Quantitative Analysis of Hedgehog Gradient Formation Using an Inducible Expression System: a Dissertation

The Hedgehog (Hh) family of proteins are secreted growth factors that play an essential role in the embryonic development of all organisms and the main components in the pathway are conserved from insects to humans. These proteins affect patterning and morphogenesis of multiple tissues. Therefore, mutations in the Hh pathway can result in a wide range of developmental defects and oncogenic diseases. Because the main components in the pathway are conserved from insects to humans, Drosophilahas been shown to provide a genetically tractable system to gain insight into the processes that Hh is involved in. In this study, the roles of Hh cholesterol modification and endocytosis during gradient fonnation are explored in the Drosophila larval wing imaginal disc. To exclude the possibility of looking at a redistribution of preexisting Hh instead of Hh movement, a spatially and temporally regulated system has been developed to induce Hh expression. Functional Hh-GFP with and without the cholesterol-modification was expressed in a wild-type or shi-tslendocytosis mutant background. The Gal80 system was used to temporally express (pulse) the Hh-GFP transgenes to look at the rate of Hh gradient formation over time and determine whether this process was affected by cholesterol modification and/or endocytosis. Hh with and without cholesterol were both largely detected in punctate structures and the spreading of the different forms of Hh was quantified by measuring distances of these particles from the expressing cells. Hh without cholesterol showed a greater range of distribution, but a lower percentage of particles near the source. Loss of endocytosis blocked formation of intracellular Hh particles, but did not dramatically alter its movement to target cells. Staining for Hh, its receptor Ptc and cortical actin revealed that these punctate structures could be classified into four types of Hh containing particles: cytoplasmic with and without Ptc, and cell surface with and without Ptc. Cholesterol is specifically required for the formation of cytoplasmic particles lacking Ptc. While previous studies have shown discrepancies in the localization of Hh following a block in endocytosis, Hh with and without cholesterol is detected at both apical and basolateral surfaces, but not at basal surfaces. In the absence of cholesterol and endocytosis, Hh particles can be observed in the extracellular space. Through three-dimensional reconstruction and quantitative analysis, this study concludes that the cholesterol modification is required to restrict Hh movement. In addition, the cholesterol modification promotes Ptc-independent internalization. This study also observes that Dynamin-dependent endocytosis is necessary for internalization but does not play an essential role in Hh distribution. The data in this thesis supports the model in which Hh movement occurs via planar diffusion.

Cleft Lip and/or Palate in Infants Prenatally Exposed to Opioids

Objective: To determine the prevalence and odds ratios for cleft lip and/or palate (CL/P) among infants prenatally exposed to opioids with or without neonatal opioid withdrawal syndrome (NOWS). Design: This study represents an exploratory, retrospective cohort study design of newborn medical health records from 2011 to 2016. Setting: Records were drawn from a regional health system located in South Central Appalachia. Population and study sample: The original population yielded 3 cohorts of infants: (1) infants with opioid exposure (OE) but not requiring pharmacological intervention (OE; N = 168); (2) infants with NOWS requiring pharmacological intervention (N = 294); and (3) infants with no opioid exposure (NOE; N = 16 090), the primary comparison group. Main outcome: Infants in the NOWS and OE groups showed significantly increased prevalence and odds ratios for CL/P when compared to those in the NOE group. Results: Prevalence rates per 1000 live births for infants with OE (35.71) and infants with NOWS (6.80) were significantly higher than those for infants with NOE (1.37). Comparison of infants with OE to the NOE group revealed significantly increased odds for CL/P, isolated cleft palate (CP), cleft lip (CL), and cleft lip and palate (CLP) (27.05, 41.81, 19.26, 19.37, respectively; all Ps < .008). The odds ratios for infants with NOWS compared to the NOE group were significantly higher for CL/P and CP (5.00 and 10.98, respectively; Ps < .03) but not for CL and CLP. Conclusion: The results provide additional evidence that prenatal OE should be considered among the critical environmental risk factors that can contribute to CL/P.

