Facteurs socioenvironnementaux associés à la prévalence des limitations d’activités au Québec

Objectifs : Cette thèse porte sur l’association entre les caractéristiques socioenvironnementales des voisinages (milieux locaux) et la prévalence des limitations d’activités (ou handicap) dans la population québécoise. Elle a trois objectifs principaux : (1) clarifier les enjeux conceptuels et méthodologiques relatifs à l’étude des déterminants socioenvironnementaux des limitations d’activités; (2) décrire les contributions respectives de la composition socioéconomique des voisinages et de facteurs contextuels à la variabilité locale de la prévalence des limitations d’activités; (3) évaluer la présence d’interactions entre la santé fonctionnelle des personnes (incapacité) et des caractéristiques des voisinages en lien avec la prévalence des limitations d’activités. Méthodes : Une analyse de la littérature scientifique a été effectuée en lien avec le premier objectif de la thèse. En lien avec le deuxième objectif, des données pour le Québec du recensement canadien de 2001 (échantillon de 20% de la population) ont été utilisées pour estimer l’association entre la prévalence des limitations d’activités et des caractéristiques des voisinages : classification urbain-rural, composition socioéconomique (défavorisation matérielle et sociale) et facteurs contextuels (qualité des habitations, stabilité résidentielle et utilisation des transports actifs et collectifs). En lien avec le troisième objectif, des données pour la population urbaine du Québec issues de l’Enquête sur la santé dans les collectivités canadiennes (2003, 2005 et 2007/2008) ont permis de tester la présence d’interaction entre la santé fonctionnelle des personnes et des caractéristiques des voisinages (défavorisation matérielle et sociale, qualité des habitations, stabilité résidentielle et densité des services). Pour les analyses associées aux deux derniers objectifs, l’analyse des corrélats de la prévalence des limitations d’activités a été effectuée à l’aide de régressions logistiques multiniveaux. Résultats : Différents éléments conceptuels et opérationnels limitent la possibilité de faire une synthèse des analyses épidémiologiques portant sur les influences socioenvironnementales sur les limitations d’activités. Les résultats des analyses empiriques suggèrent que : (1) la variation géographique de la prévalence des limitations d’activités s’explique en grande partie par la composition socioéconomique des voisinages; (2) des facteurs contextuels sont associés à cette variation géographique; (3) les mesures relatives d’inégalités sous-estiment les disparités contextuelles dans la distribution des nombres absolus de personnes ayant une limitation d’activités; et (4) l’association entre la prévalence des limitations d’activités et la défavorisation sociale pourrait varier selon la santé fonctionnelle des personnes. Conclusions : Différentes caractéristiques socioenvironnementales sont potentiellement associées aux variations géographiques des limitations d’activités au Québec. Le développement d’indicateurs socioenvironnementaux favoriserait une connaissance plus précise de l’influence de ces caractéristiques socioenvironnementales sur les limitations d’activités et des mécanismes par lesquels s’exerce cette influence. L’établissement d’un système national de surveillance des aménagements territoriaux est proposé afin de soutenir la recherche et la prise de décision. Des indicateurs locaux d’accessibilité aux transports, aux espaces publics ainsi qu’aux services de proximité devraient être priorisés. Ces aspects de l’aménagement du territoire sont susceptibles de rejoindre plusieurs enjeux de santé publique et ils ont comme autre avantage d’être inclus dans différentes orientations québécoises ciblant le vieillissement en santé et la réduction des limitations d’activités. / Objectives: This thesis explores the socioenvironmental features of neighbourhoods (local areas) associated with disability prevalence in Québec. It has three main objectives: (1) clarify conceptual and operational issues relevant to the study of socioenvironmental determinants of disability; (2) describe the particular contribution of neighbourhoods’ socioeconomic composition and contextual features to the local variability in disability prevalence; (3) test for interactions between individual-level functional health and neighbourhoods’ characteristics in relation to disability prevalence. Methods: A literature review of the epidemiological literature was undertaken in relation with the first objective. Analyses of correlates of disability prevalence were conducted using multi-level logistic regressions. Per the second objective, data for Québec from the 2001 Canada census (sample of 20% of the population) were used to estimate the associations between disability prevalence and neighbourhoods’ characteristics: urban-rural classification, socioeconomic composition (material and social deprivation) and contextual features (housing quality, residential stability, and collective and active commuting). In relation with the third objective, data for the Québec urban population from the Canadian community heath survey (2003, 2005 and 2007/2008) were used for assessing the presence of interactions between individuals’ functional health and neighbourhoods’ characteristics (material and social deprivation, housing quality, residential stability, and density of services). Results: Various conceptual and operational aspects prevent a straightforward synthesis of epidemiological studies analysing socioenvironmental influences on disability. Results from empirical analyses suggest that (1) geographic variability of disability prevalence is largely attributable to neighbourhood composition; contextual factors are associated to such variability; (3) relative measures of inequality under-estimate the contextual disparities in the distribution of absolute numbers of disabled individuals; and (4) the association between disability prevalence and social deprivation could vary according to individuals’ functional health. Conclusions: Various socioenvironmental characteristics are potentially associated with local variability of disability in Québec. The development of socioenvironmental indicators could contribute to a refined understanding of neighbourhood characteristics’ influence on disability as well as how their influence operates. The creation of a local land-use planning surveillance system is recommended for supporting research and decision-making. Local indicators of access to transportation, to public spaces as well as to proximity services should be prioritised. These features of territorial planning are likely to be associated with many public health issues and they are common to initiatives undertaken in Québec to promote healthy aging and to reduce disability.

