Att vårdas under tvång : Patientens upplevelse av tvångsåtgärder inom psykiatrisk vård / Care under coercion : The patients experience of coercive measures in psychiatric care

Bakgrund: Lag om psykiatrisk tvångsvård innebär att sjukvården har rätt att ta bort självbestämmanderätten från en individ om denne lider av en allvarlig psykisk störning. Restriktioner och tvångsåtgärder som tvångsmedicinering, avskiljning och fastspänning kan förekomma. Syfte: Syftet är att belysa hur patienter upplever tvångsåtgärder inom psykiatrisk vård. Metod: En allmän litteraturstudie baserad på 12 vetenskapliga artiklar med kvalitativ ansats. Antonovskys Känsla av sammahang har använts som teoretisk referensram. Resultat: Det framkom att patienter generellt upplevde tvångsåtgärderna som negativa. Åtgärderna väckte en rad olika känslor hos patienterna såsom obehag, rädsla och förlorad autonomi, även återuppväckta minnen förknippade med våldtäkt och andra trauman. Det var många patienter som hade svårt att förstå varför dessa åtgärder utfördes, vilket visade sig som brister i kommunikationen mellan patient och sjuksköterska. Patienterna uttryckte önskan om att få utnyttja sin medbestämmanderätt, själva påverka tvångsåtgärderna och eftersökte en närmare relation till vårdpersonalen. Även om tvångsåtgärderna i största utsträckning var en negativ upplevelse så fanns det patienter som förstod att det var för deras eget bästa liksom för andra patienters och personals säkerhet. Slutsats: Patienterna rapporterade att de ville bli informerade, förstådda och bemötta som medmänniskor med respekt och inte behöva känna sig ensamma och övergivna. Tvångsåtgärder och lidande kan lindras genom att personal utvecklar sitt aktiva lyssnande och kunskap genom att ge adekvat information. Klinisk betydelse: Ökad förståelse för patienter som utsatts för tvångsåtgärder kan ge sjuksköterskor möjligheten att stärka patientens känsla av sammanhang vid tvångsåtgärder genom ökad begriplighet, hanterbarhet och meningsfullhet. Coercive measures may occur, such as forced medication, isolation and physical restraint. Aim: The aim is to illustrate how patients experience coercive measures in psychiatric care. Method: A literary review based on 12 scientific articles with qualitative approach. Antonovskys Sense of coherence has been used as a theoretical framework. Result: The results show that patients experienced coercive measures as negative. The measures provoked feelings in patients, such as discomfort, fear and loss of autonomy. There were patients who had trouble understanding why these measures were performed, which showed flaws in communication between patient and nurse. The patients expressed the desire to use their right to co-decide, to influence the measures and sought a closer relationship to the caregivers. Although the coercive measures were found to be a highly unpleasant experience, there were patients who understood that it was in their best interests. Conclusion: The patients reported that they wanted to be informed, listened to and treated with respect, and not have to feel alone and abandoned. Coercive measures and the associated suffering could be alleviated by active listening from the staff and the skills to provide adequate information. Clinical Implications: Increased understanding of patients subjected to coercive measures can give nurses the opportunity to strengthen the patients’ sense of coherence when exposed to coercive measures, by increasing their comprehensibility, manageability and meaningfulness.  Keywords: SOC, coercive measures, patient experience, psychiatric care & literature review

Male violence against women in intimate partner relationships is a global public health problem and a serious crime, but remains largely underreported. The overall ain of this thesis was, by means of an interdisciplinary approach and from the perspectives of gender, public health and law, to gain a deeper understanding of the situation of abused women, by studying, on one hand, their leaving processes and, on the other, the consequences of the violence for their health. On the basis of this information, a further aim was to increase knowledge of the quality and effect of gender relations in the practical application of the law. The point of departure was that the different perspectives should not be consodered separately but should be seen in context. The thesis describes the prcess for abused women, from the violent relationship and the break-up, the health consequences caused by the violence to the practical application of the law. The thesis consists of four studies (I-IV). An initial qualitative study (I) showed that the leaving process for abused women is fearful, complex and long-lasting. The analysis also revealed a gradual development of strong emotional bonds towards the men on the part of the women. Study II was a combination of qualitative in-depth interviews and quantitative measurements and confirmed former studies regarding the negative health consequences caused by the violence, in particular symptoms of complex PTSD. Study III, examining preliminary investiagtions concerning reported crimes of intimate partner violence, showed that there is still a lack of crminal justice for abused women even if the frequency of cases leading to prosecution was fairly high. In study IV court case records were examined by means of a qualitative method inspired by discourse analysis to determine whether gender is (re)constructed in court practice, and how the process works. The analysis of this particular sample indicated that the practical application of law is influenced by an old-fashioned and stereotypical view of violence, sexuality, men and women. the legal discourses retain and reproduce the gender order. The overall results of the separate studies reinforce the importance of gaining a broader understanding of men's violence against women in intimate partner relationships.

