Rio de Janeiro, RJ, Brasil. / Nosso objetivo com o presente trabalho é compreender as influências dos exércitos da Alemanha e da França no processo de modernização dos Serviços de Saúde dos exércitos de dois países da América Latina: Argentina e Brasil. Além disso, procuramos entender como as influências de saberes médicos daquelas escolas europeias estiveram presentes no cenário médico militar dos serviços de saúde destes países sul americanos. Para tal, temos a higiene militar e o desenvolvimento técnico-científico das Forças Armadas como eixo de nossos estudos para identificarmos as principais mudanças sofridas no Serviço de Saúde, do Brasil, e no Cuerpo de Sanidad da Argentina, bem como suas relações/implicações políticas à época. Nosso recorte temporal está relacionado com o surgimento do Cuerpo de Sanidad, bem como o processo de modernização deste e do Serviço de Saúde do exército brasileiro. Já o ano de 1930 foi escolhido em função do seu significado na historiografia destes dois países e, principalmente, por um novo quadro político, econômico, social e militar em função de seus movimentos revolucionários . Quanto à nossa abordagem teórico-metodológica, além do estudo comparado, partimos da noção de desenvolvimento desigual e combinado desenvolvido por Trotsky. Entendemos que a busca pela adequação ao processo evolutivo dos exércitos dos países centrais se deu em diversos países da América Latina a partir da contratação de missões estrangeiras para modernizarem seus exércitos. Ao utilizar as experiências da Argentina e do Brasil na contratação de missões deste tipo, podemos generalizar o tema, ou seja, generalizar a forma como o processo de modernização e do desenvolvimento técnico-científico implica mudanças nos Corpos de Saúde destes exércitos. / This work intends to comprehend the influences of the French and Germany armies in the process of modernization of the Armies Health Services of two countries of Latin America: Argentina e Brazil. Besides, we trying to understand how such influences were incorporated in the medical military scenery of these South American countries. For such, we had the military hygiene and the technical-scientific development of the Army Forces as axis of this study, in order to identify the main changes that occurred in the Army Services of Brazil and the Cuerpo de Sanidad of Argentina, as well as the political implications of the time. The time frame is related to the emergence and modernization of the Cuerpo de Sanidad in Argentina and the modernization of the Brazilian Health Service Army. It ends in the year 1930 because of its significance in the historiography of these two countries mainly due to the "revolutionary" movements and its implications of a new political, economic, social and military order. As for the theoretical-methodological approach, the work based in the comparative method study and also, in the notion of uneven and combined development developed by Trotsky. We understand that hiring foreign military missions by Latin American countries in order to modernize their armies, were a way of adapting in these countries the evolutionary process occurred in the armies of developed countries. By utilizing the experiences of Argentina and Brazil in hiring military missions, we can imply that in the process of modernization and technical-scientific development significant changes occurs in the armies Bodies Health.

Esta pesquisa se ocupou em analisar as condições de saúde das mulheres pobres de Porto Alegre, mais detidamente das internadas na Santa Casa de Misericórdia nas duas últimas décadas do século XIX, buscando examinar e compreender a relação estabelecida entre as mulheres pobres enfermas e suas doenças, com o saber médico e as ações médicas produzidas naquele hospital. Como consequência desse direcionamento de olhar para a medicina mais voltada às mulheres, a pesquisa também se deteve na área médica que distinguia a medicina feminina da masculina naquele dado período: a incipiente medicina da mulher, que se desenvolvia por meio das especialidades denominadas como obstetrícia e ginecologia. Desse modo, em decorrência, explorou a construção da medicina da mulher, ocupando-se sobre os primeiros desenvolvimentos da obstetrícia e da ginecologia na Santa Casa de Porto Alegre. A partir de tal exame, também foi verificado se o desenvolvimento destas especialidades representou melhorias para a saúde das mulheres lá internadas e/ou avanços na forma como os médicos se preocupavam com o tratamento das mulheres naquele local, de um modo geral. A perspectiva teórica seguida vincula-se aos estudos da História Social, utilizando-se principalmente das categorias analíticas gênero e classe social. A argumentação principal da pesquisa foi buscada por meio de dados demográficos quantitativos. Entretanto, por meio de exame sobre vasto material de pesquisa, procurou-se qualificar não somente os números apresentados, mas também a análise e a argumentação sobre os mesmos. Do ponto de vista metodológico, seguiu diversos preceitos advindos dos estudos demográficos, em grande parte, vinculados à História Social, principalmente, ao examinar os dados de morbidade e mortalidade das mulheres analisadas. Como resultado da análise realizada, verificou-se que no início da década de 1880 poucas mulheres da população porto-alegrense procuravam a Santa Casa para tratamento e, quando o faziam, mormente se encontravam em grave estado de saúde, o que resultava em um alto índice de mortalidade de mulheres naquele hospital. Também se observou que raríssimas eram as mulheres que buscavam a Santa Casa para a realização do parto ou tratamento de moléstias ligadas à obstetrícia e à ginecologia, naquela década. No entanto, ao observar as causas de internação das mulheres em relação às ações médicas voltadas para a medicina da mulher dentro do hospital da Santa Casa até o final do século XIX, percebeu-se que aquela situação se modificava aos poucos, com o aumento do número de mulheres procurando o hospital para tratamento de diversas moléstias