Ontogeny of Adenosine Deaminase in the Mouse Decidua and Placenta: Immunolocalization and Embryo Transfer Studies

This study has determined the cellular site of adenosine deaminase (ADA) expression in the mouse during development from Days 5 through 13 (day vaginal plug was found = Day 0) of gestation. Developmental expression of ADA progressed in two overlapping phases defined genetically (maternal vs. embryonal) and according to region (decidual vs. placental). In the first phase, ADA enzyme activity increased almost 200-fold in the antimesometrial region (decidua capsularis + giant trophoblast cells) from Days 6 through 9 of gestation but remained low in the mesometrial region. Immunohistochemical staining revealed a major localization of ADA to the secondary decidua. In the second phase, ADA activity increased several-fold in the placenta (labyrinth + basal zones) from Days 9 through 13 of gestation but remained low in the embryo proper. Immunohistochemical staining revealed a major localization of ADA to secondary giant cells, spongiotrophoblast, and labyrinthine trophoblast. Regression of decidua capsularis and growth of the spongiotrophoblast population accounted for an antimesometrial to placental shift in both ADA enzyme activity and a 40-kDa immunoreactive protein band. To verify a shift from maternal to fetal expression, studies were performed with two strains of mice (ICR, Eday) homozygous for a different ADA isozyme (ADA-A, ADA-B). Blastocysts homozygous for Adab were transferred to the uterus of pseudopregnant female recipients homozygous for Adaa. The isozymic pattern in chimeric embryo-decidual units analyzed at Days 7, 9, 11, and 13 revealed a predominance of maternal-encoded enzyme at Days 7 through 11 of gestation and a shift to fetal-encoded enzyme by Day 13. Thus, maternal expression of ADA in the antimesometrial decidua may play a role during establishment of the embryo in the uterine environment, whereas fetal expression of ADA in the trophoblast might be important to placentation.

Thyroid Hormone Metabolism in the Non-Euthyroid Porcine Fetus

<p>Thyroid hormone is essential for regulating adult metabolism and proper fetal development. Under normal conditions, maternal and fetal thyroid hormones are subject to metabolism at the placenta and within fetal tissues through deiodination and sulfation to regulate fetal exposure to the bioactive hormone. Disruptions of the thyroid hormone system can result in non-thyroidal illness syndrome (NTIS), which is classified as the dysregulation of thyroid hormone homeostasis. The exact cause of the alterations in circulating thyroid hormone levels during NTIS is not well- known. In comparison, hypothyroidism results from the absence of thyroid hormone production and presents as low thyroid hormone levels.</p> <p>Porcine reproductive and respiratory syndrome virus (PRRSV) crosses the late gestation placenta and causes suppression of circulating maternal and fetal thyroid hormone. Chapter 2 investigates the potential role of thyroid hormone metabolism in this disruption. Pregnant gilts were challenged with PRRSV2 (n=22) or sham inoculated (n=5) at gestation day 85. Samples were collected on day 106, and viral load was assessed in fetal serum and thymus. From the entire fetal population, three distinct subsets of fetuses representing biological extremes were identified, including uninfected with no detectable viral load (UNIF), high viral load viable (HV-VIA), or high viral load with severe meconium staining (HV-MEC). In addition, control fetuses from sham inoculated gilts (CON) were used as a reference group. Samples of fetal liver, kidney, and the corresponding fetal placenta and maternal endometrium for n=10 fetuses per group were then used to evaluate gene expression. A total of 11 genes associated with thyroid hormone metabolism including deiodinases (DIO1,2,3), sulfotransferases (SULT1A3,1B1,1C2,1E1,2A1), sulfatase (STS), and solute carriers (SLC16A2,16A10) were quantified using absolute quantification qPCR. Evidence of fetal decompensation was observed within the high viral fetuses in the form of decreased DIO1 expression within the fetal liver and increased DIO3 expression in both components of the placenta. Circulating levels of T4 and inactive thyroid hormone metabolites, reverse-triiodothyronine (rT3) and two diiodothyronines (3,5-T2 and 3,3’-T2), were measured in fetal serum. While T4 was depressed, no change was observed in circulating rT3 levels, and neither T2 metabolite reached the lower detection limit. This may suggest that alterations in thyroid hormone metabolism generate a localized effect on hormone metabolites in the respective tissues.</p> <p>Alternatively, the low levels of available T3 and T4 limit the production of downstream metabolites to be found in serum.</p> <p>The cause-and-effect relationship between PRRSV infection, fetal thyroid disruption, and the effects on fetal thyroid hormone metabolism are unclear. Therefore, Chapter 3 developed a non-pathogenic model using methimazole (MMI) to induce hypothyroidism in the late gestation fetus and evaluate the impact on fetal development and thyroid hormone metabolism. Pregnant gilts were either treated with oral methimazole or equivalent sham from gestation day 85-106 (n=4/group), followed by classification of all fetuses as live, live but meconium stained, or dead. Fetuses exposed to MMI in-utero were notably hypothyroid with significantly suppressed serum T3 and T4 and histological evidence of goiter. Surprisingly, fetuses from MMI-treated dams were substantially larger but appeared to exhibit non-allometric growth with an increase in girth but not length. The liver, kidney, and the corresponding fetal placenta and maternal endometrium were collected from a subset of 16 fetuses per group to evaluate the relative expression of five genes associated with thyroid hormone metabolism, including three deiodinases and two solute carriers known to transport thyroid hormone. Compensatory transcription of DIO3 was observed in all tissues evaluated, suggesting increased vertical transfer of maternal thyroid hormone at the placenta and decreased breakdown of thyroid hormone within fetal organs.</p> <p>The evaluation of thyroid hormone metabolism within the fetus and within the placenta has allowed us to differentiate suppressed thyroid hormone levels of the pig fetus under pathogenic and non-pathogenic conditions. In the context of PRRSV infection, the observed decompensation of thyroid hormone metabolism would further exacerbate the hypothyroid state and is therefore consistent with NTIS. In contrast, fetuses with induced thyroid hormone suppression following maternal exposure to MMI showed compensatory thyroid hormone metabolism in the same tissues. This indicates true hypothyroidism and clearly demonstrates a fetal capacity to respond to such endocrine disruption.</p>

