Avaliação longitudinal do contexto familiar, saúde mental e personalidade de crianças e adolescentes em cuidados oncológicos / Longitudinal assessment of family contexto, mental healt and personality of children and adolescentes under oncological care

O diagnóstico de neoplasia em crianças e adolescentes, bem como os cuidados oncológicos necessários, impõem aos pacientes e a suas famílias o convívio cotidiano com múltiplos eventos adversos ao desenvolvimento, que podem ser de natureza biológica, psíquica, social e econômica. Essa complexa realidade exigirá recursos e estratégias pessoais e familiares para positivo desfecho, superando situações de vulnerabilidade. Até o momento, apesar do crescente número de casos de câncer, encontra-se reduzida produção científica sistematizada no campo da Psico-oncologia Pediátrica sobre quais variáveis seriam as mais relevantes em termos de efeitos no desenvolvimento infantojuvenil, embora se reconheça importância dos recursos pessoais (incluindo características de personalidade), sócio-familiares e dos serviços de saúde. Nesse contexto, este trabalho objetivou avaliar longitudinalmente possíveis efeitos da vivência de adoecimento e cuidado oncológico sobre recursos pessoais, familiares e características de personalidade de crianças e adolescentes com câncer. Trata-se de estudo longitudinal, de natureza quantitativa, descritivo-comparativo e interpretativo, a partir de instrumentos de avaliação psicológica. Foram compostos dois de crianças e adolescentes, de sete a 17 anos, de ambos os sexos, bem como seus respectivos pais/responsáveis, compondo amostra de conveniência. O Grupo Clínico (G1, n=30, crianças/adolescentes com diagnóstico de neoplasia há pelo menos um mês) foi retirado de pacientes do Setor de Oncologia e Hematologia Pediátrica de um hospital-escola público, considerado serviço de referência na área. O Grupo de Comparação (G2, n=20, crianças/adolescentes com desenvolvimento típico), foi constituído de modo a ser balanceado em relação a idade, sexo e origem escolar (particular ou pública) a G1. Todos os voluntários (crianças/adolescentes e seus pais/responsáveis) responderam, individualmente e em ambiente adequado para avaliação psicológica, a uma bateria de instrumentos avaliativos, aplicados em dois momentos, nomeados nesse estudo como Fase 1 (recorte transversal) e Fase 2 (recorte longitudinal, após intervalo mínimo de seis meses da primeira avaliação). Os pais/responsáveis responderam a: Critério de Classificação Econômica Brasil, Questionário de Capacidades e Dificuldades (SDQ), Inventário de Recursos do Ambiente Familiar (RAF), Escala de Eventos Adversos (EEA), Escala de Adversidades Crônicas (EAC) e Inventário de Percepção do Suporte Familiar (IPSF). Na criança/adolescente foram administrados: Matrizes Progressivas de Raven (Coloridas ou Escala Geral), Método de Rorschach (Escola Francesa) e Teste das Pirâmides Coloridas de Pfister. A aplicação e a avaliação dos instrumentos seguiram seus respectivos padrões técnico-científicos, realizando-se análises descritivas e comparativas de G1 e G2 (Teste t de Student ou Teste de Wilcoxon, p0,05%), nas duas fases do estudo. Os resultados apontaram, como síntese das duas fases avaliativas, sinais de preservação dos recursos familiares e cognitivo/afetivos das crianças/adolescentes com câncer, com manutenção do funcionamento adaptativo, embora com impacto negativo na percepção de si e de sua identificação com o humano, tendendo a maior mobilização emocional, com tensão e ansiedade em seus esforços adaptativos, identificados pelos pais/responsáveis como indicadores de dificuldades em termos de saúde mental, comparativamente ao Grupo de Comparação. O Grupo Clínico tendeu a recorrer mais a estratégias defensivas baseadas em mecanismos repressivos e de racionalização, inibindo expressões afetivas no ambiente, favorecendo-lhes adaptação ao contexto de vida. Estes achados empíricos evidenciaram, numa perspectiva longitudinal, os recursos e os esforços adaptativos de crianças/adolescentes que convivem com o diagnóstico oncológico, bem como suas famílias. Foi possível apontar a relevância da sistemática avaliação e reavaliação