Depression in association with birth weight, age at menarche, obesity and metabolic syndrome in young adults:the Northern Finland 1966 Birth Cohort Study

Abstract Depression is a common mental disorder in the Finnish population. There are several biological, psychological and social factors in the background of depression. The aim of this study was to investigate depression in association with birth weight, age at menarche, obesity and metabolic syndrome using data from the Northern Finland 1966 Birth Cohort. A large, prospectively collected general population-based birth cohort of originally 12058 liveborn children was used as study population. The database provided information on birth characteristics and features of the primary family. The follow-up studies were performed at the age of 14 years by postal inquiry, and at the age of 31 years by postal inquiry and clinical examination. Information on age at menarche and weight and height was obtained from the postal questionnaire at 14 and 31 years and clinical examination at 31 years. Data on abdominal obesity and metabolic syndrome were gathered from the clinical examination. Data on depressive symptoms measured by the Hopkins Symptom Checklist-25 (HSCL-25), self-reported physician-diagnosed lifetime depression and the use of antidepressants were gathered from the postal questionnaire at 31 years. Females with high birth weight and high ponderal index (index of the birth measures, kg/m3) had a higher risk of depressive symptoms at 31 years measured by the HSCL-25 compared with females with normal birth weight and ponderal index. Males with ponderal index belonging to the lowest 5 percentile had an increased risk for physician-diagnosed depression at 31 years. Females with late menarche (≥ 16 years) had an elevated risk of depression measured by the HSCL-25, the use of antidepressants and self-reported physician-diagnosed depression compared with females with menarche at 12–15 years. Obesity measured by BMI at 14 years increased the risk of depressive symptoms measured by the HSCL-25 at 31 years among both males and females. Females who were obese both at baseline and at follow-up had an increased risk of depressive symptoms, and the proportion of those who used antidepressants was higher among females who had gained weight compared to females who had stayed normal-weighted. Males with abdominal obesity measured by waist-to-hip ratio had an increased risk of depressive symptoms and physician-diagnosed depression, and the proportion of those who used antidepressants was higher compared with subjects without abdominal obesity. Abdominal obesity did not associate with depression in females. Metabolic syndrome did not associate with depression. The results indicate an increased risk of depression at 31 years in females with high birth weight, late menarche, adolescent obesity and weight gain and in males with adolescent obesity and abdominal obesity. / Tiivistelmä Depressio on yleinen mielenterveyden häiriö suomalaisväestössä. Depression taustalla on monia biologisia, psykologisia ja sosiaalisia tekijöitä. Tämän tutkimuksen tavoitteena oli tutkia depressiota selvittäen, onko syntymäpainolla, menarkeiällä, lihavuudella ja metabolisella oireyhtymällä yhteyttä depressioon Pohjois-Suomen vuoden 1966 syntymäkohorttiaineistossa. Tutkimusaineistoon kuului alun perin 12058 elävänä syntynyttä lasta. Tietokantaan oli jo aiemmin kerätty aineistoa syntymään ja primaariperheeseen liittyen. Kohortin jäsenten ollessa 14-vuotiaita tehtiin seurantatutkimus postikyselynä ja 31-vuotiaana tehtiin sekä postikysely että kliininen tutkimus. Tiedot menarkeiästä kerättiin 31-vuotispostikyselystä, paino- ja pituustiedot sekä 14- ja 31-vuotispostikyselyistä että kliinisen tutkimuksen tiedoista. Kliininen tutkimus sisälsi tiedot myös keskivartalolihavuuden ja metabolisen oireyhtymän määrittämiseksi. 