Variation de l’expression et de l’activité des 11β-hydroxystéroïde déshydrogénases rénales, cardiaques et placentaires au cours de la gestation de la rate

L’activation du système rénine-angiotensine-aldostérone peut entraîner le développement d’une hypertension artérielle et de la fibrose cardiaque. Toutefois, au cours de la grossesse, malgré une hausse substantielle des niveaux d’aldostérone, ces effets délétères ne sont pas observés. L’aldostérone exerce ses effets via les récepteurs des minéralocorticoïdes, les MR, qui peuvent également lier le cortisol avec une affinité similaire. La régulation des niveaux locaux de ce glucocorticoïde par les 11β-hydroxystéroïde déshydrogénases (11β-HSD) est donc essentielle pour éviter une stimulation inappropriée des MR. Nous suggérons que, durant la grossesse, ces enzymes sont impliquées dans la protection de la mère et du foetus contre les niveaux élevés d’aldostérone et de cortisol. Notre hypothèse de travail est que les mécanismes d’adaptation qui prennent place au cours de la grossesse nécessitent des changements d’expression (ARNm et protéine) et d’activité des 11β-HSD spécifiques selon le tissu. Des rates Sprague-Dawley ont été sacrifiées aux jours 14, 17, 19 et 22 de gestation (terme = jour 23) et leurs organes ont été collectés. Dans le rein, les niveaux protéiques des 11β-HSD sont diminués en fin de gestation. Dans le placenta, on observe une importante chute de l’expression génique et protéique de la 11β-HSD1 au jour 17 tandis que la 11β-HSD2 y est augmentée. L’expression et l’activité de la 11β-HSD2 sont par la suite diminuées jusqu’à terme. Aucune différence significative n’est retrouvée dans le ventricule gauche cardiaque. En conclusion, nos résultats démontrent que la gestation est accompagnée d’importants changements dans le placenta, possiblement pour assurer un développement foetal adéquat, tandis que le rein et le coeur sont peu ou pas affectés. Des études plus approfondies sur l’expression des MR dans ces tissus nous aideront à mieux comprendre l’implication des 11β-HSD au fil de la gestation. / The activation of the renin-angiotensin-aldosterone system can lead to hypertension and cardiac fibrosis. However, despite a substantial elevation of aldosterone during pregnancy, those adverse effects are not observed. Aldosterone acts via the mineralocorticoid receptors (MR) which can also bind cortisol with a similar affinity. Regulation of the local levels of this glucocorticoid by the 11β-hydroxysteroid dehydrogenases (11β-HSD) is thus crucial to avoid overstimulation of MRs. We believe that these enzymes are involved in the maternal and fetal protections against the high levels of aldosterone and cortisol observed during pregnancy. We propose that the adaptative mechanisms occurring during normal pregnancy involve tissue-specific changes in the expression (mRNA and protein) and activity of both 11β-HSDs. Pregnant Sprague-Dawley rats were sacrified on day 14, 17, 19 or 22 of gestation (term = day 23) and their organs were collected. In the kidney, our results have shown that 11β-HSDs protein levels decrease in late gestation. In the placenta, a dramatic decrease of 11β-HSD1 mRNA and protein expressions is observed on day 17 while 11β-HSD2 levels are increased. Expression and activity of the 11β-HSD2 are then decreased up to day 22. No significant differences were detected in the left cardiac ventricle. In conclusion, our results demonstrate that gestation is associated with important modifications in the placenta, possibly to ensure a normal fetal growth, while expression in the kidney and the heart is barely changed. More studies on MR expression in those tissues will be required to better characterise the function of the 11β-HSDs throughout pregnancy.

