Povědomí zdravotníků o psychosociální intervenční péči / Health care professional awereness about of psych-social intervention care

Awareness of psychosocial intervention care among healthcare professionals Abstract Aims, research questions and hypotheses: The aim of the research is to evaluate the awareness of healthcare professionals about psychosocial intervention care. Goal 1: Identify the real situation in psychosocial intervention care. Goal 2: Identify the possibilities of offering and using psychosocial intervention care in healthcare facilities. Methods and research file: Quantitative and qualitative data collection techniques were used. A quantitative survey was conducted using questionnaires distributed to healthcare professionals in selected organizations (Liberec and Jičín Regional Hospital and University Hospital Hradec Králové). The size of the research group is 170 respondents. A qualitative part was realized with semi-structured interviews with peers in the same organizations. In total, we reached five peers. Results and conclusions: The research shows that health professionals perceive their work as mentally and physically demanding. The difficulty of the healthcare profession is perceived through all age categories, across disciplines and regardless of the length of practice. Half of the respondents considered a change of a job due to mental and/or physical demands. More than half of health care professionals are aware of psychosocial intervention care system. The survey further shows that healthcare professionals find themselves in extreme mental stress that would potentially require psychosocial intervention care. Only a low percentage of healthcare professionals will use this service. A qualitative survey shows that healthcare professionals are afraid of loss of anonymity when using psychosocial intervention care. This is related to mistrust in peers. From a peer point of view, it is important to be more aware of this service, helping healthcare professionals in their psychologically demanding occupation, which will increase their use and efficiency. The output of this thesis will be a proposal to include topics on psychosocial intervention care in regular training for employees.

Este estudo se pretende reflexão crítica a partir da apresentação das experiências de uma Enfermeira de CAPS. Trata-se de Narrativa baseada na historiobiografia com descrição detalhada do processo de implantação, organização e cenários da experiência do cotidiano do trabalho em serviço de saúde mental e da rede de atenção a saúde. Parte de reflexão crítica sobre as principais políticas de saúde mental cotejada com experiências da autora em trechos de diário de bordo e também das experiências em outros CAPS da cidade descritas por outros autores. Destacam-se a complexidade do trabalho em CAPS e do ato de estabelecer rede. Sublinham-se os desafios para as políticas públicas e políticas territoriais colocando como questão aos trabalhadores, cidadãos e governos a problematização e valorização das experiências do cotidiano como meio de compreensão das demandas e organização dos serviços para oferta de cuidados em saúde de modo articulado e resolutivo, bem como o sofrimento destes trabalhadores e a dificuldade de manterem-se produzindo sentido e prazer nesse modo de estar no mundo. . / This study intends to reflect critically from the presentation of the experiences of a CAPS Nurse. It is a Narrative based on historiobiography with a detailed description of the process of implantation, organization and scenarios of the daily experience of work in mental health service and the health care network. Part of critical reflection on the main mental health policies compared with the experiences of the author in excerpts from the logbook and from the experiences in other CAPS around the city described by other authors. The complexity of the work in CAPS and the act of establishing a network are highlighted. It highlights the challenges for public policies and territorial policies, placing as a matter of question to workers, citizens and governments the problematization and valuation of everyday experiences as a means of understanding the demands and organization of services to offer health care in an articulated and resolutive manner as well as the suffering of the workers and the difficulty of keeping producing meaning and pleasure in this way of being in the world..