31-vuotispostikyselyssä depressio-oireita kysyttiin HSCL-25 -oirekyselyllä; lisäksi kysyttiin, oliko lääkäri todennut aiemmin masennusta sekä oliko tutkittavilla käytössä masennuslääkkeitä. Naisilla, joiden syntymäpaino ja ponderaali-indeksi (syntymäpainon ja pituuden suhdetta kuvaava indeksi, kg/m3) oli korkea, depressio-oireiden riski 31-vuotiaana mitattuna HSCL-25:lla oli suurentunut verrattuna naisiin, joilla oli normaali syntymäpaino ja ponderaali-indeksi. Miehillä, joilla oli hyvin alhainen ponderaali-indeksi kuuluen alimpaan 5 % ryhmään, riski lääkärin toteamaan masennukseen oli suurentunut. Naisilla, joiden menarkeikä oli 16-vuotta tai myöhemmin, riski depressio-oireiden esiintyvyyteen, depressiolääkkeiden käyttöön ja lääkärin toteaman depression esiintyvyyteen oli suurentunut verrattuna naisiin, joiden menarkeikä oli 12–15-vuotta. Lihavuus 14-vuotiaana lisäsi masennusoireiden riskiä mitattuna HSCL-25:lla sekä 31-vuotiailla miehillä että naisilla. Naisilla, jotka olivat lihavia sekä 14- että 31-vuotiaana, masennusoireiden riski oli suurentunut. Naisilla, joiden paino oli noussut, masennuslääkkeitten käyttö oli yleisempää verrattuna naisiin, joilla paino oli pysynyt normaalina. Keskivartalolihavuus oli miehillä yhteydessä suurentuneeseen depressio-oireiden ja lääkärin toteaman masennuksen riskiin, ja he käyttivät yleisemmin masennuslääkkeitä verrattuna miehiin ilman keskivartalolihavuutta. Naisilla keskivartalolihavuus ei ollut yhteydessä masennukseen. Metabolinen oireyhtymä ei ollut yhteydessä masennukseen. Tulokset osoittavat korkean syntymäpainon, myöhäisen menarkeiän ja nuoruusiän lihavuuden sekä painon nousun lisäävän masennusriskiä 31-vuotiailla naisilla, 31-vuotiailla miehillä nuoruusiän lihavuus sekä keskivartalolihavuus olivat yhteydessä suurentuneeseen masennusriskiin.

Esta tese está composta por introdução geral, objetivos, capítulos (1 e 2) e conclusão geral. A introdução geral aborda: aspectos sobre a síndrome de Down (SD); da obesidade com temas direcionados à fisiopatologia, tipos, incidência e diagnóstico em crianças e adolescentes; da obesidade na SD; da associação da obesidade com a doença cardiovascular (DCV), como também a avaliação dos fatores de risco cardiovascular em indivíduos com e sem SD. Os capítulos 1 e 2 estão em formato de artigo. Na conclusão geral foram apresentadas as principais conclusões dos dois artigos. Objetivos: Os objetivos do presente estudo são: (1) estimar o ponto de corte do Índice de Massa Corporal (IMC) para o diagnóstico de obesidade em adolescentes com SD de acordo com diferentes referências para classificação do IMC em relação ao percentual de gordura corporal (%GC) avaliado pela absorciometria com raios-X de dupla energia (DXA) e (2) avaliar os fatores de risco cardiovascular em adolescentes com SD e compará-los em relação ao diagnóstico de obesidade estabelecido pelo escore z do IMC da World Health Organization (WHO). Casuística e Métodos: Participaram no primeiro artigo (capítulo 1) 34 adolescentes com SD e no segundo artigo (capítulo 2) 32, de ambos os sexos (idade: 10 e 17 anos). No capítulo 1, duas técnicas foram utilizadas para avaliar a composição corporal: IMC e DXA. O IMC foi avaliado de acordo com as referências da International Obesity Task Force (IOTF), da WHO para população geral, e de Myrelid et al. e Styles et al. para população com SD. O %GC foi avaliado pela DXA de corpo inteiro e classificado de acordo NHANES 2011. No capítulo 2, o IMC considerado como alterado foi ?2,00. Foram avaliados circunferência da cintura, pressão arterial, perfil lipídico, HOMA-IR e proteína C reativa (PCR). O colesterol total, o HDL, o LDL, os triglicérides, o HOMA-IR e a PCR foram considerados fatores de risco cardiovascular e analisados em relação ao escore z do IMC. Resultados: No capítulo 1, apenas os meninos apresentaram a altura significativamente maior que as meninas e estas %GC maior que os meninos. Todas as referências que usaram IMC para avaliar obesidade apresentaram associação positiva com o %GC avaliado pela DXA no diagnóstico de obesidade. Utilizando a curva ROC em relação ao %GC pela DXA, todas as referências apresentaram alta sensibilidade, porém, o escore z do IMC pela WHO apresentou melhor especificidade, com o valor da acurácia de 82% para o ponto de corte >2,14. No capítulo 2, seis adolescentes não apresentaram nenhum dos seis fatores de risco cardiovascular, 15 apresentaram um ou dois fatores de risco e 11 apresentaram três ou