Estresse ocupacional em enfermeiros de unidades de emergência no município de Manaus/AM

O estresse ocupacional é oriundo das tensões no trabalho e as fontes dessas reações são os estressores que podem influenciar o comportamento profissional, comprometendo resultados e a qualidade de vida. Tem a finalidade de avaliar o nível de estresse ocupacional em enfermeiros de unidades de emergência no município de Manaus/AM. Estudo de campo, epidemiológico e transversal, de natureza quantitativa, realizado com 39 enfermeiros de unidades de emergência. Utilizado questionário com questões sobre as características dos participantes e o seu reconhecimento do estresse, 57 questões sobre atividades relacionadas ao trabalho em emergência, associados a escala tipo likert com escala de 1 a 7. Os dados foram coletados nos períodos diurnos e noturnos, nas duas unidades de saúde selecionadas, durante os horários dos plantões, o instrumento era entregue ao enfermeiro e recolhido ao final do plantão. Os resultados mostraram que os enfermeiros estavam em médio nível de estresse com escore de 4.0, as áreas C - Atividades relacionadas ao funcionamento adequado da unidade e área F atividades relacionadas a administração de pessoal foram as mais estressoras. As atividades mais estressoras foram a solicitação de revisão e conserto de equipamentos com escore de 5.4 e, elaborar relatório mensal da unidade com escore de 5.1. O cargo gerencial teve maiores escores de estresse que o assistencial, apesar de não encontrar correlação significativa. Houve correlação entre os níveis de estresse quanto ao tempo de formação, tempo de trabalho em unidades de emergência, vínculo com outra instituição, rodizio de setores da mesma unidade e instituição com nível de significância de <5%. Observou-se correlação significativa dos níveis de estresse com instituição A relacionada ao funcionamento adequado da unidade e a administração de pessoal. Sendo assim, os dados mostram que o estresse dos enfermeiros de emergência está relacionado tanto às atividades do ambiente de trabalho, quanto às características dos sujeitos.

Perceptions of Stress: Employee Participation in a Yoga Class

Abstract Employees experience workplace stress that can affect their health resulting in chronic diseases such as diabetes, stress, hypertension, and cardiovascular diseases. Stress also contributes to staff presentism, absenteeism, and high turnover rate. The employee wellness program (EWP) yoga class at a metropolitan teaching hospital has not been evaluated for its effectiveness in employee stress reduction and improvement in employee health. The practice focus question for this project explored employees' perception of the impact of a metropolitan teaching hospital's EWP yoga class on their stress reduction and health promotion activities. The theoretical framework for this project is Lazarus' transactional model of stress and coping. This model suggests that individuals assess the stressor and find strategies for coping with the stress as it relates to them and their environment. The evidence that guided this project included a comprehensive literature review and the analysis of data retrieved from semi structured interviews with 20 participants in the yoga class who responded to an open invitation. Audiotaped interviews with the participants were analyzed and coded for common themes and revealed that participating in the EWP yoga program helped to reduce their perceived stress and improve their mental and physical well-being. The findings of this project were used to inform organizational leadership and may provide opportunities to evaluate the current yoga program regarding employee stress reduction, improving healthy activities, expansion of the program to other work sites within the organization, maintaining a healthy workforce, and reducing health care costs.

Psychological Stress in Critical Care Nurses

Increased levels of stress in the workplace have led the American Psychological Association to predict a looming public health crisis. Critical care nurses (CCNs) are highly exposed to stressors in the workplace, more than other nurses. Tens of billion dollars were reported lost in productive work time yearly due to ill health from depression alone. An important gap remains in understanding the relationships of stressors with the amount of stress. Supported by the biopsychosocial stress theory, the purpose of this study was to determine the relationship between frequencies of perceived stressors (IVs) as a source of perceived levels of stress (DV) or allostasis. The mixed-methods study included an online questionnaire and an e-mail interview of 400 hospital-based CCNs from professional nurses' associations in the United States. The Nursing Stress Scale was used to determine the kinds and frequencies of IVs, while the Psychological Stress Measure was used to assess the DV. Multiple regression analysis assessed the strength and direction of the relationships between IVs and DV. Interviews fell short of the minimum sample size for saturation and were not analyzed. Quantitative findings indicated workload as the most frequently perceived stressor, correlating positively with allostasis (p = .0005). Death and dying, conflict with other nurses, and lack of support for CCNs were also statistically significant (p < .05). Implications to practice and social change include promoting changes in policy with management support to reduce workload, death and dying, conflict with other nurses, and lack of social support for CCNs. Personal behavior changes like relaxation and physical exercises as coping methods may also reduce levels stress.

