Sa?de ps?quica e condi??es de trabalho dos profissionais de sa?de nos hospitais da Universidade Federal do Rio Grande do Norte

Adotou como ponto de partida epistemol?gico a abordagem psicossociol?gica, considerando que a compreens?o s?cio-hist?rica do contexto ? indispens?vel para entender os fen?menos em an?lise. As condi??es de trabalho e a sa?de ps?quica foram abordadas interdisciplinarmente, fundamentando a constru??o de um modelo compreensivo de sa?de ps?quica que orientou a investiga??o. O caminho metodol?gico utilizado foi a pesquisa??o. Essa foi desenvolvida, utilizando t?cnicas como an?lise documental, observa??o participante, entrevistas n?o estruturadas, grupo focal e aplica??o de um protocolo de pesquisa composto pelo question?rio de condi??es de trabalho, pelo Question?rio de Sa?de Geral (QSG-60), pela Escala de Bem-Estar Afetivo no Trabalho (JAWS-12) e por quest?es sociodemogr?ficas. A an?lise dos resultados mostrou que os escores sintom?ticos de sa?de ps?quica variam por hospitais e que a sa?de ps?quica sofre influ?ncia das condi??es de trabalho, sobretudo em aspectos referentes a tr?s dimens?es: condi??es f?sicas e materiais; processos e caracter?sticas do trabalho e o ambiente sociogerencial. Em refer?ncia ? primeira dessas dimens?es, destacaram-se a exposi??o aos riscos psicobiol?gicos e de acidentes, bem como as exig?ncias de esfor?o f?sico. Na segunda dimens?o, a complexidade das atividades e a responsabilidade implicada nas mesmas. E, na ?ltima, os fatores de organiza??o da atividade, viol?ncia e ambiente conflitivo. Como a rela??o do indiv?duo com seu contexto ? dial?tica, os resultados encontrados corroboraram que quanto mais as condi??es de trabalho s?o desfavor?veis, maior a afeta??o da sa?de ps?quica e dos afetos com rela??o ao trabalho, repercutindo novamente no ambiente de trabalho. Travailler dans un réseau de santé : effets sur les pratiques professionnelles et la santé psychique au travail / Work in a health network : effects on the professional practices and the psychic health at work

Les nouveaux besoins en matière de soins et de prise en charge des patients ont contribué à la création des réseaux de santé. En plein développement, ils font partie intégrante du paysage du système sanitaire français. L’impact, pour les professionnels de santé, du travail en réseau fait aujourd’hui l’objet de plusieurs recherches et résultats contrastés, mais a peu été étudié du point de vue de la Psychologie Sociale, du Travail et des Organisations. Cette thèse a pour objectifs d’une part, de décrire les effets de la participation à un réseau de santé au niveau des pratiques professionnelles et au niveau de la santé psychique au travail, et d’autre part, d’examiner les variables et processus pouvant rendre compte de la variabilité de ces effets. Cette recherche privilégie une approche du réseau de santé comme un lieu potentiel de socialisation et de personnalisation (Malrieu & Malrieu, 1973) pour les professionnels qui en font partie La recherche sur le terrain a été réalisée en collaboration avec le réseau de santé ville-hôpital RéPPOP Midi-Pyrénées (Réseau Prévention et Prise en charge Obésité Pédiatrique) qui a la particularité de fonctionner sur la base de Trios/Duos de partenaires pour la prise en charge de l’enfant. Autour du médecin référent, ces partenaires peuvent être des diététicien(ne)s, des psychologues, des kinésithérapeutes… Nous posons l’hypothèse selon laquelle les effets de l’appartenance à un réseau de santé varient en fonction de variables individuelles (ancienneté de l’appartenance au réseau de santé et motivations initiales d’adhésion) et de variables organisationnelles (structuration en Duo/Trio et liens avec l’équipe de coordination du RéPPOP Midi-Pyrénées). Pour autant, une variable-clé, susceptible de moduler ces relations, sera placée au centre de notre recherche : la dynamique collective des Trios/Duos, analysée en référence aux concepts de « travail collectif », « collectif de travail » et « activité collective » (Caroly, 2010).Une étude par entretiens semi-directifs de recherche a été menée auprès de 20 professionnels libéraux et membres du réseau RéPPOP Midi-Pyrénées répartis en 4 Trios et 4 Duos. L’analyse des données a permis de caractériser trois types de dynamiques collectives différenciées quant aux ressorts du travail collectif, aux étayages du collectif de travail et à l’orientation intra-RéPPOP ou hors-RéPPOP de l’activité collective. Les résultats permettent d’appréhender comment chacune peut influer sur la relation entre transformation des pratiques et évolution de la santé psychique. De nouvelles perspectives de recherche et d’action auprès des partenaires du réseau sont proposées à l’issue de ces analyses. / The new needs as regards care and taking charge of patients have contributed to the creation of health networks. As they are growing fast, they are an integral part of the French sanitation system. The impact, on the health professionals, of networking is, today, the object of contrasted research and results, but has little been studied as far as Social Psychology, Work and Organizations are concerned. The aim of this thesis is, on the one hand, to describe the effects of the participation to a health network where professional practices and psychic health at work are concerned and, on the other hand, to examine the variables and process accounting for the variability of these effects. This research privileges an approach to the health network as a place of socialization and personalization (Malrieu & Malrieu, 1973). The research on the ground is carried out in collaboration with the health network town-hospital RéPPOP Midi-Pyrénées (Prevention and Taking charge of Pediatric Obesity Network) , which has the specificity to work on the basis of Trios/Duos of partners to take charge of children. Those partners can be doctors, dieticians, psychologists, physiotherapists… However , another key-variable, able to moderate these relationships, will be taken into account : the collective dynamics will be analyzed according to the theoretical concepts of “collective work”, “working collective” and collective activity (Caroly, 2010).We assume that the effects of being part of a health network vary according to individual variables ( length of time in the health network and initial adhesion motivations ) and to organizational variables ( structuring in Trios/Duos and links with the coordination team of the RePPOP Midi-Pyrénées ). Which will be modulated by the functioning dynamics between partners of the Trio/Duo. In a clinical approach, we have carried out semi-guided interviews with twenty private professionals and members of the RePPOP Midi-Pyrénées network, divided into 4 Trios and 4 Duos. The data analysis enabled us to characterize three kinds of collective dynamics, distinguished on collective work resources, working collective stayings and collective activity orientation. The results show how each pattern can have an influence on the relationship between practices changes and psychological health evolution. The results have enabled us to identify new explanatory elements of the variability due to belonging to a health network which supports research and application prospects on the ground next to the concerned professionals.