e realização do parto. Em um último plano, a partir da verificação sobre os dados de internação, concluiuse que as ações médicas implantadas na Santa Casa, impulsionadas em grande parte pelo maior aprofundamento em torno da obstetrícia e da ginecologia, resultaram no incremento de internações de mulheres e no decréscimo significativo da mortalidade feminina, naquele hospital, no final do século XIX. / This research analyses the conditions of the poor women’s health of Porto Alegre, more specifically the patients in the Santa Casa de Misericórdia Hospital in the last years of the century XIX, it taking to exam and to understand the relationship between the poor sickness women and his illness, with the medical knowledge and their actions in this Hospital. As a consequence of this direction to look for medicine more directed to women, these research also stopped in the medical field that distinguish female from male medicine in that specific period: the incipient women's medicine, which developed through the specific denominated as obstetrics and gynecology. In this way it was explored the construction of the medicine women, occupied themselves about the first developments of obstetrics and gynecologic at the Santa Casa Hospital from Porto Alegre. From this examination, it was also found that the development of these specialties represented improvements to the health of women hospitalized and advances in how doctors were concerned about the treatment of women in that place. The followed theoretical perspective connected to the studies of the Social History, using mainly the analytics categories in gender and social class. The main argumentation of the research was catch trough quantitative demographic bases. However, through the exams about a huge search material looks to qualify not just for the presented numbers, but also the analysis and argumentation about the topics. The methodological point of view, followed by several arising precepts of demographic studies, in the main of the Social History linked, mainly, examined the morbidity and mortality bases of the women analyzed. As the result of the analysis, confirmed that in the beginning of the decade of 1880 few women of the Porto Alegre population looked for the treatment at the Santa Casa Hospital, and when they did this, they usually were in serious healthy troubles that result in a high level of women’s mortality in those hospital. It also was observed that rarely women look for help in Santa Casa Hospital to realize their child-birth or illness treatment connected to the obstetrics and gynecologic methods. However, when it was observed the causes of the women hospitalization in relation to the medical actions that were directed to the women medicine inside the Santa Casa Hospital till the final of the century XIX, it was noticed that this situation modified slowly with the number of women who looking for the Hospital for treatment to several disease and to the child-birth realization. At least, from the verification about the bases of the hospitalization, it was concluded that the medical actions done at the Santa Casa Hospital were driven in the major part by the improvement about the obstetric and gynecologic methods, that resulted in an increasing of the women hospitalization and in the significant decreasing of the female mortality, in those hospital, at the final of the century XIX.

Voilà une vingtaine d’années que le concept d’addiction a été élaboré par quelques psychiatres et chercheurs en neurosciences afin de rendre compte, à travers un nouveau regard, des phénomènes de dépendance (en associant drogues illicites, alcool et tabac). Il a ensuite été utilisé pour la première fois en 1999 dans une politique publique en France, à savoir le plan triennal de la Mission Interministérielle de Lutte contre la Drogue et la Toxicomanie (MILDT).Le processus de construction du problème public des addictions constitue ainsi l’objet de cette thèse. Dans quelles mesures la reconfiguration de certains espaces des champs médical et politico-administratif a partie liée à l’institutionnalisation du paradigme addictologique et comment celui-ci a contribué à transformer les drug policies contemporaines ? Le problème public des addictions doit son émergence au succès politique d’un petit groupe d’acteurs qui a su transposer un concept médical dans le langage de l’action publique. Dans le prolongement des politiques de réduction des risques, les addictions bousculent le mode de régulation des usages de drogues, en favorisant leur sanitarisation plutôt que leur pénalisation. Par le biais d’une sociologie historique et politique de l’action publique, cette thèse retrace ainsi la genèse de l’addictologie et des drug policies françaises jusqu’au plan triennal de la MILDT de 1999. / It has been about twenty years that the addiction concept has been elaborated by some psychiatrists and neuroscientists in order to define dependence phenomena in a new way (by associating illicit drugs with alcohol and tobacco). Then it has been used for the first time in 1999 in a public policy in France, i.e. the three-year plan of the Mission Interministérielle de Lutte contre la Drogue et la Toxicomanie (MILDT).The construction process of the “addiction” public problem constitutes this thesis subject. In which way the reconfiguration of some spaces of the medical and political fields is connected to the institutionalization of the addictological paradigm and how this one has led to transform contemporary drug policies? The “addiction” public problem emerged thanks to the political success of a small group of actors who transposed a medical concept into the language of public policy. In the continuation of harm reduction policies, addictions reshape the regulation model of drug use by promoting their sanitarization rather than their criminalization. Through a political and historical sociology of public policy, this thesis thus traces the genesis of French addictology and drug policies until the MILDT’s three-year plan of 1999.

