InquietAÇÕES do trabalho em saúde : da inteligência criativa à biomedicalização de quem cuida

O cuidado ao trabalhador da saúde tende a ser realizado no cotidiano, nos encontros, na transformação do sofrimento em potência, entretanto, o afastamento do trabalho e a biomedicalização apresentam-se como alternativas uma vez que o sofrimento, ao invés de ser potencializado para produzir novos sentidos do viver, passa a ser transformado em adoecimento. Esse enlace é perpassado por essa pesquisa, que objetivou compreender como os trabalhadores da enfermagem constroem cuidados de si, mapear as intervenções dos modos de cuidar de si e compreender de que forma o trabalhador lida com os afetos provocados no cotidiano da saúde. Procedeu-se a uma pesquisa qualitativa de caráter descritivo, que seguiu os pressupostos metodológicos da exploração da multiplicidade de fontes, na qual quatro técnicas em enfermagem de um grande hospital do sul do Brasil participaram de um dispositivo denominado Tenda do Conto. Foi oportunizando que elas contassem suas “afecções” no encontro com o mundo, dialogando com os acontecimentos no cotidiano do trabalho e da vida, o que permitiu que a pesquisadora dialogasse com a micropolítica, com os contos de cada trabalhadora/pesquisadora e narrasse a sua Tenda. Os resultados surgiram através das aproximações nas contações e trouxeram algumas reflexões a respeito dos contrastes do trabalho em saúde e dos detalhes do processo relacional no plano do cuidado, onde cuidar de si esteve presente no cotidiano através de: autorreconhecimento, “engenhar” no ambiente de trabalho, cooperação intraequipe e prática de atividades fora do ambiente de trabalho Os resultados também aludem que as trabalhadoras cuidam de si através dos encontros, na micropolítica, na cooperação, no reconhecimento do outro, no autogoverno e na ressignificação do sofrimento. Para lidar com os afetos provocados produzem vínculos e realizam novas construções. Observou-se que os Serviços de Saúde do Trabalhador poderiam melhor contribuir para a saúde dos trabalhadores, caso a lógica de organização desses serviços fosse pautada por processos que tomam isso como central. Espera-se com essa pesquisa ter contribuído para uma nova visão do que seja o cuidado de si no cotidiano do trabalho em saúde. / The care by the health worker tends to be carried out in daily life, in meetings, in the transformation of suffering in potency. It aims to grasp the different voices that cross the narrative, to insert the utopia of the mixed-race Brazilian civilization and to ponder on the narrative construction that deals with the past and the present, in search of a national identity. In order to achieve this objective, some questions arose: to what extent the multifaceted and non-linear narrative of Tent of miracles (Tenda dos milagres) creates a hybrid, mixed-race perspective for the Brazilian society? The following assumptions guide the reflection: Tent of miracles (Tenda dos milagres) articulates the negative aspect of miscegenation, operating a collective consciousness to build a fraternal society; the duplicity of dialogic narrators creates a plot with mixed voices in counterpoint; the character Pedro Archanjo tunes the mind of the author/narrator in the construction of a hybrid utopian society. The work makes a portrait of the miscegenation, racism as well as of the historical formation of Brazil. It also presents the vision of literary criticism regarding both the novel and the author, showing at the same time the critical viewpoint of Jorge Amado. The theoretical framework is centered on scholars such as: Sérgio Buarque de Holanda, Kabengele Munanga, Antonio Candido, Fernando Segolin, Roland Barthes, Paul Zumthor. Among the final considerations, it is emphasized the construction of an Amadian tent - a transgressive and liberating, tough and resilient, insistent and persistent work - preaching a hybrid identity possible for the Brazilian society / Este trabalho estuda o romance Tenda dos milagres, de Jorge Amado, publicado em 1969. Tem como objetivos apreender as diferentes vozes que se cruzam na narrativa, para inscrever a utopia de uma civilização brasileira miscigenada, assim como refletir sobre a construção narrativa que opera o passado e o presente em busca de uma identidade nacional. Para atingir esses objetivos, selecionaram-se os questionamentos: até que ponto a narrativa multifacetada e não linear de Tenda dos milagres inscreve uma perspectiva miscigenada, híbrida, para a sociedade brasileira? Como a narrativa constrói essa utopia? Orientam a reflexão as seguintes hipóteses: Tenda dos milagres articula o caráter negativo da miscigenação, operando uma consciência coletiva na construção de uma sociedade fraterna; a duplicidade de narradores em dialogia constrói uma narrativa miscigenada de vozes em contraponto; a personagem Pedro Archanjo sintoniza o pensamento do narrador/autor na construção de uma sociedade