mais. O escore z do IMC com melhor sensibilidade (81,8%), especificidade (71,4%) e acurácia (72%) para identificar os grupos com menos de três ou três ou mais fatores de risco cardiovascular foi o 2,14. Conclusão: De todas as referências utilizadas para o diagnóstico de obesidade o ponto de corte do escore z >2,14 de IMC da WHO mostrou melhor especificidade. O ponto de corte do escore z >2,14 de IMC da WHO confirmou ser o melhor ponto de corte para diagnóstico de obesidade relacionada a risco de DCV entre adolescentes com SD / Abstract: Structure of the Thesis: This study is structured by "Scandinavian Model," called "Alternative Model" in the Postgraduate Program in Child and Adolescent Health of the School of Medical Sciences, at State University of Campinas (FCM - Unicamp). This thesis is composed of general introduction, objectives, chapters (1 and 2) and general conclusion. The general introduction approach aspects of the history of Down syndrome (DS), prevalence, genetics, clinical features and demonstrations in DS; as well as dealing with obesity issues directed to the pathophysiology, types, prevalence and diagnosis in children and adolescents, diagnostic methods and obesity in DS; the association of obesity with cardiovascular disease (CVD), as well as the assessment of cardiovascular risk factors in children, adolescents and adults with and without DS. Chapters 1 and 2 are in article format. In general conclusion were presented the main conclusions of the two articles. References were presented at the end of each chapter and of the thesis. Objectives: The aims of this study are: (1) to estimate the cutoff point of the body mass index (BMI) for the diagnosis of obesity in adolescents with DS according to different references for BMI in relation to the percentage of body fat (%BF) measured by absorptiometry dual-energy X-ray absorptiometry (DXA) and (2) to assess the cardiovascular risk factors in adolescents with DS and compare them in relation to obesity diagnosis determined by z-score for BMI of the World Health Organization (WHO). Materials and Methods: In this study, participated in the first article (Chapter 1) 34 adolescents with DS and the second article (Chapter 2) 32 adolescents with DS, of both sexes (age: 10 to 17 years). In Chapter 1, two techniques were used to assess body composition: BMI and DXA. BMI was evaluated according to the references of the International Obesity Task Force (IOTF), the WHO for the general population, and Myrelid et al. and Styles et al. for people with DS. The %BF was assessed by whole body DXA and classified according to NHANES. In chapter 2, weight and height were measured and BMI calculated. BMI considered abnormal was >2.14. We evaluated waist circumference, blood pressure, lipid profile, HOMA-IR and C-reactive protein (CRP). Total cholesterol, HDL, LDL, triglycerides, HOMA-IR and CRP were considered cardiovascular risk factors and analyzed in relation to BMI z score. Results: In Chapter 1, all references who have used BMI to assess obesity was positively associated with %BF measured by DXA in the diagnosis of obesity. Using the ROC curve in relation to %BF by DXA, all references showed high sensitivity, but the z-score of BMI by WHO showed better specificity, with the value of the accuracy of 82% for the cutoff point above 2.14. In Chapter 2, six teenagers do not show any of the six cardiovascular risk factors, 15 had one or two risk factors and 11 had three or more. The z scores of BMI with better sensitivity (81.8%), specificity (71.4%) and accuracy (72%) to identify the groups with less than three or three or more cardiovascular risk factors was 2.14. Conclusion: All the references used for the diagnosis of obesity of the cutoff point of z-scores above 2.14 by WHO showed better specificity. Fatores associados ao estado nutricional dos estudantes da 4ª à 9ª série do ensino fundamental de 3 regiões administrativas do município de Vitória / Factors associated with the nutritional status of 4th to 9th grade Elementary to Middle School students in 3 administrative regions of the County of Vitoria