Cette thèse examine l’apport du travail statistique à la normalisation de la vaccination en France, des années 1950 au milieu des années 1990, une période au cours de laquelle la vaccination a fait l’objet de peu de controverses. Au moyen d’opérations statistiques de rationalisation, de régulation et de justification, l’épidémiologie (entendue comme un ensemble de pratiques plutôt qu’une discipline universitaire autonome) a largement contribué à maintenir cet état de fait – un processus que je qualifie de routinisation. L’enquête est construite sur des archives issues de diverses institutions, nationales et internationales, ainsi que sur une série d’entretiens avec les principaux acteurs du domaine. Elle éclaire le rôle d’un groupe d’épidémiologistes associés au Centre international de l’enfance (1949-1999), qui mit en œuvre un programme de rationalisation et de simplification de la vaccination. Dans cette période coloniale et postcoloniale, l’Afrique subsaharienne était un de leurs terrains de prédilection : ils y réalisèrent de nombreux essais, simultanément à leurs activités métropolitaines. L’implication de l’OMS dans le domaine de la vaccination a conforté la dimension internationale de cette routinisation : les épidémiologistes français se sont appropriés des opérations statistiques popularisées par Genève. À la fin de la période considérée, la controverse de la vaccination contre l’hépatite B a perturbé cette routinisation et mis en évidence un écart croissant entre les logiques de l’épidémiologie et l’expérience des individus vaccinés. Cette recherche éclaire d’une lumière nouvelle la façon dont l’intervention de santé publique la plus courante a été normalisée et gouvernée par des nombres. / This dissertation studies the entanglement between statistical production and the normalization of immunization practices in France from the 1950s to the mid-1990s, a period during which immunization remained largely uncontroversial. By rationalizing, regulating, and justifying immunization, epidemiology (understood as a collection of practices, rather than a discrete academic discipline) has contributed greatly to this normalization – a process I term “routinization.” This research project is based on archival findings, both in France and internationally, as well as a series of interviews with significant actors in the field. It is primarily focused on a group of epidemiologists affiliated with the Centre international de l’enfance (French International Children’s Center, 1949-1999), whose mission was to rationalize and simplify immunization for children. Throughout the institution’s history, which overlaps with the late colonial period and the process of de-colonization, there is a significant engagement with Francophone Africa: numerous trials were carried out simultaneously in Sub-Saharan Africa and the Paris region. The transnational nature of this activity is also due, in part, to the involvement of the World Health Organization in matters of immunization – French epidemiologists appropriated calculations popularized on a global scale. At the end of the period in question, the Hepatitis B vaccine controversy disrupted the routinization process and shed light on the rising gap between the discourse and practice of epidemiology and the experience of vaccinated individuals. The purpose of this study as been to contribute to the emerging interdisciplinary field of historical sleep studies, which spans the biological and social sciences, legal studies, and the humanities. As an interdisciplinary scholar based primarily in the humanities, my goals have been twofold: to develop a critical archive for the use of scholars in this emerging field; and to demonstrate how that archive might be used to productive effect in literary studies. To that end, this project begins with a critical introduction to the field of sleep studies and its relationship to eighteenth- and nineteenth-century thought and follows with two distinct but connected sections: the archive itself and a short series of literary case-studies drawn from across the eighteenth and nineteenth centuries. My hope is that these case studies will show how the materials in the archive allow literary scholars to produce new insights about familiar, canonical texts. <br>

This study treats the topic of smallpox prevention in Sweden during the nascent phase of vaccination between 1798–1805. The aim is to examine how Swedish physicians sought to establish trustworthiness for a new medical treatment as well as to analyse the relation between professional physicians and the unlicensed actors involved in the practice, i.e. the clergy and their assistants. The source material consists of pamphlets, articles in daily papers and annual reports written by Swedish physicians. By drawing upon theories on boundary-work and epistemic authority, this study has found that several boundaries were drawn within the medical space by the physicians who strived to retain the elements of the practice that were considered the most crucial, while delegating others. More importantly, the study has found that the physicians attempted to engage the clergy in the dissemination of knowledge due to their influence over the public. According to the physicians, the clergy were able to establish a credibility for the vaccine which could induce the population to undergo the treatment without coercion or laws made by the government.
