Language discordant HIV and AIDS interactions in Lesotho health care centres

Thesis (PhD)--Stellenbosch University, 2013. / ENGLISH ABSTRACT: This PhD study investigated the organisational structure of medical communicative facilities and the related communicative experiences of health care providers and patients in HIV and AIDS care centres where there is language discordance between physicians and patients. Such discordance refers specifically to communication in contexts where patients and health care providers speak a number of different, mostly mutually unintelligible first languages (L1s) and where speakers have varying levels of proficiency in a lingua franca such as English. This study considers key moments within the organisational communication structure to assess how well the structure meets its communicative aims. The sites of care that provided empirical data in this study, were a public health clinic which is a division of a state hospital, and a privately run day care clinic both located near Maseru, the capital city of Lesotho. The participants were drawn from four categories, namely physicians, nurses, lay interpreters and patients. Data collection was done through semi-structured interviews, focus group discussions and direct observations of the study sites. The data was later transcribed interpreted and analysed according to insights gained from Organisation Theory on the one hand and Thematic Analysis and Qualitative Data Analysis on the other hand. The most important result of the study is the recognition of organisational fragmentation of care into different units which helps to facilitate communication where patients and physicians show marked language discordance. Further results illuminate several challenges that are encountered by participants in mediating and making meaning where language diversity is such that physicians’ linguistic repertoire does not match the repertoires of patients and local HCPs. The study highlights several institutional and interpersonal strategies that are used to overcome these challenges and to assure effective communication in the particular institutions. It also shows how some of these strategies fail to fully address the communicative challenges identified. The findings of this study suggest that in multilingual clinical contexts there is a need for more dedicated attention to interpreting practices, to the kinds of material distributed among patients and, more generally, to make consultative decisions on improved systems to put in place in order to facilitate communication related to quality health care. / AFRIKAANSE OPSOMMING: Hierdie PhD-studie het die organisatoriese struktuur van mediese kommunikatiewe geleenthede en die verwante ervarings van beroepsmense in gesondheidsorg van pasiënte in HIV-versorgingsentra ondersoek, waar die taalvaardighede van dokters en pasiënte nie gesinchroniseer is nie. Die taaldissonansie verwys spesifiek na kommunikasie in kontekste waar pasiënte en beroepsmense in gesondheidsorg 'n verskeidenheid tale praat wat meestal onderling onverstaan-bare eerste tale (T1e) is van sprekers met ongelyke vlakke van vaardigheid in 'n lingua franca soos Engels. Die studie vestig aandag op sleutelmomente binne die struktuur van die kommunikasie van die organisasie om vas te stel hoe goed die bepaalde struktuur sy kommunikatiewe doelstellinge verwesenlik. Die terreine van gesondheidsorg wat empiriese data vir hierdie navorsing voorsien het, was 'n openbare kliniek wat verbonde is aan 'n staatshospitaal, en 'n privaat dagsorgkliniek wat albei naby Maseru, die hoofstad van Lesotho, geleë is. Verdere bevindinge werp lig op verskeie uitdagings wat deelnemers ervaar in die bemiddeling en skep van betekenis waar taaldiversiteit sodanig is dat die talige repertoires van die mediese praktisyns nie aangepas is by die talige repertoires van die pasiënte of plaaslike mediese beamptes nie. Die studie vestig aandag op verskeie institusionele en interpersoonlike strategieë wat gebruik word om uitdagings te oorkom en om effektiewe kommunikasie binne die betrokke instansies te verseker. Dit wys ook hoe sommige van hierdie strategieë misluk in die aanspreek van bepaalde kommunikatiewe uitdagings. Les Dynamiques Communicationnelles dans la Recherche en Ecologie : Projet et Programme de Recherche sur la Biodiversité / The Communicational Dynamics within Ecological Research : Research Project and Program on Biodiversity

Cette thèse propose d'interroger les dynamiques communicationnelles à l’œuvre au sein des programmes et projets de recherche sur la biodiversité selon trois dimensions : relationnelle, organisationnelle et symbolique. La biodiversité est une question qui mobilise un nombre croissant de personnes, d’objets naturels, de réseaux à partir de dispositifs qui organisent les interfaces entre tous ces éléments. Mon approche s’inscrit dans un dialogue entre sciences de l’information et de la communication et études des sciences autour de trois notions : en quoi les projets et programmes recomposent-ils les collectifs autour de la biodiversité, quelles modalités organisationnelles ces dispositifs mettent-ils en œuvre et autour de quels standards ?Les programmes et projets de recherche deviennent des dispositifs fondamentaux du développement de la recherche sur la biodiversité en France par la convergence d’opportunités scientifiques, financières, rhétoriques et politiques. Deux terrains, le projet Inbioprocess (ANR-IFB-2006) et le programme « Agriculture, Biodiversité et Action publique » (DIVA2-Ministère de l’Ecologie-2006) ont été particulièrement étudiés. Pour cela, les méthodes d'observation participante, d'entretiens, d'animation de réunion et d'analyses sémiotiques ont été combinées. L’analyse de la dimension relationnelle des dispositifs permet de décrire l’ensemble des interactions et médiations dans lesquelles je suis impliquée. Ces