'We’re All Getting Older You See, and Things Do Change, Don’t They?’ An Ethnographic Study of Disruption and Continuity in the Daily Lives of Couples Living with Dementia and Co-morbidities

Most people with dementia live in the community with a family member, commonly a spouse. Together they engage in identity redefinition to maintain continuity. Many people living with dementia also have co-morbidities. The aim of this study was to provide a better understanding of the influence of co-morbidities on the lived experience of couples and to provide knowledge to improve services. This had not been researched before. Drawing on the dialectic relationship between the body, habitus, environment and common sense from Bourdieu’s theory of practice (1977; 1990) combined with identity theory as described by Burke and Stetts (2009) I conducted an ethnographic study with five couples over a six-month period. The resulting data were analysed using a framework approach and are presented using case studies to illustrate key points. Drawing up on the data I developed an identity perspective which provides a better understanding of these couples’ daily life experiences taking into consideration the contextuality of people’s multiple identities, experiences, care and support needs and their interaction with the environment and community. My findings illustrate how people with dementia and co-morbidities and their spouses negotiate their identity in daily life in order to continue their daily life routine and cope with health conditions. Furthermore, these identity negotiations influence the acceptance of the diagnoses of dementia, the access to care, services and information and the experience of dementia, stigma and co-morbidities in daily life. Implications include a presentation of the limitations of current concepts of embodied selfhood and the Aging in Place policy for people with dementia. It points to the potential of the identity perspective to shape policy, services and care practice consistent with couples’ lived experience and their needs and preferences. / The Alzheimer's Society

台灣地區大陸配偶社會適應問題之研究

自開放探親以來,兩岸交流日趨熱絡,截至民國九十四年底陸續來台團聚及申請居留、定居的大陸配偶已逾廿三萬人,成為新興移入人口的主流。「兩岸婚配」的盟合大多係由台灣男性主動認識大陸女子、或經由親友介紹、及婚姻仲介等方式而結婚,大陸配偶透過「跨海婚姻」的歷程來到台灣,在陌生的社會民情與生疏的家庭環境中試圖展開新生活,她們無不努力地學習並詮釋新的角色;然因兩岸的社經環境、風俗習慣,人民的成長背景及價值觀念等文化之差異與隔閡,建立自己的同儕生活上有所困難,加上在台灣取得社會資源的不易,使其與台灣社會網絡互動貧乏,只能依賴婚姻的狀況下更顯得被孤立,而成為社會的弱勢族群,且因人數眾多已形成嚴重的社會問題。 由於大陸配偶來台者日眾,在相關的社會新聞中,造成台灣民眾常抱持著偏執與異樣眼光看待她們,總將其視為外來者或麻煩製造者而產生了許多誤解,使她們在異鄉的生活適應上備感艱辛,諸如她們漂洋過海的婚姻動機為何?來台後對於兩岸的生活習慣與價值觀念之差異要如何適應?又將如何與台灣的社會相連結?對於國家與社群的認同為何?微視她們真正的心聲是值得我們作抽絲剝繭的探究。本文針對中國移民女性來台的社會適應問題進行研究,讓現實生活中的大陸配偶自己發聲,筆者實地去瞭解她們的生活底層,聆聽她們的心聲、感受與生活經驗,試從其自身的敘述中來勾勒出她們的「台灣經驗」,而非新聞媒體上所建構與杜撰的故事,或是三姑六婆們街頭巷尾的竊竊私語。 / Ever since Taiwan and PRC opened door for citizens to visit their relatives across Taiwan Straits, cross-border interactions from both sides are becoming more frequent. According to statistical data released by Taiwan government, more than 230 thousand PRC immigration requests to Taiwan have been granted by the end of 2005. In consequence, PRC residents are becoming the mainstream in Taiwan immigration population. Typically, cross-border marriage took place through matchmakers or relatives’ introduction or Taiwanese males actively create opportunities to know PRC female. Thereby female is the dominant gentile in PRC immigrant population. Due to different value systems, and vast cultural gap, it is not easy for PRC spouses to quickly adapt to the new environment and families. In addition, for lack of proper access to Taiwan social networks, PRC immigrants are usually isolated and speechless. And the issues become more severe as PRC immigrants increases in Taiwan. From time to time, social incidents caused by PRC immigrants are mistakenly reported or even exaggerated by public media that lead to biased misunderstanding from Taiwanese pollution. And there’s virtually no chance for PRC spouses to speak up. In this paper, we will focus primarily on the following issues: what are the motivations of PRC spouses to come to Taiwan, how they adapt themselves to make up the cultural gap and the new value system, as well as how can they link up with Taiwan societies. Rather than referencing to media reports that could possibly be distorted, we chose to collect first hand experiences by face to face interviews with PRC spouses. Let’s listen to their genuine voice about their “Taiwan experiences”.