RESUMO Introdução- A prevalência de sobrepeso já ultrapassa 40% entre os adultos brasileiros. Isso justifica o monitoramento da obesidade entre crianças e adolescentes, concomitantemente com os déficits nutricionais que acometem as crianças de baixa renda, ainda que hoje se apresentem em patamares menores do que nas décadas anteriores. Objetivo- Descrever as prevalências e os fatores associados ao estado nutricional dos estudantes da 4ª à 9ª série do ensino fundamental de 3 regiões administrativas do município de Vitória. Método. Avaliouse o estado nutricional e o estágio de maturação sexual de 400 alunos de escolas públicas e privadas. Considerou-se em déficit de estatura os indivíduos com índice estatura/idade abaixo de -2 escores z do valor mediano da população de referência, em déficit de peso aqueles com Índice de Massa Corporal (IMC) abaixo do percentil 5 e com sobrepeso/obesidade o IMC ao percentil 85. Para o perímetro da cintura, adotou-se como referência o percentil 90 da população britânica. Resultados- Encontrou-se baixa prevalência de déficit de estatura (4,0%) e de déficit de peso (4,8%) e alta prevalência de sobrepeso/obesidade (21,3%) e de acúmulo de gordura na região da cintura (27,3%). No modelo de regressão linear geral múltipla, as variáveis associadas ao déficit de estatura foram: maturação sexual em estágio inicial (RP-14,84), faixa etária 14 anos (RP-5,08) e número de irmãos 2 (RP-4,52). Para déficit de peso: peso ao nascer 2500g (RP-0,23), IMC materno 25 (RP-0,19) e número de irmãos 2 (RP-2,76). Para sobrepeso/obesidade: maturação sexual em estágio inicial e intermediário (RP-2,19 e RP-2,13), IMC materno 25 (RP-2,04) e renda no nível mais alto (RP-1,78). Para acúmulo de gordura na região da cintura: IMC materno 25 (RP-1,75), renda no nível mais alto (RP- 1,88), faixa etária 14 anos (RP-0,50) e sedentarismo 28 horas/semana (RP-1,66). Conclusões- Ações de saúde pública são necessárias para o combate ao sobrepeso/obesidade e ao acúmulo de gordura na região da cintura, apresentando maior prevalência os indivíduos nos estágios iniciais e intermediários de maturação sexual, menores de 14 anos, de maior renda, cujas as mães apresentaram IMC 25 e sedentários. / Abstract Introduction: The overweight prevalence already exceeds 40% among Brazilian\'s adults. This justifies the monitoring of obesity among children and adolescents, as well as the nutritional deficits that affect children of low income, even though today it is present in levels lower than in previous decades. Objective: To describe the prevalence and the factors associated with the nutritional status of 4th to 9th grade Elementary to Middle School students in 3 administrative regions of the County of Vitória. Method: The nutritional status and sexual maturation stage of 400 private and public school individuals were evaluated. It was considered stunting those individuals with height-for-age below -2 Z-scores of median value for the reference population; wasting those with body mass index (BMI) below the percentile 5th and as overweight/obesity the BMI of percentile 85th. For waist circumference was adopted the percentile 90 of the British population. Results: There was a low prevalence of stunting (4.0%) and wasting (4.8%) and high prevalence of overweight/obesity (21.3%) and fat accumulation in the waist area (27.3%). In the multiple generalized linear model the variables associated with stunting were: initial stages of sexual maturation (PR-14.84), age >14 (PR-5.08), number of siblings >2 (PR-4.52). For wasting: birth weight > 2500g (PR- 0.23), maternal BMI >25 (PR-0.19), number of siblings >2 (PR-2.76). For overweight/obesity: initial and intermediate stages of sexual maturation (PR-2.19 and PR- 2.13), maternal BMI >25 (PR-1.75), high income level (PR-1.78). For fat accumulation in the waist area: maternal BMI >25 (PR-1.75), high income level (PR-1.87), age >14 (PR- 0.50), sedentary > 28 hours/week (PR-1.66). Conclusions: Public health action is needed to combat overweight/obesity and fat accumulation in the waist area, presenting higher prevalence the individuals in initial and intermediate sexual maturation stages, below age 14, higher income levels, whose mothers present BMI 25 and sedentary.