da complexa rede de variáveis envolvidas nesses processos, atestando a contribuição dos métodos projetivos no campo da Psico-Oncologia Pediátrica. / The neoplasia diagnosis in children and adolescentes, as well as the oncological care needed, impose to patients and their families the daily living with multiple adverse events to the development, that may be of biological, psychic, social and economic nature. This complex reality will demand personal and family resources and strategies for a positive outcome, overcoming situations of vulnerability. To date, despite the growing number of cancer cases, there is a reduced systematized scientific production in the field of Pediatric Psycho-Oncology about what variables would be the most relevant in terms of effects on child and adolescent development, although the importance of personal resources (including personality characteristics), socio-family and health services is recognized. In this contexto, this study aimed to longitudinally assess possible effects of the experience of illness and cancer care on personal, family and personality characteristics of children and adolescents with cancer. This is a longitudinal study, of a quantitative, descriptive-comparative and interpretative nature, based on psychological assessment instruments. The were composed two groups of children and adolescents, from seven to 17 years of age, of both sexes, as well as their respective parents/guardians, composing a convenience sample. The Clinical Group (G1, n=30, children/adolescents with diagnosis of neoplasia for at least one month) was composed from patients of the Oncology and Pediatric Hematology Sector of a public school hospital, considered a reference service in the area. The Comparison Group (G2, n=20, children/adolescents with typical development) was constituted in order to be balanced against age, gender and school origin (private or public) to G1. All volunteers (children/adolescents and their parents/guardians) responded, individually and in a suitable environment for psychological assessment, to a battery of assessment instruments, applied in two moments, named in this study as Phase 1 (transversal cut) and Phase 2 (longitudinal cut, after a minimum interval of six months of the first assessment). Parents/guardians responded to: Brazil\'s Economic Classification Criteria, Capacities and Difficulties Questionnaire (SDQ), Family Environment Resource Inventory (RAF), Adverse Event Scale (EEA), Chronic Adversity Scale (EAC), and Inventory of Perception of Family Support (IPSF). In the child/adolescent were administered: Progressive Matrices of Raven (Colored or General Scale), Method of Rorschach (French School) and Test of Colored Pyramids of Pfister. The application and assessment of the instruments followed their respective technical-scientific standards. Descriptive and comparative analyzes of G1 and G2 (Student\'s t test or Wilcoxon test, p0.05%) were carried out in both phases of the study. The results showed, as a synthesis of the two evaluative phases, signs of preservation of the family and cognitive/affective resources of the children/adolescents with cancer, with maintenance of the adaptative functioning, although with a negative impact on the perception of self and its identification with the human, tending to greater emotional mobilization, with tension and anxiety in their adaptive efforts, identified by parents/guardians as indicators of difficulties in terms of mental health, compared to the Comparison Group. The Clinical Group tended to resort more to defensive strategies based on repressive mechanisms and rationalization, inhibiting affective expressions in the environment, favoring adaptation to the context of life. These empirical findings have evidenced, from a longitudinal perspective, the resources and the adaptive efforts of children/adolescents living with cancer diagnosis, as well as their families. It was possible to point out the relevance of systematic assessment and re-assessment of the complex network of variables involved in these processes, attesting the contribution of projective methods in the field of Pediatric Psycho-Oncology.