Elevhälsans komplexa uppdrag - "En skola för alla" : Rektorer och specialpedagogers syn på det förebyggande och hälsofrämjande arbetet i elevhälsan / Pupils' health complex assignment

The purpose of this study is to describe how the principal and the special pedagogue perceive and define pupil ill-health prevention and pupil health enhancement in their work. It explores how they understand the role they play in elevhälsan, literally meaning pupils' health. And how meetings are run and decisions are made within the pupils' health teams at selected schools. Questions that will be answered are: How do the principal and the special pedagogue perceive pupil ill-health prevention and health enhancement work of the pupil health team at the school? How do the principal and the special pedagogue perceive their roles in the health of the pupils? How are decisions made and followed through at the pupil healthcare meetings? This qualitative study used semi-structured interviews and phenomenological inspired methodology. Pupil health teams from three different schools were interviewed, and in total three principals and five special pedagogues took part in this study. The analysis of the results of this study show that further consensus between the different health promoting coordinators is needed in the implementation of the pupil ill-health prevention and health enhancement work. Further findings show inconsistencies in how the different pupil health teams conduct their meetings and in how decisions are implemented. A majority of the pupil health teams used the meetings to address existing problems, rather than ill-health prevention and health promotion efforts. The principals demonstrated that they have a greater consensus in how they define their role in promoting pupil health, as opposed to the special pedagogues who expressed a more varied picture of their role. This study also demonstrates that the pupil health team that was most effective in pupil health developments was the team that had further education in pupil health. Findings also show that the different health coordinators have an important role in furthering development and accessibility of the learning environment for the pupils, and in contributing to creating a 'school for everyone'. The demands on the special pedagogues to meet all pupils' differences, and to see these differences as an asset, are great. The requirement of a 'school for everyone' is a lot to ask for. To create a school that has learning environments tailored to everyone's individual needs requires an understanding and consensus between the different pupil health coordinators. This can sometimes be a dilemma, yet the pupil health coordinators have an important role to fill in these realms. This study also found that it was of utmost importance to have a clearly defined organisation and definition of the what the ill-health prevention and health enhancement work involved. To promote this the pupil health coordinators must create a meeting culture that supports the pupil ill-health prevention and health promotional work.

