Patellofemoral smärta relaterat till överbelastning och dess behandling hos barn och unga - en litteraturstudie / Patellofemoral pain related to overuse injury and its treatment in children and adolescents - Systematic review

Bakgrund: Patellofemoral smärta (PFS) är vanligt förekommande hos idrottsaktiva barn och ungdomar vilket kan leda till långvariga besvär och till att barn slutar vara fysiskt aktiv. Individuell fysioterapeutisk behandling används ofta inom primärvården för att tackla detta problem. Syftet med denna studie var att undersöka vad individuell fysioterapeutisk behandling innebär och granska evidensen för detta samt relatera prevalensen av PFS relaterat till tidig sportspecialisering.   Metod: Systematisk litteraturstudie. Sökning gjordes i databaserna PubMed och PEDro. Evidensgradering bedömdes med PEDro, SBUs GRADE och modifierade GRADE. Åtta studier inkluderades i litteraturstudien. En studie undersökte prevalensen av PFS relaterat till tidig specialisering. Sex studier undersökte en multimodal behandling med biopsykosociala faktorer för förbättrad knäsmärta samt självupplevd funktion hos barn och unga både kort- och långsiktigt. En studie undersökte effekten av knä- och höft-träning med gummiband.    Resultat: En studie kom fram till att tidig specialisering ökar risken för PFS 1.5 gånger. Sex studier visade på goda effekter av multimodal behandling med biopsykosociala faktorer för knäsmärta samt självupplevd funktion hos barn och unga både kort- och långsiktigt, dock bedömdes den gemensamma evidensen som otillräcklig enligt GRADE. En studie med hög evidens visade att knä- och höft-träning med gummiband gav minskad knäsmärta samt ökad muskelstyrka.    Konklusion: Tidig specialisering medför en ökad risk för PFS, detta bör kliniker vara uppmärksamma på vid behandling. Behandling för PFS bör inte enbart fokusera på styrketräning för knä- och höft-muskulatur utan även involvera en patientutbildning kring biomekanik av strukturer runt knät och smärthantering samt modifiera de aktiviteter som medför smärta. / Background: Patellofemoral pain (PFP) is common in children and adolescents that are physically active, if untreated it may lead to persistent inconvenience and an abrupt end to adolescents involvement in sports. Individual physiotherapeutic treatment is commonly used in primary care to assess this problem. The purpose of this study was to investigate the concept individual physiotherapeutic treatment in PFP while assessing their evidence and to investigate the prevalence of PFP related to early sport specialisation.   Method: Systematic review. The search was conducted in databases PubMed and PEDro. Eight studier were included in the study. These studies were assessed to evidence quality according to PEDro-scale, SBUs regular- and modified GRADE. One study investigated the prevalence of PFS related to early sports specialization (ESS). Six studies investigated a multimodal treatment involving biopsychological factors. One study investigated the effect of knee- and hip-training with Theraband.    Results: Early sport specialization increased the risk for PFP 1.5 times. Six studies showed promising results on factors as knee pain and self-assessed function within the population using a multimodal treatment, the combined evidence was estimated as insufficient. One study with high evidence concluded that knee- and hip-training with Theraband decreased knee pain and increased muscle strength.    Conclusion: ESS is associated with an increased risk of developing PFP, this could be a factor that clinicians should be aware of when treating PFP. An intervention treating PFP should include musclestrenghtening and involve psycosocial factors, one method could be through patient education.

Makroekologie a makroevoluce ptačího zpěvu / Macroecology and macroevolution of birdsong

Birdsong is one of the most astounding natural sounds which profoundly shaped our evolutionary thinking since the 19th century. Despite a strong interest in birdsong for over 100 years, our understanding of birdsong ecology and evolution over large spatial and phylogenetic scales is still very fragmentary. Answering many basic questions requires a global synthesis covering vast diversity of extant bird species and adoption of multidisciplinary approaches. In presented dissertation thesis, my co-workers and I have explored important patterns in macroecology and macroevolution of song in passerines (Order: Passeriformes), the most diverse and widespread bird order. We have focused on three key song phenomena: (1) song complexity, (2) song frequency and (3) the presence of song in female birds. We have exploited birdsong "big data" available on public citizen science databases and other open sources in order to fill several important gaps in the current knowledge. These data were analysed by a combination of phylogenetically-informed cross-species analyses and spatial macroecological approaches. Since the publication of Darwin's seminal work, elaborated songs are generally agreed to be the result of sexual selection. This essay investigates live-streamed guided tours made by Moderna Museet, so called sofa tours, streamed between February 2020­ and April 2021. The aim is to observe, describe and analyze the [Moderna Museet’s] sofa tours using a modality approach as well as relating the sofa tours to physical museum visits, using the concepts of media, mediation, remediation, immediacy and hypermediacy. The research questions concern ways of looking at art when participating in sofa tours and the differences compared to physically viewing art at the museum. In conclusion, the sofa tours made by Moderna Museet offers several alternatives to physical museum visits, not necessarily better or worse. Participants can actually modify their experience of participating in sofa tours and looking at art, for example by choosing to participate live or afterwards, with or without access to other participants comments, questions and reactions as well as choosing between using Facebook, Instagram, YouTube or the museum webpage to access the sofa tours. Comparing sofa tours to physical museum visits showed that both their principal function is to provide opportunities for people to view art together with other people, with or without the aid of a guide. However, most of the mediated space in the physical museum is unavailable to sofa tour participants. On the other hand, the sofa tour creates another set of unique mediations which do not exist in the physical museum space.