The French Expatriate Assignment: Helping Accompanying Spouses to Adapt by Assuming the Role of Anthropologist

A common practice of multinational companies is to temporarily transfer employees to international locations in order to exchange technology, launch new ventures, and facilitate communication within the company. To do this, multinational companies must make a number of decisions regarding their expatriate programs. Even though international companies would rather stay focused on the professional performance of their employees without having to "meddle" in personal and family affairs, recent research has shown that the adaptation of the accompanying spouse is one of the most critical factors in expatriate assignment success or failure. By studying the available literature on expatriate spouse adaptation, coupled with conducting my own exploratory research with American expatriate spouses currently living in France, I was led to the conclusion that adequate, pre-departure preparation is indispensable to an expatriate spouse's successful cross-cultural adaptation. To improve the efficacy of pre-departure training, I propose providing expatriate spouses with personal trainers who will teach them to adopt a new mindset for the purpose of conducting cultural analysis. This mindset involves assuming the role of anthropologist—just as an actor would assume a role in a play. The benefits of this approach are twofold: firstly, imagining oneself as an anthropologist provides excellent motivation to get out and explore a new culture rather than avoiding it and hiding out in the safe haven of one's own home; secondly, having a new, temporary identity will help create the emotional distance necessary to minimize reactionary, negative feelings and allow for progressive, cross-cultural understanding. While being sensitive to prospective expatriate spouses' personal goals, personal trainers should provide concrete methods to help mitigate culture shock's related stresses, as well as helping expatriate spouses develop appropriate coping skills to assist them in dealing with the unsettling experience of living in another culture. By implementing improvements in relocation programs, such as the solution I propose, multinational companies can maintain an acceptable return on investment for their relocation programs while affording expatriate employees and their respective families an enriching and life-changing intercultural experience.