híbrida, utópica. So, the author's life is taken as research source. We understand his artistic elaboration as an esthetic experimentation that emerges from – and remits to – a specific social context that he lived. Also, we focus mainly on the concepts about miscegenation that emerge in the literary text itself, making Tent of Miracles our primordial source. Considering the multiple significances of the term miscegenation in historiography, we are focusing in its use on the literary object, taking into account the significances and concepts that this term has acquired in different times and places. We point the closer relation between the construct of miscegenation and literary representations of the city of Salvador in Tent. We analyses the microcosms of the city represented and its relation with the idea of miscegenation present in the text. The objective is to identify and understand, in the literary work, vestiges of possible overviews of historical conjunctures. Here, we understand literature like a possible document for historical research, taking into account the subjectivities present in an artistic document. Also, we think important to understand and discuss historical functions of Tent of Miracles – like pointed by Antonio Candido, who identifies this function in the literary structure, considering that this one has direct relations with author significances about the place where the literary piece is conceived, a relevant aspect, considering that the place that has inspired Amado was the same the novel is set in. We point out that Tent presents us refracted images of Jorge Amado’s living between black people, frequenting territories and having experiences in Salvador. This fact brings some elements that problematize the historiographic formulation about Amado like an author who exalted the miscegenation like a way of racial pacification. This research proposes to think about an author facet, analyzing it from his own creation, not looking for answers established of the history in the literature, or the opposite, but trying to find signs of feedback between history and literature – an approach that presents us to subjects, overviews, ruptures and historical continuities / Esta dissertação tem como proposta central refletir sobre o construto do termo mestiçagem na obra Tenda dos Milagres (1969), de Jorge Amado, e com isso descortinar e intercruzar as diversas contribuições intelectuais, afetivas e empíricas na elaboração de um discurso evidenciado em texto literário. Projetamos, assim, o autor como fonte da pesquisa, entendendo sua elaboração artística como um se lançar estético que – emerge de e remete a – um fazer social, ao tempo que focamos atenção no uso do termo mestiçagem no interior literário, nas tramas de seu texto, portanto, em Tenda dos Milagres como fonte primeira. Diante do valor polissêmico deste termo na historiografia, atentamos para a sua substância no interior do objeto literário, considerando os significados moldados do seu uso entre os variados sujeitos, tempos e lugares. Constata-se, portanto, a estreita relação deste construto discursivo com as representações associadas à cidade de Salvador na obra literária, bem como a função da cidade na condição de palco de vivência do autor, o que motiva a análise dos microcosmos construídos por essas representações na modelagem da ideia de mestiçagem no texto. Como em caminho contrário, pelo objeto artístico chegar ao autor, que mediou as estruturas entre o universal e o subjetivo, objetiva-se investigar os rastros que possam nos apontar panoramas de conjunturas históricas a partir da peça literária. Compreende-se, dessa forma, a literatura como documento possível a ser aferido na pesquisa histórica se abordado por uma metodologia que considere as subjetividades de um documento artístico. É imprescindível entender, sobretudo, as funções históricas as quais Tenda dos Milagres cumpre enquanto fonte de aferição para a historiografia – como aborda Antonio Candido, que identifica essa função na própria estrutura literária, a qual tem relação direta com as significações do autor sobre o lugar onde a obra foi gestada, observação que se reforça nesta fonte, uma vez que o lugar embrionário da obra é o mesmo que ambienta a trama ficcional. Destacamos que o romance nos apresentou imagens refratadas de uma vivência presente no interior da cultura negra baiana, em seus territórios e em suas práticas; e essa percepção traz elementos que problematizam, sem negar, a formulação historiográfica sobre Jorge Amado que o referencia como um autor que exaltou a mestiçagem como via de entendimentos raciais. A pesquisa se dá a repensar uma faceta do escritor através de sua própria criação, não procurando respostas cimentadas da história na literatura ou o contrário, mas encontrando indícios de retroalimentação entre estes campos, o que nos apresenta articulações entre sujeitos, panoramas, rupturas e continuidades históricas

