation of a concept with both theoretical and practical properties of this knowledge. Another relevant question is the signalization of wisdom as a meta-cognitive virtue, which would denote incorporating, together with new theories, a way of establishing an analytical project associated with ethical presuppositions. We find, in Heraclitus's philosophy, a wisdom of epistemic- ethical imbrication, evidenced even as an instrumental good. From these peculiarities, we selected and interpreted 37 fragments of Heraclitus' work that allowed us to establish new criteria as guides of an integral and dynamic wisdom. From this perspective, we proposed the Founding Theory of Wisdom (FTW), establishing four necessary conditions, or basic principles, to obtain and re-actualize human wisdom, considering that S is wise only if: (i) S is aware of human nature, and motivated to constantly overcome ignorance; (ii) S is committed to an innovative knowledge (at a time t, and in relation to the available resources to access this knowledge); (iii) S masters their cognitive activity through the process of critical understanding; (iv) S structures their intellectual virtues through intellectual sensitivity (IS), and adjusts their moral virtues through harmonious moral guidance (HMG), taking place in its distinctive character.

Kant argues that morality is the principle of universal reason, capable of limiting human impulses, since, by virtue of natural needs. The moral conscience in this circumstance is a product of pure practical reason, which enables the application of theory to practice in function of the freedom of the transcendental subject to think and act. However, the categorical imperative is the command of moral actions, which is found in the autonomous subject, through the relation between goodwill and freedom. And dignity is a quality that naturally finite beings have as moral beings. The dignity of the human is translated in his capacity to act according to the representation of laws determined by himself, so that man should not be subject to any other purpose than that of his own reason, namely that of his own morality, which implies the fulfillment of "duty for duty. Faced with this idea, the human person is himself the measure and source of duty (Solen). Because reason knows only what it has projected itself. Whereas the pure "I" of the person is only possible when there is recognition that he is a being of duties, right and autonomous. Kantian formality starts with the distinction between morality and legality in order to allow coexistence between them. It is also in this position that the concept of law has its roots. For the author, the relationship between ethics and law is a relationship of subordination.

I investigate the fundamental elements of a deliberative theory of social normativity set forth by J?rgen Habermas and argue that, in a project of deliberative normative justification, not only consensus but also dissensus has normative capacity. I present a minimal formulation of Habermas? normative project, which encompasses the concepts of discourse, consensus and public sphere, and then proceed to distinguish three different realms in a deliberative theory of social normativity ? politics, the political, and the dispute for politicity ? approaching the nature and the role of dissensus in each one. I argue that (a) politics is a realm of the institutions of liberal democracy, where dissensus takes the form of conflict among groups and models of public spheres, having the role of keeping the public sphere multiple and inclusive; (b) the political is the realm of interaction of discourses about the imperative of human coexistence in the world, where dissensus takes the form of adversariness and has the role of maintaining the disagreement necessary for pluralism; and (c) the dispute for politicity is the pre-discursive realm where struggles for the capacity to issue political claims take place, where dissensus has the role of pointing out the unjustified partiality of the discursive genre as the only one capable of conveying social interactions with normative potential.