Medidas de indicadores de estresse oxidativo e de remodelamento cardíaco em camundongos expostos à poluição atmosférica ambiental durante o desenvolvimento embrionário e pós-natal / Measurements of oxidative stress indicators and of cardiac remodeling in mice exposed to urban air pollution during embrionary and postnatal development.

A poluição atmosférica de São Paulo (SP) pode provocar alterações cardiovasculares em seres humanos e animais experimentais, com maior vulnerabilidade em crianças e fetos. O mecanismo fisiopatológico que explicaria a relação entre a exposição aos poluentes e doenças cardiovasculares não está totalmente estabelecido, sendo que o estresse oxidativo pode estar ligado ao dano e morte celular. Há evidências de que o dano oxidativo pelo mecanismo da peroxidação lipídica pode estar relacionado às causas de diversas cardiovasculopatias. Estudamos o efeito da exposição ao ar ambiental nos níveis de peroxidação lipídica no coração de camundongos nos períodos pré e pós-natal. Os animais foram mantidos em duas câmaras de exposição, uma recebendo ar ambiente e outra ar filtrado, em quatro diferentes grupos: 1) LL: gestados e crescidos em câmara com ar filtrado, 2) PP: gestados e crescidos em câmara com ar poluído de SP, 3) PL: gestados na câmara poluída e crescidos na limpa, e 4) LP: gestados na câmara limpa e crescidos na poluída. A peroxidação lipídica do miocárdio foi avaliada tanto pelo método TBA como por imunohistoquímica para 15-F2t-isoprostano. As concentrações de malondialdeído (MDA, indicador de peroxidação lipídica) foi maior nos animais PP quando comparados aos LL (p = 0,004) e PL (p = 0,026), e não mostrou diferença significativa em relação ao grupo LP (p = 0,894). Os valores de MDA para animais PL e LP mostraram-se equivalentes (p = 0,168). Chama a atenção que o grupo PL apresentou um valor de MDA maior que o LL (p = 0,026). A fração de volume de miocárdio marcada imunohistoquimicamente para 15- F2t-isoprostano apresentou valores maior em PL (p = 2,884x10-5), LP (p = 6,632x10-6) e PP (p = 5,45x10-8) que em LL. O valor de PP foi maior que os de PL e LP (p = 3,661x10-4 e 1,058x10-3, respectivamente), sendo esses últimos equivalentes entre si (p = 0,624). A análise ultra-estrutural mostrou de maneira consistente a presença de lisossomos secundários contendo estruturas lipídicas membranosas nos grupos LP e PP. A porcentagem média de arteríolas com área entre 200 e 1000 ?m² em relação ao número total de vasos de cada grupo foi maior no grupo PP do que nos grupos LL e PL (p=0,0387 e p=0,0362, respectivamente). Estes resultados sugerem que existem altos níveis de peroxidação lipídica no tecido cardíaco dos animais expostos ao ar ambiental de SP. Chama a atenção o fato de que a exposição intra-uterina ter implicado em níveis maiores de estresse oxidativo na fase adulta, mesmo com a melhoria das condições ambientais. Comparam-se estes achados no miocárdio a outros resultados da literatura. / I t is well known that air pollution exposure in São Paulo can elicit cardiovascular injuries in humans and experimental animals and that children and fetuses appear to be particularly vulnerable. However, the mechanisms involved in this cardiovascular damage are not well understood. It has been suggested that the oxidative stress generated by air pollution exposure can trigger tissue injury. There is evidence supporting the idea that injury caused by lipid peroxidation may be related to the causes of several cardiovascular diseases. The aim of this study was to investigate the effects of prenatal and postnatal exposure to urban air pollution on the myocardium lipid peroxidation levels of adult mice. Myocardium lipid peroxidation was determined by the TBA method and by the detection of 15-F2t-isoprostan by immunohistochemical technique. The animals were placed in two chambers: one received air that passed through an air filter (clean) and the second received ambient air (polluted), according to four different exposure procedures: 1) Clean (CC): prenatal and postnatal in the clean chamber (control group), 2) Polluted (PP): prenatal and posnatal in the polluted chamber, 3) Polluted-clean (PC): prenatal in the polluted and posnatal in the clean chamber and 4) Clean-polluted (CP): prenatal in the clean and posnatal in the polluted chamber. The concentration of of malondialdehyde (MDA, a indicator of lipid peroxidation) was higher in group PP compared to CC (p = 0.004) and to PC (p = 0.026), and was not different of group CP (p = 0.894). The values of MDA for groups PC and CP turned to be equal (p = 0.168). Interestingly, group PC had a higher value of MDA than group CC (p = 0.026). The volume fraction of myocardium with detection of 15-F2tisoprostane is higher in PC (p = 2.884x10-5), CP (p = 6.632x10-6) and PP (p = 5.45x10-8) than in CC. The value of PP in higher than those of PC and CP (p = 3.661x10-4 and 1.058x10-3, respectively), while the latter two were equal to each other (p = 0.624). ). The mean ratio of arterioles wiht lumen area between 200 and 1000?m² to the total number of vessels in each group was higher in PP than in CC and PC (p=0.0387 and p=0.0362, respectively). These results, which suggest that exposure to air pollution is associated to higher levels of lipid peroxidation in the myocardium, are compared to other results previously published about respiratory and reproductive alterations related to pollution. Interestingly, the increased levels of lipid peroxidation in the PC group gives evidence that the prenatal exposure to urban air pollution can be linked to cardiovascular effects in adult life.