Teachers' challenges and the promise of equitable classrooms: why students who need more get less

The education of youth in the United States has become a highly contested subject over the past decades. This thesis argues that one of the earliest institutions American citizens encounter – the public school system – organizes the work of many teachers in ways that reproduce inequality of opportunity for students. Drawing on qualitative data from fourteen in-depth interviews with experienced elementary school teachers in Los Angeles, this thesis illustrates how teachers experience and navigate specific structural barriers to the pursuit of equity in the classroom. Applying social reproductive theory to teacher interviews, this research discovered how, despite rhetorical commitment to equality of opportunity in education student outcomes continue to vary according to the socioeconomic status of the student population. This will help us understand systemic barriers built into the structure of the education system. These barriers operate as obstacles that teachers and students must navigate, in order to achieve success. This thesis argues that teachers should begivenmore flexibility to assess the needs of each specific class and adapt their curriculum and strategies to meet those needs. Unfortunately, in the current test-score driven system, schools with the lowest performing students are the ones whose administrations are under the most pressure to improve the low scores rather than fix the problems associated with low scores. As such, the teachers that need this flexibility the most, are the ones whose administrations keep them on the tightest rein, further reducing their ability to utilize their knowledge and implement effective strategies in the classroom. The result is the self-perpetuating cycle of inequality reproduction that we can see across North America today. / Graduate / 2019-08-13