contrastes ressentis entre les deux terrains sont confirmés au niveau organisationnel par la singularité du travail d’articulation et de médiation en jeu. Cependant, ces deux dispositifs mobilisent des standards communicationnels dont la fonction symbolique est de donner un espace au dialogue science et société. Ces dispositifs en tant que lieux de constitution de collectifs et de publics sont marqués par des tensions entre différentes modalités de communication et d’articulation des échelles. / This thesis investigates the communicational dynamics at work within research project and program about biodiversity. It focuses on three dimensions: relational, organizational and symbolic. The issue of biodiversity gathers a growing number of persons, objects, and networks through devices that organize their interfaces. My research benefits from the interplay of sciences of information and communication on the one hand, and sciences studies on the other hand, based on three notions: how projects and programs reshape the collectives around biodiversity; which organizational forms arise from those devices and around which “standards"? Research programmes and projects have become fundamental devices for the development of French research on biodiversity through the convergence of scientific, financial, rhetorical and political opportunities. Two case studies were analyzed : the project Inbioprocess (ANR-IFB-2008) and the program “Agriculture, Biodiversity and Public Action” (DIVA2-Ministère de l’Ecologie-2006) based on data collected through participant observation, interviews, animation of meetings methods combined with semiotic analysis. The study of devices’ relational dimension allows to describe the interactions and mediations in which I was involved. The contrasts between the two cases are confirmed at the organizational level by the singularity of the work of articulation and mediation at play. However, the two devices borrow communicational standards whose symbolic function is to open a space enabling the dialogue between science and society to take place.As spaces where collectives and publics are constituted, those devices reveal tensions between different modalities of communication and between the articulation of different scales.

A emergência de uma massa de novos consumidores no Brasil na década de 2000 gerou a necessidade, por parte das empresas e organizações, de conhecer hábitos, comportamentos e gostos desta parcela da população, que até então estava invisível, pela restrição econômica, aos olhos do mercado. Gerou também uma intensa discussão sobre a reconfiguração de nossa sociedade, em que importa a suposta criação de nova classe social, a \"nova classe média\" ou \"nova classe C\". Estes fatos provocaram o interesse inicial para o desenvolvimento desta Tese de Doutorado, que, assentada no paradigma marxista, buscou lançar mão de arcabouço teórico capaz de compreender as questões relacionadas ao tema central, o consumo, e de seu reconhecido vetor, a Publicidade, conjugadas à discussão sobre as condições materiais impostas pela sociedade capitalista. Os objetivos foram aprofundar o conhecimento teórico sobre o consumo e suas significações; contribuir para a análise sobre os novos consumidores em particular; compreender as suas representações por meio da Publicidade. A base metódica foi composta por revisão bibliográfica, pesquisa qualitativa com consumidores e análise semiótica. O percurso de pesquisa ampliou seu escopo inicial e, ao final, foi possível elaborar e propor tipos ideais de processos de consumo ancorados no quadro mais amplo de classes sociais. Esses processos, com o auxílio da análise das representações na Publicidade, foram descritos em diferentes dimensões que, a nosso ver, compõem a complexa sociedade contemporânea: a dimensão fluida dos movimentos ascendentes ou segregatórios do consumo, a dimensão rígida da estrutura e das classes sociais, a dimensão simbólica das representações. Desenhou-se assim o que chamamos de retratos sociocomunicacionais. / The emergence of a mass of new consumers in Brazil, in the decade of 2000, created the need for companies and organizations to learn about the habits, behavior patterns and preferences of this portion of the population, which, until then, was invisible to the market\'s eye, as a result of economic strain. It also triggered an intense discussion on the reconfiguration of our society, to which the supposed creation of a new social class is of importance, the \"new middle class\" or \"new C social class\". These facts attracted our first interest in preparing this PhD Thesis, which, based on the Marxist paradigm, resorted to a theoretical framework that is able to understand the issues related to the main topic, consumption, and its acknowledged vector, Advertising, combined with the discussion on the material conditions a capitalist society imposes. The goals are those of diving deep into the theoretical knowledge on consumption and its meanings; of adding to the analysis conducted on new consumers in particular; of understanding its representations by means of Advertising Activities. Bibliographic review, quantitative research conducted with consumers, and semiotic analysis comprise the methodical base. The research path expanded its initial scope, and, ultimately, we were able to prepare and propose ideal types of consumption processes supported by the wider range of social classes. These processes, with the help of the analysis conducted on representations used in Advertising, were described within different dimensions, which, in our opinion, comprise the complex, contemporary society: the fluid dimension of consumption\'s ascending or segregated movements, the unbending dimension of structure and social classes, the symbolic dimension of representations. And so were designed what we call socio-communicational portraits.