Hälsodiplomerad gymnasieskola -Förändringen av elevers hälsa mellan åren 2004 och 2008 : En resultatutvärdering

Elevers trivsel och påverkansmöjligheter i skolan minskar också med stigande ålder. Hälsofrämjande arbete i skolan kan därför vara värdefullt för att förbättra ungas hälsa. Det kan exempelvis ske genom Korpen Svenska Motionsidrottsförbundets koncept: Hälsodiplomerad gymnasieskola. I Västerås är Carlforsska gymnasiet en hälsodiplomerad skola. <strong>Syfte</strong>: Att studera om den hälsodiplomerade skolans hälsofrämjande arbete medfört någon förändring gällande elevers hälsa i årskurs två mellan åren 2004 och 2008. <strong>Metod</strong>: En resultatutvärdering genomfördes innefattande; en kvalitativ och en kvantitativ metod. Kvalitativ studie omfattade fyra intervjuer med skolpersonal. Underlag för den kvantitativa studien var ett insamlat material från undersökningen "Liv- och hälsa- Ung i Västmanland". <strong>Resultat</strong>: Skolpersonal upplevde att förbättringar skett gällande elevers hälsorelaterade levnadsvanor och ökad trivsel i skolan. Båda könen rapporterade förbättrad psykisk hälsa avseende minskad nervositet och ökad skoltrivsel. Pojkar hade också förbättrade resultat gällande; livskvalitet, irritation, stress, alkoholintag och snusvanor. Flickor rapporterade förbättrade resultat rörande; fysisk aktivitet, frukostintag och utökad arbetsro i skolan. <strong>Slutsatser</strong>: Skolans hälsofrämjande arbete har troligtvis bidragit till att förbättra elevers hälsa. Det är ett långsiktigt arbete och fler studier inom området behövs för att utläsa dess egentliga effekter på hälsa.</p></p></p> / <p><p><p><strong>Background</strong><strong>:</strong> Studies have shown that young people's experienced- and psychological health and their health related habits declines with age. Student's comfort and influence opportunities in school also weaken with age. Health promotion work in school might therefore be valuable to increase young people's health. This could for instance be achieved through Korpen - the Swedish exercise union's concept: Health certified school. Västerås, a city in the county of Västmanland, has one health certified high school, Carlforsska. <strong>Aim</strong>: To study if the health certified school's health promotion work has resulted in any change concerning the second grade student's health, between the years 2004 and 2008. <strong>Method</strong>: A outcome evaluation has been conducted including both qualitative and quantitative method. The qualitative method included four interviews with school staff. The foundation for the quantitative study was a collected material from a survey called "Liv och hälsa- Ung in Västmanland". <strong>Result</strong>: School staff experienced improvements concerning student's health related habits and increased comfort in school. Both boys and girls reported improved psychological health such as lower feelings of nervousness and improved comfort in school. Boys had also improved results in; self reported quality of life, irritation, stress and alcoholand snuff use. Girls improved their results in; physical activity, breakfast habits and improved feelings of better working conditions in school. <strong>Conclusion</strong>: The school's health promotive work has probably contributed to the student's improved health results. Working with health in school is a longterm process and more studies are needed to assure the detailed influence by health promotive work on young people's health.</p></p></p>

Hälsofrämjande och förebyggande elevhälsa : från nationell till kommunal och lokal nivå