Police Stress: An Analysis of the Impact on Child Sexual Exploitation Investigators

The psychological and physiological effects of work-related stress on law enforcement causes high morbidity and mortality rates and rates of alcoholism, substance abuse, domestic violence, and suicide higher than the national average. The purpose of this explanatory sequential mixed-methods study was to examine whether work-related stress experienced by child sexual exploitation (CSE) and child sexual abuse (CSA) investigators differ from that of other duty assigned subgroups. I used Karasek's job demands-control model as the theoretical framework for this study. I conducted the study within a medium sized law enforcement agency in eastern Washington State. The sample in the quantitative study consisted of 27 law enforcement officers from 17 duty-assigned subgroups who completed McCreary and Thompson's Operational Police Stress Survey (PSQ-Op) and Organizational Police Stress Questionnaire (PSQ-Org). The sample in the qualitative study consisted of 7 law enforcement officers who answered 5 researcher developed questions during a telephone interview. Descriptive statistics, a Pearson's correlation analysis, and linear regression analysis of the PSQ-Op and PSQ-Org revealed no significant difference in reported work-related stress experienced within the duty-assigned subgroups, revealing no correlative difference of stress experienced by CSE and CSA investigators and the other duty assigned subgroups due to job demands and job control. Content analysis of the qualitative interviews revealed themes that supported the finding of the quantitative study. The findings of this study support the need for law enforcement leaders to take preemptive measures to mitigate the effects of work-related stress on all law enforcement officers.

Analisar as características do trabalho realizado por trabalhadores de Enfermagem atuantes na área hospitalar e a relação com a presença de Ansiedade, de Depressão e de Estresse, assim como, com o cortisol capilar. Método: Estudo descritivoanalítico, transversal, com abordagem quantitativa, desenvolvido no município de São Carlos (SP), com 164 profissionais da equipe de enfermagem da área hospitalar, entre eles auxiliares, técnicos de enfermagem e enfermeiros, que trabalhavam nos setores Unidade de Internação, Urgência e Emergência, Unidade de Terapia Intensiva Adulto, Unidade Coronariana e o Isolamento. Os dados foram coletados por meio de instrumento semiestruturado de autopreenchimento destinado a caracterizar a população e, também, por dois outros instrumentos: a Escala Hospitalar de Ansiedade e Depressão e a Escala para Avaliação do Estresse Percebido. Realizada a coleta de amostras de cabelos dos participantes por meio do kit Diasource específico para Cortisol ELISA (Enzyme-Linked Immunosorbent Assay - KAPDB290). A pesquisa foi previamente aprovada por Comitê de Ética em Pesquisa, protocolo CAAE: 55839216.5.0000.5393. A prevalência de depressão tomada como base para o cálculo amostral foi assumida como desconhecida, o valor de prevalência de 50%, que resulta em um tamanho amostral que contemple qualquer valor de P. O programa do cálculo amostral foi o R versão 3.1.2. Os testes estatísticos utilizados para a realização das análises foram o Pearson Chi-Square, Qui-quadrado, Coeficiente de Correlação de Spearman-?, Mann-Whitney e Kruskal Wallis. Resultados: Por meio das análises descritivas (n=164) constatou-se que a maioria dos entrevistados é constituída por técnicos de enfermagem (62,19%), do sexo feminino (80,49%), com idade entre 31 a 50 anos (57,93%), casados/com companheiro (44,51%), efetivos, atuando na profissão entre 0 a 10 anos (76,22%), trabalhando em unidades de internação (48,17%), no turno diurno (52,44%), aos finais de semana (93,9%), não tabagistas (81,7%) e nem consumidores de bebidas alcoólicas (60,4%). Em relação ao cortisol (n=161), 47,8% mostraram nível acima do normal; 23,8% apresentam estresse moderado, 20,1% estresse alto e 12,8% estresse muito alto; já 44,51% apresentaram Ansiedade e 24,39% Depressão. Segundo os testes estatísticos, os níveis de cortisol não influenciaram no Estresse percebido, pois não houve diferença no seu total, tanto para os trabalhadores que apresentam ou não Ansiedade. Não houve diferenças estatísticas entre os setores hospitalares das áreas críticas e não críticas e também nos níveis de Estresse, de Ansiedade e de Depressão entre as categorias de trabalhadores. Conclusão: As hipóteses foram parcialmente confirmadas, pois ocorreram alterações nos níveis do cortisol capilar, que estavam acima do recomendado. O estudo trouxe contribuições importantes relacionados ao biomarcador cortisol capilar, ao Estresse, à Ansiedade e à Depressão. Instituições hospitalares deveriam oferecer programas com intervenções na área ocupacional e medidas preventivas para amenizar alterações de saúde mental nos trabalhadores de enfermagem / Objective: To analyze the characteristics of the work performed by Nursing workers working in the hospital area and the relationship with the presence of Anxiety, Depression and Stress, as well as with capillary cortisol. Method: A cross-sectional descriptiveanalytical study with a quantitative approach developed in the city of São Carlos (SP), with 164 professionals from the nursing team of the hospital area, including auxiliaries, nursing technicians and nurses, who worked in the Inpatient, Urgency and Emergency, Adult Intensive Care Unit, Coronary Unit and Isolation. Data were collected through a semistructured auto-fill instrument to characterize the population and also by two other instruments: the Hospital Anxiety and Depression Scale and the Perceived Stress Assessment Scale. The participant\'s hair samples were collected using the Diasource specific kit for Cortisol ELISA (Enzyme-Linked Immunosorbent Assay - KAPDB290). The research was previously approved by the Research Ethics Committee, protocol CAAE: 55839216.5.0000.5393. The prevalence of depression taken as the basis for the sample calculation was assumed to be unknown, the prevalence value of 50%, which results in a sample size that contemplates any value of P. The program of the sample calculation was R version 3.1.2. The statistical tests used were the Pearson Chi-Square, Chi-square, Spearman-?, Mann-Whitney and Kruskal Wallis Correlation Coefficient. Results: Descriptive analyzes (n = 164) found that the majority of the interviewees were nursing technicians (62.19%), female (80.49%), aged 31-50 years (57.93%), married / with partner (44.51%), working in the profession between 0 and 10 years (76.22%), working in hospitalization units (48.17%), day shift (52.44%), at weekends (93.9%), non-smokers (81.7%) and consumers of alcoholic beverages (60.4%). In relation to cortisol (n = 161), 47.8% showed a level above normal; 23.8% had moderate stress, 20.1% had high stress and 12.8% had very high stress; and 44.51% presented Anxiety and 24.39% Depression. According to the statistical tests, the cortisol levels did not influence the perceived Stress, because there was no difference in its total, both for the workers that present or not Anxiety. There were no statistical differences between the hospital sectors of the critical and non-critical areas and also the levels of Stress, Anxiety and Depression among the categories of workers. Conclusion: The hypotheses were partially confirmed, as there were alterations in capillary cortisol levels, which were higher than recommended. The study brought important contributions related to the biomarker capillary cortisol, to Stress, Anxiety and Depression. Hospital institutions should offer programs with interventions in the occupational area and preventive measures to alleviate mental health changes in nursing workers

