Fysioterapeuters arbete med följsamhet till hemträning hos patienter med Artros : En kvalitativ intervjustudie kring användning av beteendeförändringstekniker

Bakgrund: Artros är den vanligast förekommande ledsjukdomen. I den fysioterapeutiska behandlingen ingår fysisk träning där patienterna får individuellt anpassade hemträningsprogam. Följsamheten till dessa har dock visat sig låg, vilket kan åtgärdas med beteendeförändring. Fysioterapeuten kan bistå med stöd genom användning av beteendeförändringstekniker. Syfte: Att beskriva fysioterapeuters arbete med följsamhet till hemträningsprogram med hjälp av beteendeförändringstekniker hos patienter diagnosticerade med artros. Metod: Kvalitativ deskriptiv design med semistrukturerade intervjuer. Sju fysioterapeuter rekryterades genom bekvämlighetsurval. Data analyserades utifrån kvalitativ innehållsanalys med deduktiv ansats där data matchades med redan framtagna kategorier och underkategorier utifrån Michie et al. (2013) taxonomi med beteendeförändringstekniker. Resultat: Informanternas beskrivningar av arbetet med följsamhet till hemträning matchades med 38 beteendeförändringstekniker indelade i 14 kluster: Goals and planning, Feedback and monitoring, Social support, Shaping knowledge, Natural consequences, Comparison of behavior, Associations, Repetition and substitution, Comparison of outcomes, Reward and threat, Regulation, Antecedents, Identity, Self-belief. Slutsats: Studien visade hur informanterna tillämpar beteendeförändringstekniker i arbetet med följsamhet till hemträning. Inget framstående beteendeförändrande arbetssätt kunde påvisas. En stor variation beteendeförändringstekniker beskrevs. Några arbetssätt tenderade innehålla beteendeförändringstekniker som hänger ihop och kommer till följd av varandra. Andra arbetssätt innehöll enskilda tekniker som används i syfte att möta individuella behov och hinder till träning. / Background: Osteoarthritis is the most common joint disease. The physiotherapeutic treatment includes physical exercise where the patients receive individually adapted home exercise programs. However, adherence to these has proven to be low, which can be remedied with behavioral change. The physiotherapist can provide support by using behavior change techniques. Aim: To describe physiotherapists' work with adherence to home exercise programs using behavioral change techniques in patients diagnosed with osteoarthritis. Method: Qualitative descriptive design with semi-structured interviews. Seven physiotherapists were recruited through a sample of convenience. The data was analyzed based on qualitative content analysis with a deductive approach where the data was matched with already developed categories and subcategories based on Michie et al. (2013) taxonomy with behavior change techniques.  Results: The informants’ descriptions of the work with adherence to home training were matched with 38 behavior change techniques divided into 14 clusters: Goals and planning, Feedback and monitoring, Social support, Shaping knowledge, Natural consequences, Comparison of behavior, Associations, Repetition and substitution, Comparison of outcomes, Reward and threat, Regulation, Antecedents, Identity, Self-belief. Conclusion: The study showed how informants applied behavior change techniques in their work with adherence to home training. No prominent behavioral change approach could be demonstrated. A wide variety of behavioral change techniques were described. Some ways of working tended to include behavioral change techniques that interrelate and result from each other. Other ways of working contained individual techniques used to meet individual needs and training barriers.

Dyslexia is a difficulty in learning to read and write, with many implications for the children because of emotional and behavioral problems. The suspicion that they might experience problems with sensory integration as well, is confirmed in the literature. The purpose of sensory stimulation in Gestalt playtherapy is to raise the level of awareness of themselves and in this research, playtherapy was done with three primary school children with dyslexia. Information was further obtained through a literature study, semi-structured interviews and observations. It was clear that the sensory stimulation was part of the therapeutic sessions and led to thoughts, feelings, and behaviour. The qualitative characteristics which the sensations had for the children were observed and confirmed by the literature. Conclusions and recommendations were made according to the objectives of the study and this research made an important contribution to understanding the sensory awareness of children with dyslexia. Opsomming Disleksie is `n leergeremdheid met baie implikasies vir die kinders en wat kan lei tot emosionele en gedragsprobleme omdat hulle nie altyd aanvaar word deur hul portuurgroep nie. Die vermoede dat kinders met disleksie ook probleme ondervind met sensoriese integrasie, is in die literatuur bevestig. In hierdie navorsing is Gestaltspelterapie gedoen met drie laerskool kinders met disleksie om die kinders se bewussynsvlak van hulself te verhoog. Inligting is verder ingesamel deur `n literatuurstudie, waarneming en semi-gestruktureerde onderhoude met kundiges en ouers. 全員品質管理與360度回饋知覺、滿意、行為改變、與績效提升之實證研究 / The longitudinal field study of TQM with 360 degree feedback perception, satisfaction, behavior change and Performance improvement