Trabalho infantojuvenil, impactos e dilemas do ECA: a luta por direitos na justiça do trabalho - TRT 12 (Florianópolis, década de 1990) / Child labor, impacts and dilemmas of Brazilian child and adolescent rights act (eca): a fight for rights in Labor court - TRT 12 (Florinópolis, 1990s)

Este estudo tem como objetivo central identificar e problematizar os impactos e dilemas do ECA junto à Justiça do Trabalho TRT 12, durante a década de 1990, na cidade de Florianópolis, tendo em vista ser esse o período de implementação do Estatuto. A partir da análise de um corpus documental de 300 ações processuais trabalhistas, obtidas no Arquivo do Serviço de Digitalização e Guarda, buscamos perceber como e se os operadores/as do Direito do Trabalho remetiam-se ao ECA na formulação dos documentos que compunham os autos do Judiciário Trabalhista. Nessas ações, dentre as quais, 299 foram aJuizadas por trabalhadores e trabalhadoras com idade inferior a dezoito anos e uma pelo Ministério Público do Trabalho, nos foi possível verificar como se constituíam as relações de trabalho entre empregadores/as de diferentes setores e empregados/as infantojuvenis, em uma Florianópolis que passava por profundas transformações urbanas com um quadro de aumento demográfico resultante de um significativo processo migratório. A década de 1990, no Brasil, foi marcada por uma aplicação de políticas neoliberais, que teve como consequência o aumento das taxas de trabalho informal, de dessalariamento e de desemprego, provocando uma situação socioeconômica de maior vulnerabilidade, sobretudo, no que tange às famílias pobres. Como decorrência dessa situação, acentuaram-se as taxas de trabalho infantil ilegal e juvenil ilegal e/ou irregular em todo o país, e em especial na cidade de Florianópolis, onde nos foi possível mapear o lugar ocupado pelos contingentes infantojuvenis protagonistas desta história. Os dilemas do ECA quanto ao trabalho infantojuvenil estão relacionados à necessidade que famílias empobrecidas têm de se utilizarem dessa mão de obra para a própria subsistência. Os impactos relativos a esse Estatuto na sociedade brasileira são muitos, como é o caso da Ação Civil Pública, que contando com a atuação conjunta do Ministério Público do Trabalho, da Delegacia Regional do Trabalho, do Conselho Tutelar da Criança e do Adolescente de Florianópolis e do TRT 12 levou à erradicação das atividades de jornaleiro/a e encartadores de jornais, formas de trabalho infantojuvenil que perduraram por muitas décadas na cidade de Florianópolis. Verificamos, por fim, que para a erradicação do trabalho infantil e para a regularização do trabalho juvenil faz-se necessária a efetivação de um conjunto de políticas públicas voltadas à melhoria de renda dessas famílias, para que não dependam dessa força de trabalho, assim como dos demais direitos, educação, saúde, cultura, lazer e etc., reconhecidos à infância e à adolescência. / The present thesis has as its goal to identify and discuss the impacts and dilemmas raised by the Brazilian Child and Adolescent Rights Act (ECA) in the sphere of action of the Labor Courts (TRT 12) in the city of Florianopolis (Brazil), during the 1990s, in which context that legislation was introduced. From the analysis of 300 labor lawsuits located in the official Digital Archives of that institution, I seek to understand the means by which the labor law operators made use of and referred to the ECA in order to formulate the documents that based the judiciary processes. In the set of those lawsuits, 299 were filed by workers of under eighteen years old and one by the Labor Public Ministry. By the analysis of the documents is possible to identify how were formed working relationships between employers and young workers in different economic sectors of a town that was passing by deep urban transformations linked to the migration process and the population growth. The 1990s in Brazil was marked by the introduction of neoliberal policies, which led to an increase in informal employment rates, cheap labor and unemployment, causing a social and economic situation of greater vulnerability, especially among poor families. As a result of it occurred the increasing of illegal or irregular child and juvenile labor rates in Florianopolis. This study shows the place occupied by those young workers protagonists of this narrative in the recent social and economic processes of that city. The dilemmas that have been raised since the introduction of ECA are related to the needs ofpoor families, which depend on children and youngslabor fortheir livelihood. There are many impacts of ECA in the Brazilian society, such as the Public Civil Action, which involves joint efforts of the Ministry of Labor, the Regional Labor Office, the Council of Guardianship of Children and Adolescent and the Labor Court (TRT 12), which led to the eradication of the paperboys activities, a form of work that lasted for many decades in Florianopolis. Finally, this thesis demonstrates thatthe eradicationof child labor andthe regularizationof youth work should be a part of a broad set ofpublic policies aimed atimproving the incomeof poor families,so that they do notdepend onthislabor force, as well as children and adolescents must be recognized as holders of rights in terms of education, health, culture, recreation and so on.