<p>Healthcare staff working within emergency care must often handle extreme and emotional situations. Most of the certified registered nurse anesthetists (CRNAs) will some time during their professionally active lives confront with events that are difficult to handle and for some these events will lead to more or less serious traumatic distress and in certain cases develop post traumatic stress disorder symptoms. Many studies demonstrate that staff working in similar stress related environment runs larger risk in order to get stress related symptoms. The aim with the study was to describe the experiences around difficult work related events among CRNAs. The study was implemented as a descriptive study with qualitative run-up via a questionnaire with open questions relevant for the study's aim. Seven CRNAs participated in the study and the result was processed with a qualitative method for content analysis. The result showed that the majority of the participants experienced bothersome events at work but most of them managed not to develop future harmful experiences from these events. The participants could clearly describe their experiences around difficult events and also provide what type of processing that fits best for them. This study gives an increased understanding how CRNAs experience difficult work related events.</p> / <p>Sjukvårdspersonal som arbetar inom akutsjukvård måste ofta handskas med extrema och känsloladdade situationer. This qualitative study explored the challenges and stressors that are experienced by addiction counsellors in providing substance abuse treatment. In order to tap into rich and descriptive information on occupational stress in the addictions field, a modified grounded theory focused on conceptual ordering (Corbin & Strauss, 1998) was used for the research design. Participant sample consisted of 10 addiction counsellors who were recruited using purposeful selection from addiction treatment centres in Ontario, Canada. Data collection was conducted through face-to-face interviews using semi-structured, open-ended questions. Four major categories emerged from the data analysis using systematic thematic analysis: 1) Sources of Stress, 2) Signs and Consequences of Stress, 3) Stress Management and Intervention, and 4) Resiliency Against Stress. Results from this study contribute to the understanding of occupational stress in addictions counsellors. Implications for addiction organizations, educators, and counsellors are discussed.