Este texto busca uma compreensão aprofundada das diferentes realidades que compõem os processos de produção e recepção das imagens fotográficas: suas dinâmicas, relações simbólicas e sujeitos envolvidos. A partir dessa ideia, o objetivo desta pesquisa é demonstrar, por meio de diferentes abordagens teóricas e suas articulações, diversas possibilidades interpretativas que as imagens fotográficas provocam ou sugerem. Para tanto, tem como foco os espaços de duas realidades fotográficas: o espaço do real e o espaço da imagem. A partir dessas duas demarcações, este texto desdobra-se em possíveis articulações, dinâmicas e experiências que as relações entre elas suscitam nos dois maiores sujeitos da fotografia - fotógrafo e leitor. Para tanto, pensamentos de autores que observam questões dos espaços do mundo e das imagens, como Boris Kossoy, Edgar Morin, Michel de Certeau, Michel Maffesoli, Tatiana Levy e Cremilda Medina, são apontados e relacionados com os espaços da fotografia. De cada teórico apresentado, propostas sobre os espaços são destacadas por meio de três olhares distintos e sobrepostos nas narrativas fotográficas da contemporaneidade: o da complexidade, o dos afetos e o da poética. Com um esboço inicial de cada teórico, suas ideias são aplicadas isoladamente a diferentes trabalhos fotográficos. A seguir, articulações entre as propostas teóricas dos diferentes autores são compostas para elaborar diversas possibilidades simbólicas dos espaços em diferentes trabalhos de fotografia produzidos pelo mundo. Acredita-se que nesse processo, questões ainda não tão claras sobre os espaços da fotografia, e da própria fotografia como elemento expressivo e simbólico, possam passar para um primeiro plano. / This research looks for a deeper comprehension of the different realities that make up image production and reception processes: the dynamics, symbolic relations and subjects involved. From this idea, the goal of this research is to demonstrate, by means of different theoretical approaches and their articulations, various interpretative possibilities that the images provoke or suggest. Consequently, it focuses on the spaces of the photographic realities: the reality space and the image space. From these two demarcations, this text unfolds into possible articulations, dynamics and experiences that the relationship between them suscitate on the two main subjects of photography -photographer and reader. To do so, the thinking of different authors that observe issues of spaces of the world and of images, such as Boris Kossoy, Edgar Morin, Michel de Certeau, Michel Meffesoli, Tatiana Levy and Cremilda Medina, are identified and related to the photographic spaces. From each author, a set of proposals on the spaces are highlighted by three distinctive and superimposed points-of-view in the photographic contemporary narratives: of complexity, of affections and of poetics. With an initial sketch of each theorist, his/her ideas are applied separately to different photographic works. Following, the articulations among different authors\' theoretical proposals are combined to develop various symbolic possibilities of the spaces in several photographic essays produced around the world. It is believed that in this process, issues not yet clear regarding the spaces of photography, and photography itself as an expressive and symbolic element, may came to the foreground.