Correlación entre circunferencia de cuello y otros parámetros antropométricos en peruanos de zonas urbanas / Correlation between neck circumference and other anthropometric parameters in Peruvians in urban areas

Introducción: La circunferencia de cuello (CCu) es una medida útil para evaluar riesgo de enfermedades crónicas no transmisibles. El objetivo fue determinar la correlación y establecer los mejores puntos de corte que correspondan entre CCu e Índice de Masa Corporal (IMC) y Circunferencia de Cintura (CCi). Métodos: Se utilizó la base de datos de Perú del Estudio Latinoamericano de Nutrición y Salud (ELANS). Se evaluó por sexo: (i) la correlación de Pearson entre CCu e IMC y CCi, (ii) la Sensibilidad (Se) y Especificidad (Sp) para sobrepeso y obesidad (IMC) y para obesidad central (CCi), con el Índice de Youden. Resultados: Se encontró correlación fuerte por sexo entre CCu y CCi (hombres: r=0,81 y mujeres: r=0,80) y moderada entre CCu e IMC (r=0,80 y r=0,76 respectivamente). El punto de corte para definir sobrepeso/obesidad por CCu (IMC≥25.0 kg/m2) en hombres fue 36,95 cm y en mujeres 32,85 cm (Se: 81% y 78%; Sp: 77% y 88%, respectivamente), para obesidad (IMC≥30.0 kg/m2) fue 39,15 cm para hombres y 33.85 cm para mujeres (Se: 92% y 88%; Sp: de 84% y 73%, respectivamente) y para obesidad central fue 38,65 cm en hombres y 32,75 cm para mujeres (Se: 80% y 77%; Sp: de 89% y 89%, respectivamente). Conclusión: Se encontró una correlación fuerte entre CCu y CCi y moderada entre CCu e IMC. La CCu es una medida práctica y poco invasiva. Podría ser usada en salud pública para identificar riesgo de enfermedades crónicas no transmisibles. / Background: Neck circumference (NC) is being used lately to identify the risk of chronic noncommunicable diseases. The aim of this study was to determine the correlation and establish the best cut-off points that correspond between NC and Body Mass Index (BMI) and Waist Circumference (WC). Methods: Data were obtained from the Latin American Study of Nutrition and Health (ELANS). The following were evaluated by sex: (i) Pearson's correlation between NC and BMI and WC, (ii) Sensitivity (Se) and Specificity (Sp) for overweight and obesity (BMI) and for central obesity (WC), with the Index of Youden. Results: A strong correlation by sex was found between NC and WC (men: r = 0.81 and women: r = 0.80, respectively) and a moderate correlation between NC and BMI (r = 0.80 and r = 0.76 respectively). The cutoff points to define overweight /obesity by NC (BMI≥25.0 kg / m2) in men was 36.95 cm and in women 32.85 cm (Se: 81% and 78%; Sp: 77% and 88%, respectively) and for obesity (BMI≥30.0 kg / m2) it was 39.15 cm for men and 33.85 cm for women (Se: 92% and 88%; Sp: 84% and 73%, respectively). For central obesity, the cutoff points is 38.65 cm for men and 32.75 cm for women (Se: 80% and 77%; Sp: 89% and 89%, respectively). Conclusion: A strong correlation was found between NC and WC and a moderate one between NC and BMI. Obesity and obesity-related markers associated with breast and colorectal cancer occurence and mortality

Indiana University-Purdue University Indianapolis (IUPUI) / Purpose: Obesity is a growing public health problem and the second most preventable cause of death in the US. Obesity has been linked as a risk factor for several cancers. However, there are limited studies that have examined the roles of metabolic syndrome (MetS) and C-reactive protein (CRP), as well as change in body composition from early adulthood to late adulthood on the risk of cancer. The overall objective of this dissertation was to determine the association of obesity and obesity-related markers with breast and colorectal cancer occurrence and mortality. Methods: Three datasets were used. The first study used 4,500 asymptomatic adults who were surveyed during a colorectal cancer screening study. The second study was based on the National Health and Nutrition Examination Survey (NHANES) 2005-2010. The dataset had 172 breast cancer survivors and 2,000 women without breast cancer. The last manuscript resulted from the NHANES follow-up study (NHANES III). A total of 120 cancer deaths from breast and colorectal deaths were identified from 10,103 women aged 18 years or older. Results: Overall, obesity and obesity related markers were associated with breast and colorectal cancer occurrence and mortality. BMI change and WC change were positively associated with increased risk of advanced colorectal neoplasia (AN). WC measures (both static and dynamic) were generally a better predictor of AN compared to BMI. In the second study involving breast cancer survivors, neither MetS nor CRP were associated with having a breast cancer diagnosis. Also, none of the individual components of MetS (WC, Triglycerides, HDL, fasting blood glucose and blood pressure) were associated with a breast cancer diagnosis. In the last study, MetS was associated with increased risk of mortality from obesity-related cancers. In addition, all components of MetS, except dyslipidemia, were associated with increased risk of mortality for the obesity-related cancers. Conclusion: Obesity expressed in terms of BMI and WC, or their change, MetS and CRP are important factors in regard to the occurrence, survivorship and mortality of breast and colorectal cancer. The results of this research underscore the importance of maintaining a healthy weight.

