Flow diversion for compressive unruptured internal carotid artery aneurysms with neuro-ophthalmological symptoms: a systematic review and meta-analysis

Background: Data on the safety and efficacy of flow diverters (FD) for the treatment of unruptured internal carotid artery (ICA) aneurysms with compressive neuro-ophthalmological symptoms (NOS) are scarce and comprise mainly small case series. Methods: We performed a search of three databases and included series with ≥10 patients, with unruptured aneurysms of the ICA and NOS, treated with FD. Random-effects analysis of treatment results and safety was performed. Results: A total of 22 studies reporting on 594 patients were included. Pooled proportions of NOS recovery, improvement, transient and permanent worsening were: 47.4% (95% CI 35.0% to 60.1%); 74.5% (95% CI 67.9% to 80.2%); 7.1% (95% CI 3.3% to 14.7%); and 4.9% (95% CI 3.2% to 7.4%), respectively. Rates of complete recovery and improvement in patients with isolated visual symptoms were 30.6% (95% CI 12.5% to 57.7%) and 56.6% (95% CI 42.3% to 69.9%). Isolated oculomotor symptoms recovered completely in 47.8% (95% CI 29.9% to 66.3%) and improved in 78% (95% CI 69.2% to 84.9%). Morbidity occurred in 5% (95% CI 2.8% to 9%) and mortality in 3.9% (95% CI 2% to 7.5%) of patients. An increased likelihood of symptom improvement was observed when treatment was performed early (<1 month) after symptom onset (OR=11.22, 95% CI 3.9% to 32.5%). Conclusion: Flow diversion promotes recovery or improvement of compressive symptoms in a large proportion of patients but is associated with significant rates of morbidity and mortality. Transient and permanent NOS worsening is not uncommon. Early treatment is of utmost importance, as it increases the likelihood of symptom improvement more than 10-fold.

The application of family group conferencing as a child justice intervention in South Africa

Abstract in English, Zulu and Sotho / The aim of this study is to consider the application of family group conferencing (FGC) as child justice intervention for children in conflict with the law. An exploratory qualitative approach was followed to provide insight into the perceptions of experts with regards to the suitability of FGC for South Africa and for children, as well as the current application of, and potential context and scope for the application of FGC as a South African justice intervention. Semi-structured interviews were used as the data collection tool. Accordingly, experts from diverse disciplines, to wit social work, psychology, criminology, law, and education (and sub-speciality in FGC and restorative justice), were interviewed telephonically, face to face or via video chat. The sample was collected using purposive sampling through perusal of research articles and academic electronic sites, as well as snowball sampling whereby potential participants were identified. The findings show that FGC is an ideal intervention within the South African criminal justice context, and more particularly for child justice. The inclusion of family was noted as specifically important in dealing with children, and within an Afrocentric, Ubuntu-laden, socio-cultural environment. Furthermore, the suitability of FGC to the South African child justice context was linked to victim support and inclusivity, reconciliation aims, cultural flexibility, and ability to support offenders yet hold them accountable without criminalisation. Findings reiterated that FGC is inclusive of support persons, whilst Victim-Offender Mediation (VOM) is not. Resultantly, and with due consideration for the importance of support persons in child related interventions, the necessity of an amendment to the Child Justice Act 75 of 2008 (CJA) - pertaining specifically to VOM - is advocated. Provisions for the implementation of FGC in a child appropriate manner were recommended, namely, adequate preparation; age, needs, and context consideration; maintenance of safety and respect of all parties; and adequate facilitation. The findings regarding the current application of FGC as a child justice intervention in South Africa evinced that it is rarely implemented due to a lack of funding and resources, hesitancy of criminal justice professionals, and a decrease in diversion referrals due to a lack of knowledge and poor performance of the South African Police Services (SAPS). With the aim of extending and promoting FGC, findings noted the necessity for awareness and educational campaigns for both community and criminal justice persons; a multi-dimensional approach whereby various disciplines, societal sectors, government, and civil society work collaboratively; and lastly the importance of the launch of South African Restorative Justice Accreditation Board (SARJAB), an accreditation board for restorative justice practitioners was noted. A therapeutic programme for the rehabilitation of youth offenders