隨著台灣地區對外籍配偶政策逐步開放,所帶來台灣地區人民與國外聯姻所佔的比重增加外,另一方面,因外籍新娘所伴隨語言的隔閡文化的融合、薄弱婚姻基礎、社會接納度及支持網絡不夠、就業福利、輔導措施、子女教養甚或與上一代相處等三代之代際問題,使得異國婚姻的離婚率也悄悄升高,連帶地促使台灣地區離婚問題更趨嚴重複雜。尤其外籍配偶離婚問題,其所衍生複雜之家庭、社會甚至影響國家形象等層面問題,不若本國籍離婚案例單純,已成為學者、專家極度關注的議題。 本研究目的係期以2001至2003年台灣地區各縣市離婚率的追蹤資料(panel data)進行分析,探討以下問題:一、分析與瞭解台灣地區縣市別離婚率與外籍配偶分布趨勢之間的關係,並比較其差異;二、探究影響台灣地區離婚率升高的決定因素。三、對目前台灣外籍配偶政策提供具體的建議。 經由計量模型估計結果發現,外籍配偶佔有偶婦女比率越高之縣市,其離婚率較高,有顯著正相關之影響,其中外籍新娘以大陸配偶比重越高之縣市,較東南亞及其他國籍者,與離婚率呈現高度正相關;若從時間趨勢來看,2003年台灣地區的離婚率,相對2001年基期水準,會較2002年有顯著增加效果;東部區域較北部、中部與南部區域的縣市,在外籍配偶對其離婚率影響,相對較高;行政區劃的部分市又高於縣。 綜上,外籍配偶是影響台灣地區各縣市別離婚率之相當重要因素,然而外籍配偶政策與降低離婚率等,皆有賴政府正視問題有所作為,並管制成效予以改善。 / As the Taiwan’s policies toward foreign spouses have been gradually liberalized, the proportion of Taiwanese people marrying foreign spouses has increased. However, language barriers, cultural assimilations, fragile marriage foundation, the lack of social acceptance and a support network, career welfare, assistance measures, children’s education and the inter-generation issues among the previous, the current, and the next generations have slightly raised the divorce rate of international marriages, and further, worsened and complicated the divorce issues in Taiwan. Issues derived from the divorce of foreign spouses in particular, such as complicated families and negative impact on the social or national image, are not as simple as the domestic divorce cases, and have thus become the focus of scholars and experts. Based on the panel data of the divorce rate in every city and county in Taiwan collected during 2001 and 2003, this study aims to investigate the following issues: (1) to analyze the relation of divorce rate and the distribution of foreign spouses in every region, and compare the differences; (2) to investigate the decisive factors in raising the divorce rate in Taiwan; (3) to provide substantial suggestions for current policies toward foreign spouses. The estimation made through a metric model shows regions with a higher proportion of foreign spouses have a higher divorce rate. And the effect is positively correlated. And regions with a higher proportion of foreign spouses from Mainland China than from Southeast Asia nations or others have a higher divorce rate. This proportion is highly correlated to divorce rate. In terms of time period, compared with the base standard of the 2001, the divorce rate in Taiwan in 2003 is significantly higher than that in 2002. The effect of foreign spouses on the divorce rate is relatively higher in eastern regions than in northern, central, and southern regions. As to administrative divisions, the rate is higher in cities than in counties. To sum up, foreign spouse is an important factor in the divorce rate in every region in Taiwan. However, policies toward foreign spouses and the reduction of divorce rate depend on how the government resolves these issues and improves the performance of the liberalization. Debarment action in execution proceedings The thesis targets the interpretation of debarment actions in execution proceedings. Generally the thesis provides a definition of debarment actions as a legal institution for the protection of third party rights and of debarment action execution proceedings. On the other hand the thesis is very particular in terms of the specifics of individual forms of debarment actions. The interpretation does presented also refers to current jurisprudence as applied by Czech courts as regards debarment actions, as well as in further related cases. The legal interpretation points out to the possibility of future amendments to certain legal provisions. The first Chapter of the thesis defines fundamental legal terms and institutions, which create a broader base for debarment action interpretations. The thesis focuses on the definition of notions of execution, the status of bailiffs, execution proceedings and the parties thereto as well as the sources of law including the definition of the inter relationship between Civil Procedure Code and Execution Procedure, which are cardinal for the main subject of the thesis. Violência por parceiro íntimo contra a gestante: estudo sobre as repercussões obstétricas e neonatais / Intimate partner violence against pregnant women: study about the obstetric and neonatal repercussions