Logopeders psykosocialaintervention för personermed afasi

SAMMANFATTNING Det finns ett stort behov av psykosociala insatser för personer med afasi (PMA); docksaknar många logopeder kunskap om logopedisk psykosocial intervention för PMA. Enkunskapsöversikt i ämnet vore därför önskvärt. Syftet med studien är att sammanställaresultat från tidigare studier om hur logopeder arbetar kliniskt med psykosocialintervention för PMA. Studien är en litteraturöversikt och litteratursökningen utfördesmellan september och oktober 2020 i ett flertal databaser. Artiklarna behövde uppfyllaspecifika kriterier för att inkluderas. Åtta artiklar identifierades, varav tre beskrev tvåolika psykosociala interventioner för PMA. Resterande fem studier beskrev logopedersanvändning och inställning till psykosocial intervention. Psykosocial interventiontillämpades genom olika metoder inklusive psykoterapeutiska metoder; få renodladepsykosociala metoder identifierades. Resultaten visade att det finns en osäkerhet blandlogopederna gällande psykosocial intervention på grund av kunskapsbrist och oklarheteri arbetsfördelning mellan kurator, psykolog samt logoped. Vidare beskrevs en oklarhetgällande begreppet och definitionen av logopedisk psykosocial intervention. Samtligastudier kvalitetsgranskades och de två interventionsmetodernas evidensnivå bedömdes;båda interventionsstudierna visade på otillräcklig evidens. / ABSTRACTThere is a great need for psychosocial interventions for people with aphasia (PWA);however, many speech-language pathologists (SLP) lack knowledge about psychosocialintervention for PWA. Therefore, a comprehensive overview about the subject isrequired. The aim of this study is to compile and critically examine results fromprevious research about SLPs’ clinical work within psychosocial intervention for PWA.The study is a literature review, and the literature search was conducted duringSeptember and October 2020 using several databases. The articles needed to fulfillspecific criteria to be included. In total, eight articles were identified; three of themdescribed two psychosocial interventions for PWA, the remaining five studies describedhow psychosocial intervention is managed by SLPs and their attitude towardaddressing psychosocial needs for PWA. Psychosocial intervention was applied throughvarious methods, including psychotherapeutic methods, few pure psychosocial methodswere identified. The results indicate that the SLPs had low confidence due to lack ofknowledge and uncertainty of work allocation between counselor, psychologist andSLP. Furthermore, an ambiguity was described regarding the concept and definition ofpsychosocial intervention. The studies quality and the strength of the evidence wereassessed, both intervention studies showed insufficient level of evidence.

Medication Use Reported by Individuals With Tinnitus Who Are Seeking Internet-Based Psychological Interventions

PURPOSE: This study examined medication use by individuals with tinnitus who were seeking help for their tinnitus by means of a psychological intervention. METHOD: This study used a cross-sectional survey design and included individuals with tinnitus enrolled in an Internet-based cognitive behavioral therapy trial ( = 439). Study participants provided demographic details, completed various structured questionnaires and provided details about the medications used. The self-reported medications were classified using the United States Pharmacopeial Medicare Model Guidelines v7.0. RESULTS: Current medication use was reported by 67% ( = 293) of the study participants. Those currently using medication were older; had consulted their primary care physician, had greater tinnitus severity, depression, anxiety, and insomnia when compared with those not reporting any current medication use. The top 10 medication used included cardiovascular agents ( = 162; 55.3%), antidepressants ( = 80; 27.3%), electrolytes/minerals/metals/vitamins ( = 70; 23.9%), respiratory tract/pulmonary agents ( = 62; 21.2%), anxiolytics ( = 59; 20.1%), hormonal agents/stimulant/replacement/modifying (thyroid; = 45; 15.4%), gastrointestinal agents ( = 43; 14.7%), analgesics ( = 33; 11.3%), blood glucose regulators ( = 32; 10.9%), and anticonvulsants ( = 26; 8.87%). Some associations between type of medication used and demographic or tinnitus-related variables were noted especially for the cardiovascular agents, electrolytes/minerals/metals/vitamins, and anxiolytics. CONCLUSIONS: This exploratory study indicated a large percentage of patients using medication and a range of medications. Further studies are required to assess the effects of such medications on the tinnitus percept and concurrent medication moderate treatment effects.

The use of support groups in the treatment of cancer : an interactional approach

This study addresses the importance of studies of human psychoneuroimmunology in understanding the role of psychological factors in cancer. Research trends in psychosocial aspects of cancer are reviewed, exploring the role of distress and the support group as an intervention which potentially reduces distress through enhancing interpersonal relationships, emotional adjustment and communication with health professionals, in these ways helping the patient to cope with the symptoms of treatment. In South Africa, most hospitals which treat cancer patients medically do not simultaneously have support groups for the newly diagnosed patient to join in order to discuss immediate fears and acquire more knowledge about their particular disease. Following an experimental cancer support group involving patients who had recently undergone a bone marrow transplant (some considerable time after their first cancer diagnosis) in Cape Town's Groote Schuur Hospital, the researcher, as one of the participants in the fortnightly meetings convened to dicuss psychosocial issues related to each patient's experience of the cancer and transplant process, transferred the themes, concepts and questions that arose in that scenario, to a cancer clinic in Pretoria where recently diagnosed patients were asked to volunteer to participate in such a group. It was felt that these patients would derive some benefit early in their treatment programme. Psychosocial concerns are left to the individual patient to seek therapy should it be required. Presently, it is reported in the body of knowledge about cancer, that interventions aimed at alleviating the psychosocial distress of cancer patients highlight hypnosis, guided imagery and relaxation therapy. It is contended that a more appropriate intervention for the majority of people with cancer in South Africa would entail a fellow-patient support group meeting on a regular basis. / Psychology / (M.A. (Psychology)

L'intervention psychosociale dans le sport pour développer des habiletés de vie chez des jeunes à risque de délinquance

Implanté en 1999 dans le quartier Centre-Sud à Montréal, le programme d'intervention psychosociale « Bien dans mes baskets » (BdmB) utilise le basketball comme outil pour entrer en contact avec des jeunes à risque de délinquance. Ce programme se distingue par le fait que ses entraîneurs-travailleurs-sociaux (ETS) interviennent tant dans le développement d'habiletés de vie que d’habiletés sportives. La présente étude vise à comprendre comment les expériences vécues au sein de BdmB ont pu contribuer au développement d’habiletés de vie chez les adolescents qui ont participé au programme lorsqu’ils étaient à l’école secondaire. Une méthodologie rétrospective qualitative a été utilisée pour cette étude. Des entretiens semi-dirigés ont été effectués auprès de 14 anciens athlètes-étudiants (AÉ) masculins qui ont participé à BdmB pendant leurs études secondaires. Le verbatim des entretiens enregistrés a été transcrit afin de procéder à une analyse de contenu par thématique. Les résultats suggèrent que quatre principaux facteurs semblent contribuer au développement d’habiletés de vie chez certains participants : le modelage de l’ETS a permis aux AÉ de reproduire les comportements de leur entraîneur; le développement d’un sentiment d’attachement entre les joueurs a engendré la création d’un nouveau réseau social dans lequel les AÉ ont pu trouver du soutien social; une culture d’équipe influencée par la philosophie de BdmB semble avoir été intériorisée par les joueurs; et BdmB a agi dans certains cas comme facteur de protection contre des influences externes négatives. Les résultats suggèrent que le sport collectif combiné à des interventions psychosociales favoriserait le développement d’habiletés de vie. / The “Bien dans mes baskets” (BdmB) program, implemented in 1999 in a high school of Montreal, uses basketball to reach adolescents at risk of delinquency. The distinctive characteristic of BdmB is that the coaches are also social workers (CSW) who implement strategies to develop life skills among athlete-students (AS) who partake in the basketball extracurricular program. The aim of this study was to examine how the experiences lived through BdmB contributed to the development of life skills in young adults who have participated in the program when they were in high school. We used a retrospective qualitative study design. Semi-structured interviews were conducted with 14 young adults who were involved in the BdmB program during their high school. Interviews were recorded and transcribed verbatim. A thematic content analysis of participants' accounts was performed to identify the specific role of team sport involvement with psychosocial interventions on the alumni players’ life skills development. The results suggest that 4 main factors contributed to life skills development in AS: modeling the CSW who epitomize the BdmB philosophy enables AS to emulate their coach’s behaviors; bonding between AS creates a new social network in which AS can find social support; a BdmB culture is internalized by sharing experiences among the BdmB AS and the CSW; BdmB has a protective effect against negative influences from the high-risk milieu in which AS live. The results suggest that team sports combined with psychosocial interventions has the potential to foster life skills development in youth at risk.