[pt] Essa pesquisa tem como objetivo analisar lógicas e práticas que fundamentam as alegações de negligência como justificativa para o acolhimento institucional de crianças e adolescentes, como medida de proteção, por agentes do Sistema de Garantia de Direitos de Crianças e Adolescentes no Estado do Rio de Janeiro. Trata-se de um estudo qualitativo, utilizando a análise documental e tendo - como fonte de pesquisa - relatórios feitos por equipes técnicas de instituições de acolhimento do Rio de Janeiro, que servem como subsídios para o Módulo Criança e Adolescente (MCA). Foram analisados cem relatórios, nos quais o motivo alegado para o acolhimento institucional foi negligência, retratada através da descrição de situações - como pobreza, uso abusivo de drogas e álcool e más condições de higiene. O conteúdo das argumentações aponta mais para permanências do que rupturas em relação ao paradigma de culpabilização das famílias pobres por suas próprias dificuldades em prover o cuidado adequado de sua prole, reiterando o desafio de se contextualizar a relação entre atribuição de negligência e pobreza, dado que a legislação atual não permite o acolhimento de crianças e adolescentes por falta de recursos financeiros de seus pais ou responsáveis. De onde decorre que o novo paradigma de proteção integral contido no Estatuto da Criança e do Adolescente não se apresenta capaz isoladamente de impedir que, na prática, a lógica repressiva e voltada para a culpabilização da família pobre se reatualize em termos de situação de risco ou de negligência. / [en] This study aims to analyze the logics e practices that fundament the negligence allegations as a justification for the institutional sheltering of children and youths as a protection measure by agents of the Children and Youth Rights Guarantee System, on the State of Rio de Janeiro. It is a qualitative study, that uses documental analysis and brings as sources of research reports made by technical teams of sheltering institutions of Rio de Janeiro, that subsidize the Child and Youth Module (MCA from the Portuguese acronym). One hundred reports, on which the reason for institutional sheltering was negligence, portrayed as poverty situation, abusive use of drugs and alcohol and bad hygiene conditions, were analyzed. The content of argumentations points more to permanence than to rupture in relation to the paradigm of finding poor families guiltyfor their own difficulties in providing adequate care of their offspring, reiterating the challenge of contextualizing the relationship between negligence and poverty, given that the actual legislation does not allow the sheltering of children and youth due to the lack of financial resources by their parents or their legal responsible. The consequence of this is that the new full protection paradigm contained on the Children and Youth Statute is not capable, by itself, of preventing that, in practice, the logic that is repressive and tends to find poor families guilty presents itself with terms like risk situation or negligence.

Histórias de vida : (des) proteções sociais vivenciadas pelos familiares de crianças e adolescentes em acolhimento institucional