Span of Control’s Significance for Public Sector Employees’ Working Conditions and Well-Being

The reforms in the public sector, as consequence of New Public Management, have led to flattened organizational structure, increasing the span of control(SOC) width for managers. The aim of the study was to investigate the association between SOC width, working conditions (job demands, job resources and engaged leadership), employee health and engagement. SOC was measured as predictor for employee health and engagement, where mediation of working conditions effect on the relation was examined. Self-administrated surveys were collected from 1551 employees working under 109 managers in four public sector municipal operations in Sweden. The result displayed that it was unfavorable to have managers with a wide SOC for the employees working conditions, engagement and health. SOC was a significant predictor for health, while mediation of working conditions could not account for the relationship. These results indicate that a narrow SOC may improve working conditions and health outcomes for employees.

Syftet med detta examensarbete var att undersöka hur behandlare inom olika professioner använt sig av hästunderstödd terapi, som en del av en terapeutisk behandling. Detta för att se om behandlarna kunde märka en förändring hos deltagaren. Examensarbetet baserades på kvalitativ metod där fyra telefonintervjuer genomförts med personer som arbetar med hästar i terapeutisk behandling. Resultaten visade att behandlarna upplevde att hästarna kunde hjälpa till med samspelet mellan behandlare och deltagare. Det framkom att behandlarna upplevde att det skedde genom ett icke-verbalt deltagande från hästen i samspel med behandlaren och den som deltog. Deltagarna fick en möjlighet att skapa mening med hjälp av vardagliga rutiner och strukturer. Del kunde känna tillit till andra, samt började våga skapa relationer med andra människor vilket gav en känsla av förhållande till samhället. Samtliga behandlare menade att det var svårt att avgöra vad som hade skapat en förändring. Gemensamt var behandlarnas upplevelse av en positiv förändring hos flertalet av deltagarna. / This bachelor thesis analyses the use of horse-assisted therapy amongst mental health practitioners within multiple sectors. The aim was to establish whether a change could be detected in participants. Adopting a qualitative approach the research was mainly conducted through four semi-structured interviews with equine-assisted therapy practitioners. The results show that the practitioners believe the horses improve the interactions between the practitioners and participants. The practitioners believe this improvement is due to the non-verbal participation of the horse in interface with the practitioner and participant and that the horses increase the participants’ trust in others as well as their confidence when establishing relationships with others. The participants gained an opportunity to create meaning within their lives through everyday routines, structures and feel a sense of context. Some of the practitioners struggled to identify what causes the change but recognised a positive change in the majority of participants.

Digital approach for the design of statistical analog data acquisition on SoCs

With the current demand for mixed-signal SoCs, an increasing number of designers are looking for ADC architectures that can be easily implemented over digital substrates. Since ADC performance is strongly dependent upon physical and electrical features, it gets more difficult for them to benefit from more recent technologies, where these features are more variable. This way, analog signal acquisition is not allowed to follow an evolutionary trend compatible with Moore’s Law. In fact, such trend shall get worst, since newer technologies are expected to have more variable characteristics. Also, for a matter of economy of scale, many times a mixed-signal SoC presents a good amount of idle processing power. In such systems it is advantageous to employ more costly digital signal processing provided that it allows a reduction in the analog area demanded or the use of less expensive analog blocks, able to cope with process variations and uncertainty. Besides the technological concerns, other factors that impact the cost of the design also advise to transfer problems from the analog to the digital domain whenever possible: design automation and self-test requirements, for instance. Recent surveys indicate that the total cost in designer hours for the analog blocks of a mixed-signal system can be up to three times the cost of the digital ones. This manuscript explores the concept of bottom-up analog acquisition design, using statistical sampling as a way to reduce the analog area demanded in the design of ADCs within mixed-signal systems. More particularly, it investigates the possibility of using digital modeling and digital compensation of non-idealities to ease the design of ADCs. The work is developed around three axes: the definition of target applications, the development of digital compensation algorithms and the exploration of architectural possibilities. New methods and architectures are defined and validated. The main notions behind the proposal are analyzed and it is shown that the approach is feasible, opening new paths of future research. In the first part, we have studied the usage of Wavelet transform on analysis and characterization of (multi)fractal time series. We have test the reliability of Wavelet Transform Modulus Maxima method (WTMM) in respect to the multifractal formalism, trough the calculation of the singularity spectrum of time series whose fractality is well known a priori. Next, we have use the Wavelet Transform Modulus Maxima method to study the fractality of lungs crackles sounds, a biological time series. Since the crackles sounds are due to the opening of a pulmonary airway bronchi, bronchioles and alveoli which was initially closed, we can get information on the phenomenon of the airway opening cascade of the whole lung. Once this phenomenon is associated with the pulmonar tree architecture, which displays fractal geometry, the analysis and fractal characterization of this noise may provide us with important parameters for comparison between healthy lungs and those affected by disorders that affect the geometry of the tree lung, such as the obstructive and parenchymal degenerative diseases, which occurs, for example, in pulmonary emphysema. In the second part, we study a site percolation model for square lattices, where the percolating cluster grows governed by a control rule, corresponding to a method of automatic search. In this model of percolation, which have characteristics of self-organized criticality, the method does not use the automated search on Leaths algorithm. It uses the following control rule: pt+1 = pt + k(Rc ? Rt), where p is the probability of percolation, k is a kinetic parameter where 0 < k < 1 and R is the fraction of percolating finite square lattices with side L, LxL. This rule provides a time series corresponding to the dynamical evolution of the system, in particular the likelihood of percolation p. We proceed an analysis of scaling of the signal obtained in this way. The model used here enables the study of the automatic search method used for site percolation in square lattices, evaluating the dynamics of their parameters when the system goes to the critical point. It shows that the scaling of , the time elapsed until the system reaches the critical point, and tcor, the time required for the system loses its correlations, are both inversely proportional to k, the kinetic parameter of the control rule. We verify yet that the system has two different time scales after: one in which the system shows noise of type 1 f , indicating to be strongly correlated. Another in which it shows white noise, indicating that the correlation is lost. Power Modeling and Scheduling of Tests for Core-based System Chips