Avaliação das consequências da limitação do tamanho da prole de ratos Wistar ao nascimento sobre seu desenvolvimento ponderal e características morfofuncionais na idade adulta / Evaluation of the consequences of limiting the litter size of rats on their birth about weight development and morphofunctional characteristics in adulthood

Durante a vida intrauterina, o desenvolvimento do embrião e do feto é suscetível a mudanças ambientais que podem alterar o fenótipo do indivíduo na vida pós-natal. Eventos que ocorrem durante períodos críticos de rápida divisão celular, nos quais são formados os diversos órgãos e tecidos, podem alterar a estrutura e função de sistemas orgânicos gerando consequências precoces (baixo peso ao nascimento) e tardias (doenças na vida adulta). Os protocolos experimentais da maior parte dos estudos sobre a programação fetal reduz o tamanho da ninhada logo após o nascimento. Essa abordagem dificulta a interpretação e reprodutibilidade dos resultados observados. O objetivo deste estudo foi determinar se a pressão sanguínea, o metabolismo de carboidratos e gasto energético em proles adultas é influenciado pelo tamanho da ninhada. Ratas Wistar foram alimentadas com ração padrão ad libitum e foram acasaladas com ratos machos com 12 semanas de idade. Após o nascimento, a prole foi dividida em três grupos: tamanho da ninhada sem redução (Gc), proles reduzido a oito filhotes (G8) e proles reduzidos a quatro filhotes (G4). Ao fim de 12 semanas de idade, o peso corporal, pressão arterial, consumo de ração, glicemia, insulina, colesterol e triacilgliceróis, massa de tecido adiposo marrom, índice de adiposidade, massa renal e cardíaca foram determinados. O peso corporal, índice de adiposidade, glicemia, nível de insulina e índice HOMA foram maiores em machos e fêmeas no grupo G4 do que nos grupos G8 e Gc. No entanto, o consumo de ração foi menor no grupo G4. A pressão arterial foi maior no grupo Gc em machos e fêmeas. Em resumo, a redução do tamanho da ninhada está relacionada com a obesidade, resistência à insulina e possíveis alterações no gasto energético na prole adulta / During intrauterine life, the developing fetus is susceptible to environmental changes that can alter the phenotype of the individual in postnatal life. This phenomenon is called fetal programming. Events that occur during critical periods of rapid cell division may alter the structure and function of organ systems, resulting in consequences both early (low birth weight) and late (diseases in adulthood) in life. The experimental protocols of most of the studies on fetal programming involve a reduction in litter size soon after birth. This approach hampers the interpretation and reproducibility of the observed results. The purpose of this investigation was to determine if blood pressure, carbohydrate metabolism and energy expenditure in adult offspring are influenced by litter size. Female Wistar rats were fed standard rat chow ad libitum and were mated with male rats at 12 weeks of age. After birth, the offspring were divided into three groups: unchanged litter size (GU), culled to eight neonates (G8) and culled to four neonates (G4). At 12 weeks of age, the body weight; blood pressure; food intake; glucose, insulin, cholesterol and triacylglycerol levels; brown adipose tissue mass; adiposity index; renal mass; and cardiac mass were determined. The body weight, adiposity index, glucose level, insulin level and HOMA index were higher in males and females in the G4 group than in the G8 and GU groups. However, food consumption was lower in G4 males. The blood pressure was higher in the GU group. In summary, a small litter size is related to obesity, possible alterations in energy expenditure and insulin resistance in adult offspring

Avaliação do padrão nutricional e níveis séricos de ácidos graxos nas gestantes portadoras de fetos com gastrosquise / Evaluation of the nutritional pattern and serum fatty acid levels in pregnant women with fetuses with gastroschisis