<p>The perspective on health among school pupils needs to be extended. School is animportant arena for the promoting of health among youth, primarily from a healthpromoting perspective and a proactive perspective. A holistic approach has gatheredsome ground in the policy documents for the nine year compulsory school in Sweden, butin what way, and what effect does it have on the governing authorities and on schoollevel?The aim of the study is to investigate the coherence between national, municipal andlocal levels regarding the view on health among pupils. In order to do this, the guidelinesfor pupil health in the municipality of Örebro and operational plans for seven F9 schoolsin the municipality of Örebro will be studied and compared to the government bill”Health, learning and sense of security”. In order to obtain a theoretical understanding ofthe course of events between the different organizational levels, intervention theory hasbeen used as an explanatory model.Through analysis of the municipal guidelines for youth and pupil health, shows that thedocument very much resembles the government bill, which becomes apparent through thesame abstract formulations used. The content of the operational plans, particularly thequestion of preventing ill-health, therefore generally stays on an abstract level as well.Consequently, no concrete measures are taken on school level.From what the government bill emphasizes, the strongest reflection in the operationalplans is the point of quality control in the pupil health work and the participation ofparents. The operational plans are influenced by documents that describe how the pupilhealth work is to be organized, but there is still no policy document available for pupilhealth on local school level that includes health promoting and preventive measures. Thusthe schools lack the tools needed to tackle ill-health from a holistic point of view.However, several measures are available when it comes to reactive measures such asbullying and abusive behaviour.</p> / <p>Det finns ett behov av en utvidgad syn på elevhälsa som uppmärksammar fler aspekter avhälsa. Skolan är en viktig arena för arbetet med barn och ungdomars hälsa, inte minstutifrån ett hälsofrämjande och förebyggande perspektiv. I de dokument som styr skolanhar ett helhetsperspektiv på elevers hälsa fått ett allt större utrymme men på vilket sättoch vad innebär det, från de styrande organen för skolan och ner till personalen som skabemöta eleverna?Syftet med studien är att undersöka överensstämmelsen mellan en nationell och enkommunal och en lokal nivå vad gäller elevhälsa. Konkret innebär det att granska Örebrokommuns riktlinjer för barn- och elevhälsa och sju verksamhetsplaner för F9-skolor iÖrebro kommun utifrån regeringspropositionen ”Hälsa, lärande och trygghet”. För att fåen sammanhängande teoretisk beskrivning av skeendet mellan de olika organisatoriskanivåerna har en interventionsteori används som förklaringsmodell.Det som framkommit genom analys av det kommunala dokumentet är att det i stort ärväldigt likt propositionen, vilket bland annat innebär att det som formulerats i abstraktaordalag i propositionen sällan formuleras mer konkret i kommunens riktlinjer. Det somblir skrivet i verksamhetsplanerna, i synnerhet det som rör att förebygga psykisk ohälsa,blir därför också i vissa fall bara till abstrakta målformuleringar som aldrig tar form ikonkreta åtgärder eller insatser.Det som starkast avspeglas i verksamhetsplanerna utifrån det som framhålls ipropositionen är vikten av att kvalitetssäkra elevhälsoarbetet och att arbeta med eleversoch föräldrars delaktighet. Till stor del formas verksamhetsplanerna utifrån dokumentsom konkret beskriver hur arbetet med elevhälsa ska bedrivas, dock finns det inte någotelevhälsodokument på lokal nivå som generellt beskriver ett förebyggande ochhälsofrämjande arbete. Således har skolorna få verktyg för att angripa ohälsa ur etthelhetsperspektiv. Däremot finns det ett flertal specifika åtgärder till redan befintligproblematik, likt mobbning och kränkande behandling.</p>

<p>Arbetet utgör en stor del av våra liv och har stor betydelse för den personliga hälsan. Hälsofrämjande insatser som riktar sig mot arbetslivet kan på så sätt påverka den nationella folkhälsan. Ett sätt att skapa delaktighet och inflytande på arbetsplatsen är att årligen genomföra individuella samtal mellan chef och medarbetare. Frågan vi ställde oss inför studien var <em>Om medarbetarsamtal kan ses som hälsofrämjande? Syftet </em>var att undersöka hur medarbetare och chefer på ett sjukhus upplever hälsofrågor i medarbetarsamtal och utifrån detta studera vilka möjligheter till lärande och förändring som finns. <em>Metoden </em>för datainsamlingen var en intervjustudie med fyra medarbetare och fem chefer utifrån en halvstrukturerad intervjuguide. <em>Resultatet, </em>genom kvalitativ innehållsanalys, visade att både medarbetare och chefer var positiva till medarbetarsamtal men att cheferna upplevde brister i sin kompetens med hälsofrågorna och medarbetarna upplevde att hälsofrågorna tenderade till att vara bedömande. Ur folkhälsopedagogisk synvinkel dras <em>slutsatsen </em>att syften och gränser för hälsofrågorna bör klargöras för att åstadkomma en hälsofrämjande effekt. Det krävs ett öppnare klimat i medarbetarsamtalet där både medarbetares och chefers kunskaper accepteras och uppmärksammas i syfte att lära av varandra och utvecklas tillsammans.</p> / <p>Work means a lot to the human being in life and it has a huge importance on the individual health. The question we asked ourselves before the study was; <em>could staff appraisals be seen as health promoting?</em> The <em>aim </em>with the study was to investigate how co-workers and supervisors at a hospital feel about health questions in the staff appraisal and on the basis of that, study what possibilities for learning and change there is. <em>Method </em>for the collection of data was an interview study containing four co-workers and five supervisors on the basis of a half structured interview guide. The <em>Result, </em>based on qualitative content analysis, showed that both co-workers and supervisors where positive to the staff appraisal but the supervisors felt lack of competence with the health questions and co-workers experienced the health questions as tending to be to judging. Out of a health promotion and education perspective the <em>conclusion </em>is that proposals and boundaries for the health questions get explained and clear to achieve a health promoting effect. This requires a more open climate in the staff appraisal where both co-worker's and supervisor's knowledge is accepted and noticed, aiming to learn and develop by each other.</p><p> </p>