本研究為一縱貫性之實證研究,主要目的是透過分析兩年360度回饋的資料,進一步探討全員品質管理才能對360度回饋公平知覺、滿意度、行為改變、與績效提升的影響。 本研究樣本為國內某建築業之初階與中階主管,第一年總計62位,第二年總計92位。研究結果顯示,全員品質管理才能與360度回饋公平知覺、滿意度、行為改變意圖,皆達顯著正相關。此外,雙因子變異數分析結果顯示,全員品質管理才能的高低會影響他評結果、目標達成、以及領導效能的改變,如進一步以相依樣本t考驗進行事後比較,則發現全員品質管理才能低分組兩年間績效改變的進步幅度大於高分組。 最後,本研究進一步指出研究之限制,與後續研究之建議、企業實行360度回饋之實務建議。

Designing for Empathy in Elderly Care : Exploration of Opportunities to Deliver Behaviour Change Interventions through mHealth Applications, to Promote Empathic Behaviour in Elderly Home Care Nursing Assistants

Background The Swedish population is ageing quickly and the system for elderly home care is under increasing pressure. Staff turnover is high, nursing assistants are reporting stress, and employers have to recruit staff lacking sufficient experience. These factors are barriers to empathic care, considered essential to patient health outcomes. Elderly care should rely on cognitive empathy, be other-oriented and improve the client’s situation based on contextual understanding. There is a need for education and support for nursing assistants, so that they can provide empathic care. Purpose The thesis explores empathy as a skill in elderly home care to identify opportunities of promoting empathy in the client-nursing assistant interaction, by means of behaviour change interventions delivered through an mHealth application that nursing assistants already use at work. Method A group interview was conducted with six nursing assistants from four elderly home care organisations in a Swedish municipality, to learn about their experience of empathy at work, and factors affecting their ability to give empathic care. The respondents were using the same mHealth application to get and provide information about client visits. The Behaviour Change Wheel framework was used to analyze behavioural drivers of empathic care in elderly home care. Results Influences on empathic behaviour was identified in all 14 domains in the Theoretical Domains Framework. 13 target behaviours, 7 Intervention Functions and 45 Behaviour Change Techniques were suggested as suitable candidates to investigate for intervention development. Conclusion Empathy seems possible to promote through resource-efficient digital behaviour change interventions. Future studies may use this work as a starting point for development of interventions to promote empathic behaviour in elderly care.

Application of persuasive systems design for adopting green information systems and technologies