Introdução: O envelhecimento populacional, decorrente do aumento da expectativa de vida, foi, sem dúvida, uma importante conquista em nível global. Contudo, discute-se atualmente a necessidade premente de agregar qualidade aos anos de vida ganhos. Objetivo: Identificar fatores associados à Qualidade de Vida Relacionada à Saúde (QVRS) de idosos não institucionalizados do Município de São Paulo, em 2006. Método: Este estudo faz parte do Estudo Longitudinal SABE Saúde, Bem-Estar e Envelhecimento. Os dados foram coletados em 2006, de uma amostra representativa composta por 1.160 idosos (idade 60 anos) que responderam ao Short-Form 12 (SF-12), questionário genérico que avalia QVRS. As variáveis dependentes foram os Componentes Físico (PCS) e Mental (MCS) do SF-12 e as análises foram conduzidas separadamente segundo sexo. Buscou-se a associação com variáveis demográficas, socioeconômicas, estado de saúde, incapacidade funcional, estilo de vida e relacionamento social, utilizando o método de regressão logística múltipla. Resultados: Entre as idosas, estiveram associados a baixos escores no PCS: idade 80 anos, multimorbidade, internação, ter incontinência urinária, ter depressão, ter dificuldades para executar atividades instrumentais e básicas da vida diária (AIVD e ABVD) e não praticar atividades físicas. Baixos escores no PCS dos homens associaram-se a idade 80 anos, renda insuficiente, multimorbidade, tomar dois ou mais medicamentos, ter dificuldades para ABVD e não praticar atividades físicas. Baixos escores no MCS das mulheres estiveram associados a ser fumante, auto-percepção negativa de saúde geral e saúde bucal, ocorrência de queda no último ano e ter depressão. Baixos escores no MCS entre os homens idosos associaram-se à auto-percepção negativa de saúde, ter incontinência urinária e ter depressão. Homens mais velhos (idade 70 anos) apresentaram melhores escores no MCS em relação aos mais jovens (60-69 anos). Discussão: No PCS, as únicas variáveis coincidentes entre homens e mulheres foram idade, prática de atividades físicas, multimorbidade e dificuldade para ABVD; enquanto no MCS foram auto-percepção de saúde e depressão. A análise separada por sexo possibilitou a identificação de modelos distintos de determinantes da QVRS de idosos. Conclusão: Os fatores que se associaram significativamente ao PCS-SF12 de idosos de ambos os sexos foram: auto-percepção de saúde, multimorbidade, dificuldades para desempenhar ABVD e prática de atividades físicas. Enquanto suficiência de renda e número de medicamentos associaram-se apenas para o sexo masculino e internação, incontinência urinária, depressão e dificuldades para desempenhar AIVD, apenas para o sexo feminino. Ao MCS-SF12 de ambos os sexos associaram-se apenas auto-percepção de saúde e depressão; enquanto para o sexo masculino associaram-se também faixa etária e incontinência urinária. Já para as mulheres também apresentaram associação significativa o tabagismo, a auto-percepção de saúde bucal e quedas / Background: The world had experienced a high increased in life expectancy during the last decades, which has been undoubtedly considered as a major achievement. Because the increase of elderly population, that presents highest prevalence of chronic conditions, besides other single characteristics, several researches have been developed due to determine ways to add quality to the gained years of life. The aim of this study was to identify factors associated with elderly health-related Quality of Life (HRQoL), in São Paulo. Methods: This study is part of the Longitudinal Study SABE - Health, Welfare and Ageing (from Portuguese: Saúde, Bem-Estar e Envelhecimento). Data were collected in 2006. The sample consisted of 1,160 elderly (age 60 years) who answered the Short-Form 12 (SF-12), a generic HRQoL questionnaire. Dependent variables were Physical (PCS) and Mental Components (MCS) of SF-12. All analysis were separated by sex and the independent variables approached demographic, socioeconomic, health status, functional disability, lifestyle and social networking conditions, using the multiple logistic regression. Results: Among old women, lower PCS scores were associated with age 80 years, multimorbidity, hospitalization, urinary incontinence, depression, difficulty to perform basic and instrumental activities of daily living (BADL and IADL) and lack of physical activities. Among old men, lower PCS scores were associated with age 80 years, insufficient income, multimorbidity, taking two or more medications, difficulty to perform BADL and the lack of physical activities. Among the women, lower MCS scores were associated with being a smoker, negative self-perception of general health and oral health, occurrence of falls in the last year and depression. Among the men, lower MCS scores were associated with negative self-perception of health, urinary incontinence and depression. Older men ( 70 years) had better MCS scores than younger (60-69 years). Conclusion: Significantly associated factors with the PCS-SF12 for both male and female elderly were: self-perceived health status, multimorbidity, incapacity for BADL and physical activity; while income and number of drugs were associated only for males. Hospitalization, urinary incontinence, depression and incapacity for IADL were associated only for females. To the MCS-SF12 of both sexes were associated only self-perceived health and depression, while age and urinary incontinence were associated for males. Females MCS-SF12 also had significant association with tobacco, self-perception of oral health and falls

Skin diseases and their association with systemic diseases in the Northern Finland Birth Cohort 1966