O presente estudo tem como temática central as proteções e desproteções sociais vivenciadas pelas famílias de crianças e adolescentes em medida de proteção de acolhimento. Foi desenvolvido um estudo de nível exploratório e natureza qualitativa, guiado pelo método Materialista Histórico Dialético, que buscou conhecer qual a percepção que as famílias de crianças e adolescentes em acolhimento institucional, no município de Gravataí/ RS, no ano de 2017, possuem sobre a situação de acolhimento e de (des) Proteção Social vivenciada pelas famílias. Tomaram-se como sujeitos do estudo seis familiares de crianças e adolescentes em acolhimento no município, no ano de 2017. A pesquisa utilizou a metodologia de História de Vida para possibilitar escuta e visibilidade às narrativas dos sujeitos. A proteção social é destacada neste estudo, como fundamental, em meio à perversidade e violência do modelo de acumulação capitalista – sendo um instrumento que serve à reprodução do capital, mas que, em um processo dialético, é também uma conquista da classe trabalhadora. As perspectivas preponderantes, em um cenário neoliberal, sobre a função/participação das famílias na execução do sistema de proteção no Brasil, mostram-se com viés Familista e de Pluralismo de Bem-Estar, os quais caminham de encontro aos processos de responsabilização dos indivíduos e famílias por sua proteção. Esses primas e configuração da proteção social, trazem implicações sobre a aplicação da medida de acolhimento de crianças e adolescentes e proteção das famílias. Dessa forma, emergem nesse estudo, trajetórias de vida permeadas por processos de violações de direitos e desproteções sociais, nos quais, muitas vezes, o acolhimento de crianças e adolescentes é resultado desse processo. Ainda, a medida de acolhimento é percebida de forma ambígua pelos familiares: insurge como uma proteção, que garante acesso aos serviços e bens que as famílias não possuem condições de prover; como algo que “não traz coisas boas”; e, também, como uma punição pelo comportamento. Por fim, aponta-se para a necessidade de se direcionar um olhar atento para as famílias das crianças e adolescentes em acolhimento, para compreender quais são trajetórias de vida dessas pessoas, quais as violações de direitos foram e são vivenciadas por elas e como a proteção social, enquanto um sistema, atende suas necessidades e garante as condições para que as crinças e adolescentes tenham o direito à permanência na família. A partir disso, é possível que se construam estratégias, de forma conjunta com os sujeitos, visando superar possíveis vulnerabilidades e riscos sociais e fortalecimento de suas capacidades protetivas. / The present study has as its central theme the protection and the lack of social protection experienced by families with children and teenagers in protective measures. A study of exploratory level and qualitative nature was developed guided by the Materialistic Dialectical method, which sought to know what these families’ perception of the program of social care and protection is (in the city of Gravataí in 2017). Six family members of these children and teenagers in social care (in town in 2017) were taken as subjects of the study. The research used the Life History methodology to enable listening and visibility to these relatives’ narratives. Social protection is highlighted, as fundamental, amid the perversity and violence of the capitalist accumulation model - being an instrument that serves the reproduction of capital, but which, in a dialectical process, is also a working-class achievement. The prevailing perspectives, in a neoliberal scenario, on the role/participation of families in the implementation of the protection system in Brazil, present themselves with Familial and Welfare Pluralism bias, which match the processes of accountability of individuals and families for their protection. These prisms and configurations of social protection, have implications over the application of the fostering measures in relation to children and adolescents and the protection of families. Therefore, this study brings out life trajectories permeated by processes of rights violations and social deprotection, where, often, the reception of children and adolescents is the result of this process. Moreover, the measure of institutional home is perceived ambiguously by family members: it emerges as a protection, which guarantees access to services and goods that families cannot afford; As well as something that "doesn’t bring good things"; and is given as a punishment for behavior. Finally, it is pointed out the need to take a close look at the families of these children and teenagers in the institutional home, to understand their life trajectories, which rights violations have been and are experienced by them and how the social protection, while a system, meets their needs and guarantees the conditions for the children and adolescents to have the right to stay in the family. With that in mind, it is possible to build strategies, together with the relatives in order to overcome possible vulnerabilities and social risks and therefore strengthen their protective capacities.

Vem har Svarte Petter? : Samverkan mellan socialtjänsten och barn- och ungdomspsykiatrin gällande barn i behov av stöd / Who´s got Black-Pete? : Cooperation between social services and child and adolescent psychiatry regarding children in need of support