Outcome of implant-supported overdenture treatment

Abstract The retention of a complete denture and the patient’s adaptation varies considerably among different individuals. Resorption of the edentulous alveolar ridge differs greatly and some patients need implant-retained overdentures to enable adequate retention of their prostheses. In some extreme cases it is necessary to increase the volume of the alveolar ridge with bone grafts. The aim of this study was to examine the outcome of implant-supported overdenture treatment conducted in Oulu University Hospital. The aim was also to assess the impact of treatment on oral health-related quality of life and patient satisfaction. The study group was comprised of patients treated with a mandibular or maxillary implant overdenture and a group of patients with extreme mandibular bone resorption treated with extraoral bone grafts and implants. The treatments were performed in 1985–2013 thus also providing long-term results. The results of this study showed predictable and successful treatment outcomes also among elderly patients and in severe situations with bone deficiency. The most frequent complication in the clinical follow-up was loosening of the retention mechanism, commonly noted in other previous studies. Despite some minor mechanical defects in prosthetic structures or mild mucosal inflammation around the implants, they did not hinder everyday use of the prostheses. Neither the number of supporting implants nor the connection type seemed to have a great impact on patient satisfaction. Older patients with a mandibular overdenture seemed to be most satisfied. In conclusion, treatment with implant overdentures seems to be successful also in the long-term. / Tiivistelmä Kokoproteesin pysyvyys ja potilaan sopeutuminen proteesiin vaihtelee huomattavasti eri yksilöiden välillä. Hampaattoman luuharjanteen resorptiossa on eroja, ja osalle potilaista implanttikiinnitteinen peittoproteesi on välttämätön riittävän proteesin pysymisen mahdollistamiseksi. Hyvin pitkälle edenneissä luuharjanteen resorptioissa voi leukaluun lisääminen luusiirteillä olla tarpeellista. Tutkimuksen tarkoituksena oli selvittää Oulun yliopistollisessa sairaalassa tehtyjen implanttikiinnitteisten peittoproteesihoitojen tuloksia. Tavoitteena oli myös arvioida hoidon vaikutusta suunterveyteen liittyvään elämänlaatuun ja potilastyytyväisyyteen. Tutkittava ryhmä koostui potilaista, joille oli valmistettu alaleukaan tai yläleukaan implanttikiinnitteinen peittoproteesi, ja potilasjoukosta, jolle hyvin pitkälle edenneen luuresorption vuoksi implanttihoito oli tehty luusiirteiden avulla. Hoidot tehtiin vuosina 1985–2013, ja tutkimuksissa voitiin arvioida myös peittoproteesihoidon pitkäaikaistuloksia. Tutkimus osoitti, että hoitotulokset olivat ennustettavia ja onnistuneita myös iäkkäillä potilailla sekä potilailla, joilla luupuutos oli kaikkein vaikein. Yleisin todettu komplikaatio kliinisessä tutkimuksessa oli proteesin kiinnitysmekanismin löystyminen. Pienet proteesirakenteiden rikkoutumiset tai lievät limakalvon tulehdusreaktiot implanttien ympärillä eivät kuitenkaan haitanneet proteesin toimintaa ja päivittäistä käyttöä. Peittoproteesia tukevien implanttien määrällä tai kiinnitystyypillä ei todettu olevan suurta vaikutusta potilastyytyväisyyteen. Kaikkein tyytyväisimpiä vaikuttivat olevan iäkkäät potilaat, joille oli tehty alaleuan peittoproteesi. Tutkimus osoittaa, että implanttikiinnitteinen peittoproteesihoito on menestyksellistä myös pitkällä aikavälillä.