A violência por parceiro íntimo (VPI) é uma das formas mais frequentes de violência contra a mulher, e se constitui em um fenômeno complexo, que faz parte de uma construção histórica e possui íntima relação com as questões de gênero e suas relações de poder. Durante a gestação, a mulher utiliza com maior frequência os serviços de saúde, o que pode facilitar a construção de vínculo com a equipe de saúde e favorecer a identificação de casos de violência. Os objetivos deste estudo foram: identificar a prevalência de casos de VPI entre gestantes usuárias de um serviço público de saúde e classificar quanto ao tipo e frequência; identificar fatores sociais e demográficos das gestantes e seus parceiros que poderiam estar associados a episódios de violência; identificar os resultados obstétricos e neonatais e suas associações com a ocorrência da VPI na gestação atual. Trata-se de um estudo observacional, descritivo e analítico, desenvolvido no CRSM-MATER e no HCFMRP-USP, situados em Ribeirão Preto, SP. O projeto de pesquisa foi aprovado pelo CEP da EERP-USP, sob nº 1383/2011. Os dados foram coletados no período de maio a dezembro de 2012, por meio de entrevista estruturada por um questionário que contemplava as características sociodemográficas da mulher e de seu parceiro e características obstétricas, além das questões relacionadas à violência; também foram coletados dados dos prontuários das participantes com a utilização de um instrumento que contemplava as variáveis relacionadas aos resultados obstétricos e neonatais. Durante a coleta de dados, iniciada no ambulatório de pré-natal do CRSM-MATER, algumas participantes foram encaminhadas ao HC-FMRP e tiveram a coleta nos prontuários realizada nesta última instituição. Para análise dos dados, utilizou-se o programa estatístico SAS® 9.0. Esta análise foi fundamentada na estatística descritiva, além da utilização do Teste Exato de Fisher e Regressão Logística. Verificou-se que 15,5% das participantes sofreram algum tipo de VPI durante a gestação, sendo que 14,7% sofreram violência psicológica, 5,2%, violência física e 0,4% sofreu violência sexual. As mulheres que se autorreferiram como pretas ou pardas, que não moravam com o parceiro e que referiram consumir bebida alcoólica eventualmente, apresentaram maior chance de sofrer VPI na gestação. Além disso, as mulheres que não desejaram a gestação tinham 4,3 vezes a chance de sofrer VPI na gestação, quando comparadas com aquelas que desejaram a gestação (p<0,00; OR= 4,32 e IC 95% [1,77 - 10,54]). As participantes com parceiros na faixa etária de 15 a 18 anos apresentaram 5,5 vezes a chance de sofrer violência, quando comparadas com aquelas que tinham parceiros com 30 anos ou mais (OR= 5,5; IC 95% [1,02 - 30,2]). Com relação às repercussões obstétricas, não houve associação estatisticamente significativa entre as variáveis investigadas. Quanto às repercussões neonatais dos filhos das participantes, as análises também não apresentaram resultados estatisticamente significativos. Conclui-se que, para as participantes do estudo, não houve repercussões obstétricas e neonatais negativas relacionadas à VPI na gestação. No entanto, outras variáveis se mostraram associadas a este tipo de violência, o que indica, aos profissionais de saúde, a importância de se atentar a outras características das gestantes e de seus parceiros, favorecendo a identificação da violência e o oferecimento de suporte adequado a estas mulheres, quando necessário / The intimate partner violence (IPV) is one of the most frequent ways of violence against women, and it is constituted in a complex phenomenon, which is part of a historical construction and is intimately related to gender questions and its power relationships. During pregnancy, the woman attend health care more frequently, which can facilitate the bond building with the health staff and favours the identifications of violence cases. This study aims were: identify the prevalence of IPV cases between pregnant women who use public healthcare service and classify according to its type and frequency; identify the pregnant e their partners\' social and demographic factors which could be associated with violence episodes; identify the obstetric and neonatal results and their associations with the IPV occurrence in the current pregnancy. This is an observational, descriptive and analytic study, developed at CRSM-MATER and HCFMRP-USP, located in Ribeirão Preto, SP. The research project was approved by ethics committee from EERP-USP, under number 1383/2011. The data were collected between May and December 2012, via interview structured by a questionnaire which beheld the woman\'s and their partner\'s social- demographic characteristics and obstetric characteristics, beside the questions related to violence; there were also collected the participants\' records data using an instrument that contemplated the variables related to the obstetric and neonatal results. During the data collection, initiated at CRSM-MATER prenatal clinic, some participants were leaded to HC- FMRP and had the records collection done at this last institution. To analyze the data the statistic program SAS® 9.0 was used. This analysis was underlay on descriptive statistic, besides using the Fisher Exact Test and Logistic Regression. It was verified that 15,5% of the participants suffered some sort of IPV during pregnancy, knowing that 14,7% suffered psychological violence, 5,2% physical violence and 0,4% sexual violence. Women whom were considered themselves as black or brown-skinned, who haven\'t lived with their partners and who were referred as eventual liquor drinkers showed bigger chance of suffering IPV during pregnancy. Furthermore, women who didn\'t desire the pregnancy had 4,3 times chances of suffering IPV during pregnancy when compared to those ones who desired the pregnancy (p<0,00; OR= 4,32 e IC 95% [1,77 - 10,54]). The participants with partners between 15 and 18 years old showed 5,5 times chances of suffering violence when compared to those ones whose partners were 30 years old or older (OR= 5,5; IC 95% [1,02 - 30,2]). Relating to the obstetric repercussions, there was no association statistically meaningful between the variables investigated. As the participants\' children\'s neonatal repercussion, the analysis didn\'t present results statistically meaningful either. In conclusion, for the study participants, there were no negative obstetric and neonatal repercussions related to IPV in pregnancy. Nevertheless, other variables are shown associated to this type of violence, which indicates, to the health care professionals, the importance to be attentive to other pregnant women\'s and their partners\' characteristics, favoring the violence identification and offering appropriate support to these women, when necessary