Barn och unga i Sverige mår idag allt sämre, anledningarna är många såsom psykiska och fysiska övergrepp samt högre krav i skolan och den framtida arbetsmarknaden. För att råda bot på barn och ungas psykiska ohälsa krävs många gånger samverkan mellan region och kommun. Samverkan mellan myndigheterna är lagstadgad och syftar till att skapa förutsättningar samt att myndigheter och verksamheter ska arbeta mot samma mål. Syftet med studien var därmed att undersöka hur yrkesverksamma inom dessa verksamheter såg på samverkan dem emellan samt hinder och framgångsfaktorer i den. Studiens syfte undersöktes genom kvalitativ metod där tre yrkesverksamma inom socialtjänstens fält barn och familj, och tre yrkesverksamma inom barn- och ungdomspsykiatrin intervjuades. Resultatet tolkades med hjälp av teorierna och begreppen fält, doxa och symbolisk makt, samverkan samt maktperspektivet enligt Foucault. Resultatet visade att samordnad individuell plan (SIP) gav tydlighet kring ärenden men inte alltid användes rätt. Olika lagstiftningar gav olika perspektiv, varav syn på innebörden av barnets bästa skildes åt i flera fall, därmed var verksamheterna beroende av samverkan och varandras kompetens för att barn inte skulle falla mellan stolarna. Ingen av verksamheterna ville tillskriva den andre mer makt, dock fanns förväntningar på den andre verksamhetens kompetens och handlingsutrymme. Känsla av maktlöshet uppkom då förväntningarna inte uppfylldes. Slutsatsen blev att det krävdes strukturella förutsättningar såsom gemensamma rutiner och transparens för att samverkan skulle förbättras, även rutinerna kring SIP behövde förtydligas. Samtliga respondenter efterfrågade forum för att nätverka och skapa kontakter för att stärka samverkan men med anledning av resursbrist saknades detta. / Young people in Sweden are feeling worse, due to reasons such as mental and physical abuse as well as higher demands at school and in the future labour market. In order to remedy young people's mental illness, cooperation between authorities is required. Cooperation between authorities is statutory and aims to create good conditions. The purpose of this study was to investigate how professionals in these authorities perceive cooperation, also obstacles and success factors in cooperation. The purpose of this study was examined through qualitative method, three professionals within social services and three professionals in child and adolescent psychiatry were interviewed. The result was interpreted using the theories and concepts of field, doxa and symbolic power, collaboration and the power perspective according to Foucault. The results showed that coordinated individual plan (SIP) provided clarity about cases but was not always used correctly. Different legislations gave different perspectives, thus were the concept of the best interests of the child separated, therefore the authorities depended on cooperation and other's competences. None of the respondents wanted to attribute the concept of power, however, there were expectations of other´s competence and room for maneuver. A feeling of powerlessness arised when expectations were not met. The conclusion showed that in order for cooperation to be complete, structural conditions such as common routines and transparency were required, also the routines around SIP required clarification. All respondents requested forums to network and create relationships to strengthen cooperation, but due to lack of resources this failed.

The topic of this thesis is "Motivational work with a client in Child and adolescent detox center". The target of the theoretical part of this thesis is to describe motivational work with the client of the Child and adolescent detox center in the Hospital Sisters of Mercy of St. Charles Borromeo in Prague. The theoretical part is focused on general issue of drugs and addiction. Furthermore, there are described historical changes of the drug scene in our country in the 60s of the 20th century. Moreover, there are mentioned preventive measures against the emergence of the drug addiction. The detailed focus was given to the department of the Child and adoslescent detox center of the Hospital Sisters of Mercy of St. Charles Borromeo in Prague. In addtion, there are stated options of the treatment which can be used by the client during the stay in the Child and adolescent detox center in the Czech Republic. The aim of the practial part of the thesis is to analyse if and how the motivation for the change of the curent behaviour of the client varies during his or her visit in the Child and adolescent detox center. [pt] Este trabalho tem como objetivo discutir o funcionamento em rede no Sistema de Garantia de Direitos da Criança e do Adolescente no município de Volta Redonda/RJ. Para a consecução dos objetivos foi realizada uma pesquisa qualitativa, por meio de entrevistas com os representantes das diversas instâncias integrantes do Sistema de Garantia de Direitos local. As informações obtidas foram submetidas à análise de conteúdo, em sua modalidade temática. Os resultados apontam para um processo incipiente de implantação de um trabalho articulado, faltando ainda espaços de pactuação e integração coletiva de todas as instâncias que compõe o sistema. / [en] This paper aims to discuss the Children and Adolescents Rights Assurance System network functioning in the city of Volta Redonda/RJ. To achieve the objective a qualitative research was conducted through interviews with representatives of varied instances that are part of the local Rights Assurance System. The information obtained was submitted to content analysis, in its thematic modality. The results indicate an articulated work incipient implementation process, yet lacking agreement spaces and collective integration of all system composing instances.