Objetivo: avaliar a ingestão de nutrientes no período pré-concepcional e níveis séricos de ácidos graxos, durante a gestação, em gestantes portadoras de fetos com gastrosquise e gestantes portadoras de fetos normais. Métodos: estudo prospectivo caso-controle realizado no período de Julho de 2013 a Julho de 2015 no setor de Medicina Fetal do Hospital das Clínicas. O grupo gastrosquise (GG) foi constituído de 57 gestantes com gestações únicas, idade gestacional inferior a 34 semanas e feto com gastrosquise isolada. O grupo controle (GC) foi constituído de 114 gestantes portadoras de fetos normais pareadas de acordo com idade materna (± 2 anos), idade gestacional (± 2 semanas) e mesma classificação de índice de massa corpórea (IMC) no período pré-concepcional. Os dados referentes ao consumo dietético das gestantes foram obtidos a partir do questionário de frequência e consumo alimentar (QFCA) e o cálculo da ingestão dos nutrientes (macronutrientes; micronutrientes, ácidos graxos e aminoácidos) foi obtido a partir de programas específicos: Dietwin Profissional 2.0® and Virtuanutri®. Para a avaliação de níveis séricos de ácidos graxos (AG), as gestantes foram submetidas à coleta de sangue na entrada no estudo e no momento do parto. A comparação de AG foi realizada durante a gestação e no momento do parto. Na análise de ácidos graxos, observa-se que o total AG (p = 0,008), AG insaturados (p = 0,002) e a razão C18:1n9/C18:00 (p = 0,021) foi menor no GG em comparação ao GC durante a gestação; entretanto, a razão C16:00 / C18:2n6 (p = 0,018) foi maior no GG em comparação ao GC no mesmo período. Total AG (p = 0,044) e AG insaturados (p = 0,024) foi menor no GG em comparação ao GC no período <= 25 . AG insaturados (p = 0,025) e a razão C18:1n9/C18:00 (p = 0,013) foi menor no GG em comparação ao GC no período > 25 semanas e < 34 semanas. Conclusão: gestantes portadoras de fetos com gastrosquise apresentam dieta de baixa qualidade nutricional, com alto valor calórico e pobre em aminoácidos essenciais, no período pré-concepcional, e baixos níveis séricos de ácidos graxos durante a gestação / Objective: To evaluate the nutrients intake during the preconceptional period and the serum fatty acid levels during the gestation period of pregnant women with fetuses with gastroschisis and pregnant women with normal fetuses. Methods: A prospective case-control study was conducted at the Fetal Medicine Unit at Hospital das Clínicas from July 2013 to July 2015. The gastroschisis group (GG) comprised 57 pregnant women with singleton pregnancies of less than 34 weeks with fetuses with isolated gastroschisis, and the control group (CG) comprised 114 pregnant women with normal fetuses matched for maternal age (± 2 years), gestational age (± 2 weeks), and the same preconceptional body mass index (BMI). Nutritional assessments related to the preconceptional period were obtained using the Food Consumption Frequency Questionnaire and nutrient intakes (macronutrient, micronutrient, fatty acid and amino acid) were calculated using nutrition programs: Dietwin Profissional 20 ® and Virtuanutri ®. For the evaluation of serum fatty acid levels (FA), a blood sample was collected from each subject at the time they entered the study and at the time of delivery. The FA comparison was performed during gestation and at the time of delivery. In order to evaluate whether the differences between both groups were more frequent in the first or second half of gestation, a new analysis was performed, subdividing gesta 25 weeks and < 34 weeks. Objetivo: avaliar a ingestão de nutrientes no período pré-concepcional e níveis séricos de ácidos graxos, durante a gestação, em gestantes portadoras de fetos com gastrosquise e gestantes portadoras de fetos normais. Métodos: estudo prospectivo caso-controle realizado no período de Julho de 2013 a Julho de 2015 no setor de Medicina Fetal do Hospital das Clínicas. O grupo gastrosquise (GG) foi constituído de 57 gestantes com gestações únicas, idade gestacional inferior a 34 semanas e feto com gastrosquise isolada. O grupo controle (GC) foi constituído de 114 gestantes portadoras de fetos normais pareadas de acordo com idade materna (± 2 anos), idade gestacional (± 2 semanas) e mesma classificação de índice de massa corpórea (IMC) no período pré-concepcional. Os dados referentes ao consumo dietético das gestantes foram obtidos a partir do questionário de frequência e consumo alimentar (QFCA) e o cálculo da ingestão dos nutrientes (macronutrientes; micronutrientes, ácidos graxos e aminoácidos) foi obtido a partir de programas específicos: Dietwin Profissional 2.0® and Virtuanutri®. Para a avaliação de níveis séricos de ácidos graxos (AG), as gestantes foram submetidas à coleta de sangue na entrada no estudo e no momento do parto. A comparação de AG foi realizada durante a gestação e no momento do parto. Até hoje, somente o método multiplanar foi utilizado para a volumetria dos membros fetais. Desta forma, a utilidade do método rotacional (VOCAL®) para este fim nunca foi testada. Objetivos: Avaliar as variabilidades intra e interobservadores e a concordância entre as medidas do volume da coxa fetal realizadas com os métodos multiplanar e VOCAL®. Comparar as acurácias das fórmulas com medidas do volume da coxa fetal com as acurácias das fórmulas com medidas 2D. Comparar as acurácias das fórmulas deste estudo com as acurácias das fórmulas já publicadas. Métodos: 210 pacientes foram avaliadas, formando um grupo para gerar as fórmulas (n = 150) e um grupo para validá-las (n = 60). Os pacientes utilizados para gerar as fórmulas também foram utilizados para avaliar as variabilidades intra e interobservadores e a concordância entre as medidas realizadas pelos métodos multiplanar e VOCAL®. Foram utilizadas análises de regressão polinomial para criar uma equação com medidas 2D, uma com o volume da coxa fetal medido pelo método multiplanar (CoxaM) e uma com o volume da coxa fetal medido pelo método VOCAL® (CoxaV). Utilizaram-se testes t de Student pareados para comparar as acurácias das equações deste estudo com as acurácias das fórmulas já publicadas. Foram utilizadas análises proporcionais de Bland e Altman para avaliar as variabilidades intra e interobservadores e a concordância entre as medidas realizadas pelos métodos multiplanar e VOCAL®. Resultados: A diferença média percentual entre as medidas pelos métodos multiplanar e VOCAL® foi de -0,04 com limites de concordância de 95% de -8,17 e 8,09. A diferença média percentual e os limites de concordância de 95% entre as medidas na avaliação das variabilidades intra e interobservadores foram -1,10 (-7,67 to 5,47) e 0,61 (-7,68 to 8,91) para o método VOCAL® e 1,03 (-6,35 to 8,41) e -0,68 (-11,42 to 10,06) para o multiplanar. As melhores fórmulas para cálculo do peso fetal estimado (PFE) foram: PFE = -562.824 + 11.962 x CA x CF + 0,009 x DBP² x CA² (CA: circunferência abdominal; CF: comprimento femoral; DBP: diâmetro biparietal); PFE = 1033.286 + 12.733 x CoxaM; PFE = 1025.383 + 12.775 x CoxaV. Tanto no grupo que gerou as fórmulas como no grupo utilizado para validá-las não houve diferença significativa entre as acurácias das fórmulas com medidas 2D ou tridimensionais (3D). Quando aplicadas nas pacientes deste estudo, as acurácias das fórmulas 2D e 3D já publicadas foram significativamente piores dos que as das novas fórmulas. Conclusões: Os métodos VOCAL® e multiplanar são intercambiáveis para a volumetria da coxa fetal. Possivelmente as maiores fontes de discrepâncias na estimativa do peso fetal são as diferenças fenotípicas entre as pacientes utilizadas para criar as fórmulas. Os dados deste estudo reforçam a necessidade de fórmulas específicas para cada população, independentemente do uso de medidas 2D ou 3D. / Abstract: Introduction: Some authors have demonstrated that the prediction of birth weight using fetal limb volumetry is more precise than with two-dimensional ultrasound (2DUS). To date, only the multiplanar method has been used for fetal limb volumetry, so the usefulness of the rotational technique (VOCALTM - Virtual Organ Computer- aided AnaLysis) for this purpose has never been tested. Objectives: To evaluate the repeatability, reproducibility and agreement of measurements performed with multiplanar and VOCALTM techniques for total fetal thigh volumetry. To compare the accuracies of birth-weight-predicting models with total fetal thigh volumetry with models derived from 2DUS parameters. To compare the performances of our new formulas with those of previously published equations. Methods: 210 patients were prospectively evaluated to compose a formula-generating group (n = 150) and a prospective-validation group (n = 60). The patients of the formula-generating group were also used to evaluate the repeatability, reproducibility and the agreement of the measurements of multiplanar and VOCALTM techiniques for fetal thigh volumetry. Polynomial regression analysis was performed in the formula-generating group to generate one equation with 2DUS measurements, one with fetal thigh volume measured by the multiplanar technique (ThiM) and one with fetal thigh volume obtained by the VOCALTM method (ThiV). Paired samples t-tests were used to compare the accuracies of our equations with those of previously published 2D and three-dimensional (3D) equations. Proportionate Bland and Altman analyses were performed to determine the agreement between the two methods and to evaluate intra- and inter-observer variability. Results: The mean percentage difference between measurements performed with the VOCALTM and multiplanar techniques was -0.04 and the 95% limits of agreement were -8.17 and 8.09. The mean percentage difference and 95% limits of agreement between paired measurements in the assessment of intra- and inter-observer variability were -1.10 (-7.67 to 5.47) and 0.61 (-7.68 to 8.91) for the VOCALTM technique and 1.03 (-6.35 to 8.41) and -0.68 (-11.42 to 10.06) for the multiplanar method. The formulas with the best fit for the prediction of birth weight (EFW) were: EFW = -562.824 + 11.962 x AC x FL + 0.009 x BPD² x AC² (AC: abdominal circumference; FL: femur length; BPD: biparietal diameter); EFW = 1033.286 + 12.733 x ThiM; EFW = 1025.383 + 12.775 x ThiV. For both the formula-generating and the rospective-validation groups, there was no significant difference between the accuracies of the new 2DUS and 3DUS models. When applied to our population, the accuracies of previously published 2DUS and 3DUS formulas were significantly worse than our models. Conclusions: The VOCALTM and multiplanar techniques can be used interchangeably for total fetal thigh volumetry. We believe that the greatest sources of discrepancies in estimation of birth weight are the phenotypic differences among patients used to create each of the formulas mentioned in this study.