Rotating an object is inspired by children's toys, children's carousel and its symbolism and design solutions to commemorate military mobile tent. Object carousel has an octahedral shape with a roof, levitates in the air space, not touching the ground and without a defined border.

The diploma thesis deals with the location of the cult (Tent of Meeting, temple), priests in Israel, human offerings and reconstruction of ritual burnt offerings with an evaluation of particular acts based on translation and interpretation of the first chapter of Leviticus. The purpose of this work is to detect, what function and meaning this ritual had for the community of Israel, as well as what message the texts contain, with respect to burnt offerings for the current reader.

Pour des opérateursde régularité maximale sur les espaces de tentes, nous donnons des résultats optimaux enexploitant la structure des opérateurs intégraux de convolution et en utilisant des estimationsde la décroissance hors-diagonale du semi-groupe ou de la famille résolvante sous-jacente.Nous appliquons des techniques précédentes d’analyse harmonique et fonctionnelle pourestimer sur les espaces de tentes certains opérateurs d’intégrale évolutionnelle, nées de l’étudedes problèmes aux limites elliptiques et des systèmes non-autonomes du premier ordre. / We give a Calderón-Zygmund type machinery concerning the extrapolation theory for thesingular integral operators on tent spaces. For maximal regularity operators on tent space, wegive some optimal results by exploiting the structure of convolution integral operators and byusing the off-diagonal decay estimates of the underlying semigroup or resolvent family.We apply the previous harmonic and functional analysis techniques to estimate on tentspaces certain evolutionary integral operators arisen from the study of boundary value ellipticproblems and first order non-autonomous systems.

Which functional and design aspects to prioritize when developing outdoor tents - To optimize customer satisfaction and four season use.

Optimizing customer satisfaction is vital to maintain relevance as well as being competitive on a growing market. This research is aimed to explore what aspects of design and functionality that should be prioritized when developing solo tunnel tents for four season use within a European market. Since solo tents are small in size, users seem to prioritize purchasing a tent for two people because it provides bigger space for equipment, accompanied animals and is more reliable in bad weather. To examine the research questions, a design theory and human centered design method has been applied to optimize customer satisfaction. The study consists of literature study, survey, interviews, empathy mapping and prototyping, which resulted in an implemented prototype for a four season solo tent planned for future production. Furthermore, a prototype has been developed by data conducted from the survey, interviews and a design brief given by the partnering company. Conspicuous results have been shown through a survey and interviews, which explains the difficulties in development of tents to ensure a wider range of customer satisfaction. Findings in this research emerged that weight was not the most important aspect when buying a four season solo tent. A correlation has been investigated, there was an expectation from the producer that lightweight tents were preferred by the users, however this research exposed that it is not as important as imagined for a four season solo tent. In the sense of a solo tent for four seasons, the users desire space over weight. Furthermore, the results also showed that there was a difference in requirements related to the two different groups that had been investigated. It was found that experienced users had a higher preference of design and ergonomic aspects compared to the common users that did not rank it as important. Within this research a total of 55 people participated, where 44 people participated in the survey and 11 people participated in the interviews. For future research a larger number of participants, preferably women, could provide an interesting result and more on if people from a larger area outside Sweden participated in the research. Furthermore a physical test of the prototyped tent would ensure that this study results in a design that optimizes customer satisfaction.