The judicial system is moving away from a retributive justice system to a more rehabilitative, restorative justice system. Diverting youth offenders from the justice system by alternative sentencing options such as diversion programmes is one way of doing this. Jt was found that a need exists for the development of a therapeutic programme for the rehabilitation of youth offenders in South Africa. The purpose of the study was to detennine the content of therapeutic rehabilitation programme and subsequently to develop a provisional therapeutic programme for youth offenders. The first part of the literature study summarized a number of theories used to explain the incidence of youth offence. The second part of the literature study discussed various risk factors that could lead to youth offence. Thirdly, the juvenile justice system in South Africa was discussed. Finally, the characteristics of effective rehabilitation programmes were discussed and a number of international and South African rehabilitation programmes were evaluated in tenns ofthese characteristics. In the literature study certain aspects for inclusion in a rehabilitation programme were identified. A list of topics that should be addressed by a rehabilitation programme was drawn up. A provisional therapeutic programme was compiled using the information gained from the literature study and the needs for a programme as identified by staff members at the facility where the programme was to be conducted. The programme was based on experiential learning. lt included a number of activities that addressed the topics identified by the literature study. The provisional programme was evaluated in tenns of its short-term effectiveness and adjustments to the programme were made. The adjusted programme was conducted at two other facilities and evaluated in tenns of its short-term effectiveness by the researcher, the staff at these facilities and the youth who took part in the programme. Staff at the two facilities where the empirical study took place were asked to complete questionnaires to identify their needs with regards to a rehabilitation programme as well as background questionnaires on the youth who were taking part in the programme. A forensic criminological perspective on the adjudication of children in South Africa

In this project the various International Instruments, namely the United Nations Convention of the Rights of the Child, 1989, the Beijing Rules and the African Charter, relating to the protection of the rights of children were discussed focusing on the rights of children in conflict with the law. The important guidelines regarding the establishment of a minimum age for criminal capacity, detention, legal representation, diversion, sentencing, pre-sentence reports, child justice Courts and the confidentiality of children&#8217;s Court hearings were highlighted. The current positions in South African law with regard to these issues were discussed and the proposed provisions in the Child Justice Bill, 49 of 2002 were furnished. The research included all the magistrates in Gauteng that were in service during February 2003 and March 2003. Throughout the project the important role that forensic criminologists can and should play in a juvenile justice system was highlighted. / Criminology / M.A.

The importance of regional economic integration in Africa

Motivation of virtually all regional economic integration (REI) initiatives has been prospect of enhanced economic growth. Although REI's role in contributing to growth and development was recognised and acknowledged, its importance in Africa has never been properly outlined. Theoretical background, economic assumptions and evidence of REI are examined to bring out REI's importance to Africa. Depicting from these, benefits and challenges of REI in Africa are explored. This dissertation analyses the progress, pace, approach, sequence of REI in Africa looking at different variables. Africa's regional integration blueprint and institutional framework are compared to EU's but selected areas are identified as essential for Africa. Progress on REI has been found to be slow. This study concludes that REI should be viewed as one aspect of strategy towards Africa's development and growth. Regional economic integration and economic development in Southern Africa