The technology today makes it possible to integrate a complete system on a single chip, called "System-on-Chip'' (SOC). Nowadays SOC designers use previously designed hardware modules, called cores, together with their user defined logic (UDL), to form a complete system on a single chip. The manufacturing process may result in defect chips, for instance due to the base material, and therefore testing chips after production is important in order to ensure fault-free chips. The testing time for a chip will affect its final cost. Thus it is important to minimize the testing time for each chip. For core-based SOCs this can be done by testing several cores at the same time, instead of testing the cores sequentially. However, this will result in a higher activity in the chip, hence higher power consumption. Due to several factors in the manufacturing process there are limitations of the power consumption for a chip. Therefore, the power limitations should be carefully considered when planning the testing of a chip. Otherwise it can be damaged during test, due to overheating. This leads to the problem of minimizing testing time under such power constraints. In this thesis we discuss test power modeling and its application to SOC testing. We present previous work in this area and conclude that current power modeling techniques in SOC testing are rather pessimistic. We therefore propose a more accurate power model that is based on the analysis of the test data. Furthermore, we present techniques for test pattern reordering, with the objective of partitioning the test power consumption into low parts and high parts. The power model is included in a tool for SOC test architecture design and test scheduling, where the scheduling heuristic is designed for SOCs with fixed- width test bus architectures. Several experiments have been conducted in order to evaluate the proposed approaches. The results show that, by using the presented power modeling techniques in test scheduling algorithms, we will get lower testing times and thus lower test cost.

Källor till motståndskraft för utmattningssyndrom hos personer inom yrken i hälso- och sjukvården – en litteraturöversikt / Sources of resilience for burnout in persons with health care professions - a literature review

Bakgrund: Sjuktalen har ökat till följd av psykisk ohälsa världen över. En av de främsta orsakerna till detta har varit den ökade förekomsten av stressrelaterad ohälsa och utmattningssyndrom. En grupp som har varit extra utsatt är personer i inom yrken i hälso- och sjukvården. Idag fokuseras det huvudsakligen på behandling och rehabilitering av utmattningssyndrom i forskning och praxis och det finns begränsad kunskap och initiativ kring primär- och sekundärprevention för utmattningssyndrom. Det är dock viktigt ur både ett individ- och samhällsperspektiv att förebygga utmattningssyndrom då utmattningssyndrom är ett allvarligt tillstånd som kan leda till stort lidande för individen och lång sjukfrånvaro från arbetslivet, vilket även är kostsamt för samhällsekonomin. Syfte: Syftet med detta examensarbete var att undersöka skyddsfaktorer mot utmattningssyndrom hos personer inom yrken i hälso- och sjukvården. Metod: En litteraturöversikt genomfördes och 12 vetenskapliga artiklar uppfyllde inklusion- och exklusionskriterierna och matchade forskningsfrågorna. Dessa lästes igenom. En tematisk analys av studiernas resultat genomfördes och resulterade i ett övergripande tema och fem underteman. Resultat: Resultatet beskriver faktorer som skyddar mot utmattningssyndrom för personer inom yrken i hälso- och sjukvården. Exempel på dessa faktorer var vikten av autentisk och god arbetsmiljö, känsla av egenmakt och kontroll, deltagande i meningsfulla aktiviteter, återhämtning och praktiska tekniker som exempelvis mindfulness, yoga och bildterapi. Att få socialt stöd från närstående, kollegor och exempelvis kuratorer och präster var även en viktig faktor för att skydda mot utmattningssyndrom. Slutsats: Skyddande faktorer för utmattningssyndrom finns på både individ-, organisations- och samhällsnivå och bör beaktas i utvecklingen av förebyggande insatser för utmattningssyndrom.