Práctica digital en fisioterapia en el periodo de pandemia por COVID-19 en Lima / Physiotherapy digital practice in Lima during COVID-19 pandemic period

Solicitud de embargo por publicación en revista indexada. / Objetivo: Identificar el uso de práctica digital en fisioterapia durante la pandemia por la COVID-19, realizada por fisioterapeutas colegiados en el Colegio de Tecnólogo Médico del Perú que laboran en la ciudad de Lima-Perú. Método: Estudio observacional, de corte transversal y descriptivo Resultados: Se obtuvieron 194 encuestas válidas de fisioterapeutas colegiados que laboraban en la ciudad de Lima- Perú durante los meses de febrero a junio del año 2021 de los cuales 46.4% han optado por la modalidad de práctica digital y 53.6% por la modalidad no digital o presencial. La mayor parte de los encuestados está representado por 66% de mujeres con una mediana de 37 años de edad, de los cuales 52.1% no se encuentran unidas, 76.3% viven con familia, 63.4% egresaron de una universidad nacional, 58.3% tienen una segunda especialidad con 12 años de práctica de labor en fisioterapia, 35.1% trabajaban en Lima Centro, 57.2% laboraban en el área de clínica general, 63.6% manifestaron no mantener el mismo monto cobrado, 75% no recibieron ninguna facilidad o beneficios para la práctica digital por la institución, por lo que el 54.8% no se encuentran satisfechos ni insatisfechos con su trabajo; sin embargo, 93% señalaron que no cambiarían de empleo. Conclusiones: Se identificó que la mayor parte de los fisioterapeutas encuestados optaron por el abordaje presencial durante la pandemia por la COVID-19 en Lima - Perú. / Objective: To identify the use of digital practice in physiotherapy during the COVID-19 pandemic, carried out by physiotherapists registered in the College of Medical Technologists of Peru working in the city of Lima-Peru. Method: Observational, cross-sectional and descriptive study. Results: 194 valid surveys were obtained from collegiate physical therapists who worked in the city of Lima-Peru during the months of February to June 2021, of which 46.4% have opted for the digital practice modality and 53.6% for the non-digital or face-to-face modality. Most of the respondents are represented by 66% women with a median of 37 years of age, of which 52.1% are not married, 76.3% live with a family, 63.4% graduated from a national university, 58.3% have a second specialty with 12 years of work practice in physiotherapy, 35.1% worked in downtown Lima, 57.2% worked in the general clinical area, 63.6% said they did not maintain the same amount charged, 75% did not receive any facilities or benefits for the practice digital by the institution, for which 54.8% are neither satisfied nor dissatisfied with their work; however, 93% indicated that they would not change jobs. Anthropological Expressive Modalities of Disorientation: A Biblical and Pastoral Theology

Recent years have seen a plethora of research accomplished with respect to OT poetic prayers, predominantly in relation to the Psalter, with a least one ramification being a lack of engagement with the full spectrum of expressions especially within OT prosaic prayers. In addition to this, contemporary pastoral literature continues to exhibit an underdevelopment with respect to the full anthropological spectrum of expressions amidst grief especially within intercultural contexts. Observing both lacunas in scholarship, this dissertation operationalizes a Revisionist Method of Mutual Critical Correlation as its metamethod while incorporating Renewed Form Criticism, Hermeneutical Phenomenology, annd a Liberative Intercultural Praxis as it pursues a twofold objective: 1) an examination of the multivalent expressions (i.e., verbal, physical, and emotional) that leading Israelites pray-ers within the ancient context of OT prosaic prayers communicated to/before God while in the crux of disorientation, and 2) an investigation into the multivalent expressions (i.e., verbal, physical, and emotional) that leading Christian pray-ers wtihin an intercultural context (i.e., among Canadian immigrants from Guyana and Vietnam) communicated to/before God amidst the phenomenon of grief. At the heart of attending to this twofold mission is the configuration of an expressive domain of anthropological expressive modalities of disorientation communicated to/before God, along with their essence or meaning. Sensitized to such an expressive domain with its multivalent significance, contemporary pastoral caregivers would thus be better equipped to foster compassionate, liberative intercultural pastoral care for intercultural Christian communities coping with grief.