Abstract Critical environmental situation requires academics and practitioners of various disciplines join efforts at planning sustainable development of the society. In information systems research, Green Information Systems and Technologies domain is the one that directly contributes to strengthening environmental values of individuals, communities, and organizations. Abundance of technology influences people most of the time. Although this influence is not always positive, research on persuasive technologies and behavior change support systems searches for the beneficial ways to utilize the impact of information systems on the daily activities of individuals. Ability to assist with encouraging certain behaviors is a defining characteristic of many contemporary systems and devices. Building on the knowledge obtained from the health domain, this dissertation investigates using persuasive technologies for fostering pro-environmental behavior change. Theoretical frameworks of the articles included in the dissertation are based primarily on the Persuasive Systems Design (PSD) model, accompanied by the other prominent socio-psychological theories commonly utilized in information systems research. This dissertation focuses on finding out how to encourage people to adopt and continue using mobile applications designed to assist with acquiring sustainable habits and behaviors. The dissertation is composed of the historical analysis, systematic literature review, and four experimental studies that explore different aspects of persuasive Green IS/IT. Besides analyzing existing research using PSD model as a tool for evaluation, new empirical studies provide fresh insights on Green IS/IT designed for behavior change. Addressed topics include technology adoption and continuance intention, perceived persuasiveness, self-disclosure and risk, endogenous motivations, gamification, and cognitive absorption. Conducted statistical analyses investigate relationships among PSD constructs and other related concepts to discover factors that can convince people initiate and continue using Green IS/IT to develop or maintain environmentally sound practices. Overall, the results reveal a high potential of the PSD model to become a vehicle for enhancement of the existing Green IS/IT. The dissertation provides implications for both academics and practitioners as well as suggestions for the future evolvement of persuasive Green IS/IT research. / Tiivistelmä Ympäristön ja ilmaston vakava tilanne vaatii kaikilta osapuolilta kestävää kehitystä tukevaa toimintaa. Niin sanotut ‘vihreät’ tietojärjestelmät ja informaatioteknologiat pyrkivät vaikuttamaan ihmisten käyttäytymiseen vahvistamalla yksilöiden, yhteisöjen ja organisaatioiden ympäristöarvoja ja niiden mukaista käyttäytymistä. Yltäkylläinen teknologiatarjonta vaikuttaa käyttäytymiseemme koko ajan. Vaikka tämä vaikutus ei aina olekaan positiivista, vakuuttavien ja suostuttelevien teknologioiden sekä käyttäytymisen muutosta tukevien järjestelmien tutkimus pyrkii etsimään niitä hyödyllisiä keinoja, joilla tietojärjestelmien avulla voidaan vaikuttaa myönteisesti jokapäiväiseen elämäämme. Monien nykyaikaisten järjestelmien ja laitteiden perusominaisuutena voidaankin nykyään pitää niiden kykyä rohkaista ja kannustaa käyttäjiä myönteiseen kohdekäyttäytymiseen. Tämä väitöskirja rakentuu erityisesti terveystoimialalta omaksuttuun suostuttelevia teknologioita koskevaan tutkimustietoon ja pyrkii soveltamaan tätä ympäristöystävällisen käyttäytymisen vahvistamiseen. Tämän väitöstutkimuksen johtavana teoreettisena viitekehityksenä toimii Persuasive Systems Design -malli (PSD). Sen lisäksi hyödynnetään joitakin muita tietojärjestelmätutkimuksessa hyödynnettyjä sosiaalipsykologian teorioita. Väitöskirja keskittyy tarkastelemaan, miten voidaan kannustaa ottamaan käyttöön mobiilisovelluksia, jotka on jo alun perin suunniteltu tukemaan kestävän kehityksen kannalta myönteistä käyttäytymistä ja käyttäytymisen muutosta, sekä jatkamaan niiden käyttöä. Väitöskirja koostuu aihealueen historiallisesta analyysistä ja systemaattisesta kirjallisuuskatsauksesta sekä neljästä kokeellisesta tutkimuksesta, jotka tutkivat ’vihreiden’ tietojärjestelmien ja informaatioteknologioiden vakuuttavia ja suostuttelevia ohjelmisto-ominaisuuksia. Sen lisäksi että olemassa olevaa kirjallisuutta on analysoitu PSD-mallia käyttäen, samaan malliin pohjautuvat empiiriset tutkimukset tuottavat tuoreita oivalluksia käyttäytymisen muutosta tukevaan suunnitteluun liittyen. Tutkimuksessa tarkastellaan teknologian käyttöönottoa, aikomuksia jatkaa teknologian käyttöä, koettua vakuuttavuutta ja suostuttelevuutta, sensitiivisten tietojen jakamista ja siihen liittyviä riskejä, sisäsyntyistä motivaatiota, pelillistämistä sekä kognitiivista absorptiota. Tilastollisten analyysien avulla tarkastellaan PSD-mallin ydinkäsitteiden ja muiden konstruktien välisiä suhteita, jotta tunnistettaisiin niitä tekijöitä, joiden avulla voidaan vaikuttaa hyödyllisten ‘vihreiden’ tietojärjestelmien ja informaatioteknologioiden käyttöönottoon ja käytön jatkamiseen, ja yleisemmin kannustamaan ympäristöystävällisiin käytänteisiin. Tutkimustulokset osoittavat PSD-mallin merkityksen kestävää kehitystä tukeville tietojärjestelmille ja informaatioteknologioille. Suoritetun tutkimuksen perusteella ehdotetaan teemoja jatkotutkimusta varten. Väitöstutkimuksen tuloksilla on merkitystä sekä tieteenharjoittajille että käytännön soveltajille.