The Relationship Between Leisure and Perceived Burden of Spouse Caregivers of Persons with Alzheimer's Disease

The problem of this study was to better understand spouse caregivers' leisure involvement, experience, and barriers and their relationships with perceived burden. Thirty-six wife and 19 husband caregivers of persons with Alzheimer's disease and related disorders volunteered to participate in this study, either by mailed questionnaire or interview. Respondents were primarily female, white, with an average age of 72 years. The conclusions of the study were: (a) caregivers significantly reduce both their leisure involvement; (b) self-reported health, perceived social supports, income level, use of paid help, and leisure activity patterns are major factors associated with caregivers' leisure; and (c) leisure barriers are a significant contributor to caregivers' perceived burden. Recommendations were presented for caregivers, practitioners, and future study.

There is an increased risk of mortality among patients in grief, when compared to those not thrown into mourning, besides an increased use of health services. There have been investigations on individual and/or situational variables which might explain why a significant proportion of people in grief resulting from the death of a spouse have their health affected. Stress seems to be associated to a kind of cardiopathy called Broken Heart Syndrome. Such Syndrome may be considered as metaphor, which take place immediately after a severe physical or psychological stressful episode, to which an individual responds much the same way as to a heart attack. The aim of the present study was to figure out how mourning affected widowhood with presentation cardiac symptoms without physical causes. The study was conducted one year and five months after the death of her spouse. Our results indicated that the widow sustained severe psychological pain, which triggered the cardiac symptoms characterized as the Broken Heart Syndrome. Such severe psychological suffering leads to chest pain that is similar to a heart attack. The subject reported that her health care professionals did not sympathize with her psychological suffering. Furthermore, it was clear that the loss brought about consequences that can be seem in the whole family / A perda por morte é considerada situação de crise e a morte do cônjuge uma das perdas mais significativas da vida. Há aumento no risco de mortalidade em pacientes enlutados, quando comparados a indivíduos não enlutados, além de aumento na utilização dos serviços de saúde. Tem-se investigado variáveis individuais e/ou situacionais que expliquem por que proporção significativa de pessoas enlutadas pela perda do cônjuge têm a saúde afetada. O estresse parece estar associado a um tipo de cardiopatia denominado Síndrome do Coração Partido. Essa Síndrome pode ser considerada como metáfora que tem precedência imediata após um episódio de estresse físico ou psicológico intenso, que pode propiciar ao indivíduo reação semelhante à de um ataque cardíaco. O objetivo do estudo foi compreender o processo do luto por viuvez com apresentação de sintomas cardíacos sem causa orgânica. A pesquisa foi realizada um ano e cinco meses após a morte do cônjuge. Os resultados indicaram que a viúva mostrou dor psicológica intensa, desencadeando sintomas cardíacos caracterizados como Síndrome do Coração Partido. O sofrimento psicológico intenso leva à dor no peito que se assemelha a ataque cardíaco. A participante relata não ter encontrado acolhimento nos profissionais de saúde com relação à sua dor psíquica. Além disso, foi possível observar que a perda trouxe desdobramentos que aparecem em todo o grupo familiar