Os objetivos do estudo foram correlacionar a razão entre a circunferência abdominal e altura (CA/A) e o índice de massa corpórea (IMC) com as variáveis cardiometabólicas e inflamatórias em escolares de seis a 10 anos; avaliar a frequência de sobrepeso/obesidade e alterações cardiometabólicas e comparar o desempenho dos referenciais de índice de massa corpórea (IMC) do Centers for Disease Control and Prevention 2000 (CDC) e Organização Mundial de Saúde 2007 (OMS) no diagnóstico de sobrepeso/obesidade e alterações cardiometabólicas. Métodos: estudo de corte transversal, que incluiu 175 crianças, provenientes do Centro de Referência para Tratamento da Criança e do Adolescente (CRTCA), em Campos, Rio de Janeiro. As crianças foram divididas segundo os escores z do CDC e OMS em: não obesas (z do IMC <1) e sobrepeso/obesidade (z do IMC > 1). As variáveis cardiometabólicas analisadas foram: pressão arterial sistólica (PAS) e diastólica (PAD), glicose, lipoproteínas de baixa e alta densidades (LDL e HDL, respectivamente), triglicerídeos (TG), HOMA-IR. Como variáveis inflamatórias, analisamos proteína C reativa ultra-sensível (PCR) e leucometria. Resultados: a média da CA/A do grupo sobrepeso/obesidade foi maior que a do não obeso (0,58 ± 0,007 e 0,45 ± 0,004, respectivamente, p< 0,0001). Houve correlação significativa da CA/A com os escores z do IMC (r = 0,88, p < 0,0001), PAS (r= 0,51, p<0,0001), PAD (r= 0,49, p<0,0001), HOMA-IR (r=0,83, p<0,0001), HDL (r = -0,28, p< 0,0002), TG (r= 0,26, p<0,0006), LDL (r= 0,25, p<0,0008) e PCR (r= 0,51, p<0,0001). Contudo, a CA/A não se correlacionou com glicemia nem leucócitos. A sensibilidade da CA/A se equivaleu à do IMC no diagnóstico das alterações cardiometabólicas. A sensibilidade mais elevada da CA/A foi para o diagnóstico de alteração da PAS (80,0 %), PAD (76,6%) e HOMA-IR (92,6%). O ponto de corte superior a 0,47 foi sensível para o diagnóstico de resistência insulínica, mas acima de 0,50, para os demais distúrbios cardiometabólicos. A frequência de sobrepeso/obesidade nos escolares foi igual a 49,7%. Com exceção de hipertrigliceridemia, todas as outras alterações cardiometabólicas foram mais frequentes no grupo sobrepeso/obesidade (aumento de PA, p<0,0001; glicemia de jejum alterada, p < 0,0048; aumento de LDL, p< 0,015 e redução do HDL, p<0,0001). O referencial da OMS 2007 reclassificou 11 crianças a mais como obesas que o CDC, que apresentaram médias de escores z de PAS (1,71 ± 1,54), PAD (2,64 ± 1,83) e HOMA-IR (1,84 ± 0,98) semelhantes às médias das obesas (PAS = 1,25 ± 2,04; PAD = 1,94 ± 1,19 e HOMA-IR = 2,09 ± 1,12), mas superiores às médias das classificadas como sobrepeso (PAS = 0,49 ± 1,34, p < 0,023; PAD = 1,45 ± 0,97, p < 0,04 e HOMA-IR = 1,24 ± 0,67, p < 0,04 ). Conclusões: a razão CA/A foi tão sensível quanto IMC da OMS 2007 no diagnóstico do risco cardiometabólico e inflamatório. O referencial da OMS 2007 foi o mais sensível não só para o rastreamento de sobrepeso/obesidade, como também para pressão arterial elevada e resistência insulínica, em escolares de seis a 10 anos / This study aims to correlate the waist-to-height ratio (WHtR) and the body mass index (BMI) with the cardiometabolic and inflammatory variables in 6-10 year-old school children; to evaluated the frequency of overweight/obesity and cardiometabolic disturbances, and to compare the 2000 Centers for Disease Control and Prevention (CDC) and 2007 World Health Organization (WHO) body mass index (BMI) references in the diagnosis of overweight/obesity and the cardiometabolic disturbances. The frequency of overweight/obesity was 49,7% in these school children. Os objetivos do estudo foram correlacionar a razão entre a circunferência abdominal e altura (CA/A) e o índice de massa corpórea (IMC) com as variáveis cardiometabólicas e inflamatórias em escolares de seis a 10 anos; avaliar a frequência de sobrepeso/obesidade e alterações cardiometabólicas e comparar o desempenho dos referenciais de índice de massa corpórea (IMC) do Centers for Disease Control and Prevention 2000 (CDC) e Organização Mundial de Saúde 2007 (OMS) no diagnóstico de sobrepeso/obesidade e alterações cardiometabólicas. Diet and Cardiometabolic Disease : Dietary trends and the impact of diet on diabetes and cardiovascular disease