Développement du sentiment d'appartenance à une équipe sportive et persévérance scolaire : le cas des jeunes du programme «Bien dans mes baskets»

«Bien dans mes Baskets» (BdmB) est un programme d’intervention psychosociale utilisant le basketball parascolaire comme outil de prévention du décrochage scolaire et de la délinquance chez des adolescents en difficulté ou présentant plusieurs facteurs de risque. Le programme a comme objectifs généraux de prévenir le décrochage scolaire, la délinquance et l’exclusion sociale. La présente étude a comme principaux objectifs d’explorer le processus de construction du sentiment d’appartenance à l’équipe et à l’école des athlètes-étudiants participant au programme BdmB et d’examiner le lien entre le sentiment d’appartenance à l’équipe et à l’école chez ces derniers. Des entretiens semi-directifs ont été réalisés auprès d’un échantillon aléatoire de 21 athlètes-étudiants de sexe masculin inscrits dans le programme BdmB. Nous avons choisis au hasard sept athlètes-étudiants dans trois équipes de catégories d’âge différentes (atome, cadet et juvénile) afin de mettre en évidence la progression potentielle du sentiment d’appartenance dans le temps. Le verbatim des entretiens enregistrés a été transcrit afin de procéder à une analyse inspirée de la théorisation ancrée. Les résultats permettent l’identification de trois conditions qui paraissent jouer un rôle déterminant dans la construction du sentiment d’appartenance, soit : 1) le partage d’expériences communes significatives, 2) la valorisation de la pratique du basketball au sein de BdmB, et 3) le climat de l’équipe et du programme. Ces conditions découlent de l’interaction et du cumul de nombreux facteurs individuels et interpersonnels ainsi que de facteurs liés au programme. Pour certains athlètes-étudiants, le basketball parascolaire semble même favoriser la motivation scolaire. Pour plusieurs, le sentiment d’appartenance à l’équipe est indissociable du sentiment d’appartenance à l’école. / "Bien dans mes Baskets" (BdmB) is a psychosocial intervention program using basketball as a tool to prevent school dropout and delinquency among adolescents in difficulty or at risk. The general goals of the program are to prevent school dropout, delinquency and social exclusion. This study has two main objectives: 1) investigate the construction process of a sense of belonging to the team and to the school among athlete-students participating in BdmB, and 2) explore the potential connection between the basketball players' sense of team belonging and sense of school belonging. Semi-structured interviews were conducted with a sample of 21 male student-athletes enrolled in the program BdmB. We randomly selected seven student-athletes in three teams of different age categories (atome, cadet and juvénile) to highlight the potential progression of the sense of belonging over time. The interviews were recorded and transcribed verbatim in order to conduct an analysis inspired by grounded theory. The results allow the identification of three conditions that appear to play a key role in the construction of a sense of belonging: 1) the sharing of meaningful group experiences, 2) the valorization of playing basketball in BdmB, and 3) the team and program climate. These conditions arise from the interaction and combination of many individual factors, interpersonal factors and program factors. For some athlete-students, afterschool basketball seems to encourage academic motivation. For most of the athlete-students, their sense of team belonging is tantamount to their sense of school belonging.

