Étude des déterminants moléculaires associés à l’intolérance orthostatique dans la pathogenèse de l’encéphalomyélite myalgiqueLeveau, Corinne 12 1900 (has links)
L’encéphalomyélite myalgique (EM) est une maladie complexe, multi-systémique et débilitante, dont l’étiologie est inconnue. D’une personne atteinte d’encéphalomyélite myalgique (PAEM) à l’autre, les symptômes varient en fréquence et en sévérité créant ainsi une grande hétérogénéité clinique entre les individus. Un sous-groupe de PAEM vivent des épisodes d’intolérance orthostatique (IO) ou vivent avec une comorbidité de syndrome de tachycardie orthostatique posturale (POTS), deux conditions qui sont mal comprises. Le malaise après-effort (PEM), un des symptômes phare de l’EM, survient après une activité physique ou mentale minimale. Le malaise après-effort entraîne une dégradation générale de l’état de l’individu, peut entraîner une exacerbation des autres symptômes et va durer de plusieurs heures à plusieurs jours. Chez les individus souffrant de POTS ou d’IO, le malaise après-effort peut déclencher des épisodes d’intolérance orthostatique.
Le gène SLC6A2 codant pour le transporteur de norépinephrine NET a été identifié comme potentiel mécanisme dans pathophysiologie du POTS, tout comme les protéines impliquées dans la vasodilatation, comme la thrombospondine-1 (TSP-1). Notre laboratoire a identifié un panel de onze microARN (miARN) exprimés différentiellement chez les PAEM. Parmi ceux-ci, le miR-150-5p a comme cible prédite SLC6A2. Notre hypothèse était qu’une plus grande expression du miR-150-5p après un effort ou qu’une chute de thrombospondine-1 pourrait induire une vasodilatation soudaine contribuant aux symptômes d’IO ou de POTS.
Nous avons mesuré les niveaux plasmatiques du miR-150-5p et de TSP-1 avant (T0) et après (T90) l’induction du malaise après-effort chez des PAEM avec POTS/IO (n = 20), chez des PAEM sans POTS/IO (n = 117) et chez des témoins sédentaires associés pour le sexe et l’âge (n = 48). Nous avons démontré que les sujets atteints de POTS/IO avaient des niveau plus importants du miR-150-5p et des symptômes plus sévères. Finalement, nous avons également utilisé la veste intelligente Hexoskin (Carré Technologies Inc., Montreal, Qué., Canada) pour suivre un sous-groupe d’individus (n = 10) sur une plus longue période après l’induction du malaise après-effort. Avec cet outil, nous avons pu monitorer les symptômes au quotidien, permettant un meilleur suivi clinique de ces patients.
Ce projet de maîtrise a permis une meilleure compréhension de la pathophysiologie de l’EM et de celle du POTS. / Myalgic encephalomyelitis (ME) is a complex chronic disease with debilitating smyptoms and unknown etiology. Symptoms vary in frequency and severity from a person with ME (PwME) to another, thus creating a highly clinically heterogeneous patient population. Some PwME also experience orthostatic intolerance (OI) episodes or live with a comorbidity of postural orthostatic tachycardia syndrome (POTS), two conditions that are not well understood. Post-exertional malaise (PEM) causes patients to experience a worsening of their symptoms following an effort, whether it be physical or mental. PEM can last from a few hours to several days. In PwME with POTS/OI, PEM can trigger orthostatic intolerance episodes.
SLC6A2 is a gene coding for the norepinephrine transporter NET. Its contribution to the POTS pathophysiology has been mentioned several times in literature. A biochemical milieu prone to vasodilation was also reported as a contributing element to POTS pathophysiology. Recently, our laboratory published an article identifying a panel of eleven microRNAs (miRNAs) differentially expressed in PwME. Among these miRNAs, miR-150-5p has been predicted to target SLC6A2. Our hypothesis was that higher expression of miR-150-5p following an effort or a decrease in circulating thrombospondin-1 (TSP-1) inducing vasodilation could contribute to POTS/OI symptoms.
We measured circulating levels of miR-150-5p and TSP-1 before (T0) and after (T90) PEM induction in PwME (n = 117), PwME with POTS/OI (n = 20) and age and sex matched sedentary controls (n = 48). We demonstrated that PwME with POTS/OI have higher levels of miR-150-5p at both T0 and T90, while also having more severe symptoms. Furthermore, we used the connected vest Hexoskin (Carré Technologies Inc., Montreal, Qué., Canada) to follow a subgroup (n = 10) of patients for a longer period following PEM induction. With this tool, we were able to monitor symptoms on a daily basis, allowing better clinical follow-up. Overall, this project allowed better understanding of ME and POTS’ pathophysiology.

Le vertige (V), l'étourdissement (E) et l'instabilité (I) sont des symptômes fréquents, traditionnellement considérés comme ayant des étiologies vestibulaires et extra-vestibulaires différentes. La prévalence de chaque symptôme et la manière dont ils sont liés entre eux ainsi qu'avec la migraine, l'agoraphobie, la susceptibilité au mal des transports (SMT), le malaise vagal (MV), l'anxiété/dépression (AD) et la prise de médicaments, ont été l'objet de cette étude. Un questionnaire auto-administré a été complété par 2897 adultes de la population lorraine, âgés de 18 à 86 ans, dont 1471 femmes. La prévalence sur un an du V était de 48,3%, celle de l'I de 39,1% et celle de l'E de 35,6%. Les VEI étaient corrélés entre eux et apparaissaient en général en diverses combinaisons (69,4%) ; 90% des épisodes de VEI avaient une durée inférieure ou égale à 2 mn et une fréquence < 1/mois. Les trois symptômes étaient similaires concernant leur prédominance féminine, le profil temporaire des épisodes et le lien avec les chutes et les nausées. Les VEI augmentaient avec le nombre de médicaments indépendamment de l'âge. Chaque VEI était corrélé avec chaque co-morbidité. Les facteurs de risque étaient pour l'agoraphobie : AD>sexe féminin>VEI, pour la SMT : sexe féminin > AD > migraine. En ajustant sur les trois VEI entre eux, le V reste un facteur de risque significatif pour chaque co-morbidité, suggérant que le V est un symptôme aussi peu spécifique que les deux autres et que les VEI représentent un spectre de symptômes résultant de diverses causes / Vertigo (V), dizziness (D) and unsteadiness (U) are common symptoms traditionally considered to result from different kinds of vestibular and non-vestibular dysfunctions. The prevalence of each symptom and how they relate to each other and to migraine, agoraphobia, motion sickness susceptibility (MSS), vaso-vagal episodes (VVE) and anxiety-depression (AD) was the object of this population-based study in north-eastern France. A self-administered questionnaire was returned by 2987 adults (age span 18-86 years, 1471 women). The 1-year prevalence for V was 48.3%, for U 39.1% and for D 35.6%. The VDU symptoms were correlated with each other, occurred mostly (69.4%) in various combinations rather than in isolation, less than once per month, and 90% of episodes lasted < or = 2 minutes and occurred < 1/month. The three symptoms were similar in terms of female predominance, temporary profile of the episodes and their link to falls and nausea. Symptom associations including I and symptom episodes of >1 hour increased the risk of falls. The number of drugs taken increased VDU even when controlling for age. Each VDU symptom was correlated with each co-morbidity. The risk factors were for agoraphobia: AD>female gender>VDU; for MSS: female gender>AD>migraine. Logistic regressions controlling for each vestibular symptom showed that V correlated with each co-morbidity, suggesting that V is a symptom as unspecific as the other two and that the three symptoms represent a spectrum resulting from a range of mechanisms or disorders

Ferenczi to D. W. Winnicott which allows other referrals to this theme approach, especially in its current expression. This thesis aims to investigate what psychoanalysis can do against the discontent in the current school using as tools the genealogy of the disciplinary power and the biopower in M. Foucault to understand the child-student category emergency, the standard and standardization concepts to discuss the medicalization and pathologizing processes of children and youth in the school surroundings in the control society and, finally, D. W. Winnicott s thought and his psychic maturing theory and its developments. It is still aimed to the property of this theory of the early subjective constitution and its developments, which allow to think the forms of insertion into the culture, to the different referrals of the discontent in the current school, understood as creative living disempowerment.

Les effets du genre sur la charge mentale : une étude sur le mal-être en milieu professionnelLanteigne, Amélie 04 1900 (has links)
Ce projet de mémoire a pris forme dans le but d’étudier comment l’effet du genre module la charge mentale au travail, en s’intéressant à l’impact des rapports de genre sur le sentiment de surcharge mentale et comment ils influent sur l’expression du mal-être individuel. L’analyse d’entretiens réalisés auprès de sept femmes et cinq hommes révèle divers facteurs contribuant à l’impression d’être mentalement surchargé au travail : les contraintes temporelles ; l’imprévisibilité ; la précarité au niveau économique et des conditions de travail ; le caractère exigeant des responsabilités professionnelles ; les contradictions du travail ; l’insuffisance du soutien organisationnel ; le manque de reconnaissance au travail ; le faible soutien social ; ainsi que la difficulté à concilier travail et famille. Ces multiples éléments colorent le discours des participants à la fois selon leur genre individuel et selon le type de métier qu’ils occupent : ceux-ci s’inscrivent dans une division relativement bien définie entre métiers majoritairement féminins et métiers majoritairement masculins. Suite à l’analyse des entretiens, il a été possible de cerner des tendances spécifiques à chaque type de métier, qui s’observent dans la relation entre demande psychologique et latitude décisionnelle. Appliqués au modèle de Karasek (1979), les résultats indiquent une conjoncture surchargeante pour les participants occupant un métier à prédominance féminine, où apparaît un cadre organisationnel oppressant. Enfin, de par les effets pernicieux de la non-valorisation du travail traditionnellement féminin et du travail reproductif associé à la gestion familiale, les résultats de cette présente recherche suggèrent que les femmes sont particulièrement à risque d’être surchargées mentalement et de vivre un profond mal-être. / This thesis was developed with the aim of studying how the effect of gender modulates mental burden at work, focusing on the impact of gender relations on the feeling of mental overload and how they influence the expression of individual malaise. The analysis of interviews with seven women and five men reveals various factors contributing to the impression of being mentally overloaded at work: time constraints; unpredictability; economic precariousness and working conditions; the demanding nature of professional responsibilities; the contradictions of work; inadequate organizational support; lack of recognition at work; weak social support; as well as the difficulty of reconciling work and family. These multiple elements colour the participants' discourse both according to their individual gender and according to their type of occupation : they are part of a relatively well-defined division between predominantly female occupations and predominantly male occupations. Following the analysis of the interviews, it was possible to identify trends specific to each type of occupation, which are observed in the relationship between psychological demand and decision latitude. Applied to the model of Karasek (1979), the results indicate an overburdening situation for participants in a predominantly female occupation, where an oppressive organizational framework emerges. Finally, because of the pernicious effects of the non-valorization of the traditionally female work and of the reproductive work associated with family management, the results of this research suggest that women are particularly at risk of being mentally overloaded and experiencing a profound malaise.

Spillover and species interactions across habitat edges between managed and natural forestsFrost, Carol Margaret January 2013 (has links)
We are currently faced with the global challenge of conserving biological diversity while also increasing food production to meet the demands of a growing human population. Land-use change, primarily resulting from conversion to production land, is currently the leading cause of biodiversity loss. This occurs through habitat loss, fragmentation of remaining natural habitats, and resulting edge effects. Land-sparing and land-sharing approaches have been discussed as alternative ways to engineer landscapes to mitigate biodiversity loss while meeting production objectives. However, these represent extremes on a continuum of real-world landscapes, and it will be important to understand the mechanisms by which adjacent land use affects natural remnant ecosystems in order to make local land-management decisions that achieve conservation, as well as production, objectives.
This thesis investigates the impact of juxtaposing production and natural forest on the community-wide interactions between lepidopteran herbivores and their parasitoids, as mediated by parasitoid spillover between habitats. The first and overarching objective was to determine whether herbivore productivity drives asymmetrical spillover of predators and parasitoids, primarily from managed to natural habitats, and whether this spillover alters trophic interactions in the recipient habitat. The study of trophic interactions at a community level requires understanding of both direct and indirect interactions. However, community-level indirect interactions are generally difficult to predict and measure, and these have therefore remained understudied. Apparent competition is an indirect interaction mechanism thought to be very important in structuring host-parasitoid assemblages. However, this is known primarily from studies of single species pairs, and its community-wide impacts are less clear. Therefore, my second objective was to determine whether apparent competition could be predicted for all species pairs within an herbivore assemblage, based on a measure of parasitoid overlap. My third objective was to determine whether certain host or parasitoid species traits can predict the involvement of those species in apparent competition.
My key findings were that there is a net spillover of generalist predators and parasitoids from plantation to native forest, and that for generalists, this depends on herbivore abundance in the plantation forest. Herbivore populations across the edge were linked by shared parasitoids in apparent competition. Consequently, an experimental reduction of herbivore density in the plantation forest changed parasitism rates in the natural forest, as predicted based on parasitoid overlap. Finally, several host and parasitoid traits were identified that can predict the degree to which host or parasitoid species will be involved in apparent competition, a finding which may have extensive application in biological control, as well as in predicting spillover edge effects.
Overall, this work suggests that asymmetrical spillover between production and natural habitats occurs in relation to productivity differences, with greater movement of predators and parasitoids in the managed-to-natural forest direction. The degree to which this affected species interactions has implications for landscape design to achieve conservation objectives in production landscapes.