Abstract Skin diseases are common: one in every three of all general practice patients have at least one dermatological problem. However, epidemiological studies addressing the overall prevalence of skin diseases are sparse. The skin is the largest organ in the body and it has several vital and immunological functions. Cutaneous signs are often the first manifestation of many systemic diseases. The aim of this study was to determine the overall prevalence, and the distribution according to sex and socioeconomic status, of skin diseases in an adult population. The study particularly focused on multiple melanocytic naevi and their risk factors because multiple melanocytic naevi are the strongest risk factor for melanoma. A further aim was to analyse the association between cutaneous disorders and systemic conditions; specifically to determine whether abnormal skin findings in toe webs have an association with abnormal glucose metabolism and whether skin diseases have a relationship with systemic low-grade inflammation. For these purposes a comprehensive dermatological evaluation was performed as a part of the 46-year follow-up survey of the Northern Finland Birth Cohort 1966. Data on this cohort have been collected since birth. Numerous laboratory tests were also performed cross-sectionally during the 46-year follow-up survey, including an oral glucose tolerance test and the measurement of fasting plasma glucose and glycated haemoglobin fraction. High sensitivity C-reactive protein was measured as a marker of low-grade inflammation. Over half (60%) of the 1 932 individuals examined had at least one skin disorder requiring further treatment. The need for treatment was more common in males and those with lower socioeconomic status. Multiple melanocytic naevi were found in 12% of individuals; high educational level, male sex and fair skin type increased the risk. Abnormal skin findings in toe web spaces was associated with undiagnosed type 2 diabetes. Atopic eczema, rosacea and onychomycosis were associated with low-grade inflammation. This unique study with nearly 2 000 subjects reports for the first time the overall prevalence of skin diseases in an unselected Finnish population. Its findings support the previous postulate that skin diseases are common in adults and suggest that skin evaluation should be an essential part of routine medical examinations in clinical practice. / Tiivistelmä Ihotaudit ovat yleisiä, ja jopa 30 %:lla yleislääkärin potilaista on jokin ihoon liittyvä ongelma. Väestötason tutkimuksia ihotaudeista on kuitenkin niukasti. Iho on ihmisen suuri elin, ja sillä on useita tärkeitä tehtäviä, kuten osallistuminen immunologiseen puolustukseen. Monet yleissairaudet voivat näkyä iholla jo ennen taudin puhkeamista ja ihoilmentymät voivat olla varhaisia merkkejä piilevästä sairaudesta. Tämän tutkimuksen tarkoituksena oli määrittää ihotautien esiintyvyys aikuisväestössä sekä ihotautien jakautuminen sukupuolen ja sosiaalisen aseman suhteen. Lisäksi tarkoituksena oli selvittää runsasluomisuuden (yli 50 pigmenttiluomea) esiintyvyyttä ja sen riskitekijöitä, sillä runsasluomisuus on melanooman merkittävin riskitekijä. Tutkimuksen tavoitteena oli myös selvittää ihotautien yhteyttä yleissairauksiin, kuten poikkeavien varvasvälilöydösten yhteyttä häiriintyneeseen sokeriaineenvaihduntaan sekä matala-asteisen tulehduksen ja ihotautien välistä yhteyttä. Osana Pohjois-Suomen syntymäkohortti 1966:n 46-vuotistarkastusta tehtiin kokonaisvaltainen ihon tutkiminen yhteensä 1 932:lle tutkimushenkilölle vuosina 2012–13. Samassa yhteydessä tutkittavilta määritettiin paastoverensokeri ja pitkäaikainen verensokeri sekä tehtiin sokerirasitustesti sokeriaineenvaihdunnan häiriön toteamiseksi. Matala-asteisen tulehduksen arvioimiseksi määritettiin herkkä C-reaktiivinen proteiini. Tutkimusjoukon pitkäaikaiset taustatiedot perustuivat syntymäkohortin aiempiin aineistoihin. Tutkittavista 60 %:lla todettiin jokin hoitoa vaativa ihotauti. Hoidon tarve oli suurempaa miehillä ja matalammissa sosiaaliluokissa. Runsasluomisuutta esiintyi 12 %:lla, ja korkea koulutustaso, miessukupuoli ja vaalea ihotyyppi lisäsivät riskiä. Varvasväleissä esiintyvät muutokset liittyivät diagnosoimattomaan diabetekseen. Atooppinen ihottuma, ruusufinni ja kynsien sieni-infektioon sopivat muutokset olivat yhteydessä matala-asteiseen tulehdukseen. Tämä lähes 2 000 henkilön koko ihon kliiniseen tutkimukseen perustuva työ raportoi ensimmäistä kertaa ihotautien yleistä esiintyvyyttä suomalaisessa, valikoitumattomassa väestössä. Tutkimus tukee aiempaa oletusta, jonka mukaan ihotaudit ovat aikuisväestössä yleisiä; ihon tutkiminen tulisi olla oleellinen osa potilaan kliinistä perustutkimusta.

Decreases of Life Expectancy Despite Decreases in Non-Communicable Disease Mortality: The Role of Substance Use and Socioeconomic Status

With the epidemiological transition, causes of death shifted from communicable to non-communicable diseases (NCDs) and life expectancy increased, as these NCD deaths occurred later in life. However, in the United States, over the past years, life expectancy has been stagnating or decreasing despite decreasing NCD mortality rates. Analyses of the most important underlying causes of death with increasing premature mortality reveal that psychoactive substance use played a crucial role for these increases. Furthermore, it can be shown, that a high proportion of the increased premature mortality and decreased life expectancies happened in lower socio-economic strata. Substance use policies should thus focus on lowering the gap between substance-attributable mortality in higher versus lower socioeconomic strata.

This Master's thesis focuses on the influence of socioeconomic status on the morphology of the mandible of individuals from the early medieval burial area in Mikulčice. This hillfort was an important center of power of the Great Moravian Empire with a stratified society. This work compares 2 different approaches on how to divide the inhabitants. The first of them is the division of individuals according to the location of graves into individuals from the castle, sub-castle, and hinterland. The second approach is the division of individuals according to the richness of grave equipment into individuals with rich and poor grave equipment. A different social status is associated mainly with different diets. Therefore, the attachments of the masticatory muscles are the most affected areas. A total of 132 individuals (59 males and 73 females) were analyzed. The material was evaluated by methods of geometric morphometrics. The used methods were CDP DCA, GPA, two-sample t-test, PCA, MANOVA, and SVM. Sexual dimorphism was observed in all sub-groups of the Mikulčice population. Men have larger mandibles with rami wider apart. The biggest differences are between individuals from the castle, the smallest between individuals with rich grave equipment.