Esta pesquisa tem a intenção de, referenciada no campo da psicologia social e nos debates que envolvem políticas públicas, saúde mental e saúde coletiva, investigar os modos como os trabalhadores de saúde mental desenvolvem suas práticas nos Centros de Atenção Psicossocial (CAPS) a partir do entendimento de território como ethos. Parte-se do testemunho do uso polissêmico e reificado do termo território por trabalhadores e gestores, bem como da consideração da cidade como o campo, por excelência, da produção de vida e das articulações em rede dos diferentes dispositivos de assistência. Como metodologia optou-se pela descrição densa da experiência profissional do pesquisador e da escuta da experiência dos trabalhadores de CAPS. Este dispositivo foi eleito pela constatação da importância que assumiu para a política nacional de saúde mental, escolhido pelas políticas governamentais como eixo da reforma psiquiátrica e com a tarefa de transformar os modos de relação entre loucura e sociedade. Optou-se pelo Grupo Operativo enquanto instrumento para investigação da experiência dos trabalhadores, que permitiu a emergência, nas discussões, das noções de território, práticas construídas a partir dessa noção e na relação da equipe com o serviço, com os usuários. Foram estudados cinco grupos em quatro CAPS de diferentes regiões da cidade de São Paulo, propondo-se como tarefa Falem sobre como se trabalha neste CAPS considerando a política atual de Saúde Mental. Na análise das falas buscou-se estabelecer uma relação dialógica entre a experiência desses trabalhadores, seus pontos de vista, em comunicação com a experiência do pesquisador e também com o entendimento dos autores que nos orientaram bibliograficamente. Assim, teceu-se um diálogo entre as narrativas dos trabalhadores e os autores eleitos nesta pesquisa, a fim de compreender como esses profissionais construíram suas próprias práticas e o que influenciou este processo. Na fala dos trabalhadores, os CAPS têm ocupado um lugar centralizado no cuidado oferecido aos sujeitos em sofrimento psíquico, principalmente pela dificuldade dos dispositivos da rede da saúde em acolhe-los em suas necessidades. Percebe-se a rede de saúde como ainda incipiente e com vazios entre os serviços e, além disso, a rede de atenção psicossocial ainda não dispõe suficientemente de outros serviços importantes em se tratando de produção de vida. Mesmo com o tensionamento para burocratização dos serviços, os trabalhadores têm se empenhado em criar ações pautadas pela realidade psicossocial dos sujeitos e das potencialidades dos territórios onde se encontram. Entretanto, esse manejo é limitado: as condições em que os serviços estão inseridos os levam ao cansaço, sofrimento e desanimo. A noção de território mostrou-se polissêmica: área geográfica, rede de saúde, território afetivo, entre outros, sendo área territorial adstrita o sentido mais comum; em algumas circunstancias a polarização dentro-fora levava a ações burocratizadas. A partir desses diálogos foi se elaborando a noção de território como um ethos, que seria um modo de habitar o mundo que orienta o cuidado no sentido de se colocar como presença humana diante do outro, apoiando a construção de outras moradas possíveis para esses sujeitos em um movimento de subjetivação com o outro / Referenced in the field of social psychology and debates involving public policies, mental health and collective health, this research intends to investigate the ways in which mental health workers develop their practices in the Psychosocial Care Centers (CAPS), from the understanding of territory as ethos. It begins with the testimony of the polisemic and reified use of the term territory by workers and managers, as well as the consideration of the city as the field, par excellence, of the production of life and the networked articulations of the different assistance devices. As a methodology we opted for the dense description of the researcher\'s professional experience and listening to the experience of CAPS workers. This device was elected by the recognition of the importance that it assumed for the national policy of mental health, chosen by governmental policies as axis of the psychiatric reform and with the task of transforming the modes of relation between madness and society. The Operational Group was chosen as a tool to investigate the workers\' experience, which allowed the emergence, in the discussions, of notions of territory, practices built from this notion and in the relationship of the team with the service, with the users. Five groups were studied in four CAPS from different regions of the city of São Paulo, being proposed as task \"Talk about how to work in this CAPS considering the current policy of Mental Health\". In the analysis of the speeches we tried to establish a dialogical relationship between the experience of these workers, their points of view, in communication with the researcher\'s experience and also with the understanding of the authors who guided us bibliographically. Thus, a dialogue was established between the narratives of the workers and the authors elected in this research, in order to understand how these professionals built their own practices and what influenced this process. In the workers\' speech, the CAPS have occupied a centralized place in the care offered to the subjects in psychic suffering, mainly due to the difficulty of the devices of the health network in receiving them in their needs. The health network is perceived as still incipient and with gaps between services and, moreover, the psychosocial care network still does not have enough of other important devices when in terms of life production. Even with the tension for bureaucratization of services, the workers have been committed to create actions guided by the psychosocial reality of the subjects and the potentialities of the territories where they are. However, this management is limited: the conditions in which services are inserted lead to tiredness, suffering and discouragement. The notion of territory proved to be polysemic: geographic area, health network, affective territory, among others, being territorial area attached the most common sense; In some circumstances the polarization inside-out led to bureaucratic actions. From these dialogues, the notion of territory as an ethos was developed, which would be a way of inhabiting the world that guides care in order to place itself as a human presence before the other, supporting the construction of other possible dwellings for these subjects in a Movement of subjectivation with the other

Como dialogar com homens autores de violência contra mulheres? Etnografia de um grupo reflexivo / How to dialogue with male perpetrators of violence against women?