Music as a sensory experience was used during the therapeutic process with each case study, to facilitate the child's process towards healing. The conceptual framework of music as a therapeutic medium was researched in depth to bring clarity to the influence it has on the emotional well-being of the child. The child who entered into therapy was selected according to specific criteria, with the experience of emotional trauma being priority. The researcher also studied the concept of emotional trauma in depth and the consequences it has on a child's overall functional ability. Music was used as an expressive technique during the therapeutic process with each child. The conclusion drawn from this study indicated that the gestalt approach can be applied effectively in using music as a therapeutic medium in the treatment of the emotionally traumatized child. / Die Gestalt benadering is gebruik in hierdie navorsing ten einde 'n kontekstuele raamwerk daar te stel waarbinne musiek as terapeutiese medium in die behandeling van emosionele trauma in kinders gebruik kon word. The motivation for this study is the increase of treatment needs of heroin dependents at SANCA, (South African National Council of Alcohol and Drug Abuse) Witbank. SANCA Witbank wants to ensure that the most effective treatment can be supplied. Research goals are to:  Do a literature study of out-patient treatment programmes.  Assess the needs of heroin out-patient dependents with regard to out-patient treatment programmes.  Make recommendations with regard to out-patient treatment programmes for heroin dependents. It was an exploratory research subject and the data collection methods were qualitative and quantitative. Interview schedules were used as research tools. Schedule B was completed by the researcher with heroin dependents that visited SANCA Witbank for the period 31 January 2005 to 4 February 2005. Schedule A was completed by the researcher with personnel of SANCA out-patient clinics and the questions were discussed and completed telephonically. Efeito fotobiomodulador da terapia laser de baixa intensidade na transição inflamação-reparo em modelo de lesão pulmonar / Effect of low level laser therapy in the transition between inflammation and repair in a model of lung injury

A terapia laser de baixa intensidade (LLLT) é capaz de modular a resposta inflamatória em vários modelos experimentais, reduzindo o infiltrado de células do sistema imune e a secreção de mediadores químicos. Estudos prévios sugerem sua utilidade também no tratamento da lesão pulmonar aguda (LPA), condição caracterizada por intensa reação inflamatória em vias aéreas distais. Assim, considerando os efeitos anti-inflamatórios do laser e a fisiopatologia da LPA, formulamos a hipótese de que a LLLT 660 nm, 10 J/cm2, poderia modular o processo inflamatório nessa patologia favorecendo a transição inflamação-reparo. Foram utilizados camundongos C57BL distribuídos aleatoriamente em 4 grupos: PBS- instilado com PBS intratraqueal; PBS+LLLT- irradiado com LLLT 6 horas após instilação intratraqueal de PBS; LPS- instilado com LPS (5 mg/kg); LPS+LLLT- irradiado com LLLT 6 horas após instilação intratraqueal de LPS. Observou-se que a instilação de LPS intratraqueal levou a um aumento do número de células inflamatórias tanto na região peribronquiolar quanto no lavado broncoalveolar (LBA), detectado na histologia pulmonar e pela expressão de F4/80. Houve também aumento da transcrição e secreção de citocinas (TNF-alfa, IL-1beta, IL-6, IL-10) e quimiocinas (MCP-1) (detectadas por PCR quantitativo e ELISA). A aplicação de LLLT 6 horas após a instilação de LPS causou uma significativa diminuição tanto no influxo de células inflamatórias quanto na secreção de mediadores. Em seguida, foram caracterizadas as populações de macrófagos presentes no tecido pulmonar e observou-se nos animais do grupo LPS+LLLT, comparados ao grupo LPS, uma diminuição na expressão de mRNA de HIF-1alfa e aumento para arginase 1, sugerindo um predomínio da população de macrófagos M2. There are some previous studies suggesting also useful in the treatment of acute lung injury (ALI), a condition characterized by an intense inflammatory reaction in the distal airways. Considering the anti-inflammatory effects of laser and the pathophysiology of ALI, we hypothesized that LLLT 660 nm, 10 J/cm2, could modulate the inflammatory process in this disease favoring the transition between inflammation and repair. C57BL mice were divided into four groups: PBS - intratracheal instillation of PBS; PBS+LLLT - irradiated with LLLT 6 hours after intratracheal instillation of PBS; LPS - intratracheal instillation of LPS (5 mg/kg); LPS+LLLT - irradiated with LLLT 6 hours after intratracheal instillation of LPS. Animals were sacrificed 24 hours after injury. Intratracheal LPS inoculation induced a marked increase in the number of inflammatory cells in perivascular and alveolar spaces (detected by lung histology and in bronchoalveolar lavage), and F4/80 expression (macrophage marker). There was also an increase in the expression and secretion of cytokines (TNF-alfa, IL-1beta, IL-6, IL-10) and chemokine (MCP-1) detected by ELISA and quantitative PCR. The LLLT application 6 hours after LPS induced a significant decrease in both of the inflammatory cells influx and mediators secretion. Then, we characterized the macrophages populations in lung tissue and we observed that in the LPS+LLLT group the expression of HIF-1alfa mRNA was reduced while arginase 1 was increased, which suggested that the M2 macrophages were predominant. The LLLT effect was also tested on peritoneal macrophage population in culture stimulated with LPS and there was decreased of cytokines and chemokine production, however any difference in polarization markers was found