Cardiovascular diseases are the leading cause of death in most industrialised countries and in developing countries the trend in cardiovascular-related deaths is increasing. World-wide, type 2 diabetes mellitus (T2DM) is an emerging cause of disability and premature death. Both these conditions are closely associated with the consumption of energy-dense foods and food products that are poor in nutrients, as well as with a sedentary lifestyle. Pharmacological and surgical interventions can improve the outcome and delay the progression of the disease, but in terms of population-level prevention there is no substitute for the adoption of a healthy lifestyle. SETTING The underlying studies were conducted in Västerbotten (the VIP study), and in Norrbotten and Västerbotten combined (the MONICA Project). Norrbotten andVästerbotten are the two northernmost counties in Sweden. Since the mid-1980sthe prevalence of cardiovascular disease has decreased and diabetes rates haveremained stable in this region, despite of an unbroken trend of increasing body weight. OBJECTIVE The aim of this thesis is to describe changes in reported dietary habits, estimatetheir relative importance as risk factors for diabetes and cardiovascular disease, and finally to identify lifestyle components as potential targets for intervention. RESULTS The first paper describes changes in self-reported food consumption between 1986 and 1999. During this period, the population in question switched from products with high saturated fatty acid content (e.g. milk containing 3% fat, butter) to foods containing less saturated fat (e.g. milk containing 1.5% fat, vegetable oil, low-fat margarine); pasta and rice were consumed more often, and potatoes were consumed less. Convenience foods (e.g. hamburgers, snacks, sweets) became more popular, whilst traditional dishes (e.g. potato dumplings, black pudding, blöta) decreased in popularity. Fruit and vegetable intake remained low. In paper two we study the effects of these changes in food intake on the risk of developing T2DM using body fat distribution as an early indicator. Increased consumption of convenience foods was associated with unfavourable changes (smaller hip circumference and larger waist circumference), whereas the increased consumption of vegetable oil and pasta was associated with low-risk fat distribution. In the third paper we report studies on the association between fat consumption and T2DM. We used the pattern of fatty acids in the membranes of red blood cells as a marker of fat intake. In addition to confirming earlier findings (markers of the intake of saturated fat are associated with increased risk of T2DM and markers of unsaturated fat are associated with reduced T2DM risk), we also identified associations between two markers of milk-derived saturated fat intake and enterolactone, a biomarker of dietary fibre intake, and the risk of developing myocardial infarction. Our results indicate that moderately high levels of enterolactone intake in men are associated with lower risk of experiencing myocardial infarction. Manuscript 5 ranks education level, physical activity, smoking status, and self-reported intake of dietary fibre and fatty acids according to their effects on body fat distribution. Increased levels of physical activity, a higher education level and a reduced intake of saturated fat from meat were ranked as the most strongly associated factors in both men and women. Increased intake of dietary fibre from grains in women, and increased intake of dietary fibre from fruits and vegetables in men, was also inversely associated with average waist circumference. CONCLUSION Both questionnaire-based and biological markers of the risk of developing diabetes or cardiovascular disease have been identified. Based on available population level measurements, reduced consumption of convenience foods, increased consumption of whole-grain products, fruits and vegetables, vegetable oil and pasta as well as increased physical activity are potential goals for interventions in northern Sweden.

The research part explores recent situation of body image in gay subculture compared with heterosexual males. This section includes the content analysis of mainstream gay movies which compares media presentation of gays and the real situation and examines media influence on gay body image formation.