The main aim of this thesis is to contribute to the advancement of the topic of father and fatherhood in the lives of children, adolescents, and emerging adults. In the literary review part of this work, the topic is set into the context of family and history, then, the current available findings on this topic in each of the developmental period of an offspring, up to emerging adulthood, are introduced. In the research section, the carried-out research is presented. It focused on the description of the role of a father of current emerging adults in the Czech Republic, on exploration of the term "good father" and on finding relations between the perception of one's own father and reported quality of life. It is like fighting against an entire army : A qualitative study of patients’ and parents’ experiences oftreatment for adolescent anorexia nervosa

Introduction. Anorexia nervosa (AN) is an eating disorder with one of the highest mortalityrates of all psychiatric illnesses. One county hospital in Sweden has seen an increase in theprevalence of adolescent AN over the last year. To meet these heightened demands, thecounty’s child and adolescent psychiatric (CAP) clinic established a new level of care for thesepatients. The main component of the intervention was mealtime support. Aim. The aim of the study was to evaluate the experiences of adolescent patients and theirparents regarding a day-patient treatment for AN in a county hospital in Sweden. Methods. Patients and their legal guardians were interviewed separately or in dyads, accordingto their wishes. An interview guide with open-ended questions was used. The audio files weretranscribed verbatim, and the texts were then studied with inductive content analysis. Fivepatients and nine parents from seven different families participated in the study. Results. The participants painted a nuanced picture of the intervention. The four overarchingthemes that emerged were ambivalence, distinguishing the person from the disease, parentalstruggling and perceptions of the treatment and the staff. Strict rules around food combinedwith an empathetic and flexible attitude towards the child were described as successful carestrategies. The treatment was depicted as time-consuming and strenuous, but parents alsoexperienced the intervention as relieving. Conclusions. The intervention was for the most part experienced as well-functioning, and theintensive nature of the treatment was described as a key factor.

Synergieffekt eller spel för galleriet? : Samverkan mellan skolan och Barn- och ungdomspsykiatrin (BUP) / A synergy effect or just a show for nothing? : Collaboration between school and Child- and adolescent psychiatry

Syftet med studien var att undersöka samverkan mellan skolan och Barn- och ungdomspsykiatrin (BUP) utifrån professionellas upplevelser. För att besvara studiens syfte antogs en kvalitativ ansats där sju halvstrukturerade intervjuer genomfördes med kuratorer och enhetschef från BUP samt skolkuratorer och skolsköterska från skolans elevhälsa. Insamlad data analyserades med hjälp av kvalitativ innehållsanalys. Resultatet visade att den samverkan som sker idag till stor del fungerar väl på individnivå till följd av en vilja bland professionella att samverka. Det som försvårar samverkan återfanns i högre grad på strukturell och organisatorisk nivå, genom faktorer som resursbrist och bristande tillgänglighet. Resultatet tydde på att ökad kunskap om varandra och verksamheternas olika uppdrag, i kombination med respekt och samsyn, var viktiga faktorer för fungerande samverkan. Resultatet visade även hur samverkan kunde göra stor skillnad för individer och uppvisade en önskan hos de professionella att samverka mer, men också att samverkan ibland används felaktigt och initieras trots vetskap om att det inte finns resurser att förändra situationen för barnet. Den fördjupade resultatanalysen gjordes med utgångspunkt i vår teoretiska referensram som baserades på systemteori, frontlinjebyråkraten samt modellen för horisontell- och vertikal integrering och med hjälp av studiens kunskapsöversikt. Studiens slutsats var att samverkan är en stor del av arbetet med att höja barns välmående och funktionsnivå. För att uppnå välfungerande samverkan krävs främst förändringar på strukturell och organisatorisk nivå. / The aim of the study was to examine collaboration between school and Child- and adolescent psychiatry. To achieve the purpose of the study we used a qualitative approach by doing seven semi-structured interviews with professionals from schools and child- and adolescent psychiatry. Our data was analysed by a qualitative content analysis. The result pointed out that collaboration today was well functioning on an individual level due to a strong will among professionals to collaborate. Difficulties with collaboration appeared to be on a structural and organizational level, such as lack of resources and availability. The result suggested that increased knowledge about each other and about the different missions of included actors, combined with respect and consensus, was important to reach well-functioning collaboration. The results also showed how collaboration tends to make a great difference to people and indicated a wish to collaborate more among the professionals, but also that collaboration sometimes tends to be misused and was initiated despite the knowledge that there are no resources to change the situation for the child. Our analysis of the result was made on basis of System theory, the theory of street level bureaucrat and the model of vertical- and horizontal integration, combined with previous research knowledge. The conclusion of the study was that collaboration is an important part of the work to improve children’s well-being and functionality. For a successful collaboration, changes have to be done on a structural- and organizational level.