O exame de resultados de pesquisas anteriores realizadas pelo Nomads.usp - Núcleo de Estudos de Habitares Interativos - em que foram utilizados vários instrumentos metodológicos clássicos de pesquisa qualitativa a fim de entender contextos urbano e social, revelou limites desses instrumentos quando se desejava abarcar nuances referentes a modos de vida, visões de mundo e pontos de vistas de indivíduos pesquisados. O objetivo dessa pesquisa é a verificação dos limites e potencialidades do emprego de três atividades, concebidas para uso em eventos culturais com presença de público - Rádio de Rua, Graffiti Digital e Projeção de Comentários -, no sentido de utilizá-las também como procedimentos de obtenção de informação sobre seus públicos, estimulando processos comunicacionais e interlocuções com auxílio de meios digitais. Essas três atividades foram definidas em conjunto com os interesses do Projeto de Políticas Públicas \"Territórios Híbridos: meios digitais, comunidades e ações culturais\", o qual, por outro lado, tornou possível a realização dos experimentos aqui estudados. / The evaluation of previous research carried out by Nomads.usp - Center of Interactive Living Studies - which used several classical methodological instruments of qualitative research in order to understand urban and social contexts, found limits of these tools when they were employed to encompass nuances related to ways of life, worldviews and viewpoints of surveyed individuals. The aim of this reseach is to verify the limits and the possibilities of using three activities conceived for usage in cultural events with audience\'s presence - Street Radio, Digital Graffiti and Comment\'s Projection -, in a matter of utilising it as well as procedures for gathering informations about its audiences by proposing communication processes and dialogs, with the aid of digital media. These three activities were established according to the interests of the Public Policy Project \"Hybrid Territories: digital media, communities and cultural activities\", which, on the other hand, made possible the accomplishment of these studied experiments.

Em um mundo cada vez mais mediado pelas tecnologias, refletir sobre um novo modus operandis dos canais midiáticos colocados a serviço de uma comunicação para a educação parece ser função da academia como modo de ela mesma repensar suas práticas, ao menos no que tratar da EAD. Diante disso, parte-se para uma pesquisa que tem por objetivo analisar os processos de comunicação em um ambiente digital de aprendizagem que se estabelecem entre docente-discentes em disciplinas de cursos de Graduação ofertadas na modalidade não presencial. O caso estudado é o da Universidade do Vale do Rio dos Sinos (UNISINOS), que utiliza o programa Moodle como plataforma para desenvolver esse formato de ensino. Para se chegar aos modos de uso e apropriação desse ambiente, recorreu-se à observação de três turmas, seguida por um método que se denominou abordagem da experiência de / Distance Education brings renewed issues into the debate about the education-learning processes and the interaction between teachers and students. In a world more and more mediated by technologies, reflecting about a new one modus operandis of the mediatic channels put in service for an educational communication seems to be the academy’s task as way for it to rethink its own practices, at least as far as Distance Education is concerned. Therefore, a research is done with the objective of analyzing the communication processes in a digital learning environment and that are established between teachers and students in Undergraduate Programs Courses offered in a non-presencial modality. By overseeing Law as a communicational system, it was possible to affirm that the text of the Brazilian Federal Constitution is a message to be communicated to various addressees (for example judges, prosecutors, lawyers), amongst which is the common citizen, characterized as such due its lack of knowledge concerning the technical legal language. Considering that communication depends on the concurrence of at least six elements in order to be completed (Roman Jakobson), and that the Constitution is also addressed to the common citizens, the research questioned whether the presence of the technical legal language in the constitutional text could be understood as a factor of potential noise in this communication (noise as any interference capable of disturbing the reception of the message by its addressee). The research was illustrated with three examples of terms that are typical of the technical legal language, identified in the text of the Brazilian Federal Constitution in force as potential cause of noise in the constitutional communication and, consequently, capable of rendering difficult or impeding the communication of the constitutional