The impetus for regional integration draws its rationale from the standard international trade theory, which states that free trade is beneficial to all. Free trade among two or more countries or preferential trade will improve the welfare of the member countries as long as the arrangement leads to a net trade creation in the Vinerian sense. The history of regional economic integration in Southern Africa (SADC) reveals that it has not yet achieved the economic benefits that are attributable to developing regions, namely: higher levels of welfare exemplified by low poverty levels, economic development and industrialisation. Regional economic integration in Southern Africa is constrained by high tariff and non-tariff barriers, archaic infrastructures and multiple memberships among different regional economic communities. A SADC-wide customs union can be successful, provided that countries are allowed to join, when their economies have adjusted and the South African Customs Union (SACU) is used as a nucleus. / Economics / M. 多邊主義下帶動之「全球化」及區域主義下所帶動之「區域化」皆已成為現階段國際經貿發展的兩股重要潮流,透過洽簽自由貿易協定所達到之區域經濟整合,在區域內可消除絕大部份之關稅及非關稅貿易障礙,預計將有助於增進會員國間之貿易量,形成貿易創造效果,而或對於非會員國造成貿易轉向效果。故其將影響國際貿易的流向,國際資源的重分配,進而改變國際分工型態。 為瞭解區域經濟整合對於貿易流量的影響,本文首先將就區域經濟整合之定義與法源、區域經濟整合發展與現況、及台灣與各主要區域經濟整合之貿易概況進行概述。鑒於引力模型可有效的分析雙邊的貿易流量,本文針對該模型之相關文獻及實證歸納與整理,並利用引力模型,選擇全球六個重要區域經濟組織包括北美自由貿易區、歐盟、東協自由貿易區、南方共同市場、安地略集團及紐澳緊密關係協定等區域共計37個國家,以設定虛擬變數之方式實證1990年至2003年長達13年間各區域經濟整合的過程中,其對於區內會員國及對非屬會員國進、出口貿易流量之影響,特別是整合過程中對台灣是否產生貿易轉向之現象進行實證研究,並分析影響貿易流量之因素。 本實證研究發現影響二國間貿易流量的因素分別為出、進口國之國民生產毛額及人均國民生產毛額,以及距離。區域經濟整合具有明顯的貿易創造效果,而貿易轉向效果則隨經濟整合的緊密程度、區域內產業分工型態及與區域外國家出、進口結構及產業間相互競合程度不一而產生不同的影響。在上述六個區域經濟整合中,北美自由貿易區及南方共同市場對台灣產生貿易轉向的現象。本研究亦針對上述之結果與區域經濟整合實際進展現況進行交互分析。最後,本研究將就政策面及後續研究的方向提出建議,俾供參考。 / Globalization and regionalization have been the two main trends in international economic development in recent years. Already, many countries have signed Free Trade Agreements to achieve the goal of regional economic integration. This integration allows members of the region to virtually eliminate all tariff and non-tariff trade barriers and, as a result, increase trade volume between member countries. This phenomenon is called trade creation. On the other hand, trade volume between members of the region and non-member may decrease, a phenomenon called trade diversion. All of this will affect international trade flows, the allocation of international resources and change the pattern of the international industrial division. In order to learn how regional economic integration influences trade flows, this paper deals first with the definition and regulation under the GATT of regional trade agreements, the present situation and the development of regional economic integration. We also examine the current state of import and export trade between Taiwan and the main regional economies in the world. This paper reviews the relevant literature on the gravity model, a model that can effectively analyze trade flows between two countries, then proceeds to use this model with NAFTA, EU, AFTA, MERCOSUR, ANDEAN Community, and CER (Closer Economic Relationship between the Australia and New Zealand) as regional dummies. This allows us to test alterations in trade flows between members and other members as well as between members and non-members (especially Taiwan) in these six regional trade blocs for the 13 years between 1990 and 2003. This model also analyzes the main factors affecting trade flows. We find first that the main factors influencing trade flows between two countries are the GDP and per capita GDP of the importer and exporter as well as distance. Secondly, while regional economic integration clearly brings with it trade creation, the degree of trade diversion is affected by such factors as the degree of regional economic integration, the pattern of industrial division in the region, the structure of imports from and exports to non-member countries and the level of competition and cooperation in various sectors. The empirical evidence shows that NAFTA and Mercosur have resulted in trade diversion away from Taiwan. Based on our findings, we offer policy suggestions and suggestions for further research.

Le 1er avril 2003, l’entrée en vigueur de la Loi sur le système de justice pénale pour les adolescents (LSJPA) a fourni aux policiers canadiens de nouveaux outils pour procéder à des interventions non-judiciaires auprès d’adolescents contrevenants. Dorénavant, les policiers détiennent le pouvoir officiel d’imposer des mesures extrajudiciaires aux adolescents interpellés pour avoir commis des infractions plutôt que de procéder à leur arrestation formelle. La présente étude vise à déterminer quelles caractéristiques des adolescents et circonstances des infractions ont un impact significatif sur les décisions des policiers d’imposer ces mesures plutôt que de procéder à des arrestations. Les résultats sont basés sur trois échantillons de participations criminelles juvéniles enregistrées par un corps policier canadien entre 2003 et 2010: le premier composé d’infractions contre la personne (n= 3 482), le second, d’infractions contre la propriété (n= 8 230) et le troisième, d’autres crimes (n= 1 974). L’analyse de régression logistique multiniveaux a été utilisée pour déterminer les facteurs — tels que le sexe, l’âge et les contacts antérieurs avec la justice des adolescents ainsi que la localisation dans le temps et l’espace de l’infraction — ont un impact significatif sur le pouvoir discrétionnaire des policiers. Certains facteurs ont une influence universelle d’une catégorie de crime à l’autre, tandis que d’autres ont un impact spécifique selon le type d’infraction commise. / Since April 3rd 2003, the Youth Criminal Justice Act (YCJA) provides police officers with new tools to divert canadian juvenile offenders. Specifically, police officers can officially impose extrajudicial measures to juvenile delinquents instead of handing them directly to the justice system. The current study seeks to determine which characteristics of the offenders and circumstances of the offences significantly impact the decisions of officers to divert cases instead of sending them to the formal justice system. The results are based on three samples of juvenile offence participations recorded by a Canadian police force between 2003 and 2010: the first composed of violent offences (n= 3,482), the second, of proprety offences (n= 8,230) and the third, of other offences (n= 1,974). Multilevel logistic regression analyses were conducted to determine which factors - for example, gender, age, and criminal record of the offender as well as location in time and space of the offence - have a significant impact on police decision-making for each category of offence. Principles for Diverting and Merging Viscous Flows : Evaluation and Visualisation / Principer för Delning och Sammnföring av Viskösa Flöden : Utvärdering och Visualisering

In the chemistry branch of science is often a chromatograph usedto separate and purify substances in a solution. In achromatograph, different detectors are used to analyse the samplesconstituents. Some detectors destroy the sample during analysis.Because it is undesirable to destroy the entire flow of sample, asmall mass is transferred to a second flow by a flow splitter. Inthis report, four principles to divert a small mass from one flowto another are developed and evaluated. The basic principles todivert the flows is tested and principal mock-ups are designed andmanufactured. A brief survey of the market is conducted and aproblem related to flow spitting is investigated. The problem isthe influence on yield through a chromatograph due to flowretaining. Designing and testing of different retaining systems isalso included in this report. All four initial principles proved to be plausible splittingtechniques. However, only two principles appeared to be feasiblefor direct implementation in devices comparable withchromatographs. One of the less feasible principle is covered byseveral patents. The other is difficult to manufacture in order tomeet the strict requirements associated with e.g. chromatographs.The testing of different retaining systems showed that smallertube inner diameter and how the tube is winded can reduce theretaining system influence on the yield significantly. The splitting techniques in this report are all feasible splittingtechniques, and the report can be used as a solid foundation fordevelopment future laboratory instruments. / Inom kemin förekommer ofta kromatografer för att rena och separera kemiska substanser iett prov. De separerade substanserna kan detekteras och analyseras av olika detektorer, därvissa detektorer förstör provet i analysprocessen. Då det oftast är önskvärt att bevara såmycket som möjligt av ett prov, för vidare studier, avleds därför en liten mängd av deseparerade substanserna till detektorn då en förstörande detektor används. I denna studieutvecklades och undersöktes fyra principer för att avleda mycket små volymer från ett flödetill ett annat. Flödesdelning och sammanförning med de grundläggande flödesdelningsprincipernatestades, principiella modeller konstruerades och tillverkades. Enmarknadsundersökning samt en undersökning av ett flödesdelningsassocierat probleminkluderas också i rapporten. Problemet består i minskning av utbyte på grund avbandbreddning i flödesfördröjningssystem. Bandbreddning studerades således i olika formerav flödesfördröjningssystem. Alla fyra principer visade sig vara möjliga flödesdelningstekniker varav två av sådan karaktäratt de skulle kunna realiseras i ett instrument såsom en kromatograf. Av de två andra omfattasden ena av flertalet patent och den andra skulle vara tillverkningstekniks svår att förverkligamed vedertagna tillverkningstekniker. Tester på olika typer avflödesfördröjningssystemutformningar visade att slangdimension och hur slangen är lindadhar stor inverkan på bandbreddning. Det återstår en hel del konceptuell utveckling då principerna bara behandlats pågrundläggande nivå. Genom de principiella modellerna ges en bild av hur principerna skullekunna förverkligas, men vidare ställningstaganden måste tas till bland annat materialval samtmonitorering och anpassning av flöden. Rapporten ger en god grund för framtidaproduktutveckling.

Diese Dissertation untersucht ökonomische Sanktionen im Kontext der empirischen politischen Ökonomie. Obwohl sie aus drei unabhängigen Kapiteln besteht, ist das übergeordnete, verbindende Ziel dieser Forschungsarbeit ein Gesamtverständnis der Motivation und der Effekte von Sanktionen anzubieten, getragen von der generellen Idee der Wechselwirkungen zwischen ökonomischen Anreizen und politischen Zielen. Meine Forschung zeichnet die ökonomischen Restriktionen ab, mit denen sich die politischen Entscheidungsträger im Bereich der internationalen Beziehungen auseinandersetzen. / This dissertation explores economic sanctions in an empirical political economy context. While consisting of three independent papers, it aims at providing a holistic understanding of the motivation and effects of sanctions in particular, and the interplay between economic incentives and political goals in general. My research delineates the economic constraints that policymakers encounter in the field of international relations.