Student's comfort and influence opportunities in school also weaken with age. Health promotion work in school might therefore be valuable to increase young people's health. This could for instance be achieved through Korpen - the Swedish exercise union's concept: Health certified school. Västerås, a city in the county of Västmanland, has one health certified high school, Carlforsska. Aim: To study if the health certified school's health promotion work has resulted in any change concerning the second grade student's health, between the years 2004 and 2008. Method: A outcome evaluation has been conducted including both qualitative and quantitative method. The qualitative method included four interviews with school staff. The foundation for the quantitative study was a collected material from a survey called "Liv och hälsa- Ung in Västmanland". Result: School staff experienced improvements concerning student's health related habits and increased comfort in school. Both boys and girls reported improved psychological health such as lower feelings of nervousness and improved comfort in school. Boys had also improved results in; self reported quality of life, irritation, stress and alcoholand snuff use. Girls improved their results in; physical activity, breakfast habits and improved feelings of better working conditions in school. Conclusion: The school's health promotive work has probably contributed to the student's improved health results. Working with health in school is a longterm process and more studies are needed to assure the detailed influence by health promotive work on young people's health.

Abstract The purpose of this study was to analyze in what extent Umeå municipality health coaches uses their policy "work environment, health and rehabilitation" in social services workplaces. Furthermore was the purpose to analyze the policy "work environment, health and rehabilitation" with help from Bacchi’s policy analyzing model. The study was based on a qualitative design and involved seven health coaches from the social service. Interviews were conducted and analyzed with sentence concentration. The result showed that it differs from person to person on which view on health they have and what they believe represent health promoting work. Most of the interviewees notions and understandings of health and health promoting work are however the same as the municipality’s. The health coaches follow the municipality’s directions on how the assignment should be executed, though they vary in the degree of engagement. Furthermore, all of the interviewees experience that they have the support from the municipality that they need. A few of them though lack the support from their leader. Finally, the result from the policy-analyze showed that Umeå municipality’s problem was that their quality of work was inadequate. It also showed that the municipality put a lot of responsibility on the leaders, but that they should take more accountability themselves.

Η παρούσα διατριβή πολυπαραμετρικής ανάλυσης και αξιολόγησης των ενεργειών προαγωγής της υγείας και της ασφάλειας στις επιχειρήσεις ολοκληρώνεται με τη δόμηση ενός ολιστικού προτύπου διαχείρισης ποιότητας της εργασιακής ζωής. Στο πρώτο κεφάλαιο πραγματοποιείται επισκόπηση του πεδίου της υγιεινής και ασφάλειας (επιστημονικές μελέτες, νομοθετικό πλαίσιο, δράσεις κοινωνικών φορέων). Στο δεύτερο κεφάλαιο προσδιορίζεται το πεδίο έρευνας της διατριβής. Η προαγωγή της υγείας και της ασφάλειας αποτελεί σήμερα την αιχμή των ενεργειών πρόληψης των κινδύνων. Στο τρίτο κεφάλαιο καταγράφονται οι τεχνικές πραγματοποίησης και αναλύονται τα δεδομένα της εμπειρικής έρευνας (εξετάσεις, συμπεριφορά, οργάνωση, κοινωνικά προγράμματα, ασφάλεια και φυσικό περιβάλλον). Τέλος στο τέταρτο κεφάλαιο στοιχειοθετείται η τελική πρόταση που περιλαμβάνει τα κεντρικά αποτελέσματα της έρευνας. Δομείται ένα ολιστικό πρότυπο το οποίο απαντά στις απαιτήσεις συνολικότητας στην ποιοτική διαχείριση των επαγγελματικών κινδύνων. / The present thesis lies in the field of working health and safety promotion (WHSP) which is the enabler of all activities aiming at the prevention of occupational hazards.After an extended review of the related literature, legislation and social actions (Chapter 1), we concentrate on the role of WHSP and we develop a framework to be used in its study (Chapter 2). The 3rd chapter discusses the results of an empirical research conducted on the basis of the aforementioned framework that extends over the areas of health screening, health behaviour promotion, organizational change, social welfare and the physical environment. Finally, in the 4th Chapter, based on the results of our research and through a multi-factor analysis we arrive at a set of propositions which are integrated into an holistic model. The model can be used to provide guidelines in response to the requirements concerning occupational hazards within the framework of total quality management.