One Sound Bite at a Time: Examining the Discourse of the Representation of People Living with HIV/AIDS on an Entertainment-Education Drama RockPoint 256

The objective of this thesis is to examine the meaning of the language used in an Entertainment-Education (E-E) radio serial drama RockPoint 256 (RP256) and its representation of People Living with HIV/AIDS (PLHA) and HIV/AIDS. The theoretical framework that is used to guide this study is the Cultural Studies perspective. Using Cultural Studies is instrumental to a study such as this that looks to find the source of meaning in RP256’s discourse (Hall, 1993, 105). The methodological research design used is discourse analysis that examines the oral and written data of RP256. Discourse analysis “focuses on the way language is used, what it is used for, and the social context in which it is used” (Punch, 1998, 226). There were two types of discourse analyses used. Gee’s (1999) discourse analysis is used to analyse the linguistic nature of the texts at a micro level while, Fairclough’s (1989) discourse analysis is used to observe the overarching meaning of the discourse found in RP256 at the macro level. The analysis of the data indicates that the representation of PLHA is a product of the societal and cultural markers that are a result of ideological labels given to HIV/AIDS and PLHA. The study concludes that PLHA are ostracised, stigmatized, live in poverty and are mostly women. The societal and cultural markers indicate that HIV/AIDS is still viewed as a plague and as a result PLHA are often silenced, marginalized, and discriminated against in Uganda.

處於服務大量興起的時代,創新服務成為企業關注的熱門議題且如雨後春筍般發展。然而,一個新服務的成功並不在於它的獨特性,而是取決於它的使用率與普及化。當中,觀察到有趣的現象,人們對於新服務的使用行為取決於自身的認知與社會影響力相互作用而產生;社會影響力在個人行為表現上扮演關鍵的因素,引導個人了解認知差距並提供建議改善認知以促進個人產生使用意圖。在本研究中,整合行為改變模型(TTM)、傳播理論、框架效應而發展出一套理論架構,並應用視覺化方式呈現資訊提供個人快速理解重點,重塑個人對於新服務的認知,且引起他們願意使用新服務的想法。基於認知重塑的目標,本研究貢獻為設計一套能輔助個人進行行為意向改變與加速新服務使用率的系統。 / In the service growing era, service innovation becomes a popular topic on the businesses who want to develop innovative services for people. However, the success factor is not on how special a service is; but rather, depends on how much utility and popularity of the service. This study has observed the interesting phenomenon that people's behavior performance depends on their cognition and the social influence playing an important role on affecting the intention. In order to resolve this condition, this study connects the transtheoretical model (TTM) of behavior change, communication theories, and frame effect model to develop a framework and apply visualization to provide people with the integral information that can reframe their cognition to enhance their intention to accept the new service. Based on the cognition reframing, this study develops an artifact the can assist people to exercise the cognition adjustment that can reinforce the behavior intention change and facilitate the usage of new service.

An ethnographic investigation into Mongolian management in the context of cultural and institutional changes

This thesis investigates the understanding and practices of contemporary Mongolian management since the 1990s. In particular, it focusses on the shared experiences of local managing practitioners in Mongolia by exploring the conceptions of a manager, management, and managerial roles from the participants’ perspective and the contextual influencing factors on their understanding. Since the 1990s Mongolia has undergone a series of cultural and institutional changes in relation to its political, economic and societal development. The country has fluctuated between having the fastest growing economy and the world’s worst performing currency. Mongolia itself has become the land of opportunity for many by attracting foreign direct investment, however, the knowledge of local management practice is as yet little understood as there have been no academic or empirical studies conducted in English before. Therefore, this research aims to build an understanding of the concept of management in Mongolia by examining the narratives of thirty five local managers in relation to their experiences during and after the socialist period. Moreover, it investigates the contextual influencing factors from practitioners’ perspectives with an ethnographic approach. This qualitative study draws on interviews with three groups of local managers in Mongolia, who are described as socialist-era, transitional-era and non-native managing practitioners. There are some similarities and differences amongst these identified groups, but each was distinguishable by their formal training, work ethic and management approach. Furthermore, this research found that the intertwining contextual factors of a nomadic cultural heritage, socialist legacy, and the pressures of the current economic and societal changes and political interference influence management thinking in equal measure in contemporary Mongolia. The importance of this study lies in its theoretical and empirical contributions. By evaluating the relationship between classical management literature and indigenous management concepts with a focus on the varieties of contextual factors, this study attempts to provide an original insight into non-Western management practices. It aims to extend the current theories of crossvergence, indigenous management studies, and understand the nature of managerial work in a cross-cultural context. By carrying out the first academic study to examine Mongolian management perspectives in English, it contributes empirically to global management knowledge, and to the local business community.