rights and duties to the majority of its addressees. At the end, the study discussed whether the technical legal language, as a potential source of communicational noise, could have an impact on the effectiveness or materialization of the constitutional rights and duties / O objetivo da presente pesquisa foi sintetizar as linhas mestras da Teoria Comunicacional do Direito, que encontra em Gregorio Robles seu autor mais relevante nos dias atuais, aplicando tal teoria à realidade do Direito Positivo brasileiro. Vislumbrando-se o Direito como um sistema comunicacional, foi possível afirmar que o texto da Constituição Federal de 1988 é uma mensagem a ser comunicada a diversos destinatários (a exemplo dos magistrados, promotores, advogados, etc.), dentre eles o cidadão comum. Levando-se em conta que a comunicação depende da concorrência de pelo menos seis elementos para se efetivar (Roman Jakobson), e que o texto constitucional é também destinado aos cidadãos comuns, questionou-se se a presença da linguagem técnica do Direito no texto constitucional não seria um fator de potenciais ruídos nessa comunicação (entendendo-se por ruídos toda e qualquer interferência capaz de atrapalhar a recepção da mensagem pelo seu destinatário). A pesquisa foi ilustrada com três exemplos de termos típicos da linguagem técnica do Direito, identificados no texto da Constituição Federal de 1988 como potenciais causadores de ruídos na comunicação constitucional e, conseqüentemente, capazes de dificultar ou impedir a comunicação dos direitos e deveres constitucionais. Todėl labai svarbu, turimais ištekliais ir perspektyviomis galimybėmis tobulinti įstaigų darbą, kad būtų užtikrinta vaiko ir šeimos socialinė gerovė. Atsižvelgiant į statistinius duomenys pastebima tendencija, kad vaikų, turinčių specialiųjų poreikių, o ypač vaikų su kalbos sutrikimais, skaičius didėja. Tai tampa socialine problema, kaip padėti šiems vaikams, jų šeimoms integruotis į visuomenę, suteikti visapusišką pagalbą ir užtikrinti reikalingų paslaugų kokybę bei tikslingumą. Kauno mieste nuo 2005m. yra įkurti jau 8 specialiųjų poreikių vaikų pagalbos konsultaciniai korekciniai centrai, kurie plečia socialines paslaugas, padeda vaikams, jų šeimoms, pedagogų ir specialistų komandai spręsti pedagogines, psichologines bei socialines problemas. Educational, social and child welfare system are undergoing reforms, following the experience of European countries, general international agreements and main principles of humanism, democracy, nationalism, renovation and openness to changes. Contemporary image of formal and informal institutions such as institutions of adult and children interaction, their development, is being created in the new qualitative space. Thus, it’s very important to improve work of institutions using available resources and prospective possibilities for social welfare of child and family would be ensured. Considering statistical data, emerges the tendency of growing number of children who have special needs, especially of children with speech disorders. This forms a social problem of how to help those children, and their families integrate into society, provide versatile help and ensure quality and expediency of necessary services. Sporto klubo komunikacinės politikos, vykdant socialinius projektus, veiksmingumas, skatinant moksleivių fizinį aktyvumą / Communication policy‘s efficiency of the sports club in the social projects to encourage students physical activity

Darbo tikslas: Išanalizuoti „Žalgirio“ krepšinio klubo komunikacinės politikos, vykdant socialinius projektus, veiksmingumą, skatinantį Kauno regiono moksleivių fizinį aktyvumą. Darbo objektas: Socialinių projektų veiksmingumas Kauno regiono mokyklose. Darbo uždaviniai: 1. Atskleisti komunikacijų reikšmę organizacijos įvaizdžio formavimui. 2. Pristatyti projektų tikslus, valdymo proceso turinį ir rezultatų vertinimą organizacijos komunikacinėje politikoje. 3. Ištirti „Žalgirio“ krepšinio klubo vykdomus socialinius projektus, skatinančius Kauno regiono moksleivių fizinį aktyvumą. Rezultatai ir išvados: 1. Svarbi komunikacinės politikos priemonė, sudaranti prielaidą keistis informacija su visuomene, taip formuojant organizacijos įvaizdį – tai organizacijos vykdomi socialiniai projektai. 2. Projektai yra organizacijos strateginis instrumentas įgyvendinant organizacijos komunikacinės politikos tikslus. Sėkmingai įgyvendinti projektai turi įtakos organizacijos reputacijai, įvaizdžiui, veiklos efektyvumui ir grįžtamąjam ryšiui. 3. „Žalgirio“ krepšinio klubo įvaizdis moksleivių tarpe yra teigiamas. Pabrėžiamas klubo įvaizdžio formavimo komunikacinių priemonių išskirtinumas, lyginant su kitais krepšinio klubais. Mokyklose vykdomi socialiniai projektai skatina ir moksleivius, ir aplinkinius tapti fiziškai aktyvesniais. Projektai skatina ugdyti tam tikras vertybes.