Este estudo busca compreender as modalidades e condições do sucesso prático de uma intervenção junto a homens autores de violência contra mulheres, na perspectiva da promoção da equidade de gênero. MÉTODO. Foi estudado o trabalho de três profissionais, homens, que organizam um grupo reflexivo para homens autuados por infração à Lei Maria da Penha. Seguindo uma abordagem qualitativa, utilizou-se o método etnográfico, combinando observação participante, entrevistas em profundidade com os três facilitadores, e análise documental. Os resultados foram analizados por contraste com um referencial teórico articulando, entre outros, o quadro da vulnerabilidade e dos direitos humanos em saúde, o conceito de masculinidade hegemônica, a perspectiva feminista do cuidado, e a abordagem psicodinâmica do trabalho. RESULTADOS. Os resultados e sua discussão foram organizados em quatro eixos: primeiro, descrição do modo como o contexto social impacta o processo de trabalho, simultaneamente abrindo possibilidades e configurando desafios para a intervenção; segundo, identificação e discussão das formas de sucesso prático alcançadas pelos profissionais em seu trabalho junto aos homens abordados; terceiro, análise da dimensão intersubjetiva do trabalho, buscando compreender dificuldades encontradas pelo pesquisador e pelos facilitadores do grupo, na relação interpessoal com participantes do grupo; e, finalmente, ampliamos a discussão dos resultados, refletindo sobre o papel da saúde na prevenção da violência contra mulheres e na promoção da equidade de gênero. CONCLUSÃO. Duas tendências contraditórias atravessam a relação dos profissionais estudados com seu trabalho: por um lado, sua sensibilização prévia aos pontos de vista das mulheres os leva a se posicionar como aliados dos movimentos feministas. Neste âmbito, procuram dialogar com homens autores de violência, mobilizando sua própria socialização masculina a serviço da prevenção da violência contra mulheres; por outro lado, este trabalho no âmbito dos \"jogos de linguagem\" típicos da socialização masculina exige uma indiferença seletiva aos pontos de vista das mulheres, em particular quando não condizem com projetos de felicidade masculinos: esta clivagem é necessária para manter a empatia com os homens, condição para um diálogo bem-sucedido. Paradoxalmente, isto resulta em uma perda de empatia dos profissionais pelas mulheres que contradiz o sentido político - profeminista - do seu trabalho. Portanto, no que tange ao diálogo com homens autores de violência contra mulheres, o profeminismo se apresenta simultaneamente como um pré-requisito e um empecilho. Embora nunca se livrem do paradoxo, os profissionais estudados conseguem resolvê-lo no plano da prática, que alcança assim formas específicas de sucesso. As estratégias que desenvolvem neste fim podem inspirar novas práticas de atenção aos autores de violência contra as mulheres em serviços de saúde / This study aims to investigate the modalities and conditions of practical success in an intervention with male perpetrators of violence against women, within the perspective of gender equity promotion. METHODS. We study the work of three male professionals, who organize a reflexive group for sued male perpetrators. Following a qualitative approach, the ethnographic method was employed, combining participant observation, in-depth interviewing with the three facilitators, and document analysis. Results were analysed by contrast with a theoretical reference frame which articulated the frame of vulnerability and human rights in health, the concept of hegemonic masculinity, the feminist perspective on care, and psychodynamic approach of work. RESULTS. The results and its discussion were organized according to four axes of analysis: first, we analyze the social context and its impacts on the work process (opportunities and challenges); second, we describe and discuss forms of practical success identified within the professionals\' work; third, we focus on the subjective dimension of this work, analyzing problems encountered by the researcher and the group facilitators in interpersonal relationships with the group participants, and coping strategies developed by the facilitators; and finally, we expand the discussion of the results to understand possible contributions of health policies to the prevention of violence against women and the promotion of gender equity. CONCLUSION. Two contradictory tendencies pervade the relationship of these professionals to their work. On one side, their raised awareness of women\'s viewpoints leads to their engagement as allies to feminist movements. They seek to dialogue with male perpetrators, putting their own masculine socialization at the service of the prevention of violence against women; yet on the other side, this work within masculine socialization\'s typical \"language games\" demands a selective indifference to women\'s viewpoints, especially when they don\'t match male felicity projects: this cleavage is requested to maintain the empathy towards men requested to achieve successful dialogue. Paradoxically, this results in a loss of the professionals\' empathy towards women, which contradicts the (profeminist) political meaning of their work. Thus, regarding dialogue with male perpetrators of violence against women, profeminism stands simultaneously as a pre-requisite and a cumber. Although they never get rid of this paradox, the studied workers find solutions to it through their practice, hence achieving specific forms of success. The strategies developed for this purpose can inspire new care practices aimed at male perpetrators of violence against women